6. Lifetech Konar Cihazıyla Kapatılan VSD’ler; Çok Merkezli Çalışma Kısa ve Orta Dönem Sonuçlarımız

Abstract

AMAÇ: Ventriküler septal defektler (VSD) konjenital kalp hastalıkları içinde en sık görülen defektlerdendir. Yenigeliştirilen cihazlar büyük VSD'lerin transkateter yöntemle kapatılmasına imkan sağlamaktadır. Çok merkezliçalışmada transkateter yöntemle ve Konar-Multifunctional Occluder (MFO) ile kapatılan VSD’li hastalardaki kısave orta dönem sonuçlarımızın paylaşılması amaçlandı.GEREÇ-YÖNTEM: 2018 Kasım ve 2022 Ocak ayları arasında, 4 farklı merkezde gerçekleştirilen, 5 tanesi 24aydan küçük toplam 32 hastaya MFO cihazı ile VSD kapama işlemi yapıldı. Hastaların ortanca yaşı 5,9 yıldı. (5,5ay-17 yıl). 27 hasta perimembranöz 5 hasta mid-muskuler VSD’liydi. Ortalama Qp/Qs: 1,88 ±SD (1,58-2,3),defektlerin sağ orifis çapı:4,6 mm ±SD (3,5-8), sol orifis çapı: 6,7 mm ±SD (4-10) ölçüldü. Hastaların 24’üantegrad yol ile, 8 tanesi retrograd yol ile kapatıldı. Kullanılan en büyük cihaz 12x10 mm, en küçük cihaz ise 5x3mm boyutlarındaydı. Tüm vakalar ele alındığında işlem başarı oranı %100 olarak hesaplandı. Bir hasta da dahaönce ADO-II ile kapatılan cihaz embolize olduğu yerde güvenli olarak park edildi ve hastaya MFO ile başarılıkapama yapıldı. Ortalama takip süremiz 635 gün ±SD (30-1140). Hiçbir hastada majör komplikasyon gelişmedi.Bir hastada minör rezidü VSD, bir hastada femoral bölgede hematom ve 3 hastada da geçici küçük ritm bozukluğuizlendi.TARTIŞMA: LifeTech MF-Konar cihazı kullanılarak transkateter yöntemle uygun hastalarda hem konjenital hemde rezidü VSD’lerin kapatılması etkili bir şekilde yapılabilmektedir. Diğer cihazlara karşılaştırıldığında; dahaesnek ve hafif yapıda olması, antegrad ve retrograd iki yönlü kullanıma imkan sağlaması, daha büyük çaplıVSD’lerde kullanılabilmesi ve düşük kilolu hastalarda bile güvenli kullanılabilmesi önemli avantajlarsağlamaktadır.SONUÇ: Lifetech Konar MFO cihazı ile pediatrik hastalarda perimembranöz ve musküler VSD’ler transkateteryöntemle başarılı bir şekilde kapatılabilir. Cihazın büyük septal defektlere yüksek uyumluluk sağlayan tasarımı,esnekliği ve iki yönlü kullanılabilirliği önemli avantajlar olarak göze çarpmaktadır. Ayrıca küçük yaşlarda vedüşük kilolarda kullanımı diğer cihazlara göre fark yaratmaktadır. 7. The Effect of Transcatheter Ventricular Septal Defect Closure on Children's Appetite, Hormones, and Growth.

Abstract

Objective: Children with congenital heart disease are at risk for malnutrition. The aim of this study is to investigate the changes in appetite, nutritional hormones, and anthropometric measurements of patients before and after the transcatheter closure of ventricular septal defect. Materials and Methods: Twenty patients whose defect was closed percutaneously and 26 children as healthy control group were included in the study. The anthropometric evaluation, symptoms, and blood tests of patient group were enrolled at initial and follow-up (1st and 6th month visit). Results: The median age of patient and control group was 27 and 29.5 months, respectively. Lack of appetite, inadequate weight gain, and fatigue were higher in the patient group at initial visit (p=0.027, p=0.029, and p=0.033, respectively). At 1st month after closure, the rate of decrease in lack of appetite, inadequate weight gain, and fatigue were statistically significant (p=0.016, p=0.031, and p=0.031, respectively). After closure, increasing body mass index, its z score, and mid-upper arm circumference were statistically significant at 1st month (p=0.008, p=0.018, and p=0.018, respectively). Insulin-like growth factor-1, insulin-like growth factor binding protein-3, and their z scores were increased compared to the initial values at 1st month (p=0.001, p=0.033, p=0.002, and p=0.048, respectively). Ghrelin levels showed a linear decrease; leptin levels showed a linear increase for 6 months. Conclusion: Children with ventricular septal defect are under the risk of malnutrition. One of the goals of our treatment plan for these children should be the prevention of malnutrition; therefore, the timing of interventional therapy should be before malnutrition develops [ABSTRACT FROM AUTHOR]

12. Which Inotropic Drug, Dobutamine or Milrinone, Is Clinically More Effective in the Treatment of Postligation Cardiac Syndrome in Preterm Infants?

Abstract

Objective  This study aimed to detect which of the two main medicines suggested in the treatment of postligation cardiac syndrome (PLCS)—dobutamine or mirinone—possesses a more therapeutic effect. While doing this, clinicians are provided with a broader perspective on the treatment and follow-up of cases. The desire was to increase the treatability and monitor ability of the cases in question and hence their survivability. Study Design  A retrospective review of a cohort of infants with PLCS was conducted between March 2012 and December 2018. In the treatment of infants with PLCS, dobutamine (dobutamine study group-DSG) or milrinone (milrinone study group-MSG) was used. The respiration, cardiac, echocardiography, and perfusion parameters of the cases were assessed both before and after ligation. Based on the data obtained, both the effects of the medicines on PLCS and the difference between their therapeutic effects were studied. The accuracy of prognostication was assessed with receiver operating characteristic analyses. Results  PLCS was detected in 29 (34.1%) of 85 patent ductus arteriosus ligation cases in total. Of all the PLCS cases, 13 (44.8%) were treated with dobutamine and 16 (55.2%) with milrinone. It was observed that the effects of the medicines on the respiratory system and cardiovascular system manifested in the third and 6th hour, respectively. It was detected that both medicines had more effect on the systolic blood pressure (SBP) (area under the curve [AUC]: 0.997/0.996, p  = 0.001/0.002) than on the diastolic blood pressure (AUC: 0.911/0.843, p  = 0.032/0.046). Conclusion  Dobutamine and milrinone, two primary medicines that can be used in the treatment of cases with PLCS, possess similar therapeutic effects on this pathology. Abstract

Dusuk dogum agirlikli premature bebekler de dahil, tum yeni dogan bebeklerin aort koarktasyonunda birincil tedavi cerrahidir. Balon anjiyoplasti islemi, tekrar daralma oranlarinin yuksekligi, anevrizma olusumunda artmis risk ve girisimin neden oldugu periferik arter hasari gibi komplikasyonlar nedeniyle yenidogan nativ koarktasyonunun tedavisinde tartismalidir. Balon anjiyoplasti, genellikle cerrahi mortalite oranlarinin yuksek oldugu kalp fonksiyonlari iyi olmayan yenidoganlarda palyatif olarak yapilmaktadir. Cerrahi deneyimi yeterli olmayan merkezlerde, dusuk dogum agirlikli bebekler icin bu islemin palyasyon amacli yapilabilecegini dusunuyoruz. Bu yazida, dusuk dogum agirlikli premature aort koarktasyonlu bebekte femoral arter yolu kullanilarak gerceklestirdigimiz basarili balon anjiyoplasti islemi tanimlandi. Surgery is the primary treatment of aortic coarctation in all newborns, including low birth weight premature infants. Balloon angioplasty procedure in the treatment of native coarctation of the newborn is controversial due to high rates of restenosis and complications such as increased risk of aneurysm formation and damage to the peripheral artery involved in the intervention. Balloon angioplasty is often performed as a palliative treatment particularly in newborns with impaired cardiac functions indicating high rates of mortality. We believe that this procedure may be performed as a palliative treatment for low birth weight infants in centers lacking adequate surgical experience. Herein, we define a successful balloon angioplasty procedure which we carried out using the femoral artery route in a low birth weight premature infant with coarctation of the aorta.
