Abstract: Background: The Italian Society of Echocardiography and Cardiovascular Imaging (SIECVI) conducted a national survey to understand better how different echocardiographic modalities are used and accessed in Italy. Methods: We analyzed echocardiography laboratory activities over a month (November 2022). Data were retrieved via an electronic survey based on a structured questionnaire, uploaded on the SIECVI website. Results: Data were obtained from 228 echocardiographic laboratories: 112 centers (49%) in the northern, 43 centers (19%) in the central, and 73 (32%) in the southern regions. During the month of observation, we collected 101,050 transthoracic echocardiography (TTE) examinations performed in all centers. As concern other modalities there were performed 5497 transesophageal echocardiography (TEE) examinations in 161/228 centers (71%); 4057 stress echocardiography (SE) examinations in 179/228 centers (79%); and examinations with ultrasound contrast agents (UCAs) in 151/228 centers (66%). We did not find significant regional variations between the different modalities. The usage of picture archiving and communication system (PACS) was significantly higher in the northern (84%) versus central (49%) and southern (45%) centers (P < 0.001). Lung ultrasound (LUS) was performed in 154 centers (66%), without difference between cardiology and noncardiology centers. The evaluation of left ventricular (LV) ejection fraction was evaluated mainly using the qualitative method in 223 centers (94%), occasionally with the Simpson method in 193 centers (85%), and with selective use of the three-dimensional (3D) method in only 23 centers (10%). 3D TTE was present in 137 centers (70%), and 3D TEE in all centers where TEE was done (71%). The assessment of LV diastolic function was done routinely in 80% of the centers. 2. Impact of forest management on threatened epiphytic macrolichens: evidence from a Mediterranean mixed oak forest (Italy)

Abstract: Forest management practices may heavily affect epiphytic cryptogams. This study was conceived in March 2016, as soon as we were informed about an authorized logging for timber within a Mediterranean mixed oak forest in Tuscany (central Italy), which threatened a large population of the forest macrolichen Lobaria pulmonaria (L.) Hoffm., composed of hundreds of fertile thalli. Lobaria pulmonaria is often used as an ecological indicator of high quality habitats hosting rare lichens, and in general, cryptogams worthy of conservation. The species has suffered a general decline throughout Europe as a consequence of air pollution and intensive forest management, and currently it is red-listed in several countries, where it is considered also as a “flag species”. We estimated that 40% of the lichen biomass (8.5-12.3 kg ha-1) has been lost due to logging operations (in the core area, up to 1.8 kg every 100 m2), in particular large and fertile thalli. One year after the conclusion of logging operations, the analysis of chlorophyll fluorescence emission (indicator of the photosynthetic performance of the lichen photobionts), revealed a significant reduction of the vitality of the thalli on retained-isolated trees. The article provides issues for conservation in Mediterranean oak forests and outlines the outmost importance of ensuring the safeguard of forest ecosystems hosting fertile populations of this model species, especially in the case of unprotected forests.

Abstract: Forest management practices may heavily impact epiphytic (tree inhabiting) organisms. Retaining tree patches and buffer strips in logged stands may contribute to preserve ecosystem functioning and the vitality of epiphytic organisms in managed forests. To test these statements, the threatened forest macrolichen Lobaria pulmonaria (L.) Hoffm. was used as a model species, since it is a “flag” indicator species of forest ecosystems with long ecological continuity. To this purpose, photosynthetic performances, thallus anatomy and water holding capacity (WHC) of samples of L. pulmonaria were investigated in a logged mixed oak forest (Tuscany, Italy), confronting lichen thalli from retained-forest patches and retained-isolated trees, 18 months after logging. Compared with those of retained-forest patches, thalli on the trunks of retained-isolated trees were thinner and showed lower vitality (as indicated by the potential quantum yield of primary photochemistry - FV/FM and the index of overall photosynthetic performance - PIABS), as well as lower water holding capacity. In contrast, thalli from forest patches had performances comparable to those of healthy samples from unlogged forests.

Abstract: IDPlanT is the Italian Database of Plant Translocation, an initiative of the Nature Conservation Working Group of the Italian Botanical Society. IDPlanT currently includes 185 plant translocations. The establishment of a national database on plant translocation is a key step forward in data sharing and techniques improvement in this field of plant conservation. Abstract: Evaluating metagenomic software is key for optimizing metagenome interpretation and focus of the Initiative for the Critical Assessment of Metagenome Interpretation (CAMI). The CAMI II challenge engaged the community to assess methods on realistic and complex datasets with long- and short-read sequences, created computationally from around 1,700 new and known genomes, as well as 600 new plasmids and viruses. Here we analyze 5,002 results by 76 program versions. Substantial improvements were seen in assembly, some due to long-read data. Related strains still were challenging for assembly and genome recovery through binning, as was assembly quality for the latter. Profilers markedly matured, with taxon profilers and binners excelling at higher bacterial ranks, but underperforming for viruses and Archaea. Clinical pathogen detection results revealed a need to improve reproducibility. Runtime and memory usage analyses identified efficient programs, including top performers with other metrics. The results identify challenges and guide researchers in selecting methods for analyses.

Abstract: Background: Patients with chronic lymphocytic leukemia (CLL) may be more susceptible to COVID-19 related poor outcomes, including thrombosis and death, due to the advanced age, the presence of comorbidities, and the disease and treatment-related immune deficiency. The aim of this study was to assess the risk of thrombosis and bleeding in patients with CLL affected by severe COVID-19. Methods: This is a retrospective multicenter study conducted by ERIC, the European Research Initiative on CLL, including patients from 79 centers across 22 countries. Data collection was conducted between April and May 2021. The COVID-19 diagnosis was confirmed by the real-time polymerase chain reaction (RT-PCR) assay for SARS-CoV-2 on nasal or pharyngeal swabs. Severe cases of COVID-19 were defined by hospitalization and the need of oxygen or admission into ICU. Development and type of thrombotic events, presence and severity of bleeding complications were reported during treatment for COVID-19. Bleeding events were classified using ISTH definition. STROBE recommendations were used in order to enhance reporting. Results: A total of 793 patients from 79 centers were included in the study with 593 being hospitalized (74.8%). Among these, 511 were defined as having severe COVID: 162 were admitted to the ICU while 349 received oxygen supplementation outside the ICU. Most patients (90.5%) were receiving thromboprophylaxis. During COVID-19 treatment, 11.1% developed a thromboembolic event, while 5.0% experienced bleeding. Thrombosis developed in 21.6% of patients who were not receiving thromboprophylaxis, in contrast to 10.6% of patients who were on thromboprophylaxis. Bleeding episodes were more frequent in patients receiving intermediate/therapeutic versus prophylactic doses of low-molecular-weight heparin (LWMH) (8.1% vs. 3.8%, respectively) and in elderly. Age and LMWH intermediate/therapeutic dose administration were prognostic factors in multivariate model for bleeding (OR = 1.062, 95%CI 1.017-1.109 and OR = 2.438, 95%CI 1.023-5.813, respectively). Conclusions: Patients with CLL affected by severe COVID-19 are at a high risk of thrombosis if thromboprophylaxis is not used, but also at increased risk of bleeding under the LMWH intermediate/therapeutic dose administration.

Abstract: A future energy system for road transport requires optimised energy use and primary energy decarbonisation to achieve global CO2 reduction goals. Simultaneously decarbonising transport with other sectors of the economy places additional demands on limited low-carbon energy sources, requiring efficient processes within a fuel pathway from energy source to energy use. Battery electric vehicles (BEVs) and fuel-cell electric vehicles (FCEVs) are low-carbon options that reduce tailpipe emissions, but differ in overall efficiency, associated carbon intensity, and cost. Current commercialised technologies, as well as theoretical maximums, are aggregated in a stochastic analysis to quantify the energy efficiency and CO2 differences for BEV and FCEV energy systems. Carbon capture and storage improves source-to-wheels CO2 intensity for hydrogen produced from steam methane reformation (27 gCO2/km with carbon capture and store and 140 gCO2/km without for light-duty FCEVs). Light-duty BEVs have a lower CO2 intensity (11 gCO2/km) using decarbonised grid electricity and are 65% more efficient than light-duty FCEVs using grid energy. These effects translate to heavy-good vehicles but with added complexity. In a maximised trailer volume scenario, electric and fuel-cell heavy-good vehicles have similar projected carbon intensities from a natural gas primary energy source, but electric heavy-good vehicle using conventional battery systems or an electric road system are able to achieve a 55% and 67% carbon reduction (gCO2/m3 km) compared to fuel-cell heavy-goods vehicles, respectively.

Abstract: Secondary acute myeloid leukemia (sAML) after myelodysplastic or myeloproliferative disorders is a high-risk category currently identified by clinical history or specific morphological and cytogenetic abnormalities. However, in the absence of these features, uncertainties remain to identify the secondary nature of some cases otherwise defined as de novo AML. To test whether a chromatin-spliceosome (CS) mutational signature might better inform the definition of the de novo AML group, we analyzed a prospective cohort of 413 newly diagnosed AML patients enrolled into a randomized clinical trial (NILG AML 02/06) and provided with accurate cytogenetic and molecular characterization. Among clinically defined de novo AML, 17.6% carried CS mutations (CS-AML) and showed clinical characteristics closer to sAML (older age, lower white blood cell counts and higher rate of multilineage dysplasia). Outcomes in this group were adverse, more similar to those of sAML as compared to de novo AML (overall survival, 30% in CS-AML and 17% in sAML vs 61% in de novo AML, P<0.0001; disease free survival, 26% in CS-AML and 22% in sAML vs 54% of de novo AML, P<0.001) and independently confirmed by multivariable analysis. Allogeneic transplant in first complete remission improved survival in both sAML and CS-AML patients. In conclusion, these findings highlight the clinical significance of identifying CS-AML for improved prognostic prediction and potential therapeutic implications. Abstract: Ibrutinib represents extraordinary progress in the treatment of chronic lymphocytic leukemia (CLL). However, treatment-related adverse events limit the benefit of this agent. This obser-vational, multicenter study focused on the incidence, risk factors, and prognostic impact of infections in 494 patients with CLL treated with an ibrutinib-based treatment. Ibrutinib was given to 89 (18%) previously untreated patients (combined with rituximab, 24) and 405 (82%) relapsed/refractory patients. Pneumonia (PN), grade ≥3 non-opportunistic infections (NOI), and opportunistic infections (OI) were recorded in 32% of patients with an overall incidence rate per 100 person-year of 15.3% (PN, 10%; NOI, 3.3%; OI, 2%). Infections were the reason for the permanent discontinuation of ibrutinib in 9% of patients. Patients who experienced pneumonia or a severe infection showed a significantly inferior survival than those who were infection-free (p < 0.0001). A scoring system based on the three factors associated with a significant and independent impact on infections—PN or severe infection in the year before starting ibrutinib, chronic obstructive pulmonary disease, ≥2 prior treatments—identified patients with a two-to threefold increase in the rate of infections. In conclusion, the results of this study highlight the adverse impact of infectious events on the outcomes of CLL patients treated with ibrutinib.

Abstract: Autoimmune cytopenias (AICs) affect 5% to 9% of patients with chronic lymphocytic leukemia (CLL). Targeted drugs—ibrutinib, idelalisib, and venetoclax—have a prominent role in the treatment of CLL, but their impact on CLL-associated AICs is largely unknown. In this study, we evaluated the characteristics and outcome of preexisting AICs and described the incidence, quality, and management of treatment-emergent AICs during therapy with targeted drugs in patients with CLL. We collected data from 572 patients treated with ibrutinib (9% in combination with an anti-CD20 monoclonal antibody), 143 treated with idelalisib-rituximab, and 100 treated with venetoclax (12% in combination with an anti-CD20 monoclonal antibody). A history of preexisting AICs was reported in 104 (13%) of 815 patients. Interestingly, 80% of patients whose AICs had not resolved when treatment with a targeted drug was started experienced an improvement or a resolution during therapy. Treatment-emergent AICs occurred in 1% of patients during ibrutinib therapy, in 0.9% during idelalisib therapy, and in 7% during venetoclax therapy, with an estimated incidence rate of 5, 6, and 69 episodes per 1000 patients per year of exposure in the 3 treatment groups, respectively. The vast majority of patients who developed treatment-emergent AICs had unfavorable biological features such as an unmutated IGHV and a del(17p) and/or TP53 mutation. Notably, despite AICs, 83% of patients were able to continue the targeted drug, in some cases in combination with additional immunosuppressive agents. Overall, treatment with ibrutinib, idelalisib, or venetoclax seems to have a beneficial impact on CLL-associated AICs, inducing an improvement or even a resolution of preexisting AICs in most cases and eliciting treatment-emergent AICs in a negligible portion of patients.

Abstract: The Maremma Natural History Museum is active in the field of Citizen Science at the Tuscan, national and international level. Among the founding bodies of the European Citizen Science Association (ECSA), it has promoted meetings for a national Citizen Science strategy in Italy, actively participated in the drafting of national guidelines on the subject and developed Citizen Science projects in Tuscany and other regions nearby. BioBlitzes are one of the best-known activities in the field of environmental Citizen Science. In May 2016, the Museum organized its fourth BioBlitz at the Accesa Lake Special Protection Area. On this occasion, data collected attested the presence of 784 different taxonomic entities, 678 of which were identified at the species level and 18 at the subspecies level. Nineteen alien species and two endemic species have been identified. Thirty-nine species protected by national or international laws and 13 endangered species have been detected. The area has been confirmed to be an important biodiversity hotspot; future utilization plans must be developed with due consideration for the fragility of the habitats close to the lake from the conservation point of view.

Abstract: The threat to the survival of many species, caused by emerging ecological crises, has favored the development of a new environmental awareness. In recent decades, especially in the most industrialized nations, an increasing participation of citizens in the collection of scientific data has also been recorded. The active involvement of society in different aspects of science has been defined by the sociologist Alan Irwin as “Citizen Science”. Although environmental Citizen Science brings together the largest number of projects, the fields of application of this new way of doing science are innumerable. BioBlitz is one of the best-known examples of citizen science activities, aimed at monitoring the biodiversity of an area. The Maremma Natural History Museum, as part of its activities, regularly organizes BioBlitz on an annual basis, starting from 2013. During BioBlitz 2017 in the SPZ and SCZ “Monte Labbro and Alta Valle dellAlbegna” data were collected attesting the presence of 403 different taxonomic entities, of which 367 identified at the species level and 5 at the subspecies level. Six alien and 2 endemic species were identified. In terms of environmental legislation, 38 protected species and 7 endangered species were detected in the area. The most relevant aspect that emerges from the collected data is the consistent number of protected species. This confirms the environmental value of the area and the need for management and conservation plans for the entire site.

Abstract: In recent decades, many natural history museums in Europe have integrated their traditional functions with new ways of interacting with visitors and local communities. In particular, the need to adapt their mission according to the changes in modern society has led them to develop initiatives aiming at an active involvement of the public in scientific research. This is one of the inspiring principles of Citizen Science. BioBlitz are among the most popular activities dedicated to biodiversity in this field. In line with the inspiring principles of this cultural revolution, since 2012 the Maremma Natural History Museum organized public participation initiatives for the collection of biodiversity data. We hereby report the main results of the BioBlitz 2018 organized by the Maremma Natural History Museum in the Talamone area, located in the southern portion of the Uccellina Hills. Four hundred and ninety eight taxa were sampled and identified, 447 of which at the level of species and 10 at the level of subspecies; the remaining refer to higher taxonomic levels. 31 protected species and 10 endangered species were found in the area; moreover, 6 alien species and one endemic species were detected. The main feature that distinguished BioBlitz 2018 to others conducted in previous years was the large number of participants. The data collected increased the scientific knowledge of the southern portion of the Maremma Regional Park, less known than other areas of the same protected area.

Abstract: Microbiology depends on the availability of annotated microbial genomes for many applications. Comparative genomics approaches have been a major advance, but consistent and accurate annotations of genomes can be hard to obtain. In addition, newer concepts such as the pan-genome concept are still being implemented to help answer biological questions. Hence, we present proGenomes2, which provides 87 920 high-quality genomes in a user-friendly and interactive manner. Genome sequences and annotations can be retrieved individually or by taxonomic clade. Every genome in the database has been assigned to a species cluster and most genomes could be accurately assigned to one or multiple habitats. In addition, general functional annotations and specific annotations of antibiotic resistance genes and single nucleotide variants are provided. In short, proGenomes2 provides threefold more genomes, enhanced habitat annotations, updated taxonomic and functional annotation and improved linkage to the NCBI BioSample database. The database is available at http://progenomes.embl.de/.

Abstract: Chronic lymphocytic leukemia (CLL) is a disease of the elderly, characterized by immunodeficiency. Hence, patients with CLL might be considered more susceptible to severe complications from COVID-19. We undertook this retrospective international multicenter study to characterize the course of COVID-19 in patients with CLL and identify potential predictors of outcome. Of 190 patients with CLL and confirmed COVID-19 diagnosed between 28/03/2020 and 22/05/2020, 151 (79%) presented with severe COVID-19 (need of oxygen and/or intensive care admission). Severe COVID-19 was associated with more advanced age (≥65 years) (odds ratio 3.72 [95% CI 1.79–7.71]). Only 60 patients (39.7%) with severe COVID-19 were receiving or had recent (≤12 months) treatment for CLL at the time of COVID-19 versus 30/39 (76.9%) patients with mild disease. Hospitalization rate for severe COVID-19 was lower (p < 0.05) for patients on ibrutinib versus those on other regimens or off treatment. Of 151 patients with severe disease, 55 (36.4%) succumbed versus only 1/38 (2.6%) with mild disease; age and comorbidities did not impact on mortality. In CLL, (1) COVID-19 severity increases with age; (2) antileukemic treatment (particularly BTK inhibitors) appears to exert a protective effect; (3) age and comorbidities did not impact on mortality, alluding to a relevant role of CLL and immunodeficiency.

Abstract: By way of a Next-Generation Sequencing NGS high throughput approach, we defined the mutational profile in a cohort of 221 normal karyotype acute myeloid leukemia (NK-AML) enrolled into a prospective randomized clinical trial, designed to evaluate an intensified chemotherapy program for remission induction. NPM1, DNMT3A, and FLT3-ITD were the most frequently mutated genes while DNMT3A, FLT3, IDH1, PTPN11, and RAD21 mutations were more common in the NPM1 mutated patients (p < 0.05). IDH1 R132H mutation was strictly associated with NPM1 mutation and mutually exclusive with RUNX1 and ASXL1. In the whole cohort of NK-AML, no matter the induction chemotherapy used, by multivariate analysis, the achievement of complete remission was negatively affected by the SRSF2 mutation. Alterations of FLT3 (FLT3-ITD) and U2AF1 were associated with a worse overall and disease-free survival (p < 0.05). FLT3-ITD positive patients who proceeded to alloHSCT had a survival probability similar to FLT3-ITD negative patients and the transplant outcome was no different when comparing high and low-AR-FLT3-ITD subgroups in terms of both OS and DFS. In conclusion, a comprehensive molecular profile for NK-AML allows for the identification of genetic lesions associated to different clinical outcomes and the selection of the most appropriate and effective treatment strategies, including stem cell transplantation and targeted therapies.

Abstract: Chronic lymphocytic leukemia (CLL) is a disease of the elderly, characterized by immunodeficiency. Hence, patients with CLL might be considered more susceptible to severe complications from COVID-19. We undertook this retrospective international multicenter study to characterize the course of COVID-19 in patients with CLL and identify potential predictors of outcome. Of 190 patients with CLL and confirmed COVID-19 diagnosed between 28/03/2020 and 22/05/2020, 151 (79%) presented with severe COVID-19 (need of oxygen and/or intensive care admission). Severe COVID-19 was associated with more advanced age (≥65 years) (odds ratio 3.72 [95% CI 1.79–7.71]). Only 60 patients (39.7%) with severe COVID-19 were receiving or had recent (≤12 months) treatment for CLL at the time of COVID-19 versus 30/39 (76.9%) patients with mild disease. Hospitalization rate for severe COVID-19 was lower (p < 0.05) for patients on ibrutinib versus those on other regimens or off treatment. Of 151 patients with severe disease, 55 (36.4%) succumbed versus only 1/38 (2.6%) with mild disease; age and comorbidities did not impact on mortality. In CLL, (1) COVID-19 severity increases with age; (2) antileukemic treatment (particularly BTK inhibitors) appears to exert a protective effect; (3) age and comorbidities did not impact on mortality, alluding to a relevant role of CLL and immunodeficiency.

The Scientific Committee of the European Monitoring Centre for Drugs and Drug Addiction calls for a strengthening of global monitoring and evidence synthesis and a renewed vigour in collaborative efforts to expand training, research and the quality and comparability of data across Europe and beyond.

Abstract: Interactions between the ocean and the atmosphere occur at the air-sea interface through the transfer of momentum, heat, gases and particulate matter, and through the impact of the upper-ocean biology on the composition and radiative properties of this boundary layer. The Tara Pacific expedition, launched in May 2016 aboard the schooner Tara, was a 29-month exploration with the dual goals to study the ecology of reef ecosystems along ecological gradients in the Pacific Ocean and to assess inter-island and open ocean surface plankton and neuston community structures. In addition, key atmospheric properties were measured to study links between the two boundary layer properties. A major challenge for the open ocean sampling was the lack of ship-time available for work at “stations”. The time constraint led us to develop new underway sampling approaches to optimize physical, chemical, optical, and genomic methods to capture the entire community structure of the surface layers, from viruses to metazoans in their oceanographic and atmospheric physicochemical context. An international scientific consortium was put together to analyze the samples, generate data, and develop datasets in coherence with the existing Tara Oceans database. Beyond adapting the extensive Tara Oceans sampling protocols for high-resolution underway sampling, the key novelties compared to Tara Oceans’ global assessment of plankton include the measurement of (i) surface plankton and neuston biogeography and functional diversity; (ii) bioactive trace metals distribution at the ocean surface and metal-dependent ecosystem structures; (iii) marine aerosols, including biological entities; (iv) geography, nature and colonization of microplastic; and (v) high-resolution underway assessment of net community production via equilibrator inlet mass spectrometry.
