Abstract

Blue rubber bleb nevus syndrome (BRBNS), also called Bean's syndrome, is a rare disease associated with multiple venous malformations in the skin and gastrointestinal (GI) tract. Dermatological lesions, which are the first clinically visible manifestations, appear as skin-colored compressible protuberances or as dark-blue venous nodules, rubbery in consistency. Central nervous system (CNS) manifestations are rare, variable, non-specific, and tend to occur late in the disease, mainly reported as seizures and focal neurological deficits secondary to compression. Most cases occur sporadically, however, an autosomal dominant inheritance pattern has been reported. A 74-year-old male with history of focal epilepsy secondary to possible neurocysticercosis presented at the emergency department due to sudden onset of aphasia, left central facial paralysis, and dysphagia secondary to catastrophic intracerebral hemorrhage. Cerebral MRI showed multiple cerebral cavernous malformations (CCM)-like lesions and, on the general exploration, multiple dark-blue nodules, rubbery in consistency. One week later he died due to complicated pneumonia; a brain autopsy was performed showing multiple vascular malformations. His son had a history of focal epilepsy presumed to be related to neurocysticercosis. He had the same skin lesions and brain MRI pattern. Histological analysis of the skin lesions of the two cases showed venous vascular malformations. A non-systematic review was carried out, in which all case reports of blue nevus syndrome with neurological manifestations in adults were included.

12. Factors Predicting Prolonged Mechanical Ventilation in Guillain–Barré Syndrome

Abstract

ABSTRACT:Background:Up to 30% of patients with Guillain–Barré syndrome require mechanical ventilation and 5% die due to acute complications of mechanical ventilation. There is a considerable group of patients that will need prolonged mechanical ventilation (considered as >14 days) and should be considered for early tracheostomy. The objective of this study is to identify risk factors for prolonged mechanical ventilation.Methods:We prospectively analyzed patients with Guillain–Barré diagnosis with versus without prolonged mechanical ventilation. We considered clinical and electrophysiological characteristics and analyzed factors associated with prolonged mechanical ventilation.Results:Three hundred and three patients were included; 29% required mechanical ventilation. When comparing the groups, patients with prolonged invasive mechanical ventilation (IMV) have a lower score on the Medical Research Council score (19.5 ± 16.2 vs 27.4 ± 17.5, p= 0.03) and a higher frequency of dysautonomia (42.3% vs 19.4%, p= 0.037), as well as lower amplitudes of the distal compound muscle action potential (CMAP) of the median nerve [0.37 (RIQ 0.07–2.25) vs. 3.9 (RIQ1.2–6.4), p= <0.001] and ulnar nerve [0.37 (RIQ0.0–3.72) vs 1.5 (RIQ0.3–6.6), p= <0.001], and higher frequency of severe axonal damage in these nerves (distal CMAP ≤ 1.0 mV). Through binary logistic regression, severe axonal degeneration of the median nerve is an independent risk factor for prolonged IMV OR 4.9 (95%CI 1.1–21.5) p= 0.03, AUC of 0.774, (95%CI 0.66–0.88), p= < 0.001.Conclusions:Severe median nerve damage is an independent risk factor for prolonged mechanical ventilation.

Abstract

Epilepsy is a condition that frequently coexists with significant morbi-mortality levels, seriously affects the quality of life and, in up to one third of patients, is refractory to a variety of treatment approaches. Artificial intelligence (AI) has largely benefitted the study, treatment, and prognosis of patients with epilepsy through the course of recent years. These achievements applied the fields of diagnosis, automated seizure prediction, advanced seizure monitoring and electroencephalogram, use of genetics in diagnosis and management, imaging algorithms in the treatment, neuromodulation, and robotic surgery. This review conveys the actual and future directions of AI. a branch of science that has shown promising results in the treatment and diagnosis of patients with epilepsy. La epilepsia es una enfermedad que frecuentemente conlleva significativos niveles de morbi-mortalidad, afecta seriamente la calidad de vida y, en cerca de un tercio de los pacientes, es refractaria a diversos tratamientos. La inteligencia artificial (IA) ha beneficiado el estudio, tratamiento y pronóstico de los pacientes con epilepsia a través de los años. Estos logros abarcan diagnóstico, predicción de crisis automatizada, monitoreo avanzado de crisis epilépticas y electroencefalograma, uso de recursos genéticos en manejo y diagnóstico, algoritmos en imagen y tratamiento, neuromodulación y cirugía robótica. La presente revisión explica de forma práctica los avances actuales y futuros de la inteligencia artificial, rama de la ciencia que ha mostrado resultados prometedores en el diagnóstico y tratamiento de pacientes con epilepsia.
