【Abstract】Fracture of the proximal humerus metaphysis with coexistent dislocation of the shoulder in chil- dren is a rare injury. The injury often occurs as a consequence of high velocity trauma. Most fractures of the proximal humerus commonly associated with the epiphysis in children can be treated with closed reduction. We presented a case of 5-year-old girl who sustained this type of fracture- dislocation of the shoulder. Open reduction and internal fixation with multiple smooth K-wires was performed. At two years follow-up, the patient was pain free and regained full range of motion.

Objective : This study planed to evaluate effect of oriental medicine therapy treatment method of dyspnea that happen from cancer of the pancreas patient who accompany Multiple metasis Methods : Patient complained dyspnoea during admission into dept . Of internal medicine, college of oriental medicine, Dong-eui Univ ,was appealed patient treated by Oriental medicine therapy that is of use Herb Medication and Herbal -acupuncture Treatment being diagnosed in lung asthenia including deficiency of Eum and insufficiency of Gi of the lung(폐허(肺虛)), the fire due to deficiency(허화(虛火)), and loving warfare of symptoms evaluated through VAS (visual analog scales). Results & Conclusion : Patient's difficulty in breathing symptoms took a favorable turn after treatment. This study means that Oriental medicine therapy that is difficulty in breathing symptoms that happen from patient surely has effectiveness.

This research proposes a new method to detect R-peaks in electrocardiogram by using the prediction value from adaptive linear neuron (ADALINE) artificial neural network. With this aim, the weights of four input neurons in ADALINE are updated for each encoded ECG vector-segment and the value of an output neuron is compared with the actual ECG followed by applying finite impulse response filter. Our simulated experiments with the MIT-BIH ECG database that represents the long-term recordings from the heart disease patients show that our proposed algorithm can detect R-peaks in ECG data with the accuracy of more than 99%.

Abstract Background Polymorphisms of the human prion protein gene (PRNP) contribute to the genetic determinants of Creutzfeldt-Jakob disease (CJD). Numerous polymorphisms in the promoter regions as well as the open reading frame of PRNP were investigated. Greater than 90% of Korean, Chinese, and Japanese carry the homozygote 129 MM codon. In Korea, polymorphisms have not been comprehensively studied, except codons 129 and 219 in PRNP among Korean CJD cases. Although polymorphisms at codons 129 and 219 play an important role in susceptibility to sporadic CJD, patients with other polymorphisms in PRNP exhibited critical distinctions of clinical symptoms. Methods The genetic analyses of PRNP were carried out among probable CJD patients in comparison with the results from magnetic resonance imaging (MRI) and electroencephalogram (EEG). Results The molecular analyses revealed that three mutations at codons D178N, E200K, and M232R in heterozygosity. Patients with the D178N and M232R mutations had a 129MM codon, whereas the patient with the E200K mutation showed 129MV heterozygosity. They all revealed strong 14-3-3 positive signals. The 67-year-old patient with the D178N-129M mutation showed progressive gait disturbance and dysarthria was in progress. The 58-year-old patient with the E200K mutation coupled to the 129MV codon had gait disturbance, dysarthria, agitation, and ataxic gait, and progressed rapidly to death 3 months from the first onset of symptoms. The 65-year-old patient with the M232R mutation showed rapidly progressive memory decline and gait disturbance, and died within 16 months after onset of symptoms. Conclusion Despite differences in ethnicity, the clinical and pathological outcomes were similar to the respective mutations around the world, except absence of insomnia in D178N-129M subject.

Change detection using SAR data has been an active topic in various applications. Because conventional change detection identifies signal changes in single-pol radar observations, they cannot separately detect different kinds of change associated with different ground parameters. In this study, we investigated the comprehensive use of dual-pol parameters and proposed a novel dual-pol-based change detection framework utilizing different dual-pol scatter-type indicators. To optimize the exploitation of dual-pol change information, we presented a two-step processing strategy that divides the multiclass change detection process into a binary detection step that identifies the presence of changes and the classification step that distinguishes the types of change. In the detection stage, each dual-pol parameter was considered as an independent information source. Assuming potential conflict between dual-pol parameters, a disjunctive combination of detection results from different dual-pol parameters was applied to obtain the final detection result. In the classification step, an unsupervised change classification strategy was proposed based on the change direction and magnitude of the dual-pol parameters within the change class. Experimental results exhibited significantly improved detectability across a wide change spectrum compared with previous dual-pol-based change detection approaches. They also demonstrated the possibility of distinguishing different semantic changes without in situ ground data. [ABSTRACT FROM AUTHOR]
