Indeed, one of the areas for improvement is the reduction of the diagnostic delay, which is still often high, given the need to transfer positive blood culture bottles to agar medium. The HEMOFAST clinical study conducted at the CHUGA and presented here, aims to evaluate the medico-economic impact of a new generation of syndromic multiplex PCR, the ePlex® PCR developed by GenMark and usable directly on positive blood culture bottles, after a direct microscopic examination. This PCR comprises several panels, depending on the results of the direct examination, which make it possible to identify, in 1,5 hours, 56 species or genera of bacteria and micromycetes as well as 10 resistance genes. The primary endpoint of the study is the time to optimised treatment inferior or equal to twelve hours (T12h), compared between the two arms of the study: an arm receiving the "usual" treatment in force in our centre and an arm benefiting in addition from the ePlex rapid test. The results of the intermediate analysis presented in this thesis show a significant contribution of this rapid test, since an optimised antibiotic therapy was reached at T12h for 63.55% of the patients in the "usual" group compared to 86.67% in the group with PCR. The work presented in this thesis is part of the European project H2020 FAPIC (Fast Assay for Pathogen Identification and Characterization), which aims to develop two automated diagnostic platforms, allowing the identification of 53 pathogens and bacterial genus as well as the identification of 40 resistance genes, directly from blood samples and within a single test. The first part of the work presented in this manuscript concerns the selection process of the genetic targets that will be identified in this test, the design of primers and probes and the optimization of the protocols of the multiplex PCR and microarrays. The second part of this manuscript concerns the development of the analysis software in order to identify pathogens and resistance genes from pictures of the microarrays. The recognition of the targets is carried out through the use of intensity thresholds and/or using a recognition system, specific towards patterns associated with each pathogen. The third part of this work describes the test performance and the identification of the targets from extracted bacterial strains DNA, then using samples artificially contaminated blood sample and finally using clinical blood samples from patient, Les travaux présentés dans cette thèse ont été réalisés dans le cadre projet Européen H2020 FAPIC (« Fast Assay for Pathogen Identification and Characterization »), dont le but est de développer deux outils de diagnostic automatisés. Chacun permet l’identification au sein d’un seul test de 53 pathogènes et genres bactériens ainsi que l’identification de 40 gènes de résistance, directement à partir d’échantillons de sang. La première partie du travail présenté dans ce manuscrit concerne le processus de sélection des cibles génétiques qui seront détectées avec ce test et la conception des sondes et des amorces ainsi que l’optimisation des protocoles de PCR multiplexes et d’hybridation sur les biopuces. Une deuxième partie de ce manuscrit concerne le développement du logiciel d’analyse afin d’identifier les pathogènes et gènes de résistance à partir des images obtenues sur les biopuces. La reconnaissance des cibles s’effectue à l’aide de seuils d’intensités et/ou à un système de reconnaissance de signatures spécifiques associées à chaque pathogène. The work reported in this thesis follows the one undertaken by Ms LE GOFF. It is focused on HIFI technology, which is patented and developed during her thesis. The first part of this work concerns the HIFI Blood 96™ test, and particularly the improvements and developments adduced to the test to make it a real diagnostic tool, multiparametric and high-throughput which can be implemented in blood banks in order to constitute negative antigen inventories, thus contributing to improve blood safety. It allows to characterize 96 samples on 15 polymorphisms (divided in two panels) associated to blood group systems in approximately 4.5 hours. A mesoscale validation study has been conducted on 583 samples for panel 1 and 190 samples for panel 2. The second part of this work describes the adaptation of HIFI technology applied to diagnosis of respiratory tract infections, with the development of another platform, ReSynPlex, in partnership with 3 research teams in Grenoble, Les travaux présentés dans cette thèse font suite à celle de Melle LE GOFF. Ils se concentrent sur la technologie HIFI brevetée et développée pendant ses travaux. Une première partie du travail présenté dans ce manuscrit concerne le test HIFI Blood 96™ et plus particulièrement les améliorations et les évolutions apportées au test afin d'en faire un véritable outil de génotypage, multiparamétrique et haut-débit pouvant être installé dans les banques de sang dans le but de constituer des inventaires de sang génotypé de façon étendue, participant ainsi à améliorer la sécurité transfusionnelle. Il permet de caractériser 96 échantillons sur 15 polymorphismes (divisés en deux panels) associés aux groupes sanguins en approximativement 4h30. Cette plateforme a fait l'objet d'une étude de validation à moyenne échelle sur 583 donneurs pour le panel 1 et 190 donneurs pour le panel 2. La deuxième partie des travaux décrit l'adaptation de la technologie HIFI appliquée au diagnostic des pathologies respiratoires, avec le développement d'une autre plateforme, ReSynPlex, en partenariat avec 3 équipes de recherche de Grenoble

Cet article de synthèse se penche sur une nouvelle discipline biologique et médicale, la « protéomique clinique ». Cette approche « postgénomique » vise à utiliser l’étude du protéome, c’est-à-dire de l’ensemble des peptides et protéines présents dans un échantillon biologique, pour donner une information diagnostique, pronostique ou de suivi thérapeutique des pathologies humaines. Cette nouvelle discipline est en plein développement car elle présente d’importantes perspectives pour les pathologies complexes ou pour la détection précoce de pathologies telles que les cancers. Les éléments de cette revue résultent du travail d’un groupe thématique de la SFBC (Société française de biologie clinique) de 2004 à 2006 dont l’objectif était d’évaluer l’état actuel de la « protéomique clinique » et de s’interroger sur son développement et son application future pour les biologistes. Il s’agit donc d’un état des lieux assez large décrivant les approches déjà accessibles dans les laboratoires d’analyses biologiques (dosages multiplex...) et les outils disponibles en recherche clinique et en protéomique « fondamentale », sans oublier les aspects bioinformatiques nécessaires à l’utilisation des données générées., This review focuses on “clinical proteomics” which represents an emerging discipline in biomedical research. “Clinical proteomics” relies on the analysis of the proteome, i.e. the entire set of peptides and proteins present in a biological sample, to provide relevant data for diagnosis, prognosis or therapeutic strategies of human pathologies. This new type of approach has tremendous potential for the diagnosis of complex pathologies or for the early detection of cancers. This article reports the conclusions of a workgroup of the French Society for Clinical Biology (SFBC) 2004-2006 which evaluated the status, the impact and the future development of proteomics in the clinical field. It provides therefore a broad view going from the methods already present in the clinical laboratories (multiplex technologies...), to the tools for clinical and basis research including bioinformatics.
