Abstract Background Mining occupies a prominent place in Brazil, which, if observed, means that one must work with the contingencies that arise from its activity. Mining disasters, such as those in Mariana and Brumadinho, exemplify the impact on the health system and are models for similar situations, so the study sought to investigate the impact of these disasters on hospital admissions in the Brazilian public health system. Methods Through segmented regression, we sought to assess possible changes in the variables HAA (authorized hospital admissions), total admission value, and mortality rate in Mariana and Brumadinho. This measurement method allows the researcher to identify changes during the study period. Results The​​​​​​​ study observed significant changes in the variable mortality rate in the city of Brumadinho. Although the other variables, both in Mariana and Brumadinho, do not present a level of significance compatible with possible effects, we can still say that they present a trend that can be inferred as an effect of the disaster. Conclusions The mining disaster significantly changed the mortality profile in the city of Brumadinho, with implications for the health system. In Mariana, there have been no objective changes, but there is evidence of potential impacts.

Introdução A Biblioteca Cochrane é uma referência global no apoio à tomada de decisões em saúde englobando ensaios e respostas clínicas. A síntese de vários estudos científicos de qualidade, selecionados por critérios de inclusão e exclusão, torna possível a elaboração de uma revisão sistemática. Atualmente, os estudos publicados no Cochrane Database of Systematic Reviews (CDSR) são disponibilizados virtualmente através da internet, sendo o meio pelo qual pesquisadores buscam, avaliam e selecionam esses estudos, para a elaboração de uma revisão sistemática. Entretanto, um dos fatores que leva à demora na seleção de estudos é ter que abri-los um por um dentro das palavras-chave, o que potencializa uma seleção quantitativamente menor. O presente estudo propôs o desenvolvimento de um programa de computador capaz de navegar por todo o CDSR e exportar parágrafos e todos os outros elementos dos estudos em um banco de dados estruturado e direcionado ao pesquisador. Métodos O estudo foi conduzido no Núcleo de Avaliação de Tecnologias em Saúde (NATS) da Associação Paulista para o Desenvolvimento da Medicina (SPDM) e segmentado em duas partes, sendo a primeira o protótipo do programa de computador através de scripts nos navegadores Web e a última a consolidação do programa final, escrito na linguagem de programação Python e compatível com os principais sistemas operacionais do mercado. Resultados Como teste final, o programa de computador desenvolvido foi capaz de extrair todos os nove mil artigos presentes no CDSR e exportá-los num banco de dados preparado para ser visualizado com programas de planilhas, como o Microsoft Excel. Com o sucesso, o núcleo aprovou o programa. Discussão e conclusões O programa passou a ser utilizado como auxílio na seleção de estudos pelos pesquisadores do núcleo, os quais puderam solicitar as extrações com base em palavras-chave ou termos dispostos em quaisquer parágrafos dos textos dos estudos. A extração automática de todos os estudos presentes no CDSR levou cerca de um dia, sem requerer qualquer intervenção dos usuários, um tempo anteriormente tido como “impensável” pelos pesquisadores encarregados de fazer a busca manual dos estudos.

Abstract Background Disasters are events that bring with them effects that contribute to the disruption of the normality of a population and thus highlight the vulnerabilities of the health system. In Mariana and Brumadinho, the collapse of the dam of ore tailings brought with it several impacts that were felt in the short term and will be felt in the medium and long term. And that by being intensely intertwined with issues of economic and productive nature, has as its meaning an uninterrupted result of its activities. Methods Through the DATASUS database, two specific variables were chosen to perform the analysis: the approved amount and the approved value. For this research, a methodological device, the segmented regression line, was used to observe the influences that the disasters that occurred in Mariana and Brumadinho had on the ambulatory health systems. Results The results of the segmented regression line show that, with Mariana, the amount approved continued to grow throughout the period, which shows that there was no change because of the disaster. There was a reduction in spending. In Brumadinho, regarding the amount approved, there was an upward trend in the disaster’s month, which did not change immediately afterwards, and regarding expenditure, the growth pattern was maintained in all three periods. Corroborating this data, the relative and absolute base elements show an increase in the amount approved and in the number of services provided at various posts compared with Minas Gerais. Conclusions Based on the findings, it was possible to understand that although disasters exert an influence that may have some effect on the health system, the lack of significance sometimes cannot be interpreted as a lack of impact on the disaster. The segmented regression line outlines some effects that are not conclusive but indicative of a numerical interpretation and a trend interpretation.

Abstract Introduction Accidents at work are events that endanger the health and life of workers. They are considered a public health problem, being the object of studies and actions in the Brazilian health system. The collapsed of the ore tailings dam in the municipality of Brumadinho, Brazil, in January 2019, shocked the world due to the proportion of environmental and human damage caused. In this context, concern for the health of workers gained prominence. This paper evaluated the impact of the collapse of the mining tailings dam in Brumadinho, on notifications of occupational health problems before and after the disaster. Methods An observational, longitudinal and retrospective study, of the time series type with a statistical approach was carried out on notifications of work-related injuries and diseases, available in the local database of the Notifiable Diseases Information System between January 2017 and December 2021. Results During the study period, 520 notifications of work-related injuries and conditions were registered. Of this total, 67.3% were serious work accidents, 26.0% mental disorders, 12.3% accidents with biological material and 2.9% repetitive strain injuries/musculoskeletal disorders, occupational dermatoses and induced hearing loss by noise. Occurrences were more frequent in 2019, the year in which the mining disaster occurred, recording 65.2% of total notifications. Abstract Introduction Social participation is one of the guidelines of the Brazilian health system. Health councils are collegiate instances of participation established by Law 8.142/90. The most recent legal regulation for council organization and functioning was established through Resolution 453/2021. The institution of health councils has a permanent and deliberative nature to act in the formulation, deliberation and control of health policy implementation, including in economic and financial aspects. Objective To evaluate the compliance of health councils with the directives for the establishment, restructuring and operation of the councils from Brazil, based on Resolution 453/2012. Methods An exploratory, descriptive study that used the Health Council Monitoring System as a data source. Qualitative variables were selected to identify the characteristics related to the councils’ establishment (legal instruments for establishment), the strategies adopted for restructuring (budget allocation, existence of an executive secretariat, provision of a dedicated office) and the characteristics of the health councils’ operation (frequency of regular meetings, existence of a board of directors, the election of the board of directors). Results The study analyzed three groups of characteristics related to the constitution, strategies adopted for restructuring and the functioning of the councils. Regarding the constitution of the councils, the findings revealed that the vast majority was constituted in accordance with the legislation and, therefore, is in compliance with Resolution 453/2021. In the second group of characteristics that describe the restructuring of councils, the study found that less than half of registered councils are in compliance with the standard. And, finally, in the third group of characteristics, it was found that the boards have adopted different frequencies for regular meetings and approximately 50% of the boards studied have a board of directors. Conclusions The councils still do not meet the minimum conditions necessary to fulfil their role in the Unified Health System (SUS), as stipulated in Resolution 453/2021. This situation requires monitoring by public oversight agencies. Despite the increase in popular participation with the creation of the health councils, this study demonstrated that most councils still do not meet the minimum conditions for monitoring public health policy. The improvement of the Health Councils Monitoring System (SIACS) to become an instrument for monitoring the councils, with the definition of goals and results, may contribute to the organization of the councils and, therefore, to the realization of social participation in Brazil.

Our society has advanced in terms of technology, and health could not be different. Despite the benefits and advantages that such improvements entail, it is unknown what contributions have been added to the hospital environment and whether such technological engineering has managed to generate value and adapt to different factors within such institutions’ professional culture to establish relevance to the base of utilitarian nature. The use of tools can be conditioned to the view that the managerial sectors have such instruments. The work aims to identify and understand the perception that health managers have traceability tools such and their view on their efficiency and effectiveness in the hospital environment. The results direct us that the traceability tools have a significant expression in the hospital context, collaborating for efficiency and efficacy. Traceability tools can help the entire health system to be more uniform in service, in accountability, and in inspection processes.

Resumo O crack é uma droga devastadora pelos efeitos que provoca. É um desafio para os profissionais da saúde que atuam na prevenção e no tratamento dos usuários, e para os professores dos ciclos básicos. O acesso à informação é uma forma eficiente para combater a droga e suas consequências. A pesquisa buscou identificar as Tecnologias de Informação e Comunicação utilizadas por esses profissionais para auxiliar no conhecimento sobre o crack e no desenvolvimento de programas de prevenção ao consumo e tratamento dos dependentes. Foram usadas metodologia qualitativa, amostra intencional por critérios e técnicas de investigação por meio de entrevistas semiestruturadas, triangulação da análise e informantes-chave. A fonte de busca de informações preferida pelos informantes-chave e profissionais foi a internet, onde se concentram as fontes de informação sobre o crack. A literatura pesquisada não reflete um escopo amplo sobre a área específica, comprometendo a capacitação e a ampliação de atuação desses profissionais.

Abstract: Background: Brazilian Unified Health System presents a long waiting period for a dermatology appointment, varying from 34 to 239 days. Objectives: Analyze the prevalence of cutaneous diseases evaluated by dermatologists in a specialized center, in patients referred from the primary care, and to present possible interventions for the primary units. Methods: Retrospective analyses of the International Codes of Diseases (ICD-10) described in every dermatology appointment in a specialty center in Sao Paulo from January 2014 to August 2015. Results: A total of 7.350 consultations were included. Superficial mycosis corresponded to 1,058 (14.4%) of the main complaints and dermatophytosis was the most frequently used ICD, corresponding to 481 individual consultations (6.5%), followed by onychomycosis, responsible for 464 consultations (6.3%), acne in 347 (4,7%). and contact dermatitis in 311 consultations (4,2%). Study limitations: The study was based on retrospective analysis of ICD described and no previous orientation for a solid use of the codes was performed to the dermatology team; consultations in which the ICD was not informed or a non-especific ICD was used were excluded; different dermatologists were responsible for the consultations. Conclusion: Superficial mycosis corresponded to 14.4% of the chief complaints in the studied period and was the most frequent cause of reference from primary care doctors to dermatologists. Prevalence data obtained in the present study could assist the capacitation policies in the primary care system, focusing the dermatology teaching in the most prevalent dermatological disorders.

OBJECTIVE: To verify whether health councils in Brazil carry out permanent education activities for municipal, state and federal district councilors. METHOD: This was a cross-sectional study with secondary data collection in the Health Council Monitoring System (Sistema de Acompanhamento dos Conselhos de Saúde - Siacs) from May to August 2017. The Siacs is publicly accessible and available on the internet. It provides data from thousands of health councils throughout Brazil. Analysis and interpretation of the data were based on the literature and the enacted legislation, particularly Resolution 453/2012 and the National Policy of Permanent Education for Social Control in the Unified Health System (Política Nacional de Educação Permanente para o Controle Social no Sistema Único de Saúde). RESULTS: Despite the fact that Resolution 453/2012 establishes the deliberation, elaboration, support and promotion of permanent education for social control as functions of the councils (in accordance with the guidelines of the National Policy of Permanent Education), approximately 40% of councils do not carry out permanent education. CONCLUSIONS: It is necessary to strengthen the role of health councils in the elaboration of educational initiatives across the national territory. This includes the allocation of financial resources to increase access to and participation in these initiatives, which would strengthen social control in the Unified Health System. This study emphasizes that the discussion of permanent education is not given sufficient attention in the agendas and routines of health councils. This compromises the effectiveness of councils’ monitoring and deliberation of public health policy.

OBJECTIVE: To analyze the public expenditures of states on health care and the participation of states and the Federal District in financing the Unified Health System, better known by the acronym SUS. To develop the research, two targets were used: “to rescue expenses per government source (federal, state and municipal) during the period from 2002 to 2013” and “to rescue resource transfers from the federal SUS to the states and also to municipalities”. METHODS: This research is bibliographic, documentary and descriptive and used a quantitative approach. Data were extracted from the Information System Public Health Budget, and additional data were collected from the public managers of states, municipalities and the Federal District during the period from 2002 to 2013. Federal data from the Undersecretary of Planning and Budget (originally extracted from the Integrated System of Financial Administration of the Federal Government and available on the Budget Public Health System webpage) were also collected. RESULTS: The data revealed that during the same researched period, the Federal District has maintained the health care system budget, whereas states and municipalities have increased their budgets for the same spending. CONCLUSIONS: By analyzing the results, there is clearly a disparity regarding the investment expended by the entities of the Federation. Although municipalities and states have gradually increased their application of resources to health care, the federal state has maintained the same budget. These results reveal a bit of concern about public health funding.

Os objetivos deste trabalho são descrever e analisar a gestão da assistência farmacêutica em alguns municípios, produzindo mais evidências sobre problemas que podem comprometer o acesso a medicamentos no Sistema Único de Saúde – SUS. Realizou-se pesquisa exploratória, transversal, elaborando-se questionário com questões fechadas sobre a gestão da assistência farmacêutica. O tamanho da amostra foi definido em 431 municípios, selecionados por amostragem aleatória estratificada. Dos 431 municípios, apenas 36 responderam o questionário (8,4%). Destes, 44,4% (n = 16) informaram ter a assistência farmacêutica formalizada, das quais 93,4% (n = 15) contam com pelo menos um farmacêutico. Dos 36 municípios, 97% (n = 35) informaram ter lista de medicamentos para dispensação à população. O controle de estoque foi considerado adequado em 26 deles (72,2%). Estão cumprindo determinação judicial para o fornecimento de medicamentos 14 (38,9%) municípios e em 13 deles (36%) todas as Unidades Básicas de Saúde – UBS contam com farmacêutico. Os resultados reforçam a necessidade do SUS de avançar no fortalecimento da gestão da assistência farmacêutica a fim de garantir o acesso a medicamentos e a efetividade das ações de saúde.

Background: The aim of this study is to identify the information and communication technologies used by health professionals to assist in training and updating of technical and scientific knowledge about crack, exchange of experiences, and development of programs to prevent consumption and treatment of addicts. Methods: The qualitative methodology was used, constructed an intentional sample by criteria and applied research techniques through semi-structured interviews, triangulation of the analysis, and key informants. The study resulted in the presentation of differences between the way key informants and health professionals sought information. Results: Internet was the preferred source; however, key informants sought information on sites of scientific journals and reference centers, while health professionals did free searches on the internet to consume information. Conclusions: The literature does not reflect a broad scope of the specific area, but relates the problem of access to health information to other characteristics. The sources of information about crack are focused on digital technologies, the internet and its specific tools. It also finds that health professionals are not being capacitate solidly on the subject of study.

Fundamento: A busca pela qualidade exige ferramentas de avaliação nas diversas subdivisões de um complexo de saúde. Na medicina diagnóstica eles são escassos e em ergometria não foram encontradas sugestões de indicadores. Objetivo: Estabelecer indicador para controle de qualidade em ergometria baseado nas III Diretrizes da Sociedade Brasileira de Cardiologia Sobre Teste Ergométrico; verificar o percentual dos testes que apresentaram o indicador dentro da conformidade em dois serviços de uma mesma instituição, antes e após a publicação do documento. Métodos: Foi realizada análise crítica das diretrizes em busca de indicador que apresentasse: exatidão, confiabilidade, simplicidade, validade, sensibilidade e capacidade de medir quantitativamente as variações no comportamento dos critérios de qualidade e que fosse aplicável a todos os testes. O indicador foi aplicado nos testes de 2010, anterior à publicação e 2011, depois que ele foi adotado, por dois serviços de uma mesma instituição. Resultados: O indicador que preencheu os critérios foi o de percentual de exames ergométricos com duração do exercício entre 8 e 12 minutos. Nos anos 2010 e 2011, respectivamente, os percentuais de testes ergométricos dentro da conformidade foram 85,5% e 86,1% (p = 0,068) no Hospital Geral, e 81,5% e 85,7% (p < 0,001) no Serviço de Avaliação Periódica de Saúde. Conclusão: O tempo do exercício entre 8 e 12 minutos pode ser utilizado como critério de qualidade em ergometria e nos serviços onde ele foi aplicado, pelo menos 80% dos testes ergométricos estiveram conformes.

OBJETIVO : Analisar o papel da ouvidoria e sua contribuição para a gestão da saúde pública segundo usuários de sistema de Saúde e de conselheiros municipais de saúde. MÉTODOS : Pesquisa qualitativa, estudo de caso, descritivo e transversal. A unidade de análise foi uma ouvidoria de saúde, localizada em município do estado de Minas Gerais, de maio a agosto de 2010. O estudo foi de natureza observacional com dados coletados em entrevistas com dois grupos de stakeholders: usuários e conselheiros de saúde. Foram entrevistados 44 usuários do Sistema Único de Saúde que registraram manifestações presenciais na ouvidoria e todos os 20 conselheiros do município. As informações obtidas foram analisadas com base em três questões: (1) natureza das informações obtidas; (2) discussão sobre subsídios para qualificar o funcionamento da ouvidoria como ferramenta de gestão; (3) proposição de ações para o aprimoramento da gestão democrática no campo da saúde pública. RESULTADOS : As demandas reportadas à ouvidoria indicaram dificuldade de acesso às ações e serviços de saúde, correndo o risco de serem percebidas como atalhos para obtenção de acessibilidade, desconsiderando o princípio da justiça social. A atuação da ouvidoria contou com a aprovação dos cidadãos. Os usuários relataram como principais funções da ouvidoria: ajudar a solucionar problemas de saúde e ouvir e esclarecer sobre o funcionamento do Sistema Único de Saúde. A informação foi enfatizada pelos conselheiros de saúde como instrumento de poder e de acesso aos direitos dos usuários do Sistema Único de Saúde. Destacaram que a ouvidoria tem como tarefa garantir justiça na efetivação da política de saúde e que desempenha importante papel de mediação entre o Conselho de Saúde, os cidadãos e os gestores do sistema municipal de saúde. Além disso, atribuíram à ouvidoria caráter executivo que usualmente ela não abriga. CONCLUSÕES : A ouvidoria é uma importante ferramenta de gestão e atua na fiscalização do funcionamento do sistema de saúde. A implantação da ouvidoria é um avanço no campo da gestão democrática. Existem desafios a serem superados para que as ouvidorias contribuam para a execução das políticas de saúde e representem os cidadãos na garantia do direito à saúde.

Objetivos. Descrever e discutir a evolução do financiamento da assistência farmacêutica no Sistema Único de Saúde - SUS. Métodos. Foram identificados os valores alocados para aquisição de medicamentos, para o Programa Farmácia Popular e para estruturação de serviços farmacêuticos públicos. Os valores referentes ao financiamento da União, por meio do Ministério da Saúde, foram obtidos do sistema Siga Brasil e, dos Estados, do Distrito Federal e dos municípios, do Sistema de Informações sobre Orçamentos Públicos em Saúde - SIOPS. Resultados. Entre 2005 e 2009 houve aumento de 65,3% nos recursos financeiros da União para aquisição de medicamentos. No mesmo período, ampliou-se o volume de transferências feitas às esferas subnacionais. Verificou-se que os Estados e o Distrito Federal aumentaram em 112,4% o volume de recursos próprios alocados no financiamento de medicamentos e que para os municípios este crescimento foi de 22,7%. Em 2008, a participação das despesas com medicamentos em relação às despesas com saúde foi de 7,8%. O gasto total com medicamentos em 2009 foi de 8,9 bilhões de reais. Observou-se aumento de 20,6 vezes no valor alocado no Programa Farmácia Popular e, no caso dos recursos destinados à estruturação de serviços, crescimento de 41,6%, chegando a 10,1 milhões de reais em 2009. Conclusão: Houve ampliação do financiamento de medicamentos no SUS entre 2005 e 2009.Objectives. To describe and discuss developments in the financing of pharmaceutical services in the Brazilian public health system - SUS. Methods. The amounts allocated for drug procurement, for the Farmácia Popular Program and for structuring of public pharmaceutical services were identified. The values regarding the financing of the Federal government were obtained from the Siga Brasil database. Data regarding states, Federal District and municipalities were obtained from Information System on Public Health Budget - SIOPS. Results. Federal funds for drugs purchasing increased by 65.3% between 2005 and 2009. In the same period, the volume of transfers made to the subnational levels increased. It was found that the states and the Federal District have increased by 112.4% the volume of own resources allocated to the financing of drugs and for municipalities this growth was 22.7%. In 2008, the share of drug expenses in relation to expenditure on health was 7.8%. The total drug spending in 2009 was 8.9 billion reais. There was an increase of 20.6 times of the amount allocated in the Farmácia Popular Program, and a growth of 41.6% in the resources destined to structuring services, which reached 10.1 million reais in 2009. Conclusion. there was expansion of the funding of medicines in SUS between 2005 and 2009.

OBJETIVO: Analisar a aplicação direta de recursos financeiros pelos entes federados para a aquisição de medicamentos no Sistema Único de Saúde. MÉTODOS: Foram levantados os valores alocados em aplicações diretas para a aquisição de medicamentos no ano de 2009 em dois sistemas de informação: Siga Brasil do Senado Federal (para a União) e Sistema de Informações sobre Orçamentos Públicos em Saúde (para estados, Distrito Federal e municípios). Calculou-se o gasto per capita com medicamentos, além da média e mediana do gasto de municípios por região e por categoria de tamanho de população. O coeficiente de Spearman foi calculado para algumas variáveis definidas. A análise estatística incluiu testes de normalidade para os dados e de comparação múltipla para diferenças entre grupos. RESULTADOS: Em 2009 o valor total liquidado para a aquisição de medicamentos pelas três esferas de governo foi de R$ 8,9 bilhões. Estados e o Distrito Federal foram os principais executores, sendo responsáveis por 47,1% do valor total liquidado no Sistema Único de Saúde. Alguns estados tiveram gasto per capita muito acima da média (R$ 22,00 hab/ano) e da mediana (R$ 17,00 hab/ano). Houve diferença no gasto de municípios por região e observou-se que a média do gasto per capita dos que têm até 5 mil habitantes foi 3,9 vezes maior que dos municípios com mais de 500 mil habitantes. Municípios com até 10 mil habitantes tiveram gasto per capita maior que os demais municípios. CONCLUSÕES: Aspectos econômicos, tais como escala da aquisição de medicamentos e poder de negociação, podem explicar as diferenças de gasto per capita entre os entes federados, especialmente os municípios. O estudo sinaliza para ineficiências nas aplicações de recursos financeiros para a aquisição de medicamentos no Sistema Único de Saúde.OBJETIVO: Analizar la aplicación directa de recursos financieros por los entes federados para la adquisición de medicamentos en el Sistema Único de Salud. MÉTODOS: Se levantaron los valores ubicados en aplicaciones directas para la adquisición de medicamentos en el año de 2009 en dos sistemas de información: Siga Brasil del Senado Federal (para la Unión) y Sistema de Informaciones sobre Presupuestos Públicos en Salud (para estados, Distrito Federal y municipios). Se calculó el gasto per capita con medicamentos, así como el promedio y mediana del gasto de municipios por Región y por categoría de tamaño de población. El coeficiente de Spearman fue calculado para algunas variables definidas. El análisis estadístico incluyó pruebas de normalidad para los datos y de comparación múltiple para diferencias entre grupos. RESULTADOS: En 2009 el valor total liquidado para la adquisición de medicamentos por las tres esferas de gobierno en el Sistema Único de Salud fue de US$ 4,9 billones. Estados y el Distrito Federal fueron los principales ejecutores, siendo responsables por 47,1% del valor total liquidado en el Sistema Único de Salud. Algunos estados tuvieron gasto per capita muy por encima del promedio (US$ 12,15 hab/año) y de la mediana (US$ 9,39 hab/año). Hubo diferencia en el gasto de municipios por región y se observó que el promedio del gasto per capita de los que tienen hasta 5 mil habitantes fue 3,9 veces mayor que el de los municipios con más de 500 mil habitantes. Municipios con hasta 10 mil habitantes tuvieron un gasto per capita mayor que los demás municipios. CONCLUSIONES: Aspectos económicos tales como escala de adquisición de medicamentos y poder de negociación pueden explicar las diferencias de gasto per capita entre los entes federados, especialmente los municipios. El estudio resalta las ineficiencias en las aplicaciones de recursos financieros para la adquisición de medicamentos en el Sistema Único de Salud.OBJECTIVE: To analyze resource allocation for pharmaceutical procurement by federative entities in the Brazilian Unified Health System. METHODS: The amounts allocated to purchase pharmaceuticals during 2009 in two information systems were analyzed: Siga Brasil (Follow Brazil) for national data and Sistema de Informações sobre Orçamentos Públicos em Saúde (Information System on Public Health Budgets) for states, the Federal District and municipalities data. Per capita spending and the mean and median spending were calculated by municipalities, according to region and population size. The Spearman correlation coefficient was calculated for some variables. The statistical analysis included tests of normality and multiple comparisons for differences between groups. RESULTS: In 2009 the total amount spent by the three spheres of government for purchase of medicines was approximately R$ 8.9 billion. States and the Federal District were the main players, accounting for 47.1% of the total amount spent in the health system. Some states had per capita spending well above the mean (R$ 22.00 per resident/year) and the median (R$ 17.00 per resident/year). There were differences in municipal spending by region. The mean per capita expenditure of municipalities with less than 5,000 residents was 3.9 times that of municipalities with over 500,000 residents. Municipalities with less than 10,000 residents had higher per capita spending than other municipalities. CONCLUSIONS: Economic aspects such as the scale of procurement and bargaining power may explain differences in per capita spending between federal entities, especially among municipalities. The study indicates inefficiencies in the use of financial resources to procure medicines in the Brazilian Unified Health System.

OBJETIVO: Descrever aspectos técnicos do Programa de Medicamentos de Dispensação em Caráter Excepcional do Ministério de Saúde do Brasil, especialmente em relação aos gastos com os medicamentos distribuídos. MÉTODOS: Os aspectos técnicos foram obtidos por meio de consulta a todas as portarias que regulamentaram o Programa. Gastos no período de 2000 a 2007 foram obtidos do Sistema de Informações Ambulatoriais do Sistema Único de Saúde. Foram analisados os medicamentos dispensados de 1993 a 2009, quantidades e valor de cada procedimento informados nas autorizações de procedimentos de alta complexidade para cada estado. RESULTADOS: O Programa mudou, com aumento do número de fármacos e apresentações farmacêuticas distribuídas e de doenças contempladas. Eram distribuídos 15 fármacos em 31 diferentes apresentações farmacêuticas em 1993, passando para 109 fármacos em 243 apresentações em 2009. Os gastos totais do Ministério da Saúde com medicamentos somaram, em 2007, R$ 1.410.181.600,74, quase o dobro do valor gasto em 2000: R$ 684.975.404,43. Algumas das doenças que representaram maiores gastos nesse período foram: insuficiência renal crônica, transplante e hepatite C. CONCLUSÕES: O Programa de Medicamentos de Dispensação em Caráter Excepcional está em constante transformação, visando aprimorar os instrumentos e estratégias que assegurem e ampliem o acesso da população aos medicamentos. Devem-se buscar alternativas para reduzir o impacto financeiro do Programa para que não haja prejuízos às outras áreas do sistema de saúde, dado o custo elevado das novas tecnologias.

OBJECTIVES: To estimate the direct costs of hospital stays for premature newborns in the Interlagos Hospital and Maternity Center in São Paulo, Brazil and to assess the difference between the amount reimbursed to the hospital by the Unified Health System and the real cost of care for each premature newborn. METHODS: A cost-estimate study in which hospital and professional costs were estimated for premature infants born at 22 to 36 weeks gestation during the calendar year of 2004 and surviving beyond one hour of age. Direct costs included hospital services, professional care, diagnoses and therapy, orthotics, prosthetics, special materials, and blood products. Costs were estimated using tables published by the Unified Health System and the Brasindice as well as the list of medical procedures provided by the Brazilian Classification of Medical Procedures. RESULTS: The average direct cost of care for initial hospitalization of a premature newborn in 2004 was $2,386 USD. Total hospital expenses and professional services for all premature infants in this hospital were $227,000 and $69,500 USD, respectively. The costs for diagnostic testing and blood products for all premature infants totaled $22,440 and $1,833 USD. The daily average cost of a premature newborn weighing less than 1,000 g was $115 USD, and the daily average cost of a premature newborn weighing more than 2,500 g was $89 USD. Amounts reimbursed to the hospital by the Unified Health System corresponded to only 27.42% of the real cost of care. CONCLUSIONS: The cost of hospital stays for premature newborns was much greater than the amount reimbursed to the hospital by the Unified Health System. The highest costs corresponded to newborns with lower birth weight. Hospital costs progressively and discretely decreased as the newborns' weight increased.

OBJETIVO: Analisar as características organizacionais dos grupos de apoio e associações que representam pacientes reumáticos nos estados brasileiros. MÉTODOS: Inicialmente foi feito um mapeamento do universo desejado. Em seguida foi feita tentativa de contato com todas as entidades mapeadas, para que respondessem a um questionário eletrônico estruturado com a finalidade de entender o perfil destas organizações (iniciativas desenvolvidas, aspectos técnicos e legais, dificuldades encontradas, pontos fortes, entre outros). RESULTADOS: Foram identificadas 45 entidades, que predominam na região Sul e Sudeste do país. Do total, 30% possuem web-site e 50% apresentam contatos de difícil acesso. Recebemos resposta de 12 associações, cinco fecharam, uma se apresenta em fase de construção e o restante não foi possível estabelecer contato ou não respondeu à pesquisa. Das organizações que responderam ao questionário, somente uma declarou possuir titulação de OSCIP (Organização Social Civil de Interesse Público) ou de OS (Organização Social); assim como, somente uma declarou possuir registro no Conselho Nacional de Assistência Social (CNAS). Além do que, 50% não apresentam titulações de utilidade pública, 25% não apresentam registro em cartório e apenas 1/3 declarou possuir registro no Conselho Municipal de Assistência Social (CMAS). CONCLUSÕES: Os grupos estudados apresentam características bem heterogêneas quanto ao perfil organizacional. Dentre as entidades analisadas, algumas demonstram ser mais sólidas e estruturadas e outras apresentam dificuldades intrínsecas importantes, um perfil incipiente ou características que denotam pouca sobrevida.OBJECTIVE: To analyze organizational characteristics of support groups that represent rheumatic patients in the Brazilian states. METHODS: Initially a mapping of the desired universe was made. Contact was attempted with all organizations and they were asked to answer a structuralized electronic questionnaire with the purpose of understanding the profile of these organizations (developed initiatives, legal aspects, difficulties, strong points, among others). RESULTS: 45 organizations were identified. They predominate in the south and southeastern regions of the country. Of the total, 30% have a website and 50% presented difficulties to establish contact with. Of the 12 organizations that answered to the questionnaire, 5 had closed, one is on a construction phase, and the remaining was not possible to establish contact with, or did not answer the research. Of the organizations that answered the questionnaire, only one declared having an OSCIP title (Civil Social Organization of Public Interest) or a Social Organization title and only one declared being registered at the National Council of Social Assistance (CNAS). Moreover, 50% did not present a public utility title, 25% were registered in a registry office and only 1/3 declared being registered in the City Council of Social Assistance (CMAS). CONCLUSIONS: The studied groups present heterogeneous characteristics. Of the organizations studied, some showed to be more solid and well-structured, but others showed important intrinsic difficulties, with an incipient profile or characteristics that demonstrate little survival expectation.

JUSTIFICATIVA E OBJETIVOS: A dor crônica é muito prevalente e o custo do tratamento pode ter impacto relevante sobre as pessoas e a sociedade. O objetivo deste estudo foi avaliar os custos mensais médios dos medicamentos para tratamento ambulatorial da dor crônica. MÉTODO: Neste estudo, analisou-se o custo de medicamentos empregados por 233 pacientes com dor crônica (117 com dor nociceptiva, 59 com dor neuropática e 57 com dor mista) e atendidos no Centro Alfa da UNIFESP, entre janeiro de 2004 e janeiro de 2008. RESULTADOS: A média de custos geral foi de R$127,74 (custo mínimo de R$5,00 e máximo de R$780,00). CONCLUSÕES: O estudo revelou que os custos de medicamentos não diferem de forma significativa, levando-se em conta o tipo de dor envolvida.JUSTIFICATIVA Y OBJETIVOS: El dolor crónico ocurre muy a menudo y el coste del tratamiento puede tener un impacto relevante en las personas y en la sociedad. El objetivo de este estudio fue evaluar los costes mensuales promedios de los medicamentos para el tratamiento ambulatorial del dolor crónico. MÉTODO: En este estudio, analizamos el coste de los medicamentos utilizados por 233 pacientes con dolor crónico (117 con dolor nociceptivo, 59 con dolor neuropático y 57 con dolor mixto), y que fueron atendidos en el Centro Alfa de la UNIFESP, entre enero de 2004 y enero de 2008. RESULTADOS: El promedio general de los costes rondó los R$ 127,74 (coste mínimo de R$ 5,00 y máximo de R$ 780,00). CONCLUSIONES: El estudio reveló que los costes de medicamentos no son diferentes de forma significativa, teniendo en cuenta el tipo de dolor que existe.BACKGROUND AND OBJECTIVES: Chronic pain is very prevalent and the cost of its treatment can cause a relevant impact on people and society. The objective of this study was to evaluate the monthly cost of drugs used in the outpatient treatment of chronic pain. METHODS: In the present study the cost of the drugs used by 233 patients with chronic pain (117 with nociceptive pain, 59 with neuropathic pain, and 57 with mixed pain) followed at the Alpha Center of UNIFESP between January 2004 and January 2008 was evaluated. RESULTS: The mean general cost was R$ 127.74 (from R$ 5.00 to R$ 780.00). CONCLUSIONS: This study showed that the cost of the drugs does not differ significantly taking into consideration the type of pain.

OBJETIVO: Descrever os efeitos das ações judiciais que requerem o fornecimento de medicamentos, em relação a aspectos da política nacional de medicamentos. MÉTODOS: Pesquisa documental, com abordagem metodológica quali-quantitativa. Foram analisados todos os processos movidos por cidadãos contra a Secretaria Municipal de Saúde de São Paulo, referentes ao fornecimento de medicamentos, durante o ano de 2005. Utilizou-se formulário padronizado para a coleta de dados, realizando-se uma análise exploratória. RESULTADOS: Foram impetradas 170 ações contra a Secretaria requerendo o fornecimento de medicamentos. Os serviços do Sistema Único de Saúde originaram 59% das prescrições (26% municipais e 33% os demais). Câncer e diabetes foram as doenças mais referidas (59%). Faziam parte de listas de serviços 62% dos medicamentos solicitados itens solicitados. O gasto total foi de R$876 mil, efetuado somente para itens não selecionados (que não fazem parte da Relação Municipal de Medicamentos Essenciais), 73% dos quais poderiam ser substituídos. Do gasto total, 75% foram destinados à aquisição de antineoplásicos, cuja comprovação de eficácia necessita de mais ensaios clínicos. Dois desses medicamentos não estavam registrados no Brasil. CONCLUSÕES: A maioria das demandas por medicamentos geradas por ações judiciais poderia ser evitada se fossem consideradas as diretrizes do Sistema Único de Saúde, a organização do atendimento em oncologia e a observância das relações de medicamentos essenciais. A falta dessa observância compromete a Política Nacional de Medicamentos, a eqüidade no acesso e o uso racional de medicamentos no Sistema Único de Saúde.OBJECTIVE: To describe how lawsuits, which demand the supply of drugs, impact on elements of the national drug policy. METHODS: This is a desk-based study using qualitative and quantitative methods. All legal proceedings brought by citizens against the Municipal Secretary of State of Sao Paulo, relating to the supply of drugs in 2005 were analyzed. A standardized form was used to collect data, with a view to carrying out an exploratory analysis. RESULTS: A total of 170 cases relating to the supply of drugs were brought against the Municipal Secretary of State. The National Health System (SUS) was the source for 59% of the prescriptions: 26% from the municipal level, 33% from other levels. Cancer and diabetes were the diseases most commonly involved (59%). About 62% of drugs requested are on the lists of SUS services. Total expenditure was R$876,000 (Brazilian Reais), covering only non-selected items (i.e. those which are not included in the Municipal Register of Essential Medicines), 73% of which could be substituted. Of the total expenditure, 75% was spent on purchasing anticancer drugs, for which further clinical trials are required to prove their effectiveness. Two of these medicines were not registered in Brazil. CONCLUSIONS: The majority of demands for drugs that have led to legal proceedings could be avoided if two SUS directives were followed, namely the organization of oncology services and the observance of reporting on essential medicines. Failure to do so causes a breakdown in the National Drug Policy, in equality of access and in the rational use of drugs within the National Health System.

OBJETIVO: Verificar a magnitude da diferença de preço entre os medicamentos genéricos e os respectivos medicamentos de referência, além de avaliar o efeito da concorrência entre eles quanto aos seus preços. MÉTODOS: De janeiro de 2000 a junho de 2004, foram acompanhadas 135 apresentações de medicamentos genéricos e seus respectivos medicamentos de referência por um período de até quatro anos. Os preços foram extraídos de publicações específicas voltadas para o comércio farmacêutico, e foram anotados os preços para ambos os medicamentos, desde o lançamento do genérico e durante período de comercialização posterior. RESULTADOS: Os genéricos foram introduzidos, em média, com preços 40% menores do que os medicamentos de referência e esta diferença tendeu a aumentar ao longo do tempo. Comparando a diferença de preço do genérico em relação ao seu medicamento de referência observou-se aumento de 68% nos quatro anos seguintes ao seu lançamento. CONCLUSÕES: A introdução de medicamentos genéricos no mercado farmacêutico brasileiro contribuiu para a oferta de medicamentos a preços menores. Entretanto, a procura dos medicamentos genéricos não provocou redução dos preços da maioria dos medicamentos de referência.OBJECTIVE: To assess the magnitude of price difference between generic and innovator medicines and to evaluate the effect of the price competition between them. METHODS: From January 2000 to June 2004, 135 generic medicines and their respective innovator medicines were followed up for a period of up to four years. Prices were extracted from pharmaceutical market specific publications and recorded from the launching of generics and during their marketing period. RESULTS: The generic medicines were introduced on average at prices 40% lower than the innovator ones and this difference tended to increase over the years. The price difference between generic and innovator medicines increased in the subsequent four-year period after generic launching in 68%. CONCLUSIONS: The introduction of generic medicines in the Brazilian pharmaceutical market contributed for offering cheaper medicines. However, the competition between generic and innovator medicines did not promote markdown of the majority innovator medicines.

Gestational Trophoblastic Disease (GTD) comprises a group of disorders that derive from the placenta. Even if full recovery is generally expected, women diagnosed with GTD have to confront: the loss of a pregnancy, a potentially life-threatening diagnosis and delays in future pregnancies. The aim of the study is to evaluate the psychological impact of GTD, focusing on perceived fertility, depression and anxiety.37 patients treated for GTD at San Raffaele Hospital, Milan, took part in the study. The STAI-Y (State-Trait Anxiety Inventory), the BDI-SF (Beck Depression Scale-Short Form) and the FPI (Fertility Problem Inventory) were used. Patients were grouped on the basis of presence of children (with or without), age (< or ≥35) and type of diagnosis (Hydatidiform Mole, HM, or Gestational Trophoblastic Neoplasia, GTN). Differences in the values between variables were assessed by a t-type test statistic. Three-way ANOVAs were also carried out considering the same block factors.The study highlights that women suffering from GTN had higher depression scores compared to women suffering from HM. A significant correlation was found between anxiety (state and trait) and depression. Younger women presented higher Global Stress scores on the FPI, especially tied to Need for Parenthood and Relationship Concern subscales. Need for Parenthood mean scores significantly varied between women with and without children too.We can conclude that fertility perception seems to be negatively affected by GTD diagnosis, particularly in younger women and in those without children. Patients should be followed by a multidisciplinary team so as to be supported in the disease's psychological aspects too.

Neste momento de crise financeira do Estado, é importante o uso adequado dos recursos econômicos disponíveis, e este trabalho objetiva estudar os fatores que determinam o aumento dos gastos em saúde; é uma revisão crítica do tema, com um levantamento sistemático das publicações dos últimos dez anos, considerando-se os principais estudiosos da disciplina da economia da saúde no Brasil e de outros países. Os fatores que agem na demanda por serviços de saúde são de natureza variada, podendo se sobrepor, o que aumenta ainda mais a demanda: necessidade sentida, fatores psicossociais, seguridade social, demografía, epidemiología, utilização dos serviços, regulamentação e fatores culturais. Os que agem na oferta da assistência à saúde são: progresso técnico-médico, difusão da inovação e multiplicação dos centros de assistência à saúde. Os fatores da demanda não podem ser rapidamente contidos, enquanto que os de oferta são muito mais facilmentecontrolados, mas são poucos os exemplos encontrados na literatura sobre o sucesso das medidas de controle.In this moment of financial crisis of the State, it is important to adequately use the economic resources available. The objective of this survey is to study the factors that determine the increase in health expenditures. It is a critical review of the subject, conducting a systematic survey of publications for the past ten years, and considering the main scholars in health economics in Brazil and other countries. The health service demand factors have varied features, and may be overlapping, thus increasing even more the demand: the perceived need, social security, demography, epidemiology, the use of services, regulation, psychosocial and cultural factors. The health service supply factors are technical-medical progress, promotion of innovations and multiplication of health care centers. Demand factors cannot be quickly curbed, whereas the supply factors are more easily controlled. However, there are only a few examples in the literature on sucess of control measures.
