1. Ready, Set & Go! An anticipatory action system against droughts

Abstract

The World Food Programme, in collaboration with the Mozambique National Meteorology Institute, is partnering with several governmental and non-governmental organizations to establish an advanced early warning system for droughts in pilot districts across Mozambique. The “Ready, Set & Go!” system is operational in Mozambique for activating anticipatory action (AA) against droughts based on predefined thresholds, triggers and pre-allocated financing. The system uses bias-corrected and downscaled seasonal forecasts from the European Centre for Medium-Range Weather Forecasts (ECMWF) as core information to anticipate severe reductions in rainfall during the rainy season. This information guides the implementation of actions to reduce the impacts of rainfall deficits in the critical window between a forecast and the onset of the drought event. Within this window of opportunity, the system releases an alert for readiness (Ready) and activation (Set) preceding the mobilization of anticipatory action on the ground (Go). With the recent adoption of the Southern African Development Community “Maputo Declaration on Bridging the Gap between Early Warning and Early Action”, member states have committed to enhancing the reach of early warning system by leaving no one behind. Therefore, there is a need to assess the opportunities and limitations of the Ready, Set & Go! system to scale up drought AA information to all districts in Mozambique. This study describes the Ready, Set & Go! system, which uses ensemble forecasts of the Standardized Precipitation Index to trigger anticipatory action against droughts on a seasonal timescale. The Ready, Set & Go! optimizes the use of seasonal forecast information by choosing triggers for anticipatory action based on verification statistics and on a double-confirmation process, which combines longer lead times with shorter lead time forecasts for issuing drought alerts. In this study, we show the strengths of the system by benchmarking it against three simpler triggering approaches. Our findings indicate that the Ready, Set & Go! system has significant potential to scale up AA activities against severe droughts throughout the entire rainy season, covering on average 76 % of the Mozambican districts. This approach outperforms the three benchmarked methods, demonstrating higher hit rates, extended lead times and a lower false alarm. If efforts are concentrated on the first part of the rainy season, national coverage against severe droughts could be expanded to 87 % of all districts. By aligning with the objectives outlined in the “Maputo Declaration” and the “Early Warning for All” initiative, this research contributes to safeguarding communities against the adverse impacts of climate-related events, aligning with the ambitious goal of universal protection by 2027.

6. Pro/Anti-Inflammatory Cytokine Imbalance in Postischemic Left Ventricular Remodeling

Objectives. Cytokines play an important role in left ventricular remodeling consequent to myocardial ischemia. The aim of this study was to correlate cytokine production and lymphocyte apoptosis to post-ischemic left ventricular remodeling in patients affected by acute myocardial infarction (AMI) undergoing primary cutaneous angioplasty (PCI). Methods. In 40 patients, affected by AMI and undergoing PCI, we evaluated peripheral blood mononuclear cells (PBMCs), tumor necrosis factor-alpha (TNF-𝛼) and interleukin 10 (IL10) production and apoptosis on day 1, day 3, day 7, 1 month and 6 months after PCI. Patients were divided into two subgroups of remodeling or not remodeling by echocardiographic criteria. Results. In the subgroup of remodeling patients, at each timepoint TNF-𝛼 production was increased significantly in comparison with the subgroup of not remodeling patients. IL10 production was statistically lower in remodeling subjects than in not remodeling ones 1 and 6 months after reperfusion. There were no differences between the two groups as regards lymphomonocyte apoptosis. Conclusions. We found an increased production of pro-inflammatory cytokine TNF-𝛼 and a corresponding decrease of anti-inflammatory/regulatory cytokine IL10 in remodeling patients and we concluded that this cytokine imbalance resulted in pro-inflammatory effects which might contribute to the progression of left ventricular remodeling.

Abstract

Thermal monopoles, identified after Abelian projection as magnetic currents wrapping non-trivially around the thermal circle, are studied in $N_f = 2+1$ QCD at the physical point. The distribution in the number of wrappings, which in pure gauge theories points to a condensation temperature coinciding with deconfinement, points in this case to around 275 MeV, almost twice the QCD crossover temperature $T_c$; similar indications emerge looking for the formation of a percolating current cluster. Abstract

The contribution aims to investigate the different relationships between territory, digitalisation and work.Within the European Union, there are significant territorial inequalities aggravated by globalisation-related dynamics and the digital transition. Both processes, in fact, entail a territorial polarisation of economic development but also of technological skills, producing deep gaps within the member states. In this sense, the analysis of an instrument such as the European Globalisation Adjustment Fund aims to shed light on the importance of keeping investing in reskilling and upskilling workers and the local labour context together, in order to understand how to reduce economic and knowledge inequalities.

Abstract

In this paper, we build a computational model for the analysis of international wheat spot price formation, its dynamics and the dynamics of internationally exchanged quantities. The model has been calibrated using FAOSTAT data to evaluate its in-sample predictive power. The model is able to generate wheat prices in twelve international markets and wheat used quantities in twenty-four world regions. The time span considered goes from 1992 to 2013. In our study, a particular attention was paid to the impact of Russian Federation's 2010 grain export ban on wheat price and internationally traded quantities. Among other results, we find that wheat average weighted world price in 2013 would have been 3.55\% lower than the observed one if the Russian Federation would not have imposed the export ban in 2010., Comment: 24 pages, 14 figures, Submitted to the Journal of Artificial Societies and Social Simulation (JASSS)

The aim of this study was to evaluate the relation of inflammatory biochemical markers as neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR) and systemic inflammation index (SII) with POD occurrence.Methods: This was a single-center, retrospective, observational study. We analyzed the perioperative data of patients who had undergone elective vascular surgery procedures. The occurrence of delirium after procedure was used to divide the population in two groups: POD-pos and POD-neg group. ROC curves were performed to find the appropriate cut-off values of NLR, PLR and SII. Multivariate analysis was used to identify the independent predictors for POD.Results: A total of 646 patients were enrolled. Mean age was 76.2± 9.8 years, 68.4% were male. Seventy-three patients (11.3%) developed POD. Mean hospital stay was significantly increased in the POD-pos group (6.1± 5.4 vs 3.2± 2.8 days, p=0.0001). In-hospital reinterventions were more frequent in the POD-pos group (8.2% vs 3.8%). Blood values analysis reported significant differences: Hb, NLR, PLR, SII, creatinine and RCP were strongly increased (p< 0.05) in the POD-pos group. ROC curves identified cut-off values for NLR> 3.57, PLR> 139.2 and SII> 676.4. Multivariate analysis revealed that age, Renal Failure, peripheral revascularization procedures, major amputation, general anesthesia, hospitalization in the previous month, NLR> 3.57 and SII> 676.4 were independent risk factors for POD.Conclusion: POD represents a common complication of vascular surgery patients. Our study demonstrated that NLR, PLR and SII are reliable and readily available laboratory predictors of POD in vascular surgery that could help in POD risk-stratification.Keywords: delirium, vascular disease, risk assessment, vascular surgery, elderly

Abstract

BACKGROUND: The SAfety and FEasibility of standard EVAR outside the instruction for use (SAFE-EVAR) Study was designed to define the attitude of Italian vascular surgeons towards the use of standard endovascular repair (EVAR) for infrarenal abdominal aortic aneurysm (AAA) outside the instruction for use (IFU) through a Delphi consensus endorsed by the Italian Society of Vascular and Endovascular Surgery (Società Italiana di Chirurgia Vascolare ed Endovascolare - SI CVE). METHODS : A questionnaire consisting of 26 statements was developed, validated by an 18-member Advisory Board, and then sent to 600 Italian vascular surgeons. The Delphi process was structured in three subsequent rounds which took place between April and June 2023. In the first two rounds, respondents could indicate one of the following five degrees of agreement: 1) strongly agree; 2) partially agree; 3) neither agree nor disagree; 4) partially disagree; 5) strongly disagree; while in the third round only three different choices were proposed: 1) agree; 2) neither agree nor disagree; 3) disagree. We considered the consensus reached when ≥70% of respondents agreed on one of the options. After the conclusion of each round, a report describing the percentage distribution of the answers was sent to all the participants. RESULTS: Two-hundred-forty-four (40.6%) Italian Vascular Surgeons agreed to participate the first round of the Delphi Consensus; the second and the third rounds of the Delphi collected 230 responders (94.3% of the first-round responders). Four statements (15.4%) reached a consensus in the first rounds. Among the 22 remaining statements, one more consensus (3.8%) was achieved in the second round. Finally, seven more statements (26.9%) reached a consensus in the simplified last round. Globally, a consensus was reached for almost half of the proposed statements (46.1%). Abstract

Objective: Postoperative delirium (POD) is a common complication with a high health-related impact. The creation of a model (Siena posTopeRative dElirium in vaScular Surgery) to identify high-risk patients with consecutive prompt diagnosis and correct management. Methods: This is an observational retrospective study to evaluate POD incidence in patients who underwent elective vascular surgery procedures between 2018 and 2020. POD was detected using CAM and defined as the onset of an acute confusional state, clinically manifesting as a disturbed state of consciousness, cognitive dysfunction, or alteration in perception and behavior. The total population was divided in the development and validation subsamples. Multivariable logistic regression analysis was performed, identifying variables related to the occurrence of POD. An additive score was created and the STRESS score was internally validated using the Validation subgroup. Results: A total of 1067 patients were enrolled. POD occurred in 111 cases (10.4%). Multivariable logistic regression analysis for POD occurrence revealed as significant predictors: age>75 years, CKD, dyslipidaemia, psychiatric disease, CAD, hospitalization in the previous month, preoperative NLR >3.59, preoperative Hb < 12 g/dl, preoperative Barthel score <75, major amputation, CLTI revascularization, general anesthesia, and postoperative urinary catheter. These variables were used to create the STRESS score. The model was applied to both development and validation subgroups; AUC was respectively 0.7079 (p <.0001) and 0.7270 (p <.0001). Conclusion: The STRESS score has a good predictive potentiality for POD occurrence in elective vascular surgery procedures. Abstract

The World Food Programme, in collaboration with the Mozambique National Meteorology Institute, is partnering with several governmental and non-governmental organizations to establish an advanced early warning system for droughts in pilot districts across Mozambique. The "Ready, Set & Go!" system is operational in Mozambique for activating anticipatory action (AA) against droughts based on predefined thresholds, triggers and pre-allocated financing. The system uses bias-corrected and downscaled seasonal forecasts from the European Centre for Medium-Range Weather Forecasts (ECMWF) as core information to anticipate severe reductions in rainfall during the rainy season. This information guides the implementation of actions to reduce the impacts of rainfall deficits in the critical window between a forecast and the onset of the drought event. Within this window of opportunity, the system releases an alert for readiness (Ready) and activation (Set) preceding the mobilization of anticipatory action on the ground (Go). With the recent adoption of the Southern African Development Community "Maputo Declaration on Bridging the Gap between Early Warning and Early Action", member states have committed to enhancing the reach of early warning system by leaving no one behind. Therefore, there is a need to assess the opportunities and limitations of the Ready, Set & Go! system to scale up drought AA information to all districts in Mozambique. This study describes the Ready, Set & Go! system, which uses ensemble forecasts of the Standardized Precipitation Index to trigger anticipatory action against droughts on a seasonal timescale. The Ready, Set & Go! optimizes the use of seasonal forecast information by choosing triggers for anticipatory action based on verification statistics and on a double-confirmation process, which combines longer lead times with shorter lead time forecasts for issuing drought alerts. In this study, we show the strengths of the system by benchmarking it against three simpler triggering approaches. Our findings indicate that the Ready, Set & Go! system has significant potential to scale up AA activities against severe droughts throughout the entire rainy season, covering on average 76 % of the Mozambican districts. This approach outperforms the three benchmarked methods, demonstrating higher hit rates, extended lead times and a lower false alarm. If efforts are concentrated on the first part of the rainy season, national coverage against severe droughts could be expanded to 87 % of all districts. By aligning with the objectives outlined in the "Maputo Declaration" and the "Early Warning for All" initiative, this research contributes to safeguarding communities against the adverse impacts of climate-related events, aligning with the ambitious goal of universal protection by 2027. Abstract

Objectives: To analyse the general and primary outcome‐related characteristics of clinical trials protocols on COVID‐19 vaccines. Study Design and Setting: A meta‐research study. A search for clinical trial protocols on COVID‐19 vaccines was conducted on the ClinicalTrials.gov platform. We considered all protocols of comparative trials registered up to October 26, 2021. Results: Two hundred and eighty‐two trials were analysed. The median expected trial duration was 445 days (interquartile range [IQR] = 225), and the median target sample size was 420 participants (IQR = 1638). A retrospective registry (after the start date) was observed for 42.55% of the trials. Randomization procedures were planned by 84.75% and full‐blinding procedures by 34.75% of the 282 trials. Most trials were labelled as active or still recruiting, and 14 trials (5%) were completed. None of the 14 trials labelled as completed on our search date had results available. Industry funding was reported by 198 trials (70.2%). Most studies declared more than one primary outcome, usually a safety or immunogenicity outcome, and 59 studies (20.9%) had at least one primary efficacy outcome. The description of the primary efficacy outcomes was limited in most cases, referred to as a non‐specified 'efficacy' outcome (18.6%) or described as 'COVID‐19 cases' (32.2%). Conclusion: the primary outcomes of clinical trials on COVID‐19 vaccines are poorly described, and the registers provide insufficient information about them. The registry was retrospectively fulfilled for many trials, which may lead to bias and research waste.
