Aim: This paper reports on a two-part mixed methods scoping project, funded by the RCN Foundation, to review the available evidence and undertake a Delphi study of nurse-led interventions. Nurses in the field were surveyed to understand their role in promoting children and young people's mental health and emotional wellbeing across the UK. Methods: A scoping review of the literature was undertaken and a two-part Delphi survey was developed and managed. Findings: Results of the literature review were inconclusive. There was a paucity of literature, particularly in the UK, suggesting that nurses may be under-represented in the research seeking to make recommendations concerning their profession. 244 responses to the Delphi method survey were received. Responses confirmed the fundamental importance of the therapeutic relationship. They also suggested that nurse-led pre-crisis and early interventions which addressed the mental health and emotional wellbeing needs of children and young people were being extensively utilised. All countries and all regions appeared to have primarily community-based services. School-based services were perceived to be highly beneficial. Early interventions typically targeted levels of resilience, emotional regulation and the prevention of ongoing mental health problems. Respondents viewed these interventions as effective but often encountered organisational and practical challenges, including access to training and ongoing supervision.

COVID-19 has placed additional challenges on mental health services. Video consultations (VCs) have provided a short-term solution to lockdown restrictions but could also increase long-term capacity to meet the anticipated rising demand. A total of 7752 VCs were conducted over six weeks. Thematic analysis of 474 online survey responses identified twenty patient attributes that influenced staffs’ decisions to offer VCs. Their opinions were diverse, at times contradictory, and not always evidence based. There was reasonable consensus (and published evidence to support) of the probable suitability of VC for patients who: are IT savvy and suitably equipped, are teenagers, live in remote/rural locations, have caring responsibilities, have anxiety disorders or express a preference. No consensus was reached regarding eight attributes and there was a corresponding paucity of evidence, indicating the need for further research. Conversely, old age, paranoia, sensory impairment/communication difficulties, high risk and trauma/PTSD (posttraumatic stress disorder) were generally seen as contraindicated by staff, despite published evidence of success elsewhere. It may be possible to overcome staff’s reticence to offer these groups VCs. As staff are effectively the gatekeepers to VC interventions, it is important to understand and support them to overcome reservations that are contrary to the empirical evidence base. This will ensure that their initial anxieties do not become unnecessary barriers to services for those most in need. As with all mental healthcare, such decisions should be made collaboratively, and on an individual basis.

Background: In meeting the needs of individuals with\ud intellectual disabilities (ID) who access health services, a brief, holistic assessment of need is useful. This study outlines the development and testing of the Learning Disabilities Needs Assessment Tool (LDNAT), a tool intended for this purpose.\ud Method: An existing mental health (MH) tool was extended by a multidisciplinary group of ID practitioners. Additional scales were drafted to capture needs across six ID treatment domains that the group identified. LDNAT ratings were analysed for the following: item redundancy, relevance,\ud construct validity and internal consistency (n =1692);\ud test–retest reliability (n = 27); and concurrent validity\ud (n =160).\ud Results: All LDNAT scales were deemed clinically relevant with little redundancy apparent. Principal component analysis indicated three components (developmental needs, challenging behaviour, MH and well-being). Internal consistency was good (Cronbach alpha 0.80). Individual item test–retest reliability was substantial-near perfect for 20 scales and slight-fair for three scales. Overall reliability was near perfect (intra-class correlation =0.91). There were significant associations with five of six conditionspecific measures, i.e. the Waisman Activities of Daily Living Scale (general ability/disability), Threshold Assessment Grid (risk), Behaviour Problems\ud Inventory for Individuals with Intellectual Disabilities-Short Form (challenging behaviour) Social Communication Questionnaire (autism) and a bespoke physical health questionnaire. Additionally, the statistically significant correlations between these tools and the LDNAT components made sense clinically. There were no statistically significant correlations with the Psychiatric Assessment\ud Schedules for Adults with Developmental Disabilities\ud (a measure of MH symptoms in people with ID).\ud Conclusions: The LDNAT had clinically utility when\ud rating the needs of people with ID prior to condition-specific assessment(s). Analyses of internal and external validity were promising. Further evaluation of its sensitivity to changes in needs is now required.
