The aim of this retrospective study is to evaluate the efficacy of a single-dose anakinra during familial Mediterranean fever (FMF) attacks and its effect on the duration, severity, and frequency of attacks. The patients with FMF who had disease episode and received a single-dose anakinra during disease episode between December 2020 and May 2022 were included. Demographic characteristics, MEFV gene variants detected, concomitant medical conditions, demographics of recent and previous episodes, laboratory findings and length of hospital stay were recorded. A retrospective analysis of medical records revealed 79 attacks from 68 patients who met inclusion criteria. The patients had a median age of 13 (2.5–25) years. All patients reported that the average duration of their previous episodes lasted longer than 24 h. When the recovery time of attacks after subcutaneous anakinra application at the disease attack was examined, it was observed that 4 attacks (5.1%) ended in 10 min; 10 attacks (12.7%) in 10–30 min; 29 attacks (36.7%) in 30–60 min; 28 attacks (35.4%) in 1–4 h; 4 attacks (5.1%) in 24 h; and 4 attacks (5.1%) ended in more than 24 h. There was no patient who did not recover from their attack after a single dose of anakinra. Although the efficacy of a single-dose anakinra administration during FMF attacks in children needs to be confirmed by prospective studies, our results suggest that use of a single-dose anakinra during FMF attacks is effective in reduction of severity and duration of disease attacks. With ageing societies and increasing number of patients, there is a growing need for quality services that help transporting non-urgent patients to hospitals. In logistics, patient transportation problems are usually modeled as a dial-a-ride problem. In a Dial-a-Ride problem, a fleet of vehicles is providing the delivery services between the loading points and the delivery destinations. The demands are known in advance. In most cases the total travel distance of the vehicles is to be minimized. In this paper, we consider a specific dial-a-ride problem, where a single vehicle is used to transport patients to the same hospital. In determining the optimal route, the multiple and different travel needs of patients, such as their maximum travel time, are also taken into consideration. We introduce 4 different mixed integer linear programming models of the routing problem. The FLAIT-train operates on regular roads and aims to provide DOOR-2-DOOR transport, addressing the issues associated with fixed stops and offering increased accessibility and convenience. In its initial phase, the FLAIT-train operates on exclusive lanes, but it is designed to integrate with other traffic eventually. The vehicle technology of FLAIT-trains closely resembles that of battery electric autonomous vehicles. To assess whether FLAIT-trains can be used as a suitable alternative to conventional public transportation systems, this paper employs traffic simulations that consider key performance indicators, including the average waiting time per passenger, maximum waiting time of a single passenger, average in-vehicle time per passenger, and average occupancy rate of the vehicles. Technology and innovation have the power to totally transform the insurance business by providing novel possibilities of insuring risks. Insurtech which is a combination of insurance and technology is expanding rapidly in the Indian insurance industry. The current study aims to identify the insurtech business models in India and examine the dimensions of these business models. The study used four dimensions 'Who-What-How-Why' of the popular model 'business model innovation' to analyse each of these business models. The study revealed that the Indian insurtech startups are basically supplementing the traditional industry models with ecommerce or digitization patterns. Any real path breaking business model innovations are scarcely observed. The study contributes to the literature by providing a comprehensive analysis of the target customers, value proposition, value chain and profit mechanism of the insurtech business models. Hydrogen-based Fuel Cells (FCs) hold significant potential as energy conversion technologies. In a previous study, we presented a pump-based hydrogen generator (PHG) that utilizes a catalytic reaction between sodium borohydride (NaBH4) powder and water. The pump circulates the water solution through the powder chamber in a closed-loop reaction. The PHG demonstrated clear advantages over alternative hydrogen sources in terms of both safety and energy density. However, as operating time increases, the solution in the closed-loop PHG becomes saturated, causing the reaction rate to decline. This limitation can be overcome in cases where an external water source is available, such as marine vehicles, drones equipped with water recovery systems from their fuel cells, or systems located near pipelines. In such scenarios, introducing freshwater feeding and product emission offers intriguing possibilities for significantly enhancing the fuel's energy density and extending its effective operation time.Our current research introduces an innovative concept: a dual-cycle generator (DCG) that effectively overcomes the issue of solution saturation over time. It achieves this by combining solution circulation with freshwater feeding and product emission. Our study employed a DCG prototype to examine various operating modes and to demonstrate the effectiveness of this approach. The DCG achieved a calculated energy density for the fuel of 3868 Wh/kg, with 93% H2 extraction yield from the powder. Our findings reveal substantial improvements in terms of extended operation duration (81%), increased hydrogen flow rate (36%), enhanced energy density (33%), and improved H2 yield extraction from the powder (39%). This methodology holds promise for mobile applications or off-grid systems situated in proximity to a water source.

Este artículo tiene como objetivo abordar la relación entre el determinismo tecnológico y las prácticas culturales al interior de los debates de los estudios de comunicación, el tránsito de la televisión como emblema del broadcasting, su declive como medio hegemónico y el surgimiento de nuevas pantallas. Para ello, se retoman y analizan prácticas asociadas a los modos de consumo de las producciones ficcionales de la televisión y plataformas on demand. Además, se presentan aquellas innovaciones de los dispositivos tecnológicos que a lo largo de la historia modificaron los usos y prácticas de los sujetos, a la vez que las nuevas destrezas y conocimientos de los usuarios que influyen en los soportes. Este trabajo concluye que existe una tensión oscilante entre medios y audiencias, y entre tecnologías y prácticas, por lo que ciertas modificaciones en los procesos de uno producen cambios en el otro y viceversa.

Background: Healthcare professionals trained in palliative care (PC) improve satisfaction and decrease healthcare overutilization for patients with serious illness and their families. A continuing education (CE) series on primary PC aligned to the National Clinical Practice Guidelines for Quality PC was developed by local, interdisciplinary experts for a target audience of rural, primary care healthcare professionals. The modules were accessed on an online learning management system platform. Objective: The study objectives were to assess differences in participants' knowledge, competence, performance, and ability to improve patient care as well as commitment to change practice after taking each of the CE modules. Design and Methods: To achieve these objectives, a descriptive design was used with a convenience sample of healthcare professionals who registered for the CE series and completed at least one module. Participants completed demographic questions and an evaluation survey after completing each module. Results: So far, 158 healthcare professionals have registered for the series with the majority being nurses and social workers. Although the professionals reported having extensive healthcare experience, they did not report having the same level of PC experience. The professionals represent nine different states. All the CE modules increased teamwork skills for most participants. The CE modules on cultural aspects and self-care had the biggest influence on participants' ability to improve patient outcomes. Cultural aspects, care of the actively dying, and advance care planning had the greatest impact on participants' knowledge, competence, and performance. Conclusion: The primary PC education series improved self-reported skills in teamwork, practice habits, and meeting goals of healthcare professionals from a variety of disciplines and settings. These enhanced primary PC skills will improve the incorporation of PC into a variety of practice settings, by multiple disciplines to enhance access to PC outside of, and potentially referrals to, specialty PC programs. An important factor that influences a health professional's successful delivery of PC may be their knowledge, experiences, and confidence. This article empirically identifies and conceptualises listener expectations of podcast hosts based on a general and open research question concerning which expectations podcast listeners have for podcast hosts. From an explorative empirical study of 20 online focus group interviews with Danish radio- and podcast listeners, the article concludes that the expectations of podcast hosts are grouped into three categories: knowledge, storytelling, and parasocial relationship. The three categories have underlying themes: some are distinct expectations for podcast hosts; others are of a more general character. Additionally, the study contributes to a better understanding of the inherent qualities of podcasts as a medium and provides useful knowledge for podcast producers about user preferences. This European observational chart review assessed the efficacy/safety of recombinant von Willebrand factor (rVWF) for on-demand treatment of spontaneous/traumatic bleeds and prevention and/or treatment of surgery-related bleeding in adults with von Willebrand disease (VWD). Patients (n = 91) were enrolled at first rVWF administration (index). Data were collected for the 12 months before index and until death, loss to follow-up, or end of study (3-12 months after index). Fifteen patients reported an rVWF-treated spontaneous/traumatic bleed at index. Bleed resolution was obtained for 14 patients (unknown status, n = 1), and investigators assessed treatment satisfaction for 13 rVWF prescriptions (2 moderate, 5 good, and 6 excellent). rVWF was used to prevent/treat surgery-related bleeds at index in 76 patients. Bleed resolution was achieved in 25/58 rVWF-treated surgeries; bleed resolution was not applicable for 33 surgeries. In both groups, there were no reports of treatment-emergent adverse events after initiating rVWF, including hypersensitivity reactions, thrombotic events, and VWF inhibitor development. rVWF was shown to be effective for the on-demand treatment of spontaneous/traumatic bleeds, and for the prevention and treatment of surgical bleeds in this real-world VWD population. Simple Summary: We investigated the prognosis of intermediate-stage hepatocellular carcinoma (HCC) patients who received lenvatinib (LEN) until unacceptable adverse events or progressive disease, followed by transcatheter arterial chemoembolization (TACE) on demand. The overall survival (OS) in patients for whom LEN was re-administered after TACE (TACE-LEN) was significantly longer in comparison to patients who received other therapies, such as only TACE, other drugs after TACE, or other drugs without TACE. TACE-LEN was the most associated with OS in the Cox proportional hazard analysis. In intermediate-stage HCC patients who can tolerate LEN without discontinuation due to AEs, TACE-LEN may prolong the prognosis. The present study clarified the prognosis of intermediate-stage hepatocellular carcinoma (HCC) patients who received lenvatinib (LEN) followed by transcatheter arterial chemoembolization (TACE) on demand. We retrospectively evaluated 88 intermediate-stage HCC patients who received LEN. The median age was 74 (range: 47–92) years old, 67 patients were male, and 82 were classified as Child-Pugh A. LEN was administered until disease progression or discontinuation due to adverse events (AEs). The mean duration of LEN treatment was 7.0 months. The response and disease control rates were 51.1% and 89.8%, respectively. The median progression-free survival and overall survival (OS) after the initiation of LEN were 6.8 months and 29.9 months, respectively. The OS in patients for whom LEN was re-administered after TACE (TACE-LEN) was better than that in patients who received other therapies (e.g., only TACE, TACE-other therapy, or only other therapy) even with propensity score matching (p = 0.008). A Cox proportional hazard analysis showed that TACE-LEN was most strongly associated with the OS (hazard ratio: 0.083, 95% confidence interval: 0.019–0.362, p = 0.001). LEN was administered for approximately 11.1 months after TACE. In intermediate-stage HCC patients who can tolerate LEN without discontinuation due to AEs, TACE-LEN may prolong the prognosis. Over the past 2 years, telemedicine has skyrocketed as COVID-19 propelled innovation and implementation at unparalleled rates. Within the UC Davis academic health system, a new paradigm for telemedicine emerged: direct-to-consumer telemedicine. The video-based telemedicine program has become the largest of its kind in California and is staffed by 80 providers (MDs, APPs) across five clinical departments/groups (primary care practice group, family and community medicine department, emergency medicine department, the nursing department, and the physical medicine and rehabilitation department). September 2021 marked the 1-year anniversary of a journey that has opened access, improved coordination, and become a workforce engine for our evolving virtual health infrastructure.

Abstract Background Chest radiograph (CXR) prescribing pattern and practice vary widely among pediatric intensive care units (PICU). ‘On demand’ approach is increasingly recommended as against daily ‘routine’ CXRs; however, the real-world practice is largely unknown. Methods This was a prospective observational study performed in children younger than 12 years admitted to PICU of a tertiary care teaching hospital in India. Data were collected on all consecutive CXRs performed between December 2016 and April 2017. The primary outcome was to assess the factors that were associated with higher chest radiograph prescriptions in PICU. Secondary outcomes were to study the indications, association with mechanical ventilation, image quality and avoidable radiation exposure. Results Of 303 children admitted during the study period, 159 underwent a total of 524 CXRs in PICU. Median (IQR) age of the study cohort was 2 (0.6–5) years. More than two thirds [n = 115, 72.3%] were mechanically ventilated. Most CXRs (n = 449, 85.7%) were performed on mechanically ventilated patients, amounting to a median (IQR) of 3 (2–5) radiographs per ventilated patient. With increasing duration of ventilation, the number of CXRs proportionately increased in the first two weeks of mechanical ventilation. In non-ventilated children, about two thirds (68%) underwent only one CXR. Majority of the prescriptions were on demand (n = 461, 88%). Most common indications were peri-procedure prescriptions (37%) followed by evaluation for respiratory disease status (24%). About 40% CXRs resulted in interventions; adjustment in ventilator settings (13.5%) was the most frequent intervention. In 26% (n = 138) of radiographs, image quality required improvement. One or more additional body part exposure other than chest and upper abdomen were noted 336 (64%) images. Children with > 3 CXR had higher PRISM III score, more often mechanically ventilated, had higher number of indwelling devices [mean (SD) 2.6 (1.2) vs. 1.7 (1.0)] and stayed longer in PICU [median (IQR) 11(7.5–18.5) vs. 6 (3–9)]. Conclusion On demand prescription was the prevalent practice in our PICU. Most non-ventilated children underwent only one CXR while duration of PICU stay and the number of devices determined the number of CXRs in mechanically ventilated children. Quality improvement strategies should concentrate on the process of acquisition of images and limiting the radiation exposure to unwanted body parts.

Biopharmaceutical products are of great importance in the treatment or prevention of many diseases and represent a growing share of the global pharmaceutical market. The usual technology for protein synthesis (cell-based expression) faces certain obstacles, especially with ‘difficult-to-express’ proteins. Cell-free protein synthesis (CFPS) can overcome the main bottlenecks of cell-based expression. This review aims to present recent advances in the production process of biologic products by CFPS. First, key aspects of CFPS systems are summarized. A description of several biologic products that have been successfully produced using the CFPS system is provided. Finally, the CFPS system's ability to scale up and scale down, its main limitations and its application for biologics production are discussed.

Background: Colonoscopy has been routinely performed with sedation. Previous studies showed that on-demand sedation colonoscopy is acceptable in patients. We aim to compare the clinical outcomes of patients undergoing colonoscopy with and without sedation. Methods: A prospectively collected retrospective matched cohort study was performed. 290 consecutive patients who underwent colonoscopy without sedation were included into the study. Another 290 patients who underwent colonoscopy with sedation were then selected based on matched sex and age group distribution. Data were collected in a prospective database. Age, sex, race, and procedural variables were collected. Outcomes analyzed included adenoma detection rate, Boston bowel preparation scores, time spent in the recovery room, cecal intubation time, and presence of pain during colonoscopy. Results: A total of 580 patients were included in the study. Of the 290 patients who underwent colonoscopy without sedation, 10 patients (3.45%) required sedation to be administered to complete the scope. Intention-to-treat analysis was performed. Multivariate analysis demonstrated that the administration of sedation was a significant predictor of prolonged recovery time (increase in 34.76 min, 95% CI 29.56–39.55, p < 0.001). Comparing the group who underwent colonoscopy without sedation to the group who underwent colonoscopy with sedation, there was no statistically significant difference in mean cecal intubation time (11.28 vs. 10.38 min, p = 0.129), adenoma detection rates (25.1 vs. 35.8%, p = 0.060), percentage of patients who experienced no pain at all during the procedure (93.5 vs. 93.5%, p = 1.000), and Boston bowel preparation scores (2.23 vs. 2.34, p = 0.370). Conclusions: Our data suggest that on-demand sedation colonoscopy can be performed in patients, with a significant decrease in time spent in the recovery room prior to discharge. Cecal intubation time, adenoma detection rates, and percentage of patients experiencing no pain at all during the procedure were similar in both groups of patients. [ABSTRACT FROM AUTHOR]

Female sterilization via fallopian tube ligation is a common procedure; However, after the operation, over 10% of women seek re-fertilization, which is frequently unsuccessful. In addition, there is evidence that fallopian tubes contribute to the spread of endometriotic tissue as they serve as channels for proinflammatory media entering the abdominal cavity via retrograde menstruation. Here, stimuli-degradable hydrogel implants are presented for the functional, biocompatible, and reversible occlusion of fallopian tubes. The hydrogel implants, designed with customized swelling properties, mechanically occlude fallopian tubes in a high-performance manner with burst pressures reaching 255-558 mmHg, exceeding normal abdominal pressures (95 mmHg). Their damage-free removal can be achieved within 30 min using near-visible UV light or a glutathione solution, employing a method akin to standard fallopian tube perfusion diagnostics. Ultrasound-guided implant placement is demonstrated using a clinical hysteroscope in a human-scale uterus model and biocompatibility in a porcine in vivo model. Importantly, the prevention of live sperm as well as endometrial cell passage through blocked fallopian tubes is demonstrated. Overall, a multifunctional system is presented that constitutes a possible means of on-demand, reversible contraception along with the first-ever mechanical approach to abdominal endometriosis prevention and treatment., (© 2024 The Authors. Patients with erectile dysfunction require special approach. Any therapy should be prescribed with patient’s individual features, comorbidities and concominant medication in mind. Considering good efficacy and safety profile of tadalafil, together with appearance of its generic forms on the market, making such treatment more accessible for our patients, soon it may become the most commonly prescribed drug from phosphodiesterase-5 inhibitors class. Therefore, evidence on its use in special groups of men with specific anamnestic features should be systematized. In this article we discuss the use of 20 mg tadalafil on demand in specific categories of patients suffering from erectile dysfunction: in men with metabolic syndrome, benign prostate diseases, neurological conditions and, finally, in men who express wish to become fathers.

The growth in popularity of on-demand content consumption, boosted by large global agents such as Netflix, Amazon and HBO, has brought audience fragmentation even further. Exponential growth in the content available to users (which reduces viewer concentration based on a limited selection), its commercialisation through a subscription-based business model (removing advertising from content) and the boom in consumption on different receivers, many of them mobile or outside the home (thus complicating people meter monitoring), has generated a new ecosystem where success can no longer be assessed using traditional audience measurement systems. This article discusses audience behaviour in streaming platforms and the new dimensions used to measure the success of a television series, above and beyond data provided by television audience measurement (TAM) techniques. From this analysis, the article reviews the transformation in the concept of popularity and how new audience indicators affect the structure of the content distribution medium, which adds further dimensions (engagement, customer retention, talent acquisition, new subscriptions and branding, among others) to more traditional elements (advertisers and international sales). Finally, we examine whether a single concept of audience, valid for all consumption models and audiovisual operations, can be established. Money heist is used as a case study, as it provides a good example of two ways of understanding audience: one linked to its commercial success in the Antena 3 Televisión channel's scheduled programming and the other arising from its inclusion on Netflix, the platform that gave it worldwide popularity. Analysis of survey results from U.S. residents (N = 288) watching foreign content on Netflix found that respondents watch foreign content more than they did before starting to access it through the platform and hold favorable attitudes toward foreign countries and subtitles. The data also support a theorized synergistic relationship between viewing frequency and use of the recommendation system for foreign content discovery. Results are discussed in terms of accessibility and cultural affinity as video on demand (VOD) platform affordances that may affect global media flows from the reception side. Background: Most countries witnessed a dramatic rise in Cesarean section rates during the last decades, particularly the developed ones. Mother's demand is one of the conducive factors as they try to overcome the suffering which they fear during labour and vaginal delivery. Having children and bringing them up is a blessing given only to women, so the healthcare provider should understand their perspective and the causes for this demand. To do or not to do that is the question. Performing C-section at the mother's request without any medical indication creates a debate around the safety of the mother and her baby; maternal and professional satisfaction; and ethical issues. The case should be individualized considering general evidence-based guidelines in this context. Evidence to guide medical practice in this regard is clearly needed. Thus, detailed studies in the context of evidence-based medicine are essential. Objective: To evaluate the risks and benefits involved in the practice of caesarian delivery on maternal request without medical indication compared to the planned vaginal delivery. Material and methods: PubMed database was searched and filtered by the last 5 years. With the revision of ACOG, NICE, FIGO, RANZCOG, and SOGC guidelines. Results : 53 results were chosen from a total of 267 results. The deeper inspection of the full text revealed that only 11 quantitative articles and 23 qualitative articles were relevant from the 53-total inspected. Hydrogen-based Fuel Cells (FCs) hold significant potential as energy conversion technologies. In a previous study, we presented a pump-based hydrogen generator (PHG) that utilizes a catalytic reaction between sodium borohydride (NaBH 4 ) powder and water. The pump circulates the water solution through the powder chamber in a closed-loop reaction. The PHG demonstrated clear advantages over alternative hydrogen sources in terms of both safety and energy density. However, as operating time increases, the solution in the closed-loop PHG becomes saturated, causing the reaction rate to decline. This limitation can be overcome in cases where an external water source is available, such as marine vehicles, drones equipped with water recovery systems from their fuel cells, or systems located near pipelines. In such scenarios, introducing freshwater feeding and product emission offers intriguing possibilities for significantly enhancing the fuel's energy density and extending its effective operation time. Our current research introduces an innovative concept: a dual-cycle generator (DCG) that effectively overcomes the issue of solution saturation over time. It achieves this by combining solution circulation with freshwater feeding and product emission. Our study employed a DCG prototype to examine various operating modes and to demonstrate the effectiveness of this approach. The DCG achieved a calculated energy density for the fuel of 3868 Wh/kg, with 93% H 2 extraction yield from the powder. Our findings reveal substantial improvements in terms of extended operation duration (81%), increased hydrogen flow rate (36%), enhanced energy density (33%), and improved H 2 yield extraction from the powder (39%). The French national mapping agency, Institut national de l’information géographique et forestière (IGN France), launched a new web service in June 2016, called Minecraft® on Demand specifically for young people. This free web service is designed to provide Minecraft® maps with the raster and vector geographic databases that IGN produce on the French territories. This new service was recognized by different community: scientific, young people, French ministry. Different statistics allow analyzing the success obtained since 3 years. Introduction: This study presents a novel, student‐centred, on‐demand video learning modality that supports students as they transition from theory to clinical practice. Authentic, one‐to‐one teacher and student consultations have been video recorded and uploaded online for the peers and subsequent years of students to consume and learn from. This study describes the development and use of these communal consultation videos (CCVs), examines how students perceive this novel intervention and proposes supporting educational theory. Methods: A library of 28 CCVs has been archived online, to enable learning for students not present at the original staff consultation. A questionnaire was designed and administered to evaluate student perceptions of this novel learning intervention and administered to see how students used and perceived its benefit. Results: The CCV has proven to be popular, well received and valued by students. Students reported the videos were interactive, engaged them cognitively and supported them for assessments and in preparation for the clinical care of their own patients. Students also reported that watching the videos broadened their "clinical" experience of others cases that were different to their own. Conclusion: This novel learning modality demonstrates new engaging learning opportunities for students and teachers that support students for assessments and clinical skill practice and afford observational clinical experience. To evaluate the efficacy of on-demand use of anakinra in patients with crFMF. The Gazi FMF cohort was established in the year 2010, and from that date, 689 patients with FMF diagnosed according to the Tel Hashomer criteria were registered. Attack type, duration, severity, and their impact on life were collected either by disease diaries or a mobile phone application (FMF AIDD, AppStore, and Playstore). A retrospective cohort analysis was made from records of patients who have ever been treated with IL-1 inhibitors. A total of 78 patients were treated with IL-1 inhibitors in our cohort. Among these, 15 patients were identified who received on-demand anakinra. Rationale for on-demand use was prominent prodrome or trigger for attacks and patient's personal claim. Six patients were switched from regular use and nine were directly started as on-demand use. All were using background colchicine in maximum tolerated doses. None of the patients had evidence of persistently elevated acute phase reactants or proteinuria. The median duration of on-demand anakinra use was 6 (3-36) months. Pre- and post- on-demand anakinra periods were compared. Patient reported attack severity (p = 0.002), duration (p = 0.001), frequency (p = 0.001), absenteeism (p = 0.002), and presenteeism (p = 0.002) were significantly improved. On-demand anakinra prevented progression of prodromes to full-blown attacks which was demonstrated by decrease in the rate of attack/prodrome ratio (p = 0.02). On-demand anakinra can be continued in ten subjects on long-term. On-demand anakinra significantly improved FMF attacks in certain patients which suggest this approach would be of benefit in daily practice in selected patients. [ABSTRACT FROM AUTHOR]

In recent years, it has been witnessed a boom in the development of mobile networks and a great increase in the computing ability of mobile devices. The rapid booming in client requests lead to some new challenges for real-time on-demand data broadcasting: (1) the dynamic diversity of the data characteristics; (2) the dynamic diversity of real-time clients' demand greatly increase the volume of hot-spot data (the most access data); and (3) the clients' demands for high service quality. To date, the current research has focused on the fixed-channel models (i.e. the bandwidth and number of channels are unchangeable) and algorithms. To adapt to the characteristics of the real-time requests, an optimized channel split method (OCSM) is proposed for automatic channel split and data allocation in this paper. The experiments undertaken in this study included two aspects: (1) determining the different strategies under different data sizes and deadlines; and (2) verifying the validity of the automatic channel split and data allocation through a series of experiments with the general performance matrics. Background: Although cesarean delivery on maternal request (CDMR) has been associated with an increase in the frequency of cesarean sections (CSs), there is a lack of studies reporting the frequency of CDMR in Saudi Arabia. This study was conducted to estimate the prevalence of and the motives for CDMR and identify its associated factors. Methods: This cross-sectional study was conducted between March and June 2017 on 364 pregnant women who planned a CS at King Abdulaziz Medical City in Riyadh. The characteristics of the women and their motives for undergoing a CS for the current pregnancy term were collected. Results: The prevalence of CDMR was found to be 13.7% (50/364) [95% confidence interval (CI): 10.370%-17.706%]. Older maternal age ( ≥ 40 years) [adjusted odds ratio (aOR) = 3.9; p = 0.019], family history of CS (aOR = 2.9; p = 0.038), non-Saudi nationality (aOR = 5.0; p = 0.050), and receiving education or medical information about the possibility of delivering by CS (aOR = 13.7; p = 0.030) were significantly associated with a higher prevalence of CDMR. As the number of previous CSs increased by one (aOR = 0.6; p = 0.011), the odds of CDMR decreased by 40%. The most common motives for demanding a CS in the absence of medical indications were avoiding labor or possible complications from vaginal birth (60%) and fear of pain on vaginal delivery (46%). Conclusions: A high prevalence of CDMR was documented at King Abdulaziz Medical City, especially among women of older maternal age, having a family history of CS, of non-Saudi nationality, and who received education or medical information about the possibility of delivering by CS. Counseling programs might be helpful for pregnant women who fear pain in vaginal delivery or have had a previous traumatic birth experience. [ABSTRACT FROM AUTHOR]
