In the management of traumatic brain injury (TBI), intracranial pressure monitoring (ICPm) is crucial for the timely management of severe cases that show rapid neurological deterioration. External ventricular drains (EVDs) and intraparenchymal pressure monitors (IPMs) are the primary methods used in this setting; however, the debate over their comparative efficacy persists, primarily because of reliance on observational study data. This underscores the need for a meta-analysis to guide clinical decision-making. This study-level meta-analysis aims to assess and compare the efficacy and safety of EVDs versus IPMs in the management of TBI. A database search was conducted until February 13, 2024, to identify studies reporting clinical outcomes of patients with TBI who underwent ICPm with either EVD or IPM. Primary outcomes included mortality, ICPm duration, length of stay, and complications. From an initial pool of 537 articles, eight studies (six retrospective cohort studies and two prospective cohort studies), encompassing 7080 patients, met our inclusion criteria. Mortality rates showed no significant difference between groups (risk ratio 1.11 [95% confidence interval (CI) 0.86 to 1.42], p = 0.42). Patients monitored with IPM had shorter intensive care unit length of stay (mean difference 0.90 [95% CI 0.21 to 1.59], p = 0.01) and ICPm duration (mean difference 0.79 [95% CI 0.33 to 1.24], p = 0.0007), with a higher risk of requiring surgical decompression. Monitoring-related complications were similar across the two groups. Our findings suggest that EVD and IPM provide similar outcomes in terms of mortality. However, IPM may offer significant advantages in reducing the duration of ICPm and intensive care unit length of stay. EVD may be preferable for certain mid-term to long-term monitoring. Introduction: The main consequence of not treating tooth decay in time is the total or partial loss of the tooth. One of the alternatives to replace the lost tooth and return the functions is through fixed prostheses. Today there are 3 generations of yttrium-stabilized zirconium polycrystals (Y-TZP). Seeing that in the third Generation the yttrium content was increased to approximately 5% in moles (5Y), stabilizing the materials approximately 50% of cubic phase and 50% of tetragonal phase, thus giving a translucent zirconium (5Y) with a high aesthetics and greater naturalness to the dental crown., Objective: To identify the different optical and mechanical properties of translucent zirconium compared to other restorative materials used in fixed prostheses., Materials and Methods: This narrative review evaluated 115 scientific articles from the Medline information sources (via PubMed), Science Direct, Scopus, SciELO and LILACS from the last 5 years, published in Spanish, Portuguese and English. We included 51 articles that met the selection criteria which were randomized clinical trials and observational studies, in vitro studies and in vivo studies that evaluated the optical and mechanical properties of translucent zirconium as an optimal restorative material in fixed prostheses. We excluded systematic reviews, case reports and editorials., Results: It was observed that zirconium being a relatively new material and in constant evolution has studies mostly in vitro, among which stand out those that speak both of the mechanical and optical properties that vary depending on the generation to which the material belongs, as well as the adhesiveness of the material which is for now not viable, therefore, it is essential that more studies be carried out on this material., Conclusions: We can mention that zirconium has evolved over time, providing better optical properties, but as a consequence it has been reducing flexural strength and fracture toughness. However, this material provides good biocompatibility with oral structures. Giant cell myocarditis (GCM) is a rare disorder in which survival beyond 1 year without heart transplantation is uncommon. Long-term follow-up data on those with such survival are lacking. Twenty-six patients with biopsy-proved GCM who survived for >1 year without heart transplantation were identified from a multicenter GCM registry. The incidence of death, transplantation, ventricular assist device placement, and histologically proved disease recurrence was ascertained retrospectively. The rates of recurrent heart failure, ventricular arrhythmias, renal failure, and infectious complications were calculated. The mean age of the cohort was 54.6 ± 13.9 years (65% women). The mean follow-up duration was 5.5 years starting 1 year after diagnosis. There were 3 deaths (12%), 5 heart transplantations (19%), and 1 ventricular assist device placement (4%). Three histologically confirmed recurrences of GCM (12%) occurred between 1.5 and 8 years after diagnosis. Thirteen of 26 patients experienced a total of 30 heart failure episodes ≥1 year after initial diagnosis. There were 23 episodes of elevated creatinine in 12 patients, 41 infectious events in 13 patients, and 19 episodes of ventricular arrhythmias in 6 patients with a total of 144 years of follow-up. Starting 1 year after GCM diagnosis, the combined rate of death, transplantation, ventricular assist device placement, and GCM recurrence was 47% at 5 years.
