Contains fulltext : 287886.pdf (Publisher’s version ) (Open Access), BACKGROUND: Variants in myosin heavy chain 7 (MYH7) are responsible for disease in 1% to 5% of patients with dilated cardiomyopathy (DCM); however, the clinical characteristics and natural history of MYH7-related DCM are poorly described. OBJECTIVES: We sought to determine the phenotype and prognosis of MYH7-related DCM. We also evaluated the influence of variant location on phenotypic expression. METHODS: We studied clinical data from 147 individuals with DCM-causing MYH7 variants (47.6% female; 35.6 ± 19.2 years) recruited from 29 international centers. RESULTS: At initial evaluation, 106 (72.1%) patients had DCM (left ventricular ejection fraction: 34.5% ± 11.7%). Median follow-up was 4.5 years (IQR: 1.7-8.0 years), and 23.7% of carriers who were initially phenotype-negative developed DCM. Phenotypic expression by 40 and 60 years was 46% and 88%, respectively, with 18 patients (16%) first diagnosed at <18 years of age. Thirty-six percent of patients with DCM met imaging criteria for LV noncompaction. During follow-up, 28% showed left ventricular reverse remodeling. Incidence of adverse cardiac events among patients with DCM at 5 years was 11.6%, with 5 (4.6%) deaths caused by end-stage heart failure (ESHF) and 5 patients (4.6%) requiring heart transplantation. The major ventricular arrhythmia rate was low (1.0% and 2.1% at 5 years in patients with DCM and in those with LVEF of ≤35%, respectively). ESHF and major ventricular arrhythmia were significantly lower compared with LMNA-related DCM and similar to DCM caused by TTN truncating variants. CONCLUSIONS: MYH7-related DCM is characterized by early age of onset, high phenotypic expression, low left ventricular reverse remodeling, and frequent progression to ESHF. Heart failure complications predominate over ventricular arrhythmias, which are rare.

17. An ecomorphological analysis of the determinants of mating success

- Abstract
Investigating the factors affecting the strength of sexual selection is important for understanding the evolution of sex specific morphological and behavioural traits. Traditionally sexual selection studies focus on male ornaments although recent evidence indicates that sexual selection mechanisms also target organismal performance. In the present study we investigated the role of sexually dimorphic morphological and performance traits of the common (viviparous) lizard (Zootoca vivipara Jacquin 1787) with respect to determining mating behaviour. Using an experimental set up controlling for size differences we found that males with longer tails had a higher probability of mating a female. Unexpectedly males with lower bite forces had an advantage over males with higher bite forces whereas males with bigger heads copulated for a longer time with the female. This shows that predicting mating success is not straightforward and is sometimes counterintuitive because a longer tail appears to be beneficial whereas biting harder is not for male Z.?vivipara in a male–female interaction context.

Study of historic buildings requires a pathology analysis of the construction materials used in order to define their conservation state. Usually we can find capillary moisture, salt crystalli-zation or density differences by deterioration. Sometimes this issue is carried out by destructive testing which determine materials’ physical and chemical characteristics. However, they are unfavorable regarding the building’s integrity, and they are sometimes difficult to implement. This paper presents a technique using infrared thermography to analyze the existing pathology and has the advantage of being able to diagnose inaccessible areas in buildings. The results obtained by this technique have been compared with those obtained in the laboratory, in order to validate this study and thus to extrapolate the methodology to other buildings and materials., El estudio de edificios históricos requiere un análisis de la patología de los materiales de construcción empleados para poder definir su estado de conservación. Habitualmente nos encontramos con humedades por capilaridad, cristalización de sales o diferencias de densidad por deterioro. En ocasiones esto se lleva a cabo mediante ensayos destructivos que nos determinan las características físicas y químicas de los materiales, pero que resultan desfavorables respecto a la integridad del edificio, y en ocasiones resulta complejo llevarlos a cabo. Este trabajo presenta una técnica para analizar la patología existente mediante el empleo de termografía infrarroja con la ventaja de poder diagnosticar zonas de difícil acceso en los edificios. Para validar este estudio se han comparado los resultados obtenidos mediante esta técnica con los alcanzados en el laboratorio. De esta forma podemos extrapolar la metodología empleada a otros edificios y materiales.

Se realizó la determinación de las fracciones biodisponibles de arsénico, cadmio, plomo y mercurio en los sedimentos de 10 ríos seleccionados, los cuales abastecen a comunidades de más de 5 000 habitantes en Cuba. Se efectuó la extracción en la fracción compuesta por partículas menores de 63 mym, con ácido clorhídrico 0,5 mol/L. En los casos de arsénico y cadmio, todas las muestras presentaron niveles inferiores a los límites de cuantificación de los métodos de ensayo. Las medianas de las concentraciones de plomo y mercurio fueron en todos los ríos inferiores a 15 y 0,15 myg/g, respectivamente. Se calculó la razón concentración de metal total/concentración de metal biodisponible, en los casos en que se detectaron concentraciones cuantificables de plomo y mercurio biodisponible.The bioavailable fractions of arsenic, cadmium, lead, and mercury in the sediments of 20 selected rivers that supply water to more than 5 000 inhabitants in Cuba were determined. The extraction was made in the fraction composed of particles under 63 mum with hydrochloric acid 0.5 mol/L. In the cases of arsenic and cadmium all the samples presented levels below the limits of quantification of the test methods. The medians of the concentrations of lead and mercury in all rivers were lower than 15 and 0-15 mug/g, respectively. The total concentration of metal/concentration of bioavailable metal ratio was calculated when quantifiable concentrations of bioavailable lead and mercury were found.

Journal Article; Research Support, Non-U.S. Gov't; OBJECTIVE To investigate sensitization to third-stage Anisakis simplex larvae in a randomly selected population in northern Morocco. METHODS We studied sera obtained from clinical analysis laboratories in Tangier and Tetuouan and from fishermen at Tangier port. The age of the study population ranged from 6 to 83 years. ImmunoCAP and immunoblotting techniques were used to determine total and specific immunoglobulin (Ig) E values and the chi2 and Fisher exact tests were applied to analyze relationships between study variables. RESULTS A seroprevalence of 5.1% was found, with a higher percentage of positive sera in the 31-to-43-year age group. Sensitization was not significantly associated with the origin, sex, occupation, or age of the individuals studied. In sera positive by InmunoCAP, immunoblotting studies detected numerous bands of between 7 kDa and >209 kDa, with a predominance of bands in the approximately 20-kDa to 24-kDa range. CONCLUSIONS Although no cases of human anisakiasis have been reported in Morocco to date, part of a randomly selected population in Northern Morocco shows sensitization to A simplex proteins. Yes Objetivos: La finalidad de este estudio ha sido la de investigar la sensibilización frente a la larva L3 de Anisakis, de una población elegida al azar del norte de Marruecos.Métodos: Hemos analizado sueros de pacientes atendidos en laboratorios de análisis clínicos de Tánger y Tetuán, incluyendo una subpoblación de pescadores del puerto de Tánger. La edad de la población ha estado comprendida entre 6-83 años. Se han utilizado las técnicas de ImmunoCAP e Immunoblotting para determinar la IgE total e IgE específi ca y se ha aplicado el estadístico de 2 o el test de Fisher, para conocer el grado de signifi cación entre las distintas variables. Resultados: La seroprevalencia obtenida ha sido del 5.1%, detectándose el mayor nº de sueros positivos en el intervalo de edad de 31-43 años. La sensibilización no está asociada con la procedencia, sexo, ocupación y edad de los individuos estudiados. Entre los sueros positivos por InmunoCAP, se detectaron por inmunoblotting, numerosas bandas entre 7 kDa y >209 kDa, predominando las de ~20 kDa – 24 kDa. Conclusiones: Aunque la anisakiasis humana por ahora no ha sido denunciada en Marruecos, nosotros aportamos datos acerca de la sensibilización con proteínas de Anisakis, de una población elegida al azar en el norte de Marruecos

Se estudian los cambios del paisaje y de la calidad ambiental asociada a ellos, de un espacio protegido desde hace casi un siglo, Parque Nacional de la Montaña de Covadonga, que ha sido ampliado en las últimas décadas, configurándose el actual Parque Nacional de los Picos de Europa (PNPE). Los resultados obtenidos permiten conocer los cambios del paisaje del territorio del PNPE, en su conjunto, en él de la zona histórica (PNMC) y en él de la ampliación más reciente (PNPE-PNMC). Los cambios de paisaje, variación espacial de las tramas de relaciones ecosistémicas del territorio, condicionan cambios en los valores ambientales soportados por esos procesos ecológicos. Los valores ambientales se han definido de acuerdo con los objetivos del espacio protegido, definidos en su Ley de declaración. Los tres territorios han sufrido pautas de cambio muy parecidas, predominado la conservación del paisaje en amplias extensiones de ellos. Los cambios de usos más apreciables, si bien no muy cuantiosos, son el aumento en las superficies forestales arboladas y la disminución de las forestales arboladas ralas y de las cultivadas. En los tres territorios han aumentando los valores naturalísticos y sistémicos. Los culturales-antropológicos-educativos disminuyen muy poco, también en los tres ámbitos. Los recreativos disminuyen en PNMC, aumentan en PNMC-PNPE y no cambian en PNPE. Los educativos son los que menos variación presentan en todos los casos, con muy pequeños aumentos o disminuciones. La principal diferencia entre los tres ámbitos estudiados es que en los territorios PNPE y PNMC los cambios de calidad ambiental global responden a variaciones del mismo signo de todos los valores ambientales. En el territorio PNPE-PNMC, por el contrario, hay diferentes combinaciones de aumentos y disminuciones de Jos valores ambientales que conducen a cambios equivalentes de calidad ambiental. Métodos como este pueden ser incorporados en las actividades de planificación y gestión habituales de los parques nacionales para monitorizar sus cambios y ayudar a su gestión, de acuerdo con los objetivos del espacio protegido.

Métodos como este pueden ser incorporados en las actividades de planificación y gestión habituales de los, Landscape and environmental quality changes, related to them, in a protected a rea since almost a century, Montaña de Covadonga National Park, enlarged in the last decades configuring the current Picos de Europa National Park, are studied. Obtained results allow us to recogni ze landscape changes occurred in the whole PNPE territory, in the one corresponding to the historical park (PNMC) and in the one corresponding to the recently included area (PNPE-PNMC). Landscape changes, considered as the spatial variation in the territory's ecositemic relationships, condition changes in the environmental values supported by these ecological processes. Environmental values have been defined in accordance with the protected area's conservation objectives, stated on its declaration act. The three territories have undergone very similar landscape changes, with landscape conservation as the main trend in the most area . Noteworthy land use changes, although not very numerous, are the increase in forest areas and the decrease in sparse trees forests and cultivated ones. In the three territories naturalistic and systemic va lues have increased . Cultural-anthropologic-educative values slightly decreases. Recrea tional ones decrease in PNMC, increases in PNMC-PNPE and do not change in PNPE. Educative values show the smallest change in the three territories. The main difference observed is that in PNPE and PNMC territories global environmental guality change is related with same sign changes in the whole set of environmental values. In the PNPE-PNMC territory, on the contrary, different combinations in increases or decreases in environmental va lues lead to equivalent changes in global environmental quality. Methods as the one developed here can be implemented in the protected area planning and management routinely activities. This will allow to protected area managers monitoring landscape change and help them in the management work, in accordance with the protected area objectives.

In this article we propose a model of innovation competence of people, based on the existing literature to integrate and complement the existing models. The main contribution of this work consists in demonstrating the differences and similarities of current models and in providing a conceptual definition for each model element. The 19th century was disastrous as far as the conservation of architectonic heritage is concerned. The awareness of the importance of preserving monuments that has prevailed since the end of the last century was dazzlingly absent in the previous, leading both to the disappearance of representative heritage works and the plundering of many others. The present study establishes the methodological basis to proceed with the virtual reconstruction of many disappeared architectures, representative of emblematic architectonic typologies. A method based on the combination of deduction and induction allows benchmarks to be created that signify a starting point to which the key and specific elements of each building are later incorporated, from the data extracted from the conserved parts and the graphic, literary and archive documents. The result is the virtual recovery of the general outlines of the architecture: morphology of the plot, volumetry, exterior and interior facades, and the functional layout. The good results obtained in the study of the disappeared Ambassador Vich's Palace, allow the methodology to be extended to the analysis of other similar examples, serving investigators as a tool to carry out an arduous task of deciphering a trail that is increasingly fading with the passing of time. - Abstract
Objective: The purpose of this study was to assess the complication rates of incidental appendectomies in women who undergo benign gynecologic procedures.Study Design: This was a retrospective case-controlled study of patients who did (n=100 women) or did not (n=100 women) undergo incidental appendectomies at the time of an abdominal hysterectomy between June 1995 and January 2001. Information was abstracted from hospital and clinic records and a gynecologic oncology database. Data were obtained about age, body mass index, hypertension, diabetes mellitus, the number of days with nothing by mouth, the length of hospital stay, and postoperative complications (cellulitis, fever, ileus, pneumonia, thromboembolic disease). Data were analyzed with the use of two-sample t tests, Wilcoxon Rank sum tests, chi(2) tests, and multiple logistic regressions.Results: There was no difference in preoperative diagnosis or operative procedure for either group. The number of patients in the group that did have incidental appendectomy versus the group that did not have incidental appendectomy with additional procedures at the time of abdominal hysterectomy was bilateral salpingo-oophorectomy (66 vs 61 women), unilateral oophorectomy (19 vs 19 women), lysis of adhesions (9 vs 8 women), and others (12 vs 8 women). Compared with the group that did not have incidental appendectomy, the group that did have incidental appendectomy was younger (mean age+/-SD: 44+/-9.6 years vs 48+/-13.6 years, P=.02) and had a lower mean body mass index (26.1+/-6.0 kg/m(2) vs 29.8+/-8.9 kg/m(2), P=.0009). No significant differences were found between the two groups (the group that did have incidental appendectomy vs the group that did not have incidental appendectomy, respectively) with respect to the following postoperative complications: fever (40 vs 27 women), cellulitis (1 vs 2 women), wound collection (4 vs 6 women), wound dehiscence (1 vs 5 women), wound abscess (7 vs 6 women), ileus (3 vs 2 women), and urinary tract infection (4 vs 10 women). The mean length of hospital stay was significantly longer in the group that did have incidental appendectomy than in the group that did not have incidental appendectomy (3.6+/-1.52 days vs 3.1+/-1.1 days, P=.006). However, the difference was no longer significant when patients who were fed electively on the postoperative day 2 were excluded from the analysis (3.16+/-1.13 days vs 3.04+/-1.13 days, P=.507). Thirty-one percent of the histologic specimens were abnormal, with fibrous obliteration being most common, and there was one case of acute appendicitis.Conclusion: An incidental appendectomy at the time of benign gynecologic procedures does not increase postoperative complication rates or length of hospital stay. The inclusion of incidental appendectomies in all abdominal hysterectomies could potentially decrease the morbidity and mortality rates because of appendicitis in elderly women.
