Abstract

Skin picking disorder is not a dermatological disorder and it is a table characterized with picking skin excessively and repetitively, leading to damage in skin tissue. Unlike normal picking behaviour, psychogenic skin picking is repetitive and it can lead to severe damage in the skin and even complications which constitute vital danger. While some patients define frequent but short lasting picking attacks, others define rarer attacks which last a few hours. Skin picking disorder, which is not included in the classification systems up to DSM-5 as a separate diagnosis category, is included as an independent diagnosis in Obsessive Compulsive Disorder and Associated Disorders category in DSM-5. In case reports, open label studies and double blind studies selective serotonin reuptake inhibitors are shown to be effective in the treatment of skin picking disorder. Mostly, cognitive-behaviourial techniques are used and have been proven to be useful in psychotherapy. Habit reversal is one of the behaviourial techniques which are frequently applied, give positive results in which well-being state can be maintained. Abstract

In addition to epilepsy, antiepileptic drugs are used in neurologic conditions such as chronic pain and in the treatment of bipolar disorder in psychiatry. There are studies reporting its use in the treatment of anxiety disorders. The efficacy of antiepileptic drugs as carbamazepine, valproic asid, lamotrigine, topiramate, gabapentin, pregabalin in anxiety disorders has been shown in some clinical studies. The strongest evidence has been presented for pregabalin in generalized anxiety disorder, pregabalin and gabapentin in social anxiety disorder and lamotrigine in posttraumatic stres disorder. While certain studies report the effectiveness of antiepileptic drugs in the treatment of anxiety disordes, others report them as ineffective. Double blind placebo controlled drug trials are essential for clearly presenting anxiolytic activity of antiepileptic drugs which would lead the way to further wide scale use in clinical practice. This article focuses on recent literature to show the potential use of antiepileptic drugs in patients suffering from generalized anxiety disorder, social anxiety disorder, posttraumatic stres disorder, panic disorder along with a brief review of neuroanatomic structures and neurotransmitters associated with epilepsy and anxiety disorders.

Abstract

BACKGROUND: Haloperidol has been used as an effective antipsychotic for many years and continues to be one of the first options in difficult patients who require parenteral therapy in the acute phase. However, the depot form is less preferred in the treatment of patients with non-adherence among these patients whose clinical stabilization has been achieved by using parenteral haloperidol in the acute phase. Therefore, updating the information about the side effects of the depot form of haloperidol, which is still an effective treatment option, will be useful in reconsidering the position of this medicine among new and different options. METHODS: A total of 54 schizophrenic patients with severe symptoms and poor adherence to treatment who were hospitalized and treated with depot haloperidol following an acute stabilization period were included in this study. First, the Structured Clinical Interview for DSM-IV Axis I disorders (SCID-CV) was used to confirm the diagnosis, the Brief Psychiatric Rating Scale (BPRS), Scale for the Assessment of Positive Symptoms (SAPS) and Scale for the Assessment of Negative Symptoms (SANS) to assess the clinical severity and Global Assessment of Functioning (GAF) to assess the functionality. The Simpson-Angus Scale (SAS) was used to assess extrapyramidal side effects. With the exception of Visit 0, plasma haloperidol levels were measured at all visits. Also, measurements of waist circumference and weight, plasma fasting blood glucose, triglyceride, HDL, iron, haemoglobin (Hgb), prolactin (PRL) and HbA1c were also used for evaluation of the metabolic effects. RESULTS: Significant improvements were observed in the BPRS, SANS, SAPS scores in the long-term follow-up with the depot haloperidol treatment. While the dosage decreased over time, the plasma levels remained changed, and symptom improvement was maintained. No signs such as neuroleptic malignant syndrome or acute dystonia were observed and SAS scores were within acceptable limits during the treatment (μ = 1.40 ± 2.55). There is no statistically significant difference between measurements of the weight even there was a significant difference between three of the waist circumference values (p = 0.987). The first measurement of the waist circumference is statistically significantly higher than both the mid-measurement and the final measurement, interestingly (p = 0.002). When fasting blood glucose, triglyceride, HDL, iron, Hgb, PRL and HbA1c were measured at different times throughout the study, only prolactin levels increased significantly over time with the use of haloperidol (p

7. Turkish Validity and Reliability Study of the ECT Perception and Knowledge Scale

Abstract

INTRODUCTION: Electroconvulsive Therapy (ECT) is an effective and safe treatment method used in the treatment of various psychiatric diseases. However, negative attitudes associated with ECT are common. This causes many negative consequences, from the treatment preference to treatment response and stigma. In this study, we aimed to carry out a validity-reliability analysis of the ECT Perception and Knowledge Scale (ECT-PK), which was developed to determine the perception and knowledge levels related to ECT, and adapt it to Turkish. METHOD: The Turkish adaptation of the ECT-PK was made using the translation-retranslation method. Our study included 50 patients with schizophrenia, 50 patients with bipolar disorder, 50 patients with major depression who met the remission criteria determined separately for each disorder, and 150 healthy controls.. To measure test-retest reliability, the scale was re-applied to 30 patients randomly selected from the patient group 14–21 days after the first application of the scale. RESULTS: In our study, a significant difference was found in both the patient and control groups in terms of the history of ECT application in the past and the status of accepting ECT application when recommended, and the perception and knowledge subscales of the ECT-PK. These results support the construct and criterion validity of the ECT-PK. Cronbach’s alpha coefficient was found to be 0.85 for the perception subscale and 0.78 for the knowledge subscale. The intra-class correlation coefficient used to evaluate the test-retest reliability was 0.86 for the perception scale and 0.83 for the knowledge subscale. CONCLUSION: It has been shown that the ECT-PK is a valid and reliable measurement tool that can be used to measure the perception and knowledge levels related to ECT in both clinical and non-clinical groups.

Abstract

Major strategic functions of the basal ganglia are thought to involve the fine tuning and modulation of the activity of the most parts of the frontal cortex, control of the skeleto-motor and oculo-motor movements, and modulation of the limbic system and associative functions. In this case report, a patient with a 10-year history of synthetic cannabinoid use and bilateral symmetrical hyperintense lesions of the globus pallidus in T2 weighed images following head trauma is described in conjunction clinical signs of global amnesia, behavioral changes consistent with frontal lobe syndrome, and reduced affect display, without any movement disorders. To the best of our knowledge, this patients represents the first case who had bilateral globus pallidus lesions due to synthetic cannabinoid use together with frontal lobe syndrome-like clinical signs without movement disorder. This case report points out to the fact that synthetic cannabinoid use may lead to development of bilateral globus pallidus lesions, which may be associated with amnesia without movement disorder, suggesting that this clinical picture may have resulted from the interruption of pathways between the prefrontal cortex and basal ganglia.

Abstract

Introduction Increasing evidence from clinical practice, as well as from epidemiological and basic research shows that there are gender differences in clinical features of schizophrenia, and this may be related to estrogens. There may be a relationship between earlier puberty and later onset of the disease, because of the protective effects of estrogens in women with schizophrenia. In this study, our aim was to analyze the correlation between age of menarche and age of onset of schizophrenia and to investigate the protective effects of estrogens in schizophrenia. Method In this study, we included 289 patients who were diagnosed with schizophrenia. Those with mental deficiency or organic brain disorders were excluded from the study. All subjects were given a socio-demographic form to determine their personal information, age at menarche, age at first odd behavior, age at onset of the disease and first hospitalization. Data on factors which may affect the association between age at onset of schizophrenia and age at menarche such as family history, head or birth trauma etc. were recorded on the information form. Results We found out that age at menarche was negatively associated with age at first odd behavior and age at first psychotic symptoms. Conclusion Our study verifies the protective effects of estrogens and shows that the earlier puberty may be the cause of later onset of schizophrenia. A gender-sensitive approach in psychiatry improves our understanding of mental illness and our therapeutic strategies.

Objective: Even though bipolar patients achieve remission, they do not reach premorbid psychosocial functioning. Factors such as residual depressive symptoms and number of episodes may cause this effect. The impact of treatment modalities has not been adequately studied. The aim of this study was to evaluate the impact of treatment modalities on psychosocial functioning in remitted bipolar patients.Methods: The study was carried out with 108 patients diagnosed as having bipolar disorder. All patients were in remission and the remission state was confirmed by rating scales. In the assessment of functioning, the Functioning Assessment Short Test (FAST) covering domains such as autonomy, occupational functioning, cognitive functioning, financial issues, interpersonal relations, and leisure time activities was applied. Abstract

Septum pellucidum, which forms the medial wall of the lateral ventricles, consists of two laminates. Cavum septum pellucidum (CSP) is defined when there is a space between these laminae. In some MRI studies have shown a higher rate of large CSP in patients with schizophrenia than in normal subjects. Looking at the literature on psychiatric disorders, CSP has been shown to be most associated with schizophrenia. Large CSP supports the neurodevelopmental model, which is one of the etiological explanations of schizophrenia. In our study, two patients with a diagnosis of CSP are mentioned. One of our patients is a first episode of schizophrenia, and the other one chronic schizophrenia patient with a history of multi-drug resistance. The first episode of schizophrenia is consistent with the information available in the literature in terms of the severity of symptoms, weak-response to treatment, and insufficiency of neuropsychological tests. The apparent deficit of the chronic schizophrenia patient suggests that CSP has a neurodevelopmental model in the etiology of schizophrenia, as well as the duration of the disease and non-compliance with treatment. There is no study in the literature comparing the response to treatment with large CSP in schizophrenia. It is thought that investigation of response to treatment in future studies is important for demonstrating the effects of neurodevelopmental model on the treatment of psychiatric disorders.

Abstract

This literature review aimed to examine the relationship between cognitive impairment and obsessive-compulsive disorder (OCD). Studies based on neuropsychological testing were prioritized, but those dealing with clinical features, therapy, comorbidity, neuroimaging, and the families of OCD patients were also considered.The literature on cognitive impairment in OCD was reviewed and then the studies evaluating the relationship with these above mentioned findings were discussed.The clinically most important cognitive impairment in OCD is executive dysfunction, followed by impaired memory. Cognitive impairment has also been observed in the healthy relatives of OCD patients. Findings regarding the effects of comorbidity on cognitive function in OCD patients are inconsistent. Brain imaging studies suggest that frontostriatothalamic dysfunction might occur in OCD.Executive dysfunction in OCD patients is well documented; however, the precise nature of the relationship between the severity of cognitive dysfunction and the clinical features of OCD are not well understood. Longitudinal family studies that employ both neuropsychological testing and brain imaging are needed to more clearly elucidate the relationship between cognitive dysfunction and OCD.

There are concurrently with different results of studies about cognitive functions of Obsessive-Compulsive Disorder (OCD), impairment in non-verbal memory and executive functioning in OCD, has shown consistent results in several studies. In this study, 62 OCD patients and 40 healthy controls were participated. Firstly, cognitive functions of OCD group and healthy control group were compared in terms of scores in Stroop Test, Wisconsin Cart Sorting Test (WCST), Auditory Consonant Trigram Test (ACTT), Controlled Word Association Test (CWAT), Rey Auditory Verbal Learning Test (RAVLT), Digit Span Test (DST). And then, two patient groups of OCD patients (patients with autogenous obsessions and patients with reactive obsessions) were compared in terms of the scores of same tests, with a hypothesis that claims, cognitive functions of patients with autogenous obsessions, who shown schizotypal personality features and thought disorder in higher ratio, will show more impairment than cognitive functions of patients with reactive obsessions. Significant impairment was found in OCD patients in terms of Stroop test and WCST scores when compared to scores of healthy controls. There was no difference pointed out between cognitive functions of patients with autogenous obsessions and reactive obsessions. Due to limited number of patients with autogenous obsessions in current study, any future research with greater sample size will be helpful to explain the cognitive functions in OCD with autogenous and reactive obsessions.

Abstract

Kleptomania is an impulse control disorder characterized by a recurrent failure to resist the impulse to steal worthless objects that are not needed for personal use. Very little is known about the etiology, prevalence and treatment. This disorder usually begins during puberty and lasts until late adulthood. In some patients, it may last throughout the person's life. Patients with kleptomania are likely to suffer from comorbid conditions like mood disorders. The patients usually seek treatment for the comorbid psychiatric complaints, rather than the kleptomaniac behavior itself. The literature lacks sufficient knowledge and controlled studies about the treatment of kleptomania. Regarding the treatment of SSRIs, there are case reports and case series, using mood stabilizers, antipsychotics and opioid antagonists. Cognitive behavioral therapy techniques are also used in the treatment of kleptomania. In this study, a female patient is presented with diminishing kleptomaniac symptoms after naltrexone is added to her cognitive behavioral therapy and fluoxetine treatment. She also suffers from the comorbidities of major depressive disorder and obsessive compulsive disorder.

19. Possible association between DNA repair gene variants and cannabis dependence in a Turkish cohort: a pilot study

OBJECTIVE: Substance use disorder (SUD) has important effects on health and well-being. It is well known that genetic factors play a role in SUD. The purpose of this research was to investigate whether functional variants of DNA repair genes might be a risk factor for cannabis and/or synthetic cannabis dependence in a Turkish cohort. METHODS: In total, 131 patients with cannabis and/or synthetic dependence and 70 healthy controls were included in this case–control study. XRCC1 codon 399 (rs25487) and XRCC4 G1394 T (rs6869366), and XPD (rs13181) variants were determined by the polymerase chain reaction-restriction fragment length polymorphism assay (PCR-RFLP). RESULTS: The XRCC1 rs25487 GG genotype and G allele were significantly lower in patients compared to controls (p = 0.005; p = 0.002, respectively). XRCC4 rs6869366 TT genotype and T allele were more common in patients compared to controls (p = 0.001, p = 0.001, respectively). It was found that patients with XPD rs13181 Lys/Gln had a significantly higher risk of cannabis dependence than control did (p = 0.00). The subjects carried XPD rs13181 Gln/Gln genotype had a 2.2-fold increased risk for cannabis dependence (p = 0.010). CONCLUSIONS: We demonstrated for the first time that DNA repair gene variants may alter individual vulnerability for SUD. This observation could be of further interest to researchers, as it could suggest new candidate genes, presumably crucial for the etiopathogenesis of the cannabis and/or synthetic cannabis dependence.

Abstract

Isaacs’ syndrome (IS) is an autoimmunological hyperexcitability syndrome of the peripheral motor nerves, manifesting with progressive muscle stiffness, involuntary continuous muscle twitching, muscle pain and cramping, sweating, and decreased reflexes. We report a 31-year-old man who was suffering from muscle twitches and stiffness in lower extremities and previously diagnosed with IS in his age of 16 through electrophysiological studies and the shown presence of autoantibodies against voltage-gated potassium channels. Without any adherence to the prescribed treatment, he had been using synthetic cannabinoids and opioids for 10 years. He admitted lessened complaints by using them. Current literature offered cannabinoid receptor agonists not only for symptomatic relief in IS, but also potential modulator effects on both potassium channels and autoimmunity. Opioids were also recognized with their analgesic and antispastic effects in the management of IS. This report aimed to discuss possible medicinal effects and therapeutic mechanisms of aforementioned psychoactive molecules on the symptomatology of IS.
