Trichomoniasis is the most prevalent non-viral sexually transmitted infection worldwide. Apart from being a key biologic indicator of sexual activity, it’s been associated with the transmission of Human Immunodeficiency virus. The use of vaginal speculum is traditionally required for specimen collection. The limited application of this step in various primary health care facilities in resource-poor settings due to difficulties with sterilization of speculae among others, has grossly reduced the offer of testing for Trichomoniasis. Self-collected vaginal swab could be an acceptable alternative specimen for diagnosis of trichomoniasis in these settings. We compared the yield of Trichomonas vaginalis from speculum and non-speculum based specimens. Five hundred women between 18 – 45 years presenting with abnormal vaginal discharge in two health care facilities in Rivers State, Nigeria, were enrolled. Three specimens were collected from each woman; two non-speculum based /non-invasive and the third, speculum based. Microscopists were trained and the procedure standardized. Wet mount microscopy was performed on all specimens within thirty minutes of collection. Three Microscopists examined each specimen independently. All infected patients were managed according to existing guidelines. Ethical approval was obtained and data analyzed using SPSS version 21. Median age of participants was 31 years. Of the 500 women, 53 were infected giving an overall prevalence of 10.6%. Using the speculum-based specimens, 90.5% (48) of infected women were detected while 83.0% (44) and 30.1% (16) were detected using the self-collected swab and first void urine respectively. Interestingly, first void urine was able to detect five (5) more cases that were not detected by the speculum-based method. In low income settings, use of self-collected swabs in combination with first void urine is likely to increase detection rate of Trichomonas vaginalis when compared with the use of speculum-based specimen only.

Diagnosis of tuberculosis (TB) amongst HIV patients is a great challenge due to the low density of Acid Fast bacilli (AFB) in their sputum. The study was conducted to determine the sensitivity of direct smear microscopy (DSM) for TB diagnosis in HIV endemic setting using culture as a gold standard. During the period of study, 549 TB suspects were screened, and they comprised of 396(72%) HIV-positive and 153(28%) HIV-negative patients. Overall, TB positive rate was 221(40.3%), the rate by culture and DSM were 155(28.2%) and 12(2.2%) respectively. Among HIV-positive patients, the overall TB positive rate was 151(38.1%), while the rate by culture and DSM were 120(30.3%) and 7(1.7%) respectively (p = 0.0001). The overall positive rate for HIV-negative was 70(45.8%) and the rate by culture was 35(22.9%) and DSM was 5(3.3%) with p = 0.0004. The sensitivity of DSM in HIV- positive and negative population was 21.5 and 53.8% respectively. Findings from this study showed the low case detection rate of DSM as compared to culture, more especially in HIV positive persons. To improve case detection, we recommend the use of culture as back up to enhance the performance of DSM especially in HIV positive persons. Nanosilver was synthesised by bioreduction using aqueous extracts of Moringa oleifera(MO-AgNPs) leaf and Allium cepabulb (AC-AgNPs) as reducing and capping agents. The nanosilver was characterised with UV/vis, FT-IR, and SEM-EDX. Microbiological activity on five Mycobacteriastrains: ATCC 35882, ATCC 25177, Mycobacteriumother than tuberculosis (MOTT), Mycobacterium fortuitumand multi drug resistance M. tuberculosis(MDR-MTB) was investigated using nitrate reductase assay (NRA). Results obtained for UV/vis gave absorption maxima at 430 nm for MO-AgNPs and 440 nm for AC-AgNPs. FT-IR showed functional groups associated with the biomolecular capping agents. SEM showed the nanosilver formed aggregates spherical and/or in oval shape with smooth surface. EDX spectra exhibited the presence of elemental silver. Minimum inhibitory concentration (MIC) study for both nanosilver against Mycobacteriaindicated a higher inhibitory activity for MO-AgNPs over the AC-AgNPs. Herein, we have reported the first biosynthesised nanosilver activity against Mycobacteriausing NRA. Nanosilver may therefore be developed as nanoaerosol for targeted and effective treatment of Mycobacteriainfections.
