BACKGROUND: In a randomized, controlled trial that compared liraglutide, a glucagon-like peptide 1 analogue, with placebo in patients with type 2 diabetes and high cardiovascular risk who were receiving usual care, we found that liraglutide resulted in lower risks of the primary end point (nonfatal myocardial infarction, nonfatal stroke, or death from cardiovascular causes) and death. However, the long-term effects of liraglutide on renal outcomes in patients with type 2 diabetes are unknown. METHODS: We report the prespecified secondary renal outcomes of that randomized, controlled trial in which patients were assigned to receive liraglutide or placebo. The secondary renal outcome was a composite of new-onset persistent macroalbuminuria, persistent doubling of the serum creatinine level, end-stage renal disease, or death due to renal disease. The risk of renal outcomes was determined with the use of time-to-event analyses with an intention-to-treat approach. Changes in the estimated glomerular filtration rate and albuminuria were also analyzed. RESULTS: A total of 9340 patients underwent randomization, and the median follow-up of the patients was 3.84 years. The renal outcome occurred in fewer participants in the liraglutide group than in the placebo group (268 of 4668 patients vs. 337 of 4672; hazard ratio, 0.78; 95% confidence interval [CI], 0.67 to 0.92; P=0.003). This result was driven primarily by the new onset of persistent macroalbuminuria, which occurred in fewer participants in the liraglutide group than in the placebo group (161 vs. 215 patients; hazard ratio, 0.74; 95% CI, 0.60 to 0.91; P=0.004). The rates of renal adverse events were similar in the liraglutide group and the placebo group (15.1 events and 16.5 events per 1000 patient-years), including the rate of acute kidney injury (7.1 and 6.2 events per 1000 patient-years, respectively). CONCLUSIONS: This prespecified secondary analysis shows that, when added to usual care, liraglutide resulted in lower rates of the development and progression of diabetic kidney disease than placebo. Objective: Self-perceived health status may be helpful in identifying patients at high risk for adverse outcomes. The Euro Heart Survey on Coronary Revascularization (EHS-CR) provided an opportunity to explore whether impaired health status was a predictor of 1-year mortality in patients with coronary artery disease (CAD) undergoing angiographic procedures. Methods: Data from the EHS-CR that included 5619 patients from 31 member countries of the European Society of Cardiology were used. Inclusion criteria for the current study were completion of a self-report measure of health status, the EuroQol Questionnaire (EQ-5D) at discharge and information on 1-year follow-up, resulting in a study population of 3786 patients. Aims: Revascularization in patients with coronary artery disease changed over the last two decades, favouring the number of patients treated by means of percutaneous coronary interventions (PCI) when compared with coronary artery bypass grafting (CABG). Many randomized controlled trials (RCTs) have been performed to compare these two competing revascularization techniques. Because of the strict enrolment criteria of RCTs in which highly selected patients are recruited, the applicability of the results may be limited in clinical practice. The current study evaluates to what extent patients in clinical practice were similar to those who participated in RCTs comparing PCI with CABG. Methods and results: Clinical characteristics and 1-year outcome of 4713 patients enrolled in the Euro Heart Survey on Coronary Revascularization were compared with 8647 patients who participated in 14 major RCTs, comparing PCI with CABG. In addition, we analysed which proportion of survey patients would have disqualified for trial participation (n = 3033, 64%), aiming at identifying differences between trial-eligible and trial-ineligible survey patients. In general, important differences were observed between trial participants and survey patients. Patients in clinical practice were older, more often had comorbid conditions, single-vessel disease, and left main stem stenosis when compared with trial participants. Almost identical differences were observed between trial-eligible and trial-ineligible survey patients. In clinical practice, PCI was the treatment of choice, even in patients who were trial-ineligible (46% PCI, 26% CABG, 28% medical). PCI remained the preferred treatment option in patients with multi-vessel disease (57% in trial-eligible and 40% in trial-ineligible patients, respectively, P < 0.001); yet, the risk profile of patients treated by PCI was better than that for patients treated either by CABG or by medical therapy. In the RCTs, there was no mortality difference between PCI and CABG. In clinical practice, however, we observed 1-year unadjusted survival benefit for PCI vs. CABG (2.9 vs. 5.4%, P < 0.001). Survival benefit was only observed in trial-ineligible patients (3.3 vs. 6.2%, P < 0.001). Conclusion: Many patients in clinical practice were not represented in RCTs. Moreover, only 36% of these patients were considered eligible for participating in a trial comparing PCI with CABG. We demonstrated that RCTs included younger patients with a better cardiovascular risk profile when compared with patients in everyday clinical practice. This study highlights the disparity between patients in clinical practice and patients in whom the studies that provide the evidence for treatment guidelines are performed. © The European Society of Cardiology 2006. - Abstract
Die Extraktion von Pflanzeninhaltsstoffen mit überkritischem Kohlendioxid ist ein in den sechziger Jahren entwickeltes Verfahren, mit dem definierte Inhaltsstoffe gezielt angereichert werden können. Die Löslichkeit von polaren oder polymeren Substanzen in CO2 ist jedoch begrenzt. Die vorliegende Arbeit hatte einerseits die Erweiterung der Extraktionsmöglichkeiten auf polarere Substanzen zum Ziel, zum anderen sollten unerwünschte Begleitsubstanzen, welche nachteilige Nebenwirkungen aufweisen können, adsorptiv während des Extraktionsvorganges entfernt werden. Die zu diesem Zweck untersuchten Rohstoffe waren Camellia sinensis, Calendula officinalis L. und Piper methystikum., Extraktion of plant-ingredients with supercritical carbon dioxide, called supercritical fluid Extraktion (SFE) has been established in the sixties. This method provides the possibility to enrich defined ingredients of plants in the produced extracts. A remaining problem is the limited solubility of polar or polymer substances in supercritical CO2. Therefore, the following thesis had two aims. The possibilities of SFE should be expanded towards more polar substances and, in a second step, undesired substances, which may cause negative side-effects, should be removed from the extracts by adsorptive processes during the extraction. The tested plant-material was Camellia sinensis, Calendula officinalis L. and Piper methystikum.

Es wurde gezeigt, dass die Zerschäumungsanalyse eine effiziente Anreicherungsmethode für oberflächenaktive, auch nicht schaumbildende Verbindungen aus wässrigen Extrakten vieler Nutz- und Heilpflanzen sowie Gewürzen ist. Dabei konnten sowohl wertgebende Inhaltsstoffe angereichert werden (Antioxidantien aus Rosmarin, Scharfstoffe aus Ingwer, Carotinoide aus Fruchtsäften, Bitterstoffe aus Bier, Curcuminoide und Turmerone aus Kurkuma) als auch Inhaltsstoffe mit unerwünschter Wirkung eliminiert werden (Glykoalkaloide aus Kartofellpreßsaft und Flavokavine aus der Kava-Kava-Wurzelpflanze). Nicht nur die verschiedenen Substanzgruppen zeigten bei der Zerschäumung unterschiedliches Anreicherungsverhalten, sondern auch homologe Verbindungen einer Substanzgruppe (strukturell bedingte unterschiedliche Polarität und Schaumaktivität)., It could be shown that foam separation is an efficient method for the enrichment of surface active as well as not foaming substances from aqueous extracts of spices and herbs. Different useful substance groups, such as antioxidants from rosmarinum, pungent principles from ginger, carotinoids from fruit juices, bitter principles from beer, and curcuminoids and tumerones from curcuma, could be enriched, and constituents with adverse effects, such as glycoalcaloids from potatoe juice and flavocavins from kava-kava roots, could be eliminated. Die vorliegende Arbeit befaßt sich mit der Synthese und Charakterisierung wesentlicher Inhaltsstoffe des Multikomponenten Insektizids Toxaphen, welches seit seiner Herstellung im Jahr 1945 als weltweit meist eingesetzter Wirkstoff bezeichnet wird. Die Darstellung einzelner Komponenten erfolgte über photolytische Chlorierung drei unterschiedlicher Ausgangsstoffe mit anschließender Isolierung der entstandenen Gemische über kombinierte Normalphasen-Säulenchromatographie. Eine Zuordnung als Inhaltsstoffe konnte mit Hilfe von GC/MS-EI- und NCI-Messungen getroffen werden, während die Strukturaufklärungen anhand von 1H- und 13C-NMR-Untersuchungen, Massenspektrometrie und Röntgen-strukturanalyse erfolgreich gelöst wurden., Main topic of the present thesis is the synthesis and characterization of essential single congeners from the multi-component insecticide toxaphene, one of the worlds highest production volume insecticides since its discovery in 1945. Single congeners were obtained by photochlorination of three different starting compounds and separation from the resulting chlorinated mixtures by combined normal phase column chromatography. Isolated compounds were identified as toxaphene congeners by GC/MS-EI- and NICI and structures were elucidated by 1H-, 13C-NMR-spectroscopy and X-ray structural analysis.

Ziel der vorliegenden Arbeit war, ein Extraktionsverfahren mit überkritischen Gasen zur Gewinnung von Wirk- und Wertstoffen aus kontaminierten pflanzlichen Rohstoffen so zu modifizieren, dass die im Rohmaterial enthaltenen Pflanzenschutzmitteln und Kontaminanten während dieses Extraktionsverfahrens entfernt werden, ohne dabei Qualitätseinbußen des Extraktes hinzunehmen. Dafür wurden Versuche mit von der Industrie festgestellten Extraktionsbedingungen und anschließender Inline-Adsorption durchgeführt. Nachdem das Verhalten des Systems mit dotierter Cellulose als Modelmatrix untersucht wurde. Sind diese Ergebnisse auf belasteten pflanzlichen Rohstoffen, wie z. B. Chili (Capsicum frutescens) und Hopfen (Humulus lupulus L.), übertragen worden., The goal of the current work was the production of uncontaminated extracts from plant raw materials contaminated by pesticide. This will be done by using supercritical carbon dioxide for the enrichment of desired active ingredients. The developed process should also produce high extraction yields of the desired substances. Es wurde gezeigt, dass die Zerschäumungsanalyse eine effiziente Anreicherungsmethode für oberflächenaktive, auch nicht schaumbildende Verbindungen aus wässrigen Extrakten vieler Nutz- und Heilpflanzen sowie Gewürzen ist. Dabei konnten sowohl wertgebende Inhaltsstoffe angereichert werden (Antioxidantien aus Rosmarin, Scharfstoffe aus Ingwer, Carotinoide aus Fruchtsäften, Bitterstoffe aus Bier, Curcuminoide und Turmerone aus Kurkuma) als auch Inhaltsstoffe mit unerwünschter Wirkung eliminiert werden (Glykoalkaloide aus Kartofellpreßsaft und Flavokavine aus der Kava-Kava-Wurzelpflanze). Nicht nur die verschiedenen Substanzgruppen zeigten bei der Zerschäumung unterschiedliches Anreicherungsverhalten, sondern auch homologe Verbindungen einer Substanzgruppe (strukturell bedingte unterschiedliche Polarität und Schaumaktivität)., It could be shown that foam separation is an efficient method for the enrichment of surface active as well as not foaming substances from aqueous extracts of spices and herbs. Different useful substance groups, such as antioxidants from rosmarinum, pungent principles from ginger, carotinoids from fruit juices, bitter principles from beer, and curcuminoids and tumerones from curcuma, could be enriched, and constituents with adverse effects, such as glycoalcaloids from potatoe juice and flavocavins from kava-kava roots, could be eliminated. - Abstract
In this research, foam fractionation a method belonging to the Adsorptive Bubble Separation Methods, was employed to enrich the following active principles contained in medically valuable plants: Faradiol esters from Calendula officinalis, catechins from Camellia sinensis, tryptanthrin from Isatis Tinctoria and cannabinoids from Cannabis sativa. The enrichment methods have been developed in the batch mode for these principles aqueous dilute solutions, based on their physicochemical nature and the process parameters and conditions. The hydrophobicity of faradiol esters, tryptanthrin, cannabinoids and catechins complexes with caffeine, permitted their enrichments. Optimisation was achieved for foam fractionation columns with high residence times, the utilisation of additives and by adjusting the solutions pH values, In dieser Forschungsarbeit wurde eine Methode der Schaumfraktionierung (engl. Adsorptive Bubble Separation) angewandt, um folgende, biologisch aktive Verbindungen aus wertvollen Pflanzenmaterialien anzureichern: Faradiolester aus Calendula officinalis, Catechine aus Camellia sinensis, Tryptanthrin aus Isatis Tinctoria und Cannabinoide aus Cannabis sativa. Die Anreicherungsmethoden für wässrige Lösungen mit den jeweiligen Verbindungen wurden im Batch-Verfahren entwickelt, basierend auf den physico-chemischen Eigenschaften der Verbindungen sowie den Prozessparametern und -Konditionen. Aufgrund der Hydrophobizität konnten die genannten Verbindungen effektiv angereichert werden, wobei dies für Catechine nur im Komplex mit Koffein möglich war. Optimale Anreicherungen konnten mit zeitlich langer Fraktionierungszeit, angepasstem pH-Wert sowie mit Zusatz von Additiven erreicht werden

Ziel der Arbeit war es Enzyme unter Erhalt der katalytischen Aktivität mittels Zerschäumungsanalyse anzureichern. Als Modellenzyme dienten das extrazelluläre Enzym Laccase von Trametes spec. und Meerrettich-Peroxidase aus Armoracia rusticana. Zunächst wurde Laccase aus SNL-Nährmedium unter Variation verschiedener Prozessparameter im Batch-, Kreislauf-, Kolonnen- und kontinuierlichen Verfahren zerschäumt. Um die Effizienz der Zerschäumung zu steigern wurde das sog. Pinzetten-Verfahren entwickelt. Dieses Verfahren basiert auf Enzym-Ligand-Wechselwirkungen, welche auf die Komplexierung von Metallkationen im aktiven Zentrum des Enzyms mittels chelatisierendem Tensid zurückgeführt wurden. Durch die Integration des Pinzettenverfahrens in die Zerschäumungsanalyse konnte sowohl die Anreicherung von Laccase als auch von Meerrettich-Peroxidase deutlich gegenüber der konventionellen Batch-Zerschäumung gesteigert werden., The aim of the current work was to enrich active enzymes by foam fractionation. As model enzymes served laccase from Trametes spec. and horseradish peroxidase from Armoracia rusticana. At first, laccase was enriched from culture broth via commonly applied foam fractionation in batch-, cycle- and continous mode, under different constantly optimized process parameters. In order to increase the efficiency of enrichment, the so-called tweezing foam fractionation was developed. The tweezing foam fractionation is based on enzyme-ligand interaction, which was ascribed to the chelation of the metal cation in the active center of the enzyme through chelating surfactants. The method successively increased the enrichment of laccse, which in furtherance could be validated by application of this principle on horseradish peroxidase.

In dieser Arbeit wird die adsorptive Zerschäumung vorgestellt, mit der bestimmte Proteinfraktionen pH-selektiv im Schaum angereichert werden können. Dabei wurden verschiedene, die Zerschäumung beeinflussende Parameter wie Verdünnung, Gasflussgeschwindigkeit, Salz- bzw. Zuckerkonzentration und pH-Wert an den Referenzproteinen Rinderhämoglobin und –albumin sowie an Kartoffelfruchtwasser und Kartoffelschlempe als Realprobe mittels diskontinuierlichem Verfahren untersucht. Ferner wurde ein kontinuierliches Zerschäumungsverfahren entwickelt, das durch verschiedene apparative Modifikationen optimiert wurde., At first, the influence of different parameters such as protein concentration, gas flow, ionic strength and pH-value was elucidated with aqueous sample solutions of the reference proteins bovine haemoglobin and albumin using a discontinuous foaming procedure. In a series of application the effects of these parameters were tested with potato juice and potato slop. To optimize protein enrichment, the batch mode was replaced by continuous foam fractionation. For this purpose certain modifications of the equipment were examined.

- Abstract
With the production of pesticide-free extracts as an objective, this research investigations commenced with the extraction of spices of various provenance. Firstly, by a conventional solvent extraction and ascertaining the level of pesticide contamination. This was followed by fluid CO2 extraction. The level of pesticide contamination for these extracts from both extraction methods were compared. The work proceeded with the investigation of various parameters influencing pesticides behavior during fluid carbon dioxide extraction. The knowledge gained therefrom, was utilized for an on-line reduction or eventual elimination of pesticide residues during the extraction procedure. Furthermore, a countercurrent based clean up methods was developed for the analysis of pesticide residues in high-lipid-content plant extracts., Fünf repräsentative Gewürzproben aus verschiedenen Ländern wurden auf ihre Pestizidrückstände hin untersucht. Mit Hilfe der erzielten Daten wurde ein Belastungsprofil erstellt. Danach erfolgte die Fluidextraktion, um herauszufinden, welche der Rückstände sich bevorzugt in den HD-Extrakten anreichern. Mit Hilfe der Variation der physikalisch-chemischen Parameter der Extraktion und der Anwendung einer simultanen Extraktion-Chromatographischen Diskriminierungsmethode wurden wissenschaftlichen Kriterien ausgegearbeitet, die zur Reduzierung bzw. vollständigen Eliminierung von Pflanzenschutzmittelrückständen in den Gewürzextrakten führen. Dabei wurde besonders darauf geachtet, daß die Qualität und Stabilität der HD-Extrakte erhalten bleibt. Ferner wurde eine auf Gegenstrom-Chromatograhie basierende Aufarbeitungsmethode für die Analyse von Pesitiziden in lipidreichem Pflanzenextrakt entwickelt.

Polychlor-n-Alkane (PCA), auch bekannt als chlorierte Paraffine (CP), sind chlororganische Verbindungen, die seit Anfang der dreißiger Jahre des zwanzigsten Jahrhunderts zum industriellen Gebrauch hergestellt wurden. Heute stellt diese Verbindungsklasse weltweit die größte Gruppe an chlorierten Kohlenwasserstoffen dar, die produziert bzw. verbraucht werden. Handelsübliche Formulierungen bestehen aus einem Gemisch von Tausenden von PCA-Komponenten und werden in kurzkettige (C10-C13), mittellange (C14-C17) und langkettige (C>17) PCA unterteilt. Bedingt durch die komplexe Zusammensetzung der PCA sowie durch die Schwierigkeiten, die bei ihrer Aufarbeitung bzw. Identifizierung und Quantifizierung bestehen, gibt es weltweit im Vergleich zu anderen anthropogenen Stoffen relativ wenig brauchbare Daten über PCA-Konzentrationen in der Ökosphäre. Im Rahmen dieser Arbeit wurde in erster Linie ein selektives Aufarbeitungsverfahren für die Trennung der kurz- und mittelkettigen PCA, von störenden Lipiden und Organochlorinterferenzen (COV), insbesondere von Toxaphen, in ausgewählten fetthaltigen Lebensmitteln, entwickelt. Darüber hinaus wurden für die Bestimmung der PCA-Rückständen verschiedene unabhängige Detektionsmethoden und unterschiedliche externe Standards herangezogen. Die mit den verschiedenen Methoden erzielten Ergebnisse wurden schließlich miteinander verglichen und bezüglich ihrer Umweltrelevanz bewertet. Das hier entwickelte Clean-up-Verfahren enthält drei Reinigungsschritte. Der erste Schritt umfaßt die simultane Säulenextraktion der Fette, der mit den Fetten angereicherten PCA und anderen COV sowie die Zersetzung der Fette und anderer H2SO4-labiler Verbindungen. Im zweiten Schritt fand die Zersetzung der restliche Fette (0,01%) sowie die PCA-Abtrennung von den meisten interferierenden COV statt. Anschließend erfolgte im dritten Schritt mittels Gelpermeationschromatographie die quantitative PCA-Abtrennung von den übrig gebliebenen Interferenzen. Die Wiederfindungsrate, Polychloro-n-alkanes (PCAs), also known as chlorinated paraffins (CPs), are chlorinated organic compounds, that are produced since the beginning of 1930´s for industrial application. Today, this class of compounds represents the largest group among the chlorinated hydrocarbons, that are produced and find application. The PCA formulations consist of a mixture of thousands of PCA components, which can be categorised into short chain (C10-C13), medium chain (C14-C17) and long chain (C>17) compounds. Because of the complex composition of the PCA mixtures and the difficulties involved in the work-up, identification and quantification of their components, compared to the other anthropogenic compounds, world wide relatively little useful data are available on PCA concentrations in the ecosphere. In the frame of this work, primarily, a selective clean-up technique is developed for separation of the short and medium chain PCAs from lipid impurities and interfering organochlorine compounds (OCs), especially from toxaphene, in selected fatty foods. Furthermore, for determination of the PCA residues different independent detection methods and several external standards are applied. The results thus obtained with the different methods are compared with each other and their environmental relevance assessed. The clean-up technique developed here consists of three (work-up) steps. The first one includes the simultaneous column extraction of the fatty material, the PCAs and other interfering OCs accumulated into the fat, as well as the decomposition of the fat and other H2SO4-labile compounds. The second step involves the decomposition of the remaining lipids (0.01%), as well as the separation of the PCAs from the most part of the interfering compounds. In the concluding third step the quantitative PCAs separation from the rest of the interfering substances is carried out by a final Gel Permeation Chromatography step.
