Abstract

ObjectiveExercise is a transdiagnostic clinical feature of eating disorders, but consensus is lacking as to what constitutes, and gives rise to, excessive exercise motivated by weight control. Using a longitudinal cohort study, we aimed to describe population-level prevalence rates of varying levels of weight-control exercise and examine gender and weight status (overweight or obesity; OVOB) as cross-sectional determinants of weight-control exercise in 14-15-year-old adolescents. We then evaluated the association of OVOB at 10-11 years with weight-control exercise at 14-15 years.MethodsThe sample comprised 6329 adolescents from the Longitudinal Study of Australian Children (LSAC). Weight and height were measured in early adolescence (aged 10-11) and in mid-adolescence (aged 14-15). Participants reported weight-control exercise using the Branched Eating Disorders Test at 14-15 years.ResultsIn mid-adolescence, the estimated population prevalence for any weight-control exercise was 49 % (55 % in females). For girls, moderate levels of exercise were most prevalent, and low levels for boys. For all levels except for the very lowest, boys with (vs. without) OVOB history (10-11 years) had about twice the odds of endorsing every level of weight-control exercise. Patterns among girls were similar, though lower in magnitude (∼1.5 times).ConclusionsFor both girls and boys, across most exercise levels, rates of weight-control exercise were greatest for those with OVOB; for the highest exercise level, effects were strongest for boys with OVOB. To accurately identify at-risk adolescents, our results provide preliminary support for a fluid definition of excessive weight-control exercise, dependent on gender and weight status.

Abstract

Longitudinal cohort studies, which follow a group of individuals over time, provide the opportunity to examine causal effects of complex exposures on long-term health outcomes. Utilizing data from multiple cohorts has the potential to add further benefit by improving precision of estimates through data pooling and by allowing examination of effect heterogeneity through replication of analyses across cohorts. However, the interpretation of findings can be complicated by biases that may be compounded when pooling data, or, contribute to discrepant findings when analyses are replicated. The "target trial" is a powerful tool for guiding causal inference in single-cohort studies. Here we extend this conceptual framework to address the specific challenges that can arise in the multi-cohort setting. By representing a clear definition of the target estimand, the target trial provides a central point of reference against which biases arising in each cohort and from data pooling can be systematically assessed. Consequently, analyses can be designed to reduce these biases and the resulting findings appropriately interpreted in light of potential remaining biases. Abstract

BackgroundThe stepped-care approach, where people with early symptoms of depression are stepped up from low-intensity interventions to higher-level interventions as needed, has the potential to assist many people with mild depressive symptoms. Self-monitoring techniques assist people to understand their mental health symptoms by increasing their emotional self-awareness (ESA) and can be easily distributed on mobile phones at low cost. Increasing ESA is an important first step in psychotherapy and has the potential to intervene before mild depressive symptoms progress to major depressive disorder. In this secondary analysis we examined a mobile phone self-monitoring tool used by young people experiencing mild or more depressive symptoms to investigate the relationships between self-monitoring, ESA, and depression. ObjectivesWe tested two main hypotheses: (1) people who monitored their mood, stress, and coping strategies would have increased ESA from pretest to 6-week follow-up compared with an attention comparison group, and (2) an increase in ESA would predict a decrease in depressive symptoms. MethodsWe recruited patients aged 14 to 24 years from rural and metropolitan general practices. Eligible participants were identified as having mild or more mental health concerns by their general practitioner. Participants were randomly assigned to either the intervention group (where mood, stress, and daily activities were monitored) or the attention comparison group (where only daily activities were monitored), and both groups self-monitored for 2 to 4 weeks. Randomization was carried out electronically via random seed generation, by an in-house computer programmer; therefore, general practitioners, participants, and researchers were blinded to group allocation at randomization. Participants completed pretest, posttest, and 6-week follow-up measures of the Depression Anxiety Stress Scale and the ESA Scale. We estimated a parallel process latent growth curve model (LGCM) using Mplus to test the indirect effect of the intervention on depressive symptoms via the mediator ESA, and calculated 95% bias-corrected bootstrapping confidence intervals (CIs). ResultsOf the 163 participants assessed for eligibility, 118 were randomly assigned and 114 were included in analyses (68 in the intervention group and 46 in the comparison group). A parallel process LGCM estimated the indirect effect of the intervention on depressive symptoms via ESA and was shown to be statistically significant based on the 95% bias-corrected bootstrapping CIs not containing zero (–6.366 to –0.029). The proportion of the maximum possible indirect effect estimated was κ2 =.54 (95% CI .426–.640). ConclusionsThis study supported the hypothesis that self-monitoring increases ESA, which in turn decreases depressive symptoms for young people with mild or more depressive symptoms. Mobile phone self-monitoring programs are ideally suited to first-step intervention programs for depression in the stepped-care approach, particularly when ESA is targeted as a mediating factor. 32. The Effects on Schooling Outcomes of Early Developmental Vulnerabilities in Children

Abstract

In Australia and many other countries, the provision of the healthiest possible start in life for all children has been a priority for government. Evidence to support the importance of the early years for future development has accumulated over the past two decades [1]. Patterns of childhood development predict health, wellbeing, learning and behaviour and lay a foundation for children to grow up with the skills to succeed, bringing benefits for them and the community as a whole [2]. The Australian Early Development Census (AEDC) is one major investment within the national agenda for early childhood development. It provides information to communities, governments and schools to support their planning and service provision. It also allows identification of groups of students who are developmentally vulnerable (DV) when entering the education system. This report presents findings from a linkage study involving the 2009 AEDC and CATS (Childhood to Adolescence Transition Study) datasets. CATS is an ongoing longitudinal study of health, wellbeing and educational achievement through late childhood and early adolescence. The sample comprises over 1,200 students and their parents drawn from Year 3 in 2012 from randomly selected schools across metropolitan Melbourne. The authors obtained information annually from students, parents and teachers on students' mental health and wellbeing, peer and family relationships, school engagement and the primary to secondary school transition. Guided by the conceptual framework described in our previous report, the current report uses a linked longitudinal dataset of student development and academic achievement spanning the entire primary school years (from the first year of primary school; 5-6 years of age through to Year 7; 12-13 years of age) to address the following questions: (1) What is the relationship between early developmental vulnerability on the AEDC and later learning, school engagement and wellbeing? (2) How might any relationship between early developmental vulnerability and learning at Year 7 be affected by wellbeing and school engagement across Years 3, 4 and 5? Abstract

Many epidemiological questions concern potential interventions to alter the pathways presumed to mediate an association. For example, we consider a study that investigates the benefit of interventions in young adulthood for ameliorating the poorer mid-life psychosocial outcomes of adolescent self-harmers relative to their healthy peers. Two methodological challenges arise. Firstly, mediation methods have hitherto mostly focused on the elusive task of discovering pathways, rather than on the evaluation of mediator interventions. Secondly, the complexity of such questions is invariably such that there are no existing data on well-defined interventions (i.e. actual treatments, programs, etc.) capturing the populations, outcomes and time-spans of interest. Instead, researchers must rely on exposure (non-intervention) data to address these questions, such as self-reported substance use and employment. We address the resulting challenges by specifying a target trial addressing three policy-relevant questions, regarding the impacts of hypothetical (rather than actual) interventions that would shift the mediators' distributions (separately, jointly or sequentially) to user-specified distributions that can be emulated with the observed data. We then define novel interventional effects that map to this trial, emulating shifts by setting mediators to random draws from those distributions. We show that estimation using a g-computation method is possible under an expanded set of causal assumptions relative to inference with well-defined interventions. These expanded assumptions reflect the lower level of evidence that is inevitable with ill-defined interventions. Application to the self-harm example using data from the Victorian Adolescent Health Cohort Study illustrates the value of our proposal for informing the design and evaluation of actual interventions in the future.

42. Student Wellbeing, Engagement and Learning across the Middle Years

Abstract

There is a growing consensus amongst policy makers, education professionals, researchers, and the public that a modern education system should develop a 'whole child' providing a balanced set of cognitive, social and emotional skills to face the challenges of an increasingly uncertain and volatile world. Social and emotional development are educational goals in and of themselves, as well as important influences on academic learning. There is currently a gap in knowledge about social and emotional development and academic performance, particularly in the middle years. The primary to secondary school transition is a time when children, particularly those with vulnerabilities, need strong social and educational support systems. Despite this, there are currently few evidence-based, system-wide strategies to support students in the transition. This report examines changes in student wellbeing and learning across the middle years, drawing on recent longitudinal data on Australian students from the Childhood to Adolescence Transition Study (CATS). The CATS sample comprises over 1200 students and their parents recruited in 2012 from schools in metropolitan Melbourne. The study has obtained information annually from students, parents and teachers around students' mental health and wellbeing, peer and family relationships, school engagement, and the primary to secondary school transition. This report analyses data collected between Year 3 (2012: 8-9 years of age) and Year 7 (2016; 12-13 years of age). Guided by a conceptual model of quality education, this report uses a number of indicators of student learning, wellbeing and school engagement to examine how these intersect in the middle years. The longitudinal dataset is used to address the following questions: (1) To what degree does student wellbeing in the middle years influence school engagement and learning? (2) To what degree does student learning in the middle years influence school engagement? (3) To what degree do peer relationships in the middle years influence school engagement and learning? The report also describes the experience of students and parents during the secondary school transition. It reports on readiness for secondary school prior to transition in Year 6 and observed adjustment to secondary school in Year 7, and investigates teachers' ability to identify students at risk of a poor transition. The report also examines the support received by students and parents from schools and how this relates to adjustment in secondary school. This report is guided by a framework of quality education in which student wellbeing, school engagement and learning are all important and inter-related. The results from this report confirm the reciprocal relationships between wellbeing, engagement and learning. These findings suggest that by working on all three aspects simultaneously, schools and education systems will achieve synergies in their efforts to provide quality education. In demonstrating the relationship between wellbeing and school engagement and learning outcomes, this report provides strong motivation for education systems, schools and teachers to promote the social and emotional development of students to help each and every student reach their full learning potential. Abstract

ObjectiveDespite known associations between eating disorders and obesity, little is known about the current prevalence of symptoms of eating disorders across the weight spectrum. This study therefore aimed to estimate the population prevalence of eating disorder symptoms in relation to weight status in adolescents.MethodThe sample comprised 3,270 participants (14-15 years; 52% boys) drawn from Wave 6 of the Longitudinal Study of Australian Children. Symptoms of anorexia nervosa (AN) and bulimia nervosa (BN) were assessed using self-report on the Branched Eating Disorder Test. This measure identifies clinically significant symptoms in the past 3 months according to the Diagnostic and Statistical Manual of Mental Disorders, fifth edition. Using study-derived cross-sectional population weights, the prevalence of each symptom was estimated for the total population and by sex and weight status.ResultsThe estimated population prevalence was high (14.3-25.7%) for body image symptoms such as fear of weight gain and overvaluation of body weight but lower (0.5-3.7%) for behavioral symptoms such as binge eating and compensatory behaviors. Symptoms were more prevalent among adolescents with overweight or obesity. Although most symptoms tended to have higher prevalence among girls than boys, boys with obesity had higher prevalence of binge eating and excessive exercise than girls with obesity. The overall estimated population prevalence for AN and BN was 0.20% and 0.10%, respectively.DiscussionThe study highlights a need for clinicians to be cognizant of disordered eating behaviors regardless of weight status and has implications for both eating disorder and obesity prevention and intervention.
