Abstract

Background: Typhoid and paratyphoid fever (enteric fever) is a common cause of non-specific febrile infection in adults and children presenting to health care facilities in low resource settings such as the South Asia. A 7-day course of a single oral antimicrobial such as ciprofloxacin, cefixime, or azithromycin is commonly used for its treatment. Increasing antimicrobial resistance threatens the effectiveness of these treatment choices. We hypothesize that combined treatment with azithromycin (active mainly intracellularly) and cefixime (active mainly extracellularly) will be a better option for the treatment of clinically suspected and culture-confirmed typhoid fever in South Asia.\ud \ud Methods: This is a phase IV, international multi-center, multi-country, comparative participant-and observer-blind, 1:1 randomised clinical trial. Patients with suspected uncomplicated typhoid fever will be randomized to one of the two interventions: Arm A: azithromycin 20mg/kg/day oral dose once daily (maximum 1gm/day) and cefixime 20mg/kg/day oral dose in two divided doses (maximum 400mg bd) for 7 days, Arm B: azithromycin 20mg/kg/day oral dose once daily (max 1gm/day) for 7 days AND cefixime-matched placebo for 7 days. We will recruit 1500 patients across sites in Bangladesh, India, Nepal, and Pakistan. We will assess whether treatment outcomes are better with the combination after one week of treatment and at one- and three-months follow-up.\ud \ud Discussion: Combined treatment may limit the emergence of resistance if one of the components is active against resistant sub-populations not covered by the other antimicrobial activity. If the combined treatment is better than the single antimicrobial treatment, this will be an important result for patients across South Asia and other typhoid endemic areas.\ud \ud Clinicaltrials.gov registration: NCT04349826 (16/04/2020)

Abstract

After the devastating effects of the 2015 earthquake in Nepal, the provisions for safe drinking water, personal hygiene, and sewage management were compromised among displaced people living in temporary shelters. Typhoid fever is endemic in Kathmandu valley, which is transmitted among people by the faecal?oral route and outbreaks can occur in post-disaster situations. To reduce the risk of transmission and outbreaks, typhoid vaccine was introduced for young children and adolescents for whom the risk of typhoid fever was highest. With the collaboration of Siddhi Memorial Hospital, Nepal Paediatric Society, and Nagasaki University, a typhoid vaccination campaign was implemented in Bhaktapur district in the valley. The campaign was conducted in all 23 temporary camps in the district. Among 4,263 children aged 2 to 15 years, 4,216 (98.9%) received a single dose of the typhoid Vi polysaccharide vaccine. Most of the children (47.8%) were 11 to 15 years of age, and girls were 50.2%. Only four children (0.1%) had an adverse event following immunization (AEFI). Local camp leaders, public health officials, and local youth clubs participated in the immunization programme. In a review of admissions to the local children’s hospital, there was no apparent increase in typhoid cases in the post-earthquake period. Abstract

Levels in 102Rh have been populated in the reaction 70Zn+36S at 130 MeV. The level structure of 102 Rh has been investigated using the EUROGRAM II array. Low-lying states and four high-spin bands have been identified. The configurations of low-lying levels and two-quasiparticle bands are interpreted in the frame of the interacting boson fermion fermion model. The four observed band structures are also compared with cranked shell model calculations using a modified oscillator potential.

New high-spin bands have been observed in this nucleus up to 45/2−, 57/2− and 65/2+ states. They have been interpreted using the Nilsson-Strutinsky cranking formalism as terminating configurations. Abstract

The paper describes an investigation of task lighting for multi-occupancy offices. The possibility of energy conservation and speculations about other advantages to be gained from using task lighting in offices has stimulated a renewed interest in this method of lighting. The present study was concerned with evaluating task lighting in a realistic office setting. A total of 10 installations exhibiting a wide range of characteristics were evaluated by different types of office workers, performing typical tasks in a simulated four person office. The participants' evaluations were recorded by questionnaire and the results are discussed. The general conclusion is that task/background lighting schemes which substantially reduce overall lighting levels from normal uniform lighting can be favourably regarded by office occupants. Abstract

Abstact: High spin states of 100Rh have been populated using the reaction 70Zn+36S at 130 MeV. -rays were detected with the EUROGAM2 array. The level structure of 100Rh has been extended up to 14.41 MeV excitation energy. Several band structures are observed. A band based on a I=8 state is developed up to the I=24 level. It is assigned as the g9/25h11/2 configuration.

Abstract

This thesis focuses on host-pathogen interactions in Salmonella Typhi and Burkholderia pseudomallei infections and explores the interplay between these bacteria and the innate immune system. Typhoid fever is one of the most common causes of bacterial infection in low-income countries. With adequate antibiotic treatment it has a low mortality rate. Melioidosis also commonly causes community-acquired sepsis in Southeast Asia and northern Australia but even with appropriate antibiotic treatment the mortality is high. During severe bacterial infections such as these, uncontrolled activation of the innate immune response can lead to detrimental systemic inflammation, intravascular coagulation, tissue injury, and eventually death. This thesis presents clinical and experimental studies in which the effects of a variety of proteins involved in the hyper-inflammatory response of the innate immune system during severe typhoid fever and melioidosis are investigated and compared.
