Clotting factor concentrates were a revolutionary treatment which changed the life of people with haemophilia. However, early generation of clotting factor concentrates, without viral inactivation procedures in the manufacturing process, led to an increased risk of transmission of blood-borne viral infections, mainly due to hepatitis C virus and human immunodeficiency virus. As only 20% of HCV-infected patients clear the infection naturally, chronic HCV infection constitutes a serious health problem and a major cause of chronic liver disease in this group of patients. Fortunately, the use of viral inactivation procedures in the plasma-derived factor concentrates manufacturing process and the availability of alternative treatment options, led to a significant reduction of transfusion-associated viral infections. The advent of multiple, orally administrated, highly effective direct-acting antivirals (DAAs) is changing the natural history of HCV infection in patients with haemophilia as these drugs have an excellent safety profile and achieve very high sustained virological response rates, similar to the general population. Eradication of HCV-infection in patients with haemophilia is feasible via micro-elimination projects.Keywords: hepatitis C virus, haemophilia, direct-acting antivirals, micro-elimination projects

This study aimed to compare the characteristics of severe asthma patients with HOCS who did and did not initiate biologics.Methods: Baseline characteristics of patients with HOCS (long-term maintenance OCS therapy for at least 1 year, or ≥ 4 courses of steroid bursts in a year) from the International Severe Asthma Registry (ISAR; https://isaregistries.org/), who initiated or did not initiate biologics (anti-lgE, anti-IL5/5R or anti-IL4R), were described at the time of biologic initiation or registry enrolment. Statistical relationships were tested using Pearson’s chi-squared tests for categorical variables, and t-tests for continuous variables, adjusting for potential errors in multiple comparisons.Results: Between January 2015 and February 2021, we identified 1412 adult patients with severe asthma from 19 countries that met our inclusion criteria of HOCS, of whom 996 (70.5%) initiated a biologic and 416 (29.5%) did not. The frequency of biologic initiation varied across geographical regions. Those who initiated a biologic were more likely to have higher blood eosinophil count (483 vs 399 cells/μL, p=0.003), serious infections (49.0% vs 13.3%, p< 0.001), nasal polyps (35.2% vs 23.6%, p< 0.001), airflow limitation (56.8% vs 51.8%, p=0.013), and uncontrolled asthma (80.8% vs 73.2%, p=0.004) despite greater conventional treatment adherence than those who did not start a biologic. Both groups had similar annual asthma exacerbation rates in the previous 12 months (5.7 vs 5.3, p=0.147).Conclusion: Around one third of severe HOCS asthma patients did not receive biologics despite a similar high burden of asthma exacerbations as those who initiated a biologic therapy. Other disease characteristics such as eosinophilic phenotype, serious infectious events, nasal polyps, airflow limitation and lack of asthma control appear to dictate biologic use.Keywords: severe asthma, biologics, real-world, treatment pattern, patient characteristics

4. High-frequency analysis of Earth gravity field models based on terrestrial gravity and GPS/levelling data: a case study in Greece

Abstract: The present study aims at the validation of global gravity field models through numerical investigation in gravity field functionals based on spherical harmonic synthesis of the geopotential models and the analysis of terrestrial data. We examine gravity models produced according to the latest approaches for gravity field recovery based on the principles of the Gravity field and steadystate Ocean Circulation Explorer (GOCE) and Gravity Recovery And Climate Experiment (GRACE) satellite missions. Furthermore, we evaluate the overall spectrum of the ultra-high degree combined gravity models EGM2008 and EIGEN-6C3stat. The terrestrial data consist of gravity and collocated GPS/levelling data in the overall Hellenic region. The software presented here implements the algorithm of spherical harmonic synthesis in a degree-wise cumulative sense. This approach may quantify the bandlimited performance of the individual models by monitoring the degree-wise computed functionals against the terrestrial data. The degree-wise analysis performed yields insight in the short-wavelengths of the Earth gravity field as these are expressed by the high degree harmonics.

Abstract: Less than 5% of prenatal thymoctes express HLA-DR before week 12 of gestation. However, the number of HLA-DR- positive cells increases during the late second and third trimesters of development. To determine the role of alpha-smooth muscle actin in fetal thymic HLA-DR signaling in different stages of development we examined and compared the immunohistochemical expression of alpha-smooth muscle actin in the myoid cells of the thymic medulla stroma in the 2 nd, and 3rd trimesters of gestation respectively, over the equivalent expression of the protein in the 1 st trimester, in relation with the appearance of HLA-DR-positive thymocytes. 6. How Do Physical Activity and Sedentary Behaviour Affect Motor Competence in Children with Autism Spectrum Disorder Compared to Typically Developing Children: A Pilot Study

Abstract: Older children with Autism Spectrum Disorder (ASD) have high levels of motor impairment, however we are unsure if similar patterns exist in young children. This study aimed to investigate motor competence in four-to-seven-year-old children with (n = 17) and without (n = 17) ASD. A series of ANOVAS indicated children with ASD performed significantly poorer on all measures of motor competence, except MABC-2 manual dexterity and ball skills subscales. Results indicate that moderate-to-vigorous physical activity (PA) and sedentary behaviour (SB) may influence motor competence, regardless of diagnosis. Establishing appropriate levels of engagement in moderate-to-vigorous PA and SB during early school years is important for the development of all children and may be an important early intervention avenue for motor impairment in children with ASD.

Abstract: Background: Food protein‐induced allergic proctocolitis (FPIAP) is the leading cause of rectal bleeding in infants. Tolerance is presumed to develop until the first year of age, although natural history studies are scarce, making the determination of the ideal duration for any intervention, challenging. Intestinal microbiota (IM) is crucial in food allergy development; however, data for FPIAP remain limited. This study aimed to assess FPIAP remission after 3 months of milk avoidance and its correlation with IM longitudinal changes. Methods: A prospective observational study of infants aged ≤6 months with a diagnosis of FPIAP. After 3 months of management according to a clinical algorithm, infants were subjected to a milk challenge using either a cow (CM) or a goat (GM) milk formula in a random order. Stool samples were collected longitudinally for microbiome analysis. Results: Out of 61 infants, 57 were challenged (29 with CM, 28 with GM). Of these, 55 (96.5%) achieved tolerance, with no difference in tolerance rates between CM (28/29) and GM (27/28). The average age of tolerance development was 6.3 months. Enterobacteriaceae clusters (Klebsiella‐ and Shigella‐dominated) were most often represented in samples from symptomatic infants. In contrast, Bacteroides and Bifidobacteria clusters emerged later, in apparently healthy infants. Conclusion: A 3‐month intervention was sufficient for almost all infants to achieve tolerance. GM was tolerated equally well to CM. Symptomatic FPIAP is associated with immature enterotypes, while disease remission coincides with microbiome changes in time. [ABSTRACT FROM AUTHOR]

18. Numerical study of transient in-nozzle fuel flow phenomena and near-nozzle effects during and after the end of injection for Diesel engines

Abstract: The design of a Diesel injector is a key factor in achieving higher engine efficiency. The injector's fuel atomisation characteristics are also critical for minimising toxic emissions such as unburnt Hydrocarbons (HC). However, when developing injection systems, the small dimensions of the nozzle render optical experimental investigations very challenging under realistic engine conditions. For the present work, Computational Fluid Dynamics (CFD) was employed and transient, Volume Of Fluid (VOF), multiphase simulations of the flow inside and immediately downstream of a real-size multi-hole nozzle were performed, during and after the injection event with a small air chamber coupled to the injector downstream of the nozzle exit. A Reynolds Averaged Navier-Stokes (RANS) approach was used to account for turbulence. A moving mesh approach was followed for the movement of the needle. Models that can provide an accurate prediction of the liquid-gas interface and also capture the vapour-air mixing were used. Moreover, an evaporation model was developed. The code was validated against experimental data and data from the literature. 9 different injections were simulated for injection pressures equal to 400 bar and 900 bar, ambient pressure that varied from 60 bar to 1 bar and fuel temperature that varied from 300 K to 353 K. A high chamber temperature case and a high nozzle wall temperature case were also investigated. The results showed that the flow during the injection cannot be considered steady state and that hysteresis exists. After the end of injection, the state of the nozzle varied from being filled with liquid to being filled with air. Some form of dribble existed in all the injections while in one of them a late cycle mass expulsion was predicted. The effect of evaporation was found to be very small but it can contribute towards late cycle mass expulsion. In addition, the pressure drop due to the engine cycle could also have a similar effect.

19. In vitro and in vivo investigation of enzymatic clot lysis through non-thermal mechanisms using focused ultrasound waves as an adjunct to thrombolytic drug tenecteplase and in synergy with microbubbles

Abstract: In this thesis, the beneficial effect of ultrasound (US) energy is utilized for accelerating thrombolysis efficacy through non-thermal mechanisms. The outcome of the research, can be applied in the future for clinical use, providing alternative techniques (either noninvasive or invasive), for treating ischaemic stroke under MR guidance. To study the effect of US on thrombolysis efficacy, three different in vitro circulating flow clot models were developed. Fully retracted porcine blood clots were treated with US waves as an adjunct to thrombolytic drug tenecteplase (TNK-tPA), in the presence or absence of microbubbles (MBs). Each one of the flow clot models used, was designed to reproduce a different physiologic situation of middle cerebral artery (MCA) occlusion, since it is the most common cause of stroke. In all the proposed treatment protocols, temperature elevation at beam focus never exceeded 1 0C, providing that the contribution of thermal mechanisms to clot lysis was negligible. The first model, was reproducing a deep-seated MCA occlusion in a brain tissue. To provide a more realistic clinical environment, the study was conducted into a brain tissue mimicking phantom. Flow rate was set to 20 % of the maximum value occur in an open MCA and TNK-tPA dose was not exceeding the 30 % of the average maximum concentration in blood. Using 1.18 MHz focused US (FUS) waves, various experimental parameters that influence thrombolysis efficacy, were optimized. Maximum thrombolysis efficacy was observed when FUS pulses were used as an adjunct to thrombolytic drug in the presence of MBs. With this technique, 370 mg of clot mass was removed in 30 min, which may not be enough to achieve significant clinical benefits. However, further improvement was done (increase of flow rate and TNK-tPA concentration), to enhance thrombolysis efficacy. The second model, was reproducing a MCA occlusion occurred superficially. Flow rate was increased four times and TNK-tPA concentration was doubled. The optimum operating mode parameters obtained before, were employed in this study in order to investigate the impact of frequency and acoustic intensity on thrombolysis efficacy. Study findings established that higher FUS frequencies (1.18 MHz), are associated with enhanced thrombolysis compared to lower frequencies (0.6 MHz). Also, a linear increasing dependence between acoustic intensity and thrombolysis efficacy was observed. After 30 min of treatment with 1.18 MHz FUS exposures in synergy with TNKtPA and MBs, 1050 mg of clot mass was removed, which should be sufficient for timely recanalization of an occluded cerebral artery. The third model, was reproducing a MCA occlusion, treated invasively with a catheter-directed US device. The efficacy of two small planar ultrasonic transducers (operating at frequencies 3.7 and 5.2 MHz respectively), on clot lysis was evaluated. The values of flow rate and TNK-tPA concentration were kept the same with those used in the previous study. Using 3.7 MHz US waves in association with TNK-tPA and MBs for 30 min, 700 mg of clot mass was removed, showing that potentially, intravascular sonothrombolysis with such a transducer can be an effective method for treating stroke. Finally, the knowledge gained in vitro for enhancing TNK-tPA induced thrombolysis, was translated in vivo using an animal model. A blood clot was artificially formed into the right carotid artery of a rabbit, mimicking a MCA occlusion in humans. The complete flow blockage as well as the recanalization procedure were monitored using a 3D time of flight MR angiography. This novel technique, clearly demonstrated that the combination of 1.18 MHz FUS pulses with MBs, strongly accelerated the action of TNKtPA, leading through non-thermal mechanisms to full artery recanalization within 19 min.

Abstract: The transcription factor EHF is highly expressed in the lactating mammary gland, but its role in mammary development and tumorigenesis is not fully understood. Utilizing a mouse model of Ehf deletion, herein, we demonstrate that loss of Ehf impairs mammary lobuloalveolar differentiation at late pregnancy, indicated by significantly reduced levels of milk genes and milk lipids, fewer differentiated alveolar cells, and an accumulation of alveolar progenitor cells. Further, deletion of Ehf increased proliferative capacity and attenuated prolactin-induced alveolar differentiation in mammary organoids. Ehf deletion also increased tumor incidence in the MMTV-PyMT mammary tumor model and increased the proliferative capacity of mammary tumor organoids, while low EHF expression was associated with higher tumor grade and poorer outcome in luminal A and basal human breast cancers. Collectively, these findings establish EHF as a non-redundant regulator of mammary alveolar differentiation and a putative suppressor of mammary tumorigenesis.

Abstract: This study examined the sustained and moderating effects of a behavioural sleep intervention for autistic children in a randomised controlled trial. Autistic children (5-13 years) with sleep problems were randomised to the Sleeping Sound intervention or Treatment as Usual (TAU). At 12-month follow-up (n = 150), caregivers of children in the Sleeping Sound group reported greater reduction in child sleep problems compared to TAU (p < .001, effect size: - 0.4). The long-term benefits of the intervention were greater for children taking sleep medication, children of parents who were not experiencing psychological distress, and children with greater autism severity. The Sleeping Sound intervention demonstrated sustained improvements in child sleep. Identified moderators may inform treatment by indicating which subgroups may benefit from further support.

Abstract: Bilayer graphene encapsulated in tungsten diselenide can host a weak topological phase with pairs of helical edge states. The electrical tunability of this phase makes it an ideal platform to investigate unique topological effects at zero magnetic field, such as topological superconductivity. Here we couple the helical edges of such a heterostructure to a superconductor. The inversion of the bulk gap accompanied by helical states near zero displacement field leads to the suppression of the critical current in a Josephson geometry. Using superconducting quantum interferometry we observe an even-odd effect in the Fraunhofer interference pattern within the inverted gap phase. We show theoretically that this effect is a direct consequence of the emergence of helical modes that connect the two edges of the sample. The absence of such an effect at high displacement field, as well as in bare bilayer graphene junctions, supports this interpretation and demonstrates the topological nature of the inverted gap., QRD/Goswami Lab, BUS/TNO STAFF

Abstract: We previously described an approach called RealSeqS to evaluate aneuploidy in plasma cell-free DNA through the amplification of ~350,000 repeated elements with a single primer. We hypothesized that an unbiased evaluation of the large amount of sequencing data obtained with RealSeqS might reveal other differences between plasma samples from patients with and without cancer. This hypothesis was tested through the development of a machine learning approach called Alu Profile Learning Using Sequencing (A-PLUS) and its application to 7615 samples from 5178 individuals, 2073 with solid cancer and the remainder without cancer. Samples from patients with cancer and controls were prespecified into four cohorts used for model training, analyte integration, and threshold determination, validation, and reproducibility. A-PLUS alone provided a sensitivity of 40.5% across 11 different cancer types in the validation cohort, at a specificity of 98.5%. Combining A-PLUS with aneuploidy and eight common protein biomarkers detected 51% of the cancers at 98.9% specificity. We found that part of the power of A-PLUS could be ascribed to a single feature-the global reduction of AluS subfamily elements in the circulating DNA of patients with solid cancer. We confirmed this reduction through the analysis of another independent dataset obtained with a different approach (whole-genome sequencing). The evaluation of Alu elements may therefore have the potential to enhance the performance of several methods designed for the earlier detection of cancer.

Abstract: The pressure evolution of the Raman spectrum of graphene grown by chemical vapour deposition on polycrystalline copper is investigated with the use of a polar and a non-polar pressure transmitting medium (PTM). The G and 2D Raman bands exhibit similar pressure slopes for both PTM irrespectively of any unintentional initial doping and/or strain of the samples. Our analysis suggests that any pressure-induced charge transfer effects are negligibly small to influence the pressure response of graphene. This is determined by the mechanical stress due to the pressure-induced substrate contraction and the transfer efficiency of the latter to the graphene layer, as well as the PTM-graphene interaction. For the non-polar PTM, a peculiar pressure behavior of graphene is observed in the PTM solidification regime, resembling that of free standing graphene.

Following an approach proposed by one of the authors, wave functions are regarded as elements of suitable projective modules over C(Q). Abstract: Alain Connes' construction of the standard model is based on a generalized Dirac-Yukawa operator and the K-cycle $(\HD ,D)$, with $\HD$ a fermionic Hilbert space. If this construction is reformulated at the level of the differential algebra then a direct comparison with the alternative approach by the Marseille-Mainz group becomes possible. We do this for the case of the toy model based on the structure group $U(1)\times U(1)$ and for the $SU(2)\times U(1)$ of electroweak interactions. Connes' results are recovered without the somewhat disturbing $\gamma_{5}$-factors in the fermion mass terms and Yukawa couplings. Abstract: Hepatocellular carcinoma (HCC) constitutes a frequent highly malignant form of primary liver cancer and is the third cause of death attributable to malignancy. Despite the improvement in the therapeutic strategies with the exploration of novel pharmacological agents, the survival rate for HCC is still low. Shedding light on the multiplex genetic and epigenetic background of HCC, such as on the emerging role of microRNAs, is considered quite promising for the diagnosis and the prediction of this malignancy, as well as for combatting drug resistance. MicroRNAs (miRNAs) constitute small noncoding RNA sequences, which play a key role in the regulation of several signaling and metabolic pathways, as well as of pivotal cellular functions such as autophagy, apoptosis, and cell proliferation. It is also demonstrated that miRNAs are significantly implicated in carcinogenesis, either acting as tumor suppressors or oncomiRs, while aberrations in their expression levels are closely associated with tumor growth and progression, as well as with local invasion and metastatic dissemination. The arising role of miRNAs in HCC is in the spotlight of the current scientific research, aiming at the development of novel therapeutic perspectives. In this review, we will shed light on the emerging role of miRNAs in HCC. © 2023 by the authors.

Abstract: Background: Type 2 inflammation is common in children with asthma. Dupilumab, a human antibody, blocks the signaling of interleukin -4 and -13, key and central drivers of type 2 inflammation. In the LIBERTY ASTHMA VOYAGE (NCT02948959) study, dupilumab reduced severe asthma exacerbations and improved lung function in children aged 6 to 11 years with uncontrolled, moderate-to-severe asthma. Objective: To assess the pharmacokinetics of dupilumab and type 2 biomarker changes in children with type 2 asthma in VOYAGE. Methods: Patients were randomized to dupilumab 100 mg (≤30 kg) or 200 mg (>30 kg) or placebo every 2 weeks for 52 weeks. Dupilumab concentrations and changes in type 2 biomarkers were assessed at each visit. Results: Dupilumab concentrations in serum reached a steady state by week 12, with mean concentrations of 51.2 mg/L and 79.4 mg/L in children receiving dupilumab 100 mg every 2 weeks and 200 mg every 2 weeks, respectively (therapeutic range in adults and adolescents: 29-80 mg/L). Reductions in type 2 biomarkers were comparable between regimens, and greater in patients treated with dupilumab vs placebo. In children treated with dupilumab 100 mg and 200 mg every 2 weeks, the median percent changes (Q1-Q3) from baseline at week 52 were, respectively, −78.6% (−86.3 to −69.80) and −78.6% (−84.9 to −70.1) for serum total immunoglobulin E, −53.6% (−66.4 to −34.6) and −43.7% (−58.6 to −28.5) for thymus and activation-regulated chemokine; −25.7% (−60.0 to 27.6) and −33.3% (−60.6 to 16.6) for blood eosinophils, and −47.7% (−73.8 to 18.9) and −55.6% (−73.6 to −20.0) for fractional exhaled nitric oxide. Conclusion: Weight-tiered dose regimens achieved mean concentrations within the dupilumab therapeutic range. The median decreases in type 2 biomarker levels were similar between dose regimens. Abstract: Background: Patients with severe asthma may present with characteristics representing overlapping phenotypes, making them eligible for more than one class of biologic. Our aim was to describe the profile of adult patients with severe asthma eligible for both anti-IgE and anti-IL5/5R and to compare the effectiveness of both classes of treatment in real life. Methods: This was a prospective cohort study that included adult patients with severe asthma from 22 countries enrolled into the International Severe Asthma registry (ISAR) who were eligible for both anti-IgE and anti-IL5/5R. The effectiveness of anti-IgE and anti-IL5/5R was compared in a 1:1 matched cohort. Exacerbation rate was the primary effectiveness endpoint. Secondary endpoints included long-term-oral corticosteroid (LTOCS) use, asthma-related emergency room (ER) attendance, and hospital admissions. Results: In the matched analysis (n = 350/group), the mean annualized exacerbation rate decreased by 47.1% in the anti-IL5/5R group and 38.7% in the anti-IgE group. Patients treated with anti-IL5/5R were less likely to experience a future exacerbation (adjusted IRR 0.76; 95% CI 0.64, 0.89; p < 0.001) and experienced a greater reduction in mean LTOCS dose than those treated with anti-IgE (37.44% vs. 20.55% reduction; p = 0.023). There was some evidence to suggest that patients treated with anti-IL5/5R experienced fewer asthma-related hospitalizations (IRR 0.64; 95% CI 0.38, 1.08), but not ER visits (IRR 0.94, 95% CI 0.61, 1.43). Conclusions: In real life, both anti-IgE and anti-IL5/5R improve asthma outcomes in patients eligible for both biologic classes; however, anti-IL5/5R was superior in terms of reducing asthma exacerbations and LTOCS use. © 2023 The Authors. Abstract: Colorectal cancer (CRC) is a leading cause of mortality worldwide. We conducted a genome-wide association study meta-analysis of 100,204 CRC cases and 154,587 controls of European and east Asian ancestry, identifying 205 independent risk associations, of which 50 were unreported. We performed integrative genomic, transcriptomic and methylomic analyses across large bowel mucosa and other tissues. Transcriptome- and methylome-wide association studies revealed an additional 53 risk associations. We identified 155 high-confidence effector genes functionally linked to CRC risk, many of which had no previously established role in CRC. These have multiple different functions and specifically indicate that variation in normal colorectal homeostasis, proliferation, cell adhesion, migration, immunity and microbial interactions determines CRC risk. Crosstissue analyses indicated that over a third of effector genes most probably act outside the colonic mucosa. Our findings provide insights into colorectal oncogenesis and highlight potential targets across tissues for new CRC treatment and chemoprevention strategies.

Abstract: Asthma, rhinitis and atopic dermatitis (AD) are interrelated clinical phenotypes that partly overlap in the human interactome. The concept of "one-airway-one-disease", coined over 20 years ago, is a simplistic approach of the links between upper- and lower-airway allergic diseases. With new data, it is time to reassess the concept. This article reviews (i) the clinical observations that led to Allergic Rhinitis and its Impact on Asthma (ARIA), (ii) new insights into polysensitisation and multimorbidity, (iii) advances in mHealth for novel phenotype definition, (iv) confirmation in canonical epidemiologic studies, (v) genomic findings, (vi) treatment approaches and (vii) novel concepts on the onset of rhinitis and multimorbidity. One recent concept, bringing together upper- and lower-airway allergic diseases with skin, gut and neuropsychiatric multimorbidities, is the "Epithelial Barrier Hypothesis". This review determined that the "one-airway-one-disease" concept does not always hold true and that several phenotypes of disease can be defined. These phenotypes include an extreme "allergic" (asthma) phenotype combining asthma, rhinitis and conjunctivitis. Rhinitis alone and rhinitis and asthma multimorbidity represent two distinct diseases with the following differences: (i) genomic and transcriptomic background (Toll-Like Receptors and IL-17 for rhinitis alone as a local disease; IL-33 and IL-5 for allergic and non-allergic multimorbidity as a systemic disease), (ii) allergen sensitisation patterns (mono- or pauci-sensitisation versus polysensitisation), (iii) severity of symptoms and (iv) treatment response. In conclusion, rhinitis alone (local disease) and rhinitis with asthma multimorbidity (systemic disease) should be considered as two distinct diseases, possibly modulated by the microbiome, and may be a model for understanding the epidemics of chronic and auto-immune diseases.

44. The impact of COVID-19 on sleep for autistic children: A systematic review

Abstract: BACKGROUND: Up to 80% of children with autism experience behavioural sleep problems, predominantly relating to bedtime resistance, sleep anxiety, sleep dysregulation, and shorter duration, which are associated with increased autistic symptom expression and emotional and behavioural difficulties. Researchers predicted the COVID-19 pandemic would worsen sleep and behavioural difficulties for autistic children, due to their need for routine and certainty. This systematic review is the first to focus on delineating the role of sleep disturbance in exacerbating autistic symptoms and internalising and externalising behaviours during the pandemic. METHOD: In this PROSPERO registered systematic review, we aggregated and synthesised findings from empirical studies from 2020 onwards that included children with autism and examined sleep outcomes, using narrative and framework synthesis due to the variety of methods and designs employed. We identified additional relevant themes through inductive thematic analysis. RESULTS: Seventy-one studies met the search criteria, and we selected seventeen for review following screening and quality assessment. These studies reported mixed findings; with strongest support for worsening of sleep problems typically experienced by autistic children, including difficulties with sleep regulation and shorter sleep duration. Further, sleep problems were associated with increased expression of autistic characteristics. CONCLUSIONS: Preliminary findings of worsening sleep and increased autistic characteristics for autistic children throughout the COVID-19 pandemic highlight the need for ongoing, accessible and flexible service provision during exposure to environmental stressors. We propose that behavioural sleep interventions are well suited to telehealth adaptation and play an important role in supporting families when in-person treatment for sleep problems is not possible.

Abstract: Asthma development and exacerbation is linked to respiratory virus infections. There is limited information regarding the presence of viruses during non-exacerbation/infection periods. We investigated the nasopharyngeal/nasal virome during a period of asymptomatic state, in a subset of 21 healthy and 35 asthmatic preschool children from the Predicta cohort. Using metagenomics, we described the virome ecology and the cross-species interactions within the microbiome. The virome was dominated by eukaryotic viruses, while prokaryotic viruses (bacteriophages) were independently observed with low abundance. Rhinovirus B species consistently dominated the virome in asthma. Anelloviridae were the most abundant and rich family in both health and asthma. However, their richness and alpha diversity were increased in asthma, along with the co-occurrence of different Anellovirus genera. Bacteriophages were richer and more diverse in healthy individuals. Unsupervised clustering identified three virome profiles that were correlated to asthma severity and control and were independent of treatment, suggesting a link between the respiratory virome and asthma. Finally, we observed different cross-species ecological associations in the healthy versus the asthmatic virus-bacterial interactome, and an expanded interactome of eukaryotic viruses in asthma. Upper respiratory virome “dysbiosis” appears to be a novel feature of pre-school asthma during asymptomatic/non-infectious states and merits further investigation.

Abstract: Bilayer graphene (BLG) was recently shown to host a band-inverted phase with unconventional topology emerging from the Ising-type spin-orbit interaction (SOI) induced by the proximity of transition metal dichalcogenides with large intrinsic SOI. Here, we report the stabilization of this band-inverted phase in BLG symmetrically encapsulated in tungsten diselenide (WSe2) via hydrostatic pressure. Our observations from low temperature transport measurements are consistent with a single particle model with induced Ising SOI of opposite sign on the two graphene layers. To confirm the strengthening of the inverted phase, we present thermal activation measurements and show that the SOI-induced band gap increases by more than 100% due to the applied pressure. Finally, the investigation of Landau level spectra reveals the dependence of the level-crossings on the applied magnetic field, which further confirms the enhancement of SOI with pressure., BUS/TNO STAFF, QRD/Goswami Lab

Abstract: Asthma, rhinitis, and atopic dermatitis (AD) are interrelated clinical phenotypes that partly overlap in the human interactome. The concept of “one-airway-one-disease,” coined over 20 years ago, is a simplistic approach of the links between upper- and lower-airway allergic diseases. With new data, it is time to reassess the concept. This article reviews (i) the clinical observations that led to Allergic Rhinitis and its Impact on Asthma (ARIA), (ii) new insights into polysensitization and multimorbidity, (iii) advances in mHealth for novel phenotype definitions, (iv) confirmation in canonical epidemiologic studies, (v) genomic findings, (vi) treatment approaches, and (vii) novel concepts on the onset of rhinitis and multimorbidity. One recent concept, bringing together upper- and lower-airway allergic diseases with skin, gut, and neuropsychiatric multimorbidities, is the “Epithelial Barrier Hypothesis.” This review determined that the “one-airway-one-disease” concept does not always hold true and that several phenotypes of disease can be defined. These phenotypes include an extreme “allergic” (asthma) phenotype combining asthma, rhinitis, and conjunctivitis. Rhinitis alone and rhinitis and asthma multimorbidity represent two distinct diseases with the following differences: (i) genomic and transcriptomic background (Toll-Like Receptors and IL-17 for rhinitis alone as a local disease; IL-33 and IL-5 for allergic and non-allergic multimorbidity as a systemic disease), (ii) allergen sensitization patterns (mono- or pauci-sensitization versus polysensitization), (iii) severity of symptoms, and (iv) treatment response. Abstract: Background: The self-reporting of asthma frequently leads to patient misidentification in epidemiological studies. Strategies combining the triangulation of data sources may help to improve the identification of people with asthma. We aimed to combine information from the self-reporting of asthma, medication use and symptoms to identify asthma patterns in the users of an mHealth app. Methods: We studied MASK-air® users who reported their daily asthma symptoms (assessed by a 0-100 visual analogue scale – “VAS Asthma”) at least three times (either in three different months or in any period). K-means cluster analysis methods were applied to identify asthma patterns based on: (i) whether the user self-reported asthma; (ii) whether the user reported asthma medication use and (iii) VAS asthma. Clusters were compared by the number of medications used, VAS asthma levels and Control of Asthma and Allergic Rhinitis Test (CARAT) levels. Findings: We assessed a total of 8,075 MASK-air® users. The main clustering approach resulted in the identification of seven groups. These groups were interpreted as probable: (i) severe/uncontrolled asthma despite treatment (11.9-16.1% of MASK-air® users); (ii) treated and partly-controlled asthma (6.3-9.7%); (iii) treated and controlled asthma (4.6-5.5%); (iv) untreated uncontrolled asthma (18.2-20.5%); (v) untreated partly-controlled asthma (10.1-10.7%); (vi) untreated controlled asthma (6.7-8.5%) and (vii) no evidence of asthma (33.0-40.2%). This classification was validated in a study of 192 patients enrolled by physicians. Interpretation: We identified seven profiles based on the probability of having asthma and on its level of control. mHealth tools are hypothesis-generating and complement classical epidemiological approaches in identifying patients with asthma.
