The spatial mapping of localized events in brain activity critically depends on the correct identification of the pattern signatures associated with those events. For instance, in the context of epilepsy research, a number of different electrophysiological patterns have been associated with epileptogenic activity. Motivated by the need to define automated seizure focus detectors, we propose a novel data-driven algorithm for the spatial identification of localized events that is based on the following rationale: the distribution of emerging oscillations during confined events across all recording sites is highly non-uniform and can be mapped using a spatial entropy function. By applying this principle to EEG recording obtained from 67 distinct seizure epochs, our method successfully identified the seizure focus on a group of ten drug-resistant temporal lobe epilepsy patients (average sensitivity: 0.94, average specificity: 0.90) together with its characteristic electrophysiological pattern signature. Cross-validation of the method outputs with postresective information revealed the consistency of our findings in long follow-up seizure-free patients. Overall, our methodology provides a reliable computational procedure that might be used as in both experimental and clinical domains to identify the neural populations undergoing an emerging functional or pathological transition. L'epilèpsia és una entitat complexa, caracteritzada per la predisposició a patir crisis epilèptiques i, per les seves conseqüències neurobiològiques, cognitives, psicològiques i socials. El tractament farmacològic és la primera opció terapèutica, però aproximadament, el 25% de pacients són farmacorresistents. D'ells, la majoria presenten epilèpsia del lòbul temporal (ELT). Per a ells, la cirurgia de l'epilèpsia pot ser tractament d'elecció per al control de les crisis, malgrat això, s'associa amb un risc dèficit cognitiu, així, l'avaluació neuropsicològica basal i de seguiment, és una eina fonamental per a caracteritzar i pronosticar els efectes de cada tractament. També, és important en estudis comparatius, comptar un grup control adequat (pacients en llista d'espera per a cirurgia de l'ELT, avaluats en el mateix interval de seguiment). Els objectius són: estudiar els canvis cognitius i psicosocials intragrup i entregrups longitudinalment, segons la lateralització de l'epilèpsia; desenvolupar dades normatives de canvi a l'any mitjançant algorismes d'índex de canvi fiable (ICF), amb dades extretes del grup control i estudiar els canvis individuals, a pacients sotmesos a diferents cirurgies de l'ELT, segons la seva lateralització, aportant percentatges de pacients que van mostrar pèrdues o guanys significatius i finalment, estudiar l'associació d'aquests canvis a l'any, amb variables sociodemogràfiques o clíniques relacionades amb l'epilèpsia. La mostra total va estar formada per 72 pacientes amb ELT unilateral (grup control n=12; grups quirúrgics: estàndard n=22; selectiva n=20 i termocoagulació n=18). Per a la representació gràfica mitjançant perfils cognitius, es van transformar les puntuacions brutes de cada grup a puntuacions escalars, segons ajustos per edat, escolaritat i gènere. Per a estudiar els canvis longitudinals intragrup i entre grups, es van aplicar models lineals mixtos (ANOVA per a mesures repetides), de manera separada segons la lateralització, prenent com a efectes constants, el temps (pre-postquirúrgics) i el grup de tractament. El càlcul dels intervals de confiança (IC) va ser del 95% i dels p-valors corresponents a les comparacions, es va basar en la distribució t multivariant. Es va aplicar l'algorisme de l'ICF per a totes les variables d'estudi, corregint per l'efecte de pràctica per a les variables cognitives. L'estudi d'associació es va realitzar mitjançant models de regressió ANOVA (per a mesures categòriques) i ANCOVA (per a mesures numèriques), a més del coeficient de correlació de Pearson. Les principals conclusions es basen en la consistència amb la literatura, sobre l'impacte de la cirurgia resectiva de l'ELT de lateralització esquerra, en la memòria verbal i el llenguatge, (major després de cirurgia estàndard), especialment quan estan preservades en l'avaluació basal. El grup control i el grup sotmès a termocoagulació, no van mostrar diferències intragrup ni entre grups entre les seves tendències de canvi longitudinal. Això aporta per primera vegada, informació sobre la seguretat cognitiva de la tècnica de termocoagulació mitjançant radiofreqüència (TC-RF). La cirurgia mostra un efecte beneficiós sobre la simptomatologia psiquiàtrica, enfront del grup tractat farmacològicament (increment de símptomes). Quant a l'ICF, s'aporten taules de canvis normatius a l'any en ELT. Aplicant aquestes fórmules, van destacar alts percentatges de pacients que van patir una pèrdua significativa després de cirurgies resectives de lateralització esquerra, concretament en memòria verbal, seguits del llenguatge. Contràriament, un nombre elevat de pacients sotmesos a cirurgies resectives de lateralització dreta, van mostrar millores en la qualitat de vida, podent estar associat a majors taxes de control de crisi. Un dels factors clínics, que va demostrar una associació significativa amb el canvi en memòria verbal, va ser l'edat d'inici de l'epilèpsia, d'acord amb la literatura. La epilepsia es una entidad compleja, caracterizada por la predisposición a sufrir crisis epilépticas y, por sus consecuencias neurobiológicas, cognitivas, psicológicas y sociales. El tratamiento farmacológico es la primera opción terapéutica, pero aproximadamente, el 25% de pacientes son fármacorresistentes y la mayoría presentan epilepsia del lóbulo temporal (ELT). La cirugía de epilepsia puede ser tratamiento de elección para controlar las crisis, sin embargo, se asocia con riesgo de déficit cognitivo. , La evaluación neuropsicológica basal y de seguimiento, es una herramienta fundamental para caracterizar y pronosticar los efectos de cada tratamiento. Además, es importante en estudios comparativos, contar con un grupo control adecuado (candidatos para cirugía de ELT, evaluados en el mismo intervalo de seguimiento). Los objetivos son: estudiar los cambios cognitivos y psicosociales intragrupo y entregrupos longitudinalmente, según la lateralización de la epilepsia; desarrollar datos normativos de cambio al año mediante algoritmos de índice de cambio fiable (ICF), con datos extraídos del grupo control y estudiar los cambios individuales en pacientes sometidos a diferentes cirugías de ELT, según su lateralización, aportando porcentajes de pacientes que mostraron pérdidas o ganancias significativas y por último, estudiar la asociación de estos cambios al año, con variables sociodemográficas o clínicas relacionadas con la epilepsia. La muestra total estuvo formada por 72 personas con ELT unilateral (grupo control n=12; grupos quirúrgicos: estándar n=22; selectiva n=20 y termocoagulación n=18). Para representar gráficamente perfiles cognitivos promedio, se transformaron las puntuaciones brutas de cada grupo a puntuaciones escalares, según ajustes por edad, escolaridad y género. Para estudiar los cambios longitudinales intragrupo y entregrupos, se aplicaron modelos lineales mixtos (ANOVA para medidas repetidas), de manera separada según la lateralización, tomando como efectos fijos, el tiempo (pre-postquirúrgicos) y el grupo de tratamiento. El cálculo de los intervalos de confianza (IC) fue del 95% y de los p-valores correspondientes a las comparaciones, se basó en la distribución t multivariante. Se aplicó el algoritmo del ICF para todas las variables de estudio, corrigiendo por el efecto de práctica para las variables cognitivas. El estudio de asociación se realizó mediante modelos de regresión ANOVA (para medidas categóricas) y ANCOVA (para medidas numéricas), además del coeficiente de correlación de Pearson. Las principales conclusiones se basan en la consistencia con la literatura, sobre el impacto de la cirugía resectiva de ELT de lateralización izquierda, en la memoria verbal y el lenguaje, (mayor tras cirugía estándar), especialmente cuando están preservadas en la evaluación basal. El grupo control y el grupo sometido a termocoagulación, no mostraron diferencias intragrupo ni entregrupos entre sus tendencias de cambio longitudinal. Esto aporta por primera vez, información sobre la seguridad cognitiva de la técnica de termocoagulación mediante radiofrecuencia (TC-RF). La cirugía muestra un efecto beneficioso sobre la sintomatología psiquiátrica frente al grupo tratado farmacológicamente (incremento de síntomas). En cuanto al ICF, se aportan tablas de cambios normativos al año en ELT. Aplicando estas fórmulas, destacaron altos porcentajes de pacientes que sufrieron una pérdida significativa tras cirugías resectivas de lateralización izquierda, concretamente en memoria verbal, seguidos del lenguaje. De manera opuesta, un alto número de pacientes sometidos a cirugías resectivas de lateralización derecha, mostraron mejoras en la calidad de vida, pudiendo estar asociado a mayores tasas de control de crisis. Uno de los factores clínicos, que demostró una asociación significativa con el cambio en memoria verbal, fue la edad de inicio de la epilepsia, de acuerdo a la literatura. Epilepsy is a complex disease characterized by an enduring predisposition to generate epileptic seizures and by the neurobiologic, cognitive, psychological, and social consequences. Although antiepileptic drug treatment is the first recommended therapeutic approach, 25% of patients with epilepsy are pharmacoresistant. The majority of these patients present temporal lobe epilepsy (TLE). In these cases, epilepsy surgery can be an elective treatment to control epileptic seizures. However, epilepsy surgery can lead to cognitive impairments, highlighting the importance of neuropsychological assessments before surgery and at consequential follow-ups. Further, for the purpose of these comparative studies, it is important to have an adequately matched control group (patients on the waitlist for TLE surgery). The main aim of this thesis is to examine cognitive and psychosocial changes, both within (intragroup) and between groups longitudinally, taking into account the lateralization of epilepsy. The second aim is to provide matched control norms for neuropsychological testing, in order to examine improvements or declines in cognition through the Reliable Change Index (RCI). Thirdly, this thesis will examine the association between demographic and clinical variables in relation to neuropsychological changes at a one-year follow up. The overall sample was comprised of 72 participants with unilateral TLE (standard surgery, n=22; selective surgery, n=20; thermocoagulations, n=18) and 12 matched controls. Cognitive raw scores were transformed into scalar scores adjusted for age, education and gender. In order to examine longitudinal changes in cognition within and between groups, linear mixed models (repeated measures ANOVA) stratified for lateralization were conducted. Fixed effects included time (pre-post surgery) and treatment group. Confidence intervals (95%) and p-values were based on the multivariate t-distribution. The RCI algorithm was applied to all variables in the study. The adjustment for possible practice effects was applied to cognitive measures. The association study was conducted using ANOVA (for categorical measures) and ANCOVA (for numerical measures) regression models, in addition to the Pearson correlation coefficient. In line with the literature, the primary results of this study showed a greater impact of standard resective surgery in left-sided TLE in verbal memory and language, especially when these functions were preserved at baseline assessment. The control group and the thermocoagulated group showed no differences within and between groups in rates of longitudinal change. These results, for the first time, provide information regarding the safety of Radiofrequency-Thermocoagulation (RT-FC) treatment with regards to cognitive functioning. Surgery had a beneficial effect on psychiatric symptoms compared to the pharmacologically treated group, which had an increase of symptoms. With regards to RCI, tables are provided of the normed changes at one-year of follow-up in TLE. Applying these formulas, a high percentage of patients suffered a significant cognitive loss after left-sided resective surgery in verbal memory followed by language. By contrast, a high percentage of patients who underwent right-sided resective surgery showed improvement in quality of life, possibly due to greater rates of controlled crisis. Finally, with regard to clinical and demographic factors, this study found that age of onset of epilepsy was associated with verbal memory change, in line with previous literature. Funder: National Institute for Health Research (NIHR) GOSH BRC, OBJECTIVE: Drug-resistant focal epilepsy is often caused by focal cortical dysplasias (FCDs). The distribution of these lesions across the cerebral cortex and the impact of lesion location on clinical presentation and surgical outcome are largely unknown. We created a neuroimaging cohort of patients with individually mapped FCDs to determine factors associated with lesion location and predictors of postsurgical outcome. METHODS: The MELD (Multi-centre Epilepsy Lesion Detection) project collated a retrospective cohort of 580 patients with epilepsy attributed to FCD from 20 epilepsy centers worldwide. Magnetic resonance imaging-based maps of individual FCDs with accompanying demographic, clinical, and surgical information were collected. We mapped the distribution of FCDs, examined for associations between clinical factors and lesion location, and developed a predictive model of postsurgical seizure freedom. RESULTS: FCDs were nonuniformly distributed, concentrating in the superior frontal sulcus, frontal pole, and temporal pole. Epilepsy onset was typically before the age of 10 years. Earlier epilepsy onset was associated with lesions in primary sensory areas, whereas later epilepsy onset was associated with lesions in association cortices. Lesions in temporal and occipital lobes tended to be larger than frontal lobe lesions. Seizure freedom rates varied with FCD location, from around 30% in visual, motor, and premotor areas to 75% in superior temporal and frontal gyri. The predictive model of postsurgical seizure freedom had a positive predictive value of 70% and negative predictive value of 61%. SIGNIFICANCE: FCD location is an important determinant of its size, the age at epilepsy onset, and the likelihood of seizure freedom postsurgery. Our atlas of lesion locations can be used to guide the radiological search for subtle lesions in individual patients. Our atlas of regional seizure freedom rates and associated predictive model can be used to estimate individual likelihoods of postsurgical seizure freedom. Data-driven atlases and predictive models are essential for evidence-based, precision medicine and risk counseling in epilepsy.
