Abstract

The present study addresses the pattern distribution of recent landslides in Italy. The main objective is the detection and mapping of spatio-temporal clusters of landslides that occurred in the period 2010–2017 in the country. To this aim, a subdivision of the national area into 158 warning zones, as identified by the 21 civil protection regional centres to deal with weather-induced hydro-geological hazards, is adopted. Information on landslides comes from FraneItalia, a geo-referenced catalogue developed consulting online news sources. Analyses are performed both at national scale and at a regional scale, focusing on the Campania region. The space–time permutation scan statistics model is applied to detect statistically significant clustering, accounting for the geographical spatial dimension and for the temporal dimension. Two types of analyses are performed: annual, considering each single year; and multi-annual, encompassing the entire 8-year study period. In both cases, spatio-temporal cluster analyses are able to detect areas and frame-periods characterised by relevant and recurrent landslide activity. Finally, the obtained results are compared with a standard landslide density map, highlighting the complementarity of the two approaches. [ABSTRACT FROM AUTHOR]

Abstract

Background: To report health-related quality of life outcomes as assessed by validated patient-reported outcome measures (PROMs) after radical prostatectomy (RP). ­Methods: This study analyzed patients treated with RP within The PROState cancer monitoring in Italy, from the National Research Council (Pros-IT CNR). Italian versions of Short-Form Heath Survey and university of California los Angeles-prostate cancer index questionnaires were administered. PROMs were physical composite scores, mental composite scores and urinary, bowel, sexual functions and bothers (UF/B, BF/B, SF/B). Baseline unbalances were controlled with propensity scores and stabilized inverse weights; differences in PROMs between different RP approaches were estimated by mixed models. Results: Of 541 patients treated with RP, 115 (21%) received open RP (ORP), 90 (17%) laparoscopic RP (LRP) and 336 (61%) robot-assisted RP (RARP). At head-to-head ­comparisons, RARP showed higher 12-month UF vs. LRP (interaction treatment * time p = 0.03) and 6-month SF vs. ORP (p < 0.001). At 12-month from surgery, 67, 73 and 79% of patients used no pad for urinary loss in ORP, LRP and RARP respectively (no differences for each comparison). Conversely, 16, 27 and 40% of patients declared erections firm enough for sexual intercourse in ORP, LRP and RARP respectively (only significant difference for ORP vs. RARP, p = 0.0004). Conclusions: Different RP approaches lead to significant variations in urinary and sexual PROMs, with a general trend in favour of RARP. However, their clinical significance seems limited.

Abstract

Objectives: To investigate screening with the Edinburgh Postnatal Depression Scale (EPDS) as part of Queensland prenatal care services, as well as maternal and socio-demographic factors associated with not being screened. Design, setting: Cross-sectional retrospective analysis of data from the Queensland population-based Perinatal Data Collection for July 2015 – December 2015. Participants: All women giving birth in Queensland during the second half of 2015. Main outcome measures: Screening with the EPDS, with the values “yes” (health professional recorded an EPDS score), “no” (health professional reported it was not performed), and “not stated”. Results: Of 30 468 women who gave birth in Queensland, 21 735 (71.3%) completed the EPDS during pregnancy; 18 942 pregnant women were enrolled as public patients (91.0%) and 2762 as private patients (28.8%). After adjusting for other socio-demographic factors, screening was less likely for women who were aged 36 years or more (v 25 years or younger: adjusted odds ratio [OR], 0.69; 95% CI, 0.60–0.79), enrolled as private patients (aOR, 0.05; 95% CI, 0.05–0.06), born overseas (aOR, 0.75; 95% CI, 0.68–0.82), Indigenous Australians (aOR, 0.47; 95% CI, 0.39–0.56), single or separated (aOR, 0.83; 95% CI, 0.73–0.94), or of higher socioeconomic status. Conclusions: Four years after clinical guidelines recommending universal screening with the EPDS were published, screening rates for private and public health care patients differed markedly. Our results may inform future comparisons and analyses of the impact on screening of recent changes to Medicare definitions intended to increase that of women in private health care.

It has to be distinguished from the mental dimensions: impulsivity, mood, anxiety and body image connected to the emotional regulation system producing eating behaviors. Obesity subjects differ in eating behaviors: gorging, snacking, sweeteating, grazing and binge that are characterised by different level of psychopathology. Mental disorders are also associated to eating behaviors. Bariatric surgery is considered gold standard therapy for obesity. However, follow-up studies underline that the association obesity-mental disorders determines weight loss failure. Methods: 2205 obese subjects underwent psychiatric assessment before bariatric surgery. Patients were divided into two groups as result of psychiatric assessment: 1392 obese subjects without association with mental disorders and 813 with mental disorders. These last (mean age 37,63 SD ± 12,07; 181 M, 632 W; mean body mass index (BMI), 45,16 SD ± 12,14) were enrolled in this study. Every patient underwent psychiatric evaluation. The absence of mental disorders was considered an exclusion criteria. Results: In our sample, Binge Eating Disorder (BED) and Night Eating Syndrome (NES) have the major prevalence. General Anxiety Disorder (GAD) and Major Depressive Disorder (MDD) are also related to obesity. Educational level, in our sample, does not influence in a significant way the mental disorders. Age does not change the psychopathological frame. Conclusion: This study emphasizes that the association between obesity and mental disorders is high among bariatric surgery candidates of our region. The relationship with eating behaviors is connected to the general features of the mental disorders but is not yet clear the reciprocal influences nor why some obese subjects suffer from both and some others does not.

Abstract

"High-dose methotrexate (MTX) is a key component of most treatment protocols for childhood and adolescent non-Hodgkin lymphoma (NHL). Recent studies have suggested that the toxicity of antifolate drugs, such as MTX, is affected by inherited single nucleotide polymorphisms (SNPs) in folate metabolizing genes. The aim of our study was to investigate the potential influence of the C677T and A1298C genetic variants of the methylenetetrahydrofolate reductase (MTHFR) gene on the clinical toxicity and efficacy of MTX in pediatric patients with NHL (n = 95) treated with therapeutic protocols Associazione Italiana Ematologia Oncologia Pediatrica (AIEOP) LNH-97 and EURO LB-02. We demonstrated that patients with the 677T genotype had an approximately six-fold greater risk of developing hematological toxicity compared with wild-type carriers, especially in the 1 g\/m2 treatment group (p = 0.01). Moreover, we identified a correlation between the risk of relapse and the T genotype: T carriers had reduced disease-free survival compared with wild-type patients (67% vs. 100%). Our data suggest a pharmacogenetic influence on the adverse effects of high-dose MTX in the 1 g\/m2 treatment group.. . "

Abstract

La leucemia linfoblastica acuta(LLA) è la neoplasia pediatrica più frequente. Sebbene molti progressi siano stati fatti ancora oggi il 30% va incontro a ricadute dopo un primo trattamento. Le basi molecolari della farmacoresistenza non sono ancora del tutto note.Dati recenti indicano che l’upregulation di Bcl-xL,Erk1 e AKT attivate possa essere associata alle neoplasie ematologiche (1). Di recente è stata isolata un'isoforma della Manganese Superossido Dismutasi (rMnSOD)con funzione oncosoppressiva, facilmente iniettabile ed in grado di essere internalizzata in tutte le cellule, normali e tumorali dove, trasforma i radicali liberi in H2O2. Solo le cellule normali possono convertire il perossido in ossigeno molecolare ed acqua, mentre quelle tumorali ,carenti di catalasi, muoiono per apoptosi Anche nelle leucemie è stata osservata una carenza di catalasi.La proteina rMnSOD è risultata protettiva per le cellule e tessuti normali e citotossica per quelle tumorali(2). Essa entra nelle cellule attraverso il suo peptide leader, non clivato,che costituisce perciò il suo carrier molecolare. Studi recenti effettuati su cellule di carcinoma ovarico, mammario e polmonare utilizzato come carrier coniugato al cisplatino, mostrano che le cellule muoiono per apoptosi rapidamente. Obiettivi 1) Valutare la capacità della rMnSOD o del peptide carrier coniugato al cisplatino o alla epirubicina di essere internalizzati nei blasti leucemici di bambini affetti da LLA e di svolgere una funzione citotossica, come dimostrato su linee tumorali 2) Valutare la modulazione del livello d’espressione di alcuni geni coinvolti nella regolazione dell'apoptosi quali Bax, Bcl2, Bcl-xL e la fosforilazione di AKT e ERK1 prima e dopo trattamento con peptide-cisplatino, peptide-epirubicina e rMnSOD. Materiali e Metodi I blasti leucemici sono stati separati da sangue midollare di bambini affetti da Leucemia Linfoblastica Acuta (LLA) B(n°4) e T (n°4)e 1 campione di linfoma non Hodgkin leucemizzato. Le cellule sono state coltivate in RPMI, FBS 10% e PENSTREP 1%. I test di vitalità utilizzati sono:Trypan Blue e Saggio MTT. Con tecniche immunocitochimiche è stata valutata la internalizzazione delle molecole mediante l’uso di un anticorpo policlonale diretto contro la sequenza del peptide leader della rMnSOD sia in M.O che al TEM. L'analisi quantitativa delle proteine è stata effettuata mediante Western Blotting. Risultati:I dati ottenuti al M.O. e TEM evidenziano una marcata positività citoplasmatica che dimostra l'ingresso del peptide-cisplatino,peptide-epirubicina e della sola proteina rMnSOD nelle cellule leucemiche. L’analisi molecolare sulle cellule leucemiche trattate in vitro in presenza della rMnSOD indica chiaramente che essa esplica un effetto citotossico in maniera dose-dipendente, inducendo l’apoptosi nelle cellule. In particolare nei blasti leucemici di pazienti ad alto rischio e di linfoma leucemizzato ,clinicamente chemioresistente ,provoca una significativa modulazione del gene Bax, principale responsabile del processo apoptotico. La fosforilazione di AKT non sembra essere influenzata dal trattamento con rMnSOD mentre quella di ERK1 è attivata dopo trattamento con rMnSOD nelle LLA a cellule B a basso rischio. Conclusione. Le osservazioni preliminari ottenute in questo studio lasciano ipotizzare un possibile impiego della rMnSOD e del peptide leader quale veicolo piu' efficace di citostatici nel trattamento delle LLA pediatriche. Inoltre l'impiego di citostatici con dosaggi farmacologici minori potrebbe condurre ad una terapia più specifica, selettiva e priva di effetti collaterali.

Abstract

The Erectile Dysfunction Observational Study (EDOS) is a 6-months observational prospective multicentric study enrolling men with erectile dysfunction (ED) who asked, to be started on a treatment or to change a previous treatment. Aims of the study were to analyse the pattern of treatment and compare the efficacy of treatments used. Patients were enrolled during a normal hospital visit and were prescribed a treatment for ED. They were asked at baseline and after 3 and 6 months, to answer a set of questions from the International Index of Erectile Function, Erectile Dysfunction Inventory of Treatment Satisfaction (EDITS) and Short Form of the Psychological and Interpersonal Relationships Scale questionnaires (SF-PAIRS). Clinicians were free to prescribe any therapy for ED available in the market, and to change therapy at any time during the study. Out of 1 338 patients, available for analysis at 6 months, 624 (47%) changed their treatment during the study and 714 (53%) continued with the drug prescribed at baseline. Patients assuming tadalafil had a significantly higher probability of maintaining the same treatment compared to sildenafil or vardenafil. There was no clinically significant difference in terms of efficacy, patient satisfaction, self-confidence and spontaneity between the different inhibitors of PDE5. The 'time concerns' domain score of SF-PAIRS, was statistically better in patients assuming tadalafil. In conclusion sildenafil, vardenafil and tadalafil show similar efficacy in the clinical practice. However, patients receiving tadalafil display a lower risk to discontinue or change the treatment.
