Abstract

Background: Psoriasis (Ps) is a chronic systemic autoimmune disease associated with pruritus in 64–98% of patients. However, few modestly sized studies assess factors associated with psoriatic pruritus. Objective: To investigate factors associated with Ps pruritus intensity. Methods: Psoriasis patients 18years or older seen in one of 155 centres in Italy between September 2005 and 2009 were identified from the Italian PsoCare registry. Patients without cutaneous psoriasis and those with missed information on pruritus were excluded. Results: We identified 10802 patients, with a mean age 48.8±14.3years. Mild itch was present in 33.2% of patients, moderate in 34.4%, severe in 18.7% and very severe in 13.7%. Higher itch intensity was associated with female gender, lower educational attainment compared to university degree, pustular psoriasis, psoriasis on the head, face, palmoplantar areas, folds and genitalia, more severe disease, disease duration

Abstract

The recent developments in space exploration have reinstated the Moon as a primary target for near future space missions. The principal reasons include the Moon being the closest test bed and analogue for planetary space missions and the prospect of scientific lunar bases and orbital stations within the next decade. Previous space missions have vastly improved our understanding on hazards of human spaceflights but not fully regarding the threats affecting a prospective lunar base or orbital station. The micrometeorite hazard has been partially addressed as an issue which can potentially impact both astronauts' health and safety as well as create issues for lunar bases and orbital stations, such as degradation or permanent damage of equipment and facilities. The current understanding is based partly on dust and micrometeoroid flux measurements and impact flash observations. However, observations with improved spatial and temporal resolution are imperative for advancing existing hazard models. In this paper, a mission concept of a constellation of nanosatellites is proposed that can both observe larger parts of cis-lunar and trans-lunar space while providing higher temporal resolution. Nanosatellite missions are a cost-effective solution providing data for significant improvement of our current understanding of lunar micrometeoroid flux models, and thus directly the scale of hazards caused by micrometeoroid impacts to future lunar missions. Additionally, such a distributed constellation mission will offer countless opportunities for academia, students and young scientists worldwide. Abstract

Knowledge of the food web structure and complexity are central to better understand ecosystem functioning. A food-web approach includes both species and energy flows among them, providing a natural framework for characterizing species’ ecological roles and the mechanisms through which biodiversity influences ecosystem dynamics. Here we present for the first time a high-resolution food web for a marine ecosystem at Potter Cove (northern Antarctic Peninsula). Eleven food web properties were analyzed in order to document network complexity, structure and topology. We found a low linkage density (3.4), connectance (0.04) and omnivory percentage (45), as well as a short path length (1.8) and a low clustering coefficient (0.08). Furthermore, relating the structure of the food web to its dynamics, an exponential degree distribution (in- and out-links) was found. This suggests that the Potter Cove food web may be vulnerable if the most connected species became locally extinct. For two of the three more connected functional groups, competition overlap graphs imply high trophic interaction between demersal fish and niche specialization according to feeding strategies in amphipods. On the other hand, the prey overlap graph shows also that multiple energy pathways of carbon flux exist across benthic and pelagic habitats in the Potter Cove ecosystem. Although alternative food sources might add robustness to the web, network properties (low linkage density, connectance and omnivory) suggest fragility and potential trophic cascade effects.

Abstract

SummaryBackground The nationwide prevalence of latent tuberculosis infection (LTBI) in Italian patients with psoriasis has never been investigated. Objectives To estimate the nationwide prevalence of LTBI in Italian patients with psoriasis who are candidates for systemic treatment. Methods Data were obtained from the Psocare Registry on those patients (n = 4946) with age > 18 years, systemic treatment at entry specified and tuberculin skin test (TST) performed according to the Mantoux method. LTBI diagnosis was based on a positive TST result in the absence of any clinical, radiological or microbiological evidence of active tuberculosis. Results Latent tuberculosis infection was diagnosed in 8·3% of patients with psoriasis (409 of 4946). 17. Fingerprint of persistent organic pollutants in tissues of Antarctic notothenioid fish

Abstract

In the present work, persistent organic pollutants (POPs), such as polychlorinated biphenyls (PCBs), dichlorodiphenyltrichloroethane (DDT) and metabolites, polybrominated diphenyl ethers (PBDEs), and hexachlorocyclohexane (HCH) were analyzed in three Antarctic notothenioids fish species: Trematomus newnesi (TRN), Notothenia coriiceps (NOC) and Notothenia rossii (NOR). The contribution of each POP-family to the total load was as follows: SPCB (40%) > SDDT (27%) > SPBDEs (23%) > SHCH (10%). Among the 23 PCB congeners analyzed, penta-CBs homologues were the prevalent group, followed by hexa-CBs and hepta-CBs. DDT and its metabolites presented the following trend: p,p'-DDT > p,p'-DDE ~ p,p'-DDD. PBDE profile was dominated by BDE-47 and BDE-99 congeners, followed by BDE-100 > BDE-28 > BDE-154, BDE-153. Among HCHs, the ?-HCH isomer was detected in all samples, constituting 69% total HCH load, while a-HCH and ß-HCH contributions were 15% and 16%, respectively. The levels of POPs reported here suggest that NOR and NOC are more susceptible to accumulate the analyzed contaminants than TRN, a species not previously analyzed for POPs. Distribution of POPs among different tissues of the three species (muscle, liver, gonads, and gills) was also investigated. Considering lipid weight, the general pattern of POPs distribution in tissues indicated that while gonads showed higher levels of PCBs, DDTs and HCH, the most significant PBDE concentrations were recorded in gills. Also, a comparative analysis of POPs concentration in fish samples from Antarctic area was included.

Abstract

Background: Some studies have shown that switching patients from one tumor necrosis factor (TNF)-alfa inhibitor to another may be beneficial when they have an inadequate response or an adverse event. Objective: We sought to assess the variables predicting the efficacy of the second TNF-alfa inhibitor in patients discontinuing the first TNF-alfa inhibitor. Methods: Data from all 5423 consecutive patients starting TNF-alfa inhibitor therapy for psoriasis between September 2005 and September 2010 who were included in the Italian Psocare registry were analyzed. Results: In 105 patients who switched to a second TNF-alfa inhibitor who had complete follow-up data, 75% improvement in the Psoriasis Area Severity Index score (PASI 75) was reached by 29% after 16 weeks and by 45.6% after 24 weeks. Patients who switched because of secondary loss of efficacy (loss of initial PASI 75 response) or adverse events/intolerance were more likely to reach PASI 75 than those who switched as a result of primary inefficacy (PASI 75 never achieved) (hazard ratio 2.7, 95% confidence interval 1.3-5.5 vs hazard ratio 2.0, 95% confidence interval 1.0-3.9 and 1, respectively). Limitations: There was a small number of patients with complete follow-up data. Conclusion: PASI 75 response in patients who switched from one antie-TNF-alfa agent to another was significantly reduced in patients who showed primary inefficacy of the first antie-TNF-alfa.

Abstract

OBJECTIVE: To evaluate variations in laboratory parameters and diagnoses of selected clinical conditions up to 16 weeks after starting a new systemic psoriasis treatment for Psocare Registry enrollees. DESIGN: Prospective cohort study. SETTING: Italian public referral centres for psoriasis treatment. PATIENTS: First-time recipients (n = 10,539) of continuous systemic psoriasis treatment for at least 16 weeks. MAIN OUTCOME MEASURE: Mean variations in (weeks 8 and 16) and proportions of patients reaching a clinically meaningful increase in serum levels (week 16) of total and low-density lipoprotein cholesterol, triglycerides, aspartate amino transferase, alanine amino transferase and creatinine, as well as week-16 cumulative incidences of new diagnoses of diabetes mellitus and arterial hypertension. RESULTS: Mean cholesterol and triglyceride levels significantly increased in patients treated with acitretin or cyclosporine. Mean triglyceride levels also increased in efalizumab- and etanercept-treated patients. Mean transaminase values increased in methotrexate-treated patients, and mean aspartate amino transferase levels increased in infliximab-treated patients. The average serum creatinine value increased in cyclosporine-treated patients. Acitretin and cyclosporine were associated with risk of hypercholesterolaemia (odds ratios 1.51 and 1.34) and acitretin with risk of hypertriglyceridaemia (odds ratio 1.43). Methotrexate and infliximab were associated with risk of more than doubling the upper normal aspartate amino transferase (odds ratios 2.06 and 1.87) and alanine amino transferase (odds ratios 2.38 and 1.74) values. The relative risk of developing arterial hypertension and diabetes was increased for patients receiving cyclosporine (odds ratios 3.31 and 2.88). CONCLUSION: Systemic treatments for psoriasis resulted in heterogeneous effects on the parameters analysed.

It is clear from this review that fish are important predators of krill, in particular the larger myctophids and some channichthyids and nototheniids, and that the importance of krill in fish diets varies substantially both with time and location on various scales, as well as with the availability of alternate prey in the different regions in the Southern Ocean. Ecosystem models therefore need to account for their role. However, several key areas of uncertainty exist, which need to be considered in ecosystem and food-web models for the Southern Ocean. Studies that adequately capture the seasonal variation in consumption rates and changing importance of krill in the diet of fish are also rare. Furthermore, the effects of large changes in abundance and community structure of fishes brought about by industrial fishing need to be considered when evaluating patterns that have emerged in Southern Ocean ecosystems in the course of the 20th century. As a first step towards a modelling approach to include fish in krill-based food-web models, it is suggested to incorporate data on Champsocephalus gunnari from the western Atlantic sector and Dissostichus mawsoni from the Ross Sea into modelling approaches within the CCAMLR Ecosystem Monitoring Program. Both species are important predators of krill (C. gunnari) and fish (D. mawsoni) in turn form key prey for top predators in each region.

In Potter Cove, King George Island, Antarctica, macroalgae provide asignificant food resource for herbivores. The demersal fish Notothenia coriiceps feeds on macroalgae. Eighteen algal species were identified in stomach contents: two chlorophytes, ten rhodophytes and six phaeophytes. Among these the rhodophyte Palmaria decipiens, the phaeophyte Desmarestia menziesii and the chlorophyte Monostroma hariotii comprised the greatest proportions of algal biomass. A food selection study showed four algae to be preferred (P. decipiens, M. hariotii, D. menziesii, Iridaea cordata) and two species to be avoided (Desmarestia anceps and Himantothallus grandifolius) by N. coriiceps. The present investigation indicates that this fish feeds not only intentionally, but also selectively, on macroalgae. Preference for particular algal species is not related to associated epifaunal biomass or to associated amphipod biomass. [ABSTRACT FROM AUTHOR]

Abstract

There are important questions surrounding the potential contribution of outdoor and indoor air quality in the transmission of SARS-CoV-2 and perpetuation of COVID-19 epidemic waves. Environmental health may be a critical component of COVID-19 prevention. The public health community and health agencies should consider the evolving evidence in their recommendations and statements, and work to issue occupational guidelines. Evidence coming from the current epidemiological and experimental research is expected to add knowledge about virus diffusion, COVID-19 severity in most polluted areas, inter-personal distance requirements and need for wearing face masks in indoor or outdoor environments. The COVID-19 pandemic has highlighted the need for maintaining particulate matter concentrations at low levels for multiple health-related reasons, which may also include the spread of SARS-CoV-2. Indoor environments represent even a more crucial challenge to cope with, as it is easier for the SARS-COV2 to spread, remain vital and infect other subjects in closed spaces in the presence of already infected asymptomatic or mildly symptomatic people. The potential merits of preventive measures, such as CO2 monitoring associated with natural or controlled mechanical ventilation and air purification, for schools, indoor public places (restaurants, offices, hotels, museums, theatres/cinemas etc.) and transportations need to be carefully considered. Hospital settings and nursing/retirement homes as well as emergency rooms, infectious diseases divisions and ambulances represent higher risk indoor environments and may require additional monitoring and specific decontamination strategies based on mechanical ventilation or air purification.

47. Emergency Management in the Event of Radiological Dispersion in an Urban Environment.

Abstract

Dispersion of a radiological source is a complex scenario in terms of first response, especially when it occurs in an urban environment. The authors in this paper designed, simulated, and analyzed the data from two different scenarios with the two perspectives of an unintentional fire event and a Radiological Dispersal Device (RDD) intentional explosion. The data of the simulated urban scenario are taken from a real case of orphan sources abandoned in a garage in the center of the city of Milan (Italy) in 2012. The dispersion and dose levels are simulated using Parallel Micro Swift Spray (PMSS) software, which takes into account the topographic and meteorological information of the reference scenarios. Apart from some differences in the response system of the two scenarios analyzed, the information provided by the modeling technique used, compared to other models not able to capture the actual urban and meteorological contexts, make it possible to modulate a response system that adheres to the real impact of the scenario. The authors, based on the model results and on the evidence provided by the case study, determine the various countermeasures to adopt to mitigate the impact for the population and to reduce the risk factors for the first responders.

48. The mitigating role of climate smart villages to the impacts of COVID-19 pandemic in the Myanmar rural communities.

Abstract

Climate smart village approach is identified as an important strategy laid out in the Myanmar Climate Smart Agriculture Strategy (MCSAS, 2016) Four climate smart villages were established in 2017 to facilitate participatory action research to develop the CSV approach as well as to generate evidence of outcomes. The CSV approach is based on the principle of community-directed research process where community-members collaborate with an external researcher to investigate community challenges and their solutions. Like other countries in 2020, the height of the COVID-19 pandemic, Myanmar implemented wide-scale national and local restrictions on mobility that impacted trade and business resulting to an economic slowdown. Rural communities dominated by smallholder agriculture in Myanmar are not spared from the negative impacts of these restrictions. This paper seeks to assess the impacts of the COVID-19 pandemic to the 4 climate smart villages in Myanmar by analyzing household survey data (N = 527) collected in 2020 during the height of economic disruptions and comparing these data to the household survey conducted during the pre-pandemic period of 2018. Our analysis indicated that overall, the effect of the pandemic to agriculture production in 2020 production season in the 4 CSVs has been minimal as evidenced by the continued agriculture production at the same levels as the pre-pandemic conditions in 2018. The effects to household food security and diet diversity has been varied. Abstract

Concentrations of polybrominated diphenyl ethers (PBDEs) and methoxylated polybrominated diphenyl ethers (MeO-PBDEs); are reported in specimens of fish notothenioids Chaenocephalus aceratus (SSI), Trematomus bernacchii (ERN), and Nototheniops nudifrons (NOD) from the South Shetland Islands, Antarctica. Significant differences in the accumulation of 2'-MeO-BDE-68 and 6-MeO-BDE-47 were detected among the analysed species. MeO-BDEs were significantly higher in SSI (11.7, 8.6, and 14.1 ng g -1 lw) than in NOD (1.63, 1.63, and 3.0 ng g -1 lw) in muscle, liver, and gill, respectively. Feeding ecology traits explain the accumulation patterns of MeO-PBDEs. SSI has a higher feeding activity with a broader diet, followed by ERN, whereas NOD is a benthic/sedentary fish with a narrower diet. The accumulation of PBDEs was neither species-, nor tissue-specific. The current study expands the knowledge concerning the accumulation of PBDEs and MeO-PBDEs in Antarctic marine fish and supports the importance of species-specificity in the accumulation of MeO-PBDEs., (Copyright © 2021 Elsevier Ltd. Abstract

This study evaluates the effects of nutrition education on improving knowledge, attitude, and practice (KAP) of mothers and the improvement of the nutritional status of their children. A cluster randomized controlled design using multistage sampling was employed. The integrated school-based nutrition programme included gardening, nutrition education for parents, and supplementary feeding for children (GarNESup). KAP of mothers was assessed using pretested questionnaires administered by teachers. The randomly selected schools were randomly allocated into two groups: Both schools provided lunch to targeted children with one-dish indigenous vegetable recipe, but School 1 received iron-fortified rice whereas School 2 was provided ordinary rice. Eighty wasted and/or anaemic children in each school were fed for 120 days. Nutrition education for children's parents was done every school card claim day and during parent-teacher meetings using 10 developed modules. Weight, height, and haemoglobin level of children and KAP of mothers were measured at baseline and endpoint using standard techniques. KAP of mothers who had completed more than six modules had significantly increased from baseline to endpoint: Negative consequence of worm infestation (33.3% to 60.6%, P = 0.035), importance of serving breakfast for children (42.4% to 78.8%, P = 0.004), cooking vegetables (63.6% to 93.9%, P = 0.002), and purchasing fortified foods was recorded (51.5% to 93.9%, P = 0.000). Children in School 1 had significantly higher weight gain (1.33 ± 0.72) and haemoglobin level (0.49 ± 0.99) than children in School 2 (0.84 ± 0.59; 0.12 ± 0.70). Nutrition education resulted to significant increase of mother's KAP and the implementation of the integrated school-based nutrition model significantly improved children's nutritional status., (© 2019 The Authors.
