1. Mycoplasma associated stroke in a 4 year old child- A multifactorial etiology

Abstract

Mycoplasma pneumonia is a common cause of community-acquired pneumonia in India. Stroke due to infection-induced thrombosis is a rare complication of this infection and etiology can be multifactorial. We report a four-year-old girl with a mycoplasma infection associated stroke with thrombosis of the internal carotid artery and presence of lupus anticoagulant. She also had other risk factors for thrombosis like iron deficiency anemia and dehydration due to diarrhea which probably exacerbated an infection-induced procoagulant state. Lupus anticoagulants may be detected in many asymptomatic children with infections. The presence of other risk factors may precipitate a serious thrombotic event leading to significant morbidity and mortality. Recognising atypical pneumonia in the community and prompt treatment may reduce the serious extrapulmonary complications like stroke in children.

Abstract

Desensitization addresses safe optimal planning under parametric uncertainties by providing sensitivity function-based risk estimates. This paper expands upon the existing work on desensitization in optimal control to address safe planning for a class of two-player differential games. In the proposed game, parametric uncertainties correspond to variations of the model parameters for each player about their nominal values. The two players in the proposed formulation are assumed to have perfect information about these nominal parameter values. However, it is assumed that only one of the players has complete knowledge of the actual parameter value, resulting in information asymmetry in the proposed game. This lack of knowledge regarding the parameter variations is expected to result in state constraint violations for the player with an information disadvantage. In this regard, a desensitized feedback strategy that provides safe trajectories is proposed for the player with incomplete information. The proposed feedback strategy is evaluated for instances involving a single pursuer and a single evader with an uncertain moving obstacle, where the pursuer is assumed to only know the nominal value of the obstacle's speed. At the same time, the evader knows the obstacle's true speed, and also the fact that the pursuer knows only the nominal value of the obstacle's speed. Subsequently, deceptive strategies are proposed for the evader, who has an information advantage, and these strategies are assessed against the pursuer's desensitized strategy.

Abstract

The incidence of hepatocellular carcinoma (HCC) is growing in the United States, especially among the elderly. Older patients are increasingly receiving transplants as a result of HCC, but the impact of advancing age on long-term posttransplant outcomes is not clear. To study this, we used data from the US Multicenter HCC Transplant Consortium of 4980 patients. We divided the patients into 4 groups by age at transplantation: 18 to 64 years (n = 4001), 65 to 69 years (n = 683), 70 to 74 years (n = 252), and ≥75 years (n = 44). There were no differences in HCC tumor stage, type of bridging locoregional therapy, or explant residual tumor between the groups. Older age was confirmed to be an independent and significant predictor of overall survival even after adjusting for demographic, etiologic, and cancer-related factors on multivariable analysis. A dose-response effect of age on survival was observed, with every 5-year increase in age older than 50 years resulting in an absolute increase of 8.3% in the mortality rate. Competing risk analysis revealed that older patients experienced higher rates of non-HCC-related mortality (P = 0.004), and not HCC-related death (P = 0.24). To delineate the precise cause of death, we further analyzed a single-center cohort of patients who received a transplant as a result of HCC (n = 302). Patients older than 65 years had a higher incidence of de novo cancer (18.1% versus 7.6%; P = 0.006) after transplantation and higher overall cancer-related mortality (14.3% versus 6.6%; P = 0.03). Even carefully selected elderly patients with HCC have significantly worse posttransplant survival rates, which are mostly driven by non-HCC-related causes. Minimizing immunosuppression and closer surveillance for de novo cancers can potentially improve the outcomes in elderly patients who received a transplant as a result of HCC.

Abstract

We address the issue of safe optimal path planning under parametric uncertainties using a novel regularizer that allows trading off optimality with safety. The proposed regularizer leverages the notion that collisions may be modeled as constraint violations in an optimal control setting in order to produce open-loop trajectories with reduced risk of collisions. The risk of constraint violation is evaluated using a state-dependent relevance function and first-order variations in the constraint function with respect to parametric variations. The approach is generic and can be adapted to any optimal control formulation that deals with constraints under parametric uncertainty. Simulations using a holonomic robot avoiding multiple dynamic obstacles with uncertain velocities are used to demonstrate the effectiveness of the proposed approach. Antepartum depression, a non-psychotic mood disturbance occurring during pregnancy, is influenced by hormonal fluctuations and environmental endocrine disruptors. Despite its association with adverse postpartum outcomes, it has been studied to a limited extent. Hence, this study aims to investigate the association of neuroactive steroids, endocrine-disrupting compounds, and nutritional status of pregnant women with the manifestations of antepartum depressive symptoms.
