Abstract

One of the main issues of every business process is to be compliant with legal rules. This work presents a methodology to check in a semi-automated way the regulatory compliance of a business process. We analyse an e-Health hospital service in particular: the Hospital at Home (HaH) service. The paper shows, at first, the analysis of the hospital business using the Business Process Management and Notation (BPMN) standard language, then, the formalization in Defeasible Deontic Logic (DDL) of some rules of the European General Data Protection Regulation (GDPR). The aim is to show how to combine a set of tasks of a business with a set of rules to be compliant with, using a tool.

15. Histologic Features of External Cervical Resorption Affecting Impacted Maxillary Canines: A Report of 2 Cases.

Abstract

Reports on the histopathologic features of external cervical resorption (ECR) in unerupted teeth are scarce. This article reports on 2 maxillary impacted canines from different patients that showed ECR lesions and were surgically removed and histologically evaluated. Case 1 showed symptoms associated with oral communication of the dental follicle and pulpal exposure. Radiographs and cone-beam computed tomographic imaging showed that resorption affected a large part of the root and the crown. Case 2 was asymptomatic, and the resorption cavity was restricted to the tooth crown. Both teeth were extracted and subjected to histologic processing and analysis. In case number 1, the coronal pulp was necrotic and infected, showing areas of exposure to the resorption channels. The apical pulp was vital and uninflamed. The pulp space was surrounded by a continuous pericanalar resorption-resistant sheet. Part of the resorbed area of dentin was occupied by inflamed granulation tissue. More apically, a network of trabecular bone was present in the resorbed area. In case number 2, the pulp around the resorbed area was uninflamed and vital. A large fraction of the lost dentin was replaced by trabecular bone, closely adapted to the irregularly resorbed dentin surface, and no inflammatory cells. Our findings indicate that impacted teeth with ECR may remain asymptomatic until associated with infection. Histologic features of resorption in impacted teeth were similar to those in erupted teeth. In the case associated with infection, the resorbed area was occupied by inflamed tissue and newly formed bone. 21. TElewoRk-RelAted Stress (TERRA), Psychological and Physical Strain of Working From Home During the COVID-19 Pandemic: A Systematic Review

Abstract

Background: Many workers shifted to working from home due to the COVID-19 pandemic. This review aims to investigate if this sudden change caused an increase in TElewoRk-RelAted stress (TERRA) which is defined as physical and mental stress caused by telework.Methods: A systematic review using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines was performed of three scientific databases (PubMed, ISI Web of Knowledge, and Scopus), which also included a quality assessment. Articles measuring stress, psychological or physical, in remote workers, published from December 2019 through August 2021 were included in the review. Results were extracted by reporting: authors, country, study design, type of workers, sample, questionnaires and measurements, and outcomes. Data were synthesized quantitatively for country, type of workers, and outcomes.Results: Out of the 518 articles found in the three databases, 19 articles were included in the systematic review (10,012 participants overall), and 78.9% of these highlighted an increase in TERRA levels in remote workers. Among 85.7% of the studies considering gender as a variable, TERRA levels were higher in female workers. Twelve (63.2%) of the studies investigated psychological well-being, two (10.5%) focused on the physical well-being of remote workers, three (15.8%) investigated both, and two studies had other outcomes (10.5%).Conclusions: Considering the redefinition of workplaces dictated by the COVID-19 pandemic, this review highlights the emerging issue of remote work and the use of technology in working from home, emphasizing a rapidly growing occupational health problem. Remote workers need to be provided with emotional and technical support to prevent TERRA in remote workers.

Analysis and automation of business processes are a relevant topic in Industry 4.0. This document describes a framework called BP-M* for the analysis, restructuring and implementation of business processes, starting with the creation of a process model and ending with the implementation of the process itself on a workflow management system. The BP-M* framework has been applied to a real case study, the production of fabrics for the collection that will be distributed worldwide by an Italian woolen mill. Abstract

OBJECTIVE: Recently, Zirconia and polyaryletherketone (PEEK) have attracted increasing interest as reliable and safe materials in dental applications, mainly because of their good biomechanical characteristics. The aim of this study was to investigate the response to different loads by prosthetic frameworks for supported fixed partial dentures (FPDs), thus simulating osseointegrated implants. MATERIALS AND METHODS: The specimens were divided into two groups (n= 5 each). Group A: FDPs in zirconia-ceramic; Group B: FDPs in PEEK-composite. These 2 groups were subjected to vertical loads so to evaluate structural deformation; then, they have been analyzed by scanning electron microscopy (SEM) at different magnifications. RESULTS: In tested samples, different types of mechanical failures have been observed. In Zirconia-specimens, chipping is the main failure noticed in this study, mostly in distal margins of the structure. Also, peek-specimens show failure and fracture. CONCLUSIONS: Zirconia and PEEK could be considered both good materials, but several investigations are needed to use these materials as an alternative to metals for fixed partial dentures. Abstract

Background/objectives: To compare the number of eye surgical procedures performed in Italy in the 2 months following the beginning of lockdown (study period) because of COVID-19 epidemic with those performed in the two earlier months of the same year (intra-year control) and in the period of 2019 corresponding to the lockdown (inter-year control).Methods: Retrospective analysis of surgical procedures carried out at 39 Academic hospitals. A distinction was made between elective and urgent procedures. Intravitreal injections were also considered. Percentages for all surgical procedures and incidence rate ratios (IRR) for rhegmatogenous retinal detachment (RRD) events were calculated. A pvalue <0.05 was considered significant.Results: A total of 20,886 versus 55,259 and 56,640 patients underwent surgery during the lockdown versus intra-and inter-year control periods, respectively. During the lockdown, only 70% of patients for whom an operation/intravitreal injection was recommended, finally underwent surgery; the remaining patients did not attend because afraid of getting infected at the hospital (23%), taking public transportation (6.5%), or unavailable swabs (0.5%). Elective surgeries were reduced by 96.2% and 96.4%, urgent surgeries by 49.7% and 50.2%, and intravitreal injections by 48.5% and 48.6% in the lockdown period in comparison to intra-year and inter-year control periods, respectively. IRRs for RRDs during lockdown dropped significantly in comparison with intra- and inter-year control periods (CI: 0.65–0.80 and 0.61–0.75, respectively, p< 0.001 for both).Conclusion: This study provides a quantitative analysis of the reduction of eye surgical procedures performed in Italy because of the COVID-19 epidemic.

34. Safety and feasibility of upper limb vascular access for cardiac catheterization in anticoagulated and non-anticoagulated patients

Abstract

Background: The upper limb approach utilizing transradial access for combined left and right heart catheterization (CLRHC) and ultrasound-guided antecubital venous access for isolated right heart catheterization (IRHC) are strategies that may reduce risks, especially in anticoagulated patients. combined left and right heart catheterizationObjectives: To assess safety and feasibility of upper limb approach for IRHC or CLRHC in anticoagulated versus non-anticoagulated patients.Methods: Ninety-three patients who underwent IRHC or CLRHC with ultrasound-guided antecubital venous access and transradial arterial access were prospectively enrolled. The primary outcome was a composite of procedure failure and incidence of immediate vascular complications.Results: Of the 93 patients, 44 (47%) were on anticoagulation and 49 (53%) were not. Mean age was 54 ± 17 and 53 ± 15 years, respectively. Atrial fibrillation (39% vs 15%) and chronic kidney disease (21% vs 6%) were more common in anticoagulated patients. The main indication for anticoagulation was deep vein thrombosis/pulmonary thromboembolism in 22 patients (50%). The primary outcome occurred in 4 (8%) patients in the non-anticoagulated group as compared with 0 in the anticoagulated group (p= 0.12). Procedure failure occurred in two patients (4%) and immediate vascular complications in two patients (4%) in the non-anticoagulated group (p= 0.3 for all). There was no difference between groups regarding duration of the procedure, radiation dose, fluoroscopy time, post-procedure recovery room time and median time to venous or arterial hemostasis.Conclusions: The upper limb approach for heart catheterization had similar rates of procedure failure and immediate vascular complications in anticoagulated patients when compared to non-anticoagulated patients.

Abstract

Diagnosis of non-specific aorto-arteritis (NSAA, Takaysu's arteritis) is typically based on clinical and investigational parameters. We report here about two patients with clinically suspected diagnosis of a Takayasu's arteritis already under anti-inflammatory therapy in whom percutaneous transluminal atherectomy of subclavian and axillary artery stenoses was performed to relief the patients from symptoms – intermittent dyspraxia of the arms – and to verify the clinical diagnosis by histology. In the first case aorto-arteritis could be histologically confirmed through the analysis of plaque material including media structures excised from the subclavian and axillary arteries using a new device for atherectomy. The biopsy showed diffuse inflammation and granulomatous lesions with giant cells typically for Takayasu's disease. In the second patient, biopsy showed no acute or chronic inflammatory signs but only atherosclerotic lesions. Percutaneous transluminal atherectomy is therefore not only an interventional but also a diagnostic tool and should be used in every case of interventional therapy of suspected aorto-arteritis to make the clinical diagnosis and as a major consequence the initiation of an aggressive anti-inflammatory medical therapy more reliable.

Abstract

In order to offer data on the epidemiological profile of women with low bone density (LBD), we have considered information collected in the framework of a large cross-sectional study conducted on women around menopause attending a network of first level outpatients menopause clinics in Italy.During 1997 and 2000, a large cross sectional study was conducted on the characteristics of women around menopause attending a network of first level outpatients menopause clinics in Italy for general counseling about menopause or treatment of menopausal symptoms. Eligible for the study were women consecutively observed during the study period. Up to March 2000 a total of 42464 women (mean age 53 years) were observed at the 268 participating centers. Bone mass density was measured in 25113 cases, which are considered in this analysis. LBD (i.e. osteopenia or osteoporosis) was defined according to WHO classification.The frequency of LBD increased with age: in comparison with women agedor =51 years, the odds ratio (OR) of LBD was 1.5 and 2.2, respectively, in women aged 52-55 andor =56. No association emerged between LBD and parity: in comparison with nulliparae, the OR of LBD were 1.1 and 1.0, respectively, in women reported 1 or 2 or more full term pregnancies. Smokers were at increased risk of LBD, the corresponding OR being for smokers, in comparison with non smokers, 1.2 (95% confidence interval, CI, 1.1-1.3). In comparison with women in the lower tertile of body mass index, the OR of LBD decreased in subsequent tertiles, being, respectively, 0.9 and 0.7 in the middle and highest tertile. The OR of LBD was 2.2 in postmenopausal women, in comparison with premenopausal ones and the OR decreased increasing age at menopause. Ever OC users were at slightly decreased risk of LBD: in comparison with never OC users, the OR was 0.9 (95% CI 0.8-1.0) in ever OC users.The results of this large study confirm, in a Southern European population, the association of age at menopause and body mass on the risk of LBD, and suggest that ever OC women are at decreased risk of LBD.

Abstract

Objective: To analyse determinants of hysterectomy for benign uterine conditions with or without oophorectomy in Italy, a country with among the lowest rates of hysterectomy worldwide. Methods: Between 1997 and 1999, a large cross sectional study was conducted on the characteristics of women around menopause attending a network of first level outpatients menopause clinics in Italy for general counselling about menopause or treatment of menopausal symptoms. A total of 25 644 women entered the study. Characteristics of women who underwent and women who did not undergo hysterectomy and oophorectomy were compared. Results: Out of the 25644 women considered, 4727 were hysterectomized (18.4%). Mean age at surgery was 46.8 years. The OR of hysterectomy decreased with increasing educational level: in comparison with women with primary and none education, the OR of hysterectomy was 0.8 (95% CI 0.7-0.9) and 0.6 (95% CI 0.5 0.7) respectively for women with intermediate or high university degree. Body mass index (BMI, kg m -2 ) was related to the frequency of hysterectomy; in comparison with women with BMI ≤ 23.8, the OR of hysterectomy was 1.2 (95% CI 1.1 1.3) and 1.4 (95% CI 1.2-1.5) respectively for women with a BMI 23.8-27.2 and > 27.2. The frequency, of bilateral oophorectomy increased with age at hysterectomy: in comparison with women who have had hysterectomy when aged ≤43 years, the OR of bilateral oophorectomy during hysterectomy was respectively 1.9, 3.4 and 4.3 in women who had hysterectomy at age 44 47, 48 51 and > 52 years. The frequency of bilateral oophorectomy was lower in more educated women: in comparison with women reporting none or primary degree the OR was 1.0 and 0.8 respectively for women with intermediate and high university degree. Conclusions: The results of this analysis indicate that hysterectomy for benign conditions is more frequently performed in less educated women, in nulliparae and in women with higher BMI. During hysterectomy for benign conditions, bilateral oophorectomy is more frequent in less educated women and its frequency increases with women's age.

46. MULTIPARAMETRIC EVALUATION OF FITTING ACCURACY FOR DIFFERENT COMBINATIONS OF IMPLANT-ABUTMENT COUPLING AT MARGINAL INTERFACE.

Abstract

Objectives. This study describes a method for the measurement of the marginal gap between abutment and prosthetic crown and analyses the influence of different variables influencing the fitting precision. Methods. The extent of marginal gap between titanium or dental abutment with different implant types composed of sintered titanium, milled titanium alloy, zirconia and translucent zirconia was evaluated. The gap at implant-abutment interface was analysed by optical stereomicroscope observation. To optimise measurement precision, samples were oriented with a specific inclination angle and properly stained to limit reflection phenomena. Captured images was processed by ImageJ virtual measuring instruments. The correlation between various factors including type of tooth (molar, canine), stump type (artificial, natural), prosthesis type (sintered titanium, titanium, zirconia and translucent zirconia), and gap type (proximal, distal) was statistically evaluated performing the study of variance (ANOVA). Results. There was a significant correlation between the type of abutment and the different materials used for implant on fitting accuracy. The variables type of tooth and gap type do not significantly correlate with the marginal gap size. Results of the ANOVA test demonstrated that the association titanium abutment and titanium implant represent the best fitting with a minimum level of marginal gap at the junction interface. Conclusions. The measurement of marginal gap with stereoscopic observation associated with the analysis of variance has been demonstrated to be a valuable method for marginal gap evaluation. This approach demonstrated that the best abutment-implant fitting is represented by the association between titanium abutment and titanium implant. This model could be used to perform preliminary evaluation in order to help the clinician to select the best match of components for prosthetic implant. [ABSTRACT FROM AUTHOR]
