The mental health treatment gap remains wide across the world despite mental illness being a significant cause of disability globally. Both end-user and healthcare provider perspectives are critical to understanding barriers to mental healthcare and developing interventions. However, the views of providers are relatively understudied. In this review, we synthesized findings from current literature regarding providers' perspectives on barriers to mental healthcare in Canada. We searched Medline, PsycINFO, Embase, and CINAHL for eligible Canadian studies published since 2000. Analysis and quality assessment were conducted on the included studies. Of 4,773 reports screened, 29 moderate-high quality studies were reviewed. Five themes of barriers emerged: health systems availability and complexity (reported in 72% of the studies), work conditions (55%), training/education (52%), patient accessibility (41%), and identity-based sensitivity (17%). Common barriers included lack of resources, fragmented services, and gaps in continuing education. Interestingly, clinicians often cited confusion in determining the ideal service for patients due to an overwhelming number of potential services without clear descriptions. These five domains of barriers present a synthesized review of areas of improvement for mental healthcare spanning both patients and clinicians. Canadian mental health systems face a need to improve capacity, clinician training, and in particular service navigability and collaboration.

China’s rapid growth has led to environmental, health, and economic challenges due to the widespread use of solid fuels such as coal and biomass. This issue is particularly prevalent in rural areas and affects 2.4 billion people globally. The harmful compounds released from burning these fuels are linked to various health issues. This review aims to summarize the health implications of solid fuel use in China and provide recommendations for cleaner cooking fuels. The study investigates the link between solid cooking fuel use and noncommunicable diseases (NCDs) in China. It includes data from 2000 to 2022, with a focus on indoor air pollution and NCD development. The research adheres to the Preferred Reporting Items for Systematic Reviews and Meta-analyses 2020 guidelines for systematic reviews and meta-analyses. The search yielded 16 studies examining the link between solid cooking fuel use and NCDs in China. These studies covered a range of NCDs and indoor pollutants across 26 provinces, offering a comprehensive exploration of the relationship between indoor air pollution and NCDs. The findings revealed a significant prevalence of chronic obstructive pulmonary disease and lung cancer attributable to indoor air pollution exposure. The study underscores the need for public health intervention to reduce the burden of NCDs and promote the adoption of clean fuel. The evidence provided could motivate policies to encourage cleaner fuel usage.

To ascertain whether lecturers had the pedagogical competency skill needed in impacting students with necessary skills they need outside school, this study developed a scale to measure Lecturers' Pedagogical Competence (LPCS) in universities using item response theory. The pedagogical competence of university lecturers was assessed using a set of items designed and evaluated the scale's construct validity. The initial scale consisted of a 70-item four options Likert-type. The multi-stage sampling procedure was employed. Proportional sampling was used to choose four universities in South Western Nigeria. At each of the institutions selected, 20% of the staff population was selected from all faculties using stratified random sampling with a cadre as a stratum. Thus, 906 lecturers from federal universities and 294 were selected from federal state universities, respectively. In all, 1,086 of the 1200 lecturers selected completed the study's instrument. The results showed that four of the six optimal factors underlying lecturers' pedagogical competence were reliable. Eventually, 20 can measure LPC. Furthermore, the LPCS showed validity by optimally measuring the theoretical construct underlying the scale. Evaluation of learning outcome; managing student behavior during lecture; using ICT to Enhance learning; and lecturers' student interaction remains the most important factors in LPCS.

Abstract Background and aim The burden of orofacial clefts (OFCs) has declined globally except in sub-Saharan Africa, with a great disease burden in North Africa. Children with OFCs have a high risk of malnutrition, and African countries have some of the highest malnutrition rates. This scoping review assessed the status of research on OFCs and nutrition and feeding among children living in African countries. Methods We followed the Joanna Briggs Institute guidelines for conducting scoping reviews. We searched eleven databases for articles on malnutrition and feeding among children with OFCs living in African countries. No restriction was done by type of study or publication date. Books, book chapters, and reviews were excluded. Only publications in English language were included. We extracted information about the publication year, study design, setting, location, participants’ age, data collection methods, international collaboration, and funding. We classified articles into studies assessing (1) the impact of nutritional deficiencies during pregnancy on OFCs, (2) the impact of OFCs on malnutrition, (3) feeding problems in children with OFCs, and (4) the impact of nutritional status on OFCs repair outcomes. We calculated frequencies and used bar charts and a map. Results Out of 208 search results, 36 were duplicates, and 25 eventually fit the inclusion criteria, with 52% retrieved from Google Scholar. About 80% of the studies were from four countries: Nigeria, South Africa, Ghana, and Uganda; 72% were hospital-based and 52% were cross-sectional. The most frequent data collection method was clinical examination and questionnaires. Most studies focused on feeding problems in children with OFCs (44%) and the impact of OFCs on malnutrition (32%). International collaboration was observed in six studies, with one study showing South-South collaboration. Only two studies were funded. Conclusions There is a predominance of under-funded descriptive research not indexed by international databases. Minimal research has been directed to population-level OFC preventive programs in primary healthcare settings and assessing interventions supporting children with OFCs. A research agenda is needed to prioritize research needs and secure funds to support South-South collaboration to address the nutrition and feeding-related problems associated with OFCs.

Abstract Mentorship, consisting of its official and unofficial forms, transforms medical trainees into competent and well-rounded physicians. In this commentary, we reflected on existing mentorship programs at the McMaster School of Medicine, highlighted lessons from a new peer-based mentorship program, and revisited the benefits of integrating diverse mentorship styles to enhance medical education.

ABSTRACT: Botanical extracts (BE; Apex, Adisseo) have demonstrated enhanced DMI and improved gut health, whereas direct-fed microbials (DFM), such as Lactobacillus acidophilus fermentation product (Excell [EX], Pacer Technology Inc.), have demonstrated improved gut health and growth performance of growing Holstein calves. The hypothesis was this combination may be synergistic to neonatal calf growth performance and intestinal health. Eighty 2- to 5-d-old Holstein bull calves were blocked by BW and randomly assigned to 1 of 8 treatments arranged in a 2 × 4 factorial using a randomized complete block design. The main factors were milk replacer (MR) without (control) and with EX added at 5 g/d fed and calf starter (CS). The CS containing no additives (control); CS containing BE at 496 mg/kg; CS containing EX at 2.50 g/kg; and CS containing BE and EX at the same inclusion rates. The MR were fed 2×/d at 0630 and 1800 h along with free choice CS (amounts and orts weighed daily) and water. Weaning occurred after d 42 for the 56-d experiment. No MR by CS main effects interactions were detected for BW, ADG, CS intake, total DMI, feed efficiency, or body frame gain parameters. The BW gain (38.0 and 39.3 kg for control and EX, respectively) for MR main effect was similar for calves fed both MR treatments, whereas CS main effects (38.7, 39.7, 39.2, and 37.2 kg for control, BE, EX, and BE+EX, respectively) was similar among all CS. Gains in body length (10.6 and 10.8 cm), hip width (4.5 and 4.5 cm), withers height, (10.5 and 10.6 cm), heart girth (18.6 and 19.9 cm), and body length (9.1 and 7.9 cm) were similar for calves fed both MR, while CS main effects for hip height (10.5, 10.2, 10.3, and 10.9 cm), hip width (4.7, 4.6, 4.4, and 4.3 cm), withers height (10.7, 10.9, 10.3 and 10.6 cm), heart girth (19.9, 18.9, 18.9, and 19.4 cm), and body length (11.7, 9.1, 8.3, and 8.4 cm) were similar. Total days of a fecal score = 0 was greater for calves fed control MR and BE CS compared with calves fed control MR and the combination of BE+EX, with calves fed the remaining treatments being intermediated and similar. This study demonstrated little calf growth performance and health benefits when feeding a BE or EX alone or in combination compared with calves fed control treatments.

Abstract Reference genomes of cattle and sheep have lacked contiguous assemblies of the sex-determining Y chromosome. Here, we assemble complete and gapless telomere to telomere (T2T) Y chromosomes for these species. We find that the pseudo-autosomal regions are similar in length, but the total chromosome size is substantially different, with the cattle Y more than twice the length of the sheep Y. The length disparity is accounted for by expanded ampliconic region in cattle. The genic amplification in cattle contrasts with pseudogenization in sheep suggesting opposite evolutionary mechanisms since their divergence 19MYA. The centromeres also differ dramatically despite the close relationship between these species at the overall genome sequence level. These Y chromosomes have been added to the current reference assemblies in GenBank opening new opportunities for the study of evolution and variation while supporting efforts to improve sustainability in these important livestock species that generally use sire-driven genetic improvement strategies.

Background: Some adults with intellectual disabilities do not acquire oral feeding skills and are dependent on gastrostomy tube supplementation. Methods: Two adults with intellectual and multiple disabilities received intervention for oral consumption during daily meals (standardised food and liquid quantities, individualised procedural guidelines, and consumption-contingent consequences) while the frequency and amount of gastrostomy tube feedings were gradually decreased and eliminated. Results: Compared to baseline (pre-intervention) conditions, the adults increased oral consumption of food and liquid during the intervention and gastrostomy tube supplementation was eliminated. These intervention results continued through multiyear follow-up, one participant lost considerable weight, the second participant maintained weight, and both participants had good health (no hospitalisations, illnesses, and missed medications) and benefited from eating daily meals among their peers. Conclusions: Behaviourally-based intervention with interdisciplinary collaboration and appetite stimulation strategy can establish and improve oral feeding in adults with intellectual and multiple disabilities who were previously gastrostomy tube dependent.

Abstract Background Insects act as disease vectors, spreading disease-causing organisms between plants and animals. There have been studies devoted to determining ways to control these pests. One of the most effective ways to accomplish this is to reduce their vector competency. This review article explains how these factors can reduce vector competency. Main body The major ways by which vector competence can be reduced were reviewed. Entomopathogens are organisms that cause disease in arthropods like insects, mites, and ticks. Wolbachia is a genus of intracellular bacteria that mostly infects arthropods, including a significant number of insects. It is one of the most frequent insect reproductive parasites that kill or severely disables insects. Entomopathogenic nematodes are a type of worm that attacks insects and kills them. Insect-specific viruses are a relatively new class of viruses with a variety of intriguing traits that could be used to better understand and possibly inhibit arbovirus transmission. Entomopathogenic fungi are a type of fungus that kills insects by attacking and infecting their insect hosts. Disrupting the environment and nutrition of insects could also help to reduce their ability to spread diseases to humans, animals, and plants. Conclusions Chemical control has been one of the most widely used methods for controlling disease vectors, but there have been reports of insect resistance, environmental degradation, and a variety of other side effects. Instead of chemical control, there are a variety of techniques that can be used, including targeting insects' endosymbionts (bacteria, viruses, fungi, nematodes, and protozoa), changing insects' nutrition, manipulating their environment, and many others. This paper discussed the alternative ways to reduce vectors’ competence without the use of synthetic chemical.

Globally, effective and efficient healthcare is critical to the wellbeing and standard of living of any society. Unfortunately, several distant communities far from the national grid do not have access to reliable power supply, owing to economic, environmental, and technical challenges. Furthermore, unreliable, unavailable, and uneconomical power supply to these communities contributes significantly to the delivery of substandard or absence of qualitative healthcare services, resulting in higher mortality rates and associated difficulty in attracting qualified healthcare workers to the affected communities. Given these circumstances, this paper aims to conduct a comprehensive review of the status of renewable energy available to rural healthcare clinics around the globe, emphasizing its potential, analysis, procedures, modeling techniques, and case studies. In this light, several renewable energy modeling techniques were reviewed to examine the optimum power supply to the referenced healthcare centers in remote communities. To this end, analytical techniques and standard indices for reliable power supply to the isolated healthcare centers are suggested. Specifically, different battery storage systems that are suitable for rural healthcare systems are examined, and the most economical and realistic procedure for the maintenance of microgrid power systems for sustainable healthcare delivery is defined. Finally, this paper will serve as a valuable resource for policymakers, researchers, and experts in rural power supply to remote healthcare centers globally.

Abstrakt: Menschen sind soziale Tiere. Sie leben in großen Groupen und brauchen gesellschaftliche Interaktionen. Aufgrund der Masse verschiedener Individuen wird schon deutlich, dass dabei nicht immer eine heitere Atmosphäre herrschen kann. Der Konflikt, der sich aus einer möglichen toxischen Beziehung unter einer bestimmten Bevölkerung ergeben kann, führt zu verbalen Reaktionen, die in den meisten Fällen irritierend, ärgerlich und provozierend sind. Diese hauptsächlich verbale Reaktionen kōnnen in Form von Invektiven/Schimpfwörtern erfolgen. Der Beitrag untersucht die zentrale Stellung, die Invektiven sowohl in der deutschen als auch in der yorubaischen Sprache und Kultur einnehmen. Während auf ihre negativen Konnotationen in beiden Kulturen geachtet wird, wird in der Arbeit behauptet, dass sich deutsche Beschimpfungen in Art und Verwendung signifikant von Yoruba-Beschimpfungen unterscheiden. Der Beitrag identifiziert, kategorisiert, vergleicht und kontrastiert die unterschiedlichen Kontexte und Dimensionen ihrer Verwendung sowohl in der deutschen als auch in der Yoruba-Sprache und -Kultur. Background: Pregnancy and polymorphisms in drug disposition genes alter the clearance of key antiretrovirals used as part of regimens for prevention of mother-to-child transmission of HIV (PMTCT). The clinical significance of these in women initiating therapy late in pregnancy has not been investigated. The primary objective of the Viral and Antiretroviral Dynamics in HIV Mother-To-Child Transmission Fluids (VADICT) study is to investigate viral and antiretroviral dynamics in matrices associated with mother-to-child transmission (MTCT) (plasma, genital fluid and breastmilk) in women (stratified by CYP2B6 genotypes) who initiate antiretroviral therapy (ART) before or early in pregnancy versus late in pregnancy or early postpartum. Methods: A cohort of HIV-1 infected women who initiated ART containing 600 mg efavirenz before or early in pregnancy (n = 120), during the third trimester (n = 60), or early postpartum (n = 60) will be studied.  Eligible patients will be recruited from four hospitals in Benue State, North Central Nigeria and followed until the end of breastfeeding. Procedures at follow up visits will include sample collection for drug quantification and HIV-1 RNA and DNA in plasma, genital fluid and breastmilk; adherence monitoring; and newborn and infant assessment. Using newborn exposure to maternal efavirenz at birth for validation, prenatal pharmacogenetics of efavirenz will be explored using physiologically-based pharmacokinetic modelling. Three integrated methods will be used to monitor patterns and correlates of adherence across pregnancy and the breastfeeding period. A population pharmacokinetic-pharmacodynamic model will be developed to describe the observed data and simulate what to expect in women initiating ART containing 400 mg efavirenz (recently approved for non-pregnant adults) late in pregnancy or early postpartum. Discussion: This study will help in understanding residual MTCT in women receiving ART and reasons for the rise in MTCT risk during the breastfeeding period. Introduction: Climate change is the public health crisis of our time, with young people particularly at risk. Climate change has been associated with increased prevalence of psychiatric disorders. Psychological concerns pertaining to the Earth's future have also been cited as contributing to negative emotions now termed ‘climate distress’. While previous reviews have addressed the various ways climate change affects pediatric mental health, this scoping review aims to specifically explore pediatric climate distress and its implications for clinical practice. Methods: 2548 articles were extracted from multiple databases, titles, abstracts, and full texts were screened blinded and in duplicate using the web-based platform Covidence. Quantitative and qualitative original research papers published in English between January 1, 2000 and April 29, 2024 that studied pediatric climate distress were included. Results: Forty-two articles met the inclusion criteria, along with 10 additional grey literature sources. Among quantitative studies, 81 % found that 50 % or more of respondents reported negative climate emotions and 86 % of qualitative studies reported that “all” or “most” respondents expressed negative climate emotions. Additionally, 63 % of studies measuring distress severity reported high distress levels. Therapeutic interventions addressing climate distress were found to be effective and were categorized thematically into three groups: Education-Emotion Focused, Nature-Engagement Based, and Activism-Civic Related. Conclusions: This review shows that while there is a growing body of literature that illustrates how young people have negative emotions pertaining to climate change, there is need for increased diagnostic and therapeutic approaches to clinically address these growing challenges.

Water utilization for different human activities is universally crucial, but it is not readily available for consumption in some areas, such as the central parts of Lagos State, Nigeria. Moreover, there are many groundwater controlling factors (GWCFs) spanning geological, geophysical, and hydrological factors that contribute to this scenario, coupled with uncertainties that necessitate their careful selection using appropriate modelling techniques. In this study, the Dempster–Shafer Theory of Evidential Belief Function (DST-EBF) model (a coupling approach) was deployed to produce a groundwater prospectivity zonation (GWPZ) map for the study areas. The choice of the selected locations was informed by the continued dearth of water supplies, necessitating the need to discover new locations or re-appraise the existing ones for groundwater resource development. Furthermore, remote sensing, geological, field geophysical, and hydrological datasets that constituted GWCFs were integrated into a Geographic Information System (GIS) environment. Next, the computed values for the evidence of the mass functions (i.e., belief, disbelief, uncertainty, and plausibility) were combined using the Dempster–Shafer combination rule and then interpolated using the Inverse Distance Weighted (IDW) method. Subsequently, the GWPZ map was generated and classified into five zones, ranging from very low to high prospectivity zones. The GWPZ map was validated using cross-validation to estimate statistical errors, the receiver operating characteristic (ROC) curve, and the use of inverted resistivity models from the 2D electrical resistivity imaging surveys. All the metrics used for the validation provided good account for the classified GWPZ map. The north-eastern and the south-central parts are the most promising regions for groundwater, which could be harnessed for sustainable development through borehole drilling. Thus, the integration of the DST-EBF model and GIS for effective groundwater resources and uncertainty mapping was quite successful and impressively reliable.

Studies have shown the unlimited benefits attributed to using microbial fuel cell (MFC) in our scientific societies for the conversion of organic waste materials into bio-energy with the aid of micro-organisms catalyzed anode and enzymatic-microbial influenced cathode via electrochemical reactions. However, numerous shortcomings calling for serious investigations have equally been identified despite the inestimable benefits of this clean energy technology. A major one is the need for MFC cathode improvement to enhance bioenergy generation. The current situations and challenges of using MFC together with loop holes that are needed to be filled by researchers were critically explained. Additionally, this paper examines the importance of microbial fuel cells (MFCs) as a source of renewable energy generation. The major focus was on techniques of improving the output of MFCs via its cathode. The addition of catalyst on cathode surface, upscale integration of MFCs and antifouling of gas diffusion electrodes used in MFCs were discussed. Relevant areas that can be considered for future research on improvement of MFCs cathode for long term operation were stated.

Abstract Background Adverse childhood events (ACEs), psychopathy, and self-harming behaviours are prevalent among individuals in the forensic psychiatry system. While existing literature suggests that ACEs, self-harm, and psychopathy are interrelated, little is known about the interplay of psychopathic traits in this relationship. The present study aimed to determine the mediating role of psychopathy in the relationship between ACEs and self-harming behaviours in forensic patients. Methods This was a retrospective study of patients under the Ontario Review Board (ORB) between 2014 and 2015. In the analysis, we included patients with complete data on ACEs, self-harming behaviours, and a Psychopathy Checklist-Revised (PCL-R) score - a measure of psychopathic traits and their severity conducted during the reporting period. Mediation analysis was based on the Baron and Kenny approach, and sensitivity analysis was performed based on the types of ACEs. Results ​​​The sample population (n = 593) was made up of adults, with a mean age of 41.21 (± 12.35) years and were predominantly males (92.37%). While there was a partial mediating effect of psychopathy on the relationship between ACEs and incidents of self-harming behaviours in the past year, the mediation was complete in the relationship between ACEs and a lifetime history of self-harming behaviours. Following sensitivity analysis based on the types of ACE, the mediating effects were more attributed to specific ACEs, especially having experienced child abuse or having an incarcerated household member before 18 years. Conclusion Among forensic patients in Ontario, psychopathy mediates​ ​the relationship between experiencing ACEs and engaging in self-harming behaviours. Effective intervention to mitigate self-harming behaviours in this population should consider the potential role of psychopathy, especially among individuals who have experienced ACEs involving a history of child abuse and a family who was incarcerated.

Abstract Snack products are evolving as new carriers of functional ingredients with nutritional and health-promoting benefits. A blend of whole grains is increasingly being utilized to harness the functional potential of the grain mix. Amaranth, acha, and pearl millet grains flours were optimized using response surface methodology (RSM), to obtain optimum blends (90:5:5 and 47.98:26.68:25.34) with high protein content and low glycemic index. Snack bar products from the blends were labelled MBY and MBZ. A total of 40 diabetic and 10 non-diabetic subjects were recruited. Of the diabetic, about 42% were overweight while 40% were obese, the non-diabetic had normal weights. Each was allowed to consume snacks containing the equivalent of 50 g of carbohydrates. Finger prick was employed to evaluate the postprandial glucose response of snack products while venous blood was evaluated for antioxidant enzymes, carbohydrate-hydrolyzing activities, and insulin using standard methods. Consumption of the multigrain snacks elicited a stable postprandial response (133–141 mg/dL) with 16 and 24% postprandial decline. In addition, snacks had low to intermediate glycemic index (52 and 56) in diabetic and low glycemic index (43 and 45) in non-diabetics; likewise reduced α-amylase/α-glucosidase activities compared to control snacks. Similarly, glutathione level, glutathione peroxidase, superoxide dismutase, and catalase activities in serum from subjects that consumed multigrain snacks were upregulated compared to control and market sample groups. Moreso, snack products promoted a reduction in serum insulin levels in diabetic subjects (45 and 17% for MBY and MBZ respectively). Following the nutraceutical properties displayed by the formulated snack especially MBY, it can be promoted as a functional snack for the management of diabetes while solving the limited snack product choice of diabetes sufferers. Recent studies have found potentials in the utilization of biodiesel from renewable energy sources to tackle environmental pollution associated with the release of exhausts from fossil diesel fuel engines. However, it's imperative to investigate the corrosion rate and nature of the engine components fuelled with the biodiesel. This study examines the corrosion rate and nature of aluminum coupon (AC) in fossil diesel fuel (DF) and biodiesel produced from waste cooking oil via transesterification reaction (WCOB) using weight loss and electrochemical measurements. Taguchi Orthogonal Array was used to study the influence of temperature, immersion time, agitation and WCOD/DF volume. Weight loss method revealed highest corrosion rates to be 0.0052 mm y−1 and 0.0381 mm y−1 for AC in DF and WCOB respectively. Corrosion rate increased with increase in immersion time and temperature. A shift in the corrosion potential from -0.192 V to -0.574 V; and -0.178 V to -0.516 V was revealed by potentiodynamic polarization curve for AC in WCOB and DF respectively. Nyquist plots exhibited relatively perfect semicircles of different diameters and similar shape. The developed mathematical models were effective for corrosion rate prediction. Minimum corrosion rates of 1.222 × 10−3 mm y−1 and 1.822 × 10−3 mm y−1 for AC in DF and WCOB are respectively feasible at optimum point. Presence of carbon and oxygen on AC surface; pits formation; and loss of Al ion into WCOB and DF after corrosion were noticed. In conclusion, the corrosion rate of AC in WCOB was higher than the DF counterpart.

The contamination of edible agricultural goods with pesticides, including dichlorvos (DVDP), poses a substantial public health risk, promoting severe morbidity and mortality, especially in developing countries. It has been shown that hesperidin (hesperetin-7-O-rhamnoglucoside or Hes-7-RGlc) preserves cytomembrane, redox, and lipid homeostasis; unfortunately, its function on dichlorvos-incited heart damage has not been investigated. This work explored the ameliorative influence of Hes-7-RGlc on DVDP-activated cardiotoxicity. For this end, forty-two rats were randomly appropriated into seven groups (6 rats/group): Control, DVDP alone (8 mg.kg⁻¹day⁻¹), DVDP supplied with either Hes-7-RGlc (50 and 100 mg.kg⁻¹day⁻¹) or the reference medication atropine (0.2 mg.kg⁻¹day⁻¹), and Hes-7-RGlc alone (50 and 10 mg.kg⁻¹day⁻¹) were the seven groups investigated. This review focuses on the utility of non-invasive biomarkers in cancer diagnosis and treatment, their role in early detection, disease monitoring, and personalized therapeutic interventions. Through a systematic review of the literature, we identified 45 relevant studies that highlight the potential of these biomarkers across various cancer types, such as breast, prostate, lung, and colorectal cancers. The non-invasive biomarkers discussed include liquid biopsies, epigenetic markers, non-coding RNAs, exosomal cargo, and metabolites. Notably, liquid biopsies, particularly those based on circulating tumour DNA (ctDNA), have emerged as the most promising method for early, non-invasive cancer detection due to their ability to provide comprehensive genetic and epigenetic information from easily accessible blood samples. This review demonstrates how non-invasive biomarkers can facilitate early cancer detection, accurate subtyping, and tailored treatment strategies, thereby improving patient outcomes. Introduction Contemporary data on the burden of chronic respiratory diseases in sub-Saharan Africa is limited. More so, their economic burden is not well described. This study aims to establish a chronic respiratory disease observatory for Africa. Specific study aims are (1) to describe the prevalence and determinants of asthma with a target to screen up to 4000 children and adolescents across four African cities; (2) to determine the prevalence and determinants of chronic obstructive pulmonary disease (COPD) with a target to screen up to 3000 adults (≥18 years) across five African cities; (3) to describe the disease burden by assessing the frequency and severity of symptoms and exacerbations, medication use, emergency healthcare utilisation and hospitalisation; and (4) to assess the economic burden and affordability of the medicines for these diseases.Methods and analysis Surveys will be conducted in schools to identify children and adolescents with asthma using the Global Asthma Network screening questionnaire in Ghana, Nigeria, the Democratic Republic of Congo, and Uganda. Community surveys will be conducted among adults using an adapted version of the Burden of Obstructive Lung Disease Questionnaire to identify persons with COPD symptoms in Nigeria, Burkina Faso, Mozambique, Rwanda, and Sierra Leone. Fractional exhaled nitric oxide and pre-bronchodilator and post-bronchodilator spirometry will be done for children with asthma or asthma symptoms and for all adult participants. Children and adults with respiratory symptoms or diagnoses will complete the health economic questionnaires. Statistical analysis will involve descriptive and analytical statistics to determine outcomes.Ethics and dissemination Ethical approval has been obtained from participating institutions. This study’s results will inform deliberations at the United Nations General Assembly high-level meeting on non-communicable diseases in 2025. The results will be shared through academic conferences and journals and communicated to the schools and the communities.
