Este proyecto llamado Monitoreo y aseguramiento de la calidad del agua potable en el sector hidraulico BNA 03 de EMCALI, ubicado sobre la Carrera 100 entre Calles 12 y 16, permitió proponer alternativas de mitigación a los riesgos fisicoquímicos y microbiológicos asociados a los cambios en la red de distribución de agua, enfocadas en el Sector Hidráulico (SH) BNA 03. Las plantas de tratamiento de agua potable (PTAP) de Puerto Mallarino y Río cauca, inyectan a la red de abastecimiento de la ciudad de Santiago de Cali el vital líquido el cual viaja a través de redes matrices surtiendo diferentes Sectores Hidráulicos a lo largo y ancho de la ciudad hasta llegar a la estación reguladora de presión (ERP) del SH BNA 03 ubicada en la intercepción de la carrera 100 con calle 13. En su recorrido, el agua puede verse afectada por las condiciones de las tuberías y por efectos hidráulicos en el sistema. Se recopiló, procesó y analizó la información suministrada por EMCALI referente a los parámetros hidráulicos y fisicoquímicos como: pH, turbiedad, Cloro residual, temperatura, presión de entrada, presión de salida, presión en el punto crítico (PPC) y caudal permitiendo realizar un análisis estadístico descriptivo de los parámetros anteriormente mencionados y a partir de estos concluir sobre la calidad del agua potable que se brinda a los usuarios del SH BNA 03. La identificación de riesgos en las redes de distribución de agua permite la toma de decisiones oportunas, como la implementación de acciones acertadas y tempranas para reducir la afectación de la salud y asegurar la calidad en el servicio, siendo así, se hace evidente la necesidad de implementar alarmas que alerten el daño de cualquier sensor y un programa de mantenimiento intensivo que asegure la solución a la problemática encontrada en este proyecto Pasantía institucional (Ingeniero Ambiental)-- Universidad Autónoma de Occidente, 2022 Pregrado Ingeniero(a) Ambiental

7. Rapid Detection of Carbapenemase and Extended-Spectrum β-Lactamase Producing Gram-Negative Bacteria Directly from Positive Blood Cultures Using a Novel Protocol.

Background: Early and adequate antibiotic treatment is the cornerstone of improving clinical outcomes in patients with bloodstream infections (BSI). Delays in appropriate antimicrobial therapy have catastrophic consequences for patients with BSI. Microbiological characterization of multi-drug resistant pathogens (MDRP) allows clinicians to provide appropriate treatments. Current microbiologic techniques may take up to 96 h to identify causative pathogens and their resistant patterns. Therefore, there is an important need to develop rapid diagnostic strategies for MDRP. We tested a modified protocol to detect carbapenemase and extended-spectrum β-lactamase (ESBL) producing Gram-negative bacteria (GNB) from positive blood cultures. Methods: This is a prospective cohort study of consecutive patients with bacteremia. We developed a modified protocol using the HB&L
® system to detect MDRP. The operational characteristics were analyzed for each test (HB&L-ESBL/AmpC® and HB&L-Carbapenemase® kits). The kappa coefficient, sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), likelihood ratios (LR) with 95% confidence intervals (CI), and reduction in identification time of this novel method were calculated. Results: Ninety-six patients with BSI were included in the study. A total of 161 positive blood cultures were analyzed. Escherichia coli (50%, 81/161) was the most frequently identified pathogen, followed by Klebsiella pneumoniae (15%, 24/161) and Pseudomonas aeruginosa (8%, 13/161). Thirty-three percent of isolations had usual resistance patterns. However, 34/161 (21%) of identified pathogens were producers of carbapenemases and 21/161 (13%) of extended-spectrum β-lactamases. Concordance between our HB&L® modified protocol and the traditional method was 99% (159/161). Finally, identification times were significantly shorter using our HB&L® -modified protocol than traditional methods: median (IQR) 19 h (18, 22) vs. 61 h (60, 64), p < 0.001. Conclusions: Here, we provide novel evidence that using our HB&L® -modified protocol is an effective strategy to reduce the time to detect MDRP producers of carbapenemases or extended-spectrum β-lactamases, with an excellent concordance rate when compared to the gold standard. Further studies are needed to confirm these findings and to determine whether this method may improve clinical outcomes. [ABSTRACT FROM AUTHOR]- Published

Electro-osmotic consolidation is a ground improvement technique that has been successfully implemented on the field and at laboratory scale since the 30s. However, its application has been done primarily through empirical procedures and experience. The determinant variables in the effectiveness and efficiency of the improvement are numerous and require further studies. This paper presents the evaluation of the improvement process of an inorganic low plasticity clay subjected to a 150 V/m voltage during 5780 minutes. The special analysis of the strains showed differential settlements, as well as cracking at the surface and with depth. The consolidations curves obtained follow the same tendency associated with conventional consolidation. The highest strains occurred at the side of the anode, due to the induction of flow from this terminal. The volume of water expelled during the consolidation process corresponds to a 25.2% of the initial water content, expelled mainly towards the negative terminal. The water content, pH and specific gravity, increased in the surroundings of the cathode and reduce around the anode; while the opposite behavior was observed for the plasticity index.

Electro-osmotic consolidation is a ground improvement technique in which a DC voltage is applied to the soil via electrodes, in order to drain the water contained in the pores increasing the effective stress, and thus improving the geotechnical properties of the soil. The technique increases the shear strength, reduces the compressibility, and changes the chemical composition of the saturated soft clayey and silty soils to which it can be applied. Electro-osmotic consolidation has been successfully applied in different projects worldwide. This paper presents the development of a new electro-osmotic consolidation apparatus built at Universidad de Antioquia. Tests were carried on a reconstituted clay sample and the results show effective soil improvement, measured as changes in the geotechnical properties of the clay.

An evaluation of the spatial correlation length of two geological formations known as mudflows and residual soils has been conducted using standard penetration test (N
1 )60 values. The spatial correlation length is an important property in reliability-based design, in addition to the mean and standard deviation of soil parameters, but it is rarely estimated in geotechnical projects. Reported results in this paper from both geologies show the geological origin's importance in the spatial variability analysis. Residual soils are more likely to display isotropic spatial correlation lengths horizontally when compared with the mudflows. The results show that the random field represents more accurately the mudflows' soil variability when the residual soils must be complemented with shear strength tendency analyses. La necesidad de construir pavimentos con mejores especificaciones técnicas y mayores períodos de servicio requiere de una modelación rigurosa de la variabilidad en las propiedades geotécnicas con el fin de garantizar la confiabilidad de estas estructuras. Por lo tanto, se realizó un análisis basado en la confiabilidad del método Aashto-93, considerando la variabilidad geotécnica del suelo, asociada al módulo resiliente de la subrasante. Los diseños por confiabilidad fueron efectuados de acuerdo con las categorías, parámetros y estructuras establecidas por el Manual de diseño de pavimentos asfálticos en vías con medios y altos volúmenes de tránsito, de 1998, del Instituto Nacional de Vías (–Invias–, Colombia) en el Valle de Aburrá. La variabilidad del módulo resiliente fue evaluada por medio de las funciones de densidad de probabilidad tipo normal y log-normal, según los rangos definidos por el manual de diseño, donde el método de Monte Carlo fue utilizado con el fin de obtener las funciones de densidad de falla de las estructuras seleccionadas. Se realizaron un total de veinticuatro modelaciones teniendo en cuenta las condiciones de precipitación y temperatura del 98,14 % del territorio del Valle de Aburrá. De estos análisis se obtuvo un total de noventa y seis funciones de densidad de falla de pavimentos para cada una de las funciones del módulo resiliente establecidas (normal y log-normal). Los resultados permitieron obtener un rango de coeficientes de variación del módulo resiliente entre el 21 y el 43 % para garantizar una confiabilidad del 90 %, donde la función log normal presenta el rango más alto en comparación con la función normal. El rango del coeficiente de variación obtenido fue ampliamente menor que los valores geotécnicos reportados en la bibliografía nacional e internacional, lo que compromete la vida útil de las estructuras de pavimento analizadas, si no es considerada la variabilidad inherente del módulo de la subrasante.

Resumen La miocarditis es una enfermedad inflamatoria del miocardio. Las infecciones virales son la causa más común, aunque también puede deberse a reacciones de hipersensibilidad y de etiología autoinmunitaria, entre otras. El espectro clínico de la enfermedad es variado y comprende desde un curso asintomático, seguido de dolor torácico, arritmias y falla cardiaca aguda, hasta un cuadro fulminante. El término ‘fulminante’ se refiere al desarrollo de un shock cardiogénico con necesidad de soporte vasopresor e inotrópico o dispositivos de asistencia circulatoria, ya sea oxigenación por membrana extracorpórea o balón de contrapulsación intraaórtico. Cerca del 10 % de los casos de falla cardiaca por miocarditis corresponde a miocarditis fulminante. La miocarditis por influenza se considera una condición infrecuente; no obstante, su incidencia ha aumentado desde el 2009 a raíz de la pandemia de influenza por el virus AH1N1. Por su parte, la miocarditis por influenza de tipo B sigue siendo una condición infrecuente. Se describen aquí dos casos confirmados de miocarditis fulminante por el virus de la influenza B atendidos en un centro cardiovascular, que requirieron dispositivos de asistencia circulatoria mecánica. Abstract Myocarditis is an inflammatory disease of the myocardium. Viral infections are the most common cause, although it can also be due to hypersensitivity reactions and autoimmune etiology, among other causes. The clinical spectrum of the disease is varied, from an asymptomatic course, followed by chest pain, arrhythmias, and acute heart failure, to a fulminant episode. The term fulminant refers to the development of cardiogenic shock with a need for vasopressor support and inotropic or assisted circulation devices either extracorporeal membrane oxygenation (ECMO) or intra-aortic counterpulsation balloon. About 10% of cases of heart failure due to myocarditis correspond to fulminant myocarditis. Influenza myocarditis has been considered an infrequent condition. However, its incidence has increased since 2009 as a result of the AH1N1 pandemic; otherwise, myocarditis due to the Influenza type B virus remains an infrequent entity. We describe the experience in a cardiovascular center of two confirmed cases of fulminant myocarditis due to influenza B that required circulatory assistance devices.

Tesis (Pedagogía en Inglés) The present study intended to corroborate the hypothesis that both technological and traditional games can help school students in their learning process and that traditional games unlike technological ones, also help in the creation of a better classroom climate. Through a quasi-experiment in three different educational settings, control groups and experimental ones were going to be measured both in students’ performance expressed in marks and in classroom climate measured with a sociometric test. Chile’s current national contingency has prevented this study to be concluded as expected. As an alternative outcome, the researchers carried out a perception study that compared student’s acceptance of both technological and traditional games. The result of this study shows that current students, who are digital natives, showed a slight preference for traditional games over the technological ones, defying the common belief that students only react positively to technology. El presente estudio pretende corroborar la hipótesis de que tanto los juegos tecnológicos como los tradicionales pueden ayudar a los alumnos en su proceso de aprendizaje y que los juegos tradicionales a diferencia de los tecnológicos pueden ayudar, además en la creación de un mejor clima de aula. A través de un cuasi-experimento en tres diferentes establecimientos educacionales, en los grupo de control y experimentales se iba a medir tanto el rendimiento de los estudiantes expresado a través de sus calificaciones, como el clima de aula medido a través de un cuestionario sociométrico. La actual contingencia nacional de Chile ha impedido que este estudio concluya según lo previsto. Como alternativa, los investigadores llevaron a cabo un estudio de percepción que comparó la aceptación de los estudiantes hacia los juegos tecnológicos y tradicionales. El resultado de este estudio muestra que los estudiantes, siendo nativos digitales. mostraron una ligera preferencia por los juegos tradicionales por sobre los tecnológicos, desafiando la creencia general de que los estudiantes solo reaccionan positivamente ante la tecnología.

20. Influence of Limit Equilibrium Methods in the Design of Contiguous Cantilever Sheet Pile Walls Through Reliability-Based Analysis

In geotechnical engineering the design of bored- pile walls is commonly performed in terms of the embedment length (D) defined according to the limit equilibrium methods (also known as balance methods) formulated by Krey, Blum, Rowe, Hansen and the empirical method of Look in sandy soils. The limit equilibrium methods are calculations associated with the shape of the earth pressures distributions in the shaft, earth pressures theories, geotechnical properties and safety factors, while the empirical criteria establish that the relationship between embedment and the free height (H) is of the order of D/H = 1.5. These design methods are complemented by reliability-based analyzes to evaluate the uncertainties associated with soil variability and the use of deterministic concepts. The Monte Carlo method is applied to evaluate the inherent variability of soil friction angle and the soil-wall interface angle in the calculation of the stability of adjacent bored-pile walls. The results show that Hansen’s method requires a lower D/H ratio to obtain the failure probabilities of the USACE (PF = 0.001) in comparison with the other methods. The above is because the method considers that the passive earth pressure increases linearly from the excavation level to the bottom of the embedment depth, however, the other methods considers an earth pressure balance around the pivot point in the shaft. The results show that for a probability of failure established at 0.001, the obtained D/H by the Krey, Blum, Rowe and Hansen methods are 2.13, 1.97, 1.97 and 1.93, respectively. These values represent differences between 28.7% and 42.0% with respect to the deterministic design criteria of Look. Thus, the Look criterion, from the probabilistic point of view, can be considered as an inadmissible risk method, especially since it does not consider surcharge and water table.

23. Modelling Tree Contribution in Susceptibility Analysis of Shallow Llandslides

In this paper an assessment of shallow landslides susceptibility in a tropical and mountainous terrain is made. A method that allows to model slope stability over large areas is used. Tree contribution is quantified by means of three parameters: rainfall interception, root reinforcement and tree surcharge. A rainfall interception model is used to determine the rain available for infiltration and its temporal distribution during the simulation. The hydrological models included in TRIGRS are described, which allows to determine the pore water pressure depending on the initial conditions. This pore water pressure is used in the revised infinite slope stability model, along with root reinforcement and tree surcharge, in order to determine the slope stability in terms of a factor of safety (FS) for an entire basin. Different scenarios of tree density are modeled in a basin of the Valle de Aburra and the results are compared with a model performed without considering the effects of trees on the stability. The results obtained showed values of FS < 1 (predicting failure with the occurrence of the simulated rainfall event) in the delimited area due to the null presence of trees. On the 26th October 2016, months after the investigation was concluded and the paper submitted to Revista EIA, a shallow landslide occurred in the studied area (municipality of Copacabana, landslide on the Medellín-Bogotá Highway K12+200, in the El Cabuyal zone), indicating that the proposed methodology can be a useful tool for the prediction of shallow landslides.

According to the variability of the local geology of the Aburrá Valley, two geological strata with similar grain size distribution were analyzed. The geologies are composed of mudflows and residual soils from the San Diego stock in the Poblado neighborhood. These soils show a similar mechanical behavior in relation to the number of blows of the standard penetration test SPT. The analysis was carried out by reliability methods for the determination of the active earth pressure for the geotechnical design of a cantilever walls. The analysis consists in the variation of the height of the retaining wall between 1 and 10 meters and the inclinations of the filling between 0 and 25 degrees, using design parameters obtained from laboratory tests (effective cohesion and friction angle) and the results obtained from correlations of the SPT test (equivalent friction angle). These input variables were represented by their probability distribution functions, based on the results of the field tests and the direct shear tests. The results indicate that, although these geologies show similar mechanical behavior, there is a geological influence from the probabilistic point of view, since these soils have different probability functions for the design parameters, which directly affect the probability function of the active earth pressure and therefore the probability of failure for the cantilever retaining walls analyzed.

Slope stability analyzes correspond to the evaluation of the safety factors commonly performed through the limit equilibrium and finite element methods. Due to the uncertainty of the geotechnical parameters, different statistical methods have emerged such as the limit equilibrium combined with the Monte Carlo simulation method and the random finite elements, which have been widely accepted in recent years. Normally, these analyzes are not performed taking into account the geological influence on the geotechnical properties, so in this work we proceed with the comparison of several geometrical typologies of slopes for mudflows and a residual soils of the Stock San Diego. These geologies according to the standard penetration test presents similar geo-mechanical characteristics in addition to similar grain size distribution, but their statistical parameters obtained by several laboratory tests shows a noticeable difference between them. The results show that the long-term safety factors of the mudflows are more susceptible to geometric changes with respect to the residual soils. The residual soils present a better conditions of stability in the short term analysis. The statistical parameters obtain higher safety factors in the residual soils. The probabilities of failure by random finite elements are substantially greater on the residuals and, finally, that the greater deformations occur in the mudflows under 1:1 slopes and 1: 0 in the residual soils.

Slope stability is usually affected by factors that reduce soil strength and increase driving forces acting on the slope material, sometimes generating mass movements. Among these factors, vegetation has an important role related to hydrological (e.g. evapotranspiration and infiltration) and mechanical (e.g. tree surcharge and root reinforcement provided by trees) mechanisms. In this review, vegetation influences associated to slope stability are identified, describing the mechanisms involved in the occurrence of mass movements. Finally, the way these factors affect slope stability is explained according to soil properties, climatic conditions and environmental characteristics.

The present study evaluates the shallow landslides susceptibility triggered by rainfall by means of multiple numerical simulations. The simulations integrate a transient rainfall infiltration model with an infinite slope stability model, both of them developed for unsaturated soils in a basin scale. TRIGRS’ infiltration model is used to calculate pore water pressure. The geotechnical model includes the evaluation of two parameters related to vegetation: root reinforcement and tree surcharge due to the weight of the vegetation. These simulations were carried out using multiple values of these parameters so that their behavior is analyzed for the topographic conditions of the watershed located in Valle de Aburrá. The results indicate that root reinforcement benefits slope stability, while tree surcharge represents an adverse effect for the simulated soil and topographic conditions.

El siguiente documento analiza el período en el cual se incrementó sustancialmente el uso de energía eólica en el mundo, así como los países que más crecieron en su capacidad instalada en ese período, tanto en el mundo como en Latinoamérica, para investigar las políticas e instrumentos utilizados por sus gobiernos para alcanzar tal crecimiento, encontrándose como instrumentos predominantes la aplicación de tarifas garantizadas (Feed In Tariffs –FITs-), subastas y aplicación de incentivos fiscales, apoyados por un marco regulatorio específico y metas de incorporación de energías renovables ambiciosas. Asimismo, se hace una descripción del estado actual de la generación de energía eólica en Colombia, los instrumentos usados, los cuales son en su mayoría los aportados por la Ley 1715 de 2014, que incluye incentivos fiscales, creación de un fondo de financiación para proyectos de energías renovables y se trazan lineamientos para la autogeneración y la venta de excedentes en la red. Finalmente se analizan los instrumentos de promoción internacionales que se pueden utilizar en Colombia para incrementar la penetración de la energía eólica, dentro de los cuales se encuentra la implementación de subastas con tarifas tope, tal como sucede en Brasil y creación de asociaciones entre el desarrollador y el Estado, para financiar y construir las líneas de conexión Cuestecitas-Copey 500 kV y el doble enlace Cuestecitas Colectora, necesarias para transportar la energía de proyectos eólicos localizados en la Guajira con una capacidad instalada de al menos 1.200 MW. Abstract: The following document analyzes the period during which the use of wind energy in the world substantially increased, and the countries that grew the most their installed capacity during such period, globally and in Latin America, allowing the analysis of the policies and mechanisms used by each government to reach the aforementioned growth, finding as the predominant mechanisms the set up of Feed -in-Tariffs (FITs), auctions and fiscal incentives, supported by a specific regulatory framework and ambitious targets for the incorporation of renewable energy sources.Furthermore, an overview and description of the current status of wind power generation in Colombia is presented, and the mechanisms implemented, mostly those endorsed by Ley 1715, 2014, which include fiscal incentives, the creation of a financing fund for renewable energy rojects and draws up guidelines for self-generation and the sale of surpluses to the electric system.In conclusion, an analysis of international mechanisms to promote renewable energy generation that can be used in Colombia to increase the diffusion of wind energy is found in this document, among which we can find the implementation of auctions with tariffs (Brazil’s scenario) and the integration of partnerships between the developer and the government, to finance and build the cuestecitas-Copey 500 kV connection lines and the double link Cuestecitas-Colectora required to transport energy from wind projects located in Guajira with an installed capacity of at least 1.200 MW Maestría

La caracterización de las propiedades índices de los geomateriales es una de las labores más importantes en la ingeniería geotécnica, al ser estos un insumo fundamental para la identificación y determinación de propiedades geomecánicas de los suelos. Las principales propiedades índices estudiadas en la geotecnia son el contenido de agua, los límites de Atterberg, la granulometría, el peso unitario y la gravedad específica, las cuales son la base para clasificación de suelos. En este artículo se presenta el análisis estadístico de las propiedades índices de suelos tipo flujo de lodos de la ladera oriental de Medellín, a partir del análisis de 10 proyectos de acceso público y con información suficiente para los análisis, los cuales cubren un área aproximada de 3 km2. Los resultados evidencian bajos coeficientes de variación de las propiedades índices de los flujos de lodos, especialmente para la densidad húmeda y granulometría, porcentajes de saturación altos y, según los límites de Atterberg, estos suelos se encuentran primordialmente en estado plástico.

La intervención antrópica en turberas genera cambios significativos en las propiedades geotécnicas de un material orgánico, altamente compresible y con baja resistencia al esfuerzo cortante. Existen diferentes formas de intervención, dependiendo de las tradiciones de cada lugar. Sin embargo, los procesos de intervención más comunes son la precarga por medio de terraplenes y el drenaje, las cuales generan una reducción en el contenido de humedad, la compresibilidad y la permeabilidad, así como un incremento en la resistencia al esfuerzo cortante. En el presente artículo, se discute el efecto de la intervención antrópica en las propiedades geotécnicas de las turbas, a partir de diferentes investigaciones reportadas en la literatura.

Introducción. La presencia en los hospitales de enterobacterias productoras de carbapenemasas es cada vez más frecuente. Los pacientes con cáncer en estado avanzado requieren medios invasivos para el diagnóstico, el tratamiento o los cuidados paliativos, así como el uso de antimicrobianos de amplio espectro para tratar infecciones secundarias, lo cual aumenta su propensión a las infecciones causadas por estas bacterias. Objetivo. Informar el comportamiento de Klebsiella pneumoniae productora de carbapenemasas de tipo KPC en el Instituto Nacional de Cancerología de Bogotá, entre enero de 2010 y diciembre de 2012. Materiales y métodos. Mediante el análisis de la base de datos y de los registros de los pacientes con aislamientos de K. pneumoniae productores de carbapenemasas de tipo KPC, a cargo del comité de infecciones de la institución, se identificaron y describieron las características epidemiológicas de los casos detectados. La determinación de brotes se efectuó con herramientas de control estadístico de calidad. Resultados. Entre enero de 2010 y diciembre de 2012 se identificaron 45 pacientes con aislamiento de K. pneumoniae productor de carbapenemasas de tipo KPC en alguna muestra. Hubo más aislamientos en pacientes de cáncer con tumores sólidos. La identificación se logró más frecuentemente en muestras de orina; el 17,7 % de los casos correspondió a colonización y el 82,3 %, a infección; 35,5 % (16/45) de los pacientes sobrevivió. Durante este periodo se identificaron dos brotes que se controlaron aplicando una estrategia multimodal. Conclusiones. Se encontró que la presencia de KPC fue más frecuente en infecciones que en colonizaciones. Durante estos dos años ocurrieron dos brotes que fueron controlados limitando la transmisión cruzada de bacterias multirresistentes por medio de estrategias de control convencionales. Introduction: The presence of carbapenemase-producing Enterobacteriaceae in hospitals is increasingly common. Patients with advanced cancer who require invasive means for diagnosis, treatment or palliative care, and the use of broad-spectrum antimicrobials to treat secondary infections show increased susceptibility to infections caused by these bacteria. Objective: To report the behavior of carbapenemase-producing Klebsiella pneumoniae (CPKP) isolates at the Instituto Nacional de Cancerología in Bogotá between January 2010 and December 2012. Materials and methods: By analyzing the database kept by the infection committee of the institution, as well as the records of patients with CPKC isolates, we identified and described the epidemiology of detected cases. Outbreaks were determined by using quality control statistical tools. Results: Between January 2010 and December 2012, we found 45 patients with CPKC isolates recovered from any sample. There were more isolates from patients with malignant solid tumors. CPKC isolates from urine samples were more often recovered; 17.7% of CPKC isolates corresponded to colonization, and 82.3% to infection; 35.5% of patients (16/45) survived. We identified two outbreaks during this period, which were controlled using a multimodal approach. Conclusions: This study found that CPKC presence is more frequent as infection than as colonization. During the two years of the study we detected two outbreaks, which were controlled by limiting multi-resistant bacteria cross transmission using conventional control strategies.

44 p. El presente artículo de práctica pretende realizar una crítica a la figura de la extradición en Colombia. A efectos de delimitar las posibilidades y limitaciones de la extradición en nuestro país, en un primer lugar se esbozará un recuento histórico y normativo acerca de la forma cómo ha evolucionado esta institución en el ordenamiento jurídico colombiano. En un segundo momento, se analizará la información estadística, obtenida de primera mano, que describe el comportamiento o la forma cómo se ha venido aplicando la extradición en Colombia. En un tercer instante, se detallará el estudio de tres casos puntuales, que nos permita entender la extradición en situación. Finalmente, a manera de conclusión, se enunciarán una serie de críticas a través de las cuales se hace patente cómo la extradición se ha venido implementado en Colombia, de manera expansiva, al punto tal que se han desdibujado los horizontes que vienen trazados por las garantías constitucionales y legales.

Dentro de las múltiples variables que afectan el comportamiento del consumidor se presenta la edad como una variable relevante para la segmentación de mercados, debido a que delimita las diferentes etapas de su vida desde la infancia hasta la ancianidad, trayendo consigo características físicas, biológicas, psicológicas, sociales y situacionales, entre otras; se utilizará el término de entrevista directa para conocer y utilizar el “auto concepto” como herramienta, la cual será un beneficio a la hora de descubrir patrones de comportamiento y hábitos de consumo de los adultos mayores. Por otra parte, este estudio aborda un tema de gran interés tanto para los profesionales del mercadeo, como sociólogos al permitir comprender el comportamiento de los adultos mayores ante las necesidades y expectativas que realmente el mercado espera satisfacer con determinadas marcas, con el fin de que puedan diseñar ofertas de valor agregado, especializando categorías y desarrollo de productos para atender este segmento que se encuentra con una tendencia creciente a nivel mundial, por lo que se está volviendo el foco de atención para las empresas que brindan bienes y servicios a esta fracción del mercado . This paper presents the preliminary findings of a TCD/NRA vacuum preloading field trial that is part of a research project into the construction options for improving rampart roads. Vacuum preloading is a construction method used to accelerate settlement and the increase shear strength by applying a vacuum pressure to the ground by means of prefabricated vertical drains, an impervious tight sheet and a vacuum pump. Consolidating the ground by applying a vacuum load has several advantages over other techniques, for example; no fill material is required; construction periods are generally shorter; and there is no need for chemical admixtures. This paper presents a review of the literature and describes the setup and initial stage of the TCD/NRA vacuum preloading field trial, that is currently being carried out at Ballydermot bog.

37. Estudio de alternativas viales para la intersección de la Avenida Bolivar con Calle Segunda en la ciudad de Armenia

El objetivo de este trabajo es mejorar la situación actual del tránsito que se presenta en la intersección de la Avenida Bolívar con la Calle Segunda en la ciudad de Armenia, para lo cual se realizó un estudio que abarcó desde la toma de información básica del estado actual de la intersección (volúmenes vehiculares, datos históricos del tránsito, levantamiento de la intersección) hasta la evaluación económica, ambiental y multivariable de las alternativas preseleccionadas, usando para ello gran parte de la metodología propuesta en las bibliografías del Ingeniero Germán Arboleda Vélez, junto a aportes conceptuales del Ingeniero John Jairo Posada. Entre las alternativas estudiadas se tienen dejar la intersección tal como se encuentra, mejorar el sistema semafórico actual, construir una glorieta, deprimir la Calle Segunda solamente y por último bajarle el nivel a la Avenida Bolívar y al mismo tiempo deprimir la Calle Segunda. Con los criterios citados anteriormente se evaluaron las alternativas y se seleccionó definitivamente deprimir la Calle Segunda solamente, por presentar en conjunto los mejores resultados en los temas evaluados. Con este trabajo, se considera queda una herramienta de consulta que podrá ser de mucha utilidad para una futura toma de decisiones por parte de las autoridades locales / Abstract: The goal of this work is to improve the present traffic situation that is being taking place at the intersection between Bolivar Avenue and 2nd street in the city of Armenia. To do this, a study was carried out. Said study covered a basic collection of information about the current situation of the intersection (vehicle volume, historical traffic data, survey of the intersection,) the economic and environmental evaluation and the multiple variety of pre-chosen alternatives, using a great part of the proposed methodology in the bibliography of Engineer Germán Arboleda-Vélez, along with conceptual contributions from Engineer John Jairo Posada. Among the alternatives studied, one is to leave the intersection as it is now, another one to improve the existent traffic light system, another is to construct a round point, another is to compress only 2nd street and the other one is to lower the level of Bolivar Avenue and at the same time, to compress 2nd street. Based on the criteria above mentioned, the alternatives were evaluated and the option of compressing only 2nd street was chosen because it showed a joint of better results in the topics evaluated. Tesis (Licenciado en Arquitectura) De la mano del desarrollo económico del país, el aumento del ingreso percápita, las mejoras viales e infraestructura en general, el uso intenso de los medios de comunicación, la creciente capacidad de consumo y los cambios generalizados de orden socio cultural, es que se desarrolla la arquitectura en Chile; y particularmente la arquitectura del ocio en sus costas. El problema que investiga esta tesis no es este negocio propiamente tal, sino el criterio con que nosotros, los arquitectos, nos posicionamos frente a esta nueva situación. Mientras, se desarrollan proyectos, sin tomar en cuenta escalas, morfologías ni tradición de los lugares en que se implantan; independientemente de la regulación que impongan las municipalidades -si es que existe. Es por medio de la investigación donde se contemple la totalidad de elementos que influyen en el planteamiento y desarrollo de un proyecto de segunda vivienda y estudiando en profundidad una serie de proyectos desarrollados en la V Región, que se pretende encontrar el modo más armónico y coherente en que estos proyectos se deban desarrollar en el borde costero, de modo de lograr definir una serie de recomendaciones a tener en cuenta al momento de desarrollar este tipo de proyectos. La investigación se basa principalmente en el análisis semiológico de casos específicos, como modo de estructuración del trabajo; además de contemplar entrevistas a todos los agentes que tengan relación con el desarrollo de segunda vivienda: arquitectos, directores de obra, etc.; encuestas a los habitantes de los municipios, a los dueños de segundas viviendas en los proyectos a analizar; y la consideración del problema en experiencias extranjeras. Buscar el ''modo" de adaptación al borde costero a esta "arquitectura del ocio" ... Ese es el desafío.

Background Early and adequate antibiotic treatment are the cornerstones to improve clinical outcomes in patients with Bloodstream infections (BSI). Delays in appropriate antimicrobial therapy have catastrophic consequences for patients with BSI. Microbiological characterization of multi-drug-resistant pathogens (MDRP) allow clinicians to provide appropriate treatments. Current available microbiologic techniques may take-up to 96 hours to identify causative pathogens and its resistant patterns. Therefore, there is an important need to develop rapid diagnostic strategies for MDRP. However, rapid detection techniques are costly and are not widely available. We tested a modified protocol designed to detect Gram-negative bacilli (GNB) resistant to oximinocephalosporins and carbapenems from positive blood cultures. Methods This is a prospective, cohort study of consecutive patients with bacteremia. We developed a modified protocol using HB&L® system to detect MDRP. We then attempted to determine accuracy, concordance and reduction of identification time of this novel method in a reference hospital. Descriptive statistics and logistical regressions were used. Results Ninety-six patients with BSI were included in the study. A total of 161 positive blood cultures were analyzed. Escherichia coli (50%, 81/161) was the most frequently identified pathogen followed by Klebsiella pneumoniae (15%, 24/161) and Pseudomonas aeruginosa (8%, 13/161). 32% of isolations had usual resistance patters. However, in 29/161 (18%) of identified pathogens were producer of carbapenemasases and 21/161 (13%) of extended-spectrum β-lactamases. Concordance among our HB&L® modified protocol and traditional method was 99% (159/161). Finally, identification times were significantly shorter using our HB&L® modified protocol than traditional methods (Mean, hours [SD], 20.8 [6.22] vs. 62.8 [6.22], P < 0.001). Conclusion Here we provided novel evidence that using our HB&L® modified protocol is an effective strategy to reduce the time to MDRP detection/identification; with a great concordance rate when compared with the gold standard. 41. Impacto de intervenciones educativas como parte de un programa para el uso optimizado de antimicrobianos

Objetivo: Evaluar los efectos de la implementación de estrategias educativas post prescripción, sobre la calidad de la prescripción en pacientes hospitalizados y el consumo antibiótico. Materiales y métodos: Estudio cuasi experimental, de tipo antes y después, no controlado, evaluando el impacto de intervenciones educativas, sobre las prescripciones y consumo de antibióticos, Se incluyeron pacientes mayores de 18 años, hospitalizados, recibiendo tratamiento antibiótico parenteral para infección urinaria, Resultados: Se incluyeron 231 pacientes en el estudio, 103 pacientes para el periodo antes de la intervención (febrero y marzo de 2019) 41 pacientes del mes de abril, mes durante el cual se inició la intervención y 87 para el periodo después de la intervención (mayo y junio de 2019). El consumo promedio de todos los antibióticos observados, en el periodo antes, correspondiente al área de hospitalización fue de 5.34 dosis diarias definidas (DDD) y en Unidad de Cuidado Intensivo (UCI) de 19.72 DDDs, y en el periodo después fue de 5.27 DDDs en el área no UCI y en UCI 14.33 DDDs, con un valor de p en 0.982 y 0.705 respectivamente. Durante el periodo antes en el área de hospitalización el cumplimiento de la guía para el tratamiento de infección de vías urinarias fue 58.82%, en UCI del 55.55%, y en los dos 58.25%, mientras que en el periodo del después el cumplimiento fue del 67,60% para hospitalización, 68,75% en unidad de cuidado crítico, y del 67,81% de ambos, con unos resultados de p de 0.258, 0.429 y 0.175 respectivamente, al comparar las diferencias en los porcentajes de los periodos correspondientes al antes y después. Conclusiones: Después de las intervenciones educativas aumentó el porcentaje de cumplimiento a la guía institucional, y disminuyó el consumo antibiótico, diferencias que fueron estadísticamente no significativas, lo cual puede estar en relación con el tiempo de observación y tamaño de la muestra. Methods: Quasi-experimental, before and after study, uncontrolled, evaluating the impact of educational interventions, on antibiotics prescriptions and consumption. Including patients over 18 years of age, hospitalized, receiving parenteral antibiotic treatment for urinary infection. Results: Patients were included in the study, 103 patients for the period before the intervention (February and March 2019), 41 patients for the month of April, month during which the intervention began and 87 patients for the after the intervention period (May and June 2019). The average consumption of all antibiotics observed, in the before period, corresponding to the hospitalization area was 5.34 defined daily doses (DDDs) and in Intensive Care Unit (ICU) 19.72 DDDs, and in the after period it was 5.27 DDDs in the hospitalization area (non-ICU )and 14.33 DDDs in ICU, with a p value of 0.982 and 0.705 respectively. During the before period in the hospitalization (non ICU) area, the compliance to the institutional guideline for the treatment of urinary tract infection was 58.82%, 55.55% in the ICU, and 58.25% in both areas, while in the after period the compliance was 67.60% for hospitalization area (non ICU), 68.75% in the ICU and 67.81% in both, with p values of 0.258, 0.429 and 0.175 respectively, comparing the differences in the percentages of before and after periods. Conclusions: After educational interventions, the percentage of compliance with the institutional guideline increased, and antibiotic consumption decreased, differences that were statistically non-significant, which may be related to the observation time and sample size. Tesis (Licenciado en Ciencias Jurídicas) Nunca como hoy la aglomeración ha pasado a ser un fenómeno común. El aumento del nivel de vida y la "globalización" o "mundialización" de la vida del hombre ha hecho surgir con fuerza a las masas, colocándolas en el primer plano del escenario humano. Espectáculos deportivos, culturales o artísticos han hecho que el estilo psicológico de los actuales individuos los impulse a asociarse. Hoy día la sociedad asume mas importancia que el individuo y esta característica del ser humano ha rebasado el campo sociológico, político o religioso para penetrar con fuerza en el dominio del Derecho. Notoria es la recepción que éste fenómeno tiene en disciplinas jurídicas tales como el derecho comercial y civil, siendo el exponente más evidente de lo señalado la importancia que, como modo de ser y actuar colectivos, han adquirido las sociedades anónimas. Pero no solo estas ramas del derecho han sido afectadas por el nuevo orden de cosas. El Derecho Penal no ha permanecido ajeno a esta corriente que lleva a los hombres a actuar en conjunto y así las ciencias penales se han encontrado frente a un nuevo tipo de actividad delictiva: el delito de muchedumbre. Países como España, ltaHa Argentina, Uruguay contemplan en sus respectivas legislaciones jurídico - penales disposiciones relacionadas con esta clase delitos. En nuestro país no existe un tratamiento sistemático del delito de muchedumbre y solo algunas disposiciones aisladas tipifican delitos en los que el sujeto activo es una muchedumbre, como se verá mas adelante. Sin embargo, estimamos que es indispensable abordar el tema para proponer una solución legal. Hoy día vemos con que frecuencia en las reuniones de carácter deportivo, especialmente en las que se juegan partidos de fútbol, sin olvidar los partidos de tenis internacional, suelen formarse, de manera súbdita, multitudes que delinquen. Destrucción de bienes públicos y privados, riñas,lesiones y hasta muerte es el resultado de este fenómeno. Como cuestión previa. es necesario tener presente que este trabajo se propone indagar acerca de la manera como el Derecho Penal trata el tema propuesto, en especial en orden a la responsabilidad penal y su posible tratamiento en la legislación y dogmática chilena. Sin embargo, inevitablemente será necesario abordar los aspectos psicológicos y sociológicos del mencionado delito. La verdad es que la mayoría de los trabajos relacionados con la muchedumbre se refieren a su aspecto externo como fenómeno sociológico y a su aspecto interno como fenómeno psicológico. Pero no puede quedar al margen de este estudio el tratamiento jurídico penal del problema.

We developed a modified protocol using the HB&L
® system to detect MDRP. The operational characteristics were analyzed for each test (HB&L-ESBL/AmpC® and HB&L-Carbapenemase® kits). The kappa coefficient, sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), likelihood ratios (LR) with 95% confidence intervals (CI), and reduction in identification time of this novel method were calculated., Results: Ninety-six patients with BSI were included in the study. A total of 161 positive blood cultures were analyzed. Escherichia coli (50%, 81/161) was the most frequently identified pathogen, followed by Klebsiella pneumoniae (15%, 24/161) and Pseudomonas aeruginosa (8%, 13/161). Thirty-three percent of isolations had usual resistance patterns. However, 34/161 (21%) of identified pathogens were producers of carbapenemases and 21/161 (13%) of extended-spectrum β-lactamases. Concordance between our HB&L® modified protocol and the traditional method was 99% (159/161). Finally, identification times were significantly shorter using our HB&L® -modified protocol than traditional methods: median (IQR) 19 h (18, 22) vs. 61 h (60, 64), p < 0.001., Conclusions: Here, we provide novel evidence that using our HB&L® -modified protocol is an effective strategy to reduce the time to detect MDRP producers of carbapenemases or extended-spectrum β-lactamases, with an excellent concordance rate when compared to the gold standard. Further studies are needed to confirm these findings and to determine whether this method may improve clinical outcomes.- Published

- Abstract
La miocarditis es una enfermedad inflamatoria del miocardio. Las infecciones virales son la causa más común, aunque también puede deberse a reacciones de hipersensibilidad y de etiología autoinmunitaria, entre otras. El espectro clínico de la enfermedad es variado y comprende desde un curso asintomático, seguido de dolor torácico, arritmias y falla cardiaca aguda, hasta un cuadro fulminante. El término 'fulminante' se refiere al desarrollo de un shock cardiogénico con necesidad de soporte vasopresor e inotrópico o dispositivos de asistencia circulatoria, ya sea oxigenación por membrana extracorpórea o balón de contrapulsación intraaórtico. Cerca del 10 % de los casos de falla cardiaca por miocarditis corresponde a miocarditis fulminante. La miocarditis por influenza se considera una condición infrecuente; no obstante, su incidencia ha aumentado desde el 2009 a raíz de la pandemia de influenza por el virus AH1N1. Por su parte, la miocarditis por influenza de tipo B sigue siendo una condición infrecuente. Se describen aquí dos casos confirmados de miocarditis fulminante por el virus de la influenza B atendidos en un centro cardiovascular, que requirieron dispositivos de asistencia circulatoria mecánica.

- Abstract
Introduction: The presence of carbapenemase-producing Enterobacteriaceae in hospitals is increasingly common. Patients with advanced cancer who require invasive means for diagnosis, treatment or palliative care, and the use of broad-spectrum antimicrobials to treat secondary infections show increased susceptibility to infections caused by these bacteria., Objective: To report the behavior of carbapenemase-producing Klebsiella pneumoniae (CPKP) isolates at the Instituto Nacional de Cancerología in Bogotá between January 2010 and December 2012., Materials and Methods: By analyzing the database kept by the infection committee of the institution, as well as the records of patients with CPKC isolates, we identified and described the epidemiology of detected cases. Outbreaks were determined by using quality control statistical tools., Results: Between January 2010 and December 2012, we found 45 patients with CPKC isolates recovered from any sample. There were more isolates from patients with malignant solid tumors. CPKC isolates from urine samples were more often recovered; 17.7% of CPKC isolates corresponded to colonization, and 82.3% to infection; 35.5% of patients (16/45) survived. We identified two outbreaks during this period, which were controlled using a multimodal approach., Conclusions: This study found that CPKC presence is more frequent as infection than as colonization. During the two years of the study we detected two outbreaks, which were controlled by limiting multi-resistant bacteria cross transmission using conventional control strategies.
