Resumo Este artigo discute limites e possibilidades da prática de um grupo reflexivo junto a homens autores de violência contra mulheres. Apoia-se em um estudo etnográfico que incluiu observação-participante e entrevistas com os facilitadores de um grupo realizado em uma organização feminista não governamental em parceria com a justiça criminal. A partir da discussão de uma cena emblemática, argumentamos a favor de um processo de trabalho dialógico como uma alternativa que pode expandir os limites da abordagem judicial: para dialogar com o grupo de pares, os facilitadores precisam levar em consideração a posição minoritária dos pontos de vista feministas no campo das masculinidades. Só esse passo permite cooperar com os participantes para que, a partir do compartilhamento espontâneo e genuíno de experiências, reconheçam o caráter inadequado de suas expectativas em relação às mulheres. Deste modo instaura-se uma dinâmica de cuidado entre homens, favorável à mitigação das violências e complementar da promoção da equidade de gênero. Abstract This article discusses limits and possibilities of the practice of a reflection group together with male perpetrators of violence against women. It is based on an ethnographic study, which included participant observation and interviews with the facilitators of a group held in a feminist non-governmental organization in partnership with the criminal justice system. Starting with the discussion of an emblematic scene, we argue in favor of this dialogue process as an alternative that could expand the limits of the judiciary approach. In order to enter into a dialogue with the peer group, the facilitators must consider the minority position of feminist viewpoints in the field of masculinity. This step alone makes it possible to cooperate with the participants such that, as they spontaneously and genuinely share their experiences, they come to acknowledge the inadequacy of their expectations regarding women. This way, a dynamic of care takes place between men, which is conducive to the mitigation of violence and serves as a complement to the promotion of gender equality.

A adesão do paciente ao tratamento é crucial para a efetividade dos programas de aids. O Programa Brasileiro recomenda monitorar a adesão e incorporar ao cuidado atividades de promoção, tais como atendimentos individuais com foco em adesão. Revisões sistemáticas apontam maior efetividade das intervenções complexas, que incluem sessões de conversas entre profissional-paciente: informativas, educativas e aconselhamentos motivacionais. Criticam: insuficiente explicitação das bases teóricometodológicas das intervenções; enfoque excessivo no nível individual com baixa incorporação do contexto sociocultural; insuficiência de informação acerca da fidelidade aos protocolos. Poucos estudos descrevem a \"intimidade\" das sessões. Planejada para o cuidado individual, uma modalidade de intervenção psicossocial de apoio à adesão teve seu protocolo implementado em ensaio clínico com pacientes adultos, com carga viral detectável, em serviço especializado do SUS, em São Paulo- Brasil. Suas bases teórico-metodológicas: Quadro da Vulnerabilidade e Direitos Humanos na dimensão psicossocial do Cuidado, filiados a uma perspectiva construcionista de psicologia social na saúde. Focada na noção de sucesso prático, a intervenção objetivou contribuir para que as pessoas construíssem formas de conviver melhor com o tratamento que lhes fossem mais convenientes. Baseada na interação profissional-paciente buscou intensificar a dialogia mediante a exploração dos sentidos intersubjetivos que as \"tomadas\" das medicações adquirem em diferentes cenas/cenários. Para avaliar qualitativamente a implementação selecionaram-se 12 casos (4 de cada uma das profissionais que conduziram a intervenção) entre os 44 pacientes do grupo experimental. A análise contemplou dois eixos interdependentes: a) fidelidade da implementação ao protocolo; b) qualidade dialógica das conversações. Apresentamse os resultados em três artigos. A variação nas modalidades e enfoques comunicacionais implementados permitiu a classificação em quatro níveis crescentes de fidelidade ao protocolo: nível 1 (3 casos), nível 2 (6), nível 3 (1), nível 4 (2). A inflexão na direção da dialogia não ocorreu plenamente. Princípios relacionais como solidariedade, não repreensão, foram bem desenvolvidos. As conversas valorizaram experiências singulares de dificuldades com o tratamento e produziram coentendimentos sobre o que acontecia cotidianamente. Entretanto, a decodificação de sentidos na compreensão \"dos problemas\" e criação de \"soluções\" priorizou o nível individual cognitivo-comportamental. Por exemplo, ao invés de dialogar sobre atrasos/perdas de doses em situações sociais relacionadas ao estigma/discriminação, o sentido \"problemas de memória\" prevaleceu na decodificação das falhas no tratamento. Foi incipiente a incorporação de dimensões socioculturais e programáticas ao entendimento das situações e construção/imaginação de estratégias para lidar com \"impasses\" que implicavam prejuízos à adesão. Mesclados à comunicação de enfoque cognitivo-comportamental ocorreram momentos dialógicos que se mostraram mais promissores à construção pelos participantes de enunciados de satisfação, bem-estar, intenções e mudanças práticas benéficas à adesão. Situações psicossociais dos pacientes influíram sobre a comunicação. Quanto mais complexas, desafiam \"o como dialogar\" sobre aspectos graves, sinérgicos e multidimensionais que prejudicam a saúde. São necessárias, mas insuficientes, estratégias que aprimorem a qualidade comunicacional e o enfoque psicossocial das intervenções em adesão nos serviços. Devem complementálas: a coordenação do Cuidado em equipe e ações clínicas e sociais, imediatas e objetivas, para mitigar situações que implicam graves vulnerabilidades, prejudiciais tanto à adesão ao tratamento da aids como à saúde integral Patient\'s adherence to treatment is essential to the effectiveness of AIDS programs. The Brazilian Program recommends monitoring adherence and incorporating activities to care that promote adherence, such as individual care focusing adherence. Systematic reviews point towards greater effectiveness of complex interventions, which include conversation sessions - educational, informative, motivational counseling - between the health professional and the patient. They criticize: incomplete explanation of the theoretical-methodological bases of interventions; excessive focus on the individual level, with low incorporation of the sociocultural context; insufficient information on the fidelity to protocols. Few studies describe the \"intimacy\" of the conversation sessions. Planned for the individual care, a modality of psychosocial intervention supporting adherence had its protocol implemented in a clinical trial with adult patients with detectable viral load in a specialized care service of the Brazilian Unified Health System (SUS), in São Paulo. The following theoretical-methodological bases were adopted: the Framework of Vulnerability and Human Rights in the psychosocial dimension of Care, affiliated to a social psychology constructionist perspective in health. Focused on the notion of practical success, the intervention aimed at contributing so that people would build ways of coping better with the treatment which was the most convenient for them. Based on the health professional-patient interaction, it sought to intensify dialogic in exploring intersubjective meanings that the medication intake acquires in different scenes/scenarios. In order to qualitatively evaluate implementation, 12 cases (4 cases of each one of the health professionals who conducted the intervention) were selected among 44 cases of the experimental group. The analysis comprised two interdependent axes: a) implementation fidelity to protocol; and b) dialogic quality of conversations. The findings are presented in three papers. Variation in the implemented modalities and communicational approaches allowed the classification in four levels of increasing fidelity to protocol: level 1 (3 cases), level 2 (6 cases), level 3 (1 case), and level 4 (2 cases). Inflection towards dialogic has not completely occurred. Relational principles, such as solidarity and no reprimand, were well developed. The conversations valued singular experiences of difficulties with the treatment. Furthermore, they produced co-understandings on what happened in the daily routine. Nevertheless, the decoding of meanings in understanding \"problems\" and creating \"solutions\" prioritized the cognitive-behavioral individual level. For example, rather than dialoguing on delays/abolishing doses of medication intake in social situations concerning stigma/discrimination, it prevailed the sense of \"memory problems\" in decoding treatment failures. Programmatic and sociocultural dimensions were incipiently incorporated to the understanding of contexts and construction/imagination of strategies to cope with \"impasses\" which implied adherence losses. Mingled with the communication of cognitive-behavioral approach, there were more dialogic moments which showed to be more promising to the construction by the participants of utterances regarding satisfaction, well-being, intentions, and beneficial and practical changes to adherence. The psychosocial situations of patients influenced communication. The more complex, the more they challenge \"how to have a conversation\" about multidimensional, synergistic, serious issues which harm health. It is necessary, but insufficient, to have strategies which enhance the communication quality and psychosocial approach of adherence interventions in care. The coordination of Care as a team and social and clinical actions, both immediate and objective, should complement them in order to mitigate situations which imply serious vulnerabilities that impair AIDS treatment adherence as well as the overall health

Scans from the apparatus concept coined by Michel Foucault, the Sexuality Apparatus focusing on homosexuality / homophobia thus identifies the knowledge-power relationships that have involved LGBT positively and negatively, through the truths built institutions of power and of subjectivity as breaking strength. Thus, it emphasizes the power of resistance of each individual reflects on the visibility as an instance of power production and discusses the unity of purpose to face the LGBT community rights violations suffered by non-heterosexuals A presente dissertação aborda os meandros de criação e implementação da principal política pública federal de combate à homofobia: o Programa Brasil sem Homofobia. Traz o cenário atual da luta pelos direitos humanos do segmento homossexual e os avanços e retrocessos no campo da educação e da cidadania de Lésbicas, Gays, Bissexuais, Travestis e Transexuais. Analisa a partir do conceito de dispositivo forjado por Michel Foucault, o Dispositivo da Sexualidade com enfoque na homossexualidade/homofobia, deste modo identifica as relações de saber-poder que têm envolvido LGBTs positiva e negativamente, através das verdades construídas, das instituições de poder e da subjetividade como força de ruptura. Assim, ressalta o poder de resistência de cada indivíduo, reflete sobre a visibilidade como instância produtiva de poder e discute a unidade da comunidade LGBT para fins de enfrentamento das violações de direitos sofridas por não-heterossexuais.

I conducted semistructured interviews which. main subjects addressed were: militant path, differentiation between travestis and transexuais, organization of the movement, the political agenda, body modifications and demands on health. In addition, I conducted ethnographic observations during meetings of the LGBT movement, as well as during meetings of Brazilian Trans Movement, especially the XVI and XVII National Meeting of Transvestites and Transsexuals (ENTLAIDS) held in Rio de Janeiro in 2009, and in Aracaju in 2010; the Fifth Regional Conference for Latin America and the Caribbean of ILGA (International Lesbian and Gay Association), held in Curitiba in 2009, and I National March Against Homophobia, held on May, 19th 2010 in Brasilia. Based on this work, I reconstruct the history of the Trans Movement in Brazil, where it is possible to locate the emergence of travesti and transexual as identity categories and the political conflicts they produce. My analysis focus on the relationship between theses identity categories and stigmatizing ones like prostitution, and especially on the capacity of incorporation or appropriation of medical and psychiatric discourses in the construction of social identities. This includes the influence of social class and access to specialized health services, notably the process of health care for transsexuals in the Brazilian public health system. Finally, the analysis of the main demands of the movement led me to establish a link between recognition policies and processes of purification. O objeto desta dissertação é explorar os discursos militantes relativos à diferenciação entre as identidades coletivas de travestis e transexuais no âmbito do movimento de travestis e transexuais brasileiro e sua repercussão nas demandas e proposições de políticas públicas de saúde, mais especificamente no que tange às transformações corporais. Realizei entrevistas semi-estruturadas com ativistas reconhecidas como lideranças desse movimento, tendo como principais eixos: trajetória militante, diferenciação entre travestis e transexuais, organização do movimento, agenda política, transformações corporais e demandas relativas à saúde. Além disso, realizei etnografias em encontros gerais do movimento LGBT, assim como nos específicos de travestis e transexuais, com destaque para o XVI e XVII Encontro Nacional de Travestis e Transexuais (ENTLAIDS), realizados no Rio de Janeiro em 2009 e em Aracaju em 2010; a V Conferência Regional para América Latina e Caribe da ILGA (International Lesbian and Gay Association), realizada em Curitiba em 2009; e a I Marcha Nacional Contra a Homofobia, realizada em 19 de maio de 2010 em Brasília. A partir dos resultados das entrevistas e das etnografias, busco construir um histórico do movimento de travestis e transexuais no Brasil, no qual é possível localizar a emergência das categorias identitárias e seus conflitos. Assim, minha investigação passa pela análise da relação com categorias estigmatizantes como a prostituição e, especialmente, das capacidades de incorporação ou de apropriação de discursos médicos e psiquiátricos na construção de identidades, o que inclui a influência de fatores de classe e de acesso a serviços de saúde especializados, notoriamente o processo transexualizador no SUS. A análise das principais demandas do movimento me levou a uma relação entre políticas de reconhecimento e processos de purificação. Por fim, busco compreender o processo de construção de uma carreira militante no movimento de travestis e transexuais.

Those relationships are marked by mediations called meetings with the power that, from the statement of the difference, open a new field of investigation of the transgender prostitute community in Brazil as a promotion strategy of self-care and of the exercising of citizenship. Existential mapping suggest elements that retell collective histories of transgender prostitutes, requesting new opportunities for discussion between governmental organizations and other sectors of our civil society in order to favor the elaboration of new public policies. A partir de observações etnográficas e entrevistas profundas junto às travestis brasileiras militantes, cartografamos histórias de vida que organizam cenas a respeito de suas relações na infância, adolescência e a vida atual, mapeando processos de estigmatização e suas respostas de enfrentamento que promovem a produção de uma cultura de resistência. Essas relações são marcadas por mediações denominadas encontros com o poder , que a partir da afirmação da diferença, inauguram um novo campo de investigação na saúde coletiva, mostrando a importância da organização social e política da comunidade transgênero no Brasil, como estratégia de promoção do cuidado de si e do exercício da cidadania. As cartografias existenciais sugerem elementos que recontam as histórias coletivas das travestis, solicitando novas possibilidades de diálogos entre os órgãos governamentais e demais setores da sociedade civil, de modo a favorecer o surgimento de novas políticas públicas.

Orientador: Gastão Wagner de Sousa Campos Tese (doutorado) - Universidade Estadual de Campinas, Faculdade de Ciencias Medicas Resumo: O propósito da tese foi mostrar, através de um estudo de caso na Cidade de Buenos Aires, a importância de focalizar os esforços de gestão em um nível tradicionalmente pouco valorizado: o dos trabalhadores da saúde. Um avanço qualitativo para a implementação das políticas em saúde reprodutiva poderia ser alcançado ao considerar esta dimensão. Apoiados nos desenvolvimentos teóricos que destacam a dupla finalidade produtiva dos sistemas de saúde públicos ¿ produzir saúde e produzir pessoas- desenvolvemos uma pesquisa a partir de um espaço de gestão, que teve como suporte empírico a realização de entrevistas a médicos infectologistas, ginecologistas e obstetras que trabalham em HIV/aids. O estudo mostrou que, além das estruturas formais de organização do sistema (leis, programas, relações de hierarquia), há outras lógicas que governam os sistemas, com alto poder de definição sobre a implementação efetiva das políticas. Também são descritos obstáculos que se apresentam aos médicos no momento de oferecer recursos de cuidado para o exercício da sexualidade. A reconstrução do processo de atenção às pessoas que vivem com HIV, permitiu detectar um ¿buraco negro¿ em matéria de saúde reprodutiva, curto-circuito que pareceria estar afetando também a população geral. Articulando desenvolvimentos do campo da gestão, a saúde reprodutiva e a sexualidade, o estudo busca fornecer insumos que melhorem as práticas em saúde atendendo à subjetividade de trabalhadores e usuários Abstract: The purpose of this thesis is to show, through a case study of Buenos Aires, the importance of focusing the management efforts on health workers sector. We consider that taking into account this dimension it is possible to improve the implementation phase of the reproductive health policy. Literature underlines the double productive goals of public health systems: on the one hand, to produce health and on the other, to produce people. Based on this theoretical development, we developed a research through depth interviews to physician, and people who live with aids. The study shows three main results. On the one hand, we found that formal rules (such as, laws, programs, hierarchical relationship) live together with other informal ones that govern the health system, with high impact on policy implementation. On the second hand, we identified some obstacles that physicians find when they need to talk about sexual practices. Finally, the reconstruction of the health attention circuit followed by people with aids, allowed us to detect a ¿black hole¿ in the reproductive health system, which also affects general population.
