We review our experience with the application of magnetoencephalography (MEG) to the study of reorganization of the mechanisms supporting auditory language comprehension. In 3 studies, patient populations with cerebral insult of differing etiology, including epilepsy, surgical resection, and stroke, performed a running recognition task for spoken words while MEG data were collected using a whole-head magnetometer. Increased activation in the right hemisphere after left temporal lobectomy was associated with greater relative activation in that hemisphere preoperatively. Patients with chronic seizure disorder secondary to mesial temporal sclerosis exhibited a tendency toward an interhemispheric shift of language function, and those with epilepsy secondary to neoplasm showed a tendency toward an intrahemispheric shift. Patients with aphasia secondary to unilateral left-hemisphere stroke exhibited a more bilateral and diffuse overall profile of activation within the left hemisphere than control subjects of similar age. Taken together, results provide evidence that reorganization of cortex subserving auditory comprehension can occur well into the fifth and sixth decades and that the nature of the plastic response is dependent on variables such as premorbid language laterality, etiology, and, in specific groups, age at insult. We aimed to assess attitudes towards epilepsy and people with epilepsy (PWE) in Austria using a web-based survey including demographics, the Caveness questionnaire (CQ) and Scales of Attitudes toward People with Epilepsy questionnaire (SAPE). Furthermore, we compared our results with a previous Austrian study and investigated predictors for negative attitudes towards epilepsy and PWE. Results from 1017 adult respondents familiar with epilepsy revealed that 66.8% had had personal contact with PWE and 51.1% had experienced an epileptic seizure firsthand. However, only 28.3% of respondents were able to identify half of the correct answers regarding epilepsy as a disease and treatment options. This is highly important, as these were the strongest predictors for more positive attitudes towards PWE. In SAPE, another strong predictor for more favorable attitudes was prior personal contact with PWE. Compared to 2005, familiarity with epilepsy, personal contact with PWE as well as firsthand experience of seizures, and support for employing PWE like others increased significantly. While we report a favorable slight decrease of respondents who consider epilepsy a form of insanity, there was no significant difference in objection of their children having contact with children with epilepsy and marriage of their children with PWE. Thus, we demonstrate that even though familiarity with epilepsy and personal contact with PWE increased in Austria, there are still negative attitudes towards epilepsy and PWE present. Introduction: A novel focal cortex stimulation (FCS) device has recently received approval in Europe for patients with focal drug-resistant epilepsy (DRE). After 6 months of stimulation, 17 of 32 patients achieved ≥50% reduction in seizure frequency compared with their prestimulation baseline (responders). Currently, there is no established method for predicting FCS treatment response prior to implantation., Methods and Analysis: This is an ongoing combined retrospective-prospective non-interventional multicentre study. Clinical data of up to 100 patients treated with FCS are collected across 20 collaborating epilepsy centres in four European countries. The key outcome parameters, seizure frequency and severity, are measured along with metrics on cognition, mood and quality of life, both pre-electrode and postelectrode implantation. The data are complemented by demographics, medical history and information on antiseizure medication and FCS treatment parameters during the stimulation period. In addition to clinical data, MRI and electroencephalography registrations are used to gain insights into spatial and electrophysiological aspects of FCS. Multivariate statistical and machine learning analyses are employed to identify key predictive biomarkers associated with patient outcomes (responders vs non-responders). The primary goal is to improve counselling for DRE patients by identifying promising candidates for FCS treatment., Ethics and Dissemination: This study has received approval from the ethics committee of the University of Freiburg, Germany (23-1540 S1; 23-1183_1-S1-retro). The same approval is applicable for all participating centres in Germany as part of a multicentre study. Background: Temporal lobe epilepsy (TLE) can lead to structural brain abnormalities, with thalamus atrophy being the most common extratemporal alteration. This study used probabilistic tractography to investigate the structural connectivity between individual thalamic nuclei and the hippocampus in TLE., Methods: Thirty-six TLE patients who underwent pre-surgical 3 Tesla magnetic resonance imaging (MRI) and 18 healthy controls were enrolled in this study. Patients were subdivided into TLE with HS (TLE-HS) and MRI-negative TLE (TLE-MRneg). Tractography and whole brain segmentation, including thalamus parcellation, were performed to determine the number of streamlines per mm 3 between the thalamic nuclei and hippocampus. Connectivity strength and volume of regions were correlated with clinical data., Results: The volume of the entire thalamus ipsilateral to seizure onset was significantly decreased in TLE-HS compared to controls (Mann-Whitney-U test: p FDR < 0.01) with the anterior thalamic nuclei (ANT) as important contributor. Furthermore, decreased ipsilateral connectivity strength between the hippocampus and ANT was detected in TLE-HS (p FDR < 0.01) compared to TLE-MRneg and controls which correlated negatively with the duration of epilepsy (ρ = -0.512, p = 0.025) and positively with seizure frequency (ρ = 0.603, p = 0.006). Moreover, ANT volume correlated negatively with epilepsy duration in TLE-HS (ρ = -0.471, p = 0.042)., Conclusions: ANT showed atrophy and decreased connectivity in TLE-HS, which correlated with epilepsy duration and seizure frequency. Understanding the dynamics of epileptogenic networks has the potential to shed light on surgery-resistant epilepsy and refine the selection process for ideal neurosurgical candidates, consequently enhancing post-surgical outcomes., (© 2025 The Author(s).
