The left gastric artery, which is the smallest branch of the coeliac trunk, travels along the lesser curvature of the stomach. During routine abdominal dissections, we encountered multivariations of the left gastric artery in a 65 year old male cadaver. The left gastric artery arose from the coeliac trunk as the first branch, afterwards it divided into two branches. The branch which passed to the right side gave three branches; to the left lobe of the liver (an accessory hepatic artery), to the fundus of the stomach and oesophagus, respectively. The branch which passed to the left side divided into two main trunks. The anterior trunk supplied the fundus and anterior wall of the stomach and the posterior trunk supplied the fundus and the posterior wall of the stomach. Consequently the posterior wall of the stomach was supplied by the posterior trunk instead of the posterior gastric artery of the splenic artery. Knowledge of the different anatomical variations of the arterial supply of the liver and stomach is of great importance in hepatobiliary and gastric surgical procedures. Therefore we believe our case will facilitate clinical aproaches related to this region. 34. İnsan fetuslarında m. plantaris’in morfometrisi

M. plantaris ile ilgili bu detaylı fetus çalışması, erişkin ile fetus arasındaki ölçümleri karşılaştırmak ve eğitimsel anatomi disseksiyonları ile ilgili klinik prosedürlerde ek bilgi sağlaması amacıyla yapılmıştır. Çalışma, herhangi bir patoloji veya anomalisi olmayan 17-40 hafta arasındaki 24 fetusta yapılmıştır. M. plantaris’in tendon ve karın bölümünün uzunluk ve genişliği ölçülmüştür. M. plantaris bir erkek fetusta unilateral, bir kız fetusta da bilateral olarak yoktu. Fetusların çoğunda (%83) m. plantaris’in ve tendonunun şekli tipik özelliklere sahipti. İkinci trimesterde tendonun ortalama uzunluk ve genişliği sırasıyla 36.35 ve 0.43 mm, üçüncü trimesterde ise sırasıyla 65.39 ve 0.95 mm idi. İkinci trimesterde karın bölümünün uzunluk ve genişliği sırasıyla ortalama 7.48 ve 2.96 mm, üçüncü trimesterde ise sırasıyla 17.58 ve 5.82 mm idi. M. plantaris ile ilgili fetus çalışması, eğitimsel anatomi disseksiyonları ve ilgili klinik prosedürlerde yapılan cerrahi girişimler için faydalı olacaktır. This detailed fetus study about plantaris muscle was performed to compare the measurements of adults and fetuses, and to obtain additional information about clinical procedures associated with educational anatomy dissections. The study was performed on 24 fetuses with gestational ages of 17-40 weeks and without any pathology or anomaly. The length and width of the tendineous portion and muscular belly of the plantaris muscle were measured in fetuses. M. plantaris was absent unilaterally and bilaterally in a male fetus and a female fetus, respectively. Most of the fetuses (83%) had a typical shape of plantaris muscle and its tendon. Mean length and width of the tendon were 36.35 and 0.43 mm, and 65.39 and 0.95 mm, in second and third trimesters, respectively. Mean length and width of the muscular belly were 7.48 and 2.96 mm, and 17.58 and 5.82 mm in second and third trimesters, respectively. This fetus study about plantaris muscle would bu useful in surgical interventions performed in educational anatomy dissections and related clinical procedures.

Karaciğerin sağ lobunun alt tarafında 58x49x36 mm boyutlarında bir kavite vardı. Bu kavitenin derininde, safra kesesi karaciğere gömülü olarak bulunuyordu. Bu kavite tamamen omentum majus.un bir bölümüyle örtülüydü. Omentum majus kaldırıldığında, kavitenin mideye bakan tarafında pilorik ostium, duodenuma bakan tarafında ise duodenal ostium görüldü. Kavitenin bu bölgesine uyan duodenumun superior parçasının atrezik olduğu saptandı. Bu kadavranın normal yaşamında, duodenum.un superior parçası ve bulbus duodeni.nin yokluğu nedeniyle, mide içeriği pilorik ostium aracılığıyla kaviteye, duodenum ostiumu aracılığıyla da duodenuma geçtiği varsayıldı. Anahtar kelimeler: Karaciğer, safra kesesi, duodenal atrezi, gelişimsel anomali Abstract This article presents abnormal localization of gallbladder with atresia of superior part of duodenum in a 50 year-old-male cadaver. On inferior side of right lobe of the liver was a cavity with diameters of 58x49x36 mm. At the bottom of this cavity the gallbladder was embedded the liver. This cavity was completely covered with a part of the greater omentum. When the greater omentum was removed, we observed ostium of pylor on the gastric side and the ostium of duodenum on the duodenal side of the cavity. It was observed that there was atresia of superior part of duodenum corresponding to the region of the cavity. In the normal life of this cadaver, because of the absence of superior part of duodenum and bulbus duodeni, gastric content was pouring into this cavity through the pyloric ostium and into duodenum through the duodenal ostium of the cavity. Unusual settlement of the gallbladder may lead inexperienced surgeons and radiologists into error during laparoscopic interventions or radiological assessments. Yaşları 40 ile 89 arasında değişen 24'ü erkek 16'sı kadın 40 kadavranın (80 taraf) tiroid bezleri diseke edildi. 9 tarafta (%11,25) çıplak gözle farkedilemeyen tüberkül (grade 0), 16 tarafta (%20) grade 1 tüberkül gözlendi. 45 tarafta (%56,25) ise grade 2 tüberkül gözlendi. Bu tüberküller dairesel (bezelye şekilinde) ve üçgen şekilli olarak tanımlandı. Tüberküllerin çoğunun tiroid lobunun posterior'unda, 3 tarafta ise posterolateral'inde yerleştiği gözlendi. 10 tarafta (%12,5) grade 3 tüberkül gözlendi. Bu tüberküller de üçgen (okbaşı şekilinde) ve dikdörtgen şekilli olarak tanımlandı. Bu tüberküllerin genellikle tiroid lobunun lateralinde ve trachea ile oesophagus'un arasında yerleştikleri gözlendi. Sadece 2 tarafta retroösofageal yerleşimli tüberküle rastlanıldı. Grade 2 ve 3 tüberküle tarafların %68,75'inde rastlanıldı. Bu tüberküllerin genellikle tiroid lobunun posterior'unda yerleştiği gözlenmekteyken birkaç tanesinin de posterolateralde yerleştiği tespit edildi. Çalışmamızda Zuckerkandl tüberkülünün yerleşimi ve morfolojisinin anlaşılabilmesi için bulgular yüksek çözünürlüklü fotoğraflar ile ortaya konuldu. Anahtar kelimeler: Tiroid bezi – Zuckerkandl tüberkülü – nervus laryngeus inferior – bası semptomları - ösofagus Abstract Incidence and morphology of the zuckerkandl's tubercle: An anatomic dissection study The purpose of this study was to investigate incidence and morphology of the Zuckerkandl's tubercle. Forty specimens (80 sides) were examined for this project, including 24 males and 16 female Turkish cadavers between the ages of 40 and 89 years at death. Unnoticeable tubercle (grade 0) was observed in 9 sides (11.25%), while grade 1 was found in 16 sides (20%). Grade 2 was observed in 45 sides (56.25%). Its shape was circular, similar to a pea or triangular. The tubercle extended postero-lateral to the thyroid lobe in 3 sides, while posterior to it in most sides. Grade 3 was found in 10 sides (12.5%). Its shape was also triangular, like a large arrowhead, or quadrangular. The grade 3 tubercle was generally located at lateral of the thyroid gland and between the trachea and esophagus. Retro-esophageal location was found only in two sides. We found grade 2 and 3 Zuckerkandl's tubercle in 68.75% sides of 40 cadavers. They were generally located posterior to the thyroid lobe, while a few postero-lateral. To be understood the position and morphology of the Zuckerkandl's tubercle, we have documented our findings in the form of high-resolution photographs. Kasın diğer iki başı ve diğer kol kasları normal anatomik lokalizasyonundaydı. Ek olarak aynı taraf kolda, n. musculocutaneus m. coracobrachialis'i delmiyordu. M. brachialis'in origosunun medial ve üst tarafından başlayan aksesuar baş, m. biceps brachii'nin ortak tendonunda sonlanıyordu. Aksesuar baş, diğer iki başlar gibi n. musculocutaneus tarafından innerve ediliyodu. Literatürde infero-medial humeral baş olarak adlandırılan bu başın görülme sıklığı ırka göre değişmekle beraber; ortalama %7.7 - %12'dir. M. biceps brachii'nin ikiden fazla başa sahip olması, bu bölgenin cerrahisi sırasında anatomik oryantasyonda güçlük oluşturabilmektedir. Ayrıca pronator sendromunda olduğu gibi, nörovasküler yapılara bası oluşturabileceği de bildirilmiştir. Ayrıca kasın şiddetli kontraksiyonlarında, aponörotik tendonda (lacertus fibrosus, aponeurosis bicipitalis) oluşabilecek gerilme sonucu, tendonun altından geçen nörovasküler yapılar (a. brachialis, n. medianus) basıya maruz kalabilir. Bu gibi varyasyonların bilinmesi, cerrahi komplikasyonların önlenmesinde ve başarıda önemli olduğu kadar, manyetik rezonans ve bilgisayarlı tomografi gibi radyolojik uygulamalarda ayırıcı tanı bakımından önemlidir. Abstract Humeral accessory head of biceps brachii muscle: A case report During dissection of 55-year-old female cadaver, we encountered an accessory head of biceps brachii in the right upper extremity. The other two heads of the muscle were as usual. Additionally on the same side, the musculocutaneous nerve didn't pierce the coracobrachialis muscle. The accessory head originated from upper third of the humerus, superior and medial to the origin of brachialis muscle and inserted to the common tendon of biceps brachii. As the other two heads, accessory head was innervated from the musculocutaneous nerve. In the literature this head was named as infero-medial humeral head and its incidence changes between 7.7%-12% according to ethnicity. Supernumerary heads may lead confusion at surgical procedures of arm or cause compression of neurovascular structures just as pronator syndrome. To avoid clinical complications, during radiodiagnostic procedures (CT, MRI) or surgical approach of this region, awereness of such a variation is of interest not only to anatomists, but also to orthopedic surgeons, physiotherapists and radiologists. Bu amaçla, erişkin kadavralardan alınan 30 çift insan akciğeri incelendi. Çalışmamızda, kadavraların 12'sinde toplam 13 aksesuar fissür gözlendi. En sık rastlanan aksesuar fissür, sağ alt aksesuar fissürdü (%20). İkinci sıklıkla rastlanan aksesuar fissür ise, sol horizontal fissürdü (%6,66). Sağ akciğer alt lobundaki anterior ve lateral bazal segmentler arasındaki aksesuar fissür, azygos fissürü, orta lobun medial ve lateral segmentleri arasındaki aksesuar fissür, üst ve alt lingular segmentler arasındaki aksesuar fissür ve üst aksesuar fissür eşit sıklıkta (%3,33) bulundu. Akciğerlerin aksesuar fissürlerinin varlığı, pulmoner lobektomi ve torakoskopik segmentektomi için operasyon öncesi stratejiyi değiştirebilir. Bulgularımız literatürde rapor edilen diğer çalışmalarla uyumludur. Ancak, sol akciğerlerden birisinde iki adet aksesuar fissür ve fissura obliqua gözlendi. Bu nedenle akciğer dört loblu bir görünümdeydi. Literatürde böyle bir vakaya rastlayamadık. Anahtar kelimeler: Akciğer, Aksesuar fissür, Varyasyon. Abstract Accessory fissures of the lungs The purpose of this study was to classify the accessory fissures of the lung and to assess and compare with the results of previous studies which were performed by different methods. For this purpose, 30 pairs of lungs from adult cadavers were investigated. In our study, 13 accessory fissures was seen in 12 cadavers. The most common accessory fissure was the right inferior accessory fissure (20%). The second most common accessory fissure was the supernumerary left horizontal fissure (6,66%). The accessory fissure between the anterior and lateral basal segments of the right inferior lobe, azygos fissure, the accessory fissure between the medial and lateral segments of the right middle lobe, the accessory fissure between the superior and inferior lingular segments and the left superior accessory fissure were seen in equal frequencies (%3,33). Presence of accessory fissures in lungs may alter preoperative strategy for pulmonary lobectomy and thoracoscopic segmentectomy. Our findings is consistent with studies in the literature. But, one of the left lungs is included two accessory fissures and oblique fissure. Therefore the lung had a four-lobed appearence. We could not find a similar case in literature. Objective: In recent years, 3-D printing technology has been the subject of interest due to its numerous applications in medicine. The ability to produce accurate 3-D models has attracted the attention of anatomists. The aim of the study was to provide information about the ideal section thickness in order to produce the best anatomical prints and models. Methods: CT and MR images with different section thicknesses were used in the study. STL images of the skull bones were created from the obtained serial CT sections. STL images of the central nervous system structures were obtained from MR serial sections. The differences of the section thickness in the obtained STL images were evaluated comparatively in order to show the details of the anatomical formations in both soft tissues and bones. 3-D printing was done by using the Form 3B+ device. Results: The thickness of serial sections is crucial in anatomical printing and modeling. In anatomical prints and models from both CT images and MR images, the thickness of each of the serial sections must be 1 mm or less in order to show the anatomical formations in details. In our study, it was determined that the bone anatomical models produced from serial sections of 1 mm thickness were exactly compatible with real bone images. However, in the imaging of soft tissues of the central nervous system, it was noticed that the details of some anatomical structures could not be revealed even at 1 mm serial section thickness. For this reason, in 3-D anatomical printing and modeling, it is extremely important to decide on the section thickness by considering the details of the produced anatomical structure especially in soft tissues. CT and MR images with a section thickness of 1 mm or less should be used to produce the most accurate anatomical print. The use of section thicknesses greater than 1 mm affects the quality of the produced model. Conclusion: In anatomical printing and modeling of hard tissues such as bone, the use of CT images with 1 mm serial section thickness enables us to obtain very perfect anatomical 3-D models. Objective: The widespread use of 3-D printers has brought a new perspective to the field of anatomy. Although many different resins are used in anatomical printing and modeling, the most preferred materials are white, transparent, gray and elastic resins. The purposes of use of each of these resins are different from each other. The aim of this study; was to reveal the usage areas more specifically by testing the white resin and transparent resin known as hard resins on different tissues. Methods: The study was carried out on parts of the central nervous system and bones of the skull. Serial CT and MR sections taken from the head with a section thickness of 1 mm were converted into STL images by using the MIMICS Innovation Suite software. While STL images of bone structures are obtained from CT serial sections; STL images of the CNS were produced from MR serial sections. Anatomical prints and models of the obtained images in STL format were obtained with the Form 3B+ brand 3D printer. After the anatomical models produced were kept in isopropyl alcohol for 15 minutes, the supports on the anatomical models were removed. Following this process, UV curing was performed at 60 degrees for 15 minutes. Anatomical models obtained with two different resins were compared in the study. Results: All anatomical models produced with white and transparent resins are not flexible and have a rigid structure. It was determined that the transparent resin was superior in anatomical prints and models, the inner surface of which was desired to be revealed. White resin is more suitable for use in models where the outer surface is desired to be shown in more detail and to be colored and detailed. Both resins are suitable for coloring, but when the outer surface of the structure is wanted to be examined, it has been observed that a higher quality image is revealed when white resins are painted. Conclusion: 3-D anatomical printings and models have brought a new perspective in the visualization of educational materials in the field of anatomy. It is certain that these anatomical 3-D models increase the quality of education. When looking at the materials produced with a 3-D printer, when it is desired to observe the veins, cavities or pathological formations in the structure; it is appropriate to use transparent resins because of the advantage that the interior is suitable for painting. If the outer surface of the anatomical structure is to be emphasized and other important structures on its outer surface are desired to be colored, the use of white resin will be more appropriate.
