We developed a clip-on light tracker (MyLyt) for estimating light exposure in real time. This study aimed at validating and investigating the feasibility of using MyLyt in children and adults.The study was conducted in two phases. Phase 1 involved validation against a factory-calibrated digital lux meter in three separate conditions: controlled environmental set-up, outdoors and indoors where intra-test (two measurements by the same tracker), inter-test (measurements among trackers) and inter-device (MyLyt tracker and lux meter) validations were conducted. Phase 2 involved a feasibility study where MyLyt was used in a real-world setting by 21 adults and 8 children. Participants were asked to log their real-time movements in an 'activity diary', which were correlated with the lux levels measured by the tracker.A strong positive correlation and non-significant difference in the recorded mean illuminance levels were observed during intra-test (inter-class correlation: 1.00, p = 0.99), inter-test (0.91-1.00, p 0.15) and inter-device (0.91-1.00, p 0.56) validation in all three testing conditions (p 0.49), except the indoor location. While the lux level measured by MyLyt was significantly higher than that of the lux meter (p 0.01) in the indoor locations, differences were minimal and clinically insignificant. A Bland-Altman plot showed a minimal mean difference (95% limits of agreement) between the MyLyt tracker and lux meter in all three conditions (controlled environmental set-up: 641 [-949, 2230], outdoor: 74 [-2772, 2920] and indoor: -35 [-151, 80] lux). Phase 2 validation showed an expected illuminance level against the corresponding location with high sensitivity (97.8%) and specificity (99%) to accurately differentiate between outdoor and indoor locations.The MyLyt tracker showed good repeatability, strong correlation and comparable values with the lux meter in the three tested conditions, making it suitable for tracking light exposure patterns for both research and clinical purposes.

Considering that a certain proportion of high myopes have reduced visual acuity even after full optical correction, this study aimed to investigate the association between various refractive error components (sphere, cylinder and axis orientation) and reduced visual acuity in individuals with low to high myopia with and without pathologic myopia lesions.We analysed data from randomly selected eyes of 11,258 individuals with myopia (mean ± SD spherical equivalent (SE) -3.2 ± 2.9D; range: -0.5D to -21.5D). In total, 10,528 individuals had no pathologic myopia lesions. Sphere, cylinder and SE refraction were classified into mild, moderate and high categories. Astigmatism was defined as with-the-rule, against-the-rule or oblique based on the axis orientation. Reduced best-corrected visual acuity was defined as ≥0.18 logMAR. Logistic regression was performed to test factors associated with reduced visual acuity with and without pathologic myopia lesions.Overall, 6.4% (N = 720/11,258) of myopes had reduced best-corrected visual acuity. High sphere (≤-6.0D; Odd ratios [OR]: 16.1; 95% CI: 2.1-126.5), high cylinder (-2.0 DC; OR: 2.5; 95% CI: 1.8-3.4), against-the-rule (OR: 1.5; 95% CI: 1.1-2.0) and oblique astigmatism (OR: 1.6; 95% CI: 1.2-2.1) were significantly (p ≤ 0.008) associated with reduced visual acuity in the absence of pathologic myopia lesions. Both moderate SE and high myopic SE were also associated with reduced visual acuity. In the presence of pathologic myopia lesions, tessellated fundus (OR: 6.9; 95% CI: 3.5-14.1), chorioretinal atrophy (OR: 7.7; 95% CI: 2.6-19.9) and choroidal neovascularisation (OR: 37.4; 95% CI: 3.3-419.3) were significantly (p ≤ 0.003) associated with reduced visual acuity.Even after full optical correction, both refractive components and pathologic myopia lesions can independently cause reduced visual acuity, regardless of the degree of myopia.

Signals from the peripheral retina are important for myopia development. Unlike temporal vision, deficits in peripheral spatial visual functions of myopes have been investigated previously. This study investigated temporal contrast thresholds in emmetropes and myopes at different retinal eccentricities.Forty-four young adults (mean age 23 ± 3 years) including 21 emmetropes (Spherical Equivalent (SE): +0.01 ± 0.30D) and 23 myopes (SE: -3.98 ± 2.41D) participated in this prospective study. Flicker modulation thresholds (FMT) were determined monocularly (right eye) for 15 Hz flicker stimulus at 0°, nasal (23°, 10°) and temporal (-23°, -10°) retinal eccentricities along the horizontal meridian. FMTs were measured psychophysically using 5-adaptive interleaved staircases and threshold was taken as the average of the last 6 reversals.In both the groups (emmetropes and myopes), there was a naso-temporal asymmetry in FMTs with higher thresholds in the far temporal retina (Median; Interquartile range: 40.97%; 17.06) than the nasal retina (28.07%; 9.36) (p0.001). Flicker modulation thresholds were significantly higher in myopes (30.58%; 12.15) compared to emmetropes (26.77%; 7.74; p = 0.04) at far nasal retina (23°), while at other eccentricities there was no effect (p0.05). Further sub-analysis revealed only high myopes (34.48 %, 21.9) showed significantly higher FMT compared to emmetropes (26.77%; 7.74; p = 0.04).Greater FMTs were seen in high myopes than that of emmetropes in the nasal retina. Further studies exploring the structural aspects of the myopic eye with FMT would provide a better understanding of role of flicker sensitivity in myopiogenesis.

The stretching of a myopic eye is associated with several structural and functional changes in the retina and posterior segment of the eye. Recent research highlights the role of retinal signaling in ocular growth. Evidence from studies conducted on animal models and humans suggests that visual mechanisms regulating refractive development are primarily localized at the retina and that the visual signals from the retinal periphery are also critical for visually guided eye growth. Therefore, it is important to study the structural and functional changes in the retina in relation to refractive errors. This review will specifically focus on electroretinogram (ERG) changes in myopia and their implications in understanding the nature of retinal functioning in myopic eyes. Based on the available literature, we will discuss the fundamentals of retinal neurophysiology in the regulation of vision-dependent ocular growth, findings from various studies that investigated global and localized retinal functions in myopia using various types of ERGs.

Contains fulltext : 292367.pdf (Publisher’s version ) (Open Access) PURPOSE: The purpose of this study was to evaluate the epidemiology, etiology, clinical assessment, investigation, management, and visual consequences of high myopia (≤-6 diopters [D]) in infants and young children. FINDINGS: High myopia is rare in pre-school children with a prevalence less than 1%. The etiology of myopia in such children is different than in older children, with a high rate of secondary myopia associated with prematurity or genetic causes. The priority following the diagnosis of high myopia in childhood is to determine whether there is an associated medical diagnosis that may be of greater overall importance to the health of the child through a clinical evaluation that targets the commonest features associated with syndromic forms of myopia. Biometric evaluation (including axial length and corneal curvature) is important to distinguishing axial myopia from refractive myopia associated with abnormal development of the anterior segment. Additional investigation includes ocular imaging, electrophysiological tests, genetic testing, and involvement of pediatricians and clinical geneticists is often warranted. Following investigation, optical correction is essential, but this may be more challenging and complex than in older children. Application of myopia control interventions in this group of children requires a case-by-case approach due to the lack of evidence of efficacy and clinical heterogeneity of high myopia in young children. CONCLUSIONS: High myopia in infants and young children is a rare condition with a different pattern of etiology to that seen in older children. The clinical management of such children, in terms of investigation, optical correction, and use of myopia control treatments, is a complex and often multidisciplinary process.

PURPOSE We aimed to test the accommodative lag and mechanical tension theories for myopia by assessing the influence of the lag of accommodation on axial elongation by using three different near targets that are known to influence the accommodative response differently. METHODS Forty-two young adults were recruited for the study. Axial length was measured using a non-contact biometer, before and immediately after a 15 minute visual task, with one of the three near targets placed 20 cm from the eye: reading text from a paper, reading text from a smartphone and watching a video on a smartphone. The accommodative response was determined using an open-field autorefractor while the participants viewed the near target monocularly. RESULTS Lag of accommodation was significantly different for the three tasks: watching a video (mean ± standard error of the mean [SEM] 0.92 ± 0.10 D); reading text on the smartphone (0.59 ± 0.08 D); and reading text on paper (0.24 ± 0.09 D). The peripheral refraction profile in myopes with different corrective modalities varies significantly for both distance and near viewing and will have implications in managing myopia.This study investigated how the magnitude of peripheral myopic defocus induced by Ortho-K varies with and without accommodation, and how this compares to single vision spectacles and soft-contact-lenses (SCL).Relative peripheral refraction (RPR) of 18 young adults (spherical equivalent -1.00 D to -4.50 D) was determined along the horizontal meridian (±10°, ±20°, ±25°) during distance (3-metres) and near viewing (0.2-metres), and along vertical meridian (±10°, ±15°) for distance viewing alone. Measurements were obtained in an uncorrected state and with single vision spectacles, soft contact lens and Ortho-K. Changes in RPR and astigmatic components were compared between distance and near viewing with all different modalities.A significant interaction (p = 0.02) between relative peripheral refraction and the target distance (distance and near viewing) was found among different refractive modalities. Single overnight Ortho-K lens wear alone led to relative peripheral myopia for both distance (mean RPR ± SE: -0.92 ± 0.21D and -1.04 ± 0.22D) and near viewing (-0.71 ± 0.17D and -0.76 ± 0.20D). Comparisons of relative peripheral refraction between different corrective modalities at each eccentricity indicated statistical significance of RPR at extreme locations along both temporal and nasal meridian (±20 and ±25°, p 0.05). RPR with soft contact lenses and spectacles were similar for both distance and near viewing (p 0.05).Single overnight Ortho-K lens wear alone shifted the RPR in the myopic direction for both distance and near viewing in comparison with single vision spectacles and soft contact lenses. The Ortho-K lens designs that offer a large amount of mid-peripheral corneal steeping, in-turn leading to high relative peripheral myopia for both distance and near viewing and might offer beneficial effects on myopia control.

Myopia typically starts and progresses during childhood, but onset and progression can occur during adulthood. The goals of this review are to summarize published data on myopia onset and progression in young adults, aged 18 to 40 years, to characterize myopia in this age group, to assess what is currently known, and to highlight the gaps in the current understanding. Specifically, the peer-reviewed literature was reviewed to: characterize the timeline and age of stabilization of juvenile-onset myopia; estimate the frequency of adult-onset myopia; evaluate the rate of myopia progression in adults, regardless of age of onset, both during the college years and later; describe the rate of axial elongation in myopic adults; identify risk factors for adult onset and progression; report myopia progression and axial elongation in adults who have undergone refractive surgery; and discuss myopia management and research study design. Adult-onset myopia is common, representing a third or more of all myopia in western populations, but less in East Asia, where onset during childhood is high. Clinically meaningful myopia progression continues in early adulthood and may average 1.00 diopters (D) between 20 and 30 years. Higher levels of myopia are associated with greater absolute risk of myopia-related ocular disease and visual impairment, and thus myopia in this age group requires ongoing management. Modalities established for myopia control in children would be options for adults, but it is difficult to predict their efficacy. The feasibility of studies of myopia control in adults is limited by the long duration required.

Purpose This study aimed to predict myopia prevalence in urban Indian children and to describe the generational effect of myopia in different age groups over the next three decades from the year 2020. Methods A systematic review of myopia prevalence in India was performed using the Preferred Reporting Items for the Systematic Reviews and Meta-analysis (PRISMA) guidelines and included eight studies with 28 600 participants, which were published in the period 1 January 1999 to 31 December 2019. The best fit for the prediction model was assessed with the baseline prevalence data plotted against different years and fitted with multiple mathematical regressions (linear, second-order polynomial, third order polynomial and exponential). Based on the quality of the fit assessed by the coefficient of determination (R2 ) values, the sum of squared residuals and statistical significance, final predictions for myopia prevalence in the 5 to 15-year-old urban Indian children was estimated using the aptly suited linear regression model. To describe the generational effect on myopia prevalence over the next three decades, the prevalence of myopia in both children and adults, based on the available literature (1999 to 2020) was plotted against age, as the baseline. Results The prevalence of myopia in 5 to 15-year-old urban children increased from 4.44% in 1999 to 21.15% in 2019. Our predictions, based on the slope of 0.8% every year (4.05% for every 5 years) indicate that the prevalence of myopia will increase to 31.89% in 2030, 40.01% in 2040 and 48.14% in 2050. Due to the generational effect (caused by the nature of the condition lasting a lifetime once developed), there will be an overall increase in myopia prevalence across all age groups of 10.53% in the next three decades (2020 to 2050). Conclusion The estimates of myopia prevalence across all age groups indicate the possible future epidemic of myopia in India within a few decades, similar to the situation in East Asian countries, unless active intervention to prevent myopia and changes in lifestyle are instigated to counteract myopia in India. Meticulously designed eye care services with focussed anti-myopia strategies are needed to control the rising myopia prevalence in India.

PurposeAlbinism is known to disrupt emmetropisation in animal models. However, it is not clear if the same effect is seen in humans. This study aimed to investigate the refractive profile in individual diagnosed with ocular albinism (OA) and oculocutaneous albinism (OCA) based on a large dataset.MethodsRequired data from 618 individuals (61% males and 39% females) diagnosed with albinism were exported from the eyeSmart electronic medical records of L V Prasad Eye Institute. Overall, there were 112 (18%) individuals diagnosed with OA and 506 (82%) with OCA. Based on the spherical equivalent refraction (SER), individuals were classified as emmetropes, myopes, and hyperopes.ResultsThe overall spherical equivalent refraction of the individuals ranged from -25.00D to +12.00D with median 0.25 (-2.00 to 2.25). The proportion of individuals with albinism (combined OA, OCA) having hyperopia and myopia (overall: N=282;46% vs. N=245;40%) were similar (p = 0.18), and the least were with emmetropia (overall: N=91;14%). Across all the age groups (0-10, 11-20, 21-30, > 30 years) the frequency of hyperopes and myopes were significantly higher (pConclusion The presence of both high hyperopia and high myopia and very less numbers with emmetropia across all age groups indicates disrupted normal refractive development in individuals with albinism. With-the-rule astigmatism and nystagmus may result in meridional degradation of retinal image leading to impairment of normal emmetropisation process in individuals with albinism.

PURPOSE This study investigated the thickness, area, and insertion site of the medial (MR) and lateral (LR) rectus muscles in individuals with emmetropia and different degrees of myopia. METHODS Swept-source optical coherence tomography images of the MR and LR muscles in 80 participants including emmetropes (spherical equivalent refractive error [SER] ±0.50 D, N = 14) and myopes (≤ -0.75 D, N = 66), were analysed. Custom-designed, semi-automated software was used to measure parameters such as insertion distance from limbus, muscle thickness at every 1 mm interval to 3 mm periphery and muscle area from insertion site to 3 mm. Purpose Considering that ocular expansion is associated with scleral thinning, this study investigated variation in scleral thickness (anterior scleral thickness [AST] and posterior scleral thickness [PST]) in different meridians across emmetropes and a wide range of myopes. Methods A total of 95 participants (mean age, 24 ± 4 years) including emmetropes (spherical equivalent refractive error, ±0.75 diopters [D]; n = 20) and myopes (–1.00 to –27.25 D; n = 75) underwent ocular imaging with swept-source optical coherence tomography. All the images were analyzed using semiautomated custom-designed software to determine scleral thickness in 1-mm intervals. AST was estimated from limbus to 5 mm (n = 95), and PST from fovea to 5 mm (n = 25; high myopes only) along the horizontal and vertical meridian. Results The median spherical equivalent refractive error and axial length were −4.25 D (IQR, –12.50 to –1.00 D) and 25 mm (IQR, 23.72–28.35 mm), respectively. The anterior sclera was thinnest in the superior and thickest in the inferior region (475.3 ± 19.0 vs. 605.9 ± 18.6 µm; P < 0.001). The inferior AST alone decreased significantly with increasing magnitude of myopia (r = 0.27; P = 0.008). There were no differences in AST between nasal and temporal meridians (583.24 ± 15.00 vs. 587.09 ± 27.00 µm; P > 0.05). The mean subfoveal PST for the subset of high myopes was 251.7 ± 12.0 µm which was thinner than mean AST along all the meridians by more than 45%. The averaged scleral thickness peripheral to fovea (1–5 mm) was similar along different meridians (P > 0.05). Conclusions The relative significant thinning of the anterior sclera along the inferior meridian with increasing degree of myopia compared with the other three meridians indicates the potential role of AST, especially in the inferior meridian, to act as a marker for myopia progression.

To examine the progression pattern of disc and retinal lesions in highly myopic Chinese adolescents over a 10-year period in Singapore.This longitudinal study included Chinese participants who showed high myopia (spherical equivalent [SE] worse than or equal to -5 diopters [D]), no history of refractive surgery, and available fundus photographs at both 2006 (baseline) and 2016 (10-year follow-up) visits. Forty-four adolescents (aged 12-16 years at baseline) who were re-examined later at follow-up were included. Cycloplegic refraction, biometry, and fundus photography were performed at both visits. A trained grader classified myopic macular degeneration (MMD) based on the Meta-pathologic myopia classification and disc lesions from fundus photographs. Choroidal thickness (CT) measurements were performed at 10-year follow-up using swept-source optical coherence tomography. The ocular parameters and lesions were compared between baseline and follow-up.There was a significant worsening of high myopia at follow-up to -7.5±1.8 D (mean SE±SD) in 2016 versus -6.2±1.3 D in 2006; (P0.001). The 10-year changes included increased degree of tessellation (26 eyes, 29.5%), development of new tessellated fundus (19 eyes, 21.6%), disc tilt (7 eyes, 8.0%), and expansion of peripapillary atrophy size (33 eyes, 37.5%). Eyes with early-onset tessellation (present at baseline, 48 eyes) showed significantly thinner CT (P0.05), compared with eyes with late-onset tessellation (incident at 10-year follow-up, 19 eyes). No cases of MMD were recorded at baseline or 10-year follow-up.Although there was no incident MMD, the retinal and disc lesions worsened over the follow-up period. Early-onset fundus tessellation was associated with thinner CT.

Our current understanding of emmetropisation and myopia development has evolved from decades of work in various animal models, including chicks, non-human primates, tree shrews, guinea pigs, and mice. Extensive research on optical, biochemical, and environmental mechanisms contributing to refractive error development in animal models has provided insights into eye growth in humans. Importantly, animal models have taught us that eye growth is locally controlled within the eye, and can be influenced by the visual environment. This review will focus on information gained from animal studies regarding the role of optical mechanisms in guiding eye growth, and how these investigations have inspired studies in humans. We will first discuss how researchers came to understand that emmetropisation is guided by visual feedback, and how this can be manipulated by form-deprivation and lens-induced defocus to induce refractive errors in animal models. We will then discuss various aspects of accommodation that have been implicated in refractive error development, including accommodative microfluctuations and accommodative lag. Next, the impact of higher order aberrations and peripheral defocus will be discussed. Lastly, recent evidence suggesting that the spectral and temporal properties of light influence eye growth, and how this might be leveraged to treat myopia in children, will be presented. Taken together, these findings from animal models have significantly advanced our knowledge about the optical mechanisms contributing to eye growth in humans, and will continue to contribute to the development of novel and effective treatment options for slowing myopia progression in children.

Purpose Considering that time spent outdoors is protective for myopia, we investigated how ambient light levels reaching the eye varies across 9 outdoor and 4 indoor locations in 5 different environmental conditions. Methods Illuminance (lux) was recorded using a lux meter under conditions of weather (sunny/cloudy), time of a day (7:00,10:00,13:00, and 16:00 hours), seasons (summer/winter), and sun protection (hat and cap) in outdoor and indoor locations. Nine outdoor locations were “open playground”, “under a translucent artificial-shade”, “under a porch facing east”, “under a porch facing south”, “under a big tree”, “between three buildings”, “within 4 buildings”, and “canopy”. As a ninth outdoor location, “Under a glass bowl” in the outdoor location was used as a simulation for “glass classroom model” and measurement was taken at the floor level only to determine in overall the illuminance conditions with glass covered on all sides. The 4 indoor locations included “room with multiple large windows”, “room with combination light source”, “room with multiple artificial lights”, and “room with single artificial light”. Results The overall median illuminance level (median; Q1-Q3) recorded in 9 outdoor locations was 8 times higher than that of all indoor locations (1175;197–5400 lux vs. 179;50–333 lux). Highest illuminance in outdoor locations was recorded in “open playground” (9300;4100–16825 lux), followed by “under a translucent artificial shade (8180;4200–13300 lux) and the lowest in “within 4 buildings” (11;6–20 lux). Illuminance under ‘Canopy’, ‘between three buildings’ and ‘within four buildings’ was similar to that of indoor locations (1000 to Conclusions Illuminance levels in outdoors and indoors varied with location type, but not with other conditions. Given the variation in illuminance in different locations, and the impact it may have on myopia control, appropriate detailed recommendations seems necessary while suggesting time outdoors as an anti-myopia strategy to ensure desired outcomes.

Purpose: To investigate how peripheral refraction and peripheral eye length are related to retinal shape. Methods: Relative peripheral refraction RPR and relative peripheral eye length RPEL were determined in 36 young adults (M +0.75D to −5.25D) along horizontal and vertical visual field meridians to ±35° and ±30°, respectively. Retinal shape was determined in terms of vertex radius of curvature Rv, asphericity Q and equivalent radius of curvature REq using a partial coherence interferometry method involving peripheral eye lengths and model eye raytracing. Second order polynomial fits were applied to RPR and RPEL as functions of visual field position. Linear regressions were determined for the fits’ second order co-efficients and for retinal shape estimates as functions of central spherical refraction. Linear regressions investigate relationships of RPR and RPEL with retinal shape estimates. Results: Peripheral refraction, peripheral eye lengths and retinal shapes were significantly affected by meridian and refraction. More positive (hyperopic) relative peripheral refraction, more negative RPELs and steeper retinas were found along the horizontal than along the vertical meridian and in myopes than in emmetropes. RPR and RPEL, as represented by their second order fit coefficients, correlated significantly with retinal shape represented by REq. Conclusion: Effects of meridian and refraction on RPR and PEL patterns are consistent with effects on retinal shape. Patterns derived from one of these predict the others: more positive (hyperopic) RPR predicts more negative RPEL and steeper retinas, more negative RPEL predicts more positive relative peripheral refraction and steeper retinas, and steeper retinas derived from peripheral eye lengths predict more positive RPR.

Purpose: To study the relationship between the severity of myopia and the severity of diabetic retinopathy (DR) in individuals with type 1 or type 2 diabetes mellitus (DM). Methods: This retrospective study was conducted using data from electronic medical records from a multicentric eyecare network located in various geographic regions of India. Individuals with type 1 or type 2 DM were classified according to their refractive status. Severe nonproliferative DR (NPDR), PDR, or presence of clinically significant macular edema (CSME) with any type of DR was considered as vision-threatening diabetic retinopathy (VTDR). Results: A total of 472 individuals with type-1 DM (mean age 41 ± 10 years) and 9341 individuals with type-2 DM (52 ± 9 years) were enrolled. Individuals with a hyperopic refractive error had a significant positive association with the diagnosis of VTDR (odds ratio (OR) 1.26; 95%CI 1.04–1.51, P = 0.01) and moderate nonproliferative DR (OR 1.27; 95%CI 1.02–1.59, P = 0.03) in type-2 DM; however, no significant association was found in type-1 DM. After adjusting for age, gender, anisometropia, and duration of diabetes, the presence of high myopia (< - 6 D) reduced the risk of VTDR in type 2 DM (OR 0.18; 95% CI 0.04–0.77, P = 0.02), but no association was found in type 1 DM. Mild and moderate myopia had no significant association with any forms of DR in both type-1 and type-2 DM. Conclusion: Hyperopic refractive error was found to increase the risk of VTDR in persons with type 2 DM. High-myopic refractive error is protective for VTDR in type 2 DM, but not in type-1 DM.

Purpose: The optometry profession has experienced massive changes amid lockdown in COVID 19 pandemic. This study gauges the impact of COVID-19 on optometry education and practices in India. Methods: The impact of COVID-19 among key stakeholders of Indian optometry that included educators, students, and practitioners was surveyed. The content validity of the survey tool was achieved through a focused group discussion with experts. Results: Of the 1408 responses, 118 were educators, 845 were students and 445 were practitioners. Post COVID-19 lock-down, a high percentage of students (96%) and educators (94%) were now using online mode of education to learn and teach. The blended learning methods were reported to be used by 81% of educators. Practical skills were the most difficult to teach by educators and to learn by students. Almost a third of the students were concerned about their internships and job opportunities amid the pandemic. Practitioners felt confident in performing routine eye examinations with personal protective equipment (PPE). Telemedicine was found to be adopted by 55% of the private practitioners and 49% of the eye hospitals. Conclusion: The COVID-19 pandemic has provided an opportunity to reform Indian optometry education through blended learning methods. Optometry practices changed and adapted instantly to the new hygiene norms that have raised the standard of care provided to patients. Telemedicine emerged as a mode of providing care by optometrists. Overall, all key stakeholders of optometry in India were found to have adapted well to the sudden changes due to COVID 19 pandemic.

Objective To investigate annual myopia progression in individuals from South Indian states across different age groups, and its association with age of onset and severity of myopia. Methods This retrospective study included the data of 6984 myopes (range: 1–30 years), who visited at least twice to LV Prasad Eye Institute and on whom a standard retinoscopy technique was performed to determine refractive error. Based on spherical equivalent (SE) refractive error, individuals were classified into mild, moderate, high and severe myopic groups. Myopia progression was calculated as difference between SE at 1-year follow-up visit and at baseline. To determine the age-specific myopia progression, individuals were further categorized as myopes who are at least 15 years or younger and those who are above 15. Results The mean annual progression of myopia was influenced by both the age group (p < 0.001) and severity type of myopia (p < 0.001). The overall mean myopia progression ranged from -0.07 ± 0.02 D (standard error) to -0.51 ± 0.02 D across different age groups with maximum change in refractive error noted in children aged 6–10 years and the least in adults aged 26–30 years. Myopia progression was greater in severe myopes, followed by high, moderate, mild myopes and in individuals aged ≤ 15 years compared to those aged >15 years (-0.45 ± 0.01 vs. 0.14 ± 0.01, p < 0.001). Severe myopes alone had similar annual myopia progression rate irrespective of age (i.e ≤15 and >15 years, p = 0.71). Early onset of myopia was associated with high myopia in adulthood. Conclusion The magnitude of myopia progression in children from South Indian states is comparable to that of Caucasians and Chinese. The greater progression in ‘severe myopes’ across different age groups emphasize the need for regular follow-ups, monitoring axial lengths, and anti-myopia strategies to control myopia progression irrespective of the age and degree of myopia.

This is a retrospective study aimed to investigate the patterns of myopic fundus complications in Indian children and young adults. Electronic medical records of 29,592 patients, aged 10–40 years, who visited L V Prasad Eye Institute between 1st January to 31st December 2016 were analysed in the study. Data such as age, gender, refractive error and various pathologic lesions of posterior globe were considered for analysis. Among all the patients with different types of refractive errors, myopia was found in 47.4%, high myopia in 6.8% and pathologic myopia in 2.2%. There was no trend of the increased prevalence of pathologic myopia with increasing age, except for a significant difference between the children aged 10–15 years (2.7%) and those aged more than 15 years (>4%). . Although, the overall pattern of pathologic lesions was similar across different grades of myopia (2.5% in low myopes vs. 2.2% in severe myopes), lesions like staphyloma and retinal detachment increased with increasing degree of myopia. The proportion of pathologic lesions across different grades of myopia suggests the necessity for careful peripheral fundus examinations irrespective of the degree of myopia for better management and prognostic purposes.

Myopia prevalence rates differ between racial groups. If the growth of the eye is sensitive to differences in optical input, the difference in spherical aberration between East Asian and Caucasian eyes found in this study may be important in understanding myopia development.The aim of this study was to determine differences in peripheral wavefront aberrations between two racial groups.Wavefront aberrations were measured using a COAS-HD aberrometer across the 42 × 32° central visual field on 37 right eyes of young adults (18 Caucasians, 19 East Asians; mean age 21.5 ± 2.4 years). The mean spherical equivalent refraction was -1.94 ± 1.63 diopters (D) with a range of -5.87 to +0.16 D. Effect of race and visual field position on refractions, individual Zernike aberration coefficients up to the fourth order, higher-order root-mean-square aberration, and total root-mean-square aberration were assessed by repeated-measures analysis of covariance.Caucasians and East Asians had similar relative peripheral myopia across the visual field. All higher-order aberration coefficients were affected by visual field position. Race had no significant effect on any higher-order Zernike coefficient, but the difference in mean vertical coma coefficient (Equation is included in full-text article.)across the visual field (i.e., average of 38 field locations) approached significance, being less positive in Caucasians than in East Asians (P = .08). When correction was made for the Caucasian group being slightly less myopic than the East Asian group, spherical aberration coefficient (Equation is included in full-text article.)was less positive in Caucasians than in East Asians by 0.04 μm (P = .001). The rates of change of coma coefficients across the field were not affected by race.Caucasians and East Asians had similar relative peripheral myopia, but with less positive spherical aberration coefficient in Caucasians than in East Asians. It remains to be determined whether aberrations have a role in the difference of myopia prevalence rates in different countries.

Purpose Excessive axial elongation of the eye in high myopia can cause biomechanical stretching leading to various ocular complications. The purpose of this review is to provide an update on various pathologic changes, especially in the chorio-retina and sclera that have been reported recently using advanced ophthalmic bio-imaging modalities such as optical coherence tomography, magnetic resonance imaging and fundus photography. Recent findings The prevalence rates of pathologic myopia (myopic retinopathy and maculopathy) mirror the prevalence rates of high myopia as the risks of pathologic myopia increase with high myopia. Peripapillary and sub-foveal choroidal thinning, scleral thinning, and deformed/irregular eye shapes were found to be strongly associated with various pathologic myopic lesions, especially with posterior staphyloma and chorio-retinal atrophy. Considering the increasing prevalence rate of myopia and pathologic myopia, these degenerative changes are likely to increase dramatically over the next few decades due to the rapid growth in the number of individuals with high myopia and the ageing population. Summary The current prevalence rates of pathologic myopia in older adults might have significantly underestimated the future prevalence rates and warrants age of onset of myopia being considered a major risk factor for pathologic myopia. Using advanced technology, identification of novel quantifiable chorio-retinal and scleral parameters in pathologic myopia would help to identify people at risk of developing pathologic myopia and potentially may revolutionise the management of myopia especially in risk prognostication and monitoring of disease progression.

Purpose To develop a fitness tracker (FitSight) to encourage children to increase time spent outdoors. To evaluate the wear pattern for this tracker and outdoor time pattern by estimating light illumination levels among children. Methods The development of the FitSight fitness tracker involved the designing of two components: (1) the smartwatch with custom-made FitSight watch application (app) to log the instant light illuminance levels the wearer is exposed to, and (2) a companion smartphone app that synchronizes the time outdoors recorded by the smartwatch to smartphone via Bluetooth communication. Smartwatch wear patterns and tracker-recorded daily light illuminance levels data were gathered over 7 days from 23 Singapore children (mean ± standard deviation age: 9.2 ± 1.4 years). Feedback about the tracker was obtained from 14 parents using a three-level rating scale: very poor/poor/good. Results Of the 14 parents, 93% rated the complete "FitSight fitness tracker" as good and 64% rated its wearability as good. While 61% of 23 children wore the watch on all study days (i.e., 0 nonwear days), 26% had 1 nonwear day, and 4.5% children each had 3, 4, and 5 nonwear days, respectively. On average, children spent approximately 1 hour in light levels greater than 1000 lux on weekdays and 1.3 hours on weekends (60 ± 46 vs. 79 ± 53 minutes, P = 0.19). Mean number of outdoor "spurts" (light illuminance levels >1000 lux) per day was 8 ± 3 spurts with spurt duration of 34 ± 32 minutes. Conclusion The FitSight tracker with its novel features may motivate children to increase time outdoors and play an important role in supplementing community outdoor programs to prevent myopia. Translational relevance If the developed noninvasive, wearable, smartwatch-based fitness tracker, FitSight, promotes daytime outdoor activity among children, it will be beneficial in addressing the epidemic of myopia.

Purpose The eye rotation approach for measuring peripheral eye length leads to concern about whether the rotation influences results, such as through pressure exerted by eyelids or extra-ocular muscles. This study investigated whether this approach is valid. Methods Peripheral eye lengths were measured with a Lenstar LS 900 biometer for eye rotation and no-eye rotation conditions (head rotation for horizontal meridian and instrument rotation for vertical meridian). Measurements were made for 23 healthy young adults along the horizontal visual field (±30°) and, for a subset of eight participants along the vertical visual field (±25°). To investigate the influence of the duration of eye rotation, for six participants measurements were made at 0, 60, 120, 180 and 210 s after eye rotation to ±30° along horizontal and vertical visual fields. Results Peripheral eye lengths were not significantly different for the conditions along the vertical meridian (F1,7 = 0.16, p = 0.71). The peripheral eye lengths for the conditions were significantly different along the horizontal meridian (F1,22 = 4.85, p = 0.04), although not at individual positions (p ≥ 0.10) and were not important. There were no apparent differences between the emmetropic and myopic groups. There was no significant change in eye length at any position after maintaining position for 210 s. Conclusion Eye rotation and no-eye rotation conditions were similar for measuring peripheral eye lengths along horizontal and vertical visual field meridians at ±30° and ±25°, respectively. Either condition can be used to estimate retinal shape from peripheral eye lengths.

We investigated changes in eye dimensions and retinal shape with degree of myopia, gender and race. There were 58 young adult emmetropes and myopes (range –1.25D to −8.25D), with 30 East-Asians (21 female/9 male), 23 Caucasians (16/7) and 5 South-Asians (1/4). Three-dimensional magnetic resonance imaging was undertaken with a 3.0 Tesla whole-body clinical MRI system using a 4.0 cm receive-only surface coil positioned over the eye. Automated methods determined eye length, width and height, and curve fitting procedures determined asymmetric and symmetric ellipsoid shapes to 75%, 55% and 35% of the retina. With myopia increase, eye dimensions increased in all directions such that increase in length was considerably greater than increases in width and height. Emmetropic retinas were oblate (steepening away from the vertex) but oblateness decreased with the increase in myopia, so that retinas were approximately spherical at 7 to 8D myopia. Asymmetry of eyes about the best fit visual axis was generally small, with small differences between the vertex radii of curvature and between asphericities in the axial and sagittal planes. Females had smaller eyes than males, with overall dimensions being about 0.5mm less for the former. Race appeared not to have a systematic effect.

We determined racial variations in peripheral aberrations across the visual field. Caucasians and East Asians had similar relative peripheral myopia but vertical coma and spherical aberration coefficients were more negative in Caucasians than in East Asians.

The epidemic of myopia in urban Asian cities has increased over recent generations and has become a significant public health concern. Considering the potential role of time outdoors in myopia prevention, and the differences in behavioral attitudes of individuals living in Urban East Asian (more indoor-centric) and Western countries, public policies should be developed in different countries accordingly to encourage children to go outdoors to counteract myopia. This is a short manuscript (presented at the International Myopia Conference-2015 by Prof. Seang Mei Saw) about public policies that should be developed to cope with the "myopia epidemic."

PURPOSE: To investigate whether retinal shape is different between East Asians and Caucasians. METHODS: There were 36 East Asian and 40 Caucasian young adults, with refractions between +0.75D and -5.50D. Peripheral eye lengths were obtained after pupil dilation using the Lenstar partial coherence interferometer. Measurements were obtained along the horizontal and vertical meridians of the visual field out to ±35° and ±30°, respectively, in 5° steps. Retinal co-ordinates were estimated using a validated method from the peripheral eye length measurements and ray-tracing through a modified Le Grand full theoretical eye. Rays were directed normally towards the anterior cornea. Retinal shapes were described in terms of vertex radius of curvature (Rv ), asphericity (Q) and equivalent radius of curvature (REq ) along both horizontal and vertical meridians. RESULTS: Rv was smaller in East Asian than in Caucasians (mean difference ± 95% CI -0.7 ± 0.5 mm), along the horizontal meridian than the vertical meridian (-1.2 ± 0.6 mm), and in myopia than in emmetropia (-1.0 ± 0.6 mm). Rv along the horizontal meridian, but not along the vertical meridian, became smaller as myopia increased. Q did not vary significantly with meridian, refraction group or race. The same pattern of results occurred for REq as for Rv . The percentage differences of heights under the estimated retinal surfaces showed steeper retinas in East Asians than in Caucasians; the differences between East Asian and Caucasian emmetropes were 2.5% and

Recent findings: We discuss how eye and retinal shapes may be described as conicoids, and we describe an axis and section reference system for determining shapes. Explanations are given of how patterns of retinal expansion during the development of myopia may contribute to changing patterns of peripheral refraction, and how pre-existing retinal shape might contribute to the development of myopia. Direct and indirect techniques for determining eye and retinal shape are described, and results are discussed. There is reasonable consistency in the literature of eye length increasing at a greater rate than height and width as the degree of myopia increases, so that eyes may be described as changing from oblate/spherical shapes to prolate shapes. However, one study indicates that the retina itself, while showing the same trend, remains oblate in shape for most eyes (discounting high myopia). Eye shape and retinal shape are not the same and merely describing an eye shape as being prolate or oblate is insufficient without some understanding of the parameters contributing to this; in myopia a prolate eye shape is likely to involve both a steepening retina near the posterior pole combined with a flattening (or a reduction in steepening compared with an emmetrope) away from the pole.

Purpose - To compare lens dimensions and refractive index distributions in type 1 diabetes and age-matched control groups. Methods - There were 17 participants with type 1 diabetes, consisting of two subgroups (7 young [23 ± 4 years] and 10 older [54 ± 4 years] participants), with 23 controls (13 young, 24 ± 4 years; 10 older, 55 ± 4 years). For each participant, one eye was tested with relaxed accommodation. A 3T clinical magnetic resonance imaging scanner was used to image the eye, employing a multiple spin echo (MSE) sequence to determine lens dimensions and refractive index profiles along the equatorial and axial directions. Results - The diabetes group had significantly smaller lens equatorial diameters and larger lens axial thicknesses than the control group (diameter mean ± 95% confidence interval [CI]: diabetes group 8.65 ± 0.26 mm, control group 9.42 ± 0.18 mm; axial thickness: diabetes group 4.33 ± 0.30 mm, control group 3.80 ± 0.14 mm). These differences were also significant within each age group. The older group had significantly greater axial thickness than the young group (older group 4.35 ± 0.26 mm, young group 3.70 ± 0.25 mm). Center refractive indices of diabetes and control groups were not significantly different. There were some statistically significant differences between the refractive index fitting parameters of young and older groups, but not between diabetes and control groups of the same age. Conclusions - Smaller lens diameters occurred in the diabetes groups than in the age-matched control groups. Differences in refractive index distribution between persons with and without diabetes are too small to have important effects on instruments measuring axial thickness.

Purpose To assess intrasessional and intersessional repeatability of two commercial partial coherence interferometry instruments for measuring peripheral eye lengths and to investigate the agreement between the two instruments. Methods Central and peripheral eye lengths were determined with the IOLMaster (Carl-Zeiss Meditec AG, Jena, Germany) and the Lenstar (Haag Streit, Bern, Switzerland) in seven adults. Measurements were performed out to 35° and 30° from fixation for horizontal and vertical visual fields, respectively, in 5° intervals. An external fixation target at optical infinity was used. At least four measurements were taken at each location for each instrument, and measurements were taken at two sessions. Results The mean intrasessional SDs for the IOLMaster along both the horizontal and vertical visual fields were 0.04 ± 0.04 mm; corresponding results for the Lenstar were 0.02 ± 0.02 mm along both fields. The intersessional SDs for the IOLMaster for the horizontal and vertical visual fields were ±0.11 and ±0.08 mm, respectively; corresponding limits for the Lenstar were ±0.05 and ±0.04 mm. The intrasessional and intersessional variability increased away from fixation. The mean differences between the two instruments were 0.01 ± 0.07 mm and 0.02 ± 0.07 mm in the horizontal and vertical visual fields, but the lengths with the Lenstar became greater than those with the IOLMaster as axial length increased (rate of approximately 0.016 mm/mm). Conclusions Both the IOLMaster and the Lenstar demonstrated good intrasessional and intersessional repeatability for peripheral eye length measurements, with the Lenstar showing better repeatability. The Lenstar would be expected to give a slightly greater range of eye lengths than the IOLMaster across the visual field.

PURPOSE. Posterior eye shape assessment by magnetic resonance imaging (MRI) is used to study myopia. We tested the hypothesis that optical coherence tomography (OCT), as an alternative, could measure posterior eye shape similarly to MRI. METHODS. Macular spectral-domain OCT and brain MRI images previously acquired as part of the Singapore Epidemiology of Eye Diseases study were analyzed. The right eye in the MRI and OCT images was automatically segmented. Optical coherence tomography segmentations were corrected for optical and display distortions requiring biometry data. The segmentations were fitted to spheres and ellipsoids to obtain the posterior eye radius of curvature (Rc) and asphericity (Qxz). The differences in Rc and Qxz measured by MRI and OCT were tested using paired t-tests. Categorical assignments of prolateness or oblateness using Qxz were compared. RESULTS. Fifty-two subjects (67.8 ± 5.6 years old) with spherical equivalent refraction from +0.50 to -5.38 were included. The mean paired difference between MRI and original OCT posterior eye Rc was 24.03 ± 46.49 mm (P = 0.0005). For corrected OCT images, the difference in Rc decreased to -0.23 ± 2.47 mm (P = 0.51). The difference between MRI and OCT asphericity, Qxz, was -0.052 ± 0.343 (P = 0.28). However, categorical agreement was only moderate (j = 0.50). CONCLUSIONS. Distortion-corrected OCT measurements of Rc and Qxz were not statistically significantly different from MRI, although the moderate categorical agreement suggests that individual differences remained. This study provides evidence that with distortion correction, noninvasive office-based OCT could potentially be used instead of MRI for the study of posterior eye shape.

Purpose To determine whether human ocular lens position is influenced by gravity. Methods Anterior chamber depth and lens thickness were determined with a Haag-Streit Lenstar LS900 for right eyes of participants in two age groups, with a young group of 13 participants aged 18 to 21 years (mean 21 years, SD 1 year) and an older group of 10 participants aged 50 to 63 years (58 years, 4 years). There were two sessions for each participant separated by at least 48 hours, with one session for the usual upright head position and one session for a downwards head position. In a session, testing was done for minimum accommodation followed by testing at maximum accommodation. A drop of 2% pilocarpine nitrate was instilled, and testing was repeated after 30 minutes under minimum and maximum accommodation conditions. Results Gravity, manipulated through head posture, affected anterior chamber depth for both young adult and older adult groups but mean effects were only small, ranging from 0.04 to 0.12mm, and for the older group required the instillation of an accommodation-stimulating drug. Gravity had a weakly significant effect on lens thickness for the young group without accommodation or a drug, but the effect was small at 0.04±0.06mm (mean±SD, p = 0.04). Conclusion There is a small but real effect of gravity on crystalline lens position, manifested as reduction in anterior chamber depth at high levels of accommodative effort with the head in a downwards position. This provides evidence of the ability of zonules to slacken during strong accommodation.

Purpose This study investigated the association of axial length (AL) to corneal radius of curvature (CRC) ratio with spherical equivalent (SE) in a 3-year old Asian cohort. Methods Three-hundred forty-nine 3-year old Asian children from The Growing Up in Singapore towards Healthy Outcomes (GUSTO) birth cohort study underwent AL and CRC measurements with a noncontact ocular biometer and cycloplegic refraction using an autorefractor. The ratio of AL to CRC (AL/CRC) was calculated for all the participants, and subsequently AL, CRC, and AL/CRC were analyzed in relationship to SE. Results The SE showed better correlation with AL/CRC (Spearman's correlation coefficient, ρ = −0.53; 95% confidence interval [CI]: −0.66; −0.49; P < 0.001) compared to either AL or CRC alone ([ρ = −0.36; 95% CI: −0.51 to 0.51; P = 0.01] and [ρ = 0.05; 95% CI: −0.04 to 0.17; P = 0.34], respectively). Mean AL/CRC was 2.91 ± 0.06 among myopes and decreased to 2.79 ± 0.06 among hyperopes. Axial length to corneal radius of curvature was strongly correlated with SE in myopes (ρ = −0.78; 95% CI: −3.76; −0.79; P = < 0.001), but not in emmetropes and hyperopes ([ρ = −0.39; 95% CI: −10.73; −0.57; P = 0.01] and [ρ = −0.18; 95% CI: −17.28; 12.42; P = 0.38], respectively). Linear regression adjusted for gender and ethnicity showed a 0.74-diopter shift in SE towards myopia with every 0.1 increase in AL/CRC ratio (P < 0.001, r2 = 0.33). Conclusion The correlation between SE and AL/CRC is stronger than that between AL or CRC alone. This suggests that in a research setting, when cycloplegic refraction is difficult to perform on 3-year-old children, AL/CRC may be the next best reference for refractive error. Translational Relevance In the research setting, AL/CRC may be the next best reference for refractive error over AL alone when cycloplegic refraction is unavailable in 3-year old children.

In the recent literature, eye and/or retinal shape have often been inferred from peripheral refraction, and, to a lesser extent, vice versa. Because both the eye's optics and the retinal shape contribute to the peripheral refraction, and there is large variation in the latter, this inference should be made cautiously. Recently retinal shape has been measured independent of optical methods using magnetic resonance imaging. For further work on retinal shape, determining the validity of cheaper alternatives to magnetic resonance techniques is required.

To validate a simple partial coherence interferometry (PCI) based retinal shape method, estimates of retinal shape were determined in 60 young adults using off-axis PCI, with three stages of modeling using variants of the Le Grand model eye, and magnetic resonance imaging (MRI). Stage 1 and 2 involved a basic model eye without and with surface ray deviation, respectively and Stage 3 used model with individual ocular biometry and ray deviation at surfaces. Considering the theoretical uncertainty of MRI (12-14%), the results of the study indicate good agreement between MRI and all three stages of PCI modeling with
