ABSTRACT Background Method and Conclusion The terminology surrounding developmental lesions in the oral cavity is widely applied, often leading to confusion in differentiating between developmental malformations and neoplasia. Odontogenic tumor classification includes both true neoplasms and malformations which make it very complex and dynamic.In this brief report, we will first discuss the concepts of malformations and neoplasia and then focusing on their relevance to odontogenic tumors, which impacts their classification and treatment, particularly mixed odontogenic lesions. Objectives: This study aimed to assess the epidemiological and three‐dimensional (3D) radiological characterizations of odontomas, as well as the spatial relationship between odontomas and gubernaculum tracts (GT). Materials and Methods: We retrieved the cone‐beam computed tomography (CBCT) data of 87,590 patients. Dentition, location, type, diameter of the odontomas, width of the dental follicle (DF), the spatial relationship between the odontoma and GT, and the influence on adjacent teeth were evaluated. Results: Significant differences were found in age, dentition, location, Max/Min diameter, width of DF, impaction, retention, and root bending of adjacent teeth among different spatial relationships between the odontoma and GT (all p < 0.05), as well as in age, type and size, absence, impaction, malposition, and retention of adjacent teeth among different locations of odontomas (all p < 0.05). Compared to the odontomas without impaction, those with impaction had larger diameter (p < 0.05 in all directions). This statistically significant association was consistent for odontomas with malposition, while no similar result was observed in the maximum diameter. Conclusion: Our findings provide the preliminary data for clinicians to comprehensively understand the incidence, radiographic characterizations and symptoms of odontoma in Chinese population. Gardner's syndrome has been recognized as a hereditary affliction that is autosomal in nature. This disorder is known to exhibit characteristics of familial adenomatous polyposis, of which it is also considered a variation. The features of this syndrome include the gradual development of osteomas and epidermoid cysts, including a characteristic feature that is the noted presence of intestinal polyps, which are generally multiple in number. Additionally, dental anomalies have been observed and recorded in a sizeable ratio of cases encompassing an increased frequency of multiple odontomas that are benign tumors linked to the development of a tooth and tooth agenesis/hypodontia that refers to developmental absence of one or more teeth. The other dental ascertainment includes abnormal morphology of the tooth/teeth as well as the presence of supernumerary teeth and impacted or unerupted teeth. This case report outlines the case of a 59-year-old male patient who had reported to the clinic, and was then diagnosed with Gardner's syndrome post a thorough examination. On radiographic examination, the manifestation of multiple osteomas in the frontal bone was revealed. The presence of a motley of diffused benign lesions of the bone in both the upper and lower jaw as well as the presence of an epidermoid cyst on the scalp was suggestive of an exemplar presentation of Gardner's syndrome. Dentinogenic ghost cell tumor (DGCT) is an uncommon odontogenic tumor considered to be the solid, tumorous variant of calcifying odontogenic cyst. DGCT is characterized by islands of ameloblastoma-like epithelial cells that resemble the enamel organ, the presence of ghost cells, and dentinoid material. This article reports a rare case of dentinogenic ghost cell tumor as sociated with an odontoma in an adult patient, with a review of the literature. To the best of the authors' knowledge, there have only been four case reports of DGCT associated with odontoma, all of which occurred in children and adults younger than 30 years old. Calcified epithelial odontogenic tumour (CEOT) are rare benign tumours that tend to arise in the mandible. Calcifications of amyloid-like substances produced by these tumours present as irregular radiopacities on the radiographs. Herein, we report a rare case of a CEOT in the maxillary premolar, which was largely composed of massive calcification and the presence of an impacted tooth. The patient was a 37-year-old male with a chief complaint of tooth mobility and pain in the left maxillary molar. Panoramic X-rays and computed tomography imaging showed an irregular radiopaque lesion with tooth-like structures in the left maxillary premolar. An excisional biopsy was performed after a clinical diagnosis of a benign tumour was suspected. The lesion was histopathologically diagnosed as a CEOT. Introduction: Ameloblastic fibro‑odontoma (AFO) is a rare, slow‑growing neoplastic lesion classified as a benign, epithelial odontogenic mesenchymal tumor. This tumor exhibits histological features characteristic of both ameloblastic fibromas and complex odontomas. The clinical manifestation of AFO is typically characterized by the asymptomatic enlargement of the jawbones. Radiographically, it presents as a distinct radiolucent region, indicating the presence of radiopaque substances with varying degrees of irregularities in size and morphology. Standard therapeutic intervention involves enucleation. Despite its benign nature, AFO can cause significant morbidity if left untreated. Therefore, prompt diagnosis and appropriate management are essential to ensure optimal patient outcomes. The present study described the case (clinical presentation and management) of a 10‑year‑old male patient with an AFO lesion located in the posterior mandible. This particular case was treated with conservative measures involving surgical enucleation along with the extraction of the impacted tooth and the curettage of residual bone.

Calcifying odontogenic cyst, also known as Gorlin cyst is a rare benign cystic lesion primarily found in the jawbones, accounting less than 1% of odontogenic cysts. It can be associated with odontogenic tumors such as odontomas. We report a rare case of COC associated with complex odontoma in a young patient and discuss its clinical features, diagnosis, and treatment options. An 18-year-old female patient presented with a painless radiopaque lesion of the right mandibular bone at Oral Medicine and Oral Surgery department. Radiographs revealed irregular tooth-like structures in the canine-premolar area. The lesion was surgically removed, and histopathology confirmed COC with a complex odontoma. As of the World Health Organization's 2022 definition, COC is a developmental odontogenic cyst characterized by calcified ghost cells. It typically affects individuals during their second and third decades of life, with no gender preference, almost equally in the maxilla and the mandible. The main treatment is total enucleation, with a generally favorable prognosis. Histopathology is essential for diagnosis due to its mimicry of other jaw conditions. Odontomas are benign tumors characterized by slow and limited growth with a rare recurrence. Odontomas are generally detected by radiographic findings in the radiopaque stage, where calcification of the tissues is observed. This article seeks to report the recurrence of a radiologically diagnosed odontoma to show the importance of radiographic controls after enucleation as a diagnostic and follow-up method. Case report: A female patient, 9 years old, attended dental care in 2020 due to malpositioned teeth. In the intraoral clinical examination, she presented stage II mixed dentition with crowding. A radiographic exam showed no associated lesions. The patient reported a history of odontoma removal and a supernumerary tooth in sextant II in 2016. Subsequently, she was referred to orthodontics, where permanent dentition with moderate anterior crowding in the maxilla and mandible was observed. The radiographic examination showed a radiopaque area compatible with odontoma, palatal to teeth 12 and 13. Conclusions: Although recurrence is rare, complete removal in the case of an odontoma is critical. This study demonstrates the importance of performing radiographic controls 5 years after enucleation of an odontoma, considering the stages of evolution. [ABSTRACT FROM AUTHOR]

Their clinical and radiological features generally provide clinical suspicion of a benign lesion but only a histological examination may lead to the final diagnosis. Surgical management is generally easy and resolutive, but it can become more complicated in uncooperative patients and may thus require additional procedures to perform a fit surgery. We report a case of peripheral developing odontoma of the palatal aspect of the anterior maxilla that occurred in an uncooperative child subsequently treated by diode laser surgery, highlighting the benefit of this kind of surgery in pediatric patients. Data from the literature on peripheral developing odontoma have been collected and discussed with a narrative review.

This report presents the successful orthodontic management of a case of odontoma associated with impaction of the mandibular right permanent canine and Molar-incisor-hypomineralization (MIH). Although case studies have reported the management of odontomas and MIH as separate entities, there is a lack of evidence regarding the comprehensive management of patients presenting with a combination of odontoma, dental impaction, and MIH. A 15-year, 7-month-old female patient complained of the appearance of her smile and the delayed eruption of her mandibular right permanent canine. She was diagnosed with Angle’s Class I molar relationship and Class II canine relationship with the maxillary permanent right lateral incisor in a lingual crossbite. Furthermore, she had mandibular right permanent canine impaction, retained mandibular right primary canine, and MIH in the permanent anterior teeth and first molars. The management involved the removal of the odontoma to allow for orthodontic traction of the lower permanent right canine. After that, the esthetic appearance of the teeth was improved through restorative treatment.

Odontogenic tumors (OTs) are distinct conditions that develop in the jawbones, exhibiting diverse histopathological features and variable clinical behaviors. Unfortunately, the literature on this subject in Saudi Arabia remains sparse, indicating a pressing need for more comprehensive data concerning the frequency, demographics, treatment modalities, and outcomes of OTs. Objectives: The study aims to evaluate the frequency, demographic features, treatment, and outcomes of OTs across three tertiary medical centers. Methods and Material: OT cases were identified in King Abdulaziz Medical City (KAMC), King Fahad Medical City (KFMC), and Prince Sultan Military Medical City (PSMMC) from January 2010 to December 2021. Results: Ninety-two OT cases were identified from the anatomical pathology laboratories of three tertiary hospitals. KFMC contributed the highest number of cases (43.5%), followed by KAMC (30.4%) and PSMMC (26.1%). The median age of OT patients was 29 years (range: 5–83), with males representing more than half of the patients (56.5%). The mandible was the most frequent site of OT occurrence (72.5%), with ameloblastoma being the predominant OT (63.0%), followed by odontoma (19.5%). Among the treatment modalities, bone resection was employed the most (51.0%), followed by enucleation (25.6%). Notably, 11.5% of OT cases with available follow-up data exhibited recurrence, with ameloblastoma accounting for eight recurrent cases. Conclusions: Although OTs are relatively common in the jaws, they are rare in anatomical pathology laboratories and the general population. This study contributes valuable insights into the epidemiology characteristics, treatment trends, and recurrence rates of OTs in Saudi Arabia. [ABSTRACT FROM AUTHOR]

Changes in the expression of nuclear β‐catenin are responsible for tumorigenesis. Beta‐catenin acts synergistically with the TGF‐β/BMPs pathway. This interaction leads to greater dentin deposition and may explain the differences between distinct tooth morphologies and hamartomas. The aim of this study was to investigate the role of β‐catenin, BMP4 and TGF‐β in the development of odontomas. This cross‐sectional, retrospective, immunohistochemical study evaluated 30 compound odontomas, 30 complex odontomas and 17 tooth germs. The results showed that BMP4 and TGF‐β were more immunoexpressed in the ectomesenchyme of complex odontomas (median = 33.7, p < 0.001; median = 76.4, p = 0.002, respectively). Higher immunoexpression of BMP4 and TGF‐β was also observed in the epithelium of tooth germs (median = 2.0, p < 0.001; median = 120.3, p < 0.001, respectively). TGF‐β and BMP4 showed a positive and significant correlation (p < 0.001). Both TGF‐β and BMP4 were positively correlated with nuclear β‐catenin in ectomesenchyme (p = 0.047 and p = 0.023, respectively). Developing teeth exhibited higher concentrations of the proteins studied in odontogenic epithelium, especially during the bud and cap stages. Higher immunoexpression in odontomas occurred mainly in the ectomesenchyme. We therefore suggest that changes in the ectomesenchyme can lead to the development of odontomas. [ABSTRACT FROM AUTHOR]

Most common tumors of odontogenic origin are odontomas, which have slow-growing and non-aggressive behavior. In general, odontomas are more commonly associated with mixed and permanent dentition, with very few case reports of them being associated with deciduous dentition. Odontomas should be surgically managed once detected to avoid eruption disturbances and malocclusion. Odontome is the most common odontogenic tumour impeding the eruption of teeth. They are considered as hamartomatous malformations rather than true neoplasms. Usually, asymptomatic in nature and associated with unerupted permanent tooth. Compound and complex are the two types described by the World Health Organization. Hyperactive odontoblasts, inflammation, infection, trauma and genetic aberrations have been implicated as the factors contributing to the formation of odontoma. Aberrant Wnt/β-catenin signaling pathway and sonic hedgehog pathway were considered to contribute to the development of odontoma. We report two teenagers with complaint of unerupted permanent teeth as a consequence of hamartomatous odontoma in the path of eruption of permanent teeth.

This study aimed to present the case of supernumerary teeth and complex odontoma. It highlights the debate about odontomas, the most common benign tumors of odontogenic origin. Due to their hamartomatous characteristics, they are usually asymptomatic but can cause impaction of one or more teeth. Microscopically, they comprise all the tissue types found in a developed tooth. Here, we report a case of complex odontoma associated with impacted teeth and a supernumerary tooth, followed by bony expansion and failure of eruption in a 15-year-old boy.

Hybrid lesions of jaws are rare entities defined as two different lesions co-occurring in the same location, with identical histopathological origin. Ameloblastoma, calcifying cystic odontogenic tumor and odontoma are among the most common lesions that have been reported to combine with other lesions. In this study, a hybrid lesion of odontogenic keratocyst (OKC) and odontoma in the mandible of a forty-five years old male reported. Additional to the rarity of this hybrid lesion, the present case had unique radiologic features, including atypical location and extension of the lesion and profound knife-edge root resorption of the teeth in the area, which was not a common feature for any of the two lesions. The surgical procedure was marsupialization to reduce the size of the lesion. As a result of the surgery, the healing of the surgical wound was uneventful. In addition, careful follow-up for the patient was conducted, which had no recurrence till now (after 15 months).

Background: Practically every facet of the most common odontogenic tumor, odontoma, has been covered by an extensive volume of literature. However, uncertainty about its precise history has persisted. Materials and Methods: The historical evolution of odontoma was traced with reference to the original illustrations that accompanied European and American reports published at the beginning of the 19th century and also at the turn of the century. Results: The prevailing views regarding the first description of odontoma by Oudet of Paris in 1809 and the original designation "odontome" by Broca of Paris in 1867 are not entirely accurate. Before Broca's suggested term, "exostose dentaire" (dental exostosis) and "tumeur dentaire" (dental tumor) proposed by Oudet and Forget of Paris, respectively, were popular terms adopted in France, while in Briatin the terms "warty tooth" and "supernumerary teeth" proposed by Salter and Tomes of London, respectively, were widely coined. The original illustrations of complex odontoma were published by Wedl of Vienna in 1851, and in 1862 Tomes published the first drawing of compound odontoma denticles. Before the advent of diagnostic radiography in the early 1900s, spontaneous exposure or eruption of odontoma followed by secondary infection was very common. In 1887–1888, Bland Sutton of London criticized Broca's monumental research and formulated the first modern classification which, in essence, remains valid today. At that time, large osteomas of the maxilla were inappropriately classified as odontomas by many pathologists because of Bland Sutton's influential view. Interestingly, the first radiographic evidence of odontoma was published by the American oral surgeon Gilmer in 1899. Conclusion: In view of their fundamental achievements, the names of Wedl, Salter, Broca and Bland Sutton have been closely associated with the true history of odontoma. [ABSTRACT FROM AUTHOR]

Odontogenic tumors are rare tumors of the jaws that arise from remnants of the tooth forming apparatus. Some odontogenic tumors demonstrate strong predilection for pediatric patients including the unicystic ameloblastoma, adenomatoid odontogenic tumor, ameloblastic fibroma, ameloblastic fibro-odontoma, odontoma, and primordial odontogenic tumor. In this review, we discuss the clinical, radiographic, histopathologic, and molecular characteristics of select odontogenic tumors that demonstrate pediatric predilection and review management. Forensic odontology plays a crucial role in establishing the identity in mass disasters and criminal cases with high accuracy. Dental anomalies and features help in such situations. Congenital and developmental dental anomalies can be easily documented to establish distinctive and individualistic characteristics of an individual. The location, number of teeth involved, and the type of anomaly vary between individuals. Similarly, dental malformations also assist greatly in the identification process. Many types of dental anomalies have been studied in the past for their individualistic characteristics in forensic examinations. One such dental anomaly is odontoma, which is a benign odontogenic malformation. This malformation may also help in the identification of the deceased, when recorded and examined accurately. An odontome is a malformed teeth‐like structures consisting of enamel, dentin, and pulpal tissue, formed due to the growth of completely differentiated epithelial and mesenchymal cells. If antemortem (AM) dental records incorporate information regarding odontomes and other dental anomalies, including in radiographs, orthopantomograms or microradiographs, positive identification may be established by comparison of these records with postmortem (PM) records. In the present communication, a rare case of compound composite odontoma in the anterior mandible with multiple denticles has been discussed with a brief overview of congenital and developmental dental anomalies. The authors emphasize the importance of such rare dental anomalies and malformations which may be used for identifying the deceased in mass disasters and forensic identification. Odontomas are relatively asymptomatic odontogenic hamartomatous malformations, commonly in association with impacted or retained primary teeth. They appear as small, solitary or multi-layered radio-opaque lesions which often interfere with tooth eruption. These are diagnosed during routine dental examinations. This case report describes the case of a compound odontoma in a 10-year-old girl associated with an impacted permanent left maxillary incisor. Abstract Macrodontia is the enlargement of tooth dimensions of different ethologies. This work aims to show a histological evaluation of macrodontic teeth in guinea pigs. The material was obtained from animals postmortem. Ninety structural changes derived from 24 guinea pigs were evaluated. All teeth used in the study showed macrodontic changes. The samples were decalcified, dehydrated and embedded in paraffin. Material was cut in the transverse and longitudinal planes in relation to the alveolar bone. Histological evaluation included apical bud cells, pulp cavity cells, periodontium, dentin, enamel, cementum and alveolar bone tissue. Individual elements were evaluated with respect to their morphology and distribution. Moreover, the arrangement of the individual hard tooth structures was assessed on the teeth. No atypia was found among the cells that make up the apical bud, pulp or periodontal cavity. Displacement of periodontal cells or odontoblasts towards the pulp cavity as well as disorganization of the cell system in the pulp cavity were observed. Changes in the dentine ligaments and the reconstruction of the alveolar bone were also observed in areas where dentine and cement systems were affected. Dental slides were observed and showed that the enamel is also involved in structural remodelling of the pulp cavity. The histological assessment revealed that structural changes in macrodontic teeth involve the rearrangement of typical tooth tissues.

Amaç Odontomalar çenelerde en sık görülen odontojenik kökenli iyi huylu tümörlerdir. Yavaş büyürler ve agresif davranış göstermezler. Genellikle asemptomatiktirler ve sıklıkla rutin radyografi sırasında bulunurlar. Kompleks ve kompaund odontoma olarak ikiye ayrılmaktadır. Bu vaka serisinde 8’i komplex 14'ü kompaund, toplam 22 odontoma vakasının cerrahi tedavisinin anlatılması amaçlanmıştır. Olgular Sunumu Yaş aralığı 10-34 arası olan 14’ü kadın 8’i erkek toplam 22 hasta, bölümümüzde gerekli klinik ve radyolojik muayenelerden sonra cerrahi olarak opere edilmiştir. Çıkarılan dokuların histopatalojilerinin, radyolojik ve klinik bulgularla değerlendirilmesi ile 8’inin komplex 14’ünün kompound odontoma olduğu tespit edilmiştir. Bazı vakalarda odontoma ile birlikte gömülü diş alınırken bazı vakalarda diş sürmesinin takibi amacıyla veya çene fraktürü riski sebebiyle etken gömülü dişler bırakılmıştır. İyileşme süresince hastalarda herhangi bir komplikasyon görülmemiştir. Sonuç Odontomalar genellikle asemptomatik görüldükleri için hastaların diş hekimlerine rutin kontrollere gelmeleri, erken teşhiste çok önemlidir. Erken teşhis ile morbidite ve maliyet azaltılabilir.

Purpose: Odontomas represent a common clinical entity among odontogenic tumors, but are not well-addressed in the Vietnamese population. The present study aimed to determine the clinical and preclinical characteristics of odontomas and associated factors in the Vietnamese population. Materials and Methods: This retrospective study retrieved data from histopathological diagnoses from 2 central hospitals of Odonto-Stomatology in Ho Chi Minh City, Vietnam during 2004-2017. The odontomas were classified as complex (CxOD) or compound (CpOD) subtypes. The epidemiological, clinical, and radiological characteristics of the odontomas, stratified by subtype and sex, were obtained and analyzed. Results: Ninety cases, consisting of 46 CxODs and 44 CpODs, were included. The average age of patients was 32.4 (±20.2) years. The patients with CxOD were older than those with CpOD (P<0.05). Clinically, 67% of patients showed an intraoral bone expansion. Approximately 60% of patients with CxOD exhibited a painful symptom, about 3-fold more than those with CpOD (P<0.05), whereas almost all patients with CpOD exhibited perturbations of dentition, unlike those with CxOD (P<0.05). Radiologically, CxOD was characterized by a larger dimension than CpOD in both sexes (P<0.05), and CpOD induced complications in adjacent teeth more often than CxOD (P<0.05). The development of odontoma with advancing age differed significantly in odontoma subtypes related to their pathological origins, and between the sexes, resulting from different physiological states. Conclusion: The findings of this study highlight the value of clinical and radiological features of odontomas and their associated factors for the early diagnosis and adequate treatment of younger patients. Seven tooth-like structures or denticles were seen in the left anterior maxillary region from the lateral incisor to the first premolar region. Ameloblastic fibroma (AF) and ameloblastic fibro-odontoma (AFO) are mixed odontogenic tumors (odontogenic tumors with induction) that are reported only rarely in dogs. These tumors are histologically complex and, to a degree, recapitulate the early stages of tooth development, comprising 2 types of tissue: neoplastic odontogenic epithelium, and induced ectomesenchyme (dental pulp). AFOs are distinguished from AFs by the additional presence of hard dental matrices such as dentin. Herein, we describe the key diagnostic features of AF and AFO in 4 young dogs.

Background: Homeobox genes play crucial roles in tooth morphogenesis and development and thus mutations in homeobox genes cause developmental disorders such as odontogenic lesions. The aim of this scoping review is to identify and compile available data from the literatures on the topic of homeobox gene expression in odontogenic lesions. Method: An electronic search to collate all the information on studies on homeobox gene expression in odontogenic lesions was carried out in four databases (PubMed, EBSCO host, Web of Science and Cochrane Library) with selected keywords. All papers which reported expression of homeobox genes in odontogenic lesions were considered. Results: A total of eleven (11) papers describing expression of homeobox genes in odontogenic lesions were identified. Methods of studies included next generation sequencing, microarray analysis, RT-PCR, Western blotting, in situ hybridization, and immunohistochemistry. The homeobox reported in odontogenic lesions includes LHX8 and DLX3 in odontoma; PITX2, MSX1, MSX2, DLX, DLX2, DLX3, DLX4, DLX5, DLX6, ISL1, OCT4 and HOX C in ameloblastoma; OCT4 in adenomatoid odontogenic tumour; PITX2 and MSX2 in primordial odontogenic tumour; PAX9 and BARX1 in odontogenic keratocyst; PITX2, ZEB1 and MEIS2 in ameloblastic carcinoma while there is absence of DLX2, DLX3 and MSX2 in clear cell odontogenic carcinoma. Conclusions: This paper summarized and reviews the possible link between homeobox gene expression in odontogenic lesions. Based on the current available data, there are insufficient evidence to support any definite role of homeobox gene in odontogenic lesions. [ABSTRACT FROM AUTHOR]

Macrodontia is the enlargement of tooth dimensions of different ethologies. This work aims to show a histological evaluation of macrodontic teeth in guinea pigs. The material was obtained from animals postmortem. Ninety structural changes derived from 24 guinea pigs were evaluated. All teeth used in the study showed macrodontic changes. The samples were decalcified, dehydrated and embedded in paraffin. Material was cut in the transverse and longitudinal planes in relation to the alveolar bone. Histological evaluation included apical bud cells, pulp cavity cells, periodontium, dentin, enamel, cementum and alveolar bone tissue. Individual elements were evaluated with respect to their morphology and distribution. Moreover, the arrangement of the individual hard tooth structures was assessed on the teeth. No atypia was found among the cells that make up the apical bud, pulp or periodontal cavity. Displacement of periodontal cells or odontoblasts towards the pulp cavity as well as disorganization of the cell system in the pulp cavity were observed. Changes in the dentine ligaments and the reconstruction of the alveolar bone were also observed in areas where dentine and cement systems were affected. Dental slides were observed and showed that the enamel is also involved in structural remodelling of the pulp cavity. The histological assessment revealed that structural changes in macrodontic teeth involve the rearrangement of typical tooth tissues. [ABSTRACT FROM AUTHOR]

Odontoma is the most common odontogenic tumor. A literature review revealed limited data for odontomas in Taiwan. This study evaluated the radiographic characteristics of odontomas in 1280 patients in the National Taiwan University Children's Hospital (NTUCH). This retrospective study analyzed the odontomas based on examination of mainly panoramic radiographs and related radiographs of 1280 patients in the NTUCH. Chi-square test was used for trend analysis. Among 1280 patients (710 boys and 570 girls), a total of 16 patients with odontomas (10 boys and 6 girls) were identified. There were 14 compound and 2 complex odontomas. The mean age of the patients was 10.38 years with the majority of odontomas occurring in the first (37.5%) and second decade (62.5%) of life. Odontomas had a marked predilection for the mandible (56.25%) and for the anterior region of the jaws (75%), particularly for the anterior maxilla (43.75%). Ten odontomas (62.5%) were associated with the impacted corresponding permanent teeth. The sagittal position of odontomas and vertical relation of odontomas to the corresponding permanent teeth were significantly correlated with the eruption rate of corresponding permanent teeth (P < 0.05). The odontomas occur most commonly in male patients with a male to female ratio of 1.67:1. The incidence of odontomas in our 1280 patients is 1.25%. The age range of patients with odontomas is 6–17 years. The most frequent location of odontomas is the anterior maxillary region. Introduction: Odontomas are the most common odontogenic tumors. These lesions are usually considered a hamartoma than a neoplasm. Unlike normal tooth morphology, tissue arrangement is irregular in these lesions. Odontoma has self-limiting and slow growth, and if left untreated, its size will not increase over time. Most of these lesions are clinically asymptomatic and are detected on routine radiographs prepared for other purposes. Odontomas usually are located pericoronally and cause tooth eruption disorders such as the lack of permanent teeth or retention of deciduous teeth. In some cases, an odontoma can cause swelling and expansion of the jaw. The purpose of the current report is presenting an unusual complex odontoma in a 10-year-old boy, which was closely related to the roots of tooth #30. Odontomas are usually located pericoronally and near the roots of adjacent teeth, separated by a septum of bone. There are a few reported cases such as the current case, with fusion to the tooth structure. There was slight intraoral swelling and tenderness in the affected region. Unlike most reported cases, tooth eruption disturbance was not observed. Extreme proximity of odontoma and the roots of tooth were detected in Cone-beam Computed Tomography examination. Separative bone septum and the sharp outline of the tooth roots did not detected. The lesion and the related tooth were removed by surgical treatment, and the patient was followed for 2 years.

Introduction: Odontogenic tumors (OTs) are rare and account for around 1% of jaw lesions including tumor and tumor-like lesions. OTs are a diverse category of lesions, displaying different inductive interactions. Although some are hamartomas, others are benign and malignant neoplasms with varying degrees of aggressiveness. Materials and Methods: We plan to review the histopathological aspects of OTs because of their diverse properties. At a tertiary medical and dental institute in Pune, India, we studied 28 cases of OTs over 2 years, from 2013 to 2015. Results: The most common forms of OTs were ameloblastoma (42.86%) and odontogenic keratocyst (21.15%). The highest number of OTs was found in the 21–30-year-old age group, with an even gender distribution. Conclusion: Ameloblastoma is the most common OT, accounting for roughly one-quarter of all jaw tumors. The majority of OTs occurs in young people aged 20–40 years. Fortunately, malignant OTs are extremely rare.

Peripheral odontogenic neoplasms have similarities with other peripheral tumoral and nontumoral lesions. It is difficult to obtain proper diagnosis clinically and it is mostly based on the histopathologic features. Moreover, differential diagnosis of oral lesions differs between children and adults, and as a result, reports of such cases help better diagnose the lesions in this age group. Peripheral odontoma (PO), unlike central types, is uncommon. However, they have the same features as their intraosseous counterparts histopathologically. Here, we report the case of PO in a pediatric patient which was present from his infancy.

Compound odontoma is a malformation typical of young adults below the age of 20, with a slight preference for the male gender and the anterior region of the maxilla. Clinically asymptomatic, it can be detected during a radiological investigation in connection with the persistence of deciduous dental elements and the impaction of definitive ones. The treatment of choice is excisional surgery and recurrence is a rare event. The need for orthodontic therapy for impacted elements is usually not necessary because in most cases, odontomas are small, circumscribed lesions the size of a permanent tooth. In this article, the diagnostic and therapeutic surgical excision procedure is presented in three patients at developmental age with large compound odontomas associated with at least one retained canine, and in two of the cases, with serious transmigration to the impacted tooth elements. Odontoma is a hamartoma or developmental malformation of dental tissues and often causes delayed or impacted permanent teeth. This paper reports a case of surgical removal of an odontoma that is closely attached to the labial bone plait and examines the extraction of accompanying high-impacted maxillary canine beside the nose and maxillary sinus. Digital technology was used in reconstructing a part of a patient's maxilla, odontoma, related teeth, and other adjacent anatomical structures, accurately determine the lesion boundary, and design an optimal surgical plan. The minimally invasive extraction of lesions and embedded teeth with a double-fenestration technique through the upper and lower paths not only preserved the continuity of the labial alveolar bone plate in the operative area and prevented postoperative bone collapse but also reduced the risk of maxillary sinus mucosal injury and perforation, improved surgical efficiency, and reduced the surgical trauma. These features were in line with the concept of minimally invasive alveolar surgery.

Odontoma is an occasionally encountered condition that disturbs the eruption of adjacent teeth. Few reports have described multiple odontomas occurring at two adjacent sites, resulting in eruption disturbances of both primary and permanent teeth. An 8-year 2-month-old boy was referred to our hospital. Oral examination revealed that the maxillary left first molar and primary second molar were absent, and radiographic examination showed multiple compound odontomas in two regions near these unerupted teeth. The first molar gradually erupted after removal of the odontoma and excision of overlying gingiva around the tooth crown. The maxillary left second premolar spontaneously erupted at 9 years 6 months of age, but the impacted primary second molar and surrounding odontoma were located near the bottom of the maxillary sinus. The treatment plan was required to consider the completion of second premolar root development, followed by removal of the impacted primary second molar and remaining odontomas. In this case, the multiple odontomas were suspected to have disturbed the eruption of both primary and permanent teeth, and the degree of positional abnormality varied between the two teeth.
