Objective: This study aimed to explore older patients' oral health status, their opinions about oral health care and their experiences with oral health care while in hospital. Background: Improving older adults' oral health is considered an urgent priority at both the national and international levels, especially for hospitalised older patients who have been found to have poor oral health. However, a one‐size‐fits‐all standardised approach to oral care delivery may not be the answer. Materials and Methods: This study was an embedded, multiple‐case study, integrating qualitative and quantitative data. Seven patients were recruited from a geriatric ward of an acute hospital in Australia and participated in semi‐structured interviews and oral health assessments using the Oral Health Assessment Tool (OHAT) in June–July 2022. Data were analysed descriptively and presented in case summaries. Results: OHAT assessments identified oral health problems in all patients, but not all patients self‐reported problems with their mouths. Each patient valued oral health care, but the reasons given varied. Patients had established, individualised oral health care routines, which they brought to hospital. Conclusion: Clinical observations of patient's oral health status might not match the patient's own judgement of their oral health and, if not explicitly addressed, may mask the need for oral health and hygiene intervention. Assessment and care planning needs to incorporate patients' own perceptions of their oral health and existing oral health care routines. Abstract Background Children with disabilities experience poorer oral health and frequently have complex needs. The accessibility of oral health care services for children with disabilities is crucial for promoting oral health and overall well-being. This study aimed to systematically review the literature to identify the barriers and facilitators to oral health care services for children with disabilities, and to propose priority research areas for the planning and provision of dental services to meet their needs. Methods This was a mixed methods systematic review. Multiple databases searched included MEDLINE, Scopus, PsycINFO, EMBASE, and CINAHL. The search strategy included Medical Subject Heading (MeSH) terms related to children, disabilities, and access to oral health. Eligibility criteria focused on studies about children with disabilities, discussing the accessibility of oral health care. Results Using Levesque’s framework for access identified barriers such as professional unwillingness, fear of the dentist, cost of treatment, and inadequate dental facilities. Facilitators of access offered insight into strategies for improving access to oral health care for children with disabilities. Conclusion There is a positive benefit to using Levesque’s framework of access or other established frameworks to carry out research on oral healthcare access, or implementations of dental public health interventions in order to identify gaps, enhance awareness and promote better oral health practices. The evidence suggests that including people with disabilities in co-developing service provision improves accessibility, alongside using tailored approaches and interventions which promote understanding of the importance of dental care and increases awareness for professionals, caregivers and children with disabilities. Abstract Background The relationship between the microbiome and oral health is intricate, yet there is a lack of comprehensive knowledge regarding the microbiome’s impact on oral health. Integrating knowledge regarding the oral microbiome and its significance in oral and systemic diseases holds profound implications for dental professionals in patient care and professional development. This study assessed dental professionals’ oral microbiome comprehension and knowledge levels in Saudi Arabia and its implications for oral healthcare. Methods Data were gathered using a cross-sectional design by administering a comprehensive online questionnaire to 253 dental professionals from diverse demographic backgrounds. The questionnaire, administered in English, was divided into four sections: (1) Microbiome awareness and understanding, (2) Diet, nutrition, and microbiome relationship, (3) Microbiome and oral and systemic diseases, and (4) Counselling, education, and implications. Statistical analyses were used to identify and understand underlying patterns, including descriptive statistics, chi-squared tests, ANOVA, and post hoc tests. The Spearman rank correlation coefficient was applied to assess self-rated knowledge. Results Of the 253 participants, 94.6% were familiar with the term “microbiome.” Merely 13% of participants considered the oral microbiome to be the second most diverse, following the gut microbiome. About 39.9% of participants knew the connection between oral mucosal diseases and the oral microbiome. Furthermore, only 6.7% thought there was a connection between systemic diseases and the oral microbiome. Participant comprehension of oral microbiome questions averaged 9.19 out of 13, with 83.7% scoring “good”. Objectives: The present cross-sectional study aimed to evaluate if social, racial, and gender inequalities disproportionally affect the use of dental services by people with and without disabilities in Brazil in the year 2013. Materials and methods: The study used data from the 2013 National Health Survey and the dependent variable was the use of dental services. The outcome was stratified by gender, race, and social variables. Descriptive analysis was represented by absolute and relative frequencies. Two inequality measures were used to investigate the inequalities: the Slope Index of Inequalities (SII) and the Concentration Index of Inequalities (CIX). The SII expresses the absolute difference and the CIX identifies the relative inequality. Results: Of 145,580 adults evaluated, 7.7% reported disability. The proportion of adults without disabilities who used dental services in the last 12 months was higher (45.2%; 95%CI 44.2–46.1) than adults with disabilities (33.9%; 95%CI 32.0-35.9). There was an absolute difference of 40% points (SII 0.40; CI95% (0.36–0.45) in the use of dental services between poorer and richer people with disabilities. Absolute and relative inequalities were identified in the use of dental services, considering the education of the head of the family and family income, with similar results for people with and without disabilities. High inequality is observed concerning race. Racial minorities (Black, Brown, Yellow, and Indigenous) without disabilities presented a higher use of dental services in the last 12 months than racial minorities with disabilities. Conclusions: Our findings demonstrate that social and racial inequities negatively affect individuals with and without disabilities. Despite widespread awareness, understanding its diversity and implications for oral and systemic health remains limited. It is essential to address these gaps in knowledge through future research and educational interventions, considering the vital part that dental professionals play in promoting oral health through personalised dietary recommendations, lifestyle changes, and hygiene practices. Abstract Background Older people receiving home-based care (HBC) often face barriers to access preventive oral health care (OHC) and dental treatments. Leading to deterioration of their oral healthcare. It is further deteriorated by factors such as increasing burden of systemic diseases, medicinal side effects, limited mobility, financial constraints and lack of professional OHC at home. Older people also struggle to maintain necessary daily oral hygiene, leading to malnutrition, weight loss, and a risk of a further health degradation. This cross-sectional survey aimed to investigate the oral health-related quality of life (OHRQoL) and their associated factors in HBC recipients. Methods 5,280 older people (≥ 60 years) living in Hamburg, who were in need of care and insured with statutory health insurance DAK-Gesundheit received the questionnaire, which included the German version of the Oral Health Impact Profile (OHIP G-14) and, the EQ-5D health-related quality of life (HRQoL) measure as well as further questions regarding the extent of informal social support, subjective oral health status, oral health behaviour, subjective cognitive status, and socio-demographic variables. Results The participants (n = 1,622) had a median age of 83.2 years, with 72.0% of the sample being female. Nearly two thirds of the sample reported that their independence or abilities were significantly impaired (care level 2). Regarding oral health impacts, 40.0% of the participants reported experiencing at least one of the fourteen possible prevalent impacts of the OHIP-G14 fairly often or very often. A multivariate regression model on the severity of oral health impacts revealed, that a better HRQoL, a positive perception of one’s own dental status, fewer visits to dental practices, and no need for support in OHC were associated with better OHRQoL. Conversely, respondents with a negative perception of their oral health status, more frequent visits to a dental practice, a need for support in OHC, and subjective memory impairment showed poorer OHRQoL. Conclusions The results highlight the risk for poor oral health among older people in HBC. We conclude that there is an urgent need to prioritise oral health, especially as poor oral health can further compromise the systemic wellbeing of these already care dependent population.

Pediatric sleep-disordered breathing is associated with multiple health problems. Polysomnography is the reference standard for identifying this disorder, but availability is limited. Therefore, an alternative screening tool is needed. Globally, the Sleep-Related Breathing Disorder scale of the Pediatric Sleep Questionnaire (PSQ) has proven to be a feasible tool. Consequently, this study aimed to translate and culturally adapt the PSQ into Dutch and then to examine the cultural validity, internal consistency, and test-retest reliability of the Dutch version among a general population visiting oral healthcare centers. The translation, review, adaptation, pretest, and documentation approach was used to ensure cross-cultural adaptation of the PSQ. Then, 220 children (2.4–18 years) were sampled for clinimetric evaluation. We estimated the cross-cultural validity by comparing the factor analyses of the original PSQ and the Dutch version. Reliability was assessed using Cronbach's alpha, Spearman's correlation, the intraclass correlation coefficient, the standard error of measurement, and a Bland-Altman plot. The factor loading patterns of the Dutch version matched with the original study around the four predetermined factors: breathing, sleepiness, behavior, and other. The internal consistency, with a Cronbach's α of 0.77, was acceptable. The test-retest reliability with an intraclass correlation coefficient and Spearman's correlation of 0.89 and 0.93, respectively, was good to excellent. Cultural adaptation was ensured and the results support cross-cultural validity, internal consistency, and test-retest reliability of the Dutch Sleep-Related Breathing Disorder scale of the PSQ. This questionnaire could therefore be a valuable tool for screening disordered breathing in Dutch children. • TRAPD approach ensured cross-cultural adaptation of the Dutch-SRBD from the original PSQ (Chervin et al). • The Dutch-SRBD exhibited good psychometric properties, demonstrating cross-cultural validity and reliability. • The Dutch-SRBD supports screening in general populations, aiding referral decisions for sleep-disordered breathing in children. CareQuest Institute for Oral Health's mission is to improve the oral health of all. One way to achieve this is through programmatic initiatives, which train dental clinics to provide equitable, integrated and accessible care for their communities. The Community Oral Health Transformation (COrHT) Initiative, allowed CareQuest Institute to collaborate with the North Carolina Oral Health Collaboration (NCOHC) and Blue Cross Blue Shield (BCBS) of North Carolina Foundation to implement and support the initiative in North Carolina. This mixed methods study was designed to collect quantitative and qualitative data while 11 dental clinics and a control clinic participated in the program through the end of the program. Quantitative data included patient demographics, claims data, and financial and data measures. Descriptive statistics of participating clinics and the control clinic were analyzed, and aggregated clinic data showed improvements in patient care delivery measures. Qualitative interviews were also conducted at midpoint and conclusion, and an outcome evaluation was completed. This short report will provide readers with results from the COrHT Initiative, with an emphasis on medical-dental integration (MDI) as an integral component of comprehensive, person-centered care. Abstract Background Oral health in Sweden is good at the population level, but seemingly with persisting or increasing inequities over the last decades. In 2008, a major Swedish reform introduced universal partial subsidies to promote preventive care and reduce the treatment cost for patients with extensive care needs. This study aimed to apply an intersectional approach to assess the impact of the 2008 subsidization reform on inequities in self-rated oral health among adults in Sweden over the period 2004–2018. Methods Data from 14 national surveys conducted over 2004–2018 were divided into three study periods: pre-reform (2004–2007), early post-reform (2008–2012) and late post-reform (2013–2018). The final study population was 118,650 individuals aged 24–84 years. Inequities in self-rated oral health were examined by intersectional analysis of individual heterogeneity and discriminatory accuracy across 48 intersectional strata defined by gender, age, educational level, income, and immigrant status. Results Overall, the prevalence of poor self-rated oral health decreased gradually after the reform. Gender-, education- and income-related inequities increased after the reform, but no discernible change was seen for age- or immigration-related inequities. The majority of intersectional strata experienced patterns of persistently or delayed increased inequities following the reform. Conclusions Increased inequities in self-rated oral health were found in most intersectional strata following the reform, despite the seemingly positive oral health trends at the population level. Applying an intersectional approach might be particularly relevant for welfare states with overall good oral health outcomes but unsuccessful efforts to reduce inequities.

Abstract Oral health care is essential, and digital training may influence healthcare professionals’ attitudes to and knowledge of oral health. The aim, therefore, was to evaluate the impact on attitudes to and knowledge of oral health after using a digital training module among Swedish healthcare professionals working within a municipality-run healthcare service for older adults. A secondary aim was to explore the healthcare professionals’ experiences of using the digital module. The study comprised a survey of healthcare professionals (registered nurses (RNs), assistant nurses, and care assistants) caring for older adults in a municipality in Sweden. Pre-post-tests were conducted to evaluate the outcomes for attitudes to and knowledge of oral health and of their experiences of completing the digital training module in oral health. These were statistically explored by comparing differences between the pre-post-tests, while the open-ended questions were analysed with qualitative content analysis. The findings of this study indicate that healthcare professionals had similar perceptions of their attitudes to and knowledge of oral health both before and after the digital training module in oral health. The study also indicates that healthcare professionals experienced that it is easier to perform practical oral health care after completing the digital training. The results also show that healthcare professionals value oral health knowledge and that the digital training module was easy to use and to disseminate knowledge throughout the municipality. The findings have implications for developing, implementing, and promoting healthcare professionals’ attitudes to and knowledge of oral health and in using a digital training module in combination with practical exercises in oral health in municipality health care.

Background Community chemists now provide more professional and public health care, which is beneficial to the community, in addition to traditional medication distribution. While community pharmacists play a vital role in maintaining and promoting health, further research is required to accurately evaluate their knowledge, attitude, and practice toward oral health care among Jordanian pharmacists in northern Jordan. Methods A prospective cross-sectional study was conducted using an anonymous self-administered questionnaire in Arabic language, targeting 250 community pharmacists in four cities (Irbid, Jerash, Ajloun, and Al-Mafraq) in northern Jordan. The questionnaire comprised four domains: socio-demographic, knowledge, attitudes and practice concerning oral health care. The Statistical Package for the Social Sciences (SPSS) software version 22.0 was used to analyze the collected data, and the significance level was set at P ≤ 0.05. Results A total of 198 respondents returned the survey form, returning a response rate of 79.2%. Community pharmacists showed a good knowledge score of 58.2%. Only 40.9% said periodontal disease was the most frequent cause of tooth loss. The majority lacked knowledge of the optimal fluoride levels in toothpaste. The average attitude score was 71.05%. The most common oral health issues raised by patients were gingivitis and toothache (43.4% and 38.9%), respectively. Conclusion Community pharmacists have the general knowledge and attitude needed to play a significant role in providing dental care counselling to patients dental care. Nonetheless, community pharmacists stated they needed education and training to improve their practice. Continuing education courses related to oral health are recommended to reduce the risk of oral diseases and improve quality of life.

Oral care is a crucial challenge of nursing care in orally intubated patients. Oropharyngeal colonization with microorganisms is probably the first step in the pathogenesis of most bacterial pulmonary infections. This study aimed to investigate the effect of different oral care solutions on the oral health status of critically ill patients. We conducted a quasi-experimental study involving a convenience sample of 60 adult orally intubated patients, distributed equally into three groups: 20 patients received 0.12% chlorhexidine gluconate (CHX) solution as an oral rinse; 20 patients received 0.1% hexetidine (HEX) solution as an oral rinse; and a control group of 20 patients received routine hospital oral care with 0.9% normal saline (NS) solution. Oropharyngeal and tracheal cultures were obtained from patients within 24-48 h of admission, before the administration of topical oral antimicrobial solutions and then repeated on day 4 and day 7 after the oral solutions. The study revealed that CHX has a more powerful effect than HEX and NS in improving the oral mucosa and decreasing colonization of both the oropharynx and trachea. On day 7, the improvements were statistically significant in the CHX group and the HEX group (P = 0.02 and P = 0.03, respectively), but not in the NS group. This research confirms the effect of CHX and HEX in lowering the risk of tracheal and oropharyngeal colonization, and recommends the use of a CHX solution as oral mouth care in critically ill patients. A statistically significant inverse correlation was observed between dental care visits and both age (r = −0.025, p < 0.001) and BMI (r = −0.013, p < 0.001). Mean dental care visits were significantly higher in patients without hypertension compared with those with hypertension (p < 0.001). Moreover, there was an approximate 4% increase in dental care visits among females compared to males, although this difference was not statistically significant. The study identified three predictors contributing to the low utilization of dental services in PHC centers in Riyadh. These include an inverse association between both age and body mass index (BMI) and the utilization of OHC. This review aims to assess the impact of oral health care interventions on the overall health enhancement and prevention of disease among older adults residing in nursing homes. A systematic search was conducted for reviews published between January 2012 and December 2022, focusing on oral health in nursing home settings. Seventeen reviews met the inclusion criteria, encompassing studies involving older adults, their families, and nursing home staff. Various databases, including PubMed, CINAHL Complete, Scopus, and others, were searched, and quality assessments were performed using established guidelines.Findings suggest a correlation between oral health and nutritional status, with potential implications for pneumonia prevention through professional oral health care. However, recommendations for interventions remain elusive due to insufficient evidence, particularly regarding the effectiveness of oral health education for caregivers. Despite a documented low oral health-related quality of life among older nursing home residents, oral health promotion programs show promise in addressing this issue.In conclusion, there is a pressing need to enhance oral health among older adults in nursing homes. While certain interventions demonstrate potential effectiveness, the current evidence lacks consistency and comprehensiveness. Future research efforts should prioritize collaborative approaches between practice and research to explore diverse outcomes and ensure optimal practice.

BackgroundCardiovascular diseases (CVDs) are a significant cause of morbidity and mortality worldwide, resulting in a high socioeconomic burden. Growing evidence has shown a link between oral diseases and several chronic conditions including CVDs. The focus of this review is to investigate and summaries the evidence surrounding oral health interventions and their potential impact on reducing both the risk and/or severity of CVDs.MethodsA scoping review was conducted to examine oral health interventions for managing CVD outcomes and risks. The review adhered to the Joanna Briggs Institute (JBI) framework for evidence synthesis and followed the reporting standards outlined by the Preferred Reporting Items for Systematic Reviews and Meta-analysis- extension to Scoping Review (PRISMA-ScR). A systematic search across EBSCOhost, PubMed, and Scopus databases from 2012 to 2024 was utilized to identify relevant studies. Inclusion criteria focused on English language articles with a sample size of at least 50, evaluating the impact of oral health interventions on CVD outcomes.ResultsOut of the initial 2,154 studies identified in the search, 12 studies met the inclusion and exclusion criteria and were included in the final analysis. Overall, the studies revealed that along with surgical and non-surgical periodontal therapy, regular oral hygiene care practices, including toothbrushing, tongue brushing, and flossing, significantly reduced the risk of cardiovascular events and mortality. These interventions in patients with or without CVD baseline have shown a decrease in CVD risk markers as well as a reduction in bacterial colonization. Similarly, consistent oral hygiene routines, combined with regular dental visits, were associated with a lower risk of heart failure and CVD risk mortality.ConclusionThe evidence extracted from this review suggests that periodontal therapy, regular dental cleaning, and re-enforcing of oral health regimes can stabilize oral health conditions and subsequently improve CVD progression/risks. However, limited to no evidence exists regarding the therapeutic effects of oral health promotion in managing CVD markers and its direct impact on disease outcomes, warranting further investigation.

The high incidence and untreated rate of root caries, a common and frequently occurring oral disease with challenging treatment in elderly individuals, is the main cause of tooth loss among elderly people, as rapid development results in pulpitis and periapical periodontitis or residual crown and root, which has been regarded as one of the common chronic oral diseases seriously affecting the quality of life of elderly people. Thus, early intervention and prevention are important. Traditional dental materials for preventing root caries have been widely used in clinical practice; however, they have the disadvantages of tooth coloring, remineralization and low sterilization efficiency. A series of new dental materials for preventing root caries have gradually become a research hotspot recently, which have the advantages of promoting the mineralization of deep dental tissue, prolonging the action time and enhancing adhesion. Future caries prevention materials should be designed according to the characteristics of root surface caries and the application population and should be developed toward simplicity, high efficiency and low toxicity. This review describes current research regarding anti-caries prevention material application, serving as a theoretical underpinning for the research of root caries prevention materials, which is important for both promotion in the effective prevention of root caries and improvement in the status of oral health and the quality of life among old people.

Aim of the study This paper aims to review existent literature regarding the impact of COVID-19 pandemic on oral health care in children with leukaemia. Material and methods An evaluation of the literature was carried out, after searching available databases for the following terms: COVID-19, oral care, paediatric oncology patients. Results The COVID-19 pandemic seems to have strongly interfered with dental care management of paediatric patients in terms of altering or disrupting treatments, restricting procedures and techniques, and an overall decrease in paediatric dental procedures worldwide. Literature is scarce regarding oral healthcare of paediatric oncology patients not only but especially during this period. Conclusions: Further studies are required to evaluate the amplitude of factors which impacted oral health in children with leukaemia.

Introduction: This study aimed to identify social, psychological, and contextual factors that influenced attendance at routine oral health visits in a cohort of 189 preschool children who were followed over a 2-year period. Methods: Generalized estimating equation was used to examine the association between clinic attendance and the predictors. ORs and 95% CIs were reported in the multiple logistic regression models. The study was conducted in Rochester, New York, between February 2016 and February 2021. Results: Prior to the COVID-19 pandemic declaration, the rate of canceled and no-show appointments was greater for routine clinic visits (20% and 24%, respectively) than for research visits (14% and 9%, respectively) for the same participants; these rates increased during the pandemic. After adjusting for sociodemographic factors, the likelihood of a canceled or no-show appointment was associated with parental depression (OR=1.06, CI=1.03, 1.09), regardless of the type or occurrence of the visit. Conclusions: Findings from this study demonstrate that attendance to oral health care in young children is reliably reduced with parental depression and that this may provide one mechanism for early emerging health inequalities of oral health.

Aims: This study described the influence of a large‐group simulation on social and health care students´ working life competence concerning interprofessional collaboration (IPC) and patient centered‐care. Methods and results: The target group (n = 319) was social and health care students from several different degree programs taking part in a large‐group simulation concerning older people´s oral health as a part of his comprehensive well‐being and health. Data were collected with a questionnaire containing background questions, statements about interprofessional practice and open‐ended questions about learning experiences. The number of respondents was 257, including 51 oral health care students (OHCS). The data were analyzed using descriptive and statistical methods and content analysis. Working life competencies covers the overall social and collaborative skills which health‐care professionals need in their work. They were reported to be improved in IPC and patient‐centered care (PCC). The main described learning experiences in open responses included appreciating different professionals' competencies and the importance of interprofessional decision‐making, interpersonal communication competence and patient‐centered attitudes in care provision. Conclusion: The large‐group simulation is a good model for educating larger student groups simultaneously and it functioned well in improving understanding of IPC and PCC among older adults. Background/Aims: Optimum oral healthcare and dental education is vital for children and young people with congenital heart disease to reduce the risk of infective endocarditis. The aims of this study were to ascertain the information provided by cardiac nurses regarding preventative dental care for children and young people with congenital heart disease and their families. Methods: Data about preventative dental advice were collected via a cross-sectional online survey of children and young people's cardiac nurses, conducted from November 2021–March 2022 and October–November 2022. A snowball sampling method was used to recruit respondents for the study, resulting in responses from 27 cardiac nurses across seven cardiac networks in England. Results: Of the 27 participants, 24 (88.9%) gave dental advice to patients and their families, 21 (77.8%) and were moderately or highly confident about providing advice, but just over half (n=14, 51.9%) were unaware of national congenital heart disease dental standards. Three themes emerged from analysis of the survey: educational needs, resource availability and empowerment. Conclusions: Despite the relatively low response rate, inconsistencies in the implementation of cardiac standards among different networks were evident. Recommendations include improving access to dental services, improving collaboration between dental and cardiac teams, providing dental education for children and young people's cardiac nurses and giving correct advice to parents to promote optimum dental health for this high-risk group. Background: Oral health care has a significant effect on the quality of life (QoL) of elderly wellbeing. The aim of present study: was to assess relation between oral health care of elderly and their quality of life. Subjects and method: Design: This research used descriptive cross sectional research design. Setting: It was conducted in Ali Ebn Abi Talib Health Insurance Outpatient Clinics, Tanta city. Subjects: Convenience sample of 500 aged who attended the previous settings were participate in this study. Tools of study: Two tools were used to collect data Tool I: A structure interview schedule that consisted of three parts, Part (1): Socio demographic characteristics, medical history, and lifestyle of elderly, Part (2): Knowledge of aged person about oral health care and Part (3): Self-reported practices of elderly about oral care. Tool II: Oral health related quality of life scale (OHRQoL). Results: Less than twothirds (62.4%) of studied participants had high levels of knowledge, more than half (58.2 %) of the participants had unsatisfactory practices and the majority of them (84.8%) had poor levels of quality of life regarding oral health. Conclusion: There was highly significant positive correlation between total score of knowledge and practices of studies elderly, and total score of quality of life at P < 0.01. Recommendations: present study recommended that, health promotion instructions should be designed for improving the elderly's oral health practices and oral health related quality of life. Objective: To assess whether, and if so, which oral health care services for community‐dwelling older people with dementia are available. Background: Oral health in people with dementia is poor compared with people without dementia. Although multiple oral health care interventions have previously been studied for older people living in nursing homes, little is known about interventions or services for community‐dwelling older people with dementia. Materials and methods: A literature search was performed in the databases Pubmed, Embase and CINAHL. The following search terms were used: "Dementia", "Oral health", "Dental health services" and "Older person". The term "dental health services" was intended to be an as broad as possible construct because limited search results were expected. Results: The search generated 1624 unique references, of which seven studies were eligible for inclusion (four cohort studies, one cross‐sectional study, and two qualitative studies). The included studies described two actual oral health care services: a telephone help line on oral health and dementia, and a mobile geriatric dental programme in adult day health centres. A need was found for services and strategies specific to community‐dwelling older people with dementia. One identified solution was an intervention with individually tailored daily oral hygiene self‐care supported by the informal caregiver. Furthermore, increasing accessibility of oral health care professionals with treatment at other locations than their own practices, better collaboration between health care professionals and preventive oral health care are highly necessary. Conclusion: There is limited evidence on the availability of oral health care services for community‐dwelling older people with dementia, while a need was found for oral health care services that focus on good accessibility, oral hygiene self‐care, preventive strategies and collaboration among health care professionals. Aim: To examine factors that affect the performance of oral health care (OHC) for older people receiving nursing care at home. Background: Oral health is often neglected by health care providers caring for older people. Research shows that health care providers' provision of OHC may be influenced by various factors (barriers and facilitators). When this research was conducted, health care providers from home healthcare services (HHCS) and nursing homes were grouped together despite setting differences; therefore, this study focuses on the performance of OHC by home health care providers (HHCPs) as a single group. Design: Explorative design with a qualitative approach. Methods: The managers of four HHCS units recruited 17 HHCPs to participate in focus group interviews. One interview was conducted per unit, and there were four to five participants in each interview. The analysis of interviews was based on theoretical thematic analysis and the PRECEDE constructs in the PRECEDE–PROCEED model. Consolidated Criteria for Reporting Qualitative Research (COREQ) guidelines were used in reporting this study. Findings: The analysis resulted in two themes with predisposing factors (HHCPs' professional responsibilities, older people's attitude), five themes with enabling factors (knowledge and skills, older people/carer trust, available time, available equipment and collaboration with public dental service (PDS)), and two themes with reinforcing factors (routines and OHC focus on the workplace) that affect the provision of OHC. The factors were categorised as individual, organisational and collaboration factors. Conclusions: In addition to individual factors found in previous studies, factors related to the organisation of services and communication between HHCPs and PDS seem to affect HHCPs' provision of OHC for adults receiving HHCS. Implications for Practice: This study provides in‐depth knowledge that can contribute to increasing HHCPs' provision of OHC and thereby prevent oral and dental disease among older people receiving HHCS. Objective To investigate the application effect of oral health education combined with traditional Chinese and western medicine on the periodontal health in the elderly residents in the community. Methods A total of 160 permanent residents aged 65-74 in Zhaoqing City were enrolled as research subjects, and divided into the control group (n=78) and the research group (n=77) based on their recruitment numbers. The control group received routine oral health instruction, and the research group received traditional Chinese medicine oral health care methods on the basis of routine oral health education. The plaque debris index (DI), calculus index (CI), gingival index (GI) and probing depth (detection of deep and shallow periodontal pockets) of the periodontal pockets were compared before and after oral health education. Results Before the health education, there were no significant differences in DI, CI, GI, shallow and deep periodontal pocket detection rate between the two groups (P >0.05). After the health education, the periodontal health status of the subjects in two groups, such as DI, CI, GI and the detection rate of shallow and deep periodontal pockets, were improved, and the periodontal health of the study group was better than that of the control group, with all statistical significance (P <0.05). This study identifies different factors influencing the patient's decision to receive dental treatments during the ongoing pandemic and identifies their expectations at the dental office during Covid-19. A cross-sectional study was conducted among 306 people in a dental institute. A questionnaire was used to collect primary demographic data. It consisted of questions concerning patient attitude regarding the utilization of dental services and expectations from the dental setup during the pandemic. The chi-square test was used to analyze the association between variables with a significance level set at p=0.05. The mean age of the study population was 37±5.3 years, with the majority being males (52%). Many female patients (60.76%) and those from the lower middle class (38.59%) were apprehensive about getting the disease during their visit. Covid19 was not considered a threat among adolescents/middle-aged (47.27%) and upper lower-class patients (41.81%). Spending extra during this period was acceptable by 65.7% of patients, though a few (16.3%) responded or felt that the hospital should be considerate and bear the expenses. They (38.9%) had no specific complaints about the cleanliness issue of the hospital premises. All the above results were significant statistically (p≤0.05). Though apprehension of contracting the disease was observed among many patients, their dental problems were considered a priority, influencing their visit to the dental hospital. Abstract Background The World Health Organization (WHO) has recently devoted special attention to oral health and oral health care recommending the latter becoming part of universal health coverage (UHC) so as to reduce oral health inequalities across the globe. In this context, as countries consider acting on this recommendation, it is essential to develop a monitoring framework to measure the progress of integrating oral health/health care into UHC. This study aimed to identify existing measures in the literature that could be used to indicate oral health/health care integration within UHC across a range of low-, middle- and high-income countries. Methods A scoping review was conducted by searching MEDLINE via Ovid, CINAHL, and Ovid Global Health databases. There were no quality or publication date restrictions in the search strategy. An initial search by an academic librarian was followed by the independent reviewing of all identified articles by two authors for inclusion or exclusion based on the relevance of the work in the articles to the review topic. The included articles were all published in English. Articles concerning which the reviewers disagreed on inclusion or exclusion were reviewed by a third author, and subsequent discussion resulted in agreement on which articles were to be included and excluded. The included articles were reviewed to identify relevant indicators and the results were descriptively mapped using a simple frequency count of the indicators. Results The 83 included articles included work from a wide range of 32 countries and were published between 1995 and 2021. The review identified 54 indicators divided into 15 categories. The most frequently reported indicators were in the following categories: dental service utilization, oral health status, cost/service/population coverage, finances, health facility access, and workforce and human resources. This study was limited by the databases searched and the use of English-language publications only. Conclusions This scoping review identified 54 indicators in a wide range of 15 categories of indicators that have the potential to be used to evaluate the integration of oral health/health care into UHC across a wide range of countries.

The aim. To analyze the main indicators of dental care to the population, the system of financial support for dental care in Ukraine and to study modern WHO initiatives to improve dental care for the population, increase its availability and quality. Methods – system approach and system analysis, bibliosemantic, content analysis. Results. Over the past decades, the country has revealed a significant deterioration in the dental status of the population due to the unsatisfactory state of preventive activities, changes in the financing of the dental industry. Taking into account the current WHO initiatives to protect oral health, it is promising to improve the existing forms of interdisciplinary cooperation of doctors of various practices to prevent the development of dental diseases, improve interdisciplinary education in the prevention of the most common diseases of the oral cavity of doctors of various specialties. Formation of motivation among the population to preserve dental health and provide it with preventive services, introduction of new forms of financing of the dental industry, in particular, at the expense of social insurance, the introduction of public-private partnership, increasing the state's responsibility for dental care to socially vulnerable segments of the population will help reduce the financial burden for the patient when receiving dental services in the conditions of modern realities in the system of financing dental care.

Payment for dental services is one of the key determinants of treatment options, access to health care, and treatment decisions. The ability to pay for services directly affects the quality of life of an individual and also their accessibility to oral health services. The aim of the present study was to identify the different methods of payment used to obtain oral health care in Yaoundé, Cameroon This was a cross-sectional descriptive study carried out in selected dental clinics in Yaoundé using convenient sampling between January 2021 and June 2021. Data was collected using a structured questionnaire. A total of 377 patients aged 8 to 83 years made up of 194 (51.46%) females and 183 (48.54%) males participated in the study. There is no government-assisted package for oral health care in Cameroon and more than two-thirds (66.84%) made direct payments or paid out of pocket without a refund from any insurance company. More than half 147 (58.33%) of the study population did not complete the planned dental care due to insufficient funds. Direct payments covered treatments such as scaling, restorations, root canal therapy, and dental extractions while prostheses (dentures, crowns, bridges, and implants) were only afforded by very few patients. The reasons for the inability to pay for dental services included financial difficulties 107 (72.97%) and lack of time 36 (24.49%). Only half 36 (50%) of the participants employed in the formal sector had access to health insurance and treatments like scaling, dental prosthesis, orthodontics, and dental implants were rarely provided by an insurance company. These procedures were considered to be very expensive with fluctuating prices and were viewed as being for aesthetic purposes. The majority 67 (93.06%) of payments through insurance reported being reimbursed aftercare. Two-thirds of 48 (71.64%) of the insured participants said it was difficult to get reimbursed due to the administrative bottlenecks and as such some preferred to pay out of pocket despite being insured. Late consultations encountered led to caries complications with 105 (52.24%) pulpitis. In general, financial difficulty was reported by 97 (48.26%) as the main reason for not being able to afford dental care. Oral diseases present a huge burden for children and adults due to the high cost of treatment limiting their accessibility and treatment options. There is a need to create compulsory health insurance to enhance greater access to oral health services to encourage the population to seek dental care timeously. [ABSTRACT FROM AUTHOR]

The oral health of older individuals can be negatively impacted by various systemic health factors, leading to rapid oral health deterioration. This paper aims to present an overview of the published evidence on systemic health factors that contribute to rapid oral health deterioration in older individuals, and to explore the implications of these factors for both general healthcare and oral healthcare provision. Older people are at risk of experiencing adverse reactions to medications due to multimorbidity, polypharmacy, and changes in pharmacokinetics and pharmacodynamics. Hyposalivation, a significant side effect of some medications, can be induced by both the type and number of medications used. Frailty, disability, sarcopenia, care dependency, and limited access to professional oral healthcare can also compromise the oral health of older people. To prevent rapid oral health deterioration, a comprehensive approach is required that involves effective communication between oral healthcare providers, other healthcare providers, and informal caregivers. Oral healthcare providers have a responsibility to advocate for the importance of maintaining adequate oral health and to raise awareness of the serious consequences of weakened oral health. Dental hygienists (DHs) are well positioned to be part of primary care teams to increase access to oral health care, particularly for those who encounter barriers to oral care, such as pregnant people. The Michigan Initiative for Maternal and Infant Oral Health (MIMIOH) integrates DHs into obstetrics and gynecology (OB/GYN) clinics in federally qualified health centers (FQHCs) to improve pregnant people's oral health. Evaluation of the MIMIOH program revealed that selecting DHs with personal characteristics desirable for integrated-care delivery was a major factor for successfully integrating DHs into OB/GYN clinics. Additionally, designing appropriate clinical workflows, gaining buy-in from prenatal health care professionals, presenting oral health care as part of prenatal care, having co-located OB/GYN and dental clinics, and maintaining adequate funding were all critical to program success. Analysis of Medicaid data showed that the MIMIOH model increased the percentage of pregnant people receiving oral health care at FQHC dental clinics. Innovative programs like MIMIOH add to the evidence that integrating DHs into primary care settings can increase access to oral health care, especially for those who face difficulties accessing the traditional oral health care system. There is great potential for DHs to leverage collaborative practice agreements and remote supervision to increase access to oral health care for the public. Providing DHs with autonomy to practice to the highest level of their scope of practice and permitting direct Medicaid reimbursement for DHs would make oral health care more accessible for underserved populations. Objectives: The goal of the Swedish oral healthcare system is to achieve good oral health and equitable access to care for the entire population. However, considerable inequities in oral health and care are evident and occur across a range of social dimensions. This study uses an intersectional approach to examine complex inequities in unmet oral care needs among adults in Sweden over the period 2004–2021. Methods: Data were obtained from 14 Health on Equal Terms surveys conducted during 2004–2021. The final sample was 129 473 individuals aged 26–84 years. Applying intersectional analysis of individual heterogeneity and discriminatory accuracy, inequities in unmet oral care needs were estimated across 48 intersectional strata defined by gender, age, educational level, individual disposable income and immigrant status. Results: A high risk of unmet oral care needs was found among strata consisting of immigrants and those with low income. However, being an immigrant and/or having a low income did not universally entail a high risk but varied by the social position along other axes, particularly age and education. The discriminatory accuracy was moderate. Conclusion: Groups with certain social disadvantages are highly heterogeneous themselves. An intersectionality approach is important to prevent the risk of stigmatizing large heterogenous groups while failing to identify the most vulnerable strata. The discriminatory accuracy analysis suggested that further policy and/or interventions may be the most effective if approaching the whole population, combined with selected targeted interventions directed at the most disadvantaged social strata.
