Abstract

Background There is a paucity of data comparing 30-day morbidity and mortality of sleeve gastrectomy (SG), Roux-en-Y gastric bypass (RYGB), and one anastomosis gastric bypass (OAGB). This study aimed to compare the 30-day safety of SG, RYGB, and OAGB in propensity score-matched cohorts. Materials and methods This analysis utilised data collected from the GENEVA study which was a multicentre observational cohort study of bariatric and metabolic surgery (BMS) in 185 centres across 42 countries between 01/05/2022 and 31/10/2020 during the Coronavirus Disease-2019 (COVID-19) pandemic. 30-day complications were categorised according to the Clavien–Dindo classification. Patients receiving SG, RYGB, or OAGB were propensity-matched according to baseline characteristics and 30-day complications were compared between groups. Results In total, 6770 patients (SG 3983; OAGB 702; RYGB 2085) were included in this analysis. Prior to matching, RYGB was associated with highest 30-day complication rate (SG 5.8%; OAGB 7.5%; RYGB 8.0% (p = 0.006)). On multivariate regression modelling, Insulin-dependent type 2 diabetes mellitus and hypercholesterolaemia were associated with increased 30-day complications. Being a non-smoker was associated with reduced complication rates. When compared to SG as a reference category, RYGB, but not OAGB, was associated with an increased rate of 30-day complications. A total of 702 pairs of SG and OAGB were propensity score-matched. The complication rate in the SG group was 7.3% (n = 51) as compared to 7.5% (n = 53) in the OAGB group (p = 0.68). Similarly, 2085 pairs of SG and RYGB were propensity score-matched. The complication rate in the SG group was 6.1% (n = 127) as compared to 7.9% (n = 166) in the RYGB group (p = 0.09). And, 702 pairs of OAGB and RYGB were matched. The complication rate in both groups was the same at 7.5 % (n = 53; p = 0.07). Conclusions This global study found no significant difference in the 30-day morbidity and mortality of SG, RYGB, and OAGB in propensity score-matched cohorts.

Abstract

Background: Metabolic and bariatric surgery (MBS) is an effective treatment for adolescents with severe obesity. Objectives: This study examined the safety of MBS in adolescents during the coronavirus disease 2019 (COVID-19) pandemic. Methods: This was a global, multicentre and observational cohort study of MBS performed between May 01, 2020, and October 10,2020, in 68 centres from 24 countries. Data collection included in-hospital and 30-day COVID-19 and surgery-specific morbidity/mortality. Results: One hundred and seventy adolescent patients (mean age: 17.75 ± 1.30 years), mostly females (n=122, 71.8%), underwent MBS during the study period. The mean pre-operative weight and body mass index were 122.16 ± 15.92 kg and 43.7± 7.11 kg/m2, respectively. Although majority of patients had pre-operative testing for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) (n=146; 85.9%), only 42.4% (n=72) of the patients were asked to self-isolate pre-operatively. Two patients developed symptomatic SARS-CoV-2 infection post-operatively (1.2%). The overall complication rate was 5.3% (n=9). There was no mortality in this cohort. Conclusions: MBS in adolescents with obesity is safe during the COVID-19 pandemic when performed within the context of local precautionary procedures (such as pre-operative testing). The 30-day morbidity rates were similar to those reported pre-pandemic. These data will help facilitate the safe re-introduction of MBS services for this group of patients.

Abstract

Background: There is a paucity of data comparing 30-day morbidity and mortality of sleeve gastrectomy (SG), Roux-en-Y gastric bypass (RYGB), and one anastomosis gastric bypass (OAGB). This study aimed to compare the 30-day safety of SG, RYGB, and OAGB in propensity score-matched cohorts. Materials and methods: This analysis utilised data collected from the GENEVA study which was a multicentre observational cohort study of bariatric and metabolic surgery (BMS) in 185 centres across 42 countries between 01/05/2022 and 31/10/2020 during the Coronavirus Disease-2019 (COVID-19) pandemic. 30-day complications were categorised according to the Clavien–Dindo classification. Patients receiving SG, RYGB, or OAGB were propensity-matched according to baseline characteristics and 30-day complications were compared between groups. Results: In total, 6770 patients (SG 3983; OAGB 702; RYGB 2085) were included in this analysis. Prior to matching, RYGB was associated with highest 30-day complication rate (SG 5.8%; OAGB 7.5%; RYGB 8.0% (p = 0.006)). On multivariate regression modelling, Insulin-dependent type 2 diabetes mellitus and hypercholesterolaemia were associated with increased 30-day complications. Being a non-smoker was associated with reduced complication rates. When compared to SG as a reference category, RYGB, but not OAGB, was associated with an increased rate of 30-day complications. A total of 702 pairs of SG and OAGB were propensity score-matched. The complication rate in the SG group was 7.3% (n = 51) as compared to 7.5% (n = 53) in the OAGB group (p = 0.68). Similarly, 2085 pairs of SG and RYGB were propensity score-matched. The complication rate in the SG group was 6.1% (n = 127) as compared to 7.9% (n = 166) in the RYGB group (p = 0.09). And, 702 pairs of OAGB and RYGB were matched. The complication rate in both groups was the same at 7.5 % (n = 53; p = 0.07). Conclusions: This global study found no significant difference in the 30-day morbidity and mortality of SG, RYGB, and OAGB in propensity score-matched cohorts. © 2021, The Author(s).

Abstract

Background: There is a paucity of data comparing 30-day morbidity and mortality of sleeve gastrectomy (SG), Roux-en-Y gastric bypass (RYGB), and one anastomosis gastric bypass (OAGB). This study aimed to compare the 30-day safety of SG, RYGB, and OAGB in propensity score-matched cohorts. Materials and methods: This analysis utilised data collected from the GENEVA study which was a multicentre observational cohort study of bariatric and metabolic surgery (BMS) in 185 centres across 42 countries between 01/05/2022 and 31/10/2020 during the Coronavirus Disease-2019 (COVID-19) pandemic. 30-day complications were categorised according to the Clavien–Dindo classification. Patients receiving SG, RYGB, or OAGB were propensity-matched according to baseline characteristics and 30-day complications were compared between groups. Results: In total, 6770 patients (SG 3983; OAGB 702; RYGB 2085) were included in this analysis. Prior to matching, RYGB was associated with highest 30-day complication rate (SG 5.8%; OAGB 7.5%; RYGB 8.0% (p = 0.006)). On multivariate regression modelling, Insulin-dependent type 2 diabetes mellitus and hypercholesterolaemia were associated with increased 30-day complications. Being a non-smoker was associated with reduced complication rates. When compared to SG as a reference category, RYGB, but not OAGB, was associated with an increased rate of 30-day complications. A total of 702 pairs of SG and OAGB were propensity score-matched. The complication rate in the SG group was 7.3% (n = 51) as compared to 7.5% (n = 53) in the OAGB group (p = 0.68). Similarly, 2085 pairs of SG and RYGB were propensity score-matched. The complication rate in the SG group was 6.1% (n = 127) as compared to 7.9% (n = 166) in the RYGB group (p = 0.09). And, 702 pairs of OAGB and RYGB were matched. The complication rate in both groups was the same at 7.5 % (n = 53; p = 0.07). Abstract

Background: The ongoing “coronavirus disease 19” (COVID-19) pandemic has had a strong effect on the delivery of surgical care worldwide. Elective surgeries have been canceled or delayed in order to reallocate resources to the treatment of COVID-19 patients. Currently, the impact of the COVID-19 pandemic on bariatric and metabolic surgical practice remains unclear. Methods: An internet-based online survey was performed among bariatric surgeons worldwide. The survey was sent to bariatric surgeons via the International Bariatric Club Facebook group and by electronic mail via the International Federation for the Surgery of Obesity and metabolic disorders (IFSO) secretariat to members of the associated national IFSO societies. Results: One hundred sixty-nine (n = 169) bariatric surgeons participated in the survey. The majority of the respondents postponed preoperative upper gastrointestinal tract endoscopies, appointments in the outpatient clinic and bariatric operations. Most surgeons performed video calls for follow-up appointments instead of meeting the patients in the outpatient clinics. Laparoscopy was still the preferred treatment for surgical emergencies, but a trend towards conservative treatment of acute appendicitis and acute cholecystitis was shown. Rapid preoperative COVID-19 testing availability was poor; therefore, routine screening of emergency bariatric cases was not widely provided. A wide variance occurred regarding precautions and personal protection equipment among the participants. Conclusion: The COVID-19 pandemic showed a strong impact on bariatric surgical practice regarding surgical and outpatient planning as well as personnel management. Coordinated effort from the national bariatric societies should focus on strict implementation of the current recommendations regarding precaution measures and personal protection equipment. 18. Agent-oriented and ontology-driven digital libraries: the IndianaMAS experience

Abstract

This paper describes IndianaMAS, a multiagent system able to automatically classify and manage images, sketches, and multilingual documents in a cultural heritage domain. The latter has been formalized by means of an ontology, which enables the semantic integration of heterogeneous data from different sources, drives the agent communication with the internal and external environment, and provides an abstract and human-readable interface between the system and the user. IndianaMAS is able to expose to the world the classified data via a digital library. Modularity and reusability are the key engineering principles followed in the system design and implementation. We present the details of the IndianaMAS system and discuss how its architecture can be generalized to create – with the minimal effort – systems addressing similar classification, storage, and management problems, but operating in different domains and driven by different ontologies. The concrete problems we faced and their solutions are described to share our lesson learned and, at the same time, to show the applicability and reusability of our modular approach based on ontologies, agents, and digital libraries. Abstract

International audience; In this paper we present a survey conducted in order to understand how simple 3D objects, named geometaphors, can be arranged for building a model permitting to represent topological relationships in a threedimensional space. The research was conducted on twenty participants who were familiar with geographical information systems (GIS) and the notion of topological relationship. The users participating to the survey were asked to put in relation textual labels related to topological relationships with visual arrangements of geometaphors pairs. While the results of the survey often show a convergence towards specific associations, in other cases users indicate a set of associations complementary to the main choice. Therefore, in order to reduce the ambiguity, the anwsers to the survey were filtered with a set of rules that take into consideration the priority of the relationships, the category of the geometaphors and the order they are selected; the result is an interpretation model of the visual arrangements that takes into account all the answers and, in particular, takes advantage of the set of complementary associations when the primary ones can’t be applied because they are not congruent with certain categories of geometries. Such model represents an important component for the building of visual query environments compliant with the users’ mental model, contributing therefore to augment significantly the usability of such systems.

Abstract

International audience; A spatial query language, useful to querying spatial databases and retrieving spatial information, becomes efficient, effective and pleasant-to-use only if its surrounding environment is able to provide unskilled users with immediate and comprehensive access to the supplied visual operations. In this paper we describe how a usability evaluation process has been applied to improve the interface of the visual environment WebMGISQL 3D, a web application for querying and retrieving spatial information, with special focus on those phenomena where the third dimension is a relevant feature.

Abstract

This paper presents the Indiana Ontology for modeling the knowledge about Mount Bego's rock art and its exploitation in the IndianaMAS project. Although many projects use ontologies for semantic processing of cultural heritage digital objects, we are not aware of such ontologies in the rock art domain. Also, the Indiana Ontology is fully and seamlessly integrated with the IndianaMAS framework components, namely the intelligent software agents and the Digital Library used to classify and store multimedia and multilingual objects, thus making the IndianaMAS framework a good representative of an advanced and innovative information management system for the cultural heritage domain.

Abstract

This paper presents the architecture of a holonic multi-agent system for rock art interpretation and discusses the results achieved within the "Indiana MAS" project. We show how the AgentSketch and the ImageRec holons belonging to the Indiana MAS, able to cope with hand drawn sketches and images respectively, have been tested in the domain of Mount Bego's prehistoric rock art (southern French Alps), and how the Ma-nent agent-based framework for the seamless integration of Digital Libraries has been plugged into Indiana MAS to provide text classification, as well as multilingual access to structured repositories. The way Indiana MAS holons cooperate in order to provide correct interpretations of ambiguous shapes is discussed by means of an example based on hypotheses recently advanced by archaeologists. © 2014.

Abstract

The Indiana MAS project, funded by the Italian Ministry of Education, University and Research "Futuro in Ricerca 2010" program, aims at providing a framework for the digital protection and conservation of rock art natural and cultural heritage sites, by storing, organizing and presenting information about them in such a way to encourage scientific research and to raise the interest and sensibility towards them from the common people. The project involves two research units, namely Genova (Dipartimento di Informatica, Bioingegneria, Robotica e Ingegneria dei Sistemi) and Salerno (Dipartimento di Matematica e Informatica), for a period of 36 months, starting from march 8th, 2012. The technologies adopted in the project range from agents to ontologies, as requested by the complex nature of the platform, where each module is devoted to a specific task: sketch and symbol recognition, semantic interpretation of complex visual scenes, multi-language text understanding, storing, classification and indexing of multimedia and heterogeneous digital objects. All of them should cooperate and coordinate in order to enable higher level components to reason on them and to detect relationships among different digital objects, hence providing new hypothesis based on such relationships.
