Abstract Introduction Primary central nervous system lymphomas are infrequently occurring lymphomas that account for only 0.3-1.5% of all intra-cranial neoplasms in patients without acquired immune deficiency syndrome. However, a pure third ventricle lymphoma is extremely rare. Here, we discuss the similar radiological appearances of lesions localized in the third ventricle and the importance of accurately diagnosing primary central nervous system lymphomas for favorable treatment outcomes. Case presentation A 38-year-old Caucasian man from Turkey presented with a severe headache lasting for three months that failed to respond to any medication. Both severity and duration of the symptoms increased gradually, resulting in vomiting, nausea and gait disturbance that accompanied the headache for three weeks. Neuro-imaging studies showed a lesion located solely in the third ventricle, resulting in partial obstruction of the foramen of Monro. The pre-operative diagnosis was a colloid cyst. Following the surgical procedure, the results of pathological and immunochemical assays revealed that the pre-operative diagnosis was incorrect and that the lesion was a primary central system lymphoma. Conclusion Pure third ventricle lymphomas are extremely rare and are exceptionally localized. It is important to be aware of, and to differentiate between, other possible third ventricular lesions that may mimic the same radiological appearance. Accurate diagnosis is necessary for selecting appropriate treatment modalities.

Background and Objectives: This study aimed to assess if extensive surgical resection enhances outcomes in wild-type Isocitrate Dehydrogenase (IDH) butterfly glioblastoma (B-GBM) patients, despite the presence of Telomerase Reverse Transcriptase (TERT) mutation and Epidermal Growth Factor Receptor (EGFR) amplification., Methods: The study, retrospectively conducted from 2014 to 2022, involved 723 GBM patients, 41 of whom met the criteria for IDH wild-type B-GBM. Exclusion criteria comprised prior diagnoses or treatments for low-grade glial tumors. Surgeons, employing two approaches-partial and extensive surgery-categorized patients based on age, sex, tumor location, corpus callosum involvement, and genetic characteristics. The interval between initial surgery and tumor recurrence/tumor-free period (TR/TFP) and overall survival (OS) were recorded and compared between the partial and extensive resection groups, analyzing the impact of resection width on TR/TFP and OS. Preoperative assessments utilized thin-section cranial computed tomography (CT) and contrast-enhanced magnetic resonance imaging (MRI). Intraoperatively, tumor excision was guided by sodium fluorescein, and margins were delineated via neuronavigation. Genetic alterations (TERT mutations and EGFR amplifications) were correlated with surgical type, TR/TFP, and OS. Karnofsky Performance Scale (KPS) evaluations were performed pre- and post-operatively and at key intervals, comparing outcomes between surgical groups. Standard radiotherapy and chemotherapy regimens were administered to all patients., Results: Extensive resection yielded significantly longer TR/TFP compared to partial resection, despite TERT gene mutation and EGFR amplification being linked to shorter TR/TFP and OS. Its impact on OS, however, was not significant. Background: Biomechanical benefits of Total Disc Replacement (TDR) including both the restoration of normal segmental range of motion and the prevention of physiological lumbar lordosis encourage spine surgeons to perform TDR for lumbar disc disease., Methods: A total of twenty patients (mean age: 39.5) who had degenerative disc disease with unilateral disc herniation were operated on between 2003 and 2006. Microscopic anterior lumbar discectomy with TDR placement via a transperitoneal approach were performed. Each patient was evaluated using a VAS and the Oswestry index., Results: Mean ODI improved from 73.3 preoperatively to 35.0 and 20.4 at 3 and 12 months of follow-up respectively (P < 0.001). The mean VAS score improved from 8.65 preoperatively to 2.6 and 1.9 at 3 and 12 months respectively (P < 0.001)., Conclusions: Results from this series are promising and indicate that placement of TDR for degenerative disc disease with lumbar disc herniation is a valuable alternative to conventional techniques. The main advantages of this application are preservation of spinal stability, early mobilization, restoration of normal segmental range of motion and elimination of problems related to intervertebral disc tissue such as discogenic pain and recurrence of disc herniation.

Objective: Cervical laminoplasty is an effective procedure. We describe the technique of open-door and french-door cervical laminoplasty and present our clinical results., Methods: All patients undergoing laminoplasty at our clinic during the 1997- 2008 period were included in the study. All patients had myeloradiculopathy. Neurological functions and recovery in myelopathy in patients were evaluated using the Nurick score. Pre- and postoperative kyphotic evaluation was measured using the sagittal tangent method. Peri- and postoperative complications were recorded., Results: Clinical evaluation showed that all patients had reduced postoperative complaints compared to their complaints before the procedure. Average time of surgery was 180 min. Average blood loss in surgeries was 300 cc. According to the Nurick classification, no improvement was observed in seven patients; nine patients showed improvement of one grade; and three patients showed an improvement of two grades. Temporary C5 nerve root paralysis was observed in two patients as a postoperative complication. Complete recovery was observed in both patients within 2 months., Conclusions: Laminoplasty is a safe and effective alternative procedure to treat cervical spondylotic myelopathy.

Object: The authors conducted a study to assess the effects of cervical posture on the loadbearing ability of the cervical spine., Methods: Twelve cervical spine specimens obtained in 12 adult sheep were tested. The specimens were randomly separated into two groups. In Group I the specimens were fixed in a lordotic posture, and in Group II they were fixed in a straight posture. Axial compressive loads were applied at a constant rate of 5 cm/minute. Load-to-failure, time-to-failure, piston displacement at failure, and failure modes were recorded. Statistical analyses were performed to detect differences between the groups. There was no significant difference in load-to-failure values between the two groups. However, the time-to-failure and the piston displacement values for the straight spines were significantly less than those for the lordotic spines. Additionally, the straight spines failed predominantly through ventral elements, whereas the lordotic spines predominantly failed dorsally., Conclusions: It is concluded that a loss of a lordosis increases the risk of injury to the cervical spine following axial loading.
