A vena cava superior szindróma a vena cava superior részleges vagy teljes elzáródásából adódó tünetegyüttes. Az esetek többségében tüdőtumorok és lymphoma miatt másodlagosan alakul ki. A fej, a nyak, a felső végtag ödémája, tág vénák, dyspnoe jellemzi. Szemészeti elváltozások a szindrómához kapcsolódóan ritkán fordulnak elő: a szemhéjak duzzanatát, a kötőhártya chemosisát és suffusióját írták le korábban, egy esetben megemelkedett episcleralis nyomásról is beszámoltak. 57 éves nőbetegünk vena cava superior szindrómáját sztentbehelyezéssel oldották meg, kissejtes tüdőtumorát kemoterápiával kezelték. A szindróma részeként kétoldali, négy hónapos anamnézisű homályos látásra panaszkodott. Vizsgálata során kétoldali zárt zugot, mindkét oldalon 60 Hgmm-es szemnyomást és súlyos látáskárosodást találtunk. A jobb szemen goniosynechiolysissel egybekötött szürkehályog-műtétet végeztünk, ezáltal jelentős szemnyomás-csökkentést értünk el, és sikerült megőrizni a maradék látóélességet. A bal szemen Nd:YAG lézeres iridotomia után antiglaucomás cseppkezelést indítottunk a fájdalom csökkentésére. A phacoemulsificatiót követően a jobb szemen az elülső csarnok morfológiája jelentősen javult, amit elülsőszegmens-optikaikoherenciatomográfiával számszerűsíteni is lehetett. A szemészeti paraméterek az alapbetegség kezelése alatt stabilak maradtak. Jelen közleményünk célja, hogy felhívja a figyelmet a vena cava superior szindróma szemészeti tüneteire, valamint szeretnénk bemutatni azt a szindrómával esetenként együtt járó rapid, súlyos látásromlást okozó szemészeti állapotot, melyet tudomásunk szerint korábban még nem írtak le. Áttekintő közleményünkben a szívelégtelen betegek körében végzett pitvarfibrilláció-abláció témában rendelkezésre álló evidenciákat és az újabb vizsgálatok eredményeit foglaljuk össze. Az antiaritmiás gyógyszeres kezelés, valamint a katéterabláció közötti különbségeken túl a csökkent és megtartott ejekciós frakciójú szívelégtelen betegcsoportra vonatkozó eredményeket is tárgyaljuk. A jelenleg érvényben lévő európai pitvarfibrillációs irányelv alapvetően tünetorientáltan közelíti meg a betegség kezelését, azonban a szívelégtelenséggel szövődött esetekben a rendkívül magas halálozási rizikó miatt nagy szükség van a mortalitást is csökkentő terápiás lehetőségekre. A jelenlegi tudásunk alapján a klinikai kimenetel javítására megfontolandó ebben a betegcsoportban az abláció, mint elsővonalbeli terápia, azonban a különböző klinikai vizsgálatok eltérő eredménnyel zárultak. Tekintettel arra, hogy a gyógyszeres terápiának számos limitációja létezik, az ablációs stratégiának kiemelt jelentősége van. Mindazonáltal egyelőre kevés bizonyíték áll rendelkezésre arra vonatkozóan, hogy az egyes addicionális ablációs technikák mekkora klinikai haszonnal járnak. Valószínűsítjük, hogy a standardizált terápia helyett a személyre szabott gyógyszeres és eszközös kezelési stratégia javíthatja a szívelégtelen betegek klinikai kimenetelét.

Superior vena cava syndrome is a set of symptoms resulting from partial or complete blockage of the superior vena cava. In the majority of cases, it develops secondary to lung tumors and lymphoma. It is characterized by edema of the head, neck, and upper limbs, large veins, and dyspnoea. Ophthalmological changes related to the syndrome rarely occur: eyelid swelling, conjunctival chemosis and suffusion have been described previously, and in one case, elevated episcleral pressure was also reported. Our 57-year-old female patient's superior vena cava syndrome was resolved with stent placement, and her small cell lung tumor was treated with chemotherapy. As part of the syndrome, he complained of bilateral blurred vision with a history of 4 months. During his examination, we found a bilateral closed angle, an intraocular pressure of 60 mmHg on both sides, and severe visual impairment. We performed a lens exchange combined with better semolysis, which achieved a significant reduction in intraocular pressure and managed to preserve the remaining visual acuity. After Nd:YAG laser iridotomy on the left eye, we started antiglaucoma drop treatment to reduce pain. After phacoemulsification, the morphology of the anterior chamber in the right eye improved significantly, which could be quantified by anterior segment optical coherence tomography. The ophthalmic parameters remained stable during the treatment of the underlying disease. The purpose of this publication is to draw attention to the ophthalmic symptoms of vena cava syndrome as well as to present the ophthalmic condition that sometimes accompanies the syndrome causing rapid, severe visual impairment, which, to our knowledge, has not been described before. Report experience regarding an anterior capsulotomy fixated intraocular lens (IOL) designed to prevent negative dysphotopsia (ND).A prospective, nonrandomized clinical study was done at Péterfy Sándor Street Hospital-Clinic, and Jenő Manninger National Trauma Institute, Budapest, Hungary. The Morcher (Masket) 90S IOL has a circumferential groove on the optic that captures the anterior capsulotomy. Thus, part of the optic projects over and anterior to the capsule edge, while the bulk of the IOL is fixated within the capsular bag. We implanted the first version of the 90S IOL into the 40 eyes of 38 patients. These 40 eyes made up the primary investigational cohort. An additional 22 eyes received a modified version of the 90S IOL. The main outcome measure was the presence of ND. There was also a control group of 40 patients who received a single-piece monofocal aspheric hydrophobic acrylic IOL (877 FAB, Medicontur, Budapest, Hungary).None of our 66 test patients experienced ND during the follow-up period. After specific questioning, six patients reported nondebilitating PD that improved or disappeared completely in 5 cases. For the 40 eyes of the control group, there were 10 cases of ND on the first postoperative day and in 2 cases ND persisted for more than 1 year postoperatively.The 90S IOL can be used successfully to prevent ND. Since it is fixated by the anterior capsulotomy, additional advantages such as prevention of anterior capsule contraction, limited tilt, stable toric axis, perfect centration on the visual axis, and a more predictable lens position, among others, may be expected, and are under investigation.

Generally, the flow method has the advantage of a precise control over the reaction parameters and a facile modification of the reaction conditions, while a continuous flow microwave reactor allows for the quick optimization of reaction conditions owing to the rapid uniform heating. In this study, we developed a "9+4+1 method" to optimize reaction conditions based on comprehensive reaction analysis using a flow microwave reactor. The proposed method is expected to contribute to the synthesis of various fine and bulk chemicals by reducing cost and wastage, and by conserving time.

Aim: To present the early experience with the implantation technique, safety and efficiency of STARflo™ device for open angle glaucoma (OAG). Methods: referring intra- and postoperative clinical experience with a series of seven cases in three glaucoma centers in Hungary. Results: No intraoperative complications were observed. Postoperative inflammatory signs disappeared rapidly. The mean IOP reduction was from 27,6 ± 5,0 mmHg to 18,9±3,4 mmHg (32% reduction/ patient) at six months postoperatively. Conclusion: STARflo™ implant was safe and (except for one case with neovascular glaucoma) effective in our cases. The learning curve for experienced anterior segment surgeons was short.

To compare the accommodative amplitudes with three different methods in pseudophakic eyes with different types of intraocular lenses (IOLs).Fifty-one pseudophakic eyes of 44 patients (age: 72.02+/-8.53 years) were studied. One of two different types of IOL were implanted (N=30, three-piece Alcon Acrysof MA60AC and N=21, one-piece Alcon Acrysof SA60AT) in-the-bag after standard phacoemulsification. The time of the examinations was 13.85+/-7.35 months postoperatively. We measured the amplitude of accommodation with three different methods: (1) subjective minus-lenses-to-blur method; (2) a new optical device (ACMaster, Carl Zeiss, Jena, Germany) using partial coherence interference (PCI) technique under physiological stimulus; and (3) objective anterior chamber depth measuring with a standard A-scan ultrasonic device (Ultrascan Imaging System, Alcon Laboratories, Forth Worth, TX, USA) before and after pharmacological relaxation of ciliary muscle.We measured -0.83+/-0.63 D amplitude of accommodation with subjective minus-lenses-to-blur method. The IOL position did not change significantly during physiological accommodation effort measured with PCI method (-0.026+/-0.134 mm). The change in the IOLs position between near fixating and after ciliary muscle relaxation was -0.18+/-0.28 mm measured with ultrasound. There were no significant differences between values of one-piece and three-piece IOL groups.The amplitude of accommodation measured by subjective and objective methods are different and are not comparable with each other. We did not observe any difference between values of examined two types of IOLs.

Purpose To evaluate the amplitude of pseudophakic accommodation of 2 foldable intraocular lenses (IOLs) by measuring the anterior chamber depth (ACD) shift during a 1-year follow-up. Setting Department of Ophthalmology, Medical and Health Science Center, University of Debrecen, Debrecen, Hungary. Methods This prospective study comprised 44 eyes of 44 cataract patients operated on with phacoemulsification and in-the-bag implantation of Akreos Disc (22 eyes) or AcrySof MA60BM (22 eyes) IOLs. The ACD was measured by A-scan during fixation at 30 cm and again after instillation of cyclopentolate 1%. The ACD shift was calculated by subtracting the ACD under cyclopentolate from the ACD during fixation at 30 cm. Follow-up measurements were performed 3, 6, and 12 months after surgery. Statistical analysis concerning the difference between the 2 groups was done by the Student unpaired t test. Results The ACD shift was greater in the AcrySof group than in the Akreos Disc group at all 3 follow-up examinations. The difference was highly significant between the 2 groups at 6 and 12 months (both P = .004). After the operation, the ACD shift increased continuously with time in both groups. The mean ACD shift reached 0.57 mm ± 0.25 (SD) in the AcrySof group and 0.42 ± 0.24 mm in the Akreos group at 12 months. Conclusions The 3-piece AcrySof MA60BM IOL with 10-degree posterior angulation had a significantly higher capacity for pseudophakic accommodation than the 1-piece plate-haptic Akreos Disc IOL. The continuous increase in the ACD shift in the postoperative period may mean that patients learn to achieve better accommodation with an IOL as time goes on.

We compared the accuracy and the reproducibility of the measurements using a Haag-Streit, a Shin-Nippon, and a Carl Zeiss keratometer and an EyeSys videokeratoscope. Two investigators performed 20 measurements on 3 calibrated steel balls and 5 measurements on 22 normal corneas with all instruments. Calculating the accuracy of the measurements the limits of the 95% confidence intervals of the bias were determined. The accuracy was better than +/-0.1D for the keratometers, whereas it was better than +/-0.25D for the EyeSys videokeratoscope. The coefficient of reproducibility (1.96 x standard deviation) on test balls and corneas was smaller than 0.25D in the case of the Shin-Nippon and Carl Zeiss keratometers and the EyeSys videokeratoscope whereas it was between 0.25-0.5D in the case of the Haag-Streit keratometer. The Carl Zeiss keratometer was the best with respect to the accuracy and reproducibility of the measurements. Paired t-test was used to find possible significant differences between the results of the two investigators, but only clinically insignificant differences were found.

Purpose The purpose of this report is to highlight the potential risk of noncontact tonometry after routine uncomplicated penetrating keratoplasty (PK). Case report After uncomplicated PK for keratoconus, routine noncontact tonometry was performed on the second postoperative day. In spite of the adequately closed wound and the tight suture, temporary wound dehiscence occurred, and two-thirds of the anterior chamber was occupied by air. The running suture remained intact, and the Seidel test was negative. One week postoperatively, the patient’s corrected distance visual acuity was 0.4 (0.4 logMAR), and the air bubble had absorbed. Conclusion This is the first report to conclude that noncontact tonometry may not be sufficiently safe in the early postoperative period in normal PK cases. To prevent possible wound opening, we suggest the use of other tonometry methods during the first several months after PK.

Purpose: To test the hypothesis that pH value of human tears is affected by flow rate. Methods: After stimulation with a drop of 70% alcohol into the ipsilateral nasal cavity tears from the left eyes of 30 healthy volunteers were collected with glass capillaries while flow rate of tear secretion was registered. The pCO2, actual and standard pH were determined in the collected samples with the Astrup method. Standard pH is defined as the pH of the sample that has been equilibrated at 37 °C with a gas mixture having pCO2=40 mmHg.
