Abstract

This article describes initiatives undertaken by the Ohio University College of Osteopathic Medicine (OU-COM) to develop a continuum of physician training that emphasizes the integration of basic sciences throughout undergraduate and graduate medical education. These initiatives demonstrate the effectiveness of consortia in actually restructuring educational programs, decreasing fragmentation, and promoting continuity.

Abstract

Background Non-small cell lung cancer (NSCLC) frequently presents in advanced stages. A significant proportion of those with reportedly good ECOG performance status (PS) fail to receive planned multidisciplinary team (MDT) treatment, often for functional reasons, but an objective decline in physical performance is not well described. Sarcopenia, or loss of muscle mass, is an integral part of cancer cachexia. However, changes in both muscle mass and physical performance may predate clinically overt cachexia, and may be present even with normal body mass index. Physical fitness for treatment is currently subjectively assessed by means of the PS score, which may be inadequate in predicting tolerance to treatment. This study aims to evaluate whether measuring physical performance and muscle mass at baseline in NSCLC patients, in addition to PS score, is able to predict commencement and successful completion of MDT-planned treatment. Methods/design This is a prospective, single-centre exploratory study of NSCLC patients attending a Rapid Access Lung Cancer clinic. Baseline data collected are (methods in brackets): physical performance (Short Physical Performance Battery), muscle mass (bioelectrical impedance ± dual energy x-ray absorptiometry), patient and physician-assessed PS (ECOG and Karnofsky), nutritional status and presence of cachexia. Longitudinal data consists of receipt and completion of MDT treatment plan. The primary outcome measure is commencement of MDT-planned treatment, and important secondary outcomes include successful completion of treatment, length of stay in surgical patients, and risk of chemotherapy- and radiotherapy-related side effects. Discussion A more comprehensive assessment of phenotype, particularly with regards to physical performance and muscle mass, will provide additional discriminatory information of patients’ fitness for treatment. If positive, this study has the potential to identify targets for early intervention in those who are at risk of deterioration. This will subsequently enable optimisation of performance of patients with NSCLC, in anticipation of systemic treatment.

Abstract

In situ infrared (IR) spectroelectrochemistry is used to investigate the behavior of cyanide (CN) at polycrystalline platinum surfaces in aqueous perchlorate (CLO4(-) electrolyte, IR spectroelectrochemical data reveal the existence of a number of surface, as well as solution, cyanide species in the interfacial region. Within the double-layer potential region, there is IR evidence for several forms of absorbed cyanide. When the potential is made sufficiently positive, cyanide is oxidized to form cyanate. Other solution cyanide species which may be formed at the platinum-cyanide solution interface include hydrogen cyanide and square planar platinum cyanide complex.

15. In Situ Fourier Transform Infrared Spectroscopic Study of Bisulfate and Sulfate Adsorption on Gold, With and Without the Underpotential Deposition of Copper

Abstract

In situ surface infrared (IR) spectroelectrochemistry is used to investigate the adsorption of sulfate (SO4(2-)) and bisulfate (HSO(4-)) ions on polycrystalline gold surfaces in sodium sulfate and sulfuric acid, and also during copper underpotential deposition in sulfuric acid medium. In sodium sulfate solution, IR peaks due to surface sulfate and bisulfate are observed at potentials within the double-layer region on gold, and the ratio of IR peak intensities for sulfate to bisulfate increases as the applied potential is made more negative. In sulfuric acid, surface IR spectra indicate that adsorbed sulfate is favored at more positive potentials, while adsorbed bisulfate is prevalent at more negative voltages; also, a potential-dependent reorientation of water is observed in the spectra. IR spectroelectrochemical data from a sulfuric acid system containing copper sulfate indicate that adsorbed sulfate is present on gold at more positive potentials, and its coverage increases when underpotentially deposited (UPD) copper is present on the gold substrate surface.

16. Interpreting IR Difference Spectra

Abstract

From the evaluation of sample difference spectra based on Gaussian model peaks with known peak characteristics, it is shown that interpretation of some parameters from difference spectra, resulting from the rationing or subtraction of a poor background spectra, may be inaccurate or misleading. Difference spectra of this type are commonly observed using techniques such as subtractively normalized interfacial Fourier transform infrared spectroscopy, SNIFTIRS, electrochemically modulated infrared reflectance spectroscopy, EMIRS, or similar infrared spectroelectrochemical techniques, as well as some microsample analyses, studies of biochemical processes, and infrared astronomical observations, to name just a few samples. A mathematical evaluation of the problem is offered to demonstrate what information may realistically be gained from the characteristics of difference spectra. It is shown that in the worst case, where frequency, intensity, and peak width are all changing due to some perturbation of the sample (e.g., from temperature, or surface potential changes between background and sample spectra, etc.), even a qualitative interpretation may not be possible. In many practical cases, however, we show that at least a qualitative interpretation of the data can be obtained from difference spectra. Spectroelectrochemical applications for the calculations shown here are presented as examples, although these results impact a wider range of applications.

Abstract

As opposed to the much harsher conditions demanded in homogeneous solutions, a conducting polymer of toluene is readily formed by anodic oxidation of acetonitrile solutions containing the precursor. The fabrication of new conductive polymers is of interest due to the potential practical applications of these materials. We report here the electrochemical polymerization of a conducting polymer from toluene precursor. Anodic oxidation of concentrated solutions (0.5 M) toluene in anhydrous acetonitrile solution containing 0.15 M Bu4N+BF4-leads to the formation of a highly conductive poly(methylphenylene) polymer film on the surface of a platinum mirror electrode (bulk metal). The polymer can also be formed in solutions of lower concentrations (1-10 mM), but only after successive pulsing or sweeping techniques at high positive potentials (>1.8 vs Ag/Ag+) for long periods of time. Abstract

Poly(p-phenylene) (PPP) electrodes formed by anodic oxidation of biphenyl in acetonitrile solutions were examined. As a cell, we observe generally high discharge current densities and rapid discharge rates. PPP electrochemistry and cell performance were found to depend significantly on the nature of the supporting electrolyte present during electrosynthesis and doping. These electrodes were also found to catalyze O2 reduction. Raman and scanning electron microscopic results are presented. Preliminary electrochemical experiments for the the formation and characterization of the film were carried out in dry acetonitrile with Bu4NBF4 electrolyte, which had been freshly recrystallized twice from methylene chloride-diethyl ether and dried in vacuo at 75C for 24 h. Solutions were degassed with and carried out under Ar. Films were formed in the range of thickness 0.5-5 microns.

Abstract

Conducting silver and gold coatings on silicon Attenuated Total Reflectance (ATR) plates have been employed as transparent electrodes to monitor in situ surface electrochemistry. The multiple internal reflection Fourier transform infrared spectroscopy (MIRFTIRS) technique, used previously to study redox reactions on platinum and iron films, is applied in this work to the study of adsorption processes in the double-layer region, in particular, the adsorption of thiocyanate on silver and gold. The MIRFTIRS spectra were found to be essentially free of solution band interference and are compared to surface IR spectra of the same system obtained by other methods. Spectra of thiocyanate adsorbed on gold and silver surfaces have been recorded as a function of applied potential and thiocyanate concentration. Evidence of thiocyanate species absorbed to gold via both nitrogen and sulfur atoms has been obtained, while only S-bound thiocyanate was clearly observed on silver.
