Abstract

Nuclear matter under extreme conditions can be studied in the laboratory with heavy-ion collisions. In this case one can indeed vary the incident energy and explore different impact parameter and rapidity regions to access densities that extend above and below saturation. This opportunity allows one to learn about the nuclear equation of state and its implications in astrophysics phenomena such as supernovae explosions and neutron stars properties. All these phenomena involve time scales that need to be accessed with two- and multi-particle correlation measurements. In this contribution a brief report of the FARCOS array will be presented. In particular, tests with alpha sources and beams and preliminary results from a recent experiment INKIISSY performed using a FARCOS prototype made of four telescopes will be presented.

Abstract

The KM3NeT research infrastructure is under construction in the Mediterranean Sea. It consists of two water Cherenkov neutrino detectors, ARCA and ORCA, aimed at neutrino astrophysics and oscillation research, respectively. Instrumenting a large volume of sea water with $\sim$ 6,200 optical modules comprising a total of $\sim$ 200,000 photomultiplier tubes, KM3NeT will achieve sensitivity to $\sim$ 10 MeV neutrinos from Galactic and near-Galactic core-collapse supernovae through the observation of coincident hits in photomultipliers above the background. In this paper, the sensitivity of KM3NeT to a supernova explosion is estimated from detailed analyses of background data from the first KM3NeT detection units and simulations of the neutrino signal. The KM3NeT observational horizon (for a $5\,\sigma$ discovery) covers essentially the Milky-Way and for the most optimistic model, extends to the Small Magellanic Cloud ($\sim$ 60 kpc). Detailed studies of the time profile of the neutrino signal allow assessment of the KM3NeT capability to determine the arrival time of the neutrino burst with a few milliseconds precision for sources up to 5$-$8 kpc away, and detecting the peculiar signature of the standing accretion shock instability if the core-collapse supernova explosion happens closer than 3$-$5 kpc, depending on the progenitor mass. Abstract

The optical module of the KM3NeT neutrino telescope is an innovative multi-faceted large area photodetection module. It contains 31 three-inch photomultiplier tubes in a single 0.44m diameter pressure-resistant glass sphere. The module is a sensory device also comprising calibration instruments and electronics for power, readout and data acquisition. It is capped with a breakout-box with electronics for connection to an electro-optical cable for power and long-distance communication to the onshore control station. The design of the module was qualified for the first time in the deep sea in 2013. Since then, the technology has been further improved to meet requirements of scalability, cost-effectiveness and high reliability. The module features a sub-nanosecond timing accuracy and a dynamic range allowing the measurement of a single photon up to a cascade of thousands of photons, suited for the measurement of the Cherenkov radiation induced in water by secondary particles from interactions of neutrinos with energies in the range of GeV to PeV. A distributed production model has been implemented for the delivery of more than 6000 modules in the coming few years with an average production rate of more than 100 modules per month. Abstract

Insertion of venous access devices (VAD) is usually considered a procedure with low risk of bleeding. Nonetheless, insertion of some devices is invasive enough to be associated with bleeding, especially in patients with previous coagulopathy or in treatment with antithrombotic drugs for cardiovascular disease. The current practices of platelet/plasma transfusion in coagulopathic patients and of temporary suspension of the antithrombotic treatment before VAD insertion are based on local policies and are often inadequately supported by evidence, since many of the clinical studies on this topic are not recent and are not of high quality. Furthermore, the protocols of antithrombotic treatment have changed during the last decade, after the introduction of new oral anticoagulant drugs. Though some guidelines address some of these issues in relation with specific procedures (port insertion, etc.), no evidence-based document covering all the aspects of this clinical problem is currently available. Thus, the Italian Group of Venous Access Devices (GAVeCeLT) has decided to develop a consensus on the management of antithrombotic treatment and bleeding disorders in patients requiring VADs. After a systematic review of the available evidence, the panel of the consensus (which included vascular access specialists, surgeons, intensivists, anesthetists, cardiologists, vascular medicine experts, nephrologists, infective disease specialists, and thrombotic disease specialists) has structured the final recommendations as detailed answers to three sets of questions: (1) which is an appropriate classification of VAD-related procedures based on the specific bleeding risk? (2) Which is the appropriate management of the patient with bleeding disorders candidate to VAD insertion/removal? (3) Which is the appropriate management of the patient on antithrombotic treatment candidate to VAD insertion/removal? Only statements reaching a complete agreement were included in the final recommendation

Abstract

FARCOS is a modular detection system intended to extend the capacity of 4π detectors both with higher granularity and resolution and with the ability to perform correlation measurements. The system, equipped at the present stage with the first 10 telescopes worked properly in the first experiments on beam paving the way to the construction of the remaining 10 telescopes that is presently under the way. To cope with the updated experimental cases FARCOS can investigate, we redesigned the frontend electronics board, widening the granularity of the selectable dynamic ranges and optimizing the slow control and calibration tools. In addition, we report on the successful commissioning of the FARCOS system.

Abstract

Background: The management of septic patients hospitalized in Internal Medicine wards represents a challenge due to their complexity and heterogeneity, and a high mortality rate. Among the available prognostic tools, procalcitonin (PCT) is considered a marker of bacterial infection. Fur-thermore, an association between vitamin D deficiency and poor sepsis-related outcomes has been described. Objectives: To evaluate the prognostic accuracy of two consecutive PCT determinations (Delta-PCT) and of vitamin D levels in predicting mortality in a population of patients with microbiological identified sepsis admitted to Internal Medicine wards. Methods: This is a sub-analysis of a previous prospective study. A total of 80 patients had at least two available consecutive PCT determinations, while 63 had also vitamin D. Delta-PCT was defined as a reduction of PCT > 50% after 48 h, >75% after 72 h, and >85% after 96 h. Mortality rate at 28-and 90-days were considered as main outcome. Results: Mortality rate was 18.7% at 28-days and 30.0% at 90-days. Baseline PCT levels did not differ between survived and deceased patients (28-days: p = 0.525; 90-days: p = 0.088). A significantly higher proportion of survived patients showed Delta-PCT (28-days: p = 0.002; 90-days: p < 0.001). Delta-PCT was associated with a lower 28-days (p = 0.007; OR = 0.12, 95%CI 0.02–0.46) and 90-days mortality (p = 0.001; OR = 0.17, 95%CI 0.06–0.48). A significantly higher proportion of deceased patients showed severe vitamin D deficiency (28-days: p = 0.047; 90-days: p = 0.049). Severe vitamin D deficiency was associated with a higher 28-days (p = 0.058; OR = 3.95, 95%CI 1.04–19.43) and 90-days mortality (p = 0.054; OR = 2.94, 95%CI 1.00–9.23). Conclusions: Delta-PCT and vitamin D represent two useful tests for predicting prognosis of septic patients admitted to Internal Medicine wards.

Abstract

FARCOS (Femtoscope ARray for Correlation and Spectroscopy) is a novel modular detection system featuring high angular and energy resolution able to reconstruct the particle’s momentum at high precision and capable of performing correlation measurements of Light Charged Particles (LCPs) and of LCPs and Intermediate Mass Fragments (IMFs). Given the unique feature of FARCOS of aiming at the identification in charge and mass even the fragments that are fully absorbed in the first Si layer, the frontend electronics must meet stringent requirements in terms of dynamic range and integral-nonlinearity. To this aim we designed and qualified a VLSI frontend able to readout signals of both polarities and suitable to be coupled with both the DSSSD and the photodiodes reading the scintillation light. During February 2017 for the first time the performance of a workhorse of the FARCOS telescope equipped with the designed VLSI frontend electronics has been experimentally qualified in a test beam at the Laboratori Nazionali del Sud (LNS) of INFN in Catania. The $^{\bf 16}$ O beam at 88 MeV accelerated by the Tandem accelerator has been used impinging on different targets (LiF, Au, C, Ni). This contribution focuses on the results of the test beam, highlighting the performance of the designed frontend electronics when coupled with the different detection stages of the FARCOS telescopes.

Abstract

Anderson-Fabry disease (FD) is a rare genetic, progressive, and multi-systemic condition, with X-linked inheritance. This is caused by pathogenic variants in the GLA gene, coding for the lysosomal enzyme called alpha-galactosidase A (aGLA), responsible for the cleavage of globotriaosylceramide (Gb3). The reduced or absent activity of aGLA causes the intracellular accumulation of Gb3, particularly in smooth and endothelial muscle cells, which causes cellular dysfunction. The main organs involved are the central nervous system, heart, and kidneys. However, being a ubiquitous enzyme, FD disease must be considered a systemic disease involving the peripheral nervous system, ocular and audio-vestibular systems. Also, the vascular district is damaged but the pathophysiology of vasculopathy in FD is not yet entirely understood. In literature, many vascular diagnostic tests were used to evaluate this specific involvement in FD, i.e., carotid intima media thickness (cIMT), arterial stiffness (AS), flow-mediated dilation (FMD) and atherosclerotic plaques; evaluation of vascular calcifications in FD patients is not presently available. In this review, we examined the current available literature on vascular aspects in FD. Abstract

Summary Radiotherapy techniques deliver ionizing radiations (X-rays, photons, electrons, protons, etc.) inside cancerous tissues to kill the abnormal cells. Radiotherapy-related activities such as the optimization of the therapeutic radiation dose to patients, workers' radioprotection, linear accelerator commissioning, quality assurance processes and technical innovations of linear accelerators are strongly based on the ability to predict the dose distribution. Monte Carlo based simulations are so far the most accurate tool for the calculation of dosimetric parameters, with the drawback of requiring extensive computing resources to achieve statistically meaningful results in a reasonable time frame. In the last years, advanced cancer treatment clinical and research communities have used e-Infrastructures to support their activities. The present paper reports on the development of a computing facility for helping clinical researchers in using modern R&E networking and distributed computing and storage resources for a new radiotherapy technique: the Intra-Operative Electron Radiotherapy. The web application developed addresses some technical and clinical needs as the design of linear accelerators' collimation systems and the optimization of the patient therapeutic dose distribution.

Abstract

Sepsis represents a global health problem in terms of morbidity, mortality, social and economic costs. Although usually managed in Intensive Care Units, sepsis showed an increased prevalence among Internal Medicine wards in the last decade. This is substantially due to the ageing of population and to multi-morbidity. These characteristics represent both a risk factor for sepsis and a relative contra-indication for the admission to Intensive Care Units. Although there is a lack of literature on the management of sepsis in Internal Medicine, the outcome of these patients seems to be gradually improving. This is due to Internists' increased adherence to guidelines and "bundles". The routine use of SOFA score helps physicians in the definition of septic patients, even if the optimal score has still to come. Point-of-care ultrasonography, lactates, procalcitonin and beta-d-glucan are of help for treatment optimization. The purpose of this narrative review is to focus on the management of sepsis in Internal Medicine departments, particularly on crucial concepts regarding diagnosis, risk assessment and treatment. Key Messages Sepsis is a life-threatening organ dysfunction caused by a dysregulated host response to infection. The prevalence of sepsis is constantly increasing, affecting more hospital patients than any other disease. At least half of patients affected by sepsis are admitted to Internal Medicine wards. Adherence to guidelines, routine use of clinical and lab scores and point-of-care ultrasonography are of help for early recognition of septic patients and treatment optimization.

Abstract

OBJECTIVE: Sepsis is a life-threatening disease resulting from the interaction between pathogen and host response; its dysregulation causes organ dysfunction, high morbidity, and mortality. Despite the increase of septic patients admitted to Internal Medicine wards, data about clinical predictors of mortality in this setting are still lacking. The aim of this study was to evaluate the role of MEDS score and vitamin D as predictors of mortality (28-day and 90-day) in septic patients admitted to the Internal Medicine department. PATIENTS AND METHODS: Prospectively collected clinical data, lab tests including vitamin D, and clinical scores (SIRS, MEDS, SCS, REMS, SOFA, qSOFA) were retrospectively analyzed. Eightyeight microbiologically identified septic patients (median age 75 years old, IQR 65-82 years old; range 37-94 years old) were evaluated. RESULTS: Twenty-three patients (26.1%) died at 28 days, 33 (37.5%) died at 90 days. The logistic regression showed a positive effect of MEDS score (p=0.006; OR 1.24, 95% CI 1.08-1.49), and a negative effect of low vitamin D levels (p=0.008, OR 0.83, 95% CI 0.72-0.94) on mortality. Moreover, the cut-off of 7 points for MEDS score and of 7 ng/ml for vitamin D levels significantly predicted poor prognosis at 28 and 90 days. CONCLUSIONS: MEDS score and vitamin D levels represent independent predictors of mortality in a cohort of Internal Medicine septic patients. Further studies on larger samples are needed to confirm our results and to clarify the pathophysiological mechanisms at the basis of vitamin D deficiency as a predictor of mortality in septic patients. 25. Pharmacomechanical catheter-directed thrombolysis for acute iliofemoral deep vein thrombosis: our case series.

Abstract

OBJECTIVE: This is a retrospective study on Pharmacomechanical Catheter- Directed Thrombolysis (PCDT) in the treatment of acute iliofemoral Deep Vein Thrombosis (DVT). PATIENTS AND METHODS: From March 2013 to November 2016, 22 patients (26 limbs), median age 46.7 years with acute (<21 days) extensive iliofemoral DVT underwent Percutaneous Mechanical Thrombectomy (PMT) with Aspirex (Straub Medical, Wangs, Switzerland), followed by Catheter- Directed Thrombolysis (CDT). Subsequent endovascular stenting was performed for underlying obstruction. The follow-ups were conducted up to 1 year, in two Centers by experienced operators. Post-Thrombotic Syndrome (PTS) was evaluated by assessing the Villalta Scale (VS) and measuring orthostatic venous pressure. RESULTS: Post-operative iliofemoral vein patency was restored in almost all cases (95.5%). Standard urokinase dose was 80.000 IU per hour; mean infusion time was 32.5 hours. Stenting was performed in 15 cases (68%). Median follow-up was 19.9 months (6-48 months); 21/22 patients completed the 12 months follow-up. At 30 days follow-up symptoms disappeared in 21/22 cases (95.5%), with one case (4.5%) of DVT recurrence. At 1-year follow- up there were 3 cases (14.2%) of mild PTS; 18 patients (85.8%) were free from PTS. At 1-year follow-up venous pressure measurement showed normal values in 11 cases (52.4%), mild hypertension in 7 patients (33.3%), moderate hypertension (80-100 mmHg) in 2 cases (9.5%) and severe hypertension (110 mmHg) in one case (4.8%). Neither major nor minor complications were observed. CONCLUSIONS: PMT with Aspirex combined with CDT with urokinase seems to be a safe and effective treatment for acute iliofemoral DVT and it shows promising results in reducing the risk of PTS. Abstract

Radiotherapy techniques deliver ionizing radiations (X-rays, photons, electrons, protons, etc.) inside cancerous tissues to kill the abnormal cells. Radiotherapy-related activities such as the optimization of the therapeutic radiation dose to patients, workers' radioprotection, linear accelerator commissioning, quality assurance processes and technical innovations of linear accelerators are strongly based on the ability to predict the dose distribution. Monte Carlo based simulations are so far the most accurate tool for the calculation of dosimetric parameters, with the drawback of requiring extensive computing resources to achieve statistically meaningful results in a reasonable time frame. In the last years, advanced cancer treatment clinical and research communities have used e-Infrastructures to support their activities. The present paper reports on the development of a computing facility for helping clinical researchers in using modern R&E networking and distributed computing and storage resources for a new radiotherapy technique: the Intra-Operative Electron Radiotherapy. The web application developed addresses some technical and clinical needs as the design of linear accelerators' collimation systems and the optimization of the patient therapeutic dose distribution. Abstract

In the framework of the MICADO (Measurement and Instrumentation for Cleaning And Decommissioning Operations) European Union (EU) project, aimed at the full digitization of low and intermediate-level radioactive waste (radwaste) management, a set of 32 solid state thermal neutron detectors, named SiLiF, and 36 gamma-ray counters based on a scintillating fiber readout at each end by a silicon photomultiplier, named SciFi, have been built and characterized. MICADO project encompasses a complete active and passive characterization of the radwaste drums with neutrons and gamma rays, followed by a longer-term monitoring phase. The detectors described are suitable for the monitoring of nuclear materials and can be used around radioactive waste drums possibly containing small quantities of actinides, as well as around spent fuel casks in interim storage or during transportation. Suitable polyethylene moderators, for the SiLiF detectors, can be exploited to better shape the detector response to the expected neutron spectrum, according to Monte Carlo simulations that were performed. The SciFi detectors were thoroughly tested with a 22Na and a 137Cs gamma-ray sources. Both detectors described were tested with an intense AmBe source of neutron and gamma ray. The results are satisfactory and show a quite uniform and reproducible behavior. The next step will be the test in a real environment.
