Abstract

In this paper we describe the architecture of a Platform as a Service (PaaS) oriented to computing and data analysis. In order to clarify the choices we made, we explain the features using practical examples, applied to several known usage patterns in the area of HEP computing. The proposed architecture is devised to provide researchers with a unified view of distributed computing infrastructures, focusing in facilitating seamless access. In this respect the Platform is able to profit from the most recent developments for computing and processing large amounts of data, and to exploit current storage and preservation technologies, with the appropriate mechanisms to ensure security and privacy., Comment: 31 pages, 12 Figures

First, the technical aspects are introduced, including a description of the data selection, the datasets, the diagnostics used, the experimental and theoretical methods implemented and the main definitions. Second, the JET-ILW pedestal structure and stability are described. In particular, the work describes the links between the engineering parameters (power, gas and divertor configuration) and the disagreement with the peeling-ballooning (PB) model implemented with ideal magnetohydrodynamics equations. Specifically, the work clarifies why the JET-ILW pedestal tends to be far from the PB boundary at high gas and high power, showing that a universal threshold in power and gas cannot be found but that the relative shift (the distance between the position of the pedestal density and of the pedestal temperature) plays a key role. These links are then used to achieve an empirical explanation of the behavior of the JET-ILW pedestal pressure with gas, power and divertor configuration. Third, the pedestal database is used to revise the scaling law of the pedestal stored energy. The work shows a reasonable agreement with the earlier Cordey scaling in terms of plasma current and triangularity dependence, but highlights some differences in terms of power and isotope mass dependence.

Abstract

Integrated modeling (IM) of present experiments and future tokamak reactors requires the provision of computational resources and numerical tools capable of simulating multiscale spatial phenomena as well as fast transient events and relatively slow plasma evolution within a reasonably short computational time. Recent progress in the implementation of the new computational resources for fusion applications in Europe based on modern supercomputer technologies (supercomputer MARCONI-FUSION), in the optimization and speedup of the EU fusion-related first-principle codes, and in the development of a basis for physics codes/modules integration into a centrally maintained suite of IM tools achieved within the EUROfusion Consortium is presented. Physics phenomena that can now be reasonably modelled in various areas (core turbulence and magnetic reconnection, edge and scrape-off layer physics, radio-frequency heating and current drive, magnetohydrodynamic model, reflectometry simulations) following successful code optimizations and parallelization are briefly described. Development activities in support to IM are summarized. They include support to (1) the local deployment of the IM infrastructure and access to experimental data at various host sites, (2) the management of releases for sophisticated IM workflows involving a large number of components, and (3) the performance optimization of complex IM workflows., This work has been carried out within the framework of the EUROfusion Consortium and has received funding from the Euratom research and training programme 2014 to 2018 under grant agreement 633053. Abstract

In this paper we present an approach taken to run multiple Kepler sessions at the same time. This kind of execution is one of the requirements for Integrated Tokamak Modelling platform developed by the Nuclear Fusion community within the context of EUROFusion project [2]. The platform is unique and original: it entails the development of a comprehensive and completely generic tokamak simulator including both the physics and the machine, which can be applied for any fusion device. All components are linked inside work ows. This approach allows complex coupling of various algorithms while at the same time provides consistency. Work ows are composed of Kepler and Ptolemy II elements as well as set of the native libraries written in various languages (Fortran, C, C++). In addition to that, there are Python based components that are used for visualization of results as well as for pre/post processing. At the bottom of all these components there is a database layer that may vary between software releases, and require di erent version of access libraries. The community is using shared virtual research environment to prepare and execute work ows. All these constraints make running multiple Kepler sessions really challenging. However, ability to run numerous sessions in parallel is a must - to reduce computation time and to make it possible to run released codes while working with new software at the same time. Abstract

A selection of achievements and first physics results are presented of the European Integrated Tokamak Modelling Task Force (EFDA ITM-TF) simulation framework, which aims to provide a standardized platform and an integrated modelling suite of validated numerical codes for the simulation and prediction of a complete plasma discharge of an arbitrary tokamak. The framework developed by the ITM-TF, based on a generic data structure including both simulated and experimental data, allows for the development of sophisticated integrated simulations (workflows) for physics application.The equilibrium reconstruction and linear magnetohydrodynamic (MHD) stability simulation chain was applied, in particular, to the analysis of the edgeMHDstability of ASDEX Upgrade type-I ELMy H-mode discharges and ITER hybrid scenario, demonstrating the stabilizing effect of an increased Shafranov shift on edge modes. Interpretive simulations of a JET hybrid discharge were performed with two electromagnetic turbulence codes within ITM infrastructure showing the signature of trapped-electron assisted ITG turbulence. A successful benchmark among five EC beam/ray-tracing codes was performed in the ITM framework for an ITER inductive scenario for different launching conditions from the equatorial and upper launcher, showing good agreement of the computed absorbed power and driven current. Selected achievements and scientific workflow applications targeting key modelling topics and physics problems are also presented, showing the current status of the ITM-TF modelling suite. Abstract

Considering how complex Grid technology is, it is essential to provide adequate support on users' level in order to simplify interaction and attract users. Intuitive and easy-to-use graphical user interfaces could be critical for the usefulness of the whole infrastructure and may play a significant role within successful stories. We would like to present the int.eu.grid approach to a Migrating Desktop product providing a unified and intuitive graphical work environment that allows users to control sophisticated interactive services, access Grid resources, run sequential and parallel jobs -- using both batch and interactive paradigms. The Migrating Desktop provides a front-end for embedding some of the application mechanisms and interfaces, and it allows the user to have virtual access to Grid resources. Generic API for applications based on the OSGi specification provides mechanisms for the job submission phase, interaction with an application and remote visualization of the results which assemble the powerful platform for the Grid environment.

Abstract

A selection of achievements and first physics results are presented of the European Integrated Tokamak Modelling Task Force (EFDA ITM-TF) simulation framework, which aims to provide a standardized platform and an integrated modelling suite of validated numerical codes for the simulation and prediction of a complete plasma discharge of an arbitrary tokamak. The framework developed by the ITM-TF, based on a generic data structure including both simulated and experimental data, allows for the development of sophisticated integrated simulations (workflows) for physics application.The equilibrium reconstruction and linear magnetohydrodynamic (MHD) stability simulation chain was applied, in particular, to the analysis of the edgeMHDstability of ASDEX Upgrade type-I ELMy H-mode discharges and ITER hybrid scenario, demonstrating the stabilizing effect of an increased Shafranov shift on edge modes. Interpretive simulations of a JET hybrid discharge were performed with two electromagnetic turbulence codes within ITM infrastructure showing the signature of trapped-electron assisted ITG turbulence. A successful benchmark among five EC beam/ray-tracing codes was performed in the ITM framework for an ITER inductive scenario for different launching conditions from the equatorial and upper launcher, showing good agreement of the computed absorbed power and driven current. Selected achievements and scientific workflow applications targeting key modelling topics and physics problems are also presented, showing the current status of the ITM-TF modelling suite., QC 20140505. Abstract

A case study on climate models intercomparison data analysis addressing several classes of multi-model experiments is being implemented in the context of the EU H2020 INDIGO-DataCloud project. Such experiments require the availability of large amount of data (multi-terabyte order) related to the output of several climate models simulations as well as the exploitation of scientific data management tools for large-scale data analytics. More specifically, the paper discusses in detail a use case on precipitation trend analysis in terms of requirements, architectural design solution, and infrastructural implementation. The experiment has been tested and validated on CMIP5 datasets, in the context of a large scale distributed testbed across EU and US involving three ESGF sites (LLNL, ORNL, and CMCC) and one central orchestrator site (PSNC).

40. Small cell carcinoma with neuroendocrine differentiation of subglottic larynx- a case report.

Abstract

Small cell cancer (SCC) is a neuroendocrine neoplasm, which is most frequently found in the lungs. Extrapulmonary location of SCC is rare and may involve 2.5-5% of SCCs. We present a case of a 31-year-old male patient with an extremely uncommon subglottic SCC. The patient was qualified for a radical sequential chemoradiotherapy. After treatment, patient's condition suggested complete remission. Recurrence was detected one year later, and the disease rapidly progressed, despite a second line chemotherapy. The patient died 29 months after initial diagnosis. Abstract

(1) Background: The aim of this study was to determine the frequency and the pattern of post-procedural intracranial aneurysm contrast enhancement on high-resolution vessel wall magnetic resonance imaging (HR-VW MRI). We investigated the possible association between this imaging finding and factors such as time elapsed since embolization or aneurysm occlusion grade on baseline and follow-up imaging. (2) Methods: Consecutive patients presenting for follow-up after endovascular treatment of intracranial aneurysms were included. HR-VW MRI was acquired and interpreted independently by two radiologists. (3) Results: This study included 40 aneurysms in 39 patients. Contrast enhancement was detected in 30 (75%) aneurysms. It was peripheral in 12 (30.0%), central in 9 (22.5%), and both peripheral and central in 9 (22.5%) aneurysms. The statistical analysis did not reveal any relationship between follow-up period and the presence of contrast enhancement ( p = 0.277). There were no statistically significant differences in the frequency of contrast enhancement between aneurysms with total occlusion and those with remnant flow on follow-up MR angiography ( p = 0.850) nor between aneurysms with different interval changes in the aneurysm occlusion grade ( p = 0.536). Multivariate analysis did not demonstrate aneurysm size, ruptured aneurysm status, nor initial complete aneurysm occlusion to be a predictor of contrast enhancement ( p = 0.080). (4) Conclusions: Post-procedural aneurysm contrast enhancement is a common imaging finding on HR-VW MRI. The clinical utility of this imaging finding, especially in the prediction of aneurysm recurrence, seems limited. The results of our study do not support routine use of HR-VW MRI in the follow-up of patients after endovascular treatment of intracranial aneurysms.

42. The influence of pre-hospital medication administration in ST-elevation myocardial infarction patients on left ventricular ejection fraction and intra-hospital death.

Abstract

Introduction: Currently, invasive cardiology techniques are the preferred method of treatment for patients with ST-elevation myocardial infarction (STEMI). Improving the care of patients with STEMI is possible by minimizing the time that elapses from the onset of pain to the start of treatment. As studies indicate, early pharmacotherapy, especially with antiplatelet and anticoagulant medications, allows for their early effectiveness., Aim: To assess the influence of early administration of antiplatelet (clopidogrel) and anticoagulant medications in the pre-hospital period in patients with ST-elevated myocardial infarction on the frequency of in-hospital deaths and on the left ventricular ejection fraction evaluated at hospital discharge., Material and Methods: In this study, a retrospective analysis of 573 patients hospitalized due to ST-segment elevation myocardial infarction in one of Krakow's hospitals from January 2011 to December 2015 (excluding the whole of 2013) was used as a research method., Results: As many as 97% of patients received pre-hospital pharmacotherapy, but only 46.0% of respondents received unfractionated heparin, and 19.2% received clopidogrel. The in-hospital mortality rate was 6.7%, but among patients prehospitally treated with clopidogrel and unfractionated heparin, the in-hospital mortality rate was 1.1%. Prehospital administration of clopidogrel significantly decreased the possibility of reduced left ventricular ejection fraction (OR = 0.27; 95% CI: 0.09-0.90)., Conclusions: Among pre-hospital procedures, only administration of a second antiplatelet drug (clopidogrel) significantly decreased the risk of reduced left ventricular ejection fraction, and administration of clopidogrel or heparin, or a combination of both, significantly decreased the risk of in-hospital death in patients with STEMI., Competing Interests: The authors declare no conflict of interest., (Copyright: © 2021 Termedia Sp. z o. o.)

Abstract

Fabry disease is a rare X-linked recessive lysosomal storage disease, which can cause a wide range of systemic symptoms. A deficiency of the enzyme alpha galactosidase A due to mutation causes a glycolipid to accumulate within the blood vessels, other tissues, and organs. This accumulation leads to an impairment of proper heart function. Wide range of symptoms makes diagnosis difficult. We present a case of a 43 year-old male with typical Fabry disease.

Abstract

Background: Antiplatelet drugs currently constitute the basic treatment of coronary artery disease (acute coronary syndrome [ACS], stable angina and patients treated with percutaneous coronary interventions [PCI]). The number of patients with indication for dual antiplatelet therapy with comorbidities with high thrombo-embolic risk (such as atrial fibrillation [AF], venous thrombotic disease, valvular diseases) is increasing. That is why the need for simultaneous administration of dual antiplatelet and oral anticoagulant therapy (triple therapy) has become more common recently. The AF is the most common indication for chronic anticoagulation. Because of the lack of large randomised trials regarding triple therapy, characteristics of this group has not been well established., Aim: To assess the presence of cardiovascular (CV) risk factors and concomitant diseases in patients with ACS requiring triple therapy., Methods: Retrospective analysis included 2279 patients diagnosed with ACS who were admitted to the Departments of Cardiology in Cracow in 2008. In this group, 365 (16%) patients had indications for chronic anticoagulation. Demographic and clinical characteristics of these patients were compared with those of patients included in other published registries., Results: Patients requiring triple therapy were aged 73.2 ± 9.5 years. Hypertension was diagnosed in 80%, hyperlipidaemia in 63%, smoking in 36%, prior myocardial infarction in 33%, prior stroke in 15%, previous treatment with PCI in 13%, coronary artery bypass grafting in 7%, diabetes in 36%, heart failure in 46%, anaemia in 33% and chronic ulcer disease or gastroesophageal reflux disease in 9%. The mean left ventricular ejection fraction was 46 ± 15%. Compared with other registries of patients without indications for triple therapy, our patients had significantly more frequently hypertension, diabetes and were older., Conclusions: Patients after an ACS requiring triple therapy have more often a history of comorbidities and CV risk factors when compared with the group of patients with ACS without indication for triple therapy.

A 14-year follow-up of a 69 year-old male with left ventricular aneurysm and thrombus after antero-septal myocardial infarction is presented. We describe problems with thromboembolic and bleeding complications in the context of changes in the guidelines over the period of treatment.
