12. Effects of idiopathic erythrocytosis on the left ventricular diastolic functions and the spectrum of genetic mutations: A case control study

Abstract

Background: We have aimed at exposing left ventricular diastolic functions and the presence of known genetic mutations for familial erythrocytosis, in patients who exhibit idiopathic erythrocytosis. Methods: Sixty-four patients with idiopathic erythrocytosis (mean age, 46.4 +/- 2.7 years) and 30 age-matched healthy subjects were prospectively evaluated. The regions of interest of the erythropoietin receptor, hemoglobin beta-globin, von Hippel-Lindau, hypoxia-inducible factor 2 alpha, and Egl-9 family hypoxia-inducible factor genes were amplified by PCR. Left ventricular (LV) mass was measured by M-mode and 2-dimensional echocardiography. LV diastolic functions were assessed by conventional echocardiography and tissue Doppler imaging. Results: As a result of genetic analyses, genetic mutations for familial erythrocytosis were detected in 5 patients. It has been observed in our study that the risk of cardiovascular disorders is higher in patients. Interventricular septum thickness, left atrial diameter, and some diastolic function parameters such as deceleration time and isovolumetric relaxation time have been found to be significantly higher in idiopathic erythrocytosis group than in the controls. Conclusion: This study has shown that LV diastolic functions were impaired in patients with idiopathic erythrocytosis. In this patient group with increased risk of cardiovascular disorders, the frequent genetic mutations have been detected in 5 patients only. 13. Comparison of CO-RADS, RT-PCR and chest CT score; a report of 519 cases.

Abstract

Aim: Analysing the efficancy of The Dutch COVID-19 Reporting and Data System (CORADS) classification to evaluate the severity of the disease of patients with COVID-19 infection and investigating its diagnostic performance with the correlation between reverse transcriptase-polymerase chain reaction (RT-PCR) test results and severity of the disease. Materials and Methods: Retrospective evaluation of the RT-PCR and the Computed Tomography (CT) imaging results of 519 patients who were clinically accepted and threated as Covid-19. Inclusion criteria: All patients over the age of 18 who underwent CT with a pre-diagnosis of covid were included. The study excluded patients who were under the age of 18, pregnant, in poor general health but unable to undergo a CT scan, whose data could not be retrieved, or whose data were input insufficiently. Three radiologists evaluated the Chest CT images by using the CO-RADS and the CT Score classifications. Relationship between CORADS, Chest CT Score and RT-PCR results were demonstrated. Demographic data, clinical features, comorbidities, hospitalization rates and intensive care unit admission were recorded and correlated with CORADS and CT Score classification. Results: The average age was 46.41±17.37 (range 18-95). 232 cases were women, while 287 cases were men. 278 (53.5%) of 519 patients had positive RT-PCR results. According to kappa results; there is no agreement between CORADS and RT-PCR, CORADS and CT Score. 278 (53.6%) of 519 patients had positive RT-PCR results. According to the clinical symptoms; only sore throat was correlated with CORADS while fever, cough and dsypnea were not. Hospitalization and ICU admission were higher in CORADS 2-5 group. 84 of 165 patients with CORADS 1 had RT PCR positive results; while160 of 354 patients with lung involvement (CORADS 2-5 group) on CT images were RT-PCR negative. Conclusion: Using CO-RADS classification with RT-PCR improves more accurate diagnosis. Abstract

Inflammation, cytokine storm and lung damage associated with COVID-19 can cause the initiation of the thromboembolic process in patients. The 73-year-old male patient applied with sudden-onset shortness of breath to the emergency department of our hospital. Physical examination revealed an SpO2 of 80% in room air, respiratory rate of 27/min. As the chest CT scan was compatible with COVID-19, the patient was admitted to the hospital and empirical treatment for COVID-19 was started, comprising of favipravir and dexamethasone. The RT-PCR of SARS-CoV-2 test resulted positive one day later. Due to the D-dimer value that was higher than expected in COVID-19, arterial blood gas compatible with thromboembolism, and the accompanying high lactate level, a CT angiogram was ordered and was compatible with pulmonary embolism, showing partial embolic filling defects in the lobar branches in the inferior lobes, and the lingular branch of the left lung. A closer investigation of pulmonary embolism in high-risk patients during the pandemics, and prescribing anticoagulant therapy starting on especially the first day of symptoms are vital to prevent potential complications of possible thromboembolism. Abstract

Background: The efficacy of dapagliflozin has been less extensively studied in the real-life context, limiting the comparability of observational real-world evidence with results from dapagliflozin clinical trials. This study aimed to evaluate HbA1c, blood pressure (BP), and weight outcomes in dapagliflozin-treated patients with type 2 diabetes (T2D) treated in a real-world setting. Methods: A total of 1683 T2D patients (mean (SD) age: 54.6 years (9.1), 56.6% were females) initiating dapagliflozin were included in this multicenter retrospective observational cohort study. Data on patient demographics, comorbidities, duration of diabetes, and concomitant antidiabetic treatment were recorded. Change in glycated hemoglobin (HbA1c), body weight, body mass index (BMI), and BP levels with dapagliflozin treatment and dapagliflozin discontinuation rates were evaluated based on baseline, 3rd-month, and 6th-month data. Results: At the end of 6-month dapagliflozin treatment (discontinued in 4.3% of patients), HbA1c levels were ≤7% in 30.1% of patients, BP was ≤140/90 mmHg in 95.9% of patients, and >5% weight loss was evident in 38.0% of patients. Less than 5-year diabetes duration was associated with significantly higher rate of achieving HbA1c ≤7% (p = 0.032), BP ≤140/90 mmHg (p = 0.001), and BP ≤130/80 mmHg (p < 0.001) targets at the 6th month (p = 0.032) as compared with longer diabetes duration. Conclusion: In conclusion, this nationwide observational study in T2D patients treated with dapagliflozin in Turkey provided real-life evidence on significant and clinically meaningful reductions in HbA1c, body weight, and BP within 6-month therapy. Abstract

Background: Increasing amounts of evidence suggest patient-related systemic inflammatory response (SIR) as a powerful prognostic factor in cancer and applicability of SIR as a prognostic factor has been investigated. Aim: To evaluate the prognostic significance of SIR, which is among routinely analysed blood parameters in patients with all stages of gastric cancer (GC). Methods: A total of 245 patients with gastric cancer who were followed up and treated in four clinics of medical oncology were included in the study. At first admission of the patients, from routinely determined whole blood cell counts in medical oncology clinics, their neutrophil/lymphocyte ratio (NLR) and platelet/lymphocyte ratio (PLR) values were estimated and recorded before initiating chemo- or radiotherapy. A univariate non-parametric analytical method and chi-square test examined the correlation between prognostic factors, and survival rates. Survival curves were estimated using the Kaplan-Meier method. Results: Sixty-eight (27.8%) female and 177 (72.2) male patients (total n=245) were included in the study. When NLR was used as an indicator of SIR, 108 (44.1%) patients were SIR negative and 137 (55.9%) patients were SIR positive. When PLR was used as an indicator of SIR, SIR negativity and positivity were detected in 93(38%) and 152 (62%) patients, respectively. A statistically significant correlation was found between status of lymph node metastasis, stage of the disease and NLR (P=0.001, P=0.017). SIR determined with PLR was found to be correlated with the depth of tumor invasion and stage of the disease (P=0.016, P=0.033). A significant correlation was not detected between PLR and survival (P=0.405). Conclusion: According to our study, parameters of NLR and PLR calculated preoperatively from peripheral blood samples can be used in patients with various sizes of tumours in different disease stages. Still based on our results, NLR calculated during diagnostic workup is a parameter with a prognostic value. In addition, NLR is a determinative factor in the selection of surgical method and chemotherapeutic modalities, which also functions as a potential contributory marker in effective immunotherapeutic strategies.

Abstract

Introduction: Metabolic syndrome is a systemic disorder and manifests as a group of conditions including abdominal obesity, dyslipidemia, hypertension and coronary artery disease. The importance of epicardial adipose tissue has been proven through recognition of its contribution to inflammation by pro-inflammatory cytokine discharge. Several investigations have been performed on vitamin D receptors in different tissues. In this study, epicardial adipose tissue thickness (EATT) and the levels of vitamin D were measured and compared with a healthy control group. Material and Methods: 84 patients who had metabolic syndrome without diabetes and 64 healthy individuals were enrolled into the study. In all patients, the EATT was calculated by ecocardiography and the level of serum 25 (OH) vitamin D was measured. Results: It was observed that EATT in patients with metabolic syndrome increases significiantly compared to the healthy control group (P < 0.001). No significant difference between patients and control group was found for the levels of 25 (OH) vitamin D (P = 0.507). There was no correlation between 25 (OH) vitamin D and EATT (P = 0.622). Conclusions: We observed that EATT increased in patients with metabolic syndrome. In contradiction to literature; the levels of 25 (OH) vitamin D was not found to be high in patients with metabolic syndrome. Any significant correlation was not found between EATT and 25 (OH) vitamin D levels. Further studies with a larger patient population are required to assess the relationship.
