Pulsed Doppler ultrasound and spectral analysis were used to monitor nonpulsatile blood flow generated in a test rig. Two computerised techniques are described for detecting microemboli in blood by analysis of Doppler blood velocity data (sonagram).The two ultrasound microemboli indices identify different features in the ultrasound signals to detect microembolic phenomena. Both indices showed significant increases (p < 0.001) for samples of agitated blood (containing gaseous microemboli) as compared to normal blood injected sequentially into the test rig. The linear relationship demonstrated between data obtained by the two methods (r = 0.91, p < 0.01) indicates that both are providing similar quantitative information regarding the number of microemboli detected.These two computerised techniques may be applied to sonagrams obtained from arterial blood flow and thereby provide information regarding the presence of gaseous microemboli in the circulation during cardiopulmonary bypass surgery. This retrospective analysis aims to demonstrate the safety of carotid endarterectomy (CE) in patients >75 years with particular attention paid to intraoperative hemodynamics. Four hundred and ninety-six consecutive patients with >70% symptomatic and asymptomatic stenosis who underwent CE were prospectively assessed and divided into those <75 years (n = 408; mean 64 years) and those >75 years (n = 88; mean 78 years). Associated risk factors, hemodynamic parameters, surgical techniques and 30-day perioperative complications were compared. Carotid artery stump pressures of <25 mmHg (13 versus 29%) and middle cerebral artery velocities of <20 cm/sec (46 versus 29%) were more common in those >75 years (P = 0.0001 and P <0.005, respectively). However, there was no difference in the frequency of intraluminal shunt (34% in both groups) or synthetic patch usage (primary patching: 12.5%, older group versus 11%, younger group; secondary patching: 34% in both groups), and no difference in the combined 30-day stroke and death rates (3.4%, >75 years versus 1.1%, <75 years; P = 0.425). CE in this cohort of patients >75 years was not associated with increased morbidity or mortality. Altered intraoperative hemodynamics were not associated with increased use of shunting or patching. Multiple randomised trials over the last decade for both symptomatic and asymptomatic carotid stenosis have proven the efficacy of carotid endarterectomy (CE) in reducing the risk of stroke. The long-term patency of the carotid artery after CE is an important factor in the success of the operation. The incidence of recurrent carotid stenosis (excluding residual lesions) ranges from 1 to 37% with only 0-8% of patients having restenosis-related symptoms (1). Generally, recurrent carotid stenosis is attributed to myointimal hyperplasia during the early postoperative period (within 3 years) or recurrent atherosclerosis thereafter. The management of recurrent carotid stenosis after CE remains a dilemma. It is generally accepted that operation for significant recurrent carotid stenosis is indicated for symptomatic patients, and several authors also recommend CE for >80% asymptomatic recurrent stenosis. Treatment of recurrent carotid stenosis involves repeat endarterectomy with patch angioplasty, although more recently endovascular techniques have been used.

Background: Carotid endarterectomy reduces the risk of stroke and death in patients with severe carotid artery stenosis. This study examined whether the technique used to close the arteriotomy influenced the rate of perioperative transient ischaemic attack (TIA), stroke or death., Methods: A cohort of 236 patients undergoing carotid endarterectomy at a single centre was studied; 117 patients had primary closure of the arteriotomy and 119 patients in a sequential series had closure with a Dacron patch. A standard endarterectomy with completion intraoperative duplex imaging and digital subtraction angiography was used throughout., Results: Patch closure was associated with a significant reduction in the 30-day combined death, stroke and TIA rate: 10.3 per cent for primary closure versus 2.5 per cent for patch closure (P = 0.017). The risk of any cerebral event (stroke or TIA) was also significantly reduced (7.7 versus 1.7 per cent; P = 0.033). Residual stenosis on completion angiography was more common after primary closure (24.6 versus 7.4 per cent; P = 0.003)., Conclusion: Dacron patch closure had a higher technical success rate on completion imaging and was associated with a significant reduction in the risk of perioperative stroke, TIA and death., (Copyright (c) 2005 British Journal of Surgery Society Ltd.)

Objectives: Carotid endarterectomy has been used to treat both asymptomatic and symptomatic disease and this has meant that recurrent stenosis and its effect on late stroke risk have become increasingly important. In this study we compared anatomical defects and residual stenosis identified intra-operatively with recurrent stenosis and new symptoms developing in the first year after surgery., Design, Materials & Methods: Two hundred and forty-four consecutive patients undergoing carotid endarterectomy were studied prospectively. Residual anatomical defects were noted; residual stenosis was defined by intra-operative duplex ultrasound as >50%. New stenoses and clinical events during the one-year surveillance period were documented., Results: There was an increased incidence of recurrent stenosis at one year in vessels with residual stenoses (p<0.001) and in vessels containing a residual anatomical defect (p=0.037). There was no significant difference in recurrent stenosis rate with respect to closure (primary or patch) or seniority of surgeon but recurrent stenosis was increased in females (p=0.026). The majority (70%) of restenotic lesions were localised to the origin of the internal carotid artery. The late stroke rate was 0.9% and was not related to recurrent stenosis or symptoms., Conclusions: Residual stenosis and intra-luminal defects at completion increase the recurrent stenosis rate at one year. The aetiology of recurrent stenosis is multi-factorial and further studies are required to determine whether it is justified to modify the criteria for re-exploration with a view to reducing recurrent stenosis.

Background: For overall benefit, carotid endarterectomy requires low perioperative morbidity and mortality rates. Carotid thrombosis is usually secondary to technical error, which may be related to the experience of the operator. In this retrospective study the clinical and technical outcome of carotid endarterectomies performed by one consultant and five trainees were compared., Methods: Some 149 patients underwent carotid endarterectomy; 89 were operated on by the consultant and 60 by trainees. Intraoperative duplex imaging of the carotid repair was performed before wound closure, and re-exploration was carried out when there was a residual severe stenosis associated with an intimal flap., Results: There was no significant difference in clinical outcome between operations done by consultant or trainees. There was a significant increase in the number of stenoses, kinks and flaps in carotid endarterectomies performed by trainees compared with those of the consultant both before (chi2 = 12.0, 1 d.f., P < 0.001) and after (chi2 = 10.1, 1 d.f., P < 0.001) correction., Conclusion: Intraoperative duplex imaging may facilitate training by providing an objective assessment of the quality of the operation.

Background: Although carotid endarterectomy is increasing in the UK, there is evidence that the procedure is still underused. Methods of reducing cost in a single vascular unit have been assessed using a continuous audit including outcome measures., Methods: A consecutive series of 333 patients admitted over 7 years under a single consultant surgeon were studied. Outcome measures included the rate of perioperative neurological complication of any kind, and death. The length of hospital stay and the number of readmissions within 30 days were recorded prospectively by computerized audit., Results: Over the interval of the study, the number of preoperative investigations was reduced; angiography and cerebral computed tomography were reserved for specific indications. The median duration of hospital stay decreased from 7 to 2 days. There was no change in the stroke and death rate (3 per cent) during the study and only two patients required readmission within 30 days., Conclusion: Carotid endarterectomy can be performed cost-effectively using non-invasive preoperative investigations for the majority of patients. In-hospital stay has been reduced and the routine use of intensive care replaced by a 2-h stay in theatre recovery. These changes have been achieved without compromising patient safety.

Background: Duplex ultrasonography and magnetic resonance angiography (MRA) are becoming competitive alternatives to angiography for determining the degree of internal carotid artery (ICA) stenosis. Varying reports have been published regarding the suitability of each technique for grading ICA disease. This retrospective study compared the merits of these three modalities for measuring ICA stenosis., Methods: One hundred and eleven patients being considered for carotid endarterectomy underwent intra-arterial digital subtraction angiography (DSA) via arch injection. Duplex imaging was performed in all patients and MRA in 50. The degree of carotid stenosis estimated by the three modalities was compared., Results: There was good correlation between subjectively graded MRA and DSA images (r = 0.87, P < 0.001, n = 82 carotids) but poor correlation for objective estimates. MRA tended to underestimate the degree of stenosis (bias - 4.5 per cent) compared with DSA, but showed good correlation with duplex ultrasonography estimates (r = 0. 86, P < 0.001, n = 87 carotids). Both non-invasive modalities produced high values of sensitivity and specificity in estimating stenoses of greater than 70 per cent. MRA was less sensitive for distinguishing between severe stenosis and complete occlusion., Conclusion: This study did not resolve the debate regarding the method of choice as both MRA and duplex ultrasonography were accurate for imaging carotid stenoses.

BACKGROUND: There has been increasing interest in audit of outcome following surgery. A previous study reported a significant difference in clinical outcome between consultant and trainees but there is no information on technical factors. Duplex ultrasonography before wound closure was used to compare clinical and technical outcome for consultant and trainees. METHODS: Patients underwent endarterectomy (89 by consultant, 60 by six trainees). In the consultant group nine vessels were patched and 36 patients underwent shunting compared with seven and 31 respectively for trainees. RESULTS: In the consultant group there were two deaths, one stroke, one transient ischaemic attack (TIA) and two cranial nerve injuries. The neurological event rate was 2 per cent, and overall stroke and death rate 3 per cent. There were ten residual flaps (11 per cent) (three re-explored) and eight kinks (9 per cent). The residual stenosis rate was 10 per cent (nine of 89) and following re-exploration this reduced to 7 per cent. In the trainee group there was one death, two strokes, one TIA and one cranial nerve injury. The neurological event rate was 5 per cent, and the overall stroke and death rate 3 per cent. There were nine flaps (15 per cent) (four re-explored) and 13 kinks (22 per cent). The residual stenosis rate was 22 per cent (13 of 60) which reduced to 15 per cent (nine of 60) following re-exploration. There was no significant difference in clinical outcome between consultant and trainees but there was an increased incidence of technical problems among the trainees (t = 2.12, P < 0.05). CONCLUSION: Intraoperative duplex ultrasonography is a valuable method for assessing surgical technique; it gives immediate feedback to the surgeon, enables corrective measures to be taken and may facilitate training.

Purpose of Study: The aims of this study were to determine the incidence of restenosis following carotid endarterectomy with primary closure of the arteriotomy and to observe the natural history of disease progression in the 1st postoperative year., Methods: The study group consisted of a consecutive series of 126 patients undergoing carotid endarterectomy. Duplex imaging was performed preoperatively and at 8 weeks, 6 months and 1 year postoperatively., Results: Five patients (4%) had a residual stenosis. At 12 months, the overall restenosis rate was 15%: 8.5% for males and 28.9% for females. None of these restenoses were symptomatic. There was no significant difference in the diameter of the internal carotid artery between male and female patients (U = 896, P = 0.60) and no significant difference in the diameter of the arteries that had restenosed at 12 months and those that had remained patent (U = 391, P = 0.33)., Conclusions: Carotid endarterectomy with primary closure is associated with a low incidence of restenosis in men, but not in women. Criteria for selective patching should consider both gender and vessel calibre.

Objectives: This study evaluates high resolution, duplex ultrasound imaging for quality control of carotid endarterectomy in order to determine which technical factors were linked to residual stenosis and to define duplex criteria for re-exploration., Design, Material and Methods: A consecutive series of 100 patients undergoing carotid endarterectomy were evaluated. Duplex imaging was performed prior to wound closure and repeated at 6-8 weeks postoperatively. Stenoses were classified as non-significant, moderate or severe based on duplex criteria. Intimal flaps, shelves, kinks, clamp damage and fronds were identified by ultrasound imaging., Results: Five moderate stenoses were noted in the proximal endarterectomy site (PES), and at follow-up three had resolved. Adherent fronds were detected in 83% of vessels and resolved in all but three cases. At the distal endarterectomy site there were 10 severe and 12 moderate stenoses. Intimal flaps were associated with an increased incidence of residual stenosis (p = 0.010)., Conclusions: We conclude that severe stenoses with an intimal flap should be corrected immediately. Further data is required to establish the significance of kinks. Residual intimal flaps in the PES appear to remodel. The role of completion duplex may lie in the modification of surgical technique to eradicate anatomical and haemodynamic imperfections.

One-stop clinics are becoming increasingly popular with both patients and their general practitioners. Traditionally, vascular patients have needed to attend hospital two or three times for clinical examination and investigations. We have introduced a one-stop clinic for patients with lower limb arterial disease (LLAD) and aortic aneurysms. In 92 clinics over 2 years, 1194 new patients and 1409 follow-up patients were seen, with LLAD being the largest single category comprising 40% of the patients seen, followed by varicose veins (25%), carotid disease (12%), and aortic aneurysms (8%). Overall, 57% of patients had non-invasive imaging performed, either in the clinic or on a separate visit. Performing all LLAD and aortic scans in the clinic requires 1.9 h of imaging time per clinic. Extending in-clinic scanning to patients with varicose veins and carotid disease would increase this to 3.9 h of scanning per clinic and require a duplex scanner and an additional technologist in the clinic.

The measurement of blood flow in small arteries is a potential extension of magnetic resonance (MR) angiography. We have compared flow measurements based on MR phase mapping with those obtained by Doppler ultrasound and electromagnetic flowmetry in both phantom and animal models. Correlation between modalities was high for in vitro studies (R2 = 0.93-0.98). In vivo, electrocardiogram-gated MR and Doppler ultrasound flow measurements compared to electromagnetic flowmetry showed fair correlation coefficients (R2 = 0.73 and 0.66, respectively). However, limits of agreement indicated that in small vessels flow measured by the three modalities could differ by up to +/-90 mL/min. For both models, arteries in the range of 3-6.5 mm in diameter produced complementary errors in area and velocity measurements in MR studies. Ungated MR studies showed a reduced agreement (R2 = 0.88 in vitro, 0.54 in vivo), which may in part be due to poor sampling of the velocity pattern. The results show that the high correlation obtained in vitro cannot be extrapolated to the in vivo situation, where additional physiological and anatomical variables are encountered.

Objectives: To compare the subjective (eyeballed) method for measuring internal carotid artery (ICA) stenosis by non-selective intra-arterial digital subtraction angiography (IA-DSA) with objective and duplex methods., Design: Retrospective study., Materials and Methods: Fifty-three consecutive patients underwent IA-DSA prior to carotid endarterectomy providing 103 carotid angiograms. Objective assessment of ICA stenosis was by the North American Symptomatic Carotid Endarteectomy Trial (NASCET) criterion and the Carotid Stenosis Index (CSI). Duplex estimation of stenosis was derived from peak systolic and diastolic velocities in the ICA and common carotid artery (CCA)., Results: The coefficient of repeatability was poorest for NASCET stenosis estimates (40%), whilst the improved values for CSI (20%) were consistent with the lower variability recorded for measuring the CCA diameter. Correlation and agreement levels between subjective (r = 0.80, -41% to +33%) or objective assessments and duplex (NASCET: r = 0.76; -52 to +28%; CSI: r = 0.72; -27 to +39%) showed similar values., Conclusions: We conclude that the inter-observer variability for assessing angiograms obtained by arch injection is considerable and precludes high agreement when IA-DSA is compared with other methods. As the agreement of duplex ultrasound with IA-DSA is similar to the agreement between DSA methods, duplex can be offered as the first stage assessment of ICA stenosis, with the proviso that the duplex assessment is performed consistently by an experienced operator.

Indirect (cuff) and direct (intra-arterial) BPs were measured in 15 normotensive (mean age 79 years, range 72-88 years) and 21 hypertensive (mean age 76 years, range 65-89 years) elderly patients. Osler's test and arm arterial compliance, measured using Doppler-shifted ultrasound, were also assessed. In the normotensive, cuff systolic pressures were less than the direct by > 20 mmHg in 10 of the 15 (mean cuff systolic 139 +/- 13: direct 160 +/- 20 mmHg, P < 0.001); there was no statistical difference between the cuff and direct mean diastolic pressures (mean cuff diastolic 75 +/- 8; direct 70 +/- 10 mmHg). In the hypertensives, one had pseudohypertension (cuff systolic 186: direct 152 mmHg). However, cuff systolic pressures were less than the direct by > 20 mmHg in 11 (mean cuff systolic 205 +/- 26: direct 224 +/- 31 mmHg, P < 0.05); cuff diastolic pressures were greater than the direct by > 10 mmHg in 10 (mean cuff diastolic 102 +/- 14; direct 93 +/- 16 mmHg, P < 0.05). There was no significant difference between cuff and direct mean arterial pressures or the compliance in the normotensive and hypertensive groups. Osler's test was negative in the pseudohypertensive patient while in the four positive tests cuff systolic pressures were less than the direct measurements. In elderly patients there was no significant difference between the mean BP for cuff and direct measurements; systolic pseudohypotension and diastolic pseudohypertension are common and Osler's test is misleading.

Many commercially available pulse oximeters have a variable pitch blood oxygen saturation indicator, which emits a 'beep' between 175 Hz and 675 Hz with each heart beat. The frequency of this beep is dependent on the blood oxygenation measured by the oximeter, higher frequencies corresponding to higher blood saturation percentages. This is a useful feature for an anaesthetist, rapidly communicating potentially life-threatening situations in an easily detectable manner. Most available oximeters with an audio output feature are vulnerable to the effects of magnetic resonance scanning. In this article an add-on module is described for generating a beep to indicate the blood oxygen saturation which has been specifically designed to operate in the MR environment.

The ability of transcranial pulsed Doppler ultrasound (TCD) to provide a dynamic assessment of the functional capability of the Circle of Willis was assessed using conventional cerebral angiography for anatomic correlation. Eleven patients had normal four-vessel cerebral angiography prior to being investigated with ultrasound. Angiography and ultrasound both demonstrated a functional anterior communicating artery in nine of the eleven patients, giving complete agreement between the two techniques. Posterior communicating arteries were visualized angiographically in all eleven patients. Ultrasound identified bilateral functional vessels in nine, the other two patients having non-functional vessels. In these latter two patients, angiography demonstrated three of the four posterior communicating arteries to be hypoplastic and it was uncertain whether these vessels carried significant blood flow. The fourth posterior communicating artery was shown to have an absent proximal segment of the ipsilateral posterior cerebral artery, with a persistent fetal posterior communicating artery. This anatomical variation is a potential limitation of ultrasound for assessing functional posterior communicating arteries. These preliminary results indicate that a combination of the anatomical (angiographic) and dynamic (ultrasonic) data may prove to be complementary for assessing the Circle of Willis.

Magnetic resonance imaging has been reported to have advantages over conventional angiography in the diagnosis of dural sinus thrombosis. A case report is presented describing the application of MR techniques including MR angiography, to diagnose and monitor therapy for dural sinus thrombosis.

A technique for measuring cerebral reactivity using transcranial pulsed Doppler ultrasound is described; the system includes a spectrum analyser and capnometer. Data acquisition and manipulation is under software control. Main stem middle cerebral artery blood velocity is monitored continuously using the transcranial Doppler technique, whilst the operator initiates data collection and controls the inspired gas composition. The calculation of cerebral CO2 reactivity is based upon linear regression analysis of normalized, time-averaged middle cerebral velocity on end-tidal pCO2 and is displayed graphically. Measurement of middle cerebral CO2 reactivities can be completed within 15 min. Results from two subjects, a healthy volunteer and a patient with occlusive disease, are shown to illustrate the technique.

Transcranial pulsed Doppler ultrasound was used to monitor blood velocity in the middle cerebral artery (MCA) of two patients during ipsilateral carotid endarterectomy. In the first patient the ultrasound data demonstrated a non-functioning shunt which was corrected by repositioning the distal end of the shunt. In the second patient MCA blood velocity data demonstrated that clamping of the external carotid artery would have resulted in complete cessation of MCA flow throughout endarterectomy. These cases illustrate the benefit that this technique offers to the individual patient undergoing carotid surgery.

Doppler-shift ultrasound with spectral analysis permits the measurement of the relative heights of systolic peaks (the A/B ratio) in blood velocity sonograms from supra-orbital and common carotid arteries, the temporal artery occlusion test, the common carotid resistance index and transit times of flow-velocity waves in the internal carotid pathway. The results are compared with 201 carotid angiograms and expressed in terms of sensitivity (true positive) and specificity (true negative). Supra-orbital and common carotid A/B ratios, in combination with the temporal artery occlusion test, detected all grades of carotid artery disease with a sensitivity of 72 per cent and a specificity of 93 per cent. The corrected transit time gave a sensitivity of 57 per cent and a specificity of 89 per cent, whilst the Resistance Index gave a sensitivity of 60 per cent and a specificity of 80 per cent. The values of ultrasound parameters associated with severe disease, defined as reductions in lumen diameter of 75 per cent or more, of the internal carotid artery are examined. The temporal artery occlusion test gave a sensitivity of 91 per cent and a specificity of 94 per cent. These results were more accurate than those obtained for the corrected transit time (sensitivity 83 per cent, specificity 92 per cent) and for the Resistance Index (sensitivity 75 per cent and specificity 92 per cent). The results show that of the tests described the supra-orbital and common carotid artery A/B ratios, together with the temporal artery occlusion test, gave the best sensitivity and specificity for detection of both severe and minor carotid bifurcation disease.

Doppler-shifted ultrasound with spectral analysis was used to assess the internal carotid arteries of 48 patients who had undergone carotid endarterectomy (58 carotid endarterectomies). Three patients died in the immediate post-operative period, therefore 45 patients, who represented 54 endarterectomies, were assessed for up to six years following surgery (mean--thirty-four months). Ultrasound tests demonstrated total occlusion of the internal carotid artery in 7 patients within the first post-operative week. Sonograms from the supraorbital and common carotid arteries exhibit two peaks during cardiac systole (A and B). The post-operative A/B ratios were abnormal in 24 instances and these were associated with symptoms in seven. Of these 24, 7 developed severe disease in the internal carotid artery during the six year follow-up period; five were associated with symptoms. Post-operative A/B ratios were normal in 23 instances, 19 of which remained normal at follow-up. None of these nineteen developed symptoms. Only patients with abnormal post-operative A/B ratios subsequently developed severe occlusive disease in the internal carotid artery during the follow-up assessment. Thirty-eight patients underwent unilateral carotid endarterectomy, eight of whom had severe internal carotid artery disease at the contralateral bifurcation at the time of carotid endarterectomy. A further four patients developed severe occlusive disease in the contralateral internal carotid artery during the follow-up period, one of whom was symptomatic.

Twenty-seven patients were examined who were undergoing cardiopulmonary bypass (CPB) surgery with either a bubble oxygenator or a capillary membrane oxygenator. The latter incorporated an arterial filter and bubble trap. A noninvasive Doppler ultrasound technique is described for monitoring irregularities in the Doppler flow signals attributable to gaseous microemboli detected in the middle cerebral artery during CPB. The ultrasound index for detecting gaseous microemboli (MEI) indicated the presence of such microemboli in 22 of the 27 patients during insertion of the aortic cannula. Measurements during CPB showed the MEI ranged from 4 to 39 in the 17 patients with a bubble oxygenator. However, all 10 patients with a membrane oxygenator had an MEI of 0. Varying the gas flow rates in 3 patients with bubble oxygenators showed a change in MEI from 4 +/- 4 (SD) at a flow rate of 2 L/min to 17 +/- 9 at a flow rate of 5 L/min. This observation supports the assumption that the MEI is providing quantitative information regarding the presence of gaseous emboli in the middle cerebral artery.

Using continuous wave Doppler-shifted ultrasound and spectral analysis the response to the temporal artery occlusion test was compared when insonating the supraorbital and supratrochlear arteries of 203 internal carotid pathways. The test involved monitoring the periorbital artery whilst simultaneously compressing a single branch of the external carotid artery, the ipsilateral superficial temporal artery. In 41 instances carotid angiography demonstrated severe disease of the internal carotid artery, that is lesions of 75% or more reduction in lumen diameter. A positive response to the temporal artery occlusion test, when performed on the supraorbital artery, indicated severe disease with a sensitivity of 90% and a specificity of 89%. When this test was performed on the supratrochlear artery a low sensitivity of 36% was obtained, although specificity was high at 96%.

Sixteen patients (mean age 59 +/- 9 years) who were to undergo carotid endarterectomy were examined pre- and peroperatively using pulsed Doppler-shifted ultrasound; blood velocities in the middle cerebral artery were measured before and during common carotid compression in the conscious patient, and before and during measurement of carotid stump pressures at surgery in the anaesthetized patient. Measurements at endarterectomy showed a significant difference in middle cerebral artery blood velocities from patients with stump pressures of greater and less than 50 mmHg (t = 4.0, P less than 0.005). A threshold of 10 cm s-1 distinguished between stump pressures of greater and less than 50 mmHg in 15 of the 16 patients. Pre-operative blood velocity measurements during carotid compression did not correlate with those taken peroperatively at carotid clamping.

Doppler-shifted ultrasound was used as a non-invasive test to determine the presence of severe (pressure dropping) internal carotid lesions in two groups of elderly patients. Group 1 consisted of 150 hypertensive patients; these were subdivided into 77 who had had no cerebrovascular incidents (mean age 75 years, range 65-97) and 73 who had previously suffered a stroke (mean age 75 years, range 65-91). Group 2 consisted of 44 normotensive asymptomatic elderly volunteers (mean age 76 years, range 68-90). In the hypertensive patients (group 1), severe disease was detected in the non-stroke patients in 15 (10%) of the internal carotid arteries (14% of the patients), and in the stroke patients in 23 (16%) of the internal carotid arteries (23% of the patients). None of the normotensive volunteers (group 2) had evidence of severe internal carotid disease. Statistical analysis confirmed a highly significant difference in the prevalence of severe carotid disease between the normotensive and hypertensive subjects. The difference between the non-stroke and stroke hypertensive patients was not statistically significant.

The occurrence of neurological sequelae following cardiopulmonary bypass (CBP) surgery has stimulated interest in refining the techniques of extracorporeal circulation. Air micro-emboli originating from the oxygenator have been postulated as one source of cerebral damage. Since controversy still exists regarding the merits of bubble versus membrane oxygenators, this has prompted investigators to devise methods to determine the amount of micro-emboli produced during CPB. In this study, 27 patients undergoing CPB surgery for coronary artery disease (21) or valve replacement (6) were examined. The surgical and anaesthetic techniques were standardised in all patients except for the type of oxygenator used. A bubble oxygenator was used in 17 patients (Bentley Bio-10, William Harvey or Dideco) and a membrane oxygenator with a 25 microns filter in the remaining 10 patients (Bentley BOS CM50). Transcranial pulsed Doppler ultrasound was used to obtain blood velocity signals from the middle cerebral artery throughout CPB. A flow disturbance index (FDI) was defined which provided a representative index of the number of micro-emboli passing the ultrasound transducer. The FDI indicated the presence of gaseous micro-emboli during insertion of the aortic cannula in 22 of the 27 patients. In the 17 patients with a bubble oxygenator, the FDI ranged from 4-39. In the 10 patients with a membrane oxygenator, the FDI was always 0. Variation of gas flow rates in 3 patients with bubble oxygenators showed a change in the FDI from 4 +/- 4 at a flow rate of 2 l/min to 17 +/- 9 at 5 l/min.(ABSTRACT TRUNCATED AT 250 WORDS)

A method is described for non-invasively measuring the increase in lower limb blood flow during transfemoral lumbar aortography. Flow measurements were made using a continuous wave Doppler-shift ultrasound transducer placed over the contralateral femoral artery. The effect of the non-ionic contrast medium B15000 (Iopamidol), conventional contrast medium (Urografin 370) and Urografin 370 plus Lignocaine were compared in a double-blind trial. All three produced an increase in flow which reached a peak between 12 and 45 s after injection. The peak flow following Iopamidol was significantly lower than that from both Urografin 370 alone and with addition of Lignocaine. There was a difference of lesser degree between Urografin 370 plus Lignocaine and Urografin 370 alone; however, this was not statistically significant. The subjective assessments of pain and patient 'discomfort' paralleled these objective flow measurements.

Supraorbital artery blood flow was examined non-invasively with continuous wave Doppler-shifted ultrasound and spectral analysis. The results were used to assess the ipsilateral internal carotid artery in 155 patients undergoing carotid angiography. The ultrasound parameters used were the temporal artery occlusion test and A/B ratios of both resting and augmented supraorbital sonagrams. A retrospective study showed this combination to have a sensitivity of 65% for internal carotid disease which did not encroach on the lumen and for all other angiographically demonstrated lesions an average sensitivity of 94% and specificity of 92%. Augmented supraorbital pulses were more sensitive for detection of carotid artery disease than resting supraorbital pulses. Of the 61 lesions in which both resting and augmented supraorbital A/B ratios were obtained, the augmented supraorbital pulse detected 48 (79%) whilst the resting supraorbital pulse detected 29 (48%). Localised carotid bruits were poor indicators of carotid artery disease, having a sensitivity of only 27%.

Noninvasive in vivo detection of gaseous microemboli in the middle cerebral artery, by transcranial Doppler ultrasound, was used to determine the effect of filtration in the arterial catheter using 25- and 40-microns filters and bubble oxygenators in patients undergoing cardiopulmonary bypass surgery. Eighteen patients undergoing coronary artery bypass surgery were studied using a closed cardiac (unvented heart) model. Group 1 patients (no filters) had the highest incidence of gaseous microemboli, as indicated by the ultrasound microemboli index, at both high and low oxygen flow rates. Group 2 patients (40-microns filters) had a significantly lower microemboli index, particularly at low oxygen flow rates (t = 4.9, p less than 0.001). The 25-microns group patients had the lowest values of all. No microemboli were detected at low oxygen flow rates, and microemboli were detected in only 0.1% of the samples at high oxygen flow rates. Additionally, observations on vented hearts in 3 patients undergoing cardiac valve surgery indicate that the origin of gaseous microemboli may be air trapped inside the heart.

This paper describes the application of chromaticity to spectrally analysed Doppler blood velocity signals. Chromaticity enables a particular colour to be assigned to the power/frequency distribution within each instantaneous spectral sweep of a sonagram. Successive spectral sweeps of a sonagram can then be colour-coded in this fashion to produce a chromaticity-coded sonagram. Two chromaticity-coded sonagrams are presented to illustrate various blood flow features. Standard sonagram display formats are also presented for comparison purposes. It appears that the chromaticity-coded sonagram offers the ability to digitally encode the instantaneous power spectral distribution with a single chromaticity vector.

Transcranial pulsed Doppler ultrasound and spectral analysis were used for detection of blood velocities in the basal cerebral arteries. The Doppler transducer was placed superior to the zygomatic arch and during insonation of the middle cerebral artery care was taken to obtain maximum Doppler-shift frequency signals since this allowed a small angle between the ultrasound beam and this artery. Doppler signals were obtained from the middle, anterior, and posterior cerebral arteries in 20 volunteers with the average depth of the Doppler gate at 4.9 (4.6-5.2 cm), 5.2 (4.9-5.4 cm), and 6.3 cm (6.0-6.9 cm), respectively. These measurements were in agreement with those obtained for 15 cadaver studies, in whom the distance from the proposed site of the Doppler transducer to each basal cerebral artery was measured as 4.7 +/- 0.6, 5.3 +/- 0.5, and 5.9 +/- 0.9 cm, respectively. The reproducibility of middle cerebral artery blood velocity values was tested in seven subjects and showed a variation of not more than 8% in any individual. The method was used in combination with common carotid compression to assess four patients who had occlusive extracranial carotid disease; in three the disease was more severe on one side and reversal of blood flow in the proximal ipsilateral anterior cerebral artery was demonstrated, consistent with cross flow from the contralateral side via the anterior communicating artery of the Circle of Willis. In the fourth patient augmentation of posterior cerebral artery blood velocities during common carotid compression indicated the major collateral source was from the vertebrobasilar system.

Transcranial pulsed Doppler ultrasound and spectral analysis were used to monitor blood velocities in the middle cerebral artery of nineteen patients (mean age 61 +/- 9 years) during carotid endarterectomy. A Javid shunt was used in all patients. The intensity weighted mean Doppler frequency for each spectral sweep (at 5 ms intervals) was time-averaged over the cardiac cycle to obtain a mean value for blood velocity in the middle cerebral artery. The range of such values found in the 19 patients was: 12-38 cm s-1 after anaesthesia (baseline); 12-69 cm s-1 during diathermy; 0-30 cm s-1 during carotid clamping; 16-32 cm s-1 during shunting and 18-60 cm s-1 in the recovery room. The average change in middle cerebral artery blood velocity from baseline values showed significant increases during diathermy (P less than 0.005), shunting (P less than 0.05) and in the recovery room (P less than 0.005). Clamping of the internal carotid artery showed a significant decrease in middle cerebral artery blood velocities of all patients (P less than 0.005), three of whom showed no flow in the middle cerebral artery during clamping. Abnormally high amplitude Doppler signals at the commencement of shunting were detected in 17 of the 19 patients. Such Doppler signals are consistent with turbulent blood flow or the introduction of micro-air bubbles by the shunt. Backbleeding in the internal carotid artery before insertion of the shunt was associated with diminished flow in the ipsilateral middle cerebral artery of ten patients, oscillatory forward/reverse flow in three patients and cessation of flow in the remaining six patients.

Data are presented from transcranial insonation of the middle cerebral artery (MCA) performed at intervals in 23 unconscious children for whom the outcome was subsequently poor. Once an MCA signal had been observed over a 30 minute period with time averaged velocity less than 10 cm s-1 and/or a direction of flow index, DFI, defined as 1 minus the ratio of reverse to forward flow of less than 0.8, recovery to forward flow throughout diastole was never observed and no patient recovered brain stem reflexes. Recovery of forward flow in diastole, and of brain stem function, was seen in cases with time averaged MCA velocity in the range 10 to 25 cm s-1 and with reverse flow but a DFI of greater than 0.8 for short periods of time. All but one of the 13 children fulfilling clinical criteria for brain stem death had MCA signals with time averaged velocity of less than 10 cm/s and DFI of less than 0.8. This type of signal was not observed in five children who were left in a persistent vegetative state.

Blood velocities have been measured transcranially, at small Doppler angles, in the middle cerebral artery of normal volunteers. Cerebral blood flow was changed by varying carbon dioxide tension. In four volunteers, the relationships between arterial pCO2 and percentage change in intensity weighted mean, median, and maximum Doppler-shifted frequencies in the internal carotid and middle cerebral arteries were linear with slopes of 2.5 and 2.8% per mm Hg change in pCO2. In 38 volunteers, the relationship between end-expiratory pCO2 and time-averaged maximum Doppler frequency was linear over the range of pCO2 20-60 mm Hg with slopes of 2.5 and 2.9 percentage change per mm Hg, for internal carotid and middle cerebral, respectively. These results are very similar to those reported using direct methods of measuring cerebral blood flow. As the transcranial Doppler method is reproducible, this indicates that changes in middle cerebral blood velocity may be used to monitor changes in flow.

Doppler-shifted ultrasound and spectral analysis were used to assess the carotid arteries of 77 elderly stroke patients, which represented 154 carotid bifurcations. Severe carotid disease was demonstrated at 19 of the 154 carotid bifurcations and in eight of these complete occlusion of the internal carotid artery was present. In 12 of the 19 instances the severe disease was in the internal carotid artery associated with the stroke. Localized carotid bruits were present in five patients and only three of these were associated with severe disease of the internal carotid artery. Minor disease was detected in 80 of the 154 carotid bifurcations and no disease was detected at the remaining 55 carotid bifurcations. An abnormally low ankle/arm systolic pressure ratio, which indicated occlusive arterial disease of the legs, was obtained in 19 of the 43 patients examined. No relationship was found between the severity of carotid artery disease and the presence of haemodynamically significant disease of the lower limbs. Ultrasound measurement of aortic compliance gave significantly lower values in elderly stroke patients compared to elderly asymptomatic volunteers. Comparison of flow-velocity transit times from the stroke-affected and non-affected limbs showed no consistent effect of the stroke on vascular tone in the patients assessed.
