Abstract

This study aims to develop a digital twin (DT) framework to enhance adaptive proton stereotactic body radiation therapy (SBRT) for prostate cancer. Prostate SBRT has emerged as a leading option for external beam radiotherapy due to its effectiveness and reduced treatment duration. However, interfractional anatomy variations can impact treatment outcomes. This study seeks to address these uncertainties using DT concept, with the goal of improving treatment quality, potentially revolutionizing prostate radiotherapy to offer personalized treatment solutions. Our study presented a pioneering approach that leverages DT technology to enhance adaptive proton SBRT. The framework improves treatment plans by utilizing patient-specific CTV setup uncertainty, which is usually smaller than conventional clinical setups. This research contributes to the ongoing efforts to enhance the efficiency and efficacy of prostate radiotherapy, with ultimate goals of improving patient outcomes and life quality.

Abstract

Allogeneic hematopoietic cell transplantation (HCT) can be complicated by life-threatening organ toxicity and infection necessitating intensive care. Epidemiologic data have been limited by single-center studies, poor database granularity, and a lack of long-term survivors. To identify contemporary trends in intensive care unit (ICU) use and long-term outcomes, we merged data from the Center for International Blood and Marrow Transplant Research and the Virtual Pediatric Systems databases. We identified 6995 pediatric patients with HCT aged ≤21 years who underwent first allogeneic HCT between 2008 and 2014 across 69 centers in the United States or Canada and followed patients until the year 2020. ICU admission was required for 1067 patients (8.3% by day +100, 12.8% by 1 year, and 15.3% by 5 years after HCT), and was linked to demographic background, pretransplant organ toxicity, allograft type and HLA-match, and the development of graft-versus-host disease or malignancy relapse. Survival to ICU discharge was 85.7%, but more than half of ICU survivors required ICU readmission, leading to 52.5% and 42.6% survival at 1- and 5-years post-ICU transfer, respectively. ICU survival was worse among patients with malignant disease, poor pretransplant organ function, and alloreactivity risk factors. Abstract

We present CrystalBox, a novel, model-agnostic, posthoc explainability framework for Deep Reinforcement Learning (DRL) controllers in the large family of input-driven environments which includes computer systems. We combine the natural decomposability of reward functions in input-driven environments with the explanatory power of decomposed returns. We propose an efficient algorithm to generate future-based explanations across both discrete and continuous control environments. Using applications such as adaptive bitrate streaming and congestion control, we demonstrate CrystalBox's capability to generate high-fidelity explanations. We further illustrate its higher utility across three practical use cases: contrastive explanations, network observability, and guided reward design, as opposed to prior explainability techniques that identify salient features.

6. Plume: A Framework for High Performance Deep RL Network Controllers via Prioritized Trace Sampling

Abstract

Deep Reinforcement Learning (DRL) has shown promise in various networking environments. However, these environments present several fundamental challenges for standard DRL techniques. They are difficult to explore and exhibit high levels of noise and uncertainty. Although these challenges complicate the training process, we find that in practice we can substantially mitigate their effects and even achieve state-of-the-art real-world performance by addressing a factor that has been previously overlooked: the skewed input trace distribution in DRL training datasets. We introduce a generalized framework, Plume, to automatically identify and balance the skew using a three-stage process. First, we identify the critical features that determine the behavior of the traces. Second, we classify the traces into clusters. Finally, we prioritize the salient clusters to improve the overall performance of the controller. Plume seamlessly works across DRL algorithms, without requiring any changes to the DRL workflow. We evaluated Plume on three networking environments, including Adaptive Bitrate Streaming, Congestion Control, and Load Balancing. Plume offers superior performance in both simulation and real-world settings, across different controllers and DRL algorithms. For example, our novel ABR controller, Gelato trained with Plume consistently outperforms prior state-of-the-art controllers on the live streaming platform Puffer for over a year. It is the first controller on the platform to deliver statistically significant improvements in both video quality and stalling, decreasing stalls by as much as 75%.

Abstract

We investigated the impact of the number of induction/consolidation cycles on outcomes of 3113 adult AML patients who received allogeneic hematopoietic cell transplantation (allo-HCT) between 2008 and 2019. Patients received allo-HCT using myeloablative (MAC) or reduced-intensity (RIC) conditioning in first complete remission (CR) or with primary induction failure (PIF). Patients who received MAC allo-HCT in CR after 1 induction cycle had 1.3-fold better overall survival (OS) than 2 cycles to CR and 1.47-fold better than ≥3 cycles. OS after CR in 2 or ≥3 cycles was similar. Relapse risk was 1.65-fold greater in patients receiving ≥3 cycles to achieve CR. After RIC allo-HCT, the number of induction cycles to CR did not affect OS. Compared to CR in 1 cycle, relapse risk was 1.24-1.41-fold greater in patients receiving 2 or ≥3 cycles. For patients receiving only 1 cycle to CR, consolidation therapy prior to MAC allo-HCT was associated with improved OS vs. no consolidation therapy. Detectable MRD at the time of MAC allo-HCT did not impact outcomes while detectable MRD preceding RIC allo-HCT was associated with an increased risk of relapse. For allo-HCT in PIF, OS was significantly worse than allo-HCT in CR after 1-3 cycles.

Abstract

Post-transplantation cyclophosphamide (PTCy) has been shown to effectively control graft-versus-host disease (GvHD) in haploidentical (Haplo) transplantations. In this retrospective registry study, we compared GvHD organ distribution, severity, and outcomes in patients with GvHD occurring after Haplo transplantation with PTCy GvHD prophylaxis (Haplo/PTCy) versus HLA-matched unrelated donor transplantation with conventional prophylaxis (MUD/conventional). We evaluated 2 cohorts: patients with grade 2 to 4 acute GvHD (aGvHD) including 264 and 1163 recipients of Haplo and MUD transplants; and patients with any chronic GvHD (cGvHD) including 206 and 1018 recipients of Haplo and MUD transplants, respectively. In comparison with MUD/conventional transplantation ± antithymocyte globulin (ATG), grade 3-4 aGvHD (28% versus 39%, P = .001), stage 3-4 lower gastrointestinal (GI) tract aGvHD (14% versus 21%, P = .01), and chronic GI GvHD (21% versus 31%, P = .006) were less common after Haplo/PTCy transplantation. In patients with grade 2-4 aGvHD, cGvHD rate after Haplo/PTCY was also lower (hazard ratio [HR] = .4, P < .001) in comparison with MUD/conventional transplantation without ATG in the nonmyeloablative conditioning setting. Irrespective of the use of ATG, non-relapse mortality rate was lower (HR = .6, P = .01) after Haplo/PTCy transplantation, except for transplants that were from a female donor into a male recipient. In patients with cGvHD, irrespective of ATG use, Haplo/PTCy transplantation had lower non-relapse mortality rates (HR = .6, P = .04). Mortality rate was higher (HR = 1.6, P = .03) during, but not after (HR = .9, P = .6) the first 6 months after cGvHD diagnosis. Our results suggest that PTCy-based GvHD prophylaxis mitigates the development of GI GvHD and may translate into lower GvHD-related non-relapse mortality rate.

Abstract

The Space-Weather ANalytics for Solar Flares (SWAN-SF) is a multivariate time series benchmark dataset recently created to serve the heliophysics community as a testbed for solar flare forecasting models. SWAN-SF contains 54 unique features, with 24 quantitative features computed from the photospheric magnetic field maps of active regions, describing their precedent flare activity. In this study, for the first time, we systematically attacked the problem of quantifying the relevance of these features to the ambitious task of flare forecasting. We implemented an end-to-end pipeline for preprocessing, feature selection, and evaluation phases. We incorporated 24 Feature Subset Selection (FSS) algorithms, including multivariate and univariate, supervised and unsupervised, wrappers and filters. We methodologically compared the results of different FSS algorithms, both on the multivariate time series and vectorized formats, and tested their correlation and reliability, to the extent possible, by using the selected features for flare forecasting on unseen data, in univariate and multivariate fashions. We concluded our investigation with a report of the best FSS methods in terms of their top-k features, and the analysis of the findings. The role of haploidentical hematopoietic cell transplantation (HCT) using posttransplant cyclophosphamide (PTCy) for acute lymphoblastic leukemia (ALL) is being defined. We performed a retrospective, multivariable analysis comparing outcomes of HCT approaches by donor for adults with ALL in remission. The primary objective was to compare overall survival (OS) among haploidentical HCTs using PTCy and HLA-matched sibling donor (MSD), 8/8 HLA-matched unrelated donor (MUD), 7 /8 HLA-MUD, or umbilical cord blood (UCB) HCT. Comparing haploidentical HCT to MSD HCT, we found that OS, leukemia-free survival (LFS), nonrelapse mortality (NRM), relapse, and acute graft-versus-host disease (aGVHD) were not different but chronic GVHD (cGVHD) was higher in MSD HCT. Compared with MUD HCT, OS, LFS, and relapse were not different, but MUD HCT had increased NRM (hazard ratio [HR], 1.42; P = .02), grade 3 to 4 aGVHD (HR, 1.59; P = .005), and cGVHD. Compared with 7/8 UD HCT, LFS and relapse were not different, but 7/8 UD HCT had worse OS (HR, 1.38; P = .01) and increased NRM (HR, 2.13; P ≤ .001), grade 3 to 4 aGVHD (HR, 1.86; P = .003), and cGVHD (HR, 1.72; P ≤ .001). Compared with UCB HCT, late OS, late LFS, relapse, and cGVHD were not different but UCB HCT had worse early OS (≤18 months; HR, 1.93; P < .001), worse early LFS (HR, 1.40; P = .007) and increased incidences of NRM (HR, 2.08; P < .001) and grade 3 to 4 aGVHD (HR, 1.97; P < .001). Haploidentical HCT using PTCy showed no difference in survival but less GVHD compared with traditional MSD and MUD HCT and is the preferred alternative donor HCT option for adults with ALL in complete remission.

Abstract

The use of post-transplantation cyclophosphamide (PTCy) for graft-versus-host disease (GVHD) prophylaxis in recipients of haploidentical and fully matched transplantations is on the increase. Published studies have reported an increased incidence of cytomegalovirus (CMV) infection with the use of PTCy. Limited data exist on the incidence and outcomes of infection with non-CMV herpesviruses (NCHV) in this setting. The aim of this study was to evaluate the cumulative incidence of NCHV infections and the association of NCHV infections with transplantation-specific outcomes in recipients of haploidentical transplantation with PTCy (HaploCy), matched sibling donor transplantation with PTCy (SibCy), and matched sibling donor transplantation with calcineurin inhibitor-based prophylaxis (SibCNI). We hypothesized that, like CMV infection, HaploCy recipients of also will have a higher risk of NCHV infections. Using the Center for International Blood and Marrow Transplantation Research database, we analyzed 2765 patients (HaploCy, n = 757; SibCNI, n = 1605; SibCy, n = 403) who had undergone their first hematopoietic stem cell transplantation (HCT) between 2012 and 2017 for acute myelogenous leukemia, acute lymphoblastic leukemia, or myelodysplastic syndrome. The cumulative incidence of NCHV at 6 months post-NCT was 13.9% (99% confidence interval], 10.8% to 17.3%) in the HaploCy group, 10.7% (99% CI, 7.1% to 15%) in the SibCy group, and 5.7% (99% CI, 4.3% to 7.3%) in the Sib CNI group (P < .001). This was due primarily to a higher frequency of human herpesvirus 6 viremia reported in patients receiving PTCy. The incidence of Epstein-Barr viremia was low in all groups, and no cases of post-transplantation lymphoproliferative disorder were seen in either PTCy group. The incidence of NCHV organ disease was low in all 3 cohorts. The development of NCHV infection was associated with increased treatment-related mortality, particularly in the HaploCy group. There was no association with the development of GVHD, relapse, or disease-free survival. Patients in PTCy cohorts who did not develop NCHV infection had lower rates of cGVHD. This study demonstrates that the use of PTCy is associated with an increased risk of NCHV infection. The development of NCHV infection was associated with increased nonrelapse mortality, especially in the HaploCy group. Prospective trials should consider viral surveillance strategies in conjunction with assessment of immune reconstitution for a better understanding of the clinical relevance of viral reactivation in different HCT settings.

Abstract

Additive manufacturing (AM) of titanium (Ti) and Ti-6Al-4V lattices has been proposed for bone implants and augmentation devices. Ti and Ti-6Al-4V have favourable biocompatibility, corrosion resistance and fatigue strength for bone applications; yet, the optimal parameters for Ti-6Al-4V lattice designs corresponding to the natural micro- and meso-scale architecture of human trabecular and cortical bone are not well understood. A comprehensive review was completed to compare the natural lattice architecture properties in human bone to Ti and Ti-6Al-4V lattice structures for bone replacement and repair. Ti and Ti-6Al-4V lattice porosity has varied from 15% to 97% with most studies reporting a porosity between 50-70%. Cortical bone is roughly 5-15% porous and lattices with 50-70% porosity are able to achieve comparable stiffness, compressive strength, and yield strength. Trabecular bone has a reported porosity range from 70-90%, with trabecular thickness varying from 120-200 {\mu}m. Existing powder bed fusion technologies have produced strut and wall thicknesses ranging from 200-1669 {\mu}m. This suggests limited overlap between current AM of Ti and Ti-6Al-4V lattice structures and trabecular bone architecture, indicating that replicating natural trabecular bone parameters with latticing is prohibitively challenging. This review contributes to the body of knowledge by identifying the correspondence of Ti and Ti-6Al-4V lattices to the natural parameters of bone microarchitectures, and provides further guidance on the design and AM recommendations towards addressing recognized performance gaps with powder bed fusion technologies.

Abstract

LPBF of Al alloys is associated with numerous challenges when compared to other commonly used alloys due to their higher reflectivity and thermal conductivity. In this work, processing diagrams, temperature prediction models, XCT, and metallography are used for establishing criteria in process parameter optimization of high reflectivity Al alloys based on AlSi$_{10}$Mg response in using 57 different process parameter combinations - 21 using a focused Gaussian laser beam and 36 using divergent beams. For LPBF systems with focused beam diameters <100 {\mu}m, divergent beams obtained by defocusing to a position above the LPBF build plate primarily lead to conduction mode melt pools, while a focused beam leads to transition and keyhole mode melt pools. Conduction mode melting helps in avoiding keyhole mode defects, resulting in parts with densities >99.98%. Additionally, an analytical model-guided selection of laser power and velocity settings for a focused beam help in stabilizing melt pool and spatter dynamics in the transition melting mode thereby enabling a potential to obtain density values close to conduction mode densities (~99.98%). A dimensionless keyhole number (Ke) was derived in this work to identify distinct regions of conduction (Ke of 0-12), transition (Ke of 12-20), and keyhole (Ke > 20) mode melting during LPBF of AlSi$_{10}$. A melt pool aspect ratio (ratio of melt pool depth to width) of ~0.4 is observed to be the threshold between conduction and transition/keyhole mode melt pools for AlSi$_{10}$, different from the conventionally assumed 0.5. Lastly, inferred laser absorptivity values (from experimental melt pools) of transition/keyhole mode melt pools are observed to be >40% higher when compared to conduction mode melt pools. This work demonstrates a dimensionless-process map method to obtain near fully dense parts that can be generalized for LPBF of high reflectivity alloys.

Cytogenetic and molecular abnormalities are known to influence post-transplant outcomes in acute myeloid leukemia (AML) but data assessing the prognostic value of combined genetic models in the HCT setting are limited. We developed an adapted European LeukemiaNet (aELN) risk classification based on available genetic data reported to the Center for International Blood and Marrow Transplant Research, to predict post-transplant outcomes in 2289 adult AML patients transplanted in first remission, between 2013 and 2017. Abstract

As organisms are faced with intense rapidly changing selective pressures, new genetic material is required to facilitate adaptation. Among sources of genetic novelty, gene duplications and transposable elements (TEs) offer new genes or new regulatory patterns that can facilitate evolutionary change. With advances in genome sequencing it is possible to gain a broader view of how gene family proliferation and TE content evolve in non-model species when populations become threatened. Freshwater bivalves (Unionidae) currently face severe anthropogenic challenges. Over 70% of species in the United States are threatened, endangered or extinct due to pollution, damming of waterways, and overfishing. We have created a reference genome for M. nervosa to determine how genome content has evolved in the face of these widespread environmental challenges. We observe a burst of recent transposable element proliferation causing a 382 Mb expansion in genome content. Gene family expansion is common, with a duplication rate of 1.16 x 10^-8 per gene per generation. Cytochrome P450, ABC transporters, Hsp70 genes, von Willebrand proteins, chitin metabolism genes, mitochondria eating proteins, and opsin gene families have experienced significantly greater amplification and show signatures of selection. We use evolutionary theory to assess the relative contribution of SNPs and duplications in evolutionary change. Estimates suggest that gene family evolution may offer an exceptional substrate of genetic variation in M. nervosa, with Psgv=0.185 compared with Psgv=0.067 for single nucleotide changes. Abstract

Community respiratory viral infections (CRVIs) are associated with pulmonary function impairment, alloimmune lung syndromes and inferior survival in human leucocyte antigen (HLA)-matched allogeneic haematopoietic stem cell transplant (HCT) recipients. Although the incidence of viral infections in HLA-haploidentical HCT recipients who receive post-transplant cyclophosphamide (PTCy)-based graft-versus-host disease (GVHD) prophylaxis is reportedly increased, there are insufficient data describing the incidence of CRVIs and the impact of donor source and PTCy on transplant outcomes. Analysing patients receiving their first HCT between 2012 and 2017 for acute myeloid leukaemia, acute lymphoblastic leukaemia and myelodysplastic syndromes, we describe comparative outcomes between matched sibling transplants receiving either calcineurin-based GVHD prophylaxis (SibCNI, N = 1605) or PTCy (SibCy, N = 403), and related haploidentical transplants receiving PTCy (HaploCy, N = 757). The incidence of CRVIs was higher for patients receiving PTCy, regardless of donor type. Patients in the HaploCy cohort who developed a CRVI by day +180 had both a higher risk of treatment-related mortality [hazard ratio (HR) 2⋅14, 99% confidence interval (CI) 1⋅13-4⋅07; P = 0⋅002] and inferior 2-year overall survival (HR 1⋅65, 99% CI 1⋅11-2⋅43; P = 0⋅001) compared to SibCNI with no CRVI. This finding justifies further research into long-term antiviral immune recovery, as well as development of preventive and treatment strategies to improve long-term outcomes in such patients.

Abstract

Hermaphroditic (perfect) flowers were a key trait in grapevine domestication, enabling a drastic increase in yields due to the efficiency of self-pollination in the domesticated grapevine (Vitis vinifera L. ssp. vinifera). In contrast, all extant wild Vitis species are dioecious, each plant having only male or female flowers. In this study, we identified the male (M) and female (f) haplotypes of the sex-determining region (SDR) in the wild grapevine species V. cinerea and confirmed the boundaries of the SDR. We also demonstrated that the SDR and its boundaries are precisely conserved across the Vitis genus using shotgun resequencing data of 556 wild and domesticated accessions from North America, East Asia, and Europe. A high linkage disequilibrium was found at the SDR in all wild grape species, while different recombination signatures were observed along the hermaphrodite (H) haplotype of 363 cultivated accessions, revealing two distinct H haplotypes, named H1 and H2. To further examine the H2 haplotype, we sequenced the genome of two grapevine cultivars, 'Riesling' and 'Chardonnay'. By reconstructing the first two H2 haplotypes, we estimated the divergence time between H1 and H2 haplotypes at ∼6 million years ago, which predates the domestication of grapevine (∼8,000 y ago). Our findings emphasize the important role of recombination suppression in maintaining dioecy in wild grape species and lend additional support to the hypothesis that at least two independent recombination events led to the reversion to hermaphroditism in grapevine.
