Tele-monitoring is an emerging concept in the field of medicine, thereby avoiding various complications that the patients face due to repeated visits like cross-infections and circumvents disadvantages that are faced by the medical person due to requirement of repeated rounds. At the present state of a pandemic, safe distancing is essential. In such a situation, tele-monitoring comes into play, bridging the gap between the patient and doctor at this tough time. Of the complications of diabetes mellitus, foot ulcers are the most dreaded complications, as they can progress at an alarming rate and can be very difficult to treat. Various modalities have been described in the treatment of diabetic foot ulcers. One such modality of phenytoin therapy uses the disadvantage of the drug, that is, gingival hyperplasia to the advantage of wound healing. We hereby report a case of diabetic foot ulcer managed with injection phenytoin sprayed topically over the wound.

The COVID-19 pandemic has changed the way plastic surgeons treat their patients. Lockdown was introduced and all elective surgeries were cancelled and only emergency procedures were done during the time of corona pandemic. Patients were followed up through telemedicine consultations using various mobile devices and web-based tools. Teaching programmes were also channelled through web-based tools in the form of webinars. Various measures were taken to uphold the norms of social distancing. Here we describe our experience regarding the way our department functioned at the time of corona pandemic.

Context: Burn injury is a complex trauma with multiple changes in physiology of the patient. Assessing prognosis of the burn patient is a subject of debate with various prognostic scores available. The presence of inhalational injury and percentage (total body surface area) of thermal burns are the most important factors affecting the outcome of the burn patient. The role of comorbidities is another very important factor and its importance cannot be undermined, especially with massive fluid shifts associated with resuscitation and body catabolism in the rehabilitative stage, respectively. Charlson Comorbidity Index (CCI) is an attractive scale that has stood the test of time as a scale to measure objectively the severity of comorbidities on the prognosis. Aims: The aim of the study was to understand the role of comorbidities have on the outcome of thermal burns. Materials and Methods: This was a retrospective analysis of burn patients admitted in the burns center over the past 1 year fulfilling the inclusion criteria. Descriptive analysis of patient records was done. Results: A total of 18 patients with comorbidities admitted in the burns center fulfilling the above-said inclusion criteria. Ten patients survived, while eight patients succumbed to the burn injuries. The average CCI of the survivor group was 2.2 (range: 1–4), whereas the mean CCI of the nonsurvivor group was 3.5 (range: 2–6). Conclusion: Comorbidities do have an impact on the prognosis of a burn patient, and objective assessment of prognosis of the burn injury must include the CCI as a tool to predict the outcome correctly and reliably.

Bacterial infection hinders wound healing. Important mechanism of which is growth in the dressing material itself. Wound dressings are of various types and they act by an individual mechanism, but none of which prevent growth of the organism in the wound dressing itself. In this case report, we would like to describe a dressing that can prevent microbial contamination of the wound, biofilm formation and contamination of the dressing itself. Background: Successful management of peritonitis is a challenge to surgeons inspite of various advancements in medicine and surgical techniques. The outcome depends on various factors and early intervention. This led to the development of disease severity grading systems that would help grouping the patients by individual risk factors and appropriately predict possible outcome.Methods: The prospective study was conducted in the Department of General Surgery, Government Mohan Kumaramangalam Medical College Hospital, Salem. The study population included 100 patients presenting with perforative peritonitis in the emergency department. Preoperative data like age, gender, preoperative symptom duration and operative data like appearance of exudate, extent of exudates, source of sepsis where noted. Tissue biopsies were followed up. Duration of hospital stay and other postoperative complications were also noted. Patients were followed up till their discharge. The study end point was at patient’s discharge or death.Results: Among the patients who had the maximum score >29, 11% of the patients belonged to age group beyond 50 years. Mortality was 7%. The specificity and sensitivity of morbidity prediction was 89% and 88.9% respectively. The specificity and sensitivity of mortality prediction was 91.40% and 100% respectively. Mortality was highest in patients with score >29.Conclusions: Mannheim peritonitis index is a simplified scoring system with variables that can be assessed preoperatively, intraoperatively and postoperatively, to assess mortality and morbidity and instil appropriate treatment protocols which will bring out a positive outcome in the patients and increase survival rate.

Background: Mesh repair has gained popularity among the surgical repair of hernias but has limitations. This study is being carried out to compare the effectiveness of Desarda’s no mesh repair, with Lichtenstein’s tension free repair.Methods: This prospective study was carried out in GMKMCH, Salem, over a period of 2 years. A total of 60 cases with inguinal hernia were included in the study. 30 patients were randomly subjected to Desarda’s technique and 30 patients underwent Lichtenstein’s repair. Breast asymmetry can result from unequal hypertrophy or neoplastic growth, leading fatty tumors to be clinically and radiographically difficult to differentiate from hypertrophy.2 Lipoma is one of the most common benign mesenchymal tumors derived from fatty tissue. Incidence ranges from 12 to 16% of all mesenchymal tumors. We present an unusual presentation of the huge right breast lump in male. A 46 years old male, painless giant sagging right breast. Underwent reduction mammoplasty. Excision biopsy revealed mature adipose tissue separated by thin fibro vascular septate with no neuronal component and malignant transformation. Huge lipoma in male are unusual, requires care evaluation for diagnosis. They associated with excellent prognosis, after successfully intervened. All surgeons should aware of this kind of presentation. To the best of our knowledge this is the first reported case of a huge lipoma of breast in a male. When considering for treatment, the challenging part is reconstruction the breast in full filling a normal symmetrical and aesthetic anatomic structure of the breast. various literatures.
