© 2015 by Türkiye Klinikleri.Although importance of expression status of caveolin-1 protein was established in different cancers including the renal cell carcinoma, its significance has not been evaluated in Wilms tumor. The aim of this study was to determine the prognostic value of caveolin-1 in Wilms tumor. Material and Methods: Immunhistochemical caveolin-1 expression was studied in a total of 50 formalin-fixed, paraffin-embedded Wilms tumor specimens and its association with different clinicopathologic parameters was evaluated. Results: Caveolin-1 expression was positive in 24 tumors (48%), while was absent in 26 tumors (52%). Using Mann Whitney U Analysis, Caveolin-1 expression was found to be associated with the weight of the kidney (p=0.013). In addition; there was statistical significance with Caveolin-1 expression and therapy response (p=0.049). But there were no statistical relationships between with Caveolin-1 and some clinical prognosis factors such as stage (p=0.093) and survive (p=0.256). Conclusion: In the present study, it was shown that the presence of caveolin-1 expression in Wilms tumor associated with both tumor size and therapy response. These findings were thought that Caveolin-1 expression may play a role in Wilms tumor evolution.

Ksantogranülomatöz kolesistit, safra kesesinde irregüler duvar kalınlaşması ile karakterize kronik inflamatuvar bir hastalıktır. Genellikle safra kesesi taşları ile birliktelik göstermektedir. Ksantogranulomatöz kolesistit olgularında oldukça nadir rastlanmaktadır. En sık 6. ve 7. dekartta saptanmakla birlikte kadınlarda erkeklere nazaran daha sık görülmektedir. Histopatolojik incelemelerde safra kesesi duvarında ilerleyici fibrozis ile karakterize olan ksantogranulomatöz kolesistit sıklıkla görüntüleme yöntemlerinde safra kesesi malignitesini taklit etmektedir. Bu makalede safra kesesi kanserini taklit eden ksantogranülomatöz kolesistitli bir olguyu sunmayı amaçladık.

Cholangiocarcinoma (CCA) is the second most common malignant tumor of the liver. It is simply classified as intrahepatic and extrahepatic CCA (including perihilar and distal extra hepatic CCA) according to the anatomic localization. Various classification systems were described for staging cholangiocarcinoma. We represent an interesting case of cholangiocarcinoma which is in the shadow area of classification by involving intrahepatic, hilar, and distal extra hepatic bile ducts. To our knowledge, this is the first case in the literature with diffuse bile duct involvement.

Giriş: Waardenburg-Shah Sendromu Hirschsprung Hastalığı ile Waardenburg sendromunun birlikte görüldüğü, değişken penetrasyon gösteren otozomal resesif bir hastalıktır. Bu sendromda tutulan barsak segmentinin uzunluğu değişken olmakla beraber, hastaların büyük çoğunluğunda ince barsağı da içeren total kolon aganglionozis mevcuttur. Tedavi halen tartışmalı olmakla birlikte, başlangıçta enterostomi ve bunu takiben kesin düzeltici operasyonlar uygulanmaktadır. Bu çalışmada Waardenburg sendrom olan ve ince barsağı da içeren total kolon aganglionozisi olan 2 kardeş ve birinci dereceden akraba olan 3 erkek hasta klinik özellikler ve tedavi yöntemleriyle beraber sunulmuştur. Hastalar ve Yöntem: Yaşamlarının ilk 48 saati içerisinde barsak tıkanıklığı bulgularıyla hastaneye yatırılan iki olgu elektif, biri acil şartlarda ameliyata alındı. Ganglionik segment uzunlukları sırasıyla 6, 8 ve 20 cm olarak saptandı. Bu hastalara aganglionik enterostomi (Ziegler ameliyatı) uygulandı. Postoperatif 3. haftada enterostomileri çalışmaya başladı ve tartı alımları gözlendi. Ancak yaşamlarının 5-12. aylarında sepsise bağlı olarak kaybedildiler. Tartışma: Waardenburg-Shah Sendromlu olgularda ince barsağı da içeren total kolonik aganglionozis daha sık görülmektedir. Ganglionik barsak uzunluğu yeterli olmayan olgularda Ziegler ameliyatı uygulanarak çocuğun büyümesi için zaman kazanılırken, total parenteral beslenmeyebağlı komplikasyonlar azaltılabilir., Introduction: The Waardenburg-Shah Syndrome is an autosomal recessive disease with varied penetration where Hirschsprung’s disease and the Waardenburg syndrome are seen together. Although the length of the involved intestinal segment varies in this syndrome, most patients suffer from total colonic aganglionosis with or without small bowel involvement. The treatment of this syndrome is controversial but definitive surgery is used following a preliminary enterostomy. We present 2 sibling patients and one first-degree relative of this family, for a total of 3 male patients with Waardenburg syndrome and total colonic aganglionosis with small bowel involvement, together with their clinical characteristics and treatment methods in this study .Patients and Method: The patients who presented with findings intestinal obstruction within of the first 48 hours after birth were operated, 2 patients under elective conditions and 1 as an emergency. The ganglionic segment lengths of the patients were 6, 8 and 20 cm respectively. Aganglionic enterostomy was performed (Ziegler operation) for these patients. The enterostomies started to function on the 3rd postoperative week and they started to gain weight. However, all died due to sepsis on the 5th to 12th month of life.Conclusion: Waardenburg-Shah Syndrome patients have a higher incidence of total colonic aganglionosis with or without small bowel involvement. The Ziegler operation may be used in patients with inadequate ganglionic bowel length to gain some time for the child to grow and to decrease TPN complications

Bu hastalıkta direkt immunofloresan yöntemi ile epidermal bazal membranda IgA'nın lineerdepolanması patognomoniktir. Tedavide sıkılıkla dapson kullanılır. İntravenöz immunglobulin tedavide yeni biryaklaşımdır. Oniki aylık erkek hasta özellikle ağız çevresinde olmak üzere yüzünde, kalçasında, kol vebacaklarında çok sayıda bül varlığı yakınması ile başvuru üzerine hastaneye yatırıldı. Cilt biyopsi örneğinindirekt immunofloresan yöntemiyle incelenmesinde dermoepidermal hat boyunca IgA depolandığı görüldü. Bubulgularla olguya çocukluk çağı kronik büllöz hastalığı tanısı konuldu. Olguda tedavi amacıyla intravenözimmunglobulin kullanıldı ve üç gün içinde tüm lezyonların tamamen iyileştiği belirlendi. A linear deposition of IgA at the epidermal basement membrane on direct immunoflorescence is pathognomonic for the disease. Dapson is commonly used in the therapy. Intravenous immunglobulin is a recent choice in the treatment. A twelve-months old boy was admitted to the hospital with the complaints of mutipl blisters on his face, especially perioral region, buttocks and extremities. Examination of the skin biopsy specimen with direct immunoflorescence method showed linear deposits of IgA along the dermoepidermal junction. In the light of the findings, condition was diagnosed as chronic bullous disease of childhood. Following the intravenous immunglobulin use for treatment, the skin lesions revealed rapid healing within three days.

Autosomal recessive polycystic kidney disease (ARPKD) is a severe form of inherited childhood nephropathy characterized by fusiform dilatation of collecting duct and congenital hepatic fibrosis. It has an incidence of 1: 7000- 20000. Up to 30% of cases die as neonates and progression to end stage renal disease occurs within 15 years in others. Only a few cases maintain renal function into adulthood where complications of liver disease predominate. The disease presentation is highly variable, but genetic linkage studies indicate the polycystic kidney and hepatic disease 1 (PKHD1) gene localized to chromosome 6p21. We herein present a stillborn female at 22 weeks of gestation to non-consanguineous parents. The presence of bilateral huge polycystic kidneys diagnosed with ultrasound examination and termination of pregnancy for this lethal condition was performed. In this case report, etiopathogenesis and recent genetic investigation findings of ARPKD were discussed., Otozomal resesif polikistik böbrek hastalığı (ORPBH)), böbrek toplayıcı duktuslarında fuziform genişlemeler ve konjenital hepatic fibrozis ile karakterli, ağır seyirli, kalıtımsal çocukluk çağı nefropatilerindendir. İnsidansı 1/7000- 20000 arasındadır. Olguların yaklaşık % 30'u yeni doğan döneminde kaybedilir. Genellikle yaşamın ilk 15 yılında son-dönem böbrek hastalığı gelişir. Az sayıda olgu, renal fonksiyonları bozulmaksızın erişkin yaşa ulaşır. Bunlarda karaciğer hastalığı bulguları ön plandadır. Hastalık farklı tablolarla ortaya çıkmakla birlikte; genetik çalışmalar sorumlu genin 6p21 kromozomunda lokalize polikistik böbrek ve hepatic hastalık 1 (PKHD 1) olduğunu işaret etmektedir. Burada akraba evliliği olmayan sağlıklı ana babadan olma 22 haftalık dişi fetus sunulmuştur. Ultrason takibinde çok büyük bilateral polikistik böbrek saptanması üzerine gebelik sonlandırılmıştır. Sunulan olgu eşliğinde kalıtımsal polikistik böbrek hastalığının etyo- patogenezi ve genetik çalışmalara ilişkin son literatür bulguları irdelenmiştir.

A case of four-month – old girl diagnosed as chest wall hamartoma is presented. This entity is an extremely rare but characteristic lesion of the ribs usually presenting in the neonate or infant with a mass or respiratory symptoms. Complete sponraneous regression of the lesion has been reported. Recently conservative management of asymptomatic childiren was recommended. Although rare, this condition ought to be kept in mind while dealing with infantile chest wall masses to avoid an erroneous diagnosis of malignancy. It has to be distinguished from benign and malignant neoplasms on the basis of distinct histological features discussed here., Göğüs duvarı hamartomu saptanan 4 aylık kız bebek sunulmuştur. Yenidoğan ve infantlarda oldukça nadir görülen bu karakteristik lezyon ; salt kitle varlığıyla tanınabileceği gibi basıya bağlı respiratuvar semptomlara yol açabilir. Spontan regresyon bildirmiştir. Günümüzde asemptomatik olgularda konservatif yaklaşım yeğlenmektedir. Ender görülmesine karşın klinik olarak malignite kuşkusu doğurabileceği için ; çocukluk çağı toraks tümörlerinin ayırıcı tanısında akılda tutulmalıdır. Burada lezyonu diğer malign ve benign neoplazmlardan ayıran histopatolojik özellikler irdelenmiştir.

Hidrosefali nedeniyle yerleştirilen santiarın enfekte olması sonucu gelişen glomerülonefrit şant nefriti olarak bilinmektedir. Glomerüler hastalıkla ilişkili myelofibrozis nadiren tanımlanmış olup, bu hastalıkların patogenezinde immun komplekslerin rolü üzerinde durulmuştur. Bu yazıda enfekte ventrikülo-atriyal şant komplikasyonu olarak gelişen enfektif endokardit, glomerülonefrit ve myelofibrozis birlikteliği olan olgu sunulmuş ve bu birlikteliğin patogenezi tartışılmıştır., Shunt nephritis is known as glomerulonephritis developing as a result of the infection of the shunts placed for hydrocephalus. Glomerular disease associated with myelofibrosis is rarely described, and immune complexes are thought to be responsible in the pathogenesis of both disorders. in this article, a case with coexistent infective endocarditis, glomerulonephritis and myelofibrosis developing as a complication of an infected ventriculoatrial shunt presented and the pathogenesis of this association is discussed.

This report concerns the finding of a clinically malignant mass in the subcutaneous tissue of the left supraclavicular region of a 6-years-old boy. Microscopic examination proved jt to be an unusual lesion recentfy entithd "catctfying fibrous pseudotumor (CFP)". İt is a rare benign tumour characterised by the presence of abundant hyalinized collagen with psammomatous or dystrophic calcification and scarce lymphoplasmacytic infiltrate. Total excision was performed. This procedure is thought to be necessary to preclude local recurrence., Bu olgu sunumu 6 yaşında bir erkek çocuğunda sol supraklaviküler bölgede subkutan dokuda saptanan klinik olarak malign kitleyi içermektedir. Mikroskobik inceleme son zamanlarda kalsifiye fibröz psödotümör olarak tanımlanan nadir bir lezyonu ortaya koydu. Çok fazla miktarda hyalinize kollagen, psammamatöz ya da distrofik kalsifikasyon ve seyrek lenfoplazmositik infiltrasyonla karakterize nadir benign bir tümördür. Total eksizyon uygulanmıştır. Bu girişim lokal rekürrensi önlemek için gerekli görülmüştür.

Griscelli Hastalığı otozomal resesif geçişli nadir bir hastalık olup, açık ten rengi, gümüş grisi saçlar, tekrarlayan ateş atakları ve enfeksiyonlarla karakterizedir. Sendromda ayrıca değişik derecelerde immun yetersizlik ve geniş bir spektrum içerisinde nörolojik bozukluklar görülebilir. Bu yazıda ateş yüksekliği ve öksürük yakınmalarıyla başvuran 3.5 aylık bir kız olgu sunulmuştur. Öyküsünden, iki ay önce idrar yolu enfeksiyonu nedeniyle hastanede yatırıldığı, 3. dereceden akraba bir anne-babanın, 2. çocukları olup, ilk çocuklarının da benzer fizik özelliklere sahip olduğu, 5 aylıkken ateş nedeniyle yatırıldığı hastanede eksitus olduğu öğrenildi. Fizik bakısında; gelişimi normal, ateş 37.8 C(aksiller), cildinin açık renkli, saçlarının gümüş grisi renkte, karaciğer ve dalağın midklavikuler hatta 3'er cm palpabl olması dışında, diğer sistem bakıları olağan olarak değerlendirildi. Rutin laboratuvar incelemelerinde, lökopeni dışında patoloji saptanmadı. Kemik iliği aspirasyonu normal, lökopeni sepsise sekonder olarak değerlendirildi. Serum immunoglobulinleri normal, lenfosit panelinde; CD19: %11 (ND:31-48) olarak düşük bulundu. Soygeçmişinde kardeş öyküsü olması ve hastanın fenotipik özellikleri ile Griscelli Hastalığı düşünüldü. Ýkili antibiyotik tedavisinin 4. gününde ateş yüksekliğine hakim olundu ve pansitopenisinde de düzelme saptandı.Tanı saç telinin ışık mikroskobik incelemesi ve kliniğe dayalı olarak kondu ve hasta kemik iliği transplantasyonu yapılan bir merkeze sevk edildi. Tekrarlayan enfeksiyon etyolojisinde, belirgin fenotipik özellikler gö steren olgularda Griscelli Sendromu da düşünülmelidir. Griscelli syndrome is an uncommon autosomal recessive disorder characterized by relatively light skin colour, hair with a silvery gray sheen, recurrent episodes of fever with or without a detectable infection. Syndrome most often includes variable immunodeficiency and a broad spectrum of neurologic disorders. In this article we presented a 3.5 months old girl who presented with fever and cough. Her history revealed that she had been hospitalised two months ago with a urinary tract infection, having been the second child of third degree related parents, whose fırst child with similar physical characteristics had died at age of 5 months after hospitalization due to high fever. Our patent's clinical findings consisted of normal development, low grade fever (37,8 oC, axillar), light skin color, silvery gray sheen to the hair and progrossive hepatosplenomegaly. The laboratory analysis showed leucopenia considered to be secondary to the sepsis. Bone marrow aspiration was normal. Immunologic study was also normal, except for the lymphocyte panel in which CD 19 : 11% (Normal range: 31-48) was low. Family history and fenotypical features suggested Griscelli syndrome. In fourth day of treatment with antibiotics, the fever was under control and the pancytopenia had improved. The syndrome was confirmed by pathognomonic light microscopic features in fair. The patient was referred for bone marrow trasplantation. Griscelli syndrome should be considered when dealing with reccurent infections in patients with typical fenotypical features.

The sex-cord stromal tumors of the ovary comprise 5% of ali ovarian cancers. Some endocrine changes lıke puberty precocs, amenorrhea, vırilisation may associate. A 22 month old female with juvenile granulosa celi tumor of ovary is presented. She had puberty precocs and abdominal mass. A suprapubic painless mass (12x11x9 cm) was detected during clinical examination and abdominal ultrasonography. The mass was mobile and encapsulated with well defined round borders. Right salphingooopherectomy was performed. Macroscopically the mass was solid. lobular, white-qray in color and encapsulated. On microscopic examination, the neoplastic granulosa cells were arranged in nodules, clusters or cords. Foci of edema and microcystic changes were observed. The Kİ-67 activity was found to be low and P53 negative. The case was stage la according to FİGO staging system. The endocrine changes regressed during follow-up. The patient is alive and healty during 8 months follow-up., Överin seks-kord stromal tümörleri tüm över kanserlerinin yaklaşık %5'ini oluşturmaktadır. Hastalıkta puberte prekoks, amenore, virilizan semptomlar gibi hormonal değişiklikler görülür. Bu çalışmada puberte prekoks nedeniyle 22 aylık kız çocuğunda saptanan juvenil granüloza hücreli tümör nadir görülmesi nedeniyle klinikopatolojik özellikleri ile tartışılmaktadır. Olgu 15 gündür karında şişlik yakınması ile başvurdu. Genel muayene ve ultrasonografik incelemelerinde, umblikus altında 12x11x9 cm boyutlarda, mobil. düzgün yüzeyli, kapsüllü, nonhomojen ağrısız kitle saptandı. Olgunun pubik kıllanma dışında diğer sistem muayeneleri olağandı. Sağ salfingooferektomi uygulanan hastanın ameliyat materyali incelemesinde, kitlenin lobüler, solid gri krem renkli, sağlam kapsüllü olduğu gözlendi. Mikroskobik incelemede neoplastik, granüloza hücrelerinin nodüller ya da kordonlar tarzında solid alanlar oluşturduğu, yer yer ödem ve mikrokistler içerdiği izlendi. Nükleer proliferasyon göstergesi olan Kİ67 aktivitesi düşük ve p53 negatif bulundu. FİGO evrelendirmesine göre la olarak değerlendirilen olgu izleme alındı. Operasyon sonrası endokrin bulgularında düzelme görüldü. 8 aylık izlemde olgu sağ ve sağlıklıdır.

Wilms' tumor is the most common malignant neoplasm of kidney in childhood. The patients enjoy a better prognosis by new advances in medicine. Histologically, the classic Wilms' tumor has a triphasic appearance, composed of variable mixtures of blastemal, epithelial and stromal elements. Stromal or epithelial elements may show different levels of differantiation. The aim of this study is to evaluate the role of histological components and the presence of differentiation on survival in pediatric renalWilms' tumors. This study includes 30 cases of renal Wilms' tumor diagnosed and followed at Dr. Behçet Uz Children's State Hospital between 1989 and 1997. The mean age of patients was 51 months. Thirteen cases were male and 17 cases were female. Poor prognosis (exitus) was observed in 8 cases. The 4 year survival rate was %73,3. One case was monomorphic (blastemal) while 7 cases were biphasic and 22 cases triphasic. The stromal component was dominant in 14 cases, blastemal component in 9 cases and epithelial component in 7 cases. Striated muscle was observed in 8 cases, cartilage in 1 case and squamous tissue in 1 case. Statistical analysis showed no relationship between survival rate, histologic components and occurance of differantiation in this series. We conclude that studying the known and new prognostic factors in Wilms' tumor will be useful in the assessment of patients.

Although malignant melanoma is the most frequently seen type of skin cancer in children, its frequency among solid tumours is quite rare. Congenital giant melanocytic nevus is a risk factor for malignant melanoma and it is well known that neurogenic sarcomas, liposarcomas, rhabdomyosarcomas, undifferentiated small round and pinnece cell tumours can also be formed on this nevus. In this report, a 3 year old patient with a primary diagnosis of round cell sarcoma who later diagnosed as malignant melanoma is presented.

Objective: Neurological injury due to transient cerebral ischemia is a potential complication of cardiovascular surgery. The neuroprotective effect of magnesium, when given subcutaneously before the ischemia, was assessed in a rat model of transient global cerebral ischemia. Methods: Thirty-six male Wistar albino rats were included to this randomized, controlled, prospective study. In 24 animals, ischemia was induced with four-vessel occlusion technique with the duration of 15 min. MgSO4 was given 600 mg/kg subcutaneously 48 h before the procedure in group 1 (n = 12). Similar volume of saline solution was used in animals of control group (group 2, n = 12). The animals in group 3 (sham group, n = 12) were anesthetized and subjected to operative dissections without vascular occlusion. Physiological parameters and somatosensory evoked-potentials (SEP) were monitored in animals before ischemia, during ischemia and in the first 30 min of reperfusion. Their neurological outcome had been clinically evaluated and scored up to 4 days postischemia. The intergroup differences were compared. Then the animals were sacrificed and their brains were processed for histopathological examination. Results: In group 3, SEP amplitudes did not change during the procedures, and all animals recovered without neurologic deficits. At the end of ischemic period, the average amplitude was reduced to 5 ± 3% of the baseline in all ischemic animals. This was followed by a gradual return to 87 ± 10% and 83 ± 8% of the initial amplitude after 30 min of reperfusion in group 1 and group 2, respectively (P > 0.05). The average neurological score was significantly higher in group 1 than in group 2 at 48, 72 and 96 h after the ischemic insult (P < 0.05). Histological observations were clearly correlated with the neurological findings. Conclusion: The results suggest that subcutaneous MgSO4 reduces cerebral injury and preserves neurologic function when given two days before the transient global ischemia in rats.
