Мета - проаналізувати клініко-епідеміологічні особливості корової інфекції в дітей у період спалаху 2017-2018 рр. в Запорізькій області для підвищення настороженості лікарів первинної ланки надання медичної допомоги щодо кору. Матеріали та методи. Проведено ретроспективний аналіз клініко-епідеміологічних особливостей корової інфекції в 58 дітей, які перебували на стаціонарному лікуванні в КНП «Обласна інфекційна клінічна лікарня» Запорізької обласної ради у 2017-2018 рр. Результати. 78,5% дітей госпіталізовані пізніше 3-ї доби хвороби. У катаральному періоді в усіх дітей спостерігалася гіпертермія та наявність плям Бєльського-Філатова-Копліка. Період висипки тривав 5,3±0,5 доби. У всіх хворих висип був плямисто-папульозним і поширювався зверху вниз із появою пігментації з 5-ї доби хвороби. Усі діти мали типову форму хвороби, у 91,4% випадків - середньотяжку, у 8,6% - тяжку. 91,4% дітей мали ускладнення. The single-pass one-step method for printing conductive silver tracks on a glass surface, using the laser-induced forward transfer (LIFT) technique, was proposed, providing a unique opportunity for high-throughput printing of surface micro- and nanostructures with high electrical conductivity and positioning accuracy. This method was developed via our multi-parametric research, resulting in the selection of the optimal material, laser irradiation, and transfer conditions. Optical, scanning and transmission electron, and atomic force microscopy methods, as well as X-ray diffraction, were used to characterize the surface structure and phase state of the printed structures, while energy-dispersive X-ray and X-ray photoelectron microscopy were employed for their chemical microanalysis. Depending on the laser irradiation parameters, the specific electrical conductivity of the printed tracks varied from 0.18 to 83 kS/cm, approaching that of donor magnetron-sputtered films. This single-pass one-step method significantly facilitates fast, large-scale, on-demand local laser printing of metallic (sub)microcomponents of microelectronic devices.

The topical issue of pediatric surgery is the treatment of ovarian cysts based on the development of new technologies for surgical correction of the disease. Aim. To analyze the effectiveness of using a new minimally invasive method for removal of cystic ovarian neoplasms in children. Material and methods. A retrospective analysis of 77 female patients with ovarian cysts, aged from 3 months to 17 years, was carried out. The patients were divided into 3 groups depending on the various surgical techniques used for their treatment. Group I consisted of 32 (41 %) girls who underwent cystectomy according to the new transabdominal technique for removing ovarian cysts in children. Group II included 19 (25 %) children who underwent a laparoscopic surgery. Group III comprised 26 (34 %) children who underwent a hybrid laparoscopy-assisted cystectomy. The following indicators were analyzed: the age of patients, the nature and number of complications of the disease course, the size of cystic neoplasms, the duration of surgical interventions, the number of cases accompanied by cyst contents leaking into the abdominal cavity during surgical manipulations, and the length of hospital stay. Results. The mean age of all patients was 11.23 ± 0.57 years. Planned hospitalizations amounted to 46 (59.7 %) cases. The duration of surgery in group I was almost 1.5 times lower (P < 0.05) than that in patients of group II and 2 times shorter than in children of group III (P < 0.05). Free cyst contents leaking into the abdominal cavity was observed in 35 (91.4 %) patients of Group III, which required additional measures aimed at the abdominal cavity sanation. The proposed minimally invasive transabdominal surgical technique prevented this complication in all 32 patients of Group I. The length of hospital stay did not differ significantly (P > 0.05) between children groups I and II and did not exceed 7.50 ± 0.35 (M ± SEM) days. This indicator was almost 1.5 times higher in Group III patients. Conclusions. The new minimally invasive transabdominal method for removing cystic ovarian neoplasms in children provided minimal trauma with maximum ablasticity and cosmetic effect of the operation. The proposed technique made it possible to prevent the development of intraoperative and postoperative complications, significantly reduce the duration of surgery and the recovery time of patients.

Мета — показати доцільність застосування ганцикловіру при внутрішньоутробній цитомегаловірусній інфекції (ВЦМВІ), навівши два клінічні випадки. Клінічні випадки. З часом змінювалися підходи до лікування ВЦМВІ із застосуванням ацикловіру до призначення ганцикловіру. У 2011 р. у недоношеної дитини на основі наявності поліорганності ураження (легенів, печінки, селезінки, підшлункової залози, очей, центральної нервової системи) і позитивних результатів полімеразної ланцюгової реакції (ПЛР) на ЦМВ у крові, сечі та лікворі діагностовано ВЦМВІ. В якості етіотропного препарату використано лише ацикловір у зв'язку з відсутністю на той час доказової бази щодо безпечності застосування специфічного препарату ганцикловіру. Лікування виявилося неповністю ефективним: у катамнезі у 2-річному віці дитина має грубе порушення слуху та зору і глибоку інвалідизацію за рахунок резидуальних явищ із боку центральної нервової системи. У 2019 р. у новонародженої дитини із тяжким геморагічним синдромом, респіраторними порушеннями та неврологічною симптоматикою за результатами позитивної ПЛР крові на ЦМВ діагностовано ВЦМВІ та призначено ганцикловір із розрахунку 6 мг/кг кожні 12 год внутрішньовенно під контролем показника гемограми та кліренсу креатиніну. З другого тижня лікування відмічено позитивну динаміку клінічно, з 6-го тижня — негативну ПЛР крові на ЦМВ. Дитину виписано на 70-ту добу лікування без резидуальних явищ із боку центральної нервової системи. В 9 місяців засвідчено туговухість ІІ–ІІІ ступеня та вчасно проведено протезування з повним відновленням слуху. В однорічному віці: дитина сидить, перевертається, повзає; адекватно реагує на оточуючих; фіксує погляд та стежить за предметами. Висновки. Два клінічні випадки через описання змін підходів до специфічної терапії вродженої ЦМВІ з ацикловіру до ганцикловіру та наочну демонстрацію різниці виходів захворювання від тяжкої інвалідності при відмові від ганцикловіру до повної реабілітації в разі його застосування показують доцільність призначення саме ганцикловіру. Дослідження виконано відповідно до принципів Гельсінської декларації. На проведення досліджень отримано інформовану згоду батьків дітей. Автори заявляють про відсутність конфлікту інтересів.

Optical-range bulk diffraction nanogratings were fabricated via challenging direct inscription by ultrashort (femtosecond, fs) laser pulses inside heat-shrinkable polymers (thermoplastics) and VHB 4905 elastomer. The inscribed bulk material modifications do not emerge on the polymer surface, being visualized inside the materials by 3D-scanning confocal photoluminescence/Raman microspectroscopy and by the multi-micron penetrating 30-keV electron beam in scanning electron microscopy. The laser-inscribed bulk gratings have multi-micron periods in the pre-stretched material after the second laser inscription step, with their periods continuously reduced down to 350 nm on the third fabrication step, using thermal shrinkage for thermoplastics and elastic properties for elastomers. This three-step process allows facile laser micro-inscription of diffraction patterns and their following controlled scaling down as a whole pattern to pre-determined dimensions. In elastomers, utilizing the initial stress anisotropy, the post-radiation elastic shrinkage along the given axes could be precisely controlled until the 28-nJ threshold fs-laser pulse energy, where elastomer deformation ability is dramatically reduced, producing wrinkled patterns. In thermoplastics, the fs-laser inscription does not affect their heat-shrinkage deformation up to the carbonization threshold. The measured diffraction efficiency of the inscribed gratings increases during the elastic shrinkage for the elastomers and slightly decreases for the thermoplastics. High 10% diffraction efficiency was demonstrated for the VHB 4905 elastomer at the 350 nm grating period. No significant molecular-level structural modifications were observed by Raman micro-spectroscopy in the inscribed bulk gratings in the polymers. This novel few-step method paves the way for facile and robust ultrashort-pulse laser inscription of bulk functional optical elements in polymeric materials for diffraction, holographic and virtual reality devices.

Гемолітико-уремічний синдром (ГУС) є важким ускладненням бактеріальних та вірусних діарейних захворювань, однією з причин розвитку у дітей гострої ниркової недостатності, а ефективність його лікування та результат прямо залежить від своєчасності діагностики. Мета: продемонструвати особливості перебігу ГУС у дітей раннього віку з інфекційною діареєю. Матеріали і методи. Проаналізовані два випадки ГУС, асоційованого з діарейним синдромом, що був проявом ротавірусної інфекції у одного хворого та проявом токсикозу у другого хворого, а також результати проведеного обстеження і терапію із зазначенням складності діагностики та лікування. Результати та висновки. В обох випадках захворювання починалось з діарейного синдрому з гемоколітом. У дитини з ротавірусною інфекцією на тлі токсико-ексикозу та гемоколіту прояви ГУС розвинулися на 4-у добу захворювання, а у дитини з гострою кишковою інфекцією нез'ясованої етіології з гемоколітом прояви ГУС розвинулися на 6-у добу лікування. У подальшому хворим проводилося обстеження та лікування в умовах відділення анастезіології, інтенсивної терапії та хроніодіалізу (ВАІТ та ХД) із проведенням гемодіалізу. Хворим було проведено 17 та 14 сеансів гемодіалізу, відповідно. Для виключення тромботичної тромбоцитопенічної пурпури проводилися дослідження в лабораторії Доктора Рьодгера на наявність ADAMTS-13-антитіл. Після проведеного лікування діти були виписані на 56-й та 51-й день. Через три та шість місяців після виписки із стаціонару дітям було проведено повне контрольне обстеження, яке не виявило відхилень від норми за всіма раніше зміненими показниками. Стан їх стабільний. Результати катамнестичного спостереження за дітьми протягом вісьмох місяців свідчать про клініко-лабораторну ремісію. Протягом цього часу діти приймають з метою нефропротекції інгібітори АПФ. Діти залишаються під диспансерним спостереженням із періодичним контролем лабораторних показників протягом наступних п'яти років. Дослідження виконані відповідно до принципів Гельсінської Декларації. Протокол дослідження ухвалений Локальним етичним комітетом установи. На проведення досліджень було отримано інформовану згоду батьків дітей. Автори заявляють про відсутність конфлікту інтересів.

Вроджена цитомегаловірусна інфекція (ЦМВІ) є провідною причиною вроджених інфекцій немовлят у світі, оскільки може призвести до значних рівнів захворюваності, смертності або довгострокових наслідків. Мета — навести результати скринінгового обстеження вагітних і дітей перших місяців життя щодо ЦМВІ; описати клінічний випадок тяжкої вродженої ЦМВІ з труднощами добору специфічної терапії. Матеріали та методи. Проведено ретроспективний аналіз специфічного обстеження 302 вагітних у різних термінах, які мешкають у Запорізькій області. Оцінено наявність специфічних до цитомегаловірусу (ЦМВ) антитіл класів M і G. Ретроспективно оцінено клінічний перебіг ЦМВІ у 56 дітей раннього віку. Описано клінічний випадок вродженої ЦМВІ у новонародженого з тяжким ускладненим перебігом (гепатит, нефрит, кардит, пневмонія, тромбоцитопенічна пурпура), якому призначено специфічну терапію ганцикловіром. Статистичну обробку отриманих результатів виконано методами варіаційної статистики. Результати. За результатами дослідження специфічного імунологічного профілю до ЦМВ вагітних показано, що більшість жінок фертильного віку, мешканок Запорізької області, були серопозитивними до ЦМВ. У близько 3% вагітних жінок фіксували в крові позитивні IgМ проти ЦМВ, що вказало на прояв гострої форми ЦМВІ і можливе інфікування плода. Серед 35 (59,3%) пацієнтів першого року життя домінуючим клінічним симптомом вродженої ЦМВІ була затяжна неонатальна жовтяниця (у 15 дітей виявили підвищений рівень АЛТ, в 1 дитини — тромбоцитопенію). Другим за частотою реєстрації був синдром перинатального ураження центральної нервової системи. З метою визначення особливостей перебігу, полісиндромності ураження, труднощів діагностики та добору специфічної терапії за тяжких форм вродженої ЦМВІ наведено клінічний випадок. Висновки. Більшість жінок фертильного віку є імунними до ЦМВ та близько 3% вагітних переносять гостру первинну форму ЦМВІ. ЦМВ — найчастіша причина внутрішньоутробного інфікування з тяжким загрозливим життю новонародженого станом, із можливим формуванням тяжких резидуальних наслідків, частіше у вигляді сенсоневральної глухоти. Діагностика вродженої ЦМВІ ускладнюється як полісиндромністю проявів хвороби, так і відсутністю чіткої системи тестування вагітних й новонароджених. Настороженість лікарів-неонатологів і педіатрів щодо ЦМВІ — запорука своєчасної діагностики хвороби новонароджених. Труднощі добору етіотропної терапії при ЦМВІ зумовлені низькою чутливістю збудника до ацикловіру та необхідністю використання токсичного препарату — ганцикловіру. Своєчасне призначення високоспецифічної анти-ЦМВ-терапії дає змогу не тільки зберегти життя пацієнту, але й зменшити ймовірність тяжких резидуальних явищ з боку нервової системи, органів зору і слуху! Дослідження виконано відповідно до принципів Гельсінської Декларації. Протокол дослідження ухвалено Локальним етичним комітетом зазначених у роботі установ. Atopic dermatitis (AD) is a chronic recurrent inflammation of the skin with a prevalence 10-15% in children worldwide. The pathogenesis of AD is complex and insufficiently studied. Patients with AD are sensitive to viral, bacterial and fungal skin infections, because of the skin barrier function defection in an acute period. Irreversible changes of the skin barrier structure and transformation of the skin microbiota, excessive overgrowth of bacteria and fungi lead to chronization of AD. Early detection of the skin superinfection in AD may prevent chronization in early childhood. The purpose is to study the role of conditionally pathogenic fungal strains in the course of AD in children. Methods. 98 children with AD (age 3 months – 3 years) were. Transepidermal water loss, skin pH meas-urements and microbiological study were performed. Results. A group of 56 children among the 98 patients with AD had signs of a complicated course of AD due to rash and its localization, as well as the ineffective therapy with moisturizers, anti-inflammatory drugs and elimination diet, which is the first stage in the AD treatment according to the ICON recommendations. This rash was regarded as a possible fungal superinfection. 12 children out of the 56 had microbiologically confirmed contamination with fungi: Candida albicans (n = 6), Candida nonalbicans (n = 3), Hormodendrum compactum (n = 2), Aspergillus nidulans (n = 1). Detection of Spp. Malassezia did not give positive results. 89% of the children (n = 50) had verified changes of the skin pH (5.8). The patients with nontypical rash, changed pH ( 5.8) had an effective treatment with antifungul drugs and achieved remission in AD. Conclusion. Morphologically altered elements of moderate to severe rash should be regarded as a possible fungal super infection. Revealed changes in the skin pH and the background of altered clinical manifestations of AD (peeling spots with raised or scalloped edge, double contour, clear demarcated, cracks with blood secretion) should be regarded as activation of fungal microorganisms which is considered to be ethiotropically treated due to the guidelines.

Aim: to delineate the clinical presentation and laboratory profile of primary Epstein-Barr viral infection (EBVI) in children. Materials and methods. A retrospective study was conducted on the demographic, clinical and laboratory data of 104 children with infectious mononucleosis due to Epstein–Barr virus (EBV) infection in children. The analysis included serologically confirmed cases – the presence of IgM to the capsid EBV antigen (VCA IgM) with the absence of antibodies to the nuclear EBV antigen (EBNA IgG). Results. The mean age of children was 4.0 (2.6; 9.0) years. The maximum hospital morbidity for infectious mononucleosis was observed in children from 2 to 5 years – 35.6 % whereas in patients from 12 years to 17 years – 19.2 %. The mean peak temperature was higher in young children. The rate of the cervical lymphadenopathy was significantly higher in adolescents compared with infants (P < 0.001). Tonsillitis syndrome was more commonly seen in children aged from 2 to 11 years (P < 0.05). Rash was detected in 22.1 % (n = 23) of children. The presence of petechiae was more common in older children. Hepatosplenomegaly syndrome was reported in 50 % of older children and only in 8.7 % of infants. Cytolytic syndrome was reported in 54.2 % of patients from 6 to 11 years and in 100 % of the adolescents from 12 to 17 years, although in infants it was only in 34.8 % (P < 0.001). Atypical mononuclear cells were detected in 8.7% of early age children and in 24.3–37.5 % of older children. Conclusions. Age differences were noted in clinical findings for cervical lymphadenopathy and splenomegaly, which were less common in children of the first 2 years. A significant difference in the incidence of hepatitis was found: with increasing age of the patient, the mean level of ALT increased significantly and gradually.

Вітряна віспа на сьогодні є важливою медико-соціальною проблемою через можливий розвиток ускладнень, зумовлених як прямою дією вірусу, так і приєднанням вторинної бактеріальної інфекції. Ускладнені бактеріальною інфекцією форми вітряної віспи в Запорізькій області реєструвалися у 56 (16,0%) пролікованих хворих. Серед ускладнень частіше зустрічалися: піодермія — у 24 (42,8%) дітей, пневмонія — у 13 (23,2%), лімфаденіт — у 5 (8,9%), гострий тонзиліт — у 4 (7,1%), середній гнійний отит — у 3 (5,4%), слизово-гнійний кон'юнктивіт, блефарит — у 3 (5,4%), флегмона — у 2 (3,6%). У 2 (3,6%) дітей зафіксовано поєднання вітряної та скарлатини. Мета — навести рідкісний клінічний випадок перебігу в дитини суперінфекції — вітряної віспи зі скарлатиною. Клінічний випадок. Проведено клініко0лабораторне спостереження за дитиною 11 років, яка отримувала лікування з приводу нетипового перебігу вітряної віспи протягом 20 діб стаціонарного етапу. Описаний клінічний випадок нетипового перебігу вітряної віспи в дитини розцінено як суперінфекцію з агресивною бактеріальною флорою. Це вказує на можливість розвитку генералізованої форми стрептококової інфекції (скарлатини) як суперінфекції при вітряній віспі, на необхідність своєчасної діагностики вітряної віспи та насторожі лікарів щодо приєднання вторинної бактеріальної інфекції в такому випадку, а також на доцільність вакцинації кожної дитини проти вітряної віспи. Дослідження виконано відповідно до принципів Гельсінської Декларації. Протокол дослідження ухвалено Локальним етичним комітетом зазначеної в роботі установи. На проведення досліджень отримано інформовану згоду батьків дитини.

Менингит – острое инфекционное поражение мозговых оболочек, которое вызывают бактерии, вирусы, грибы или простейшие. В этиологической структуре преобладают серозные менингиты. На современном этапе самые тяжелые последствия серозного вирусного менингита ассоциируют с герпесвирусным поражением ЦНС. Цель: выявить особенности течения серозных менингитов герпетической этиологии у детей и взрослых. Материалы и методы. В статье представлен ретроспективный анализ историй болезни 25 пациентов, которые в течение 2018–2019 гг. лечились в КУ «Областная инфекционная клиническая больница Запорожской ОГА» по поводу серозного менингита. Всем пациентам было проведено определение с помощью полимеразной цепной реакции ДНК герпесвирусов (ВЭБ, ВПГ 1 и 2 типа, ЦМВ) в ликворе. По результатам обследования больные были разделены на две группы: I группа – 11 больных с менингитом герпетической этиологии; II группа – 15 больных с серозным менингитом неустановленной этиологии. Результаты и выводы. 73,4% больных I группы были госпитализированы с диагнозом «Менингит?», среди пациентов II группы 6 (40%) больных были с диагнозом ОРВИ, тонзиллит и лихорадка неясной этиологии. Герпетическая этиология заболевания чаще регистрировалась в старшем возрасте, а в группе серозных менингитов другой этиологии было достоверно больше больных в возрасте 10–18 лет (р=0,02). Герпетическое поражение проявлялось более яркой менингеальной и очаговой симптоматикой в дебюте заболевания. При лабораторном исследовании у больных менингитом негерпетической этиологии достоверно чаще имел место лимфоцитоз крови (р=0,04) и лимфоцитарный плеоцитоз ликвора с количеством клеток от 100 до 300. У пациентов с герпетическим менингитом достоверно чаще были снижены хлориды ликвора (р=0,02), отмечался низкий (до 100 клеток) или очень высокий (более 300 клеток) лимфоцитарный плеоцитоз. У больных герпетическим менингитом достоверно чаще сохранялись повышение температуры тела до субфебрильных цифр (р=0,09), нистагм (р=0,04) и симптом Кернига. Исследование было выполнено в соответствии с принципами Хельсинкской Декларации. Протокол исследования был одобрен Локальным этическим комитетом учреждения.

Вірусом Епштейн–Барр інфіковано понад 95% населення планети. Поліморфізм клінічних проявів цього захворювання обумовлює труднощі діагностики. Мета: на клінічному прикладі показати багатогранність проявів інфекційного мононуклеозу у дітей. Матеріали і методи. Проаналізовано клінічні прояви нетипового важкого перебігу інфекційного мононуклеозу у дитини раннього віку на етапі стаціонарного лікування. Результати та висновки. У дитини з нефропатією в анамнезі інфекційний мононуклеоз перебігав у важкій формі зі значною гепатоспленомегалією та виразним набряковим синдромом та за відсутності типових симптомів у вигляді тонзиліту, системної запальної реакції та лімфаденопатії. Це ускладнило діагностику і вимагало проведення додаткових лабораторних досліджень та включення до терапії глюкокортикостероїдів. Наведений клінічний випадок демонструє поліморфізм клінічних проявів захворювання та труднощі діагностики важких випадків інфекційного мононуклеозу за рахунок виникнення нетипових синдромів — полісерозиту та набрякового синдрому з нівелюванням типових проявів хвороби. Дослідження виконані відповідно до принципів Гельсінської Декларації. Протокол дослідження ухвалений Локальним етичним комітетом установи. На проведення досліджень було отримано поінформовану згоду батьків дитини.

Introduction. In this article the main emphasis of the research were done on the theoretical and practical approaches to the military budget expenditures. The military finance is one of the biggest components of the financial system of Ukraine. So, according to the nowadays situation and military conflict in Ukraine we searching a new ways to increase this kind of expenditures. Purpose. That’s why the problem of data evaluation and comparison is so popular today. When we are talking about NATO integration and cooperation, we need to understand and compare financial statement. Our purpose in this article was to find out that comparison basis for Ukraine and the other countries according to SIPRI and WMEAT approaches. It could become the source for future researches. Results. In results of the research, we should mention that time series analysis of the budget in national currency could be done only in national currency, but according to the article, inflation should be taken into account. But the most complex approach is international currency evaluation during the discount time. Another valuable thing is the way of the total military expenditure estimation, because it’s not only the Military Forces. In the process of work, it was concluded that alliances, with the involved countries, has an impact on expenditures. The general situation in the regions and geopolitical goals play a significant role. It is expedient to carry out an analysis of cost-effectiveness using expert approaches and econometric models that take into account not only GDP but also the overall state of the economy. This is because the cost effectiveness does not depend on their volume. Conclusions. The analyses conducted by world economists argue that military spending assumes the role of a financial instrument against the recession and the stagnation of the economy by increasing effective demand or solving the problem of scarcity of consumption. Military expenditures create high alternative costs for consumption, investment, and other public expenditures. Moreover, military spending a serious burden on the budget and accelerates inflation. In particular, all these preconditions concern the implementation of economic analysis of the defense spending, their share in the state and their quality and orientation, such as the defense budget. We can conclude that it is necessary to construct complex econometric models of estimating the budgets of neighboring countries and regions, but they will need to take into account the theoretical and practical aspects of price determination, inflation, purchasing power parities, and different approaches to evaluating and converting budgets of different countries.

Introduction. Military conflicts cause a lot of destructive influences not only on the political situation in the country but of course on its the financial system. That’s why in this article, the main emphasis of the research was done on risk-ratio analysis of the Ukraine and Israel economies since the role of military budget expenditures in the whole financial system is growing. Military finance is one of the biggest components of the financial system of Ukraine. So, according to the nowadays situation and military conflict in Ukraine appears the necessity of searching for new ways to increase this kind of expenditures efficiently. Aim and tasks. The problem of the research was to measure risks of the economies according to the GDP trends and to find the place of the military expenditures in all government expenditures. Results. In the research results, it should be mentioned that the most difficult approach for comparing is the international currency, which is weighed against the depreciation of cash flows in time. However, the research used the current exchange rate for convenience. But, this does not have a significant effect on the interpretation of the results of the research, presented in the form of a CAPM analysis of the economies of countries and their defense expenditures. After all, trends are generally observed over a period of time that has been analyzed. The general state of financial systems in the regions was analyzed as the value of volatility, aggregate risks and the overall growth of the economy. The US GDP, as for an example of the largest economy in the world, served as a comparative basis. It correlates and affects all other financial markets. The obtained results made it possible to draw conclusions about the attractiveness of the state for potential investors, despite armed conflicts with neighbors in the territory and areas of influence on the regions under control. In this paper, a standard statistical analysis package was used using an example of linear regression with one variable. The power of the influence of military expenditures was measured by the ratio of variables and their correlations. The obtained results were characterized by a high level of interconnection power and reliability of the hypotheses obtained. This gives an opportunity to describe the role of military conflicts and support the Armed Forces for the state economy. Conclusions. The result of the presented research of the role of support and financing of the Armed Forces as one of the key elements of the stability of the Ukrainian economy and ensuring its sustainable growth is the sustaining of the military expenditures for Ukraine. The presented conclusion was approved on the Israel example and calculations. This approach and methodology is universal and can be used for other researches.

Background. Food hypersensitivity (FH) remains actual medical problem. Aim of the work was to study the prevalence, correlation of fecal calprotectin (FC), biological marker of the mucosal inflammation, with age, sex, course of the skin symptoms of FH in children. Materials and methods. One hundred two patients (51 (50 %) boys, 51 (50 %) girls) with skin symptoms of FH were included into the study, mean age was 25.36 [12.00; 60.00] months. FC was detected by Calprotectin RAPIDTEST (Italy). Results. Calprotectin was detected in feces in 45 % of patients (n = 46), 13 % of them were children aged up to 1 year, 47.82 % — from 1 to 3 years. The number of children with positive FC test didn’t differ in different age groups (Kruskal-Wallis ANOVA test H (4, n = 102) = 4.45, p > 0.05). FC test was more often positive in boys, but statistically significant gender difference was detected only among children aged 1–3 years (c2 = 9.09, р 0.05) and sex (j = 0.006, p > 0.05), age of the first symptoms and duration of the disease (r = +0.06, p > 0.05 and r = +0.02, p < 0.05, respectively). Conclusions. 45 % of children with skin symptoms of FH have organic changes in the intestinal wall, which was proved by biological marker — FC. Such changes have no correlation with age, sex, age of the first symptoms and duration of the disease.

Проаналізовано роль веб-аналітики як діяльності, яка спрямована на проведення аналізу користувачів веб-ресурсу. Незважаючи на велику кількість зарубіжних і вітчизняних праць стосовно дослідження веб-аналітики та її інструментів, потрібно констатувати, що в науковій літературі так і не сформовано усталеної, повноцінної дефініції цього поняття. Визначено та схарактеризовано методи веб-аналітики. Методи дають можливість створювати більш гнучкі засоби оцінювання, а також розраховувати ризики на основі статистичних даних, зроблених у досліджуваному середовищі з урахуванням періоду часу й поведінки користувачів. Застосування методів дає змогу відображати результати як у кількісній, так і в якісній формі. У цій роботі проаналізовано та обґрунтовано важливість використання веб-аналітики для проведення моніторингу відвідувачів сайтів. Завдання й цілей цього дослідження досягнуто аналізом інформаційних джерел із цієї теми. Проведено порівняльний аналіз інструментів веб-аналітики, визначено їх переваги та недоліки. З’ясовано особливості й випадки використання аналітичних систем. Виокремлено проблеми, які розв’язуються за допомогою застосування веб-аналітики та її інструментів. Ідентифіковано критерії, яких потрібно дотримуватися проводячи аналіз відвідувачів сайтів за допомогою таких інструментів веб-аналітики, як Google Analytics і Yandex Metrika. У статті розкрито та узагальнено сильні й слабкі сторони обраних засобів веб-аналітики. У процесі наукового дослідження застосовано такі методи: наукове узагальнення, аналіз інформації, узагальнення, порівняння. Практичну цінність цієї роботи становить порівняльний аналіз інструментів веб-аналітики. Отримані результати та окремі положення праці можуть бути використані в подальших дослідженнях із цієї тематики.

Background. Further search for the ways to correct the key links of food hypersensitivity (FH) skin symptoms remains perspective. Aim of the study was to evaluate pathogenetically grounded therapy of the skin FH symptoms in children. Materials and methods. From 2011 to 2016, after analyzing source medical records of the 1780 children aged 1 month to 18 years, 148 eligible patients were randomly divided into 2 groups of treatment. Group 1 (n = 88) was administered pathogenetically grounded therapy for 10 days, which was aimed at the correction of epidermal and mucosal barrier dysfunction; group 2 (comparison) received standard treatment. Results. Median and interquartile variation of the SCORing Atopic Dermatitis results in group 1 were statistically lower (p < 0.05). Exacerbations (U[79; 53] = 1492.5; p = 0.001) and duration of the skin symptoms persistence (U[79; 53] = 1516.5; p = 0.001) were rare as compared to the group 2. Children with onset of the symptoms before 12 months of age had better prolonged effect (p < 0.05). Assessment of the relative humidity of the skin enables to control and to predict the incidence and duration of the FH skin symptoms equally. Conclusions. Pathogenetically grounded therapy of the skin FH symptoms in children had both short- and long-term effects regardless of clinical phenotype of the rash.

The article presents the analysis of diagnosis and treatment of 67 patients with gastrointestinal stromal tumors. The mostfrequent location was the stomach, the esophagus took the second place. Among the complications, gastrointestinal bleeding is leading, taking place in half of all cases. The tumor (GIST) develops from the interstitial cells of Cajal and is potentially malignant, the signs of which are established. The incidence of 1 million population is 10-20 cases. The expression of the proto-oncogene c-kit in GIST is detected by an immunohistochemical reaction with the CD117 marker. An erroneous diagnosis in a number of cases was a pseudocyst of the pancreas, found in a CT scan. Tumors bigger than 2 cm in diameter are subject to surgical treatment, smaller ones are recommended for dynamic observation. Organ-preservation operations without lymphodissection are considered acceptable in the course of surgical treatment. Target therapy with tyrosine kinase inhibitors (imatinib, sunitinib, etc.) is used. Two people received chemotherapy in the postoperative period. Histological examination of tumors in most cases detected spindle cell and epithelioid cell variants, in some cases there was a mixed version. GISTs remain the mostfrequent mesenchymal malignant neoplasms of the esophagus, stomach, small intestine, colon and rectum. The article demonstrates variants of GIST in 4 patients with localization of the process in the upper gastrointestinal tract, treatment options, when the final diagnosis was established during surgery with histological and histochemical studies.

Background. Food hypersensitivity (FH) is commonly diagnosed in children. Positive skin tests and elevated serum IgE levels indicate anaphylactic mechanisms of allergy symptoms, although in some cases they do not correlate with clinical manifestation. Thus, the search for the effective approaches to the correct adjusted use of these methods in concordance with symptoms remains important. The purpose of our study was to evaluate age features of the IgE-dependent FH in children, which was proven with skin prick- and patch-tests, specific IgE (sIgE) levels. Materials and methods. The study included 148 patients aged 1 month to 18 years with skin symptoms of FH. The next methods were used: questioning, skin prick- and patch-tests, serum sIgE immunoassay detection. Results. 69.5 % of patients were children of early age — up to 3 years. Sensitization to the cow milk prevailed — 43 % (n = 10). 10 % of children had positive patch-tests with food allergens. Most part of children with non-IgE-dependent FH was 4–6 years old. Ele­vated total IgE levels were detected in 49 % of children. SIgE was increased in 42 % (n = 25) of children with normal total IgE, sIgE to cow milk and egg prevailed. Only in 63 % of children, high sIgE le­vels were clinically relevant and were proved with skin tests. The higher was children’s age the less was sIgE clinical relevance. Conclusions. Diagnostic algorithm of FH in children under and over 3 years should be different and take into account possible formation of the natural oral tolerance and decreased prevalence of the IgE-dependent reactions.

Завдання та цілі цього дослідження досягнуті завдяки аналізу інформаційних джерел із цієї теми. Ідентифіковано заходи, які потрібно вжити, описано важливість безпеки ІС. У процесі наукового дослідження застосовано такі методи: метод наукового узагальнення, метод аналізу інформації, узагальнення. Практичну цінність цієї роботи становить детермінація заходів безпеки ІС організацій. Результати дослідження можуть бути використані в подальших дослідженнях із цієї тематики.

It has been a clear increase in the number of enteroviral infection (EI) activation in recent years. EI are the widespread agents of infectious diseases in children and adults. Moreover this virus affects various organs and tissues and it results in different symptoms of EI in patients of different age groups. All these facts lead to the difficulties in diagnostics, treatment and prophylactics of the EI clinical forms. Aim of the study was to improve diagnostics of the enteroviral infections (EVI) in children and adults based on the epidemiological features of the definitive clinical forms in season 2016. Materials and Methods. The study was carried out as a part of the Grant Agreement between Zaporizhzhia State Medical University (ZSMU) and "Regional Training Centre of Research in the HealthCare System" (theRepublic ofKazakhstan). 185 patient’s medical documentation was analyzed in the inpatient department. Only symptomatic patients with clinical manifestation of enteroviral infection different forms were included into the study. Polymerase chain reaction (PCR) was used for enteroviruses detection. Results and Discussion: Enterovirus was identified in biological liquids in 36.7 % (68 patients) of cases. Patients with meningitis (35) prevailed among them. Other 9 patients had: exanthema, gastroenteritis, herpangina. An increase in the incidence of morbidity has been noted in August - November with the prevalence of meningitis in August – September, herpangina – in November. Most frequently EVI was detected in children aged 7 – 12 years and in young children. Meningitis dominated in school-aged children, exanthema – in children up to 3 years old. Conclusions. EVI is widespread cause of meningitis, vesicular or ulcerative lesions of the posterior oropharyngeal structures and exanthemas in young and early school-age children. The article presents the results of epidemiological surveillance of acute flaccid paralysis and the level of vaccination against poliomyelitis coverage in the Zaporizhzhia region in 2012–2016.

The article presents the results of the analysis of clinical symptoms of escherichiosis, which were caused by different serotypes of Escherichia coli in children living in the Zaporizhzhia region in 2010–2016 years. 178 cases of escherichiosis were registered in children aged 1 month to 15 years, among whom 104 (57.14 %) were children under the age of 3 years. Enteroinvasive (serotype O124 and O144) and enteropathogenic E.coli (serotype O18 and O44) were dominated in the structure of the escherichiosis in older children, and in the children of the first 3 years of life, it were enterotoxigenic (O25 and O126, O6) and enterohemorrhagic E.coli (O26 and O11). The disease course was clinically favorable, and no severe and complicated course was recorded.

Actuality. Food hypersensitivity (FH) remains an important problem of the childhood that has medical and social sides. For the decades, classification of FH skin manifestations is based on the combination of not clear pathophysiological and clinical signs and is caused by insufficient use of the method of morphological determination of rash elements and its comparison with a past medical history. Objective. To propose updated definitions and the project of classification of clinical manifestations of food hypersensitivity that will enable to improve clinical significance, to identify targets of possible therapeutic intervention at different pathogenetic mechanisms of FH, to increase the efficacy of clinical diagnostics, to develop common approaches to the data interpretation. Materials and methods. Advanced review of the literature sources, both paper and electronic (including Medline and the Cochrane Library, 2010–2016), as well as the results of own researches served as the basis for the creation of the project. Results. The current classification of FH is ambiguous and reflects the lack of understanding of the nature of the disease, which may have immune and non-immune mechanisms. Problems with verifying the diagnosis are also due to the fact that the same symptom in the skin can occur at different pathophysiological mechanisms of development, that is why it is important to take into account anamnesis and subjective symptoms, which differ in specific forms of FH. The article offers the draft classification of clinical manifestations of FH, based on recent literature data and the results of own observations. Target organ and pathogenetic mechanism, some morphological elements are taken as the basis. Conclusions. The proposed method will improve the clinical significance and make it possible to create a unified approach to the data interpretation.

The patient’s destiny often depends on the pediatrician infectious disease specialist’s ability to appraise the abdominal pain. In this article we analyzed the incidence of development of the intussusception in children with dyspepsia, also we represented the clinical features of this surgical disease.

У період 2012–2016 рр. у клініці дитячої хірургії м. Запоріжжя спостерігали 329 дітей з гемангіомами різноманітної локалізації. Застосовувались наступні методики лікування: кріодеструкція (33%), склерозуюча терапія (26%), оперативне втручання (30%), терапія пропранололом (11%). У 37 дітей з великими та проблемними гемангіомами лицьової ділянки та промежини виконано лікування пропранололом. У дітей, лікування яких розпочато в проліферативній фазі розвитку гемангіоми, ефект розвивається раніше та він є виразнішим, ніж у фазі стабілізації. Лікування великих гемангіом та гемангіом з високою проліферативною активністю, які локалізуються в проблемних зонах, рекомендовано починати з пропранололу. Інші методи можуть бути застосовані як один з етапів комплексного лікування.

Background. It is well known that allergy is genetically predispose. But gene change takes much time, ages. Some researches have doubts that it occurred during last two decades, but allergic pathology spread increased in several times more and continue to grow. That’s why the cause should be searched in the epigenetic influences (environmental, food, ets.). More than the half of the modern hypothesis concerning allergy spread are associated with food (7 from 13). EAACI data showed that quantity of children with self reported food hypersensitivity and positive provocation test results decrease with age. But specific IgE levels to food showed other pattern: it increases in young school children in two times and decreases in elders. We supposed different pathogenetic mechanisms of food intolerance (FI) in school children of different ages. The aim of this study was to assess prevalence, risk factors and features of the FI in children of different school age. Materials and methods. 250 children 6-17 years old were included into the study. All of them were citizens from the industrial region of the Zaporizhia city. 217 respondents returned questionnaire. 137 of them undergone physical examination. Questionnaire was created on the base of the ISAAC survey and included questions on the life style, food, early age. For the assessment of the skin symptoms severity SCORAD scale was used. Transepidermal water lost was estimated with humidity meter (Queentone,France). Skin pH metria was performed with pH-meter (Ezodo,Taiwan). Statistical analysis was made with Microsoft Exel, Statistica 6.0 programs. Results and discussion. 217 respondents returned questionnaire (86,8%). 129 (59,4%) of them were males, 88 (40,6%) - females. 85 of children (38,6%) – were pupils from the elementary school (age 6-10 y.o.), 82 children (37,7%) – from the 5-8 classes (age 10-14 y.o.), 50 (23,7%) - 10-11 classes (age 15-17 y.o.). More than ½ of the respondents (54,3%) had skin food allergy (FA) symptoms in the anamnesis and/or at the exam. And only half of them (28,1%) were consulted by doctor and had verified diagnosis. Quantity of the respondents with eruptions in the elementary school exceeded so in the elderly. That corresponds to the modern tendency of the allergy spread during last decades. Except genetically determined factors big importance had: food during pregnancy and in early childhood, vit D and probiotics intake. Controversial effect had environmental factors and other vitamins. Conclusions. Only 37,7% of children could name cause of the eruptions. More often it were: citruses, chocolate, honey, strawberry, milk, eggs. Citruses and chocolate mostly cause dose-dependent reactions. Quantity of this reactions increases with age. More than ½ of the children with FA in elementary school had family allergic anamnesis. Hereditary had influence only in children of early school age, but in older age quantity of such patients with reported FI was not more than 30%.Epigenetic influences had bigger influence in this age. FA was reported frequently in families where parents were employed in the industrial factory and rarely in ones where mothers during pregnancy and lactation were on the diet. The most frequent phenotype of children with anamnesis and/or symptoms of FI was dry skin. Algorithm of the FA prophylaxis was proposed. Further perspectives. Deep allergologic examination should be done for further distinguishing FI.

The increase in the incidence of the food allergy (FA) occurs more rapidly than changes in the genome sequence. To investigate the impact of genetically predisposed skin barrier function abnormalities associated with filaggrin (FLG) genes polymorphism on the FA onset and course, we studied 53 children with skin FA symptoms. Genotyping was performed for FLG variants R501X and 2282del4. Results. The frequency of 2 variants in the study population was 0.018 for R501X and 0.075 for 2282del4. The combined allele frequency was 0.096. All patients were heterozygotes. Children with mutations showed significant odds ratio (OR) of 31.3 (95% CI 2.1; 452.0) in terms of early onset of the skin allergy symptoms — during 1st month of life. And had no influence on the FA clinical signs start after 12 months of age (OR 0.5 (0.05; 4.85), p < 0.05). Null mutations were clinically important risk factors for the persistent transepidermal water loss (TEWL). Conclusion. Genetically predisposed skin barrier function abnormalities have impact on early FA onset and are important for persistent TEWL.

Clinical reasoning formation is impossible without correct organization of the teaching process. Aim of the study was to indicate didactic approaches to teaching pharmacotherapy to students in pediatrics, to define methodological principles of the practical lessons planning. Methods of the study: theoretical analysis of the methodical and medical literature, achieved experience. Results. Analysis enabled to indicate main approaches to the organization of the teaching process following principle from simple to complex. The integration of forms of training and supervision within the faculty and hospital didactic approaches is expected. The need of individualization, the requirements for knowledge of the principles of pharmacoeconomics and evidence-based medicine is considered. Conclusions. An integrated approach, maintaining continuity of didactic teaching approaches will consolidate the knowledge and skills of the student.

The article highlights the clinical and immunological features of infectious mononucleosis in children (based on the analysis of the data for children of different ages treated in Zaporizhya regional hospital of infectious diseases in connection with infectious mononucleosis in 2007–2012). The analysis of the results of the immunological and microbiological examination of relevant patients is given.

Resume. It is less known about role of the genetically predisposing factors in the onset and course of the food allergy (FA). We aimed to determine the association of the polymorphism in SPINK5 gene with FA. Materials and methods. DNA genotypes were identified in children aged 2 months – 5 years with FA skin symptoms (n=53) depending on the presence of the guanine or adenine in both alleles in the 1258 position of the 14th exzone. Results. Genomic frequencies of the SPINK5 polymorphism site were 15 (28.3%) for AA, 29 (54.7%) – AG, 9 (16.9%) – GG. Abnormal barrier gene STAT5 did not influence on the severity and kind of the FA symptoms, comorbidity. Children with AA

Erythema nodosum is rare pathology in childhood and usually associated with big antigen load. Aim of the work was to make differential analysis of the described case with developed multiple, nontender, depth erythematose papules in the epidermis-dermis (like nodules) on the head, trunk, extremities, associated with adopted cow's milk-based formula intake. Methods and results. The next methods, as deep literature review and differential analysis, helped to underline probable pathogenetic mechanisms of such type of allergy skin symptoms, to diagnose Erythema nodosum in the child. Conclusion. This clinical case showed presence of rare clinical food allergy skin symptoms that have mixed mechanisms of onset. Further studies of the gear of onset and histology specificity are needed.

Actuality of the study of the food allergy (FA) in children is caused by it’s polisystemic and polispecific clinical symptoms. Food allergy – conception more pathophysiological, than clinical. There are several diseases in the international diseases classification that are associated with FA concept. All they have similarity – allergic inflammation in the basis, other are differences up to target-organ. These facts prove need to work up new diagnostic and treatment strategies for FA management. The aim of our work was to find out difficulties in FA diagnosis. Materials and methods. We had analyze indexes of the work of the allegrocenter in the Zaporizhia city multifield children hospital in 2011-2013 years. Case histories of the 67 children with FA were studied. All patients passed questionnaire about clinical symptoms and their manifestations. Results. In spite of the decrease in demographic indices allergological morbidity prolongs to grow. We founded out the next difficulties in the management of the FA patients. Most part of children are in the outpatient department. Frequently practitioners persistent skin elements diagnose as atopic dermatitis although it can be true food allergy or food intolerance or atopic exema. Each of these conditions require separate special treatment and further recommendations. Sometimes children with gastroenterological symptoms of the FA are treated against infections due to the similarity of clinical sighs to the intestinal infectious invasion. Questionnaire of the 67 children with food allergy and their parents showed that about 13% of them at least once had specific gastroenterological food allergy symptoms, that were caused by food intake and repeated in time periods after recurrent food intake. Conclusion. Conceptional and clinical difficulties associated with food allergy in children requires further studies of the mechanisms of it’s occurrence. This will allow to increase quality of the FA diagnostics and treatment.

The laser-induced damage threshold (LIDT) was measured for a Z n G e P 2 crystal exposed to 0.3-9.5 ps 1030-nm laser pulses. Single-pulse LIDT fluence was ∼0.22 J / c m 2 for the laser pulse widths of 0.3-3.5 ps and increased until 0.76 J / c m 2 for 9.5-ps pulses. Multi-pulse LIDT fluence for 0.3-ps pulses at repetition frequencies in the range of 100 Hz-1 kHz was ∼0.053 J / c m 2 and decreased further at higher, multi-kHz, pulse repetition frequencies. The coating of the Z n G e P 2 crystal surface with an anti-reflection multi-layer thin film increased the multi-pulse LIDT by one order of magnitude, up to 0.62 J / c m 2 (about 2 T W / c m 2 ). The significant increase in LIDT coupled with a decrease in reflection losses provides a way to cardinally improve efficiency of frequency conversion of popular 1-µm ultrashort pulses into mid- and far-IR ranges with a thin AR-coated Z n G e P 2 crystal sample.
