In the light of a high rate of distant recurrence and poor compliance of adjuvant chemotherapy in high risk rectal cancer patients the total neoadjuvant treatment was logical approach to gaining acceptance. We aimed to evaluate toxicity and efficiency of this treatment in patients with rectal cancer and high risk factors for local or distant recurrence.

The majority of rectal cancers are discovered in locally advanced forms (UICC stage II, III). Treatment consists of preoperative radiochemotherapy, followed by surgery 6–8 weeks later and finally by postoperative chemotherapy. The aim of this study was to find out if tumor regression affected long-term survival in patients with localy advanced rectal cancer, treated with neoadjuvant radiochemotherapy.

Low recurrence rates and long term survival are the main therapeutic goals of rectal cancer surgery. Complete, margin- negative resection confers the greatest chance for a cure. The aim of our study was to determine whether the length of the distal resection margin was associated with local recurrence rate and long- term survival.

Background The widespread nature of coronavirus disease 2019 (COVID-19) has been unprecedented. We sought to analyze its global impact with a survey on colorectal cancer (CRC) care during the pandemic. Methods The impact of COVID-19 on preoperative assessment, elective surgery, and postoperative management of CRC patients was explored by a 35-item survey, which was distributed worldwide to members of surgical societies with an interest in CRC care. Respondents were divided into two comparator groups: 1) ‘delay’ group: CRC care affected by the pandemic; 2) ‘no delay’ group: unaltered CRC practice. Results A total of 1,051 respondents from 84 countries completed the survey. No substantial differences in demographics were found between the ‘delay’ (745, 70.9%) and ‘no delay’ (306, 29.1%) groups. Suspension of multidisciplinary team meetings, staff members quarantined or relocated to COVID-19 units, units fully dedicated to COVID-19 care, personal protective equipment not readily available were factors significantly associated to delays in endoscopy, radiology, surgery, histopathology and prolonged chemoradiation therapy-to-surgery intervals. In the ‘delay’ group, 48.9% of respondents reported a change in the initial surgical plan and 26.3% reported a shift from elective to urgent operations. Recovery of CRC care was associated with the status of the outbreak. Practicing in COVID-free units, no change in operative slots and staff members not relocated to COVID-19 units were statistically associated with unaltered CRC care in the ‘no delay’ group, while the geographical distribution was not. Conclusions Global changes in diagnostic and therapeutic CRC practices were evident. Changes were associated with differences in health-care delivery systems, hospital’s preparedness, resources availability, and local COVID-19 prevalence rather than geographical factors. Strategic planning is required to optimize CRC care., Global changes in both diagnostic and therapeutic practices in colorectal cancer care were evident in this survey conducted to analyze the impact of COVID-19 outbreak. The importance of this finding is that changes were associated with differences in health care delivery systems, hospital’s preparedness, resources availability, and local COVID-19 prevalence rather than geographical variations.

8. Giant pelvic schwannoma presenting with increased urinary frequency - Case report

Schwannoma is a relatively slow-growing and usually benign tumor that consists of myelin-producing Schwann cells. It is most often associated with 8th cranial nerve (vestibular schwanomma) and only rarely presents outside the head and neck. Extremely rarely it occurs as a retroperitoneal mass that can remain asymptomatic until it reaches a size so big that it compresses the adjacent organs and therefore requires surgical intervention.We present a case of 58 years old male with a benign pelvic schwannoma, adherent to S1-S2 intervertebral foramen, presenting with increased urinary frequency and treated with surgical en block excision.Diagnostic procedure and surgical treatment of giant pelvic schwanomma could be both challenging. Radiologic investigation is essential for preoperative planning and helpful in diagnosis but does not offer a definitive diagnosis of pelvic schwannoma. Definitive diagnosis is therefore based on histological and immunohistochemical analysis of surgically removed specimens. Possible surgical interventions include complete or partial resection, both bearing its risks and advantages in terms of postoperative morbidity.Although sometimes challenging, complete surgical resection remains the mainstay of treatment. If possible, it should be performed by en block resection since it provides the best short and long term prognosis for the patient.

Abstract Background Preoperative capecitabine-based chemoradiation is a standard treatment for locally advanced rectal cancer (LARC). Here, we explored the safety and efficacy of the addition of bevacizumab to capecitabine and concurrent radiotherapy for LARC. Methods Patients with MRI-confirmed stage II/III rectal cancer received bevacizumab 5 mg/kg i.v. 2 weeks prior to neoadjuvant chemoradiotherapy followed by bevacizumab 5 mg/kg on Days 1, 15 and 29, capecitabine 825 mg/m2 twice daily on Days 1-38, and concurrent radiotherapy 50.4 Gy (1.8 Gy/day, 5 days/week for 5 weeks + three 1.8 Gy/day), starting on Day 1. Total mesorectal excision was scheduled 6-8 weeks after completion of chemoradiotherapy. Tumour regression grades (TRG) were evaluated on surgical specimens according to Dworak. The primary endpoint was pathological complete response (pCR). Results 61 patients were enrolled (median age 60 years [range 31-80], 64% male). Twelve patients (19.7%) had T3N0 tumours, 1 patient T2N1, 19 patients (31.1%) T3N1, 2 patients (3.3%) T2N2, 22 patients (36.1%) T3N2 and 5 patients (8.2%) T4N2. Median tumour distance from the anal verge was 6 cm (range 0-11). Grade 3 adverse events included dermatitis (n = 6, 9.8%), proteinuria (n = 4, 6.5%) and leucocytopenia (n = 3, 4.9%). Radical resection was achieved in 57 patients (95%), and 42 patients (70%) underwent sphincter-preserving surgery. TRG 4 (pCR) was recorded in 8 patients (13.3%) and TRG 3 in 9 patients (15.0%). T-, N- and overall downstaging rates were 45.2%, 73.8%, and 73.8%, respectively. Conclusions This study demonstrates the feasibility of preoperative chemoradiotherapy with bevacizumab and capecitabine. The observed adverse events of neoadjuvant treatment are comparable with those previously reported, but the pCR rate was lower.

- Abstract
The optimal management of early gastric cancer should be stage-oriented, with the lymph node status as the main criterion. Nodal status can be assessed preoperatively using modern radiological imaging techniques, sentinel node biopsy, molecular or genetic markers of the tumour, and computer analysis. Theaim of this study was to evaluate the feasibility and accuracy of preoperative prediction of lymph node status based on computer analysis. We used the WinEstimate computer programme, and cons-tructed our own computer model using data of patients with gastric cancer treated in the University Medical Centre Ljubljana between 1993 and 2003. The prediction of two computermodels was then tested on 110 patients treated between 2004 and 2005 in the same manner as the patients from the database. The goal of the study was to compare the lymph node status in the control group predicted by the computer programme with the real data collected postoperatively. The accuracy rate of computerized preoperative predictions of N0/N1 status with the first computer model (WinEstimate) was 91% (sensitivity 94% and specificity 87%), and with the second computer model, 86% (sensitivity 91% and specificity 79%).Accuracy rates of preoperative predictions of nodal status are very high and, with some restrictions, can be used to select stage-appropriate therapy in patients with early gastric cancer. The applicability is improved by employing this new approach with standard diagnostic methods.

- Abstract
Strategy of surgical treatment of diverticular disease has changed over recentyears. In order to confirm the value of a more modern therapeutic approach to the disease, we rewieved the available literature and analysed 101patients operated on for complicated diverticular disease at the Departmentof Abdominal Surgery, University Medical Centre Ljubljana, from the beginning of 2006 to November 2009. There were 45 (44.6 %) emergency operations and 56 (55.4 %) elective procedures. Postoperative mortality in theemergency group was 28.9 %; none of the patients operated on electively died. High mortality rates in patients operated on on an emergency basis is attributable to comorbidities; the influence of surgical technique on the outcome is not yet clear. The optimal timing for elective surgery has not yetbeen determined, but laparoscopic-assisted resection has proved to be the most appropriate surgical technique for diverticular disease.

In Slovenia the incidence of colorectal cancer is growing rapidly. In 2000591 new cases of colon cancer and 503 new cases of rectal cancer were registered. Our study compares results of two groups of patients with rectal cancer. In the period from January 1, 1991, to December 31, 2000, 528 patients with rectal carcinoma underwent resection. We divided them in two groups, one operated in the first 5 year and the other in later 5-year period. 5-year survival was estimated by means of Kaplan-Mleier statistical analysis. Patients who died within 30 days after the operation were censored. Differences in survival curves between both groups were assessed by the log rank test. We resected 528/591 (90. 6 %) patients. RO resection was performed in 431 (81. 6 %) patients, R1 in 13 (2.5 %), and R2 in 84 (15,9%) patients. Postoperative mortality rate in resected patients was 3. 6 % (19/528), in the group with paliative operations was 12. 7 (7/55). Overall five year survival rate was 52.4 % for rectal cancer. Five year survival rate for the patients with radical resection (RO) was 59.5 %. year survival for RO-resected patientswith rectal cancer was in the period from 1996 to 2000 statistically significantly better compared with the period from 1991 to 2995 (63 vs. 55 %) (p = 0. 03627) i n stage III (p = 0. 01663).

32. Sodobno zdravljenje raka danke: The modern treatment of rectal cancer

- Abstract
Rectal cancer is a common malignant disease. Its incidence has been rapidly increasing during the last decades. With improved preoperative diagnostic procedures and histopathological evaluation, rectal cancers can be divided into several groups according to various treatment options. Well differentiated (G1/G2) small cancers without lymph node involvement (T1, NO) can be cured with local full wall excision. G3/G4 tumors require transabdominal resection. In advanced cancers (T3/T4 or N+ or M1) the patient should also receive preoperative or postoperative chemoradiotherapy. High quality resection is based on the principles of modern surgical treatment of rectal cancer, such as total mesorectal excision (TME), "en bloc" resection, radicular dissection of inferior mesenteric vessels and an adequate safety margin around the tumor. This therapeutic approach has led to a reduction in local recurrence rate and better long-term survival. Rak danke je pogosta maligna bolezen. Njegova pojavnost je v zadnjih desetletjih strmo narasla in še raste. Z natančno predoperacijsko diagnostiko in histološkim pregledom rak danke z vidika zdravljenja delimo na več skupin. Pri dobro diferenciranih (G1/G2), manjših tumorjih brez prizadetih bezgavk (T1, NO) pride kot kurativna operacija v poštev lokalna ekscizija tumorja s celo debelino stene danke. Pri slabše diferenciranih (G3/G4) karcinomih je zdravljenje izbora transabdominalna kirurška resekcija. Pri bolj razširjenih tumorjih, ki preraščajo muskularis proprijo (T3, T4) in/ali so že zasevali v regionalne bezgavke (N+) ali v oddaljene organe (M1), je poleg kirurškega zdravljenja potrebna tudi pred- ali pooperacijska radiokemoterapija. Moderna kirurška načela zdravljenja raka danke vključujejo popolno izrezanje mezorektuma (totalna ekscizija mezorektuma - TME), "en bloc" tehniko, zadostenvarnostni rob okoli tumorja in visoko ligaturo spodnjih mezenteričnih žil. Ob upoštevanju teh načel in z uporabo neoadjuvantne oziroma adjuvantne radiokemoterapije dosežemo nižjo pojavnost lokalnih ponovitev in boljše dolgoročno preživetje bolnikov.

Background The majority of rectal cancers are discovered in locally advanced forms (UICC stage II, III). Treatment consists of preoperative radiochemotherapy, followed by surgery 6–8 weeks later and finally by postoperative chemotherapy. The aim of this study was to find out if tumor regression affected long-term survival in patients with localy advanced rectal cancer, treated with neoadjuvant radiochemotherapy. Patients and methods Patients with rectal cancer stage II or III, treated between 2006 and 2010, were included in a retrospective study. Clinical and pathohistologic data were acquired from computer databases and information about survival from Cancer Registry. Survival was estimated according to Kaplan-Meier method. Significance of prognostic factors was evaluated in univariate analysis; comparison was carried out with log-rank test. The multivariate analysis was performed according to the Cox regression model; statistically significant variables from univariate analysis were included. Results Two hundred and two patients met inclusion criteria. Median follow-up was 53.2 months. Stage ypT0N0 (pathologic complete response, pCR) was observed in 14.8% of patients. Pathohistologic stage had statistically significant impact on survival (p = 0.001). 5-year survival in patients with pCR was>90%. Postoperative T and N status were also found to be statistically significant (p = 0.011 for ypT and p < 0.001 for ypN). According to multivariate analysis, tumor response to neoadjuvant therapy was the only independent prognostic factor (p = 0.003). Conclusions Pathologic response of tumor to preoperative radiochemotherapy is an important prognostic factor for prediction of long-term survival of patients with locally advanced rectal cancer. Minimalno invazivne laparoskopske tehnike vključujejo transabdominalno predperitonealno (TAPP) in ekstraperitonealno hernioplastiko (TEP). V študiji primerjamo rezultate, vključno s stopnjo ponovitve kile in kronično bolečino, po pričvrstitvi mrežice z uporabo histoakrilnega lepila v primerjavi s sponkami in brez pričvrstitve mrežice pri tehniki TAPP. Intenzivnost pooperativne bolečine je bila ocenjena z lestvico Carolinas Comfort (CCS). V študiji je bila uporabljena izključno mrežica Proceed. Bolniki in metode: Primerjali smo skupine bolnikov glede na vrsto pričvrstitve mrežice: skupina A je imela mrežico pričvrščeno z resorbirajočimi sponkami in skupina B s histoakrilnim lepilom, skupina C pa mrežice mi imela pričvrščene. V raziskavo je bilo vključenih 59 bolnikov moškega spola, starih od 26 do 76 let, pri katerih je bilo izvedenih 85 hernioplastik po tehniki TAPP. Preiskovanci so bili razdeljeni v tri skupine: v skupini B je bilo narejenih 29 hernioplastik ter v skupinah A in C po 28 hernioplastik. Udeležence v študiji smo sledili 3, 12 in 24 mesecev. Rezultati: V navedenih treh skupinah ni bilo statistično značilne razlike v ponovitvi kile. Skupna stopnja ponovitve kile je bila 3,5-odstotna. Ena ponovitev je bila zabeležena pri skupini A 3 mesece po operaciji, ena ponovitev v skupini B 15 mesecev po operaciji in prav tako ena ponovitev v skupini C 12 mesecev po operacijo. Dva bolnika v skupini A sta imela postoperativni hematom mošnje in en bolnik v skupini B je imel serom. Zdravljeni so bili konservativno. Zaključek: Študija je pokazala, da vrsta pričvrstitve mrežice ne poveča tveganja za ponovitev kile ali kronične postoperativne bolečine. Vendar pa je mesto pričvrstitve mrežice pomembno in ne bi smeli pričvrstiti mrežice v bolečinskem trikotniku in tako imenovanem »triangle of doom«. The minimally invasive laparoscopic techniques include transabdominal preperitoneal (TAPP) and total extraperitoneal hernioplasty (TEP). In our study we compare outcomes including hernia recurrences rate and chronic inguinal pain, following mesh fixation using histoacryl glue versus tacker versus no mesh fixation in TAPP. The intensity of postoperative pain was assessed with the Carolinas Comfort scale (CCS). In the study, Proceed Surgical mesh was exclusively used. Patients and methods: Three groups of patients based on the type of mesh fixation: group (A) fixed with resorbable tackers, group (B) fixed with histoacryl glue and the third (C) group without fixation were compared. The study included 59 male patients from 26 to 76 years old, who underwent 85 hernioplasty using the TAPP technique. The subjects in the study were divided in three groups: group (B) consisting of 29 hernias, group (A) and (C) - with 28 hernias each. Participants in the study were followed up at 3, 12 and 24 monts. Results : There was no statistically significant difference in the hernia recurrence between the three groups. The overall hernia recurrence rate was 3.5%. One recurrence was noted in group A at 3 months postoperatively. The second recurence occured in B group after 15 months from the index surgery and the third recurrence in C group occurred after 12 months. Two patients in group A had a postoperative haematoma of the scrotum, managed conservatively. Conclusion: Our study showed that the type of mesh fixation does not increase the risk of inguinal hernia recurrence or chronic postoperative pain. However, the site of mesh fixation is important and should not be fixed at the level of the triangles of pain and doom.
