Abstract

Computer-assisted management of antiinfective drug therapy in patients with infections may improve care and reduce the cost of treatment. LDS Hospital in Salt Lake City, Utah uses a computer program to assist physicians in choosing antibiotics. Researchers compared the treatment of 545 patients managed by the program to 1,136 patients treated before the program was instituted. The computer program helped physicians significantly reduce excess drug dosages, selection of inappropriate drugs, prescription of drugs to which the patient was allergic, and the costs of treatment.

Abstract

Adult respiratory distress syndrome (ARDS) is a serious lung condition that most commonly strikes young persons who have not had previous breathing problems; it is usually caused by severe infection, injury, or illness. Mortality from ARDS is very high, at approximately 50 percent. ARDS can occur in women during pregnancy and the immediate postpartum period. At one hospital, 14 such cases were studied over a seven-year period. The average patient age was 26, and only two were over age 30. ARDS occurred in the last trimester of pregnancy in most of the cases. Six of the women died, representing a mortality of 43 percent. In 12 of the women ARDS was caused by a complication of pregnancy, such as infection, hemorrhage, labor complications, toxemia (high blood pressure with fluid retention and protein in the urine) and eclampsia (a severe form of toxemia characterized by convulsion, coma, and high mortality). Three of the women developed difficulty breathing during an emergency cesarean section, which may have brought on the respiratory distress. Infections were diagnosed in eight women, and in six of these the infection was related to the pregnancy. Two women had ongoing medical problems (alcoholism and diabetes with high blood pressure) that may have contributed to their obstetrical complications. The patients required mechanical ventilation for an average of 16 days. It is concluded that ARDS in pregnant or postpartum women is rare, but carries a high fatality rate. Abstract

We developed a clinical decision support system--ventilation protocols--that managed tidal volume and ventilator rate settings during mechanical ventilation of patients with the Adult Respiratory Distress Syndrome (ARDS). We applied these protocols for a total of 10,903 hours in 40 ARDS patients. The clinical staff suspended the protocols for only 5% of the total application time due to medical procedures, surgeries, transient clinical problems not addressed by the protocols, or because of attending physician request. Of 3,148 instructions generated by the ventilation protocols, the clinical staff followed 2,932 (93%). The staff did not follow some instructions because of patient data errors, computer software and protocol logic errors, inability of the clinical staff to implement protocol instructions because of more pressing duties, and clinical staff objections to specific instructions. Sixty percent of the patients treated by the ventilation protocols survived. Our results demonstrate that the ventilation protocols provided a practical and safe decision support system for the mechanical ventilation of ARDS patients.

50. A strategy for development of computerized critical care decision support systems

Abstract

It is not enough to merely manage medical information. It is difficult to justify the cost of hospital information systems (HIS) or intensive care unit (ICU) patient data management systems (PDMS) on this basis alone. The real benefit of an integrated HIS or PDMS is in decision support. Although there are a variety of HIS and ICU PDMS systems available there are few that provide ICU decision support. The HELP system at the LDS Hospital is an example of a HIS which provides decision support on many different levels. In the ICU there are decision support tools for antibiotic therapy, nutritional management, and management of mechanical ventilation. Computer protocols for the management of mechanical ventilation (respiratory evaluation, ventilation, oxygenation, weaning and extubation) in patients with adult respiratory distress syndrome ((ARDS) have already been developed and clinically validated at the LDS Hospital. These protocols utilize the bedside intensive care unit (ICU) computer terminal to prompt the clinical care team with therapeutic and diagnostic suggestions. The protocols (in paper flow diagram and computerized form) have been used for over 40,000 hours in more than 125 adult respiratory distress syndrome (ARDS) patients. The protocols controlled care for 94% of the time. The remainder of the time patient care was not protocol controlled was a result of the patient being in states not covered by current protocollogic (e.g. hemodynamic instability, or transport for X-Ray studies). 52 of these ARDS patients met extra corporal membrane oxygenation (ECMO) criteria. The survival of the ECMO criteria ARDS patients was 41%, four times that expected (9%) from historical data (p<0.0002). The success of these computer protocols and their acceptance by the clinical staff clearly establishes the feasibility of controlling the therapy of severely ill patients.
