Introduction and objectives: The association between exercise and coronary atherosclerosis still remains unclarified. We aimed to analyze the prevalence of high coronary atherosclerotic burden in veteran athletes, considering cardiovascular (CV) risk and volume of exercise. Methods: A total of 105 asymptomatic male veteran athletes (48±5.6 years old) were studied. A high coronary atherosclerotic burden was defined as one of the following characteristics in coronary computed tomography angiography: calcium score >100, >75th percentile, obstructive plaques, involving left main, three-vessels or two-vessels including proximal anterior descending artery, segment involvement score >5 or CT-adapted Leaman score ≥5. CV risk was stratified by SCORE2 and volume of exercise by metabolic equivalent task score. Results: Most athletes (n=88) were engaged in endurance sports for 17.1±9.8 years, with a median exercise volume of 66 [IQR 44–103] metabolic equivalent of tasks/hour/week. The mean Systematic Coronary Risk Evaluation 2 was 2.8±1.5%; 76.9% of athletes had a low-moderate risk and none a very high risk. High coronary atherosclerotic burden was present in 25.7% athletes. Athletes with high cardiovascular risk and high exercise volume (above the median) showed significantly high coronary atherosclerotic burden compared to those with low-moderate risk and high volume (50.0% vs. 15.6%; p=0.017). Among athletes with low to moderate risk, a high volume of exercise tended to be protective, while in those with low volume, there was similar rate of high coronary atherosclerotic burden, regardless of CV risk. Conclusions: A combination of higher volume of exercise and high cardiovascular risk revealed the worst association with coronary atherosclerosis in veteran athletes. The relationship between these variables is controversial, but integrating exercise characteristics and risk assessment into preparticipation evaluation is essential. O nosso objetivo foi analisar a prevalência de alta carga aterosclerótica coronária (CASC) em atletas veteranos, integrando o risco cardiovascular (RCV) e volume de exercício. Métodos: Avaliámos 105 atletas veteranos, homens assintomáticos (48±5,6 anos). Alta CASC foi definida como uma das: score de cálcio >100, >percentil 75, placas obstrutivas, envolvimento do tronco da artéria coronária esquerda, doença de 3 vasos ou 2 vasos incluindo artéria descendente anterior proximal, Segment Involvement Score >5 ou Score Leaman adaptado por CT ≥5. Estratificámos o RCV pelo SCORE2 e o volume de exercício por equivalentes metabólicos (METs). Resultados: A maioria dos atletas (n=88) praticava desportos de resistência, durante 17,1±9,8 anos, com um volume de exercício mediano de 66 [IQR 44-103] METs/hora/semana. O SCORE2 médio foi 2,8±1,5% e 76,9% dos atletas tinham RCV baixo-moderado e nenhum RCV muito-alto. Alta CASC ocorreu em 25,7%. Atletas com alto RCV e alto volume de exercício (acima da mediana) apresentaram CASC significativamente alta em comparação com os com RCV baixo-moderado e alto volume (50,0% versus 15,6%; p=0,017). Nos atletas com RCV baixo-moderado, o alto volume de exercício mostrou uma tendência protetora, enquanto houve uma taxa semelhante de alta CASC naqueles com baixo volume, independentemente do RCV. Conclusões: Em atletas veteranos, a combinação de maior volume de exercício com alto RCV mostrou a pior associação com aterosclerose coronária. A relação entre estas variáveis é controversa, mas integrar características do exercício e o RCV na avaliação pré-participação é essencial.

Introduction and Objectives: Obstructive coronary artery disease (CAD) remains the most common etiology of heart failure with reduced ejection fraction (HFrEF). However, there is controversy whether invasive coronary angiography (ICA) should be used initially to exclude CAD in patients presenting with new-onset HFrEF of unknown etiology. Our study aimed to develop a clinical score to quantify the risk of obstructive CAD in these patients. Methods: We performed a cross-sectional observational study of 452 consecutive patients presenting with new-onset HFrEF of unknown etiology undergoing elective ICA in one academic center, between January 2005 and December 2019. Independent predictors for obstructive CAD were identified. A risk score was developed using multivariate logistic regression of designated variables. The accuracy and discriminative power of the predictive model were assessed. Results: A total of 109 patients (24.1%) presented obstructive CAD. Six independent predictors were identified and included in the score: male gender (2 points), diabetes (1 point), dyslipidemia (1 point), smoking (1 point), peripheral arterial disease (1 point), and regional wall motion abnormalities (3 points). Introduction: Proportionality of secondary mitral regurgitation (sMR) may be a key factor in deciding whether a patient may benefit from mitral intervention. The aim of this study was to evaluate the prognostic value of two different concepts of proportionality and assess their ability to improve MR stratification proposed by the American Society of Echocardiography (ASE) guidelines. Methods: We conducted a retrospective analysis in patients with reduced left ventricular ejection fraction (LVEF) (0.14; b) Lopes et al. – disproportionate sMR whenever measured EROA>theoretical EROA (determined as 50%×LVEF×LVEDVMitralVTI). Primary endpoint was all-cause mortality. Results: A total of 572 patients (69±12 years; 76% male) were included. Mean LVEF was 33±9%, with a median left ventricular end-diastolic volume of 174 mL [136;220] and a median effective regurgitant orifice area of 14 mm2 [8;22]. Abstract Background The baseline coronary plaque burden is the most important factor for rapid plaque progression (RPP) in the coronary artery. However, data on the independent predictors of RPP in the absence of a baseline coronary plaque burden are limited. Thus, this study aimed to investigate the predictors for RPP in patients without coronary plaques on baseline coronary computed tomography angiography (CCTA) images. Methods A total of 402 patients (mean age: 57.6 ± 10.0 years, 49.3% men) without coronary plaques at baseline who underwent serial coronary CCTA were identified from the Progression of Atherosclerotic Plaque Determined by Computed Tomographic Angiography Imaging (PARADIGM) registry and included in this retrospective study. RPP was defined as an annual change of ≥ 1.0%/year in the percentage atheroma volume (PAV). Results During a median inter-scan period of 3.6 years (interquartile range: 2.7–5.0 years), newly developed coronary plaques and RPP were observed in 35.6% and 4.2% of the patients, respectively. The baseline traditional risk factors, i.e., advanced age (≥ 60 years), male sex, hypertension, diabetes mellitus, hyperlipidemia, obesity, and current smoking status, were not significantly associated with the risk of RPP. Abstract Background Refractory hypoxemia after right ventricular myocardial infarction and concomitant SARS-CoV-2 infection represents an uncommon, yet particularly challenging clinical scenario. We report a challenging diagnostic case of refractory hypoxemia due to right-to-left shunt highlighting contemporary challenges and pitfalls in acute cardiovascular care associated with the current COVID-19 pandemic. Case presentation A 52-year-old patient admitted for inferior acute myocardial infarction developed rapidly worsening hypoxemia shortly after primary percutaneous coronary intervention. RT-PCR screening for SARS-CoV-2 was positive, even though the patient had no prior symptoms. A computed tomography pulmonary angiogram excluded pulmonary embolism and showed only mild interstitial pulmonary involvement of the virus. Transthoracic echocardiogram showed severe right ventricular dysfunction and significant right-to-left shunt at the atrial level after agitated saline injection. Progressive improvement of right ventricular function allowed weaning from supplementary oxygen support. Patient was latter discharged with marked symptomatic improvement. Conclusion Refractory hypoxemia after RV myocardial infarction should be carefully addressed, even in the setting of other more common and tempting diagnoses. After exclusion of usual etiologies, right-to-left shunting at the atrial level should always be suspected, as this may avoid unnecessary and sometimes harmful interventions.

Introduction: During the Covid-19 pandemic there has been a general belief that hospital admissions for non-infectious causes, especially cardiovascular diseases, have fallen. Objectives: To assess the impact of the pandemic on admissions for ST-elevation myocardial infarction (STEMI) during the first pandemic wave. Methods: We performed a multicenter retrospective analysis of consecutive patients presenting with STEMI in two Portuguese hospital centers in two sequential periods – P1 (March 1 to April 30) and P2 (May 1 to June 30). Patient's clinical data and hospital outcomes were compared between the years 2017 to 2019 and 2020 for both periods. Results: During P1 in 2020, a reduction in the number of STEMI patients was observed in comparison with previous years (26.0±4.2 vs. 16.5±4.9 cases per month; p=0.033), as well as an increase in the number of mechanical complications (0.0% vs. 3.0%; p=0.029). Percutaneous coronary interventions in the setting of failed thrombolysis were more frequent (1.9% vs. 9.1%; p=0.033). An overall trend for longer delays in key timings of STEMI care bundles was noted. Mortality was higher during P1 compared to previous years (1.9% vs. 12.1%; p=0.005). Conclusions: During the first Covid-19 wave fewer patients presented with STEMI at the catheterization laboratory for percutaneous coronary intervention. These patients presented more mechanical complications and higher mortality. Resumo: Introdução: Recentemente durante a pandemia por Covid-19 houve uma perceção global de uma diminuição de admissões hospitalares por causas não infeciosas, em particular por doenças cardiovasculares. Objetivos: Avaliar o impacto da pandemia nas admissões por enfarte agudo do miocárdio com supradesnivelamento de ST (STEMI), na primeira onda da pandemia. Métodos: Análise multicêntrica e retrospetiva de doentes consecutivos admitidos em dois hospitais portugueses por STEMI em dois períodos sequenciais - P1 (1 de março a 30 de abril) e P2 (1 de maio a 30 de junho). Foi realizada uma comparação dos dados clínicos e de evolução hospitalar entre 2017 a 2019 e 2020 para os dois períodos. Resultados: No P1 de 2020 observou-se, relativamente a anos prévios, uma redução do número de doentes com STEMI (26,0±4,2 versus 16,5±4,9 casos por mês; p=0,033) e um aumento do número de complicações mecânicas (0,0% versus 3,0%; p=0,029). Os casos de angioplastia após trombólise falhada foram mais frequentes (1,9% versus 9,1%; p=0,033). Observou-se uma tendência global para um maior atraso nos tempos-chave de abordagem de doentes com STEMI. A taxa de mortalidade destes doentes no P1 foi superior comparativamente a anos prévios (1,9% versus 12,1%; p=0,005). Conclusões: Durante a primeira onda da pandemia Covid-19 houve uma redução do número de doentes submetidos a angioplastia coronária por STEMI. Esses apresentaram mais complicações mecânicas e uma maior mortalidade.

Abstract Anomalous origin of the right coronary artery from the opposite sinus (right-ACAOS) with interarterial course (IAC) has been associated with increased risk of sudden cardiac death (SCD). Widespread use of coronary computed tomography angiography (CCTA) has led to increased recognition of this condition, even among healthy individuals. Our study sought to examine the prevalence, anatomical characteristics, and outcomes of right-ACAOS with IAC in patients undergoing CCTA for suspected coronary artery disease (CAD). We conducted a retrospective analysis of consecutive patients referred for CCTA at one tertiary hospital from January 2012 to December 2020. Patients exhibiting right-ACAOS with IAC were analyzed for cardiac symptoms and mid-term occurrence of first MACE (cardiac death, SCD, non-fatal myocardial infarction (MI) or revascularization of the anomalous vessel). CCTAs were reviewed for anatomical high-risk features and concomitant CAD. Among 10,928 patients referred for CCTA, 28 patients with right-ACAOS with IAC were identified. Mean age was 55 ± 17 years, 64% were male and 11 (39.3%) presented stable cardiac symptoms. Most patients had at least one high risk anatomical feature. During follow-up, there were no cardiac deaths or aborted SCD episodes and only 1 patient underwent surgical revascularization of the anomalous vessel. Right-ACAOS with IAC is an uncommon finding (prevalence of 0.26%). In a contemporary population of predominantly asymptomatic patients who survived this condition well into adulthood, most patients were managed conservatively with a low event rate. Additional studies are needed to support medical follow-up as the preferred option in this setting.

Abstract Patient-specific phenotyping of coronary atherosclerosis would facilitate personalized risk assessment and preventive treatment. We explored whether unsupervised cluster analysis can categorize patients with coronary atherosclerosis according to their plaque composition, and determined how these differing plaque composition profiles impact plaque progression. Patients with coronary atherosclerotic plaque (n = 947; median age, 62 years; 59% male) were enrolled from a prospective multi-national registry of consecutive patients who underwent serial coronary computed tomography angiography (median inter-scan duration, 3.3 years). K-means clustering applied to the percent volume of each plaque component and identified 4 clusters of patients with distinct plaque composition. Cluster 1 (n = 52), which comprised mainly fibro-fatty plaque with a significant necrotic core (median, 55.7% and 16.0% of the total plaque volume, respectively), showed the least total plaque volume (PV) progression (+ 23.3 mm3), with necrotic core and fibro-fatty PV regression (− 5.7 mm3 and − 5.6 mm3, respectively). Cluster 2 (n = 219), which contained largely fibro-fatty (39.2%) and fibrous plaque (46.8%), showed fibro-fatty PV regression (− 2.4 mm3). Cluster 3 (n = 376), which comprised mostly fibrous (62.7%) and calcified plaque (23.6%), showed increasingly prominent calcified PV progression (+ 21.4 mm3). Cluster 4 (n = 300), which comprised mostly calcified plaque (58.7%), demonstrated the greatest total PV increase (+ 50.7mm3), predominantly increasing in calcified PV (+ 35.9 mm3). Multivariable analysis showed higher risk for plaque progression in Clusters 3 and 4, and higher risk for adverse cardiac events in Clusters 2, 3, and 4 compared to that in Cluster 1. Unsupervised clustering algorithms may uniquely characterize patient phenotypes with varied atherosclerotic plaque profiles, yielding distinct patterns of progressive disease and outcome.

Resumo: A angina refratária define‐se como a persistência de sintomas superior a três meses apesar da terapêutica médica otimizada e revascularização. É uma entidade em crescimento, resultado da melhoria do prognóstico da doença coronária com a terapêutica farmacológica e com as técnicas de revascularização contemporâneas. A mortalidade a longo prazo enquadra‐se no espetro prognóstico da doença estável assintomática, contudo interfere com a qualidade de vida do doente e tem um impacto significativo nos sistemas de saúde.Múltiplos alvos terapêuticos têm sido investigados, contudo, a maioria com resultados dececionantes. Muitas das técnicas foram abandonadas por ausência de eficácia, problemas de segurança e limitações tanto logísticas como económicas à sua implantação.Esta revisão incide essencialmente sobre o dispositivo de redução do seio coronário, cuja evidência, embora ainda escassa, é promissora relativamente à segurança e eficácia na redução dos sintomas anginosos e na melhoria da qualidade de vida. Para além do seu efeito terapêutico, é uma opção virtualmente acessível a todos os serviços de cardiologia de intervenção. Abstract: Refractory angina is defined as persistent angina (≥3 months) despite optimal medical and interventional therapies. It is increasing in frequency, due to the success of current medical and interventional therapies in improving the prognosis of coronary artery disease. Long‐term mortality is similar to that of patients with asymptomatic stable disease, but it affects patients’ quality of life, and has a significant impact on health care resources.Several therapeutic targets have been investigated, most with disappointing results. Many of the techniques have been abandoned because of lack of efficacy, safety issues, or economic and logistic limitations to wider applicability.The primary focus of this review is the coronary sinus Reducer, supporting evidence for which, although scarce, is promising regarding safety and efficacy in improving anginal symptoms and quality of life. It is also accessible to virtually all interventional cardiology departments.

Introduction: Coronary artery calcium (CAC) scoring is used for both cardiovascular risk reclassification and as a gatekeeper for coronary computed tomography angiography (CCTA). The aims of this study were to assess to what extent CAC score results can reclassify the cardiovascular risk of patients without obstructive coronary artery disease (CAD) on CCTA, and to measure the proportion of these patients whose primary prevention medication is changed after the exam. Methods: In a retrospective analysis of a multicenter registry of individuals who underwent CCTA for suspected CAD during a two-year period, the Systematic COronary Risk Evaluation (SCORE) and Multi-Ethnic Study of Atherosclerosis (MESA) risk scores were calculated for each individual. In a subset of 184 patients, we also assessed the prescription of statins and antiplatelet agents before and after the test. Results: A total of 467 patients (248 women, mean age 60±9.10 years) were included. Median CAC score was 0 (interquartile range 0-40). Overall, 249 patients (53%) and 159 (34%) were classified as being of moderate/intermediate risk according to the SCORE and MESA risk scores, respectively. Among these, 29 (12%) and 30 (19%) patients had CAC score >100 AU, making them eligible for statin therapy. The inclusion of CAC scoring in the MESA score resulted in the reclassification of 215 patients (46%). The proportion of patients who were prescribed statins or antiplatelet agents did not change significantly after the test. Conclusion: CAC scoring can reclassify cardiovascular risk in a significant proportion of patients undergoing CCTA. Despite this, little change was seen in the prescription of statins and antiplatelet agents. Resumo: Introdução: O score de cálcio coronário (ScCa) é utilizado para reclassificação do risco cardiovascular e como gatekeeper para a realização de angiografia coronária por tomografia computadorizada (angio-TC). Os objetivos deste estudo são a avaliação do impacto dos resultados do ScCa na reclassificação do risco cardiovascular em doentes sem doença arterial coronária obstrutiva e a quantificação da proporção de doentes em contexto de prevenção primária cuja terapêutica foi modificada após o exame. Métodos: Análise retrospetiva de um registro multicêntrico de doentes submetidos a angio-TC por suspeita de doença arterial coronária significativa durante um período de dois anos. Os scores de risco Systematic COronary Risk Evaluation (SCORE) e Multi-Ethnic Study of Atherosclerosis (MESA) foram calculados em cada doente. Num subgrupo de 184 doentes, avaliámos a prescrição da estatina e da terapêutica antiagregante plaquetar antes e depois da realização do exame. Resultados: Foram incluídos 467 pacientes (248 mulheres, com idade média de 60 ± 9,10 anos). O ScCa mediano foi de 0 (intervalo interquartil de 0 a 40). Globalmente, 249 (53%) e 159 (34%) doentes foram classificados com risco moderado/intermédio, de acordo com os scores de risco SCORE e MESA, respetivamente. Neste grupo, 29 (12%) e 30 (19%) doentes apresentavam ScCa > 100, tornando-os elegíveis para terapêutica com estatinas. A inclusão dos resultados do ScCa no score de MESA resultou na reclassificação de 215 doentes (46%). A proporção de doentes que receberam estatinas ou agentes antiplaquetários não mudou significativamente após o exame. Conclusão: Os resultados do ScCa têm a capacidade de reclassificar o risco cardiovascular numa proporção significativa de pacientes submetidos a angio-TC. No entanto, pouco se alterou na prescrição de estatinas e terapia antiagregante plaquetar.

Introduction: Compared to bare-metal stents (BMS), drug-eluting stents reduce stent restenosis and improve subsequent revascularization rates. The impact on patients’ survival has been the subject of debate. Objective: To assess the long-term (10-year) survival of patients undergoing percutaneous coronary intervention (PCI) with first-generation sirolimus-eluting stents (SES) in comparison with BMS. Methods: In a single-center registry, 600 consecutive patients who underwent successful PCI with SES between April 2002 and February 2003 were compared to 594 patients who underwent PCI with BMS between January 2002 and April 2002, just before the introduction of SES. Introduction: Although there is evidence that a significant proportion of veteran athletes have coronary atherosclerotic disease (CAD), its prevalence in recreational athletes with low to intermediate cardiovascular (CV) risk is not established. This study aimed to characterize the coronary atherosclerotic burden in veteran male recreational athletes with low to intermediate CV risk. Methods: Asymptomatic male athletes aged ≥40 years with low to intermediate risk, who exercised >4 hours/week for >5 years, underwent cardiac computed tomography (CT) for coronary artery calcium (CAC) scoring and CT angiography. High coronary atherosclerotic burden was defined as at least one of the following: CAC score >100; CAC score ≥75th percentile; obstructive CAD; disease involving the left main, three vessels or two vessels including the proximal left anterior descending artery; segment involvement score >5; or CT Leaman score ≥5. Athletes were categorized by tertiles of exercise volume, calculated by metabolic equivalent of task (MET) scores. Results: A total of 105 athletes were included, all with SCORE 100, 13 (12.4%) had CAC score ≥75th percentile and six (5.7%) had obstructive lesions. O objetivo deste estudo foi caracterizar a carga aterosclerótica coronária em atletas veteranos do género masculino de nível recreativo com risco CV baixo-intermédio. Métodos: Atletas assintomáticos do género masculino com ≥40 anos com risco baixo-intermédio, praticantes de >4horas/semana de exercício durante >5 anos, realizaram tomografia computorizada cardíaca com determinação do score de cálcio (ScCa) e angiografia coronária (Angio-TC). Definiu-se elevada carga aterosclerótica coronária a presença de pelo menos uma das seguintes características: ScCa >100; ScCa≥p75 (percentil); DAC obstrutiva; doença envolvendo tronco comum, 3-vasos ou 2-vasos incluindo a artéria descendente anterior proximal; segment involvement score >5; CT-adapted Leaman score ≥5. Os atletas foram categorizados em tercis de volume de exercício, calculado por Metabolic Equivalent Task (MET) scores. Resultados: Foram incluídos 105 atletas, todos com score 100, 13 (12,4%) ≥p75 e 6 (5,7%) lesões obstrutivas. A extensão e a gravidade das placas coronárias não diferiram de acordo com o volume de exercício. Conclusões: A prevalência de DAC subclínica detetada por Angio-TC cardíaca em atletas veteranos do género masculino de nível recreativo com risco CV baixo-intermédio foi elevada. Um quarto da nossa amostra apresentou uma elevada carga aterosclerótica coronária.

Abstract Background The association between triglyceride glucose (TyG) index and coronary atherosclerotic change remains unclear. We aimed to evaluate the association between TyG index and coronary plaque progression (PP) using serial coronary computed tomography angiography (CCTA). Methods A total of 1143 subjects (aged 60.7 ± 9.3 years, 54.6% male) who underwent serial CCTA with available data on TyG index and diabetic status were analyzed from The Progression of AtheRosclerotic PlAque DetermIned by Computed TomoGraphic Angiography IMaging (PARADIGM) registry. PP was defined as plaque volume (PV) (mm3) at follow-up minus PV at index > 0. Annual change of PV (mm3/year) was defined as PV change divided by inter-scan period. Rapid PP was defined as the progression of percent atheroma volume (PV divided by vessel volume multiplied by 100) ≥ 1.0%/year. Results The median inter-scan period was 3.2 (range 2.6–4.4) years. All participants were stratified into three groups based on TyG index tertiles. The overall incidence of PP was 77.3%. O objetivo deste estudo foi avaliar o impacto da otimização de protocolos e evolução tecnológica nas doses de radiação e contraste e na qualidade de imagem dos exames de TC cardíaca prévia a ablação de fibrilhação auricular (FA). Métodos: Registo prospetivo de doentes consecutivos de centro único, foram incluídos os que fizeram TC cardíaca num contexto de avaliação prévia a ablação de FA (n = 270), distribuídos em três grupos: Grupo 1, constituído pelos primeiros 150 doentes; Grupo 2, os últimos 60 doentes feitos no mesmo aparelho; Grupo 3, os primeiros 60 doentes do novo aparelho. Avaliámos a otimização do protocolo com base na dose de radiação, no volume de contraste, na necessidade de aquisição complementar e na avaliação objectiva da qualidade de imagem (rácios sinal/ruído, contraste/ruído e homogeneização de densidade AE/AAE). Resultados: Houve uma redução significativa da radiação entre cada um dos grupos e da dose de contraste entre o primeiro e o último grupo (G1: 5,6 mSv e 100 ml; G2: 1,3 mSv e 90 ml; G3: 0,6 mSv e 65 ml). Apesar das menores doses de radiação e contraste, o Grupo 3 apresentou resultados significativamente melhores de qualidade de imagem (rácios sinal/ruído 13,5; contraste/ruído 14,8; homogeneização de densidade 0,92). Conclusão: A otimização de protocolos e a evolução tecnológica permitiram reduções significativas nas doses de radiação e de contraste usadas na TC cardíaca pré‐ablação de FA, sem prejudicar a qualidade de imagem. Abstract: Background: Cardiac computed tomography (CT) can provide a precise tridimentional anatomic map and exclude intra‐cardiac thrombus. We aimed to access the impact of CT protocol optimization and technological evolution on the contrast and radiation dose as well as on image quality previous to atrial fibrillation (AF) ablation. Methods: From a prospective registry of consecutive patients who underwent cardiac CT in a single center, we selected 270 patients in whom the CT was done for evaluation prior to AF ablation and they were distributed in 3 groups: Group1: the first 150 patients included; Group2: the last 60 patients performed with the same CT scanner; Group3: the first 60 exams performed with the new CT scanner. Quality of the protocol was access based on radiation dose, contrast volume used, the use of a second (delayed) acquisition, and on quantitative image quality analisis (signal to noise and contrast to noise ratios; density homogeneity racio between LA and LAA). Results: We found a significant radiation dose as well as contrast dose reduction between the first and last subgroups (G1: 5,6 mSv and 100 ml; G2: 1,3 mSv and 90 ml; G3: 0,6 mSv and 65 ml). Even though group 3 had less radiation and contrast used it still had better quantitative image quality (signal/noise of 13,5; contrast/noise 14,8; density homogeneity racio of 0,92). Conclusion: Protocol optimization and technology both contributed to significant lower radiation dose and contrast volume used on cardiac CTs prior to AF ablation, without compromising image quality. Background Rapid coronary plaque progression (RPP) is associated with incident cardiovascular events. To date, no method exists for the identification of individuals at risk of RPP at a single point in time. This study integrated coronary computed tomography angiography–determined qualitative and quantitative plaque features within a machine learning (ML) framework to determine its performance for predicting RPP. Methods and Results Qualitative and quantitative coronary computed tomography angiography plaque characterization was performed in 1083 patients who underwent serial coronary computed tomography angiography from the PARADIGM (Progression of Atherosclerotic Plaque Determined by Computed Tomographic Angiography Imaging) registry. RPP was defined as an annual progression of percentage atheroma volume ≥1.0%. We employed the following ML models: model 1, clinical variables; model 2, model 1 plus qualitative plaque features; model 3, model 2 plus quantitative plaque features. ML models were compared with the atherosclerotic cardiovascular disease risk score, Duke coronary artery disease score, and a logistic regression statistical model. 224 patients (21%) were identified as RPP. Feature selection in ML identifies that quantitative computed tomography variables were higher‐ranking features, followed by qualitative computed tomography variables and clinical/laboratory variables. ML model 3 exhibited the highest discriminatory performance to identify individuals who would experience RPP when compared with atherosclerotic cardiovascular disease risk score, the other ML models, and the statistical model (area under the receiver operating characteristic curve in ML model 3, 0.83 [95% CI 0.78–0.89], versus atherosclerotic cardiovascular disease risk score, 0.60 [0.52–0.67]; Duke coronary artery disease score, 0.74 [0.68–0.79]; ML model 1, 0.62 [0.55–0.69]; ML model 2, 0.73 [0.67–0.80]; all P

Introduction: Surgical risk scores are widely used to identify patients at high surgical risk who may benefit from transcatheter aortic valve implantation (TAVI). A multiparametric TAVI mortality risk score based on a French registry (FRANCE-2) has recently been developed. The aim of our study was to compare the 30-day mortality prediction performance of the FRANCE-2, EuroSCORE II and STS scores. Methods: We retrospectively studied 240 patients from a single-center prospective registry who underwent TAVI between January 2008 and December 2015. All scores were assessed for calibration and discrimination using calibration-in-the-large and ROC curve analysis, respectively. Results: The observed mortality was 5.8% (n=14). The median EuroSCORE II, STS and FRANCE-2 scores were 5.0 (IQR 3.2-8.3), 5.1 (IQR 3.6-7.1) and 2.0 (IQR 1.0-3.0), respectively. Discriminative power was greater for EuroSCORE II (C-statistic 0.67) and STS (C-statistic 0.67) than for FRANCE-2 (C-statistic 0.53), but this was not statistically significant (p=0.26). All scores showed adequate calibration. Conclusions: All scores showed modest performance in early mortality prediction after TAVI. Despite being derived from a TAVI population, FRANCE-2 was no better than surgical risk scores in our population. Resumo: Introdução: Os scores de risco cirúrgico têm sido amplamente usados para identificar doentes com alto risco cirúrgico que podem beneficiar da implantação de válvula aórtica por via percutânea (TAVI). Foi recentemente apresentado um «score de risco» multiparamétrico de mortalidade por TAVI com base num registo francês – FRANCE 2. O objetivo do nosso estudo foi comparar o desempenho do FRANCE 2, EuroSCORE II (ES II) e STS-Prom (STS) em prever a mortalidade a 30 dias nos doentes submetidos a TAVI. Métodos: Foram estudados retrospetivamente 240 doentes de um registo prospetivo de centro único que foram submetidos a TAVI entre janeiro de 2008 e dezembro de 2015. Todos os scores foram avaliados para discriminação e calibração, com o uso da análise de curvas ROC e da análise de calibration-in-the-large, respetivamente. Resultados: A mortalidade observada foi de 5,8% (n = 14). A mediana do ES II, STS II e FRANCE II foi de 5,0 (IQR 3,2-8,3), 5,1 (IQR 3,6-7,1) e 2,0 (IQR 1,0-3,0), respetivamente. O poder discriminatório foi maior para ES II (C-statistic 0,67) e STS (C-statistic 0,67) quando comparado com o FRANCE 2 (C-statistic 0,53), embora não fosse estatisticamente significativo (p = 0,26). Todos os scores apresentaram calibração adequada. Conclusões: Todos os scores apresentaram um desempenho modesto em prever a mortalidade precoce após TAVI. Apesar de ser derivado de uma população de doentes submetidos a TAVI, o FRANCE-2 não mostrou ser melhor do que os scores de risco cirúrgicos na nossa população. Regular physical exercise is responsible for various health benefits, and is recommended for primary and secondary cardiovascular (CV) prevention. Despite these recognized benefits, various clinical events can occur in athletes, including acute myocardial infarction and sudden cardiac death (SCD); the main cause of SCD in veteran athletes is coronary artery disease (CAD). The relationship between intense exercise training and CAD is controversial, and a U-shaped association has been hypothesized. If this is the case, screening for subclinical CAD in older athletes may be justified, and various different methodologies have been proposed. However, the methodology for screening veteran athletes is not consensual, and several markers of CAD, in addition to clinical CV risk factors, could improve risk stratification in this population. In the present paper we review the published data on CAD in athletes, focusing on the relationship between the dose of exercise and CAD, as well as the implications for pre-participation screening of veteran athletes. Resumo: O exercício físico associa-se a múltiplos benefícios para a saúde, estando recomendado na prevenção cardiovascular (CV) primária e secundária. Apesar dos benefícios comprovados, diversos eventos clínicos podem ocorrer em atletas, incluindo enfarte agudo do miocárdio e morte súbita, nos atletas veteranos maioritariamente devido a doença das artérias coronárias (DAC). A relação entre exercício físico intenso e DAC permanece controversa, colocando-se a hipótese de associação tipo «curva em U». Neste contexto, a deteção subclínica de DAC em atletas veteranos pode ser justificada, estando propostas algumas metodologias. No entanto, a metodologia para a avaliação pré-competitiva dos atletas veteranos não é consensual e diversos marcadores de risco, adicionais aos fatores de risco CV clínicos tradicionais, poderão melhorar a estratificação de risco nesta população. Neste artigo revêm-se os dados publicados sobre DAC em atletas, com relevância para a relação entre a dose de exercício e DAC, bem como as implicações para a avaliação pré-competitiva de atletas veteranos. Resumo: Introdução: A interpretação do eletrocardiograma (ECG) do atleta permanece controversa, com ausência de estandardização e dificuldade na aplicação de critérios específicos na sua interpretação. O objetivo deste trabalho é avaliar a variabilidade na interpretação do ECG de atletas. Metodologia: Vinte ECG de atletas foram avaliados por cardiologistas e internos de cardiologia, 11 normais ou apenas com alterações fisiológicas e nove patológicos. Cada ECG foi classificado pelos inquiridos em normal/com alterações fisiológicas ou patológico, usando ou não critérios específicos na sua interpretação. Resultados: Foram incluídas as respostas de 58 médicos, 42 (72,4%) cardiologistas. Dezasseis (27,6%) afirmaram avaliar frequentemente atletas e 32 (55,2%) não usar critérios específicos na interpretação do ECG, sendo os mais usados os critérios de Seattle (n = 13). Em média, cada médico interpretou corretamente 15 ± 2 ECG, correspondendo a 74% dos traçados (variação: 45‐100%). A interpretação dos ECG foi correta em 68% (variação: 22‐100%) dos patológicos e em 79% (variação: 55‐100%) dos normais/com alterações fisiológicas. Não houve diferença significativa na interpretação entre cardiologistas e internos (74 ± 10% versus 75 ± 10%; p = 0,724), nem entre os que avaliam frequentemente ou não atletas (77 ± 12% versus 73 ± 9%; p = 0,286), verificando‐se uma tendência para interpretação mais correta com critérios específicos (77 ± 10% versus 72 ± 10%; p = 0,092). Introduction: Sympathetic renal denervation (RDN) was developed as a treatment for the management of patients with resistant hypertension. This procedure may have a positive impact on hypertension-related target organ damage, particularly renal disease, but the evidence is still limited. Objective: To assess the impact of RDN on the albumin-to-creatinine ratio (ACR) at 12-month follow-up. Methods and Results: From a single-center prospective registry including 65 patients with resistant hypertension undergoing renal denervation, 31 patients with complete baseline and 12-month follow-up blood pressure (BP) measurements (both office and 24-h ambulatory blood pressure monitoring [ABPM]) and ACR were included in the present study. Mean age was 65±7 years, 52% were female, most (90%) had been diagnosed with hypertension for more than 10 years, 71% had type 2 diabetes and 33% had vascular disease in at least one territory. Mean estimated glomerular filtration rate was 73.6±25.1 ml/min/1.73 m2 and 15 patients (48%) had an ACR >30 mg/g. After 12 months, 22 patients were considered BP responders (73%). ACR decreased significantly from a median of 25.8 mg/g (interquartile range [IQR] 9.0-574.0 mg/g) to 14.8 mg/g (IQR 4.5-61.0 mg/g, p=0.007). When the results were split according to systolic BP responder status on ABPM, we found a significant reduction in responders (from 25.6 mg/g [IQR 8.7-382.8 mg/g] to 15.9 mg/g [IQR 4.4-55.0 mg/g], p=0.009), and a numerical decrease in the non-responder subgroup (from 165.0 mg/g [IQR 8.8-1423.5 mg/g] to 13.6 mg/dl [IQR 5.7-1417.0 mg/g], p=0.345). Conclusions: Besides significant reductions in blood pressure (both office and 24-h ABPM), renal denervation was associated with a significant reduction in ACR, a recognized marker of target organ damage. Resumo: Introdução: A desnervação simpática renal (RDN) foi desenvolvida como uma forma de tratamento para os doentes com hipertensão arterial resistente (R-HTN). Este procedimento poderá ter um impacto favorável nas lesões de órgão alvo relacionadas com a hipertensão, nomeadamente a doença renal, no entanto, a evidência disponível ainda é escassa. Objetivo: Avaliar o impacto da RDN no rácio albumina-creatinina (ACR) aos 12 meses de seguimento após RDN. Métodos e resultados: Registo prospetivo de centro único incluindo 65 doentes com R-HTN submetidos a RDN, dos quais 31 doentes com avaliação basal e a um ano completa da pressão arterial (na consulta e na monitorização ambulatória [ABPM]) e da ACR foram incluídos no presente estudo. A idade média foi de 65±7 anos, 52% do sexo feminino. A maioria da população tinha diagnóstico de HTN há >10 anos, 71% tinha diabetes tipo 2 e 33% tinham doença vascular em pelo menos um território. Aos 12 meses de seguimento, 22 doentes foram considerados respondedores na pressão arterial (73%). A ACR teve uma descida significativa de uma mediana de 25,8 mg/g (IQR 9,0-574,0 mg/g) para 14,8 mg/g (IQR 4,5-61,0 mg/g, p=0,007). Quando os resultados foram divididos em subgrupos, de acordo com o estado de respondedor à pressão arterial na ABPM, verificou-se uma redução significativa nos respondedores (de 25,6 mg/g [IQR 8,7-382,8 mg/g] para 15,9 mg/g [IQR 4,4-55,0 mg/g], p=0,009), e uma tendência no subgrupo de não respondedores (de 165,0 mg/g [IQR 8,8-1423,5 mg/g] para 13,6 mg/dl [IQR 5,7-1417,0 mg/g], p=0,345). Conclusão: Para além da descida significativa da pressão arterial (quer na consulta quer na monitorização ambulatória de 24 h), a desnervação renal associou-se a uma redução significativa da ACR, um reconhecido marcador de lesão de órgão alvo na hipertensão arterial. Background: Fewer bleeding complications and early ambulation make radial access a privileged route for cardiac catheterization. However, transradial (TR) approach is not always successful, requiring its conversion into femoral access. Objectives: To evaluate the rate of conversion from radial into femoral access in cardiac catheterization and to identify its predictors. Methods: Prospective dual-center registry, including 7632 consecutive patients undergoing catheterization via the radial access between Jan/2009 and Oct/2012. We evaluated the incidence of conversion into femoral access and its predictors by logistic regression analysis. Results: The patients’ mean age was 66 ± 11 years, and 32% were women. A total of 2969 procedures (38.4%) were percutaneous coronary interventions (PCI), and the most used first intention arterial access was the right radial artery (97.6%). Radial access failure rate was 5.8%. Independent predictors of conversion from radial into femoral access were the use of short introducer sheaths (OR 3.047, CI: 2.380-3.902; p < 0.001), PCI (OR 1.729, CI: 1.375-2.173; p < 0.001), female sex (OR 1.569, CI: 1.234-1.996; p < 0.001), multivessel disease (OR 1.457, CI: 1.167-1.819; p = 0.001), body surface area (BSA) ≤ 1.938 (OR 1.448, CI: 1.120-1.871; p = 0.005) and age > 66 years (OR 1.354, CI: 1.088-1.684; p = 0.007). Conclusion: Transradial approach for cardiac catheterization has a high success rate and the need for its conversion into femoral access in this cohort was low. Female sex, older age, smaller BSA, the use of short introducer sheaths, multivessel disease and PCI were independent predictors of conversion into femoral access.

Objective: To evaluate the performance of traditional cardiovascular (CV) risk factors in identifying a higher than expected coronary atherosclerotic burden. Methods: We assessed 2069 patients undergoing coronary CT angiography, with assessment of calcium score (CS), for suspected coronary artery disease. A higher than expected atherosclerotic burden was defined as CS >75th percentile (CS >P75) according to age and gender-adjusted monograms. The ability of traditional CV risk factors to predict a CS >P75 was assessed in a customized logistic regression model (“Clinical Score”) and by the calculation of SCORE (Systemic Coronary Risk Evaluation). The population attributable risk (PAR) of risk factors for CS >P75 was calculated. Results: The median CS was 3.0 (IQR 0.0–98.0); 362 patients had CS >P75. The median SCORE was 3.0 (IQR 1.0–4.0). With the exception of hypertension, all traditional CV risk factors were independent predictors of CS >P75: diabetes, dyslipidemia, smoking and family history (OR 1.3–2.2, p≤0.026). The areas under the ROC curves for CS >P75 were 0.64 for the Clinical Score (95% CI 0.61–0.67, pP75 were in the two lower quartiles of the Clinical Score. Altogether, the traditional risk factors explain 56% of the prevalence of CS >P75 (adjusted PAR 0.56). Conclusion: Despite the association of CV risk factors with a higher than expected atherosclerotic burden, they appear to explain only half of its prevalence. Even when integrated in scores, the predictive power of these risk factors was modest, exposing the limitations of risk stratification based solely on demographic and clinical risk factors. Resumo: Objetivo: O objetivo deste trabalho foi avaliar o desempenho dos fatores de risco cardiovascular (CV) clássicos na identificação de carga aterosclerótica superior ao esperado. Métodos: Avaliámos 2069 doentes (dts) que realizaram AngioTC cardíaca e ScCa para exclusão de doença coronária. Definiu-se carga aterosclerótica superior ao esperado um ScCa acima do percentil 75 (ScCa>p75) de acordo com nomogramas ajustados para o sexo e idade. A capacidade dos fatores de risco clássicos preverem ScCa>p75 avaliou-se num modelo de regressão logística customizado (score clínico) e pelo SCORE. Avaliou-se o Population Attributable Risk (PAR) dos fatores de risco para ScCa>p75. Resultados: A mediana de ScCa foi 3,0 [IIQ 0,0-98,0]; 362 dts com ScCa>p75. A mediana do HeartScore foi 3,0 [IIQ 1,0-4,0]. Exceto a hipertensão arterial, todos os fatores de risco foram preditores independentes de CaSc>p75: diabetes mellitus, dislipidemia, tabagismo e história familiar (OR 1,3-2,2, p≤0,026). As áreas abaixo da curva ROC para SaCa>p75 foram 0,64 para score clínico (IC95% 0,61-0,67; pp75 encontravam-se nos dois quartis de score clínico mais baixos. No seu conjunto, os fatores de risco clássicos explicam 56% da prevalência de ScCa>p75 (PAR ajustado 0,56). Conclusão: Apesar de os fatores de risco CV se associarem a uma carga aterosclerótica superior ao esperado, justificam pouco mais de metade da sua prevalência. O poder preditor destes fatores de risco é modesto, mesmo integrados em scores, revelando as limitações da estratificação de risco baseada apenas em dados demográficos e fatores de risco clínicos. There is a marked contrast between the high prevalence of hypertension and the low rates of adequate control. A subset of patients with suboptimal blood pressure control have drug-resistant hypertension, in the pathophysiology of which chronic sympathetic hyperactivation is significantly involved. Sympathetic renal denervation has recently emerged as a device-based treatment for resistant hypertension. In this review, the pathophysiological mechanisms linking the sympathetic nervous system and cardiovascular disease are reviewed, focusing on resistant hypertension and the role of sympathetic renal denervation. An update on experimental and clinical results is provided, along with potential future indications for this device-based technique in other cardiovascular diseases. Resumo: A elevada prevalência da hipertensão está em claro contraste com a sua ainda insuficiente taxa de controlo. Um importante subgrupo destes doentes apresenta uma hipertensão resistente aos fármacos, na qual a hiperativação crónica do sistema nervoso simpático tem importantes implicações fisiopatológicas. Recentemente, a desnervação simpática renal emergiu como um tratamento de intervenção para a hipertensão arterial resistente. No presente artigo, são revistos os mecanismos fisiopatológicos subjacentes à interação entre o sistema nervoso simpático e as doenças cardiovasculares, com particular enfâse na hipertensão arterial resistente e no papel da desnervação simpática renal. É igualmente feita uma atualização dos resultados de estudos experimentais e clínicos, bem como de potenciais futuras indicações desta técnica de intervenção noutras doenças do foro cardiovascular. Resumo: Introdução: No advento dos stents eluidores de fármaco (DES), a restenose difusa (> 20 mm) intrastent (RDIS) constituía a principal limitação ao uso de stents metálicos na intervenção coronária (PCI). Das várias soluções propostas para o seu tratamento, a braquiterapia intracoronária (BT) revelou‐se uma das mais eficazes, sendo praticamente abandonada após a introdução dos DES. Objetivo: Avaliar os resultados clínicos a longo prazo (> 10 anos) do uso de BT beta com 90Sr/Y90 para o tratamento RDIS, em termos de eventos cardíacos major (MACE) e restenose angiográfica. População e métodos: Estudo retrospetivo observacional de centro único incluindo 12 doentes (dts) consecutivos entre janeiro e julho de 2001, 11 homens, com a idade média de 58,6 ± 9,9 anos (43‐77 anos). Todos tinham angor estável. Os principais fatores de risco eram dislipidemia (75%), HTA (58%), doença arterial periférica (50%) e diabetes (42%). Havia doença multivaso em seis dts, tratava‐se da 2.a ou 3.a restenose em seis dts e a função ventricular era normal em 11. A BT foi feita após dilatação com balão, usando a fonte emissora de radiação beta 90Sr/Y90 (Beta‐Cath™/Novoste). Todos permaneceram sob dupla antiagregação pelo menos até à angiografia de controlo, programada para todos aos nove meses. Foi considerada a ocorrência de morte global e cardiovascular, enfarte do miocárdio não fatal (EAM), revascularização coronária, trombose de stent e restenose angiográfica. Foi avaliada a incidência do evento hierárquico combinado (MACE) de: morte cardiovascular, EAM, revascularização coronária urgente do vaso alvo (TVRurg). Resultados: Verificou‐se sucesso angiográfico e clínico em 100% dos casos. No seguimento médio de 10,9 ± 2,5 anos, ocorreram 19 eventos em sete dts: morte em três dts (25%), apenas uma cardíaca (8,3%), EAM com elevação de ST (vaso não alvo) num dt (8,3%) e 15 novas revascularizações em cinco dts (42%): nove relacionadas com o vaso alvo (sobretudo nos primeiros dois anos). Apenas se verificou uma trombose de stent, provável. A restenose angiográfica aos nove meses de seguimento foi de 27% (3/11 dts) com duas oclusões. A taxa de sobrevivência livre de MACE aos dez anos foi de 42% (cincodts). Conclusões: A braquiterapia com radiação beta para RDIS, neste pequeno grupo de dts, demonstrou‐se eficaz e segura, sem eventos tardios atribuíveis à radiação intracoronária. Abstract: Introduction: Until the development of drug‐eluting stents (DES), diffuse in‐stent restenosis (ISR) was the main limitation of bare‐metal stents in percutaneous coronary intervention (PCI). Among the different treatments available, intracoronary brachytherapy (BT) emerged as one of the most promising, although it was almost abandoned with the increasing use of DES. Objective: To assess the Portuguese experience with 90Sr/90Y beta brachytherapy for the treatment of diffuse ISR regarding long‐term (>10 years) major adverse cardiac events (MACE) and angiographic restenosis. Methods: This single‐center, retrospective, observational study included 12 consecutive patients treated between January and June 2001, mean age 58.6±9.9 years (range 43‐77 years), 11 male. All had chronic stable angina, 75% had dyslipidemia, 58% had hypertension, 50% had peripheral arterial disease, 42% had diabetes and 50% had multivessel disease. Recurrent ISR was present in half of the patients and 11 had normal left ventricular function. After balloon dilatation, BT was performed using an Sr90/Y90 (Novoste Beta‐CathTM) beta radiation source. All patients remained under dual antiplatelet therapy until scheduled nine‐month follow‐up angiography. Patients were followed for the occurrence of death (all‐cause and cardiovascular), non‐fatal myocardial infarction (MI), revascularization, stent thrombosis and angiographic restenosis. MACE were defined as the combined incidence of cardiac death, MI and urgent target vessel revascularization. Results: In all cases there was both clinical and angiographic success. In a mean follow‐up of 10.9±2.5 years, 19 events occurred in seven patients: death in three (25%), only one cardiac (8.3%); ST‐elevation MI in one (related to a non‐target vessel) (8.3%); and 15 revascularizations in five (42%), of which nine were of the target vessel (mainly in the first two years). There was only one case of probable stent thrombosis. Angiographic restenosis at nine months was 27% (three out of 11 patients), of which two were total occlusions. Ten‐year MACE‐free survival was 42% (5 patients). Conclusions: Intracoronary beta brachytherapy for the treatment of diffuse ISR in this small cohort of patients proved to be safe and efficacious, with no late adverse events related to intracoronary radiation. Resumo: Introdução: O aumento da atividade do sistema nervoso simpático desempenha um papel preponderante na fisiopatologia da hipertensão arterial (HTA). Recentemente foi desenvolvida uma técnica de intervenção percutânea – a desnervação renal (DNR) – para o tratamento da HTA resistente. Objetivo: Avaliar a segurança imediata e a eficácia da DNR aos seis meses na redução da pressão arterial em doentes com HTA resistente. Métodos: Registo prospetivo de doentes com HTA essencial resistente submetidos a DNR entre julho de 2011 e maio de 2013. A eficácia da DNR foi definida pela redução ≥ 10 mmHg da pressão arterial sistólica (PAS), avaliada na consulta dos seis meses de seguimento. Resultados: Numa consulta de HTA resistente avaliaram‐se 177 doentes consecutivos, dos quais 34 (idade 62,7 ± 7,6 anos; 50,0% homens) efetuaram DNR. Não ocorreram complicações vasculares, nomeadamente no acesso ou nas artérias renais. Nos 22 doentes com seguimento completo aos seis meses, a taxa de respondedores foi 81,8% (n=18). Introduction: Diagnostic tests that use ionizing radiation play a central role in cardiology and their use has grown in recent years, leading to increasing concerns about their potential stochastic effects.The aims of this study were to compare the radiation dose of three diagnostic tests: single photon emission computed tomography (SPECT), invasive coronary angiography (ICA) and cardiac computed tomography (cardiac CT) and their evolution over time, and to assess the influence of body mass index on radiation dose. Methods: We assessed consecutive patients included in three prospective registries (SPECT, ICA and cardiac CT) over a period of two years. Radiation dose was converted to mSv and compared between the three registries. Differences over time were evaluated by comparing the first with the fourth semester. Resumo: Introdução: Apesar dos avanços terapêuticos, a letalidade do choque cardiogénico (CC) associado ao enfarte agudo do miocárdio (EAM) permanece elevada. Objetivo: Comparar 2 grupos de doentes com CC associado ao EAM, admitidos com um intervalo de 10 anos. Métodos: Análise retrospetiva de 2 populações de doentes com CC associado ao EAM admitidos entre maio/1998-maio/2001 (Grupo A) e maio/2008-maio/2011 (Grupo B). Compararam-se as características clínicas, diagnóstico, tratamento e complicações e analisaram-se os preditores de morte aos 6 meses. Resultados: A incidência de CC foi 3,7% no Grupo A (n = 25) e 4,8% no Grupo B (n = 42). FUNDAMENTO: O benefício clínico de intervenção coronária percutânea (ICP) para lesões coronárias longas é incerto; além disso, foram levantadas dúvidas questões sobre a sua segurança. OBJETIVO: Avaliar os preditores de eventos cardíacos adversos maiores (ECAM) associados à ICP utilizando Full Metal Jacket (FMJ), definido como a sobreposição de stents farmacológicos (SF) medindo >60 mm de comprimento, para lesões muito longas. MÉTODOS: Foram incluídos 136 pacientes consecutivos com lesões coronárias longas, requerendo FMJ em nosso cadastro de centro único. O desfecho primário incluiu a ocorrência combinada de todas as causas de morte, infarto do miocárdio (IM) e revascularização do vaso alvo (RVA). Variáveis demográficas, clínicas, angiográficas e de procedimento foram avaliadas por meio de análise de regressão de Cox para determinar os preditores independentes de desfecho. RESULTADOS: O comprimento médio do stent por lesão foi de 73,2 ± 12,3 mm e o diâmetro médio do vaso de referência foi de 2,9 ± 0,6 mm. O sucesso angiográfico foi de 96,3%. A ausência de ECAM foi de 94,9% em 30 dias e 85,3% em um ano. No acompanhamento de um ano, a taxa de mortalidade por todas as causas foi de 3,7% (1,5% por mortes cardíacas), a taxa de IM foi de 3,7%, e a incidência de trombose de stent (TS) definitiva ou provável foi de 2,9%. O gênero feminino [risco relativo (RR), 4,40; intervalo de confiança de 95% (IC), 1,81-10,66, p = 0,001) e ICP de artéria coronária não direita (RR, 3,49; p = 0,006; IC 95%, 1,42-8,59) foram preditores independentes de ECAM em um ano. A ausência de eventos adversos em um ano foi maior em pacientes com angina estável submetidos à ICP (RR, 0,33; IC 95% 0,13-0,80, p = 0,014). CONCLUSÕES: A ICP utilizando FMJ com SF para lesões muito longas foi eficaz, mas associada a uma alta taxa de TS em acompanhamento de um ano. No entanto, a taxa de mortalidade cardíaca, IM não relacionado a procedimento, e ECAM foi relativamente baixa. ICP de vaso coronário alvo, apresentação clínica, e gênero feminino são novos fatores clínicos contemporâneos que parecem apresentar efeitos adversos sobre o resultado da ICP utilizando FMJ para lesões longas.

Resumo: A tomografia computorizada cardíaca (angioTC cardíaca) permite documentar a presença de doença coronária, independentemente do seu grau de estenose. Recentemente, foi validado o valor prognóstico da doença coronária não obstrutiva documentada por angioTC cardíaca. No entanto, não existem ainda recomendações claras acerca da abordagem destes doentes, nomeadamente sobre o início de medidas farmacológicas mais agressivas em prevenção primária. A abordagem destes doentes permanece controversa, sobretudo nos casos em que existe uma discrepância entre o risco cardiovascular e a carga aterosclerótica objetivada na angioTC.Os autores descrevem o caso de um doente com discrepância entre a extensão da aterosclerose coronária objetivada e a sua estimativa de acordo com os scores de probabilidade pré-teste e de eventos cardiovasculares. Tratando-se de um indivíduo com documentação de aterosclerose coronária acima do esperado - score de cálcio superior ao percentil 90 e doença coronária não obstrutiva na angioTC cardíaca, mas por outro lado, assintomático e sem fatores de risco nem antecedentes cardiovasculares, com uma estimativa de risco cardiovascular muito baixa e atleta de competição, torna-se difícil decidir acerca do risco/benefício de medidas farmacológicas de prevenção primária. Abstract: Cardiac computed tomography (CT) documents the presence of coronary artery disease, regardless of the degree of stenosis. The prognostic value of non-obstructive coronary artery disease documented by cardiac CT has recently been validated. However, there are still no clear guidelines on the management of such patients, particularly concerning initiation of more aggressive pharmacological measures for primary prevention. The approach to these patients remains controversial, especially in cases in which there is a discrepancy between cardiovascular risk and the atherosclerotic burden as documented by cardiac CT.The authors describe the case of a patient with a discrepancy between the extent of documented coronary atherosclerosis and that estimated according to pretest probability and cardiovascular risk scores. As this individual had more severe coronary atherosclerosis than expected (calcium score above the 90th percentile and non-obstructive coronary artery disease on cardiac CT) but was a competitive athlete and otherwise asymptomatic and without risk factors or cardiovascular history, with a very low estimated cardiovascular risk, it was difficult to decide on the risks and benefits of pharmacological primary prevention. Introduction and Objectives: The purpose of this study was to assess the diagnostic yield of current referral strategies for elective invasive coronary angiography (ICA). Methods: We performed a cross-sectional observational study of consecutive patients without known coronary artery disease (CAD) undergoing elective ICA due to chest pain symptoms. The proportion of patients with obstructive CAD (defined as the presence of at least one ≥50% stenosis on ICA) was determined according to the use of noninvasive testing. Results: The study population consisted of 1892 individuals (60% male, mean age 64±11 years), of whom 1548 (82%) had a positive noninvasive test: exercise stress test (41%), stress myocardial perfusion imaging (36%), stress echocardiogram (3%) or coronary computed tomography angiography (3%). Referral without testing occurred in 18% of patients. The overall prevalence of obstructive CAD was 57%, higher among those with previous testing (58% vs. 51% without previous testing, p=0.026) and when anatomic rather than functional tests were used (81.3% vs. 57.1%, p=0.001). A positive test and conventional risk factors were all independent predictors of obstructive CAD, with adjusted odds ratios (95% confidence interval) of 1.34 (1.03–1.74) for noninvasive testing, 1.05 (1.04–1.06) for age, 3.48 (2.81–4.29) for male gender, 1.86 (1.32–2.62) for current smoking, 1.74 (1.38–2.20) for diabetes, 1.30 (1.04–1.62) for hypercholesterolemia, and 1.39 (1.08–1.80) for hypertension. Conclusions: More than 40% of patients without known CAD undergoing elective ICA did not have obstructive lesions, even though four out of five had a positive noninvasive test. These exams were relatively weak gatekeepers; functional tests were more often used but appeared to be outperformed by the anatomic test. Métodos: Estudo transversal de indivíduos consecutivos sem doença coronária conhecida submetidos a coronariografia por dor torácica. Determinação da prevalência de doença coronária obstrutiva (definida pela presença de pelo menos uma estenose ≥ 50%) de acordo com a utilização de testes não-invasivos para despiste de cardiopatia isquémica. Resultados: Foram avaliados 1892 indivíduos (60% homens, idade média 64 ± 11 anos), dos quais 1548 (82%) tinham um teste não-invasivo positivo: prova de esforço (41%), cintigrafia de perfusão miocárdica (36%), ecocardiograma de stress (3%) e angiografia coronária por tomografia computorizada (3%). Ocorreu referenciação sem teste prévio em 18% dos doentes. A prevalência global de doença obstrutiva foi 57%, sendo mais elevada nos doentes submetidos a testes não-invasivos (58% versus 51% nos doentes sem testes prévios, p = 0,026) e naqueles em que o teste era anatómico versus funcional (81,3% versus 57,1%, p = 0,001). Um teste não-invasivo positivo e fatores de risco convencionais foram preditores independentes de doença obstrutiva, com odds-ratio ajustado (intervalo confiança 95%) de: teste não-invasivo 1,34 (1,03-1,74), idade 1,05 (1,04-1,06), sexo masculino 3,48 (2,81-4,29), tabagismo ativo 1,86 (1,32-2,62), diabetes 1,74 (1,38-2,20), hipercolesterolemia 1,30 (1,04-1,62) e hipertensão 1,39 (1,08-1,80). Resumo: Os autores descrevem o caso de um doente do sexo masculino, de 59 anos, hipertenso, ex-fumador, insuficiente renal crónico em hemodiálise, com antecedentes de endoprótese por aneurisma da aorta abdominal e história de tuberculose miliar no passado, a quem é diagnosticado pericardite constritiva e aneurisma da aorta torácica. Num doente com antecedentes patológicos tão diversos, são várias as etiologias a considerar. O tratamento consistiu numa pericardiectomia e numa técnica híbrida de debranching dos ramos supra-aórticos com posterior implantação de endoprótese aórtica. Abstract: The authors describe the case of a 59-year-old man, a former smoker, with hypertension, chronic renal failure undergoing hemodialysis, and a history of stent grafting for repair of an abdominal aortic aneurysm and miliary tuberculosis, who was diagnosed with constrictive pericarditis and a thoracic aortic aneurysm. In a patient with such a complex medical history, there were several etiologies to consider. The treatment consisted of pericardiectomy and a hybrid technique of supra-aortic debranching and subsequent endovascular stent-graft repair. Catheter-based sympathetic renal denervation (RDN) is a recent therapeutic option for patients with resistant hypertension. However, the impact of RDN in left ventricular (LV) mass and function is not completely established. Our aim was to evaluate the effects of RDN on LV structure and function (systolic and diastolic) in patients with resistant hypertension (HTN).From a single centre prospective registry including 65 consecutive patients with resistant HTN submitted to RDN between July-2011 and April-2015, 31 patients with baseline and 1-year follow-up echocardiogram were included in this analysis. Mean age was 65 ± 7 years, 48% were males, 71% had type 2 diabetes. Resumo: A hipertensão arterial é um importante fator de risco cardiovascular e apesar dos inúmeros avanços na terapêutica farmacológica, existe ainda uma percentagem importante de doentes que são resistentes. Os autores descrevem dois casos clínicos de ablação da atividade simpática renal por radiofrequência, que ilustram a exequibilidade de uma nova técnica para o tratamento da hipertensão arterial resistente. O procedimento consiste na aplicação de energia de radiofrequência nas artérias renais, para ablação da atividade simpática renal aferente e eferente, implicada na fisiopatologia da hipertensão arterial. Abstract: Hypertension is an important cardiovascular risk factor and although there have been many improvements in pharmacological treatment, a significant percentage of patients are still considered resistant. The authors describe two cases of radiofrequency renal sympathetic denervation that illustrate the feasibility of this new technique for the treatment of resistant hypertension. The procedure consists of the application of radiofrequency energy inside the renal arteries to ablate afferent and efferent sympathetic renal activity, which has been implicated in the pathophysiology of hypertension. Resumo: Introdução: Segundo as recomendações atuais para o tratamento do enfarte agudo do miocárdio com supra-desnivelamento do segmento ST a intervenção coronária percutânea deve ser efetuada dentro de 90 min após o primeiro contacto médico e o tempo total de isquémia não deve exceder os 120 min.O objetivo deste trabalho foi analisar a adequação da implementação destas recomendações para o enfarte do miocárdio com supra-desnivelamento do segmento ST num centro terciário de intervenção coronária percutânea. Métodos: Registo prospetivo de centro único de 223 doentes consecutivos referenciados para intervenção coronária percutânea primária entre 2003 e 2007. Resultados: Nesta população (idade média 60 ± 12 anos, 76% de sexo masculino), a mediana do tempo total de isquémia foi 4 h 30 min (< 120 min em 4% dos doentes). Resumo: A origem anómala das artérias coronárias constitui um importante desafio clínico pela variabilidade anatómica, possíveis repercussões funcionais, mecanismos fisiopatológicos implicados e também pela ausência de grandes séries na literatura que forneçam sólida evidência científica para a sua orientação clínica e terapêutica.Os autores descrevem o caso de um doente de 55 anos, com antecedentes de hipertensão, dislipidemia e atrofia congénita da perna esquerda, que se apresentou com quadro de dor retroesternal atípica com um ano de evolução. Tendo em conta os fatores de risco cardiovasculares e exames complementares de diagnóstico previamente realizados, considerou-se ser um doente com probabilidade baixa a intermédia de doença coronária. Assim, e pela sua limitação funcional, realizou angioTC cardíaca que revelou uma origem anómala da coronária esquerda na cúspide coronária direita e com trajeto entre a aorta e a artéria pulmonar. Embora esta seja uma variante anatómica com potencial evolução maligna, optou-se por uma atitude conservadora após ponderação do risco-benefício no contexto clínico deste doente. Abstract: Anomalous origin of coronary arteries represents a clinical challenge not only because of the anatomical variability, but also the possible functional consequences, pathophysiological mechanisms involved and the absence of large series in the literature that would provide evidence for clinical and therapeutic orientation.The authors describe the case of a 55-year-old male patient with a long history of atypical chest pain who was considered to have a low to intermediate likelihood of coronary artery disease. Therefore, and also bearing in mind his physical limitations (congenital left leg atrophy), he was referred for cardiac CT to rule out coronary artery disease. The exam showed a left coronary artery arising from the right coronary cusp and with an interarterial course, between the aorta and pulmonary trunk. Although this is a potentially malignant anatomical variant with surgical indication, a conservative approach was chosen, considering the late diagnosis and particular risk-benefit profile.
