* Context.--Neoadjuvant systemic therapy refers to the use of systemic agent(s) for malignancy prior to surgical treatment and has recently emerged as an option for most breast cancer patients eligible for adjuvant systemic therapy. Consequently, treated breast carcinomas have become routine specimens in pathology practices. A standard protocol has not yet been universally adopted for the evaluation and reporting of these specimens. The American Joint Committee on Cancer staging system recognizes the challenges in staging breast carcinomas after neoadjuvant treatment and provides important data points but does not currently provide detailed guidance in estimating the residual tumor burden in the breast and lymph nodes. The Residual Cancer Burden system is the only Web-based system that quantifies treatment response as a continuous variable using residual tumor burden in the breast and the lymph nodes. Objective.--To provide clarifications and guidance for evaluation and reporting of postneoadjuvant breast specimens, discuss issues with the current staging and reporting systems, and provide specific suggestions for future modifications to the American Joint Committee on Cancer system and the Residual Cancer Burden calculator. Data Sources.--English-language literature on the subject and the data from the I-SPy 2, a multicenter, adaptive randomization phase 2 neoadjuvant platform trial for early-stage, high-risk breast cancer patients. Conclusions.--This article highlights challenges in the pathologic evaluation and reporting of treated breast carcinomas and provides recommendations and clarifications for pathologists and clinicians. * Context.--The Current Procedural Terminology (CPT) system is a standardized numerical coding system for reporting medical procedures and services, and is the basis for reimbursement of health care providers by Medicare and other third-party payers. * Context.--Integrating anatomic pathology information--text and images--into electronic health care records is a key challenge for enhancing clinical information exchange between anatomic pathologists and clinicians. The aim of the Integrating the Healthcare Enterprise (IHE) international initiative is precisely to ensure interoperability of clinical information systems by using existing widespread industry standards such as Digital Imaging and Communication in Medicine (DICOM) and Health Level Seven (HL7). Objective.--To define standard-based informatics transactions to integrate anatomic pathology information to the Healthcare Enterprise Design.--We used the methodology of the IHE initiative. Working groups from IHE, HL7, and DICOM, with special interest in anatomic pathology, defined consensual technical solutions to provide end-users with improved access to consistent information across multiple information systems. Results.--The IHE anatomic pathology technical framework describes a first integration profile, "Anatomic Pathology Workflow," dedicated to the diagnostic process including basic image acquisition and reporting solutions. This integration profile relies on 10 transactions based on HL7 or DICOM standards. A common specimen model was defined to consistently identify and describe specimens in both HL7 and DICOM transactions. Conclusion.--The IHE anatomic pathology working group has defined standard-based informatics transactions to support the basic diagnostic workflow in anatomic pathology laboratories. * Context--The World Health Organization (WHO) recently published its 4th edition of the classification of tumors of the central nervous system, incorporating a substantial number of important changes to the previous version (WHO 2000). The new WHO classification introduces 7 changes in the grading of central nervous system neoplasms, ranging in significance from minor to major, in categories of anaplastic oligoastrocytomas, meningiomas, choroid plexus tumors, pineal parenchymal tumors, ganglioglioma, cerebellar liponeurocytoma, and hemangiopericytomas. The 4th edition also introduces 10 newly codified entities, variants, and patterns, as well as 1 new genetic syndrome. A number of established brain tumors are reorganized, including medulloblastomas and primitive neuroectodermal tumors, in an attempt to more closely align classification with current understanding of central nervous system neoplasia. Objective.--To summarize and discuss the most significant updates in the 4th edition for the practicing surgical pathologist, including (1) changes in grading among established entities; (2) newly codified tumor entities, variants, patterns, and syndromes; and (3) changes in the classification of existing brain tumors. Data Sources.--The primary source for this review is the WHO Classification of Tumours of the Central Nervous System, 4th edition. Other important sources include the 3rd edition of this book and the primary literature that supported changes in the 4th edition. Conclusions.--The new edition of the WHO blue book reflects advancements in the understanding of brain tumors in terms of classification, grading, and new entities. Context.--The correlation of the diagnosis made at intraoperative consultation (IC) with the final diagnosis is one of the cornerstones of quality assurance in the anatomic pathology laboratory. Objective.--To review correlation of IC diagnoses with final diagnoses during a 1-year period in a regionalized, multisite hospital setting in a major Canadian city. Design.--One pathologist reviewed all surgical pathology cases at Calgary Laboratory Services from June 2004 through May 2005 that had an IC to extract the following data points: intraoperative diagnoses, final diagnoses, correlation between the two, anatomic site of the tissue on which the IC was requested, pathologic procedure requested of the IC, types of disagreements encountered, reasons for disagreement, and deferrals. Results.--Intraoperative consultations occurred for 2812 specimens, of which 87 were discordant and 135 were deferred. Percent agreement was 96.75% (95% confidence interval, 96.08-97.42) with a [kappa] statistic of 0.94 (95% confidence interval, 0.92-0.95). Lymph nodes for evaluation for metastases (427), thyroid/parathyroid (401), and central/peripheral nervous system (378) specimens were sent most frequently for IC, and the latter 2 tissue types accounted for the greatest number of disagreements. The most common assessments requested were the presence/ typing of a neoplasm (1161) and assessment of margins (730), which also accounted for the largest number of disagreements. Disagreements were most frequently due to interpretive (53) and gross sampling (23) errors; false-negative disagreements were nearly 3 times as common as false positives. Conclusions.--The IC was an excellent diagnostic test.
