Abstract

BackgroundWe analyzed the effect of time to initiation of antiretroviral therapy (ART) after diagnosis on the probability of HIV-1 transmission events (HIV-TE) in naïve HIV-1-infected men having sex with men (MSM).SettingMathematical model.MethodsWe used discrete event simulation modeling to estimate the probability of HIV-TE in the first 8 weeks after ART initiation; we varied ART initiation from D0 to D28 after simulated "diagnosis". The model inputs used sexual behavior parameters from the MSM population of the START trial, and transmission rates per-sex act and HIV-1 RNA from recent meta-analyses. HIV-1 RNA decay curves were modeled from the databases of Single (efavirenz [EFV] v dolutegravir [DTG]), Spring-2 (raltegravir [RAL] v DTG), and Flamingo (darunavir/ritonavir [DRVr] v DTG) trials.ResultsWe found that the number of HIV-TE per index patient in the first 8 weeks after ART initiation increased linearly for same-day ART to initiation on day 28. Small but statistically significant advantages of integrase strand transfer inhibitors (INSTI) over EFV and DRVr were found.ConclusionsRapid, if not same-day initiation of INSTI-based ART to newly diagnosed HIV-infected MSM has the potential for substantial public health benefits related to decreases in HIV-TE.

Abstract

BackgroundInitiation of antiretroviral therapy (ART) for HIV infection using regimens that include integrase strand transfer inhibitors (INSTIs) is associated with a faster decline in HIV-1 RNA than what is observed with regimens that are anchored by other ART drug classes. We compared the impact of ART regimens that include dolutegravir (DTG), raltegravir (RAL), efavirenz (EFV), or darunavir/ritonavir (DRV/r), in treatment naïve men who have sex with men (MSM) on the probability of HIV-1 sexual transmission events (HIV-TE).SettingMathematical model.MethodsWe used discrete event simulation modeling to estimate HIV-TE during the first 8 weeks after initiation of ART. HIV-1 RNA decay in men was modeled from the databases of three clinical trials: Single (DTG vs. EFV), Spring-2 (DTG vs. RAL) and Flamingo (DTG vs. DRV/r).ResultsAll regimens substantially reduced the number of HIV-TE compared to no treatment. DTG led to fewer HIV-TE than its comparator in each of the three trials: 22.72% fewer transmissions than EFV; 0.52% fewer transmissions than RAL; and 38.67% fewer transmissions than DRV/r. The number of patients needed to treat with DTG to prevent one transmission event instead of comparator was 48 vs EFV, 2,194 vs RAL, and 31 vs DRV/r.ConclusionUnsurprisingly, this mathematical model showed that all regimens reduced HIV-TE compared to no treatment. The results also suggest that that initial use of INSTIs, by virtue of their superior viral decay kinetics, have the potential to reduce HIV-1 horizontal transmission following initiation of ART in naïve MSM.Trial registrationClinicalTrials.gov NCT03183154.

Abstract

En un mundo enormemente digitalizado, resulta de vital importancia fomentar la lectura, especialmente la de clásicos intemporales como es el caso de Drácula. No obstante, la pasión por la lectura no es un factor común en la sociedad moderna. Un medio para acercar estos clásicos son los videojuegos estrategia. El proyecto consiste en crear un videojuego en el que el usuario supere retos y pruebas a través de un viaje adentrándose en el mundo literario que componen los vampiros de Drácula y Carmilla y la condesa sangrienta Elisabeth Bathory. El videojuego está dividido en una parte narrativa-RPG y una estrategia a modo de combate con cartas y tablero., In a highly digitalised world, it is vitally important to encourage reading, especially the reading of timeless classics such as Dracula. However, a passion for reading is not a common factor in modern society. One way to bring these classics closer to the public is through video game strategy. The project consists of developing a video game where the user overcomes challenges and tests through a journey into the literary world of the vampires Dracula, Carmilla and the bloody countess Elisabeth Bathory. The video game is divided into a narrative-RPG part and a strategy part in the form of card and board combat.

Abstract

[Abstract] Background: Atherosclerotic cardiovascular diseases (ASCVD) and dyslipidemia are associated to a higher risk of cardiovascular events, mortality, use of healthcare resources and costs. In Spain, the evidence about the administration of lipid-lowering treatments in clinical practice, and their clinical effectiveness in patients with ASCVD and hypercholesterolemia and patients with FH is scarce. Therefore, a multidisciplinary working group of cardiologists, family physicians, internal medicine specialists and neurologists was gathered for the Reality study. The aim of this study is to describe the demographic and clinical characteristics, comorbidities, and concomitant medication of patients with ASCVD and hypercholesterolemia and of patients with familial hypercholesterolemia (FH). The use of healthcare resources and costs associated to the management of these diseases after their diagnosis were also considered. Methods: This is an observational and retrospective study, based on the BIG-PAC® database, which includes the electronic medical registries (EMRs) of 1.8 million people from 7 Autonomous Communities in Spain (including public primary care centers and hospitals). The study includes patients who had a new or recurrent episode of ASCVD during the recruitment period (from 01/01/2017 to 31/12/2018). The index date will be defined as the date of the ASCVD event, and the follow-up period will be 24 months. According to their first diagnosis in the database, patients will be classified as ASCVD (5 groups: stable/unstable angina, acute myocardial infarction, ischemic stroke, transient ischemic attack, and peripheral arterial disease) or FH. Discussion: This study aims to analyze the treatment patterns and use of healthcare resources of ASCVD and FH in Spain. The prevalence of these disorders will also be estimated. Abstract

The electrochemical supercapacitor performance of MnO2is significantly influenced by the phase structure due to the various structural features of the different MnO2polymorphs that include tunnels or layered structures that can facilitate ion transport and intercalation. However, the effect of the crystal structure of MnO2within MnO2/carbon composites has not been fully explored or understood. Herein, we have synthesized different crystal structures of MnO2(α- and β-MnO2) within MnO2/reduced graphene oxide (rGO) composites by a hydrothermal process using various amounts of (NH4)2SO4, followed by systematic structural characterization and electrochemical capacitance measurements. An excellent capacitance performance of 403 F g–1was observed in α-MnO2/sulfur and nitrogen codoped reduced graphene oxide (S,N-rGO) composites because of the interconnection between the conductive porous 3D architectures of S,N-rGO and the α-MnO2nanorods. This work highlights how the morphology, phase structure, and mass loading of MnO2within MnO2/rGO composites directly influence the capacitance performance and rate capabilities, which provides insight into the design of MnO2-based composite materials for supercapacitor applications.

Abstract

The alkaline stability of functional cations tethered to anion exchange membranes (AEMs) is essential for long-term operation in electrochemical devices. Here, we report the use of AEMs with pure aliphatic polymer backbones containing N-spirocyclic quaternary ammonium cation groups as highly conductive, crosslinked, and reinforced separators. Synthesizing these pure aliphatic-based AEMs via irradiation with ultraviolet (UV) light at room temperature avoids the use of carcinogenic solvents and the solution casting step generally used in AEM preparation. The resultant aliphatic-based AEMs have a chloride ion conductivity of 82 mS cm−1 at 70 °C with an ion exchange capacity (IEC) of 3.0 ± 0.2 mmol g−1. The aliphatic-based AEM retains 40% of its initial IEC after immersion in 1 M KOH at 80 °C for 30 d. A direct nucleophilic substitution degradation mechanism is proposed for such AEMs, based on FT-IR, solid state 13C-NMR spectroscopy, and XPS. [ABSTRACT FROM AUTHOR]
