Los cambios en el ecosistema mediático han provocado transformaciones en los lenguajes comunicacionales, ahora en espacios imagéticos como el Instagram, incluso fortaleciendo la hipermediación proporcionada por las redes sociales. El artículo presenta una investigación métrica sobre la utilización del Instragram por la iniciativa portuguesa Aldeias Históricas en búsqueda de promoción turística de la región. Para la investigación, se ha adoptada la herramienta InstaBro, que ofreció informaciones para un análisis de carácter cuantitativo, pero también una interpretación cualitativa. Los resultados ofrecen informaciones hacia la importancia de la hipermediación en procesos de promoción del turismo rural a través de la comunicación digital. The article presents a metric research on the use of the Instragram by the Portuguese initiative Aldeias Históricas in search of tourism promotion in the region. For the investigation, the InstaBro tool has been adopted, which offered information for a quantitative analysis, but also a qualitative interpretation. Objective: The aim of this clinical trial is to examine whether it is possible to reduce postoperative complications using an individualized perioperative ventilatory strategy versus using a standard lung-protective ventilation strategy in patients scheduled for thoracic surgery requiring one-lung ventilation. Design: International, multicenter, prospective, randomized controlled clinical trial. Setting: A network of university hospitals. Participants: The study comprises 1,380 patients scheduled for thoracic surgery. Interventions: The individualized group will receive intraoperative recruitment maneuvers followed by individualized positive end-expiratory pressure (open lung approach) during the intraoperative period plus postoperative ventilatory support with high-flow nasal cannula, whereas the control group will be managed with conventional lung-protective ventilation. Measurements and Main Results: Individual and total number of postoperative complications, including atelectasis, pneumothorax, pleural effusion, pneumonia, acute lung injury; unplanned readmission and reintubation; length of stay and death in the critical care unit and in the hospital will be analyzed for both groups. The authors hypothesize that the intraoperative application of an open lung approach followed by an individual indication of high-flow nasal cannula in the postoperative period will reduce pulmonary complications and length of hospital stay in high-risk surgical patients. La enucleación de quistes óseos conlleva la aparición de defectos óseos. Se acepta que los cuando son menores de 3 cm. de diámetro y están alejados del reborde alveolar se rellenan de hueso neoformado en unos meses. Por el contrario, defectos óseos mayores no regeneran completamente. "Colloss®" parece ser un interesante material osteoinductivo. Se trata de un complejo liofilizado de las proteínas extracelulares extraídas del hueso bovino diafisario. Contiene fundamentalmente colágeno tipo I y cantidades variables de BMPs. En el período comprendido entre Febrero de 2002 y Enero de 2004 se utilizó colágeno liofilizado bovino para la reconstrucción ósea de 7 pacientes con quistes mandibulares tratados en el Hospital Universitario La Paz (Madrid). Este material se combinó con injertos óseos autólogos o con mallas de titanio en varios de los casos. Se observó en la totalidad de los casos una adecuada regeneración ósea. En dos de los casos, en los que se mezcló Colloss® con hueso autólogo se produjo el rechazo de este último. Tras sus retirada el proceso curativo discurrió sin incidencias. También se constató una tendencia a la exposición intraoral de las mallas de titanio sin que ello se viese reflejado en el proceso de curación. Podemos señalar como ventajas de la utilización del colágeno liofilizado bovino (Colloss®) para el tratamiento de quistes odontogénicos: su capacidad osteoinductiva demostrada; la ausencia de efectos adversos de importancia y su utilidad en la determinación precoz de recidivas. Como posibles desventajas destacaríamos: su difícil manejo; la conveniencia de encontrar un carrier adecuado que permitiese una manipulación más sencilla, la necesidad de ajustar la dosis necesaria de forma precisa y, por último, las complicaciones potenciales relacionadas con cualquier tipo de xenoinjerto de origen bovino. The treatment of mandible bone cysts commonly leads to the development of bone defects. When the defects are less than 3 cm and located away from the alveolar ridge, they usually heal spontaneously within a 6 to 12 month period. However, if the defect is larger, the regenerative ability of the body is overcome. Colloss® appears to be an interesting osteoinductive biomaterial. It is a lyophilized complex from extracellular matrix proteins of bovine bone. It is composed primarily of type I collagen and variable amounts of BMPs. Between February 2002 and January 2004, 7 patients with mandible cysts were treated with lyophilized collagen to enhance bone regeneration. In some of the cases this was combined with autologous bone or a titanium mesh. We observed incompatibility with autologous bone and a slight tendency for intraoral exposure of the titanium mesh. Apart from this, we noted adequate bone healing and regeneration in all the cases. For the authors of this paper, the advantages of lyophilized collagen are the following: osteoinductive capacity, absence of any major adverse effects and the role played in the early detection of recurrence. Its possible disadvantages are its difficult handling and the inherent complications related to bovine xenografts.

Las malformaciones craneofaciales son algunas de las patologías más prevalentes en la edad pediátrica. Podemos distinguir dos grandes grupos: las producidas por un cierre precoz de las suturas del esqueleto craneofacial, las craneosinostosis y faciocraneosinostosis; y las que actualmente se pueden considerar neurocristopatías, como los síndromes de primer y segundo arcos branquiales y las fisuras orofaciales como el labio leporino. Se describen las principales características de los síndromes más frecuentes y los tratamientos, desde los más empleados a los más innovadores, como las técnicas endoscópicas y las de distracción ósea.Craniofacial abnormalities are some of the most prevalent malformations in children. We can distinguish two groups: those caused by an early closure of the sutures of the craniofacial skeleton, as cranosynostosis and faciocranioynostosis; and those that are considered neural crest anomalies, like the first and second brachial arch syndromes, and cleft lift and palate. The authors discuss the major characteristics of the most frequent syndromes, as well as treatment modalities, including the most popular and the most recent ones, like endoscopic and bone distraction techniques.
