This work investigated the density, porosity, some mechanical properties and microstructure of PPS-AlMMC and compared the properties of the composites and those of the aluminium 6063 (Al60603) alloy. Periwinkle shells were milled to particle sizes of 75µm and 150µm and used to produce PPS-AlMMC at 1,5,10 and 15wt% filler loadings using two-step casting technique. The density, porosity, mechanical properties and microstructure of the composite materials were compared with those of the Al6063 alloy. The addition of PPS to aluminium alloy reduced the density of the composite. It was observed that the filler distributes uniformly in the matrix due to the two-step casting technique. The porosities of the composites were within acceptable level of 0-5% except for the composite with 15wt% PPS of 150µm particle size. Improved strength, ductility, hardness and modulus were obtained when the filler was used to reinforce the alloy. However, using a filler of higher particle size resulted to higher porosity, reduced tensile strength, ductility and toughness. [ABSTRACT FROM AUTHOR]

Pigs have been associated with several episodes of influenza outbreaks in the past and are considered to play a significant role in the ecology of influenza virus. The recent 2009 pandemic influenza A/ H1 N1 virus originated from swine and not only did it cause widespread infection in humans, but was also transmitted back to swine in Asia, Europe and America. What may be the prevailing situation in Africa, particularly in sub- Saharan Africa, with respect to the circulation of classical swine or pandemic influenza? The ecology of influenza viruses, as well as the epidemiology of human or animal influenza, is poorly understood in the region. In particular, little is known about swine influenza in Africa despite the relevance of this production in the continent and the widespread pig husbandry operations in urban and rural areas. In this review, the gap in the knowledge of classical and pandemic swine influenza is attributed to negligence of disease surveillance, as well as to the economic and public health impact that the disease may cause in sub- Saharan Africa. However, emerging serological and virological evidence of swine influenza virus in some countries in the region underscores the importance of integrated surveillance to better understand the circulation and epidemiology of swine influenza, a disease of global economic and public health importance. Objective: The utility of rapid and reliable sensory tests appropriate for the diagnosis of neuropathy in the diabetes clinic, rather than as prognostic tools for the prediction of foot complications, has been unclear because of limitations inherent in previous studies. Although clinical practice guidelines recommend annual screening for neuropathy, they are unable to support specific recommendations for screening maneuvers because of a lack of evidence for the validity of screening tests in the medical literature. The objective of this study was to assess the operating characteristics of four simple sensory screening maneuvers as compared with standardized electrophysiological tests in the diagnosis of distal symmetrical polyneuropathy.Research Design and Methods: We assessed four simple tests (the 10-g Semmes-Weinstein monofilament examination [SWME], superficial pain sensation, vibration testing by the on-off method, and vibration testing by the timed method) in 478 subjects with independent blinded evaluations compared against the criterion standard of nerve conduction studies. We present receiver-operating characteristic (ROC) curves, positive and negative likelihood ratios, and sensitivity and specificity values for each test.Results: The four simple screening maneuvers reveal similar operating characteristics. Cutoff points by ROC curve analyses reveal that a positive or abnormal test is represented by five incorrect responses of eight stimuli applied. A negative or normal test is represented by one or fewer incorrect responses of eight stimuli applied. By these criteria, the point estimates of the positive likelihood ratios for vibration testing by the on-off method, vibration testing by the timed method, the SWME, and superficial pain sensation test are 26.6, 18.5, 10.2, and 9.2, respectively. The point estimates of the negative likelihood ratios are 0.33, 0.51, 0.34, and 0.50, respectively The screening tests showed comparable sensitivity and specificity results. The 10-g SWME, superficial pain test, and vibration testing by the on-off method are rapid, each requiring approximately 60 s to administer. The timed vibration test takes longer, and the interpretation is more complicated. The combination of two simple tests (e.g., the 10-g SWME and vibration testing by the on-off method) does not add value to each individual screening test.Conclusions: Annual screening for diabetic neuropathy should be conducted using superficial pain sensation testing, SWME, or vibration testing by the on-off method. The reported operating characteristics for each sensory modality can be applied to positive findings on the physical examination of individual patients to predict the likelihood of neuropathy. Background: Our previous studies have indicated that the human immunodeficiency virus (HIV) and human T-lymphotropic virus (HTLV) groups of retroviruses are endemic among various populations in Nigeria. These viruses are antigenically distinct and antibodies to HIV and HTLV do not cross-react, so we studied the prevalence of HIV-1, HIV-2, HTLV-I and HTLV-II antibodies in sera of pregnant women attending an antenatal clinic in Ibadan, Nigeria.Methods: In all, 364 sera were screened using three different enzyme immunoassays, including those that distinguished HIV-1 antibodies from HIV-2, and HTLV-I antibodies from HTLV-II. All repeatedly reactive sera were confirmed by Western blots and synthetic peptide assays for the respective viruses.Results: Overall, 71 sera (19.5%) had antibodies to HIV, HTLV or both groups of retroviruses. Most (95.8%) of the reactive samples were from women 20-29 years old. Two of five sera from individuals < 20 years old reacted for HIV antibodies while one serum from a 40 year old woman was reactive for HTLV-I antibodies. Of the 71 reactive sera, 29 (8%) had antibodies to HIV (HIV-1 = 14, HIV-2 = 9, HIV-1/2 = 6) and 42 samples (11.5%) showed antibodies to HTLV (HTLV-I = 20, HTLV-II = 14, HTLV-I/II = 8). Seven of 71 seropositive samples (9.8%) reacted for both HIV and HTLV antibodies in various combinations, including one serum that showed antibody reactivities to all four retroviruses (i.e. HIV-1, HIV-2, HTLV-I and HTLV-II).Conclusions: The high prevalence of antibodies to subtypes of the two entirely distinct retrovirus groups in young women has important implications for defining epidemiological patterns of diseases associated with co-infections with two or more retroviruses. Summary We assessed the effect of advancing maternal age ( 35 years) on the outcome of pregnancy in first births among 50 elderly primigravidae compared with a control group of 50 younger primigravidae (20-29 years) at the University College Hospital Ibadan from 1 January 1988 to 31 December 1992. Most of the elderly primigravidae were professionals and the majority of them (68%) had involuntary infertility. Frequent hospital admissions, preterm delivery, low birth weights and very high caesarean section rates were the major problems highlighted in the elderly primigravidae, while anaemia was more frequent in the younger primigravidae. Unnecessary caesarean sections on the excuse of 'precious' baby in the elderly primigravidae should be discouraged bearing in mind its medico-social implications. It is concluded that although the elderly primigravidae may have higher rates of complications of pregnancy, delivery and neonatal deaths their overall risk of referral to special care baby unit is not appreciably increased. A total of 120 Nigerian women at term pregnancy with one previous caesarean delivery were studied between June 1988 and May 1993. Elective caesarean section was performed in 3 (2.5%). Vaginal delivery was achieved in 101 (86.5%) of those allowed a trial of labour. Intrapartum caesarean section was done in 16 (13.7%) cases. Rupture of the uterus occurred in 3 cases (2.6%) with perinatal loss of 2 babies. There was no maternal mortality. There was no significant correlation between vaginal delivery and birth weight, gestational age or initial indication for the primary caesarean section. It is concluded that trial of labour is safe after a previous caesarean section in selected patients. [ABSTRACT FROM AUTHOR]

BACKGROUND:ATCC HIV-1 drug resistance test kit was designed to detect HIV-1 drug resistance (HIVDR) mutations in the protease and reverse transcriptase genes for all HIV-1 group M subtypes and circulating recombinant forms. The test has been validated for both plasma and dried blood spot specimen types with viral load (VL) of ≥1000 copies/ml. We performed an in-country assessment on the kit to determine the genotyping sensitivity and its accuracy in detecting HIVDR mutations using plasma samples stored under suboptimal conditions. METHODS:Among 572 samples with VL ≥1000 copies/ml that had been genotyped by ViroSeq assay, 183 were randomly selected, including 85 successful genotyped and 98 unsuccessful genotyped samples. They were tested with ATCC kits following the manufacturer's instructions. Sequence identity and HIVDR patterns were analysed with Stanford University HIV Drug Resistance HIVdb program. RESULTS:Of the 183 samples, 127 (69.4%) were successfully genotyped by either method. While ViroSeq system genotyped 85/183 (46.5%) with median VL of 32,971 (IQR: 11,150-96,506) copies/ml, ATCC genotyped 115/183 (62.8%) samples with median VL of 23,068 (IQR: 7,397-86,086) copies/ml. Of the 98 unsuccessful genotyped samples with ViroSeq assay, 42 (42.9%) samples with lower median VL of 13,906 (IQR: 6,122-72,329) copies/ml were successfully genotyped using ATCC. Sequence identity analysis revealed that the sequences generated by both methods were >98% identical and yielded similar HIVDR profiles at individual patient level. CONCLUSION:This study confirms that ATCC kit showed greater sensitivity in genotyping plasma samples stored in suboptimal conditions experiencing frequent and prolonged power outage. Thus, it is more sensitive particularly for subtypes A and A/G HIV-1 in resource-limited settings.

Megakaryocytes (MKs) are precursors to platelets, the second most abundant cells in the peripheral circulation. However, while platelets are known to participate in immune responses and play significant functions during infections, the role of MKs within the immune system remains largely unexplored. Histological studies of sepsis patients identified increased nucleated CD61 + cells (MKs) in the lungs, and CD61 + staining (likely platelets within microthrombi) in the kidneys, which correlated with the development of organ dysfunction. Detailed imaging cytometry of peripheral blood from patients with sepsis found significantly higher MK counts, which we predict would likely be misclassified by automated hematology analyzers as leukocytes. Utilizing in vitro techniques, we show that both stem cell derived MKs (SC MKs) and cells from the human megakaryoblastic leukemia cell line, Meg-01, undergo chemotaxis, interact with bacteria, and are capable of releasing chromatin webs in response to various pathogenic stimuli. Introduction: Chronic hepatitis B (CHB) is an international public health problem. Treatment reduces its morbidity, mortality and infectivity. The aim of this study was to determine adherence among CHB infected patients on Tenofovir and the reasons for non-adherence., Methodology: It was a cross-sectional study of patients on tenofovir for at least 6 months. Information was obtained on bio- data, adherence to tenofovir, duration and reasons for nonadherence using an interviewer administered questionnaire. Non- adherence was defined as patient reporting missing medication at all. Chi square or Fisher exact test and Student's t-test were used to determine associations. P value less than 0.05 was considered significant., Results: A total of 150 participants comprising of 76 (50.7%) females and 74 (49.3%) males with mean age of 39.2 ± 11.4 years, participated in the study. Non adherence rate was 65%. There was no significant association between non-adherence and tribe (p=0.7), level of education (p=0.8), religion (p=0.2), sex (p=0.9), clinical state (p=0.8), treatment experience (p=0.8) and months on Tenofovir (0.1) while a significant association existed with age (0.01), the presence of comorbidity (p=0.02) and taking another medication apart from tenfovir (0.00). The reasons for non-adherence included out of station 22 (14.7%), financial constraint 19(12.5%), unavailability of the drug 19 (12.5%), forgetfulness 15 (10%), perceived side effects 12 (8%), undetectable serum DNA quantification 11 (7.3%), ignorance of continuous use of Tenofovir 10 (6.7%), and pregnancy 9 (6%) among others., Conclusion: Adherence to Tenofovir is poor among CHB patients attending University College Hospital, Ibadan., Competing Interests: The authors certifies that there is no conflict of interest in connection with the submitted article., (Copyright © 2021 by West African Journal of Medicine.)

Background: Treatment options are limited for TB/HIV-coinfected children who require PI-based ART. Rifabutin is the preferred rifamycin for adults on PIs, but the one study evaluating rifabutin with PIs among children was stopped early due to severe neutropenia., Methods: We evaluated rifabutin safety and plasma pharmacokinetics among coinfected children 3-15 years of age receiving rifabutin 2.5 mg/kg daily with standard doses of lopinavir/ritonavir. The AUC0-24 at 2, 4 and 8 weeks after rifabutin initiation was described using intensive sampling and non-compartmental analysis. Clinical and laboratory toxicities were intensively monitored at 12 visits throughout the study., Results: Among 15 children with median (IQR) age 13.1 (10.9-14.0) years and weight 25.5 (22.3-30.5) kg, the median (IQR) rifabutin AUC0-24 was 5.21 (4.38-6.60) μg·h/mL. Four participants had AUC0-24 below 3.8 μg·h/mL (a target for the population average exposure) at week 2 and all had AUC0-24 higher than 3.8 μg·h/mL at the 4 and 8 week visits. Of 506 laboratory evaluations during rifabutin, grade 3 and grade 4 abnormalities occurred in 16 (3%) and 2 (0.4%) instances, respectively, involving 9 (60%) children. Specifically, grade 3 (n = 4) and grade 4 (n = 1) neutropenia resolved without treatment interruption or clinical sequelae in all patients. One child died at week 4 of HIV-related complications., Conclusions: In children, rifabutin 2.5 mg/kg daily achieved AUC0-24 comparable to adults and favourable HIV and TB treatment outcomes were observed. Severe neutropenia was relatively uncommon and improved with ongoing rifabutin therapy. Manuka honey (MH) is currently used as a wound treatment and suggested to be effective in Methicillin-resistant Staphylococcus aureus (MRSA) elimination. We sought to optimize the synthesis of MH microneedles (MHMs) while maintaining the MH therapeutic effects. MHMs were synthesized using multiple methods and evaluated with in vitro assays. MHMs demonstrated excellent bactericidal activity against MRSA at concentrations ≥ 10% of honey, with vacuum-prepared honey appearing to be the most bactericidal, killing bacterial concentrations as high as 8 × 10 7  CFU/mL. The wound-healing assay demonstrated that, at concentrations of 0.1%, while the cooked honey had incomplete wound closure, the vacuum-treated honey trended towards faster wound closure. In this study, we demonstrate that the method of MHM synthesis is crucial to maintaining MH properties. We optimized the synthesis of MHMs and demonstrated their potential utility in the treatment of MRSA infections as well as in wound healing. This is the first report of using MH as a substrate for the formation of dissolvable microneedles. This data supports the need for further exploration of this new approach in a wound-healing model and opens the door for the future use of MH as a component of microneedle scaffolds.

"Graying of HIV epidemic" is observed globally, as people living with HIV (PLWH) are aging, due to effectiveness of antiretrovirals. The normal aging processes and HIV-induced immune dysfunction, are potential mechanisms, driving multimorbidity (MM) in PLWH. MM is the concurrent presence of two or more diseases in a single individual. Aging PLWH, are at increased risk of acute and chronic morbidities compared with counterpart without HIV. Despite increasing concern in Nigeria, research on correlates of MM in aging PLWH is lagging. This was a comparative study, of ≥60 years of age, age-matched (±5 years) HIV-positive and HIV-negative patients. Patients were recruited, from the Infectious Disease Institute and Geriatric clinics of the University College Hospital, Ibadan, Nigeria, between April and June 2018. MM was defined as the occurrence of more than two morbidities in an individual, and it was considered acute, when within 30 days and chronic, when above 3-months duration. Data analysis was done using SPSS 23. We studied 186 individuals (62 HIV-positive and 124 HIV-negative). The PLWH had lower mean age (63.9 vs. 68.1 years, p  = .00, t  = 5.68), more chronic MM (2.0 vs. 1.3, p  = .004, t  = 2.970), which occurred earlier (4.7 vs. 9.6 years, p  = .003, t  = 3.05), more overall MM (3.6 vs. 2.8, p  = .015, t  = 2.448), and lower quality of life (82.7 vs. 86.2, p  = .002, t  = 3.130). Risk estimates for "any" MM revealed the odds are in favor of the older PLWH [69.4% vs. 46.8%, p  = .004, odds ratio = 0.388 (95% confidence interval = 0.204-0.740)]. Logistic regression revealed, age >64 years, higher total body fat, lower nadir CD4 counts, and longer duration of HIV infection, were significantly associated with MM in aging PLWH ( p  = .019). Older individuals with HIV on antiretrovirals in Ibadan, had a significantly greater burden of MM compared with those without HIV. HIV treatment programs in Nigeria will need to adapt a comprehensive health care plan for aging PLWH.

Prevailing agro-ecological conditions and intermingling of human and animals in intensive farms in urban and peri-urban areas in Africa favour cross species transmission of pathogens at the human-animal interface. However, molecular epidemiology studies of zoonotic swine influenza viruses in this region are limited. In this study, isolates of pandemic influenza virus (H1N1pdm09) obtained from pigs in Nigeria were fully sequenced. BLAST of swine influenza virus genes from Nigeria was carried out in GenBank and gene alignment was done using MEGA version 7. Maximum likelihood method (PhyML program) was used to determine gene evolutionary relationships with other viruses and phylogenetic trees were constructed to infer genomic clusters and relationship. Swine influenza viruses isolated and sequenced in this study were monophyletic and 99% congenetic with human isolates from Nigeria, Cameroon, Ghana and USA suggesting reverse zoonotic transmission from humans to pigs in intensive husbandry. A Q240R and S31N substitution among others were detected in the haemagglutinin and matrix genes, respectively, indicating potentials for mutations during interspecies co-mingling and transmission. The A/H1N1pdm09 viruses circulating in pigs that are also exposed to avian influenza in the same epidemiological zones could engender emergence of novel viruses with zoonotic or pandemic potential requiring enhanced surveillance and monitoring., (© 2019 The Author(s). Background: Blood or plasma samples from rural areas are often transported under suboptimal conditions to central laboratories. The negative influence of different storage temperatures during transportation as well as long transportation times on the stability of unprotected HIV RNA is well known. Therefore, the correct and reliable quantification of HIV RNA might be very difficult. A stabilization solution for the storage and transportation of plasma samples was developed which stabilizes RNA for seven days up to 45°C without viral load changes., Methods: Blood samples from HIV positive individuals were collected into EDTA containing tubes. The isolated plasma samples in Germany were pipetted into pre-prepared RNA stabilization tubes and incubated for seven days at 45°C. HIV-1 RNA quantification was performed on a HIV-1 LCx m 2000 system from Abbott and a Qiagen/Artus HI Virus-1 RG RT-PCR Kit on a Rotor-Gene Q PCR machine. In addition, plasma samples were collected and tested using existing SOP for storage and transportation in Nigeria. Plasma samples were treated with and without stabilization solution and the AMPLICOR HIV-1 MONITORTM test was used to determine viral load., Results: Seventy-four stabilized plasma samples were tested in Germany and results were compared to those tested unprotected within two hours. No significant changes of viral load were detected up to seven days and 45°C in case of stabilized samples. In contrast RNA of the same unprotected samples was no longer detectable after one day at 45°C. Additionally, 22 plasma samples were investigated on day zero and under field conditions in Nigeria without changes of the viral load after seven days under given temperature conditions., Conclusions: No cooling chain is necessary for the storage and/or transportation of plasma samples treated with the new RNA stabilization solution for up to seven days. The use of this solution to preserve plasma RNA will be very helpful in countries where the environmental temperature is higher than 30°C, thus addressing the problem of unreliable viral load results due to suboptimal storage or transportation conditions. Further, the costs of storage and transportation of samples for viral load quantification could be significantly reduced.

Background: Influenza is an acute respiratory disease that continues to cause global epidemics and pandemics in human with significant mortality and morbidity., Objectives: This study was designed to identify the circulating influenza virus in Ibadan, Nigeria during the 2006/2007 season., Methods: Throat swab samples were collected from patients presenting with acute respiratory tract infection at the Out-Patient Departments of major hospitals in Ibadan over a period of seven months from November 2006 to May 2007. Isolation of influenza virus was performed using Madin-Darby Canine Kidney cell line and 10 days old chicken embryonated egg. Isolates was identified by haemagglutination and haemagglutination-inhibition assays using selected CDC Influenza reference antisera (A, B, subtype H1 and H3)., Results: Out of 128 patients tested, 21(16.4%) yielded positive for virus isolation. Identification of the isolates showed that 19(14.8%) were positive for influenza virus out of which 11(8.6%) and 8(6.2%) were influenza A and B viruses respectively. Influenza A virus 6(4.7%) were subtype H1; 4(3.1%) were co-subtype H1 and H3; and 1(0.8%) was not inhibited by subtype H1 and H3., Conclusion: The circulation of influenza virus A and B in this study is important to contributing knowledge and data to influenza epidemiology and surveillance in Nigeria.

Human respiratory syncytial virus (HRSV) causes high morbidity and mortality in infants and young children. Although a high prevalence of HRSV has been reported in Nigeria, the subtype of the virus circulating in the country is not known. This crosssectional study was therefore designed to determine the subtypes of HRSV circulating among children in Ibadan. Two hundred and thirty-one nasopharyngeal and oropharyngeal swabs were collected from children presenting with respiratory infections in Secondary Health Facility (SHF) as well as those attending immunization centers in Primary Health Centers (PHCs) in Ibadan, Nigeria. Viral RNA was extracted directly from the clinical specimen and used for HRSV detection with a pair of primers that targets the conserved region of the viral matrix gene. HRSV-positive samples were subtyped using subtype-specific primers targeting the second hypervariable region of the G gene. The prevalence of HRSV infection was 8.7% and 34.6% among children attending the PHCs and SHF respectively. Both subtypes of HRSV were detected (co-circulating) among the study population. None of children was co-infected with of HRSV A and B. Overall, HRSV-A was the predominant subtype detected among children presenting with respiratory infection at the SHF while subtype B was predominant among participants attending PHCs for routine immunization. Higher disease severity scores were associated with HRSV-A infection than infection with HRSV-B. Only HRSV subtype A was detected from those diagnosed of bronchopneumonia and bronchiolitis. In conclusion, subtypes A and B co-circulating among children in Ibadan, with HRSV-A being the predominant subtype. Additional study including samples collected from other parts of the country over a longer period that will cover both wet and dry season will be done to determine the pattern of HRSV circulation in Nigeria.

Occult hepatitis B virus infection (OBI) is manifested by presence of HBV-DNA in the absence of detectable Hepatitis B surface antigen (HBsAg) with or without anti-HBV antibodies. Hence it is a potential threat in blood transfusion medicine. This study was carried out to determine the prevalence of OBI as well as evaluate the effectiveness of using Hepatitis B surface antigen (HBsAg) marker alone in the diagnosis of HBV infection among HBsAg negative blood donors in Ilorin, Nigeria. A purposive sampling, including samples from 206 already donated and prescreened blood units from HBsAg negative from apparently healthy volunteer blood donors at the General Hospital Blood Transfusion Centre, Ilorin, Nigeria, were collected for further laboratory analysis for this study. Five millilitres of blood was collected and plasma sample tested for the presence of HBsAg using a commercially available ELISA kit. In addition, Polymerase Chain Reaction (PCR) was used for molecular detection of HBV DNA in each of the samples. Data was analyzed using descriptive statistics, Chi square at p = 0.05. Of the 206 HBsAg Micropoint® rapid kits pre-screened seronegative samples collected from the blood transfusion centre, 8 (3.9%) samples were positive for the presence of HBsAg when retested using ELISA in the laboratory. Eighteen of the 206 samples (8.7%) were HBV-DNA positive by a semi-nested PCR technique giving an OBI rate of 8.7%. Out of the 18 HBV-DNA positive samples, 17 (4.4%) were from males and only one (5.6%) was from a female donor. Analysis of the 18 HBV DNA positive samples using genotype specific primers into genotype A and Non-A showed that 15 (83.3%) were HBV genotype A, while 2 (11.1%) were genotypes other than A (Non-A), one (5.6%) sample had mixed genotypes (A & non-A). A prevalence of 8.7% OBI found in this study indicates substantial risk of post transfusion HBV infection in the study area in Nigeria. Hence, the need to include HBV DNA detection in the routine blood screening that is, using Nucleic Acid Testing (NAT) technique for transfusion safety in the country.

Historically, in HIV patients, the K65R mutation and thymidine analogue mutations (TAMs) have been reported to rarely coexist. We retrospectively reviewed genotype data from paired samples in a cohort of HIV-1-infected Nigerian patients failing first-line antiretroviral therapies containing zidovudine (AZT) or tenofovir (TDF). Samples for each patient were taken at initial confirmed virological failure ≥1000 copies/ml (S1) and then at the latest available sample with viral load ≥1000 copies/ml before switch to second line (S2). Among 103 patients failing AZT, 19 (18.4%) had TAM-1s, 29 (28.2%) TAM-2s, and 21 (20.4%) mixed TAMs by S2. In contrast, in the 87 patients failing TDF, drug resistance mutations at S2 included K65R in 56 (64.4%), TAM-1s in 1 (1.1%), and TAM-2s in 25 patients (28.7%). Interestingly, 30.4% of patients with K65R in our study developed TAMs. These were exclusively K219E ± D67N and were not predicted to confer a resistance cost to future AZT-containing regimens.
