- Abstract
Background: Surgery induces a temporal change in the immune system, which might be modified by regional anesthesia. Applying a bilateral preoperative anterior quadratus lumborum block has proven to be a safe and effective technique in pain management after abdominal and retroperitoneal surgery, but the effect on the immune response is not thoroughly investigated. Methods: This study is a substudy of a randomized, controlled, double-blinded trial of patients undergoing laparoscopic hemicolectomy due to colon cancer. Twenty-two patients were randomized to undergo either a bilateral anterior quadratus lumborum nerve block with a total of 60 mL ropivacaine 0.375% or placebo with corresponding isotonic saline injections. The main objective of this exploratory substudy was to investigate the systemic immune response in the first postoperative day by examining changes in blood transcript levels (n=750) and stimulated secretion of cytokines (n=17) on ex vivo activation with microbial ligands and anti-CD3/CD28. Results: Using unsupervised data analysis tools, we observed no effect of the bilateral anterior quadratus lumborum nerve block on gene expression in immune cells (permutational multivariate analysis of variance using distance matrices: F=0.52, p=0.96), abundances of major immune cell populations (Wilcoxon rank-sum test: p>0.05), and stimulated cytokine secretion (Wilcoxon rank-sum test: p>0.05). Conclusions: Our study provides evidence that administration of bilateral anterior quadratus lumborum nerve block as a part of a multimodal analgesic regimen in an enhanced recovery after surgery for laparoscopic hemicolectomy in this cohort does not alter the systemic immune response. - Abstract
INTRODUCTION: Inter-hospital variation in the management of small bowel obstruction (SBO) has been described in other countries, but the extent to which similar variations exist in Denmark remains unknown. This study aimed to compare the management of SBO between hospitals in Denmark and identify potential areas for improvement METHODS. This was a multicentre prospective study performed at six emergency hospitals. Patients aged ≥ 18 years with a diagnosis of SBO were eligible for inclusion. The primary study endpoints were the proportion of patients undergoing operative versus non-operative management, laparoscopic surgery versus open surgery and the success rate of non-operative management.RESULTS: A total of 316 patients were included. No differences were noted in diagnostic pathways or operative versus non-operative management. However, variations were noted in compliance with peri-operative care bundles, ranging from 63.2% to 95.8%. The surgical approach also varied, with the use of laparoscopic surgery ranging from 20.7% to 71.0% (p less-than 0.001). Variations were also noted in duration of surgery (63-124 minutes, p less-than 0.001), time to re-introduction of normal diet and length of hospital stay (3-8.5 days, p less-than 0.001). No differences were observed in 30-day or 90-day mortality rates.CONCLUSION: The management of SBO in Denmark is relatively standardised. Future efforts should focus on improving adherence to multidisciplinary peri-operative protocols, optimising patient selection for laparoscopic surgery and standardising nutritional therapy.FUNDING: None.TRIAL REGISTRATION: NCT04750811.

- Abstract
BACKGROUND: A trial of initial non-operative management is recommended in stable patients with adhesional small bowel obstruction. However, recent retrospective studies have suggested that early operative management may be of benefit in reducing subsequent recurrences. This study aimed to compare recurrence rates and survival in patients with adhesional small bowel obstruction treated operatively or non-operatively.METHODS: This was a prospective cohort study conducted at six acute hospitals in Denmark, including consecutive patients admitted with adhesional small bowel obstruction over a 4-month interval. Patients were stratified into two groups according to their treatment (operative versus non-operative) and followed up for 1 year after their index admission. Primary outcomes were recurrence of small bowel obstruction and overall survival within 1 year of index admission.RESULTS: A total of 201 patients were included, 118 (58.7 per cent) of whom were treated operatively during their index admission. Patients undergoing operative treatment had significantly better 1-year recurrence-free survival compared with patients managed non-operatively (operative 92.5 per cent versus non-operative 66.6 per cent, P <0.001). However, when the length of index admission was taken into account, patients treated non-operatively spent significantly less time admitted to hospital in the first year (median 3 days non-operative versus 6 days operative, P <0.001). On multivariable analysis, operative treatment was associated with decreased risks of recurrence (HR 0.22 (95 per cent c.i. 0.10-0.48), P <0.001) but an increased all-cause mortality rate (HR 2.48 (95 per cent c.i. 1.13-5.46), P = 0.024).CONCLUSION: Operative treatment of adhesional small bowel obstruction is associated with reduced risks of recurrence but increased risk of death in the first year after admission.REGISTRATION NUMBER: NCT04750811 (http://www.clinicaltrials.gov).prior (re

10. Hur arbetar lärare i skolan med elever som har diagnosen ADHD? : En kvalitativ intervjustudie

Syftet med denna studie är att undersöka hur verksamma lärare arbetar med elever som har ADHD, i skolmiljön. Studien bygger på intervjuer med 12 verksamma lärare från 5 grundskolor i Mellansverige. Det vi lagt fokus på är hur läraren arbetar för att underlätta skolarbete för elever med ADHD, samt arbetets betydelse i skolmiljön. Resultatet av intervjuerna visar att alla lärarna framhävde att barn/ ungdomar är olika och att man måste arbeta utifrån den enskilde individen. Vidare menar flera av lärarna att det är viktigt att skapa struktur i vardagen för elever med ADHD. Att skapa struktur menar lärarna att man till exempel kan hjälpa eleven att hitta rutin så att vardagen i skolan fungerar. Detta innebär att man till exempel kan skriva hålltider så att eleven vet vad som kommer hända under dagen. Placering och inredning i klassrummet är också en viktig aspekt för elever med ADHD problematik. Alla informanterna poängterar att det är det för mycket på väggarna eller att eleven eventuellt sitter för nära någon dörr där andra individer går förbi kan det skapa oro hos eleven vilket kan medföra koncentrationssvårigheter. Vår undersökning visar att alla lärarna säger att det är viktigt att skapa en öppen och rak kommunikation med eleven med ADHD, samt deras föräldrar. Slutsatsen av denna undersökning visar att det inte finns någon generell mall hur en elev med ADHD beter sig och det är lärarens uppdrag att bemöta eleven utifrån dennes behov och förutsättning.

11. 'Vilken snygg overall och sen tar vi dina coola vantar.' : En observationsstudie om pedagogers bemötande av barn i förskolan ur ett genusperspektiv

Syftet med studien var att observera pedagogers verbala och icke verbala bemötande av flickor och pojkar. Vi ville ta reda om bemötandet av flickor och pojkar skiljde sig åt utifrån de traditionella könsmönstren. Vi har gjort en kvalitativ studie som bygger på observationer som har gjorts på tre olika förskolor i södra Sverige. I observationerna som vi genomförde låg huvudfokus på pedagogerna. Observationerna har gjorts med videokamera och vi förde även anteckningar. Vårt resultat visar på att pedagogernas bemötande av flickor respektive pojkar inte var så utpräglat enligt de traditionella könsmönstren, dock fann vi skillnader utifrån bemötandet av pedagogerna gentemot flickor och pojkar. Flickorna får till övervägande delen klara sig själva i tamburen då det gäller av- och påklädning. Vissa pojkar fick mer närhet samtidigt som flickorna i högre grad inte fick det. När det gäller tillsägelser fanns skillnader då pojkarna fick mer direkta tillsägelser, medan flickorna fick mer resonerande tillsägelser. Det förekom även tillfällen då pedagogerna utmanade tankarna om att det inte går att resonera med pojkar, då pedagogen vid en konfliktsituation löste den genom att resonera med både flickan och pojken utan att skuldbelägga någon av dem. Ett annat exempel var då en pedagog blev tillfrågad av barnen om utomhusaktiviteter. Pedagogen gjorde då ingen skillnad på barnen utifrån kön, utan föreslog att de kunde plocka fram grävskopan och öppna "byggen". Övervägande delen av resultatet visar på att det kan ha skett en förändring av pedagogernas bemötande när det gäller de tidigare fasta könsrollerna i samhället.

12. Patienters förväntningar och upplevelser av arbetsterapi

Många patienter är osäkra på vad arbetsterapi innebär och vet därför inte vad de ska förvänta sig av behandlingen. Förväntningarna på arbetsterapin påverkar också hur själva behandlingen upplevs. Syftet med denna uppsats var att beskriva patienters förväntningar på arbetsterapi, upplevelser av den arbetsterapi de fick samt den upplevda nyttan av arbetsterapi. Metoden som valdes var en kvalitativ semistrukturerad intervjustudie eftersom den ger möjlighet att få förståelse för deltagarens upplevelser kring ett visst område. Fem intervjuer genomfördes. Deltagarna var mellan 65-86 år, båda könen var representerade och de hade någon form av stroke eller ortopedisk åkomma. Samtliga deltagare bodde ensamma i lägenhet. De arbetsterapeutiska åtgärder de deltagit i var bostadsanpassning, ADL-träning, hjälpmedelsförskrivning samt balansträning. Deltagarna hade fått arbetsterapi mellan 3-10 gånger. Intervjuerna analyserades utifrån Malteruds modifiering av Giorgis fenomenologiska analys. Analysprocessen ledde fram till olika kategorier. Under området förväntningar på arbetsterapi framkom kategorierna Osäkerhet om vad arbetsterapi kunde ge och Hopp om förbättring. Under området upplevelser av arbetsterapi framkom Delaktighet, Alienation och God relation till arbetsterapeuten. Under området upplevd nytta av arbetsterapi framkom kategorierna Självständighet, Meningslöshet och Trygghet. Slutsatser som kunde dras från resultatet var att några deltagare inte visste riktigt vad de kunde förvänta sig inför arbetsterapi medan andra väntade sig att bli mer självständiga inom ADL eller att få handarbeta. Deltagarna upplevde relationen med arbetsterapeuten var avgörande för hur de upplevde arbetsterapin. Att ha fått en förklaring på vad arbetsterapi innebär och hur den kan hjälpa deltagaren upplevdes vara viktigt för att kunna vara delaktig i arbetsterapin och se mening i behandlingen. Deltagarna tyckte att de blivit mer självständiga efter arbetsterapin och kände sig tryggare men några saknade en förklaring om vad arbetsterapi är.

Introduction: Inter-hospital variation in the management of small bowel obstruction (SBO) has been described in other countries, but the extent to which similar variations exist in Denmark remains unknown. This study aimed to compare the management of SBO between hospitals in Denmark and identify potential areas for improvement METHODS. This was a multicentre prospective study performed at six emergency hospitals. Patients aged ≥ 18 years with a diagnosis of SBO were eligible for inclusion. The primary study endpoints were the proportion of patients undergoing operative versus non-operative management, laparoscopic surgery versus open surgery and the success rate of non-operative management., Results: A total of 316 patients were included. No differences were noted in diagnostic pathways or operative versus non-operative management. However, variations were noted in compliance with peri-operative care bundles, ranging from 63.2% to 95.8%. The surgical approach also varied, with the use of laparoscopic surgery ranging from 20.7% to 71.0% (p less-than 0.001). Variations were also noted in duration of surgery (63-124 minutes, p less-than 0.001), time to re-introduction of normal diet and length of hospital stay (3-8.5 days, p less-than 0.001). No differences were observed in 30-day or 90-day mortality rates., Conclusion: The management of SBO in Denmark is relatively standardised. Future efforts should focus on improving adherence to multidisciplinary peri-operative protocols, optimising patient selection for laparoscopic surgery and standardising nutritional therapy., Funding: None., Trial Registration: NCT04750811., (Articles published in the DMJ are “open access”.
